Le réchauffement climatique affecte fortement les régions nordiques du Canada où le dégel du pergélisol discontinu à sa limite sud est accompagné du mouvement de la limite des arbres vers le nord en zone de pergélisol continu. Ces altérations faites aux paysages de la Taïga des Plaines sont le point de départ de plusieurs rétroactions puisque les changements apportés aux caractéristiques de la surface (au niveau de l’albédo, l’humidité du sol et la rugosité de la surface) vont à leur tour entraîner des modifications biophysiques et éventuellement influencer l’augmentation ou la diminution subséquente des températures et de l’humidité de l’air. Seulement, il y a un nombre important de facteurs d’influence qu’il est difficile de projeter toutes les boucles rétroactives qui surviendront avec les présents changements climatiques en régions nordiques. Dans le but de caractériser les échanges d’eau et d’énergie entre la surface et l’atmosphère de trois sites des Territoires du Nord-Ouest subissant les conséquences de l’augmentation des températures de l’air, la méthode micro-météorologique de covariance des turbulences fut utilisée en 2013 aux sites de Scotty Creek (forêt boréale et tourbière nordique en zone de pergélisol sporadique-discontinu), de Havikpak Creek (forêt boréale nordique en zone de pergélisol continu) et de Trail Valley Creek (toundra arctique en zone de pergélisol continu). En identifiant les procédés biotiques et abiotiques (ex. intensité lumineuse, disponibilité en eau, etc.) d’évapotranspiration aux trois sites, les contrôles par l’eau et l’énergie furent caractérisés et permirent ainsi de projeter une augmentation de la limitation en eau, mais surtout en énergie du site de Trail Valley Creek. La répartition de l’énergie projetée est semblable à celle de Havikpak Creek, avec une augmentation de la proportion du flux de chaleur sensible au détriment de celui latent suite aux modifications des caractéristiques de la surface (albédo, rugosité et humidité du sol). L’augmentation relative du flux d’énergie sensible laisse présager une boucle rétroactive positive de l’augmentation des températures de l’air à ce site. Ensuite, en comparant des données modelées de la hauteur de la couche limite planétaire et des données provenant de profils atmosphériques d’Environnement Canada entre les trois sites, les changements de hauteur de cette couche atmosphérique furent aussi projetés. Trail Valley Creek pourrait connaître une hausse de la hauteur de sa couche limite planétaire avec le temps alors que Scotty Creek connaîtrait une diminution de celle-ci. Ces changements au niveau des couches atmosphériques liés à la répartition des flux d’énergie dans les écosystèmes se répercuteraient alors sur le climat régional de façon difficile à déterminer pour l’instant. Les changements apportés désignent une boucle rétroactive positive des températures de l’air à Trail Valley Creek et l’inverse à Scotty Creek. Les deux axes d’analyse arrivent donc aux mêmes conclusions et soulignent aussi l’importance de l’influence mutuelle entre le climat et les caractéristiques spécifiques des écosystèmes à la surface.

A atenção à saúde mental (SM) das pessoas idosas é prioritária. Os problemas de SM neste grupo populacional têm aumentado sendo importante investir na prevenção e rastreio dos mesmos. Este estudo teve como objetivos caraterizar do ponto de vista da SM a população idosa da Região Autónoma da Madeira (RAM); determinar as prevalências das situações de SM positiva e negativa e avaliar a influência positiva (protetora) ou negativa (de risco) de certos fatores pessoais e do meio na SM. Foi um estudo transversal no qual participaram 342 pessoas com 65 e mais anos, dos dois géneros, residentes na comunidade. Destes 67,0 % apresentaram SM positiva. A probabilidade da SM ser mais positiva era 2,5 vezes superior quando possuíam 1 a 11 anos de escolaridade (OR = 2,5 IC 95% 1,3 – 4,8); 0,3 vezes inferior nas mulheres (OR = 0,3 IC 95% 0,1 - 0,6), nos idosos com redes sociais muito limitadas (OR = 0,3 IC 95% 0,1 - 0,9) e nos que percecionavam a saúde como razoável ou pior (OR = 0,3 IC 95% 0,1 - 0,9). Era menor 0,5 vezes quando percecionavam a saúde como pior comparativamente aos pares (OR = 0,5 IC 95% 0,3 - 0,9), e 0,3 vezes comparativamente à detida um ano antes (OR = 0,3 IC 95% 0,2 - 0,6). Era 0,1 vez inferior (OR = 0,1 IC 95% 0,1-0,7) nos idosos com limitações físicas para satisfazerem necessidades próprias. Estes fatores devem considerar-se na promoção da SM e na prevenção de perturbações da mesma.

