Aims : This study evaluates the impact of a Developmental Coordination Disorder (DCD) evidence-based online module including synthesized resources, practical strategies, and interactive component on self-reported physical therapist (PT) knowledge, skills, and practice. Methods : PTs from across Canada completed questionnaires before, immediately after, and 2 months following completion of the module. Questionnaires used 7-point Likert scale items and short open-ended questions; analyzes used paired t-tests and a thematic approach. Results : Fifty PTs completed both pre- and post-questionnaires; 41 of these completed the follow-up questionnaire. Most items (79%) evaluating self-reported knowledge and skills increased significantly following module completion and this increase was maintained two months later. Most participants (92%) reported an increase in their confidence to provide DCD evidence-based services. Participants plan to modify their evaluative practices (e.g., involving children in goal setting) and their management of DCD (e.g., using best practice principles, providing resources to families and physicians). At the 2- month follow-up, 46% of participants had returned to the module to review information (e.g., video, resources) or to download handouts. Conclusion : An online module developed collaboratively with PTs has the potential not only to increase PTs’ knowledge, but also to support them in implementing evidence-based services for children with DCD.

There are approximately 150 Admiral Nurses in the UK who work alongside other health and social care professionals to support people with dementia and their family carers. However, the stigma of the disease and the lack of recognition that dementia is a life limiting illness have led to neglect in addressing the end of life challenges. The small in-depth study reported here aimed to add to an extremely limited formal evidence base for the effectiveness of this approach and to develop a greater understanding of the range of knowledge and skills required of them in ensuring they are better able to support families in the later stages of the illness. Findings focus on the experiences of family carers, the impact of performing the Admiral Nurse role and the use of qualitative measures in this setting.

Resumo Política(s) de saúde no trabalho: um inquérito sociológico às empresas portuguesas A literatura portuguesa sobre políticas, programas e actividades de Segurança, Higiene e Saúde no Trabalho (abreviadamente, SH&ST) é ainda escassa. Com este projecto de investigação pretende-se (i) colmatar essa lacuna, (ii) melhorar o conhecimento dos sistemas de gestão da saúde e segurança no trabalho e (iii) contribuir para a protecção e a promoção da saúde dos trabalhadores. Foi construída uma tipologia com cinco grupos principais de políticas, programas e actividades: A (Higiene & Segurança no Trabalho / Melhoria do ambiente físico de trabalho); B (Avaliação de saúde / Vigilância médica / Prestação de cuidados de saúde); C (Prevenção de comportamentos de risco/ Promoção de estilos de vida saudáveis); D (Intervenções a nível organizacional / Melhoria do ambiente psicossocial de trabalho); E (Actividades e programas sociais e de bem-estar). Havia uma lista de mais de 60 actividades possíveis, correspondendo a um índice de realização de 100%. Foi concebido e desenhado, para ser auto-administrado, um questionário sobre Política de Saúde no Local de Trabalho. Foram efectuados dois mailings, e um follow-up telefónico. O trabalho de campo decorreu entre a primavera de 1997 e o verão de 1998. A amostra (n=259) é considerada representativa das duas mil maiores empresas do país. Uma em cada quatro é uma multinacional. A taxa de sindicalização rondava os 30% da população trabalhadora, mas apenas 16% dos respondentes assinalou a existência de representantes dos trabalhadores eleitos para a SH&ST. A hipótese de investigação principal era a de que as empresas com um sistema integrado de gestão da SH&ST seriam também as empresas com um (i) maior número de políticas, programas e actividades de saúde; (ii) maior índice de saúde; (iii) maior índice de realização; e (iv) maior percentagem dos encargos com a SH&ST no total da massa salarial. As actividades de tipo A e B, tradicionalmente associadas à SH&ST, representavam, só por si, mais de 57% do total. Os resultados, correspondentes às respostas da Secção C do questionário, apontam, para (i) a hipervalorização dos exames de medicina do trabalho; e por outro para (ii) o subaproveitamento de um vasto conjunto de actividades (nomeadamente as de tipo D e E), que são correntemente levadas a cabo pelas empresas e que nunca ou