Le stress oxydatif peut provenir de sources exogènes comme les UVA ou de sources endogènes comme la chaîne respiratoire (OXPHOS). L’oxydation des composants cellulaires a été associée avec la dégénération, des phénotypes de vieillissement et des pertes de fonctionnalités des tissus. Les UVA sont les plus efficaces des rayons UV à induire de l’oxydation, tel que démontré par la formation de dommages oxydatifs à l’ADN et par l’apparition de délétions mitochondriales qui en résultent. La délétion mitochondriale de 4977 pb (ADNmtCD4977), la plus commune, et celle de 3895 pb (ADNmt3895) sont deux délétions reliées au photovieillissement cutané et à l’exposition au stress oxydant. Le phénomène de vieillissement dans la peau est bien documenté et se traduit par une dégradation de la matrice extracellulaire, une perte d’élasticité et la formation de rides. Toutefois, peu d’études portent sur la cornée humaine alors qu’elle est un tissu exposé directement aux rayonnements UV au même titre que la peau. Nous avons donc tenté mieux comprendre l’effet de l’oxydation exogène et endogène sur cette structure. L’analyse de la localisation des délétions ADNmtCD4977 et ADNmtCD4977 dans l’oeil humain a permis de révéler qu’elles se concentrent principalement dans le stroma cornéen et s’accumule avec l’âge. Le stroma cornéen est la couche cellulaire qui confère la transparence et la rigidité à la cornée humaine. Ces résultats nous ont suggéré une implication des UVA dans le photovieillissement de la cornée. Nous avons donc entrepris de vérifier les changements liés à l’exposition aux UVA dans le stroma cornéen puisque les UVA sont connus pour causer des altérations à la matrice extracellulaire (ECM) au niveau cutané. Nous avons donc créé un modèle de photovieillisement par une exposition chronique aux UVA sur des kératocytes avec lesquels nous avons fait sécréter une ECM. Nos résultats nous ont démontré qu’une exposition chronique aux UVA cause des altérations à l’ECM cornéen semblable à des phénotypes de photvieillissement. En effet, nous avons dénoté des changements transcriptomiques et protéomiques pour certains collagènes et protéoglycans. Une atteinte aux collagènes par le vieillissement cornéen se traduit entre autres par une rigidification, une opacification et un changement dans son pouvoir réfractif qui mène à une perte de la vision. Par ailleurs, notre avons également investigué l’implication du stress oxydatif dans la dystrophie cornéenne endothéliale de Fuchs (FECD), une maladie dégénérative de l’endothélium cornéen, qui mène à une perte de vision et est une cause principale de greffe cornéenne. L’étiologie de la maladie est encore inconnue, mais le stress oxydatif est soupçonné de jouer un rôle important dans la pathogenèse. Nos résultats ont amené de nouvelles évidences de l’implication de l’oxydation dans la maladie par l’augmentation de la quantité d’ADN mitochondrial et un raccourcissement des télomères dans des explants de cornées pathologiques. Nos résultats nous ont également démontré que la mise en culture de cellules FECD permettait la sélection de cellules fonctionnelles et comparables à des cellules saines en termes de quantité d’ADN mitochondrial et de son intégrité, de sensibilité à l’oxydation et de longueur télomérique. Les résultats obtenus soutiennent ainsi la possibilité d’employer les cellules FECD fonctionnelles sélectionnées pour utilisation en génie tissulaire afin de créer des cornées autologues pour pallier aux manques de greffes cornéennes. Enfin, nos résultats apportent de nouvelles évidences quant à l’implication du stress oxydatif dans le photovieillissement cornéen et dans l’étiologie de la FECD.

Ocular rosacea is an important and underdiagnosed chronic inflammatory disorder observed in children. A clinical spectrum ranging from chronic eyelid inflammation, recurrent ocular redness, photophobia and/or hordeola/chalazions and conjunctival/corneal phlyctenules evolving to neovascularization and scarring may occur. Visual impairment and consequent amblyopia are frequent and corneal perforation although rare is the most feared complication. Ocular manifestations usually precede cutaneous lesions. Although few cases of pediatric ocular rosacea (POR) have been reported in the literature, many cases must have been underdiagnosed or misdiagnosed. The delay in diagnosis is greater than one year in the large majority of cases and may lead to serious ocular sequelae. This review aims to highlight the clinical features of POR, its epidemiology, easy diagnosis and effective treatment. We also propose new diagnostic criteria, in which at least three of the five clinical criteria must be present: (1) Chronic or recurrent keratoconjunctivitis and/or red eye and/or photophobia; (2) Chronic or recurrent blepharitis and/or chalazia/ hordeola; (3) Eyelid telangiectasia documented by an ophthalmologist; (4) Primary periorificial dermatitis and/ or primary features of rosacea; and (5) Positive familial history of cutaneous and/or ocular rosacea.

