Aborda aspectos da assistência domiciliar ao idoso. Durante a velhice há uma série de problemas comuns à idade, são as síndromes geriátricas. Elas não constituem um risco de morte iminente, mas podem comprometer severamente a qualidade de vida e o bem-estar dos portadores e, muitas vezes, até dos seus familiares. Por esse motivo o Ministério da Saúde recomenda a avaliação do idoso nos âmbitos da alimentação e nutrição, acuidade visual e auditiva, vacinação, mobilidade, incontinência urinária, quedas, sexualidade, depressão, avaliação cognitiva e funcional. Nas visitas domiciliares, os profissionais da área de enfermagem devem estar atentos sobre como ocorrem as queixas feitas pelos idosos, que nem sempre referenciam de forma clara e direta seu estado de saúde real. Além de identificar os possíveis riscos domésticos, como as quedas, que podem ter consequências graves para o idoso. Diante desse fato, o Ministério da Saúde também recomenda algumas medidas práticas que podem ser tomadas para minimizar as quedas e suas consequências entre as pessoas idosas. E ainda adverte sobre algumas situações de violência que podem ser mascaradas como quedas, por conta da vítima (por vergonha, medo, culpa), por conta do agressor (por negação, isolamento ou medo) e por conta dos profissionais que não se encontram preparados

Observar a conduta durante as consultas, tanto na unidade básica de saúde quanto nas visitas domiciliares, é fundamental na atuação dos profissionais da área odontológica, pois o tratamento que o idoso receberá o tornará mais receptivo, havendo maior colaboração por parte dele. As atividades de educação em saúde bucal devem ser planejadas, organizadas e supervisionadas. Orientações como o uso correto das próteses dentárias, o autoexame de prevenção do câncer bucal e o posicionamento adequado da cadeira odontológica são alguns dos principais cuidados a serem atentados pelos profissionais da área odontológica. Assim como, perceber que há pacientes com dificuldades para caminhar, subir escadas, sentar-se e levantar-se e, ainda, levar em consideração as alterações sistêmicas do paciente idoso a fim de assegurar um atendimento eficiente e eficaz

O presente trabalho foi desenvolvido na cidade de Ipaba, situada na região leste de Minas Gerais, em uma unidade de saúde denominada Centro. A partir do diagnóstico situacional realizado, observou-se uma série de problemas, dentre os quais, destaca-se o elevado índice de gestantes adolescentes. Nesse sentido, justifica-se este estudo e uma atuação da equipe pelos riscos de complicações às mães e aos bebês envolvidos. Sendo assim, o objetivo é elaborar um projeto de intervenção para a implantação de ações estratégicas para a redução da gravidez nas adolescentes residentes na área de abrangência da equipe saúde da família. Foi desenvolvido um plano operativo e um modelo de gestão visando à execução do mesmo

A utilização em excesso de psicofármacos é um problema que ocorre em todo mundo. Trata-se de um tema relevante para a Estratégia de Saúde da Família, pois este problema é uma realidade sociedade e constitui motivo de preocupação para o setor da saúde. Nesse sentido, este trabalho objetivou a elaboração de um projeto de intervenção para a implantação de estratégias que reduzam o uso de psicofármacos na Unidade de Saúde da Família VIII em Belo Oriente-MG. Para elaboração deste trabalho, buscou-se fundamentação na base de dados da Scielo, Lilacs, Biblioteca Virtual de Saúde e outros, utilizando os descritores: Psicofármacos, atenção primária a saúde e dependência química. A análise do material permitiu retratar que o número de pessoas usuárias desses medicamentos é considerável, devido à falta de projetos voltados para este público, no sentido de desestimular o uso continuo de psicofármacos; falta de integração entro os profissionais de saúde mental, a família e a sociedade, falta de capacitação dos profissionais de saúde da família para lidar com estes pacientes. O planejamento de ações que visem a qualidade de vida dos usuários, disponibilização de outras formas de tratamento e conscientização acerca dos tratamentos e diagnósticos psiquiátricos podem contribuir para o uso racional e consciente desses medicamentos. Os profissionais de saúde envolvidos neste processo devem atuar de forma preventiva, limitando o uso de psicofármacos a suas verdadeiras indicações. Outra estratégia essencial é o trabalho com grupos e famílias.