raramente são pensadas em termos de protecção e promoção da saúde dos trabalhadores. As actividades e os programas de tipo C (Prevenção de comportamentos de risco/Promoção de estilos de vida saudáveis), ainda eram as menos frequentes entre nós, a seguir aos Programas sociais e de bem-estar (E). É a existência de sistemas de gestão integrados de SH&ST, e não o tamanho da empresa ou outra característica sociodemográfica ou técnico-organizacional, que permite predizer a frequência de políticas de saúde mais activas e mais inovadores. Os três principais motivos ou razões que levam as empresas portuguesas a investir na protecção e promoção da saúde dos seus trabalhadores eram, por ordem de frequência, (i) o absentismo em geral; (ii) a produtividade, qualidade e/ou competitividade, e (iii) a filosofia de gestão ou cultura organizacional. Quanto aos três principais benefícios que são reportados, surge em primeiro lugar (i) a melhoria da saúde dos trabalhadores, seguida da (ii) melhoria do ambiente do ambiente de trabalho e, por fim, (iii) a melhoria da produtividade, qualidade e/ou competitividade.Quanto aos três principais obstáculos que se põem, em geral, ao desenvolvimento das iniciativas de saúde, eles seriam os seguintes, na percepção dos respondentes: (i) a falta de empenho dos trabalhadores; (ii) a falta de tempo; e (iii) os problemas de articulação/ comunicação a nível interno. Por fim, (i) o empenho das estruturas hierárquicas; (ii) a cultura organizacional propícia; e (iii) o sentido de responsabilidade social surgem, destacadamente, como os três principais factores facilitadores do desenvolvimento da política de saúde no trabalho. Tantos estes factores como os obstáculos são de natureza endógena, susceptíveis portanto de controlo por parte dos gestores. Na sua generalidade, os resultados deste trabalho põem em evidência a fraqueza teóricometodológica de grande parte das iniciativas de saúde, realizadas na década de 1990. Muitas delas seriam medidas avulsas, que se inserem na gestão corrente das nossas empresas, e que dificilmente poderão ser tomadas como expressão de uma política de saúde no local de trabalho, (i) definida e assumida pela gestão de topo, (ii) socialmente concertada, (iii) coerente, (iv) baseada na avaliação de necessidades e expectativas de saúde dos trabalhadores, (v) divulgada, conhecida e partilhada por todos, (vi) contingencial, flexível e integrada, e, por fim, (vii) orientada por custos e resultados. Segundo a Declaração do Luxemburgo (1997), a promoção da saúde engloba o esforço conjunto dos empregadores, dos trabalhadores, do Estado e da sociedade civil para melhorar a segurança, a saúde e o bem-estar no trabalho, objectivo isso que pode ser conseguido através da (i) melhoria da organização e das demais condições de trabalho, da (ii) participação efectiva e concreta dos trabalhadores bem como do seu (iii) desenvolvimento pessoal. Abstract Health at work policies: a sociological inquiry into Portuguese corporations Portuguese literature on workplace health policies, programs and activities is still scarce. With this research project the author intends (i) to improve knowledge on the Occupational Health and Safety (shortly thereafter, OSH) management systems and (ii) contribute to the development of health promotion initiatives at a corporate level. Five categories of workplace health initiatives have been identified: (i) Occupational Hygiene and Safety / Improvement of Physical Working Environment (type A programs); (ii) Health Screening, Medical Surveillance and Other Occupational Health Care Provision (type B programs); (iii) Preventing Risk Behaviours / Promoting Healthy Life Styles (type C programs); (iv) Organisational Change / Improvement of Psycho-Social Working Environment (type D programs); and (v) Industrial and Social Welfare (type E programs). A mail questionnaire was sent to the Chief Executive Officer of the 1500 largest Portuguese companies, operating in the primary and secondary sectors (≥ 100 employees) or tertiary sector (≥ 75 employees). Response rate has reached about 20% (259 respondents, representing about 300 companies). Carried out between Spring 1997 and Summer 1998, the fieldwork has encompassed two direct mailings and one phone follow-up. Sample is considered to be representative of the two thousand largest companies. One in four is a multinational. Union membership rate is about 30%, but only 16% has reported the existence of a workers’ health and safety representative. The most frequent workplace health initiatives were those under the traditional scope of the OSH field (type A and B programs) (57% of total) (e.g., Periodical Medical Examinations; Individual