PURPOSE: To analyze the outcomes of intracorneal ring segment (ICRS) implantation for the treatment of keratoconus based on preoperative visual impairment. DESIGN: Multicenter, retrospective, nonrandomized study. METHODS: A total of 611 eyes of 361 keratoconic patients were evaluated. Subjects were classified according to their preoperative corrected distance visual acuity (CDVA) into 5 different groups: grade I, CDVA of 0.90 or better; grade II, CDVA equal to or better than 0.60 and worse than 0.90; grade III, CDVA equal to or better than 0.40 and worse than 0.60; grade IV, CDVA equal to or better than 0.20 and worse than 0.40; and grade plus, CDVA worse than 0.20. Success and failure indices were defined based on visual, refractive, corneal topographic, and aberrometric data and evaluated in each group 6 months after ICRS implantation. RESULTS: Significant improvement after the procedure was observed regarding uncorrected distance visual acuity in all grades (P < .05). CDVA significantly decreased in grade I (P < .01) but significantly increased in all other grades (P < .05). A total of 37.9% of patients with preoperative CDVA 0.6 or better gained 1 or more lines of CDVA, whereas 82.8% of patients with preoperative CDVA 0.4 or worse gained 1 or more lines of CDVA (P < .01). Spherical equivalent and keratometry readings showed a significant reduction in all grades (P ≤ .02). Corneal higher-order aberrations did not change after the procedure (P ≥ .05). CONCLUSIONS: Based on preoperative visual impairment, ICRS implantation provides significantly better results in patients with a severe form of the disease. A notable loss of CDVA lines can be expected in patients with a milder form of keratoconus.

Introdução: O queratocone é uma ectasia progressiva e não inflamatória da córnea, relativamente comum na prática clinica, e que se manifesta por uma diminuição progressiva da acuidade visual, associada a miopia e astigmatismo miópico irregular de difícil correcção. Os anéis corneanos intraestromais são uma opção terapêutica nos doentes com queratocone, visando melhorar a acuidade visual, a maior tolerância ao uso de lentes de contacto e protelar a necessidade de um transplante de córnea. Os autores apresentam os resultados da sua experiência, nos últimos 3 anos, com o implante de anéis intraestromais INTACS SK® em doentes com queratocone. Material e Métodos: Foram incluídos 21 olhos de 20 doentes submetidos a colocação de segmentos de anéis intraestromais –Intacs SK®-, por técnica manual assistida por vácuo. A técnica cirúrgica consistiu na criação de uma incisão radial no eixo mais curvo, com zona óptica de 7 mm, e criação de túneis por dissecção mecânica sob vácuo. Os critérios de inclusão foram: queratocone moderado a severo com Km<65D, sem compromisso da transparência central da córnea, baixa acuidade visual e/ou intolerância às lentes de contacto e paquimetria superior a 400 micras na área de inserção. Todos os doentes foram submetidos a exame oftalmológico completo e estudo topográfico corneano com Pentacam®. Foram registados os seguintes parâmetros no pré e pós-operatório (3 a 6 meses): melhor acuidade visual corrigida, equivalente esférico, esfera e cilindro, queratometria e paquimetria. Foram ainda registadas as complicações pós-operatórias. Utilizou-se o Teste Paired-samples t-test e o Teste de Wilcoxon para determinar se existia diferença estatisticamente significativa entre o pré e o pós-operatório em relação aos parâmetros analisados, adoptando-se um nível de significância de 5%. Resultados: Foram incluídos 21 olhos de 20 doentes, 9 mulheres e 11 homens. A idade média foi de 33,81 anos, variando entre os 12 e os 70 anos. Para todos os parâmetros analisados encontrou-se uma diferença estatisticamente significativa (p <0,05) entre o pré e o pós-operatório. Após 3 a 6 meses de colocação dos anéis, verificou-se melhoria de acuidade visual em 85,7% dos doentes, com valor médio de 0,5 pré-operatório para 0,3 unidades LogMar pós-operatório. O equivalente esférico médio diminuiu de -7,13 dioptrias pré-operatórias para -4,19 dioptrias pós-operatórias (redução de 2,94 dioptrias) e a queratometria média de 50,44 dioptrias para 48,01 dioptrias (redução de 2,43 dioptrias). Com um tempo máximo de seguimento de 3 anos, os depósitos estéreis no túnel estromal foram a complicação mais frequente no pós-operatório, não se verificando outras complicações como extrusão ou migração do anel, neovascularização ou infecção corneana. Conclusão: O implante de anéis intraestromais INTACS SK® permitiu melhorar a acuidade visual corrigida na grande maioria dos doentes, com redução do equivalente esférico e da queratometria e aumento da regularidade topográfica.