As infecções de pele são afecções muito comuns na faixa etária pediátrica, podendo variar desde quadros superficiais com cura sem sequelas a quadros graves com envolvimento progre sivo e fatal. O objetivo principal deste trabalho foi propor um projeto de intervenção com vistas a diminuir a incidência de infecções de pele em crianças na área de abrangência da Estratégia Saúde da Família Recanto das Águas,Montes Claros,Minas Gerais.Foi elaborado um projeto de intervenção baseado no diagnóstico situacional realizado na ESF em estudo,com a utilização do método de Planejamento Estratégico em Saúde (PES) e em revisão de literatura na Biblioteca Virtual em Saúde (BVS),na base de dados da SciELO com os descritores:dermatoses,saúde da criança e educação em saúde.Também foram pesquisados Programas do Ministério da Saúde e livros relacionados ao tema de estudo.Espera-se, com a implantação do plano,a diminuição da incidência de infecções de pele na população assistida como consequência da melhoria do conhecimento dessa população sobre as doenças,suas formas de contágio e como preveni-las

BACKGROUND: The model for end-stage liver disease (MELD) was developed to predict short-term mortality in patients with cirrhosis. There are few reports studying the correlation between MELD and long-term posttransplantation survival. AIM: To assess the value of pretransplant MELD in the prediction of posttransplant survival. METHODS: The adult patients (age >18 years) who underwent liver transplantation were examined in a retrospective longitudinal cohort of patients, through the prospective data base. We excluded acute liver failure, retransplantation and reduced or split-livers. The liver donors were evaluated according to: age, sex, weight, creatinine, bilirubin, sodium, aspartate aminotransferase, personal antecedents, brain death cause, steatosis, expanded criteria donor number and index donor risk. The recipients' data were: sex, age, weight, chronic hepatic disease, Child-Turcotte-Pugh points, pretransplant and initial MELD score, pretransplant creatinine clearance, sodium, cold and warm ischemia times, hospital length of stay, blood requirements, and alanine aminotransferase (ALT >1,000 UI/L = liver dysfunction). The Kaplan-Meier method with the log-rank test was used for the univariable analyses of posttransplant patient survival. For the multivariable analyses the Cox proportional hazard regression method with the stepwise procedure was used with stratifying sodium and MELD as variables. ROC curve was used to define area under the curve for MELD and Child-Turcotte-Pugh. RESULTS: A total of 232 patients with 10 years follow up were available. The MELD cutoff was 20 and Child-Turcotte-Pugh cutoff was 11.5. For MELD score > 20, the risk factors for death were: red cell requirements, liver dysfunction and donor's sodium. For the patients with hyponatremia the risk factors were: negative delta-MELD score, red cell requirements, liver dysfunction and donor's sodium. The regression univariated analyses came up with the following risk factors for death: score MELD > 25, blood requirements, recipient creatinine clearance pretransplant and age donor >50. After stepwise analyses, only red cell requirement was predictive. Patients with MELD score < 25 had a 68.86%, 50,44% and 41,50% chance for 1, 5 and 10-year survival and > 25 were 39.13%, 29.81% and 22.36% respectively. Patients without hyponatremia were 65.16%, 50.28% and 41,98% and with hyponatremia 44.44%, 34.28% and 28.57% respectively. Patients with IDR > 1.7 showed 53.7%, 27.71% and 13.85% and index donor risk <1.7 was 63.62%, 51.4% and 44.08%, respectively. Age donor > 50 years showed 38.4%, 26.21% and 13.1% and age donor <50 years showed 65.58%, 26.21% and 13.1%. Association with delta-MELD score did not show any significant difference. Expanded criteria donors were associated with primary non-function and severe liver dysfunction. Predictive factors for death were blood requirements, hyponatremia, liver dysfunction and donor's sodium. CONCLUSION: In conclusion MELD over 25, recipient's hyponatremia, blood requirements, donor's sodium were associated with poor survival.