Protective Equipment; Assessment of Working Ability). In SMEs (< 250) it was less likely to find out some time-consuming and expensive activities (e.g., Training on OSH knowledge and skills, Improvement of environmental parameters as ventilation, lighting, heating).There were significant differences in SMEs, when compared with the larger ones (≥ 250) concerning type B programs such as Periodical medical examinations, GP consultation, Nursing care, Other medical and non-medical specialities (e.g., psychiatrist, psychologist, ergonomist, physiotherapist, occupational social worker). With regard to type C programs, there were a greater percentage of programs centred on Substance abuse (tobacco, alcohol, and drug) than on Other health risk behaviours. SMEs representatives reported very few prevention- oriented programs in the field of Drug abuse, Nutrition, Physical activity, Off- job accidents, Blood pressure or Weight control. Frequency of type D programs included Training on Human Resources Management, Training on Organisational Behaviour, Total Quality Management, Job Design/Ergonomics, and Workplace rehabilitation. In general, implementation of this type of programs (Organisational Change / Improvement of Psychosocial Working Environment) is not largely driven by health considerations. Concerning Industrial and Social Welfare (Type E programs), the larger employers are in a better position than SMEs to offer to their employees a large spectrum of health resources and facilities (e.g., Restaurant, Canteen, Resting room, Transport, Infra-structures for physical activity, Surgery, Complementary social protection, Support to recreational and cultural activities, Magazine or newsletter, Intranet). Other workplace health promotion programs like Training on Stress Management, Employee Assistance Programs, or Self-help groups are uncommon in the Portuguese worksites. The existence of integrated OSH management systems, not the company size, is the main variable explaining the implementation of more active and innovative workplace health policies in Portugal. The three main prompting factors reported by employers for health protection and promotion initiatives are: (i) Employee absenteeism; (ii) Productivity, quality and/or competitiveness; and (iii) Corporate culture/management philosophy. On the other hand, (i) Improved staff’s health, (ii) Improved working environment and (iii) Improved productivity, quality and/or competitiveness were the three main benefits reported by companies’ representatives, as a result of successful implementation of workplace health initiatives. (i) Lack of staff commitment; (ii) Lack of time; and (iii) Problems of co-operation and communication within company or establishment (iii) are perceived to be the main barriers companies must cope with. Asked about the main facilitating factors, these companies have pointed out the following ones: (i) Top management commitment; (ii) Corporate culture; and (iii) Sense of social responsibility. This sociological research report shows the methodological weaknesses of workplace health initiatives, carried out by Portuguese companies during the last ‘90s. In many cases, these programs and actions were not part of a corporate health strategy and policy, (i) based on the assessment of workers’ health needs and expectancies, (ii) advocated by the employer or the chief executive officer, (ii) planned and implemented with the staff consultation and participation or (iv) evaluated according to a cost-benefit analysis. In short, corporate health policy and action were still rather based on more traditional OSH approaches and should be reoriented towards Workplace Health Promotion (WHP) approach. According to the Luxembourg Declaration of Workplace Health Promotion in the European Union (1997), WHP is “a combination of: (i) improving the work organisation and environment; (ii) promoting active participation; (iii) encouraging personal development”.Résumée Politique(s) de santé au travail: une enquête sociologique aux entreprises portugaises Au Portugal on ne sait presque rien des politiques de santé au travail, adoptés par les entreprises. Avec ce projet de recherche, on veut (i) améliorer la connaissance sur les systèmes de gestion de la santé et de la sécurité au travail et, au même temps, (ii) contribuer au développement de la promotion de la santé des travailleurs. Une typologie a été usée pour identifier les politiques, programmes et actions de santé au travail: A. Amélioration des conditions de travail / Sécurité au travail; B. Médecine du travail /Santé au travail; C. Prévention des comportements de risque / Promotion de styles de vie