Introdução: A disponibilidade de córneas para transplante continua a ser um fator limitante na resposta terapêutica a muitos doentes com patologia de córnea. A operacionalização do programa de colheita e transplante de córneas no Hospital Prof. Doutor Fernando Fonseca, EPE (HFF) teve início em 2012 e desde então tem sido preocupação do núcleo de Coordenação Hospitalar de Doação (NCHD) a otimização do número de colheitas respeitando os padrões de qualidade definidos pela legislação. Metodologia: Os autores apresentam uma análise retrospetiva de todos os óbitos ocorridos no HFF no ano de 2014, aplicando os critérios de seleção demográficos e clínicos bem como as limitações operacionais próprias do hospital, de modo a verificar a eficiência do processo de colheita de córneas em coração parado. Resultados: Dos 1970 óbitos do HFF em 2014, 651 (33%) cumpriam o critério idade e 66 doentes foram elegíveis tendo em conta as contra-indicações clínicas e as limitações operacionais do HFF. Destes, 32 foram efectivados como colheitas. Das contra-indicações clínicas a neoplasia de órgão sólido (n=428), serologias positivas (n= 196),doença neurodegenerativa (n=132) e sepsis (n=117) foram as mais prevalentes considerando os óbitos entre os 12 e os 80 anos. Conclusões: Conclui-se que existe uma margem importante de melhoria no processo de referenciação e selecção de dadores. Um eventual ajuste nos critérios clínicos de inclusão, respeitando as guidelines internacionais poderiam possibilitar um significativo incremento da quantidade de córneas elegíveis para colheita.

Purpose: To compare signs and symptoms of dry eye in keratoconus (KC) patients versus healthy subjects. Methods: A total of 15 KC patients (KC group, n = 15 eyes) and 16 healthy subjects (control group, 16 eyes) were enrolled in this study. The Schirmer I test with no anesthetic, tear break-up time (TBUT), corneal staining characteristics, and ocular surface disease index (OSDI) scores were evaluated for both groups. Impression cytology, combined with/scanning laser confocal microscopy (LCM), was performed to evaluate goblet cell density, mucin cloud height (MCH), and goblet cell layer thickness (CLT). Finally, tear concentrations of di-adenosine tetraphosphate (Ap4A) were assessed. Results were statistically analyzed using Shapiro–Wilk and non-parametric Wilcoxon rank sum tests. Statistical significance was set at p < 0.05. Results: KC patients had lower tear volumes and greater corneal staining than did healthy subjects (p < 0.05). OSDI scores were 44.96 ± 8.65 and 17.78 ± 6.50 for the KC and control groups, respectively (p < 0.05). We found no statistically significant differences in TBUT between groups. Impression cytology revealed lower goblet cell densities in KC group patients versus control group subjects (84.88 ± 32.98 and 128.88 ± 50.60 cells/mm,2 respectively, p < 0.05). There was a statistically significant reduction in MCH and CLT in KC group patients compared with control group subjects. Ap4A tear concentrations were higher in KC group patients than in control group subjects (2.56 ± 1.10 and 0.15 ± 0.12 µM, respectively, p < 0.05). Conclusions: The parameters evaluated in this study indicate that KC patients suffer greater symptoms of dry eye and greater tear instability, primarily due to the decreased mucin production in their tears, than do healthy patients with no KC.