CONTEXT: Intestinal constipation - a common symptom among the general population - is more frequent in women. It may be secondary to an improper diet or organic or functional disturbances, such as dyskinesia of the pelvic floor. This is basically characterized by the absence of relaxation or paradoxical contraction of the pelvic floor and anal sphincter during evacuation. OBJECTIVE: To analyze, by manometric data, the anal pressure variation at rest, during evacuation effort by using the Valsalva maneuver and forced post-expiratory apnea in subjects with secondary constipation. METHODS: Twenty-one patients (19 females - 90.4%) with a mean age of 47.5 years old (23-72) were studied. The diagnosis was performed using anorectal manometry, with a catheter containing eight channels disposed at the axial axis, measuring the proximal (1) and distal (2) portions of the anal orifice. The elevation of the pressure values in relation to the resting with the evacuation effort was present in all patients. The Agachan score was used for clinical evaluation of constipation. The variables studied were: mean anal pressure of the anal orifice for 20 seconds at rest, the effort of evacuation using Valsalva maneuver and the effort of evacuation during apnea after forced expiration, as well as the area under the curve of the manometric tracing at moments Valsalva and apnea. RESULTS: The analysis of the mean values of the anal pressure variation at rest evidenced difference between proximal and distal channels (P = 0.007), independent of the moment and tendency to differ during moments Valsalva and apnea (P = 0.06). The mean of values of the area under the manometric tracing curve showed differences between moments Valsalva and apnea (P = 0.0008), either at the proximal portion or at the distal portion of the anal orifice. CONCLUSION: The effort of evacuation associated with postexpiratory apnea, when compared with the effort associated with the Valsalva maneuver, provides lower elevation of anal pressure at rest by the parameter area under the curve.

In order to evaluate the psychological variables that affect sexual dysfunction (SD) in epilepsy, where compared 60 epileptics (Group 1) with 60 healthy individuals (Group 2), through the State-Trait Anxiety Inventory (Spielberger et al., 1970), Beck Depression Inventory (Beck, 1974) and Sexual Behavior Interview (Souza, 1995). Sexual dysfunction (SD), anxiety and depression were found more frequently in Group 1 than in Group 2 and were not related to sex. Variables such as the onset duration and frequency of seizures as well as the use to medication were not associated with SD. Temporal lobe epilepsy was related to SD (p = 0.035) but not to anxiety or depression. Anxiety and depression were related to SD in both groups. Perception in controlling the seizures was closely related to anxiety (p = 0) and depression (p = 0.009). We conclude that psychological factors play an important role in the alteration of sexual behavior in epileptics and that suitable attention must be given to the control of these variables.

The individual affective-cognitive evaluations are important factors that control the way he feels the disease impact in his life. Then, the perception of seizure control is a more important factor to evaluate Quality of Life (QoL) than the illness characteristics, such as the severity, type, sickening period and seizure frequency. This study searched for the relationship among the subjective variables (perception of seizure control) and the illness characteristics to evaluate QoL. The sample consisted of 60 individuals with chronic epilepsy, aging 18 to 70 (M=37.05; SD=11.25), chosen at randon from the ambulatory of epilepsy - HC/UNICAMP, by the Questionnaire 65. The illness characteristics were not significant, except the seizures frequency, when associated to the impairment in QoL among controlled seizures and seizures with frequency higher than 10 per month (p=0.021). The perception of control was significantly associated to QoL (p=0.005).

OBJECTIVE: To verify the effectiveness of the support group in the identification of family variables linked to epilepsy. METHOD: Pre-test were applied to parents of 21 children with benign epilepsy of childhood recently diagnosed, from 5 to 15 years, who participated in the groups at HC/Unicamp. There was a presentation of an educational video, discussion and application of the post-test 1. After six months, the post-test 2 was applied. RESULTS: The beliefs were: fear of swallowing the tongue during the seizures (76.19%) and of a future mental disease (66.67%). Facing the epilepsy, fear and sadness appeared. 76.19% of the parents presented overprotection and 90.48%, expected a new seizure. In the post-test 1, the parents affirmed that the information offered had modified the beliefs. In the post-test 2, 80.95% didn't report great doubts about epilepsy and 90.48% considered their relationship with their children better. CONCLUSIONS: The demystification of beliefs supplied from the groups influenced the family positively, prevented behavior alterations and guaranteed effective care in the attendance to the child with epilepsy.