sains; D. Interventions organisationnelles / Amélioration des facteurs psychosociaux au travail; E. Gestion de personnel et bien-être social. Un questionnaire postal a été envoyé au représentant maximum des grandes entreprises portugaises, industrielles (≥ 100 employés) ou des services (≥ 75 employés). Le taux de réponse a été environ 20% (259 répondants, concernant trois centaines d’entreprises et d’établissements). La recherche de champ, conduite du printemps 1997 à l’été 1998, a compris deux enquêtes postales et un follow-up téléphonique. L´échantillon est représentatif de la population des deux miles plus grandes entreprises. Un quart sont des multinationales. Le taux de syndicalisation est d’environ 30%. Toutefois, il y a seulement 16% de lieux de travail avec des représentants du personnel pour la santé et sécurité au travail. Les initiatives de santé au travail les plus communes sont celles concernant le domaine plus traditionnel (types A et B) (57% du total): par exemple, les examens de médecine du travail, l’équipement de protection individuelle, les tests d’aptitude au travail. En ce qui concerne les programmes de type C, les plus fréquents sont le contrôle et la prévention des addictions (tabac, alcool, drogue). Les interventions dans le domaine de du système technique et organisationnelle du travail peuvent comprendre les courses de formation en gestion de ressources humaines ou en psychosociologie des organisations, l’ergonomie, le travail posté ou la gestion de la qualité totale. En général, la protection et la promotion de la santé des travailleurs ne sont pas prises en considération dans l’implémentation des initiatives de type D. Il y a des différences quand on compare les grandes entreprises et les moyennes en matière de politique de gestion du personnel e du bien-être (programmes de type E, y compris l’allocation de ressources humaines ou logistiques comme, par exemple, restaurant, journal d’entreprise, transports, installations et équipements sportifs). D’autres activités de promotion de la santé au travail comme la formation en gestion du stress, les programmes d’ assistance aux employés, ou les groupes de soutien et d’auto-aide sont encore très peu fréquents dans les entreprises portugaises. C’est le système intégré de gestion de la santé et de la sécurité au travail, et non pas la taille de l’entreprise, qui aide à prédire l’existence de politiques actives et innovatrices dans ce domaine. Les trois facteurs principaux qui encouragent les actions de santé (prompting factors, en anglais) sont (i) l’absentéisme (y compris la maladie), (ii) les problèmes liés à la productivité, qualité et/ou la compétitivité, et aussi (iii) la culture de l’entreprise/philosophie de gestion. Du coté des bénéfices, on a obtenu surtout l’amélioration (i) de la santé du personnel, (ii) des conditions de travail, et (iii) de la productivité, qualité et/ou compétitivité.Les facteurs qui facilitent les actions de santé au travail sont (i) l’engagement de la direction, (ii) la culture de l’entreprise, et (iii) le sens de responsabilité sociale. Par contre, les obstacles à surmonter, selon les organisations qui ont répondu au questionnaire, seraient surtout (i) le manque d’engagement des travailleurs et de leur représentants, (ii) le temps insuffisant, et (iii) les problèmes de articulation/communication au niveau interne de l’entreprise/établissement. Ce travail de recherche sociologique montre la faiblesse méthodologique des services et activités de santé et sécurité au travail, mis en place par les entreprises portugaises dans les années de 1990, à la suite des accords de concertation sociale de 1991. Dans beaucoup de cas, (i) ces politiques de santé ne font pas partie encore d’un système intégré de gestion, (ii) il n’a pas d’évaluation des besoins et des expectatives des travailleurs, (iii) c’est très bas ou inexistant le niveau de participation du personnel, (iv) on ne fait pas d’analyse coût-bénéfice. On peut conclure que les politiques de santé au travail sont plus proches de la médecine du travail et de la sécurité au travail que de la promotion de la santé des travailleurs. Selon la Déclaration du Luxembourg sur la Promotion de la Santé au Lieu de Travail dans la Communauté Européenne (1997), celle-ci « comprend toutes les mesures des employeurs, des employés et de la société pour améliorer l'état de santé et le bien être des travailleurs » e « ceci peut être obtenu par la concentration des efforts dans les domaines suivants: (i) amélioration de l'organisation du travail et des conditions de travail ; (ii) promotion d'une participation active des collaborateurs ; (iii) renforcement des compétences personnelles ».