Purpose. To compare the intraocular pressure (IOP) before and after Laser In Situ Keratomileusis (LASIK), measured by Diaton, Perkins, and noncontact air pulse tonometers. Methods. Fifty-seven patients with a mean age of 34.88 were scheduled for myopia LASIK treatment. Spherical equivalent refraction (SER), corneal curvature (K), and central corneal thickness (CCT) and superior corneal thickness (SCT) were obtained before and after LASIK surgery. IOP values before and after surgery were measured using Diaton, Perkins, and noncontact air pulse tonometers. Results. The IOP values before and after LASIK surgery using Perkins tonometer and air tonometers were statistically significant (). However, no significant differences were found () for IOP values measured with Diaton tonometer. CCT decreases significantly after surgery () but no statistical differences were found in SCT (). Correlations between pre- and postsurgery were found for all tonometers used, with and for the air pulse tonometer, and for Perkins, and and for Diaton. Conclusion. Transpalpebral tonometry may be useful for measuring postsurgery IOP after myopic LASIK ablation because this technique is not influenced by the treatment.

Purpose.: 5-Methoxy-carbonylamino-N-acetyltryptamine (5-MCA-NAT, a melatonin receptor agonist) produces a clear intraocular pressure (IOP) reduction in New Zealand White rabbits and glaucomatous monkeys. The goal of this study was to evaluate whether the hypotensive effect of 5-MCA-NAT was enhanced by the presence of cellulose derivatives, some of them with bioadhesive properties, as well as to determine whether these formulations were well tolerated by the ocular surface. Methods.: Formulations were prepared with propylene glycol (0.275%), carboxymethyl cellulose (CMC, 0.5% and 1.0%) of low and medium viscosity and hydroxypropylmethyl cellulose (0.3%). Quantification of 5-MCA-NAT (100 μM) was assessed by HPLC. In vitro tolerance was evaluated by the MTT method in human corneal-limbal epithelial cells and normal human conjunctival cells. In vivo tolerance was analyzed by biomicroscopy and specular microscopy in rabbit eyes. The ocular hypotensive effect was evaluated measuring IOP for 8 hours in rabbit eyes. Results.: All the formulations demonstrated good in vitro and in vivo tolerance. 5-MCA-NAT in CMC medium viscosity 0.5% was the most effective at reducing IOP (maximum IOP reduction, 30.27%), and its effect lasted approximately 7 hours. Conclusions.: The hypotensive effect of 5-MCA-NAT was increased by using bioadhesive polymers in formulations that are suitable for the ocular surface and also protective of the eye in long-term therapies. The use of 5-MCA-NAT combined with bioadhesive polymers is a good strategy in the treatment of ocular hypertension and glaucoma.

Purpose. To analyze the levels of the diadenosine polyphosphates Ap4A and Ap5A in tears, in a set of control subjects and in groups of symptomatic and nonsymptomatic persons with dry eye. Methods. Ninety-seven subjects participated in the study. The subjects were divided into five experimental groups: control subjects; symptomatic patients with normal tear secretion; symptomatic patients with low tear secretion; forced blink; and corneal mechanical stimulation provided by a gas esthesiometer. The Schirmer I test was used to measure and collect tear secretions from each subject. All samples were processed by high pressure liquid chromatography (HPLC) and their Ap4A and Ap5A levels determined. Results. The levels of Ap4A and Ap5A in tears were greater in all symptomatic patients than in control subjects, especially in symptomatic subjects with low tear secretion. Within the symptomatic subjects with normal tear secretion, significant differences in concentrations of Ap4A and Ap5A were found between men and women. In the forced blink experiments, concentrations of the Ap4A and Ap5A rose with increasing blink frequency. When the cornea was mechanically stimulated, the levels of Ap4A and Ap5A rose significantly during both moderate and high-flow rate tests. Conclusions. The increased levels of Ap4A and Ap5A in tears of patients with dry eye allow these dinucleotides to be used as objective biomarkers in dry eye conditions.