This article is part of a study considering the growing importance of the international transit of people, knowledge, and practices in the schooling and professional education processes of some social segments. Considering the public funds made available by the Coordination for the Improvement of Higher Education Personnel - Capes -, the National Council for Scientific and Technological Development - CNPq - and the State of São Paulo Research Foundation - Fapesp - to support researchers' fellowships abroad, aming to improve research and investments on Science and Technology on the context of international exchanges, we have dedicated this article to the preliminary description and analysis of the database of fellows funded abroad by these research agencies from 1970 to 2000. The movement of flows based on the quantitative methodology of the correlation of variables draws the trends of international academic exchange programs in the three research institutions and in the different areas of knowledge, and we intend to analyse them taking into account the scientific and technological development policies adopted by Brazilian State on the period.

Knowing the importance that the poultry industry represents for the Brazilian economy, this work, searched to understand and to identify new welfare pointers inherent to the animal that contributed for the increase of the productive effectiveness, studying different behavior reactions in broiler breeders, in climatic chamber. The experiment was delineated as a Latin Square 3x3x3, where the variable: temperature of air, birds ration and birds age had been controlled. The birds of different ages had been lodged in distinct boxes. Observations of the behavior of the birds in two schedules of the day had been made, being one in the morning and the other one in the afternoon, during a period of 15 minutes each through video cameras, installed in the ceiling of the climatic chamber, having no interference of human being in the register of the data. It was verified the influence of the controlled variables in diverse observed behaviors where it was concluded that the presence of food resulted in bigger occurrences of aggressiveness reactions.

The efficiency of swine production performance depends on the herd administration, such as good nutrition, sanitary control, facilities and appropriate environmental conditions. The concept of this production model is directly related with the reduction of selective losses and the process control. Each production segment is controlled to reach the optimization in the system totality, it is necessary to apply animals handling concepts, environmental control implementation, diseases control, nutrition control, information concerning in guaranteeing the animal welfare and individual identification. The present work presents as objective the development of the mathematical model to evaluate interactions among the internal atmosphere of the installation and the thermal animals preference, in the expectation of detecting a relationship among the frequency access to the drinking fountain and the atmosphere conditions - temperature, black globe temperature and relative humidity, using as tool the electronic identification. The results obtained by the mathematical model, allowed to conclude accurately the evaluation of the swine thermal preference correlating with the climatic variables in the pregnancy stage.

The physical model was based on the method of Newton-Euler. The model was developed by using the scientific computer program Mathematica®. Several simulations where tried varying the progress speeds (0.69; 1.12; 1.48; 1.82 and 2.12 m s-1); soil profiles (sinoidal, ascending and descending ramp) and height of the profile (0.025 and 0.05 m) to obtain the normal force of soil reaction. After the initial simulations, the mechanism was optimized using the scientific computer program Matlab® having as criterion (function-objective) the minimization of the normal force of reaction of the profile (FN). The project variables were the lengths of the bars (L1y, L2, l3 and L4), height of the operation (L7), the initial length of the spring (Lmo) and the elastic constant of the spring (k t). The lack of robustness of the mechanism in relation to the variable height of the operation was outlined by using a spring with low rigidity and large length. The results demonstrated that the mechanism optimized showed better flotation performance in relation to the initial mechanism.

A base-cutter represented for a mechanism of four bars, was developed using the Autocad program. The normal force of reaction of the profile in the contact point was determined through the dynamic analysis. The equations of dynamic balance were based on the laws of Newton-Euler. The linkage was subject to an optimization technique that considered the peak value of soil reaction force as the objective function to be minimized while the link lengths and the spring constant varied through a specified range. The Algorithm of Sequential Quadratic Programming-SQP was implemented of the program computational Matlab. Results were very encouraging; the maximum value of the normal reaction force was reduced from 4,250.33 to 237.13 N, making the floating process much less disturbing to the soil and the sugarcane rate. Later, others variables had been incorporated the mechanism optimized and new otimization process was implemented .