O presente relatório de qualificação profissional, como requisito (parcial) para obtenção do grau de mestre em Educação Pré-Escolar e Ensino do 1.º Ciclo do Ensino Básico, é o reflexo dos saberes e competências profissionais adquiridas pelas experiências vividas durante a prática pedagógica supervisionada, desenvolvida em dois contextos de estágio: Educação Pré- Escolar e 1.º Ciclo do Ensino Básico. Ao longo desta formação inicial a mestranda adquiriu saberes científicos, pedagógicos, didáticos, culturais e de investigação que lhe possibilitaram o desenvolvimento de práticas sustentadas. A interação entre os saberes teóricos mobilizados e a reflexão sobre a prática, possibilitaram a construção de um quadro teórico próprio, com visão e pensamento pessoal, permitindo à futura profissional de educação melhorar e transformar a prática educativa, agindo nos contextos de forma adequada, visando a inclusão e equidade. Através da adoção de uma postura crítica, reflexiva e indagadora, apoiada nos processos da metodologia de investigação-ação – observação, planificação, ação, reflexão e avaliação – a mestranda desenvolveu a práxis de forma cíclica e articulada. Com efeito, a mestranda procurou desenvolver práticas que promovessem aprendizagens significativas e efetivas para as diferentes crianças e que lhes permitissem ser construtoras ativas do seu próprio conhecimento, numa perspetiva holística da educação e do desenvolvimento da criança, encarando-a como centro do processo de ensino e aprendizagem. Em conformidade, os estágios desenvolvidos em ambos os contextos permitiram a construção de um perfil generalista, através do desenvolvimento de competências marcadas por uma (re)construção pessoal e profissional, auxiliadas por processos colaborativos e reflexivos.

Interventions using applied behaviour analysis (ABA) are widely used with children with autism spectrum disorders (ASD). Individuals who work with this population are important to target for ABA education. This study evaluated the efficacy of a self-directed program in increasing parent and student ABA knowledge and skills, self-efficacy, and new skill development in children with ASD. Study 1 was a pilot study of the newly developed evaluation materials. Study 2 tested the self-instructional package with three parents of children with ASD, three university students, and eight children diagnosed with ASD. Parents and students were given the Simple Steps ABA training package to use independently and were measured using a multiple baseline across participants and/or skills design. After training, ABA knowledge scores and self-efficacy showed variable improvement as did children’s appropriate behaviours. These results suggest that more research is needed to determine the efficacy of a self-instructional ABA package.