Purpose.: To evaluate the levels of dinucleotides diadenosine tetraphosphate (Ap4A) and diadenosine pentaphosphate (Ap5A) in tears of patients wearing rigid gas permeable (RGP) contact lenses on a daily wear basis and of patients wearing reverse-geometry RGP lenses overnight for orthokeratology treatment. Methods.: Twenty-two young volunteers (10 females, 12 males; 23.47 ± 4.49 years) were fitted with an alignment-fit RGP lens (paflufocon B) for a month, and after a 15-day washout period they were fitted with reverse-geometry RGP lenses for corneal reshaping (paflufocon D) for another month. During each period, tears were collected at baseline day 1, 7, 15, and 28. Ap4A and Ap5A were measured by high-pressure liquid chromatography (HPLC). Additionally, corneal staining, break-up time (BUT), Schirmer test, and dryness symptoms were evaluated. Results.: Ap4A concentrations increased significantly from baseline during the whole period of daily wear of RGP lenses (P < 0.001); concentration was also significantly higher than in the orthokeratology group, which remained at baseline levels during the study period except at day 1 (P < 0.001) and day 28 (P = 0.041). While BUT and Schirmer remained unchanged in both groups, discomfort and dryness were significantly increased during alignment-fit RGP daily wear but not during the orthokeratology period. Conclusions.: Daily wear of RGP lenses increased the levels of Ap4A due to mechanical stimulation by blinking of the corneal epithelium, and this is associated with discomfort. Also, orthokeratology did not produce symptoms or signs of ocular dryness, which could be a potential advantage over soft contact lenses in terms of contact lens-induced dryness.

Introduction : Malgré leur état non-prolifératif in vivo, les cellules endothéliales cornéennes (CEC) peuvent être amplifiées in vitro. Leur transplantation subséquente par injection intracamérale pourrait surmonter la pénurie de tissus associée à l’allo-greffe traditionnelle – l’unique traitement définitif disponible pour les endothéliopathies cornéennes. Objectif : Évaluer la fonctionnalité d’un endothélium cornéen reconstitué par injection de CEC dans la chambre antérieure du félin. Méthodes : Les yeux droits de 16 animaux ont été opérés. Huit ont été désendothélialisés centralement avec injection de 2x10e5 (n=4) ou 1x10e6 (n=4) CEC félines supplémentées avec Y-27632 et marquées avec SP-DiOC18(3). Deux ont été désendothélialisés complètement et injectés avec 1x10e6 CEC et Y-27632. Six contrôles ont été désendothélialisés centralement (n=3) ou complètement (n=3) et injectés avec Y-27632 sans CEC. La performance clinique, l’intégrité anatomique, le phénotype fonctionnel et l’expression de SP-DiOC18(3) du nouvel endothélium ont été étudiés. Résultats : Les cornées greffées avec 2x10e5 CEC et les contrôles désendothélialisés centralement ont réussi le mieux cliniquement. Les contrôles désendothélialisés complètement sont restés opaques. L’histopathologie a révélé une monocouche endothéliale fonctionnelle dans les cornées greffées avec 2x10e5 CEC et les contrôles désendothélialisés centralement, une multicouche endothéliale non-fonctionnelle dans les cornées désendothélialisées centralement et greffées avec 1x10e6 CEC, et un endothélium fibrotique non-fonctionnel dans les cornées désendothélialisées complètement. L’expression de SP-DiOC18(3) était rare dans les greffes. Conclusion : La thérapie par injection cellulaire a reconstitué un endothélium partiellement fonctionnel, auquel les CEC injectées n’ont contribué que peu. L’injection de Y-27632 sans CEC a reconstitué l’endothélium le plus sain. Des études additionnelles investiguant l’effet thérapeutique de Y-27632 seul sont justifiées.

