Background: Many obese patients cannot lose weight or reject conventional obesity management. Endoscopic sleeve gastroplasty (the Apollo method) is a pioneering coadjuvant, interventionist technique for the integral management of obesity. Objectives: The goals of this study were to report safety and efficacy results obtained at 6 months in patients undergoing endoscopic sleeve gastroplasty. Material and methods: A prospective study was performed in 55 patients (13 males, 42 females) who were subjected to the Apollo technique; mean age was 43.5 years (range 25-60) and mean BMI was 37.7 kg/m² (range 30-48). All received multidisciplinary follow-up for weight loss. Weight changes and presence of complications were assessed. Through the endoscope a triangular pattern suture is performed consisting of approximately 3-6 transmural (mucosa to serosa) stitches, using a cinch device to bring them nearer and form a plication. Results: A total of 6-8 plications are used to provide a tubular or sleeve-shaped restriction to the gastric cavity. No major complications developed and patients were discharged at 24 hours following the procedure. Endoscopic and radiographic follow-up at 6 months post-procedure showed a well preserved tubular form to the stomach. After 6 months patients had lost 18.9 kg and 55.3% of excess weight. Conclusions: Endoscopic sleeve gastroplasty, together with dietary and psycho-behavioral changes, is a safe, effective technique in the coadjuvant management of obese patients.

Miles e Snow’s configurational theory has received a great deal of attention from many investigators. Framing the Miles e Snow Typology with the organizational configuration concept, the main purpose of this paper is to make an empirical evaluation of what configurational theories postulate: higher organizational performance is associated to the resemblance to one of the ideal types defined. However, as it is often assumed that an organization can increase performance by selecting the adjustable hybrid type to its own exogenous environment, the relation between the organization’s effectiveness and the hybrid configuration alignment to the respective specific environment types was also analyzed. The assumption of equifinality was also considered because the configurational theory assumes that all the ideal types can potentially achieve the same performance level. A multiple regression model was made to confirm if the misfit related to the ideal and hybrid types has significant impact on the organizational effectiveness. The analysis of variance and the Kruskal-Wallis test were used to verify the equality of performance between the different organization types. In short, the empirical results obtained confirm what is postulated in the theory.

The crystalline lens allows the eye to focus on near and far objects. During the aging process, it loses its ability to focus and often becomes cloudy during cataract formation. At this point, traditional medical therapy replaces the lens with an artificial replacement lens. Although replacement lenses for the crystalline lens have been implanted since 1949 for cataract surgery, none of the FDA-approved lenses mimic the anatomy of the natural lens. Hence, they are not able to focus in a manner similar to the youthful lens. Instead, they function in a manner similar to the aged lens and only provide vision at a single distance or at a very limited range of focal distances. Patients with the newest implants are often obliged to use reading glasses when using near vision, or suffer from optical aberrations, halos, or glare. Therefore, there is a need to provide youthful vision after lens surgery in terms of focusing ability, accurate optical power, and sharp focus without distortion or optical aberrations.
This thesis presents an approach to restoring youthful vision after lens replacement. An intraocular lens (IOL) that can provide accurate visual acuity along with focusing ability is proposed. This IOL relies on the natural anatomy and physiology of the eye, and therefore is actuated in a manner identical to the natural lens. In addition, the lens has the capability for adjustment during or after implantation to provide high-acuity vision throughout life.
The natural anatomy and physiology of the eye is described, along with lens replacement surgery. A lens design is proposed to address the unmet need of lens-replacement patients. Specific care in the design is made for small surgical incisions, high visual acuity, adjustable acuity over years, and the ability to focus similar to the natural lens. Methods to test the IOL using human donor tissue are developed based upon prior experiments on the ex vivo natural lens. These tools are used to demonstrate efficacy of the newly developed accommodating intraocular lens.
To further demonstrate implant feasibility, materials and processes for building the lens are evaluated for biocompatibility, endurance, repeatable manufacture, and stability. The lens biomechanics are determined after developing an artificial anatomy testing setup inspired by the natural anatomy of the human focusing mechanism. Finally, based upon a mechanical and optical knowledge of the lens, several improved lens concepts are proposed and demonstrated for efficacy.