The learning gap created by summer vacation creates a significant breach in the learning cycle, where student achievement levels decrease over the course ofthe summer (Cooper et aI., 2000). In a review of 39 studies, Cooper and colleagues (1996) specified that the summer learning shortfall equals at least one month loss of instruction as measured by grade level equivalents on standardized test scores. Specifically, the achievement gap has a more profound effect on children as they grow older, where there is a steady deterioration in knowledge and skills sustained during the summer months (Cooper et aI., 1996; Kerry & Davies, 1998). While some stakeholders believe that the benefits of a summer vacation overshadow the reversing effect on achievement, it is the impact of the summer learning gap on vulnerable children, including children who are disadvantaged as a result of requiring special educational needs, children from low socioeconomic backgrounds, and children learning English as a second language, that is most problematic. More specifically, research has demonstrated that it is children's literacy-based skills that are most affected during the summer months. Children from high socioeconomic backgrounds recurrently showed gains in reading achievement over the summer whereas disadvantaged children repeatedly illustrate having significant losses. Consequently, the summer learning gap was deemed to exaggerate the inequality experienced by children from low socioeconomic backgrounds. Ultimately, the summer learning gap was found to have the most profound on vulnerable children, placing these children at an increased chance for academic failure. A primary feature of this research project was to include primary caregivers as authentic partners in a summer family literacy program fabricated to scaffold their children's literacy-based needs. This feature led to the research team adapting and implementing a published study entitled, Learning Begins at Home (LBH): A Research-Based Family Literacy Program Curriculum. Researchers at the Ontario Institute designed this program for the Study of Education, University of Toronto. The LBH program capitalized on incorporating the flexibility required to make the program adaptable to meet the needs of each participating child and his or her primary caregiver. As it has been well documented in research, the role primary caregivers have in an intervention program are the most influential on a child's future literacy success or failure (Timmons, 2008). Subsequently, a requirement for participating in the summer family literacy program required the commitment of one child and one of his or her primary caregivers. The primary caregiver played a fundamental role in the intervention program through their participation in workshop activities prior to and following hands on work with their child. The purpose of including the primary caregiver as an authentic partner in the program was to encourage a definitive shift in the family, whereby caregivers would begin to implement literacy activities in their home on a daily basis. The intervention program was socially constructed through the collaboration of knowledge. The role ofthe author in the study was as the researcher, in charge of analyzing and interpreting the results of the study. There were a total of thirty-six (36) participants in the study; there were nineteen (19) participants in the intervention group and seventeen (17) participants in the control group. All of the children who participated in the study were enrolled in junior kindergarten classrooms within the Niagara Catholic District School Board. Once children were referred to the program, a Speech and Language Pathologist assessed each individual child to identify if they met the eligibility requirements for participation in the summer family literacy intervention program. To be eligible to participate, children were required to demonstrate having significant literacy needs (i.e., below 25%ile on the Test of Preschool Early Literacy described below). Children with low incident disabilities (such as Autism or Intellectual Disabilities) and children with significant English as a Second Language difficulties were excluded from the study. The research team utilized a standard pre-test-post-test comparison group design whereby all participating children were assessed with the Test of Preschool Early Literacy (Lonigan et aI., 2007), and a standard measure of letter identification and letter sound understanding. Pre-intervention assessments were conducted two weeks prior to the intervention program commencing, and the first set of the post-intervention assessments were administered immediately following the completion of the intervention program. The follow-up post-intervention assessments took place in December 2010 to measure the sustainability of the gains obtained from the intervention program. As a result of the program, all of the children in the intervention program scored statistically significantly higher on their literacy scores for Print Knowledge, Letter Identification, and Letter Sound Understanding scores than the control group at the postintervention assessment point (immediately following the completion of the program) and at the December post-intervention assessment point. For Phonological Awareness, there was no statistically significant difference between the intervention group and the control at the postintervention assessment point, however, there was a statistically significant difference found between the intervention group and the control group at the December post-intervention assessment point. In general, these results indicate that the summer family literacy intervention program made an immediate impact on the emergent literacy skills of the participating children. Moreover, these results indicate that the summer family literacy intervention program has the ability to foster the emergent literacy skills of vulnerable children, potentially reversing the negative effect the summer learning gap has on these children.