La tuberculosis TB es una de las principales causas de muerte en el mundo en individuos con infección por VIH. En Colombia esta coinfección soporta una carga importante en la población general convirtiéndose en un problema de salud pública. En estos pacientes las pruebas diagnósticas tienen sensibilidad inferior y la enfermedad evoluciona con mayor frecuencia hacia formas diseminadas y rápidamente progresivas y su diagnóstico oportuno representa un reto en Salud. El objetivo de este proyecto es evaluar el desempeño de las pruebas diagnósticas convencionales y moleculares, para la detección de TB latente y activa pacientes con VIH, en dos hospitales públicos de Bogotá. Para TB latente se evaluó la concordancia entre las pruebas QuantiFERON-TB (QTF) y Tuberculina (PPD), sugiriendo superioridad del QTF sobre la PPD. Se evaluaron tres pruebas diagnósticas por su sensibilidad y especificidad, baciloscopia (BK), GenoType®MTBDR plus (Genotype) y PCR IS6110 teniendo como estándar de oro el cultivo. Los resultados de sensibilidad (S) y especificidad (E) de cada prueba con una prevalencia del 19,4 % de TB pulmonar y extrapulmonar en los pacientes que participaron del estudio fue: BK S: 64% E: 99,1%; Genotype S: 77,8% E: 94,5%; PCRIS6110 S: 73% E: 95,5%, de la misma forma se determinaron los valores predictivos positivos y negativos (VPP y VPN) BK: 88,9% y 94,8%, Genotype S: 77,8% E: 94,5%; PCRIS6110 S: 90% y 95,7%. Se concluyó bajo análisis de curva ROC que las pruebas muestran un rendimiento diagnóstico similar por separado en el diagnóstico de TB en pacientes con VIH, aumentando su rendimiento diagnostico cuando se combinan

Introducción: Las guías de Tokyo de 2013 lograron un consenso respecto al manejo antibiótico de la infección biliar. Sus recomendaciones están sustentadas en estudios internacionales de la epidemiología bacteriana, pero también recalcan la importancia de conocer la microbiología local para ajustar las guías de manejo. Materiales y métodos: Se diseñó un estudio descriptivo tipo serie de casos de pacientes tratados por colecistitis aguda moderada y severa en Méderi Hospital Universitario Mayor (HUM), describiendo los aislamientos microbiológicos y perfiles de resistencia de los cultivos de bilis tomados durante la cirugía. Resultados: Se analizaron 131 pacientes con una edad promedio de 63 años, la mayoría sin comorbilidades médicas. Se encontró un 48% de positividad en los cultivos, predominantemente enterobacterias siendo la más frecuente Escherichia coli, seguida de especies de Klebsiella y de Enterococcus. Los perfiles de resistencia evidenciaron un 93% de multisensibilidad antibiótica y se aislaron 4 microorganismos multirresistentes. No se encontraron diferencias en comorbilidades, alteraciones paraclínicas, presencia de síndrome biliar obstrutivo, pancreatitis o instrumentación previa de la vía biliar entre los pacientes con cultivo positivo y negativo. Conclusiones: Los resultados concuerdan con los reportes internacionales en cuanto a la flora bacteriana aislada, pero los perfiles de resistencia evidenciados en esta serie son diferentes a los que sustentan las guías de manejo de Tokio revisadas en 2013. Este hallazgo obliga a ajustar las guías de manejo institucionales con base en la epidemiología local.

La capacidad de gestión del personal se ha convertido en un imperativo para las organizaciones modernas. Por tanto se vienen introduciendo temas como la administración en valores y el engagement de los trabajadores. Sin embargo, la relación entre estos dos aún no ha sido estudiada. El presente estudio tiene como objetivo analizar el efecto que tiene el grado de articulación y reconocimiento de los valores organizacionales y personales y organizacionales sobre los niveles de engagement de los empleados. Para esta investigación se utilizó una muestra constituida por 54 trabajadores de una organización del sector salud de la ciudad de Bogotá a quienes les fueron aplicadas dos escalas: el Inventario para Medir la Articulación entre la Persona y la Organización (Inventario APO) y Utrech Work Engagement Scale (UWES). Como principal resultado se obtuvo que de las tres dimensiones consideradas predictoras del engagement, solamente el reconocimiento de los valores organizacionales tuvo un efecto estadísticamente significativo.