Wireless Sensor Networks (WSNs) are widely used for various civilian and military applications, and thus have attracted significant interest in recent years. This work investigates the important problem of optimal deployment of WSNs in terms of coverage and energy consumption. Five deployment algorithms are developed for maximal sensing range and minimal energy consumption in order to provide optimal sensing coverage and maximum lifetime. Also, all developed algorithms include self-healing capabilities in order to restore the operation of WSNs after a number of nodes have become inoperative. Two centralized optimization algorithms are developed, one based on Genetic Algorithms (GAs) and one based on Particle Swarm Optimization (PSO). Both optimization algorithms use powerful central nodes to calculate and obtain the global optimum outcomes. The GA is used to determine the optimal tradeoff between network coverage and overall distance travelled by fixed range sensors. The PSO algorithm is used to ensure 100% network coverage and minimize the energy consumed by mobile and range-adjustable sensors. Up to 30% - 90% energy savings can be provided in different scenarios by using the developed optimization algorithms thereby extending the lifetime of the sensor by 1.4 to 10 times. Three distributed optimization algorithms are also developed to relocate the sensors and optimize the coverage of networks with more stringent design and cost constraints. Each algorithm is cooperatively executed by all sensors to achieve better coverage. Two of our algorithms use the relative positions between sensors to optimize the coverage and energy savings. They provide 20% to 25% more energy savings than existing solutions. Our third algorithm is developed for networks without self-localization capabilities and supports the optimal deployment of such networks without requiring the use of expensive geolocation hardware or energy consuming localization algorithms. This is important for indoor monitoring applications since current localization algorithms cannot provide good accuracy for sensor relocation algorithms in such indoor environments. Also, no sensor redeployment algorithms, which can operate without self-localization systems, developed before our work.

O presente trabalho tem como objetivo estudar a técnica da Expansão Rápida da Maxila, comparando os tipos de aparelhos recomendados consoante as diferentes propriedades, vantagens e desvantagens associadas. Quando pensamos em um padrão normal para a oclusão temos em mente uma correta relação entre as bases apicais em que a arcada maxilar deverá englobar a arcada mandibular permitindo o equilíbrio entre ambas. Quando ocorre uma redução das dimensões transversais da maxila nos vemos diante de atresias, entre elas uma anomalia de má oclusão bastante frequente na nossa clínica, a mordida cruzada posterior. Quando isto acontece o profissional precisa buscar uma correção da conformação do arco através da expansão rápida da maxila abrindo a sutura palatina mediana por meio de aparelhos expansores fixos tais como o de Haas (dento-muco-suportado) ou os classificados como dento-suportados como o Hyrax ou o disjuntor de McNamara, na dentição decídua, mista ou permanente e até mesmo em pacientes adultos com protocolos diferenciados para cada faixa etária. Caso o indivíduo apresente uma calcificação óssea avançada da sutura mediana deve ser encaminhado para a expansão rápida da maxila cirurgicamente assistida.

Les biofilms bactériens sont composés d’organismes unicellulaires vivants au sein d’une matrice protectrice, formée de macromolécules naturelles. Des biofilms non désirés peuvent avoir un certain nombre de conséquences néfastes, par exemple la diminution du transfert de chaleur dans les échangeurs de chaleurs, l’obstruction de membranes poreuses, la contamination des surfaces coques de navires, etc. Par ailleurs, les bactéries pathogènes qui prolifèrent dans un biofilm posent également un danger pour la santé s’ils croissent sur des surfaces médicales synthétiques comme des implants biomédicaux, cathéters ou des lentilles de vue. De plus, la croissance sur le tissu naturel par certaines souches des bactéries peut être fatale, comme Pseudomonas aeruginosa dans les poumons. Cependant, la présence de biofilms reste difficile à traiter, car les bactéries sont protégées par une matrice extracellulaire. Pour tenter de remédier à ces problèmes, nous proposons de développer une surface antisalissure (antifouling) qui libère sur demande des agents antimicrobiens. La proximité et la disposition du système de relargage placé sous le biofilm, assureront une utilisation plus efficace des molécules antimicrobiennes et minimiseront les effets secondaires de ces dernières. Pour ce faire, nous envisageons l’utilisation d’une couche de particules de silice mésoporeuses comme agents de livraison d’agents antimicrobiens. Les nanoparticules de silice mésoporeuses (MSNs) ont démontré un fort potentiel pour la livraison ciblée d’agents thérapeutiques et bioactifs. Leur utilisation en nano médecine découle de leurs propriétés de porosité intéressantes, de la taille et de la forme ajustable de ces particules, de la chimie de leur surface et leur biocompatibilité. Ces propriétés offrent une flexibilité pour diverses applications. De plus, il est possible de les charger avec différentes molécules ou biomolécules (de tailles variées, allant de l’ibuprofène à l’ARN) et d’exercer un contrôle précis des paramètres d’adsorption et des cinétiques de relargage (désorption). The primary aim of this study was to generate normative handgrip strength (HG) data for 10- to 17.9-year-olds. The secondary aim was to determine the relative proportion of Colombian children and adolescents that fall into established Health Benefit Zones (HBZ). This cross-sectional study is enrolling 7268 schoolchildren (boys n=3129 and girls n=4139, age 12.7 (2.4) years old. HG was measured using a hand dynamometer with an adjustable grip. Five HBZs (Needs Improvement, Fair, Good, Very Good, and Excellent) have been established that correspond to combined-HG. Centile smoothed curves, percentile and tables for the 3rd, 10th, 25th, 50th, 75th, 90th and 97th percentile were calculated using Cole’s LMS method. HG peaked in the sample at 22.2 (8.9) kg in boys and 18.5 (5.5) kg in girls. The increase in HG was greater for boys than for girls, but the peak HG was lower in girls than in boys. The HBZ data indicated that a higher overall percentage of boys than girls at each age group fell into the “Needs Improvement” zone, with differences particularly pronounced during adolescence. Our results provide, for the first time, sex- and age-specific HG reference standards for Colombian schoolchildren aged 9-17.9 years.