Ce rapport de recherche porte sur une étude s’intéressant au transfert des connaissances tacites chez les gestionnaires, c’est-à-dire le partage de ces connaissances et leur utilisation informelle, durant une situation de coordination dans un service municipal. La thèse est articulée autour des questions suivantes : Quelles sont les situations de coordination vécues par les gestionnaires municipaux? Quelles sont les sources de connaissances tacites partagées et utilisées? Quelles sont les relations de connaissances mobilisées de façon informelle lors du transfert des connaissances tacites? Quels sont les facteurs encourageant ou inhibant le transfert informel des connaissances tacites? À partir d’un modèle basé sur une approche situationnelle (Taylor, 1989 et 1991), nous avons revu la documentation touchant nos questions de recherche. Nous avons défini notamment la récursivité des connaissances et le réseau de connaissances, de même que présenté le modèle de la conversion des connaissances (Nonaka, 1994) et celui de l’actualisation de soi (St-Arnaud, 1996). Nous avons questionné 22 répondants à l’aide d’instruments de mesure qui combinent les techniques de l’incident critique, de l’entrevue cognitive et réflexive, le questionnement sur les réseaux organisationnels et l’observation participante. Tels des filets, ces instruments ont permis de traquer et d’obtenir des données d’une grande richesse sur les connaissances tacites et les comportements informels durant le transfert de connaissances en situation de coordination. Ces données ont été analysées selon une approche méthodologique essentiellement qualitative combinant l’analyse de contenu, la schématisation heuristique et l’analyse des réseaux sociaux. Nos résultats montrent que la complexité d’une situation de coordination conditionne le choix des mécanismes de coordination. De plus, les sources de connaissances sont, du point de vue individuel, le gestionnaire et ses artefacts, de même que son réseau personnel avec ses propres artefacts. Du point de vue collectif, ces sources sont réifiées dans le réseau de connaissances. Les connaissances clés d’une situation de coordination sont celles sur le réseau organisationnel, le contexte, les expériences en gestion et en situation complexe de coordination, la capacité de communiquer, de négocier, d’innover et celle d’attirer l’attention. Individuellement, les gestionnaires privilégient l’actualisation de soi, l’autoformation et la formation contextualisée et, collectivement, la coprésence dans l’action, le réseautage et l’accompagnement. Cette étude fournit un modèle valide du transfert contextualisé des connaissances qui est un cas de coordination complexe d’activités en gestion des connaissances. Ce transfert est concomitant à d’autres situations de coordination. La nature tacite des connaissances prévaut, de même que le mode informel, les médias personnels et les mécanismes d’ajustement mutuel. Les connaissances tacites sont principalement transférées au début des processus de gestion de projet et continuellement durant la rétroaction et le suivi des résultats. Quant aux connaissances explicites, les gestionnaires les utilisent principalement comme un symbole à la fin des processus de gestion de projet. Parmi les personnes et les groupes de personnes d’une situation de transfert contextualisé des connaissances, 10 % jouent des rôles clés, soit ceux d’experts et d’intermédiaires de personnes et d’artefacts. Les personnes en périphérie possèdent un potentiel de structuration, c’est-à-dire de connexité, pour assurer la continuité du réseau de connaissances organisationnel. Notre étude a élargi le modèle général de la complexité d’une situation (Bystrom, 1999; Choo, 2006; Taylor, 1986 et 1991), la théorie de la coordination (Malone et Crowston, 1994), le modèle de la conversion des connaissances (Nonaka, 1994), celui de l’actualisation de soi (St-Arnaud, 1996) et la théorie des réseaux de connaissances (Monge et Contractor, 2003). Notre modèle réaffirme la concomitance de ces modèles généraux selon une approche constructiviste (Giddens, 1987) où la dualité du structurel et la compétence des acteurs sont confirmées et enrichies.