It is an Olympic year and we have just witnessed the fantastic games hosted by Rio de Janeiro. Well done to team USA for winning the most medals overall but also well done to so many other nations and individuals who performed so well or were ambassadors in other ways. Teenage swimmer Yusra Mardini who swam for the refugee team and South Africa's Wayde van Niekerk who broke the longstanding 400 m record of Michael Johnson that has stood since 1999. Of course, we must mention sprinter Usain Bolt and swimmer Michael Phelps, who have now transcended superstar status and entered a new level of icon. My personal highlight was the sportsmanship witnessed in the 5000 m when American Abbey D’Agostino was accidentally felled by New Zealand runner Nikki Hamblin. D’Agostino helped Hamblin back to her feet but slumped to the track after realising her own injury. Hamblin helped her up and stayed with her so that both completed the race. The International Olympic Committee has awarded both with the prestigious Pierre de Coubertin award, also known as the International Fair Play Trophy. Fair play is of paramount importance in publishing in peer-reviewed papers. At CLAE we try and maintain, as do other journals, this by ensuring double blind peer review and allowing authors to select the most appropriate handling editor for their submission. Our handling editors are placed across the world (2 in Europe, 1 in the Americas, 1 in Australia and 1 in Asia) and part of their role is to encourage submissions from their region. Over the last decade we certainly have seen more and more papers from places that haven’t previously published in CLAE. In this issue of CLAE we have a true international blend of papers. We have papers from authors from the UK, USA, Iran, Jordan, France, Poland, Turkey, Nigeria, France, Spain and Brazil. I think it's a testament to the continued success of the journal that we are attracting new writers from so many parts of the world and retain papers from more established authors and research centres. We do continue to attract many weaker papers that are rejected early in the review process. Often these will be unexceptional case reports or papers describing a surgical technique. Case reports are published but only those that offer something original and especially those with interesting photographs. In this issue you will see Professor James Wolffsohn (UK) has an interesting paper around a lot of the focus of his recent research activity into clinical evaluation of methods of correcting presbyopia. In this paper he highlights predictors to aid success of presbyopic contact lenses. If you have been involved in any clinical work or research in the field of dry eye disease then you will know well the CLDEQ (Contact Lens Dry Eye Questionnaire) devised by Robin Chalmers and her colleagues (USA). This issue of CLAE details the latest research using the CLDEQ-8 (the 8 item version of the CLDEQ). The Shahroud Eye Cohort Study has produced many papers already and in this issue we see Fotouhi Akbar (Iran) looking at changes in central and peripheral corneal thickness over a five year period. These days we use a lot of new instrumentation, such as optical low-coherence reflectometry. In this issue Emre Güler (Turkey) compares that to a new optical biometry unit. Dry eye is more common and in this issue we see a study by Oluyemi Fasina (Nigeria) to investigate the disease in adults in South-West Nigeria. The TearLab™ is now commonly used to investigate osmolarity and Dorota Szczesna-Iskander (Poland) looks at measurement variability of this device. Following the theme of dry eyes and tear testing Renaud Laballe (France) looks at the use of scleral lenses as a reservoir-based ocular therapeutic system. In this issue we have a couple of papers looking at different aspects of keratoconus. Magdalena Popiela (UK) looks at demographics of older keratoconic patients in Wales, Faik Orucoglu (Turkey) reports a novel scoring system for distinguishing keratoconus from normal eyes, Gonzalo Carracedo (Spain) reports the effect of rigid gas permeable lens wear on dry eye in keratoconus and Hatice Nur Colak (Turkey) compares topographic and aberrations in keratoconus. Purpose: The primary outcome of this study is to compare the axial length growth of white European myopic children wearing orthokeratology contact lenses (OK) to a control group (CT) over a 7-year period. Methods: Subjects 6–12 years of age with myopia −0.75 to −4.00DS and astigmatism ≤1.00DC were prospectively allocated OK or distance single-vision spectacles (SV) correction. Measurements of axial length (Zeiss IOLMaster), corneal topography, and cycloplegic refraction were taken at 6-month intervals over a 2-year period. Subjects were invited to return to the clinic approximately 5 years later (i.e., 7 years after the beginning of the study) for assessment of their ocular refractive and biometric components. The CT consisted of 4 SV and 12 subjects who switched from SV to soft contact lens wear after the initial 2 years of SV lens wear. Changes in axial length relative to baseline over a 7-year period were compared between groups. Results: Fourteen and 16 subjects from the OK and CT groups, respectively, were examined 6.7 ± 0.5 years after the beginning of the study. Statistically significant changes in the axial length were found over time and between groups (both p <0.001), but not for the time*group interaction (p = 0.125). The change in the axial length for the OK group was 22% (p = 0.328), 42% (p = 0.007), 40% (p = 0.020), 41% (p = 0.013), and 33% (p = 0.062) lower than the CT group following 6, 12, 18, 24, and 84 months of lens wear, respectively. Conclusion: A trend toward a reduction in the rate of axial elongation of the order of 33% was found in the OK group in comparison to the CT group following 7 years of lens wear.