Regarding canal management modernization, water savings and water delivery quality, the study presents two automatic canal control approaches of the PI (Proportional and Integral) type: the distant and the local downstream control modes. The two PI controllers are defined, tuned and tested using an hydraulic unsteady flow simulation model, particularly suitable for canal control studies. The PI control parameters are tuned using optimization tools. The simulations are done for a Portuguese prototype canal and the PI controllers are analyzed and compared considering a demand-oriented-canal operation. The paper presents and analyzes the two control modes answers for five different offtake types – gate controlled weir, gate controlled orifice, weir with or without adjustable height and automatic flow adjustable offtake. The simulation results are compared using water volumes performance indicators (considering the demanded, supplied and the effectives water volumes) and a time indicator, defined taking into account the time during which the demand discharges are effective discharges. Regarding water savings, the simulation results for the five offtake types prove that the local downstream control gives the best results (no water operational losses) and that the distant downstream control presents worse results in connection with the automatic flow adjustable offtakes. Considering the water volumes and time performance indicators, the best results are obtained for the automatic flow adjustable offtakes and the worse for the gate controlled orifices, followed by the weir with adjustable height.

Advanced cell cultures are developing rapidly in biomedical research. Nowadays, various approaches and technologies are being used, however, these culturing systems present limitations from increasing complexity, requiring high costs, and not easily customization. We present two versatile and cost-effective methods for developing culturing systems that integrate 3D cell culture and microfluidic platforms. Firstly, for drug screening applications, many high-quality cell spheres of homogeneous size and shape are required. Conventional approaches usually have a dearth of control over the size and geometry of cell spheres and require sample collection and manipulation. To overcome this difficulty, in this study, hundreds of spheroids of several cell lines were generated using multi-well plates that housed our microdevices. Tumor spheroids grow at a uniform rate (in scaffolded or scaffold-free environments) and can be harvested at will. Microscopy imaging are done in real time during or after the culture. After in situ immunostaining, fluorescence imaging can be conducted while keeping the spatial distribution of spheroids in the microwells. Drug effects were successfully observed through viability, growth, and morphologic investigations. Also, we fabricated a microfluidic device suitable for directed and selective cell culture treatments. The microfluidic device was used to reproduce and confirm in vitro investigations carried out using normal culture methods, using a microglia cell line. The device layout and the syringe pump system, entirely designed in our lab, successfully allowed culture growth and medium flow regulation. Solution flows can be finely controlled, allowing treatments and immunofluorescence in one single chamber selectively. To conclude, we propose the development of two culturing platforms (microstructured well devices and in-flow microfluidic chip), which are the result of separate scientific investigations but have the primary goal of performing treatments in a reproducible manner. Our devices shall improve future studies on drug exposure testing, representing adjustable and versatile cell culture systems.