RÉSUMÉ : Problématique : En dépit du nombre croissant de médecins immigrants qui choisissent de s’installer au Québec, et de la pénurie de ces professionnels de la santé, beaucoup d’entre eux ne parviendront pas à exercer leur profession au Québec. Les études réalisées auparavant n’étaient souvent pas spécifiques à l’intégration professionnelle des médecins immigrants au Québec. Et de nouvelles barrières émergeaient à chaque étude, justifiant la nécessité d’une démarche exploratoire pour mieux comprendre le sujet. Objectifs : L’objectif de cette étude est d’identifier et analyser les barrières à l’intégration professionnelle ainsi que les facteurs facilitants perçus par les médecins immigrants diplômés hors Canada et États-Unis qui choisissent de s’installer au Québec. Méthodologie : La stratégie retenue est la recherche synthétique de type étude de cas. C’est une recherche qualitative principalement basée sur l’analyse de 22 entrevues semi-structurées, réalisées avec des médecins immigrants diplômés hors Canada et États-Unis, originaires de 15 pays différents. L’échantillon est de type variation maximale, selon le pays d’origine, la formation initiale, l’expérience professionnelle et selon le parcours personnel au Québec. Le recrutement a été réalisé principalement à l’aide de la collaboration du Centre d'évaluation des sciences de la santé de l’Université Laval, de l’organisme « l’Hirondelle » et du département d’administration de la santé de l’Université de Montréal. Le cadre théorique est celui de Diaz (1993) et les différents thèmes qui ont été abordés dans l’entrevue sont inspirés de la synthèse des barrières et des perceptions citées dans la littérature dont la plupart ne sont pas spécifiques aux médecins immigrants. Ces thèmes généraux sont représentés par les contraintes linguistiques, socioculturelles, informationnelles, financières, procédurales et discriminatoires. Pour ce qui est des thèmes des facteurs facilitants, ce sont : les facteurs financiers, informationnels, d’aide et de support à l’intégration professionnelle. Résultats : Les 22 participants étaient composés de 13 hommes et de 9 femmes, âgés de 29 à 53 ans, originaires de 15 pays différents et ayant une durée de séjour au Québec de 2 à 15 ans. Lors des entrevues, les émergences recueillies grâce aux questions ouvertes ont permis d’appréhender les difficultés spécifiques perçues par les médecins immigrants qui n’ont pas été soulevées ii par la littérature. Les barrières à l’intégration professionnelle perçues par les médecins immigrants sont surtout de nature procédurale, mais également institutionnelles, et dans une moindre mesure, socioculturelles. Les principales barrières procédurales sont relatives aux facteurs informationnels, à l’évaluation des connaissances et des compétences médicales, mais surtout à l’absence de garantie de l’octroi des postes de résidence même après la réussite des examens exigés. Les facteurs facilitants les plus importants sont l’harmonisation des procédures provinciales et fédérales, l’amélioration relative de l’information, le soutien linguistique pour les nonfrancophones et la pénurie des médecins. Conclusion : La réduction des nombreuses barrières identifiées dans cette recherche, permettra non seulement de faciliter l’intégration professionnelle des médecins immigrants, mais également de contribuer à réduire la pénurie de ces professionnels de la santé au Québec.

Afin de lutter contre la mortalité maternelle dont les taux restent encore élevés avec 401 pour 100 000 naissances vivantes en 2005, le Sénégal a mis en œuvre dans la continuité de ses stratégies novatrices de délégation de compétences pour renforcer l’accès aux soins obstétricaux et néonataux d’urgence, un programme alternatif de formation au D.E.S de Gynécologie et Obstétrique de médecins généralistes basés dans les zones éloignées. A partir du curriculum du programme classique, ce programme allie l’enseignement à distance à l’aide des Nouvelles Technologies de l’Information et de la Communication, des stages pratiques dans les sites du CHU, le coaching par les enseignants du CHU dans les Centres de santé où officient les médecins apprenants, des évaluations en ligne et les évaluations annuelles formelles du programme de formation classique. Il a été mis en œuvre dans deux districts du Sénégal par la Chaire de Gynécologie et d’Obstétrique du CHU de Dakar. Ce travail présente l’évaluation à mi-parcours de ce programme en analysant son implantation et ses effets à travers une étude de cas, grâce à des données qualitatives et quantitatives. Les résultats montrent : une évolution favorable des indicateurs sanitaires dans les deux sites du programme comparativement aux sites témoins, une augmentation du niveau de connaissance et de compétence des apprenants, et un niveau de satisfaction élevé des deux apprenants, des enseignants et des bénéficiaires. Cependant, des améliorations sont à apporter dans la mise en œuvre du programme en particulier en termes d’opérationnalisation des innovations pédagogiques, de coordination et de pérennisation par le Ministère de la Santé.