This work aims at improving the knowledge on fog formation and its evolution in the Alentejo region (Portugal). For this purpose, brief regional fog climatology, essentially based on information from the Beja Air Base meteorological station, was produced and several numerical high resolution simulations were performed using the Meso-NH. The ECOCLIMAP database used to generate the model physiography was improved to include the Alqueva reservoir (~250 km2), filled in 2003. The model results were compared with surface and satellite observations, showing good agreement in terms of fog occurrence and persistence. Various forcing mechanisms for formation, development, and dissipation of fog were identified, confirming the influence of two small mountains that block the moist air from the Atlantic Ocean, preventing the fog from reaching innermost regions. The introduction of the Alqueva large reservoir induces changes in the landscape and environment. The effects of the water vapour addition and of the changes in mass and energy surface fluxes on fog formation and evolution were studied. It was found that the reservoir may have a direct impact on fog formation over the lake and its vicinity. Depending on the large scale meteorological conditions, their influence can be both positive and negative, in terms of spatial coverage and temporal persistence.

Objective: To compare the effectiveness of the STRATIFY falls tool with nurses’ clinical judgments in predicting patient falls. Study Design and Setting: A prospective cohort study was conducted among the inpatients of an acute tertiary hospital. Participants were patients over 65 years of age admitted to any hospital unit. Sensitivity, specificity, and positive predictive value (PPV) and negative predictive values (NPV) of the instrument and nurses’ clinical judgments in predicting falls were calculated. Results: Seven hundred and eighty-eight patients were screened and followed up during the study period. The fall prevalence was 9.2%. Of the 335 patients classified as being ‘‘at risk’’ for falling using the STRATIFY tool, 59 (17.6%) did sustain a fall (sensitivity50.82, specificity50.61, PPV50.18, NPV50.97). Nurses judged that 501 patients were at risk of falling and, of these, 60 (12.0%) fell (sensitivity50.84, specificity50.38, PPV50.12, NPV50.96). The STRATIFY tool correctly identified significantly more patients as either fallers or nonfallers than the nurses (P50.027). Conclusion: Considering the poor specificity and high rates of false-positive results for both the STRATIFY tool and nurses’ clinical judgments, we conclude that neither of these approaches are useful for screening of falls in acute hospital settings.

From an initial sample of 747 primary school students, the top 16 percent (n =116) with high self-esteem (HSE) and the bottom 15 percent (n = I1 I) with low selfesteem (LSE) were se/eeted. These two groups were then compared on personal and classroom variables. Significant differences were found for all personal (self-talk, selfconcepts) and classroom (teacher feedback, praise, teacher-student relationship, and classroom environment) variables. Students with HSE scored more highly on all variables. Discriminant Function Analysis (DFA) was then used to determine which variables discriminated between these two groups of students. Learner self-concept, positive and negative self-talk, classroom environment, and effort feedback were the best discriminators of students with high and low self-esteem. Implications for educational psychologists and teachers are discussed.

The implementation of ‘good governance’ in Indonesia’s regional government sector became a central tenet in governance research following the introduction of the national code for governance in 2006. The code was originally drafted in 1999 as a response to the Asian financial crises and many cases of unearthed corruption, collusion, and nepotism. It was reviewed in 2001 and again in 2006 to incorporate relevant political, economical, and social developments. Even though the national code exists along with many regional government decrees on good governance, the extent of implementation of the tenets of good governance in Indonesia’s regional government is still questioned. Previous research on good governance implementation in Indonesian regional government (Mardiasmo, Barnes and Sakurai, 2008) identified differences in the nature and depth of implementation between various Indonesian regional governments. This paper analyses and extends this recent work and explores key factors that may impede the implementation and sustained application of governance practices across regional settings. The bureaucratic culture of Indonesian regional government is one that has been shaped for over approximately 30 years, in particular during that of the Soeharto regime. Previous research on this regime suggests a bureaucratic culture with a mix of positive and negative aspects. On one hand Soeharto’s regime resulted in strong development growth and strong economic fundamentals, resulting in Indonesia being recognised as one of the Asian economic tigers prior to the 1997 Asian financial crises. The financial crises however revealed a bureaucratic culture that was rife with corruption, collusion, and nepotism. Although subsequent Indonesian governments have been committed to eradicating entrenched practices it seems apparent that the culture is ingrained within the bureaucracy and eradication of it will take time. Informants from regional government agree with this observation, as they identify good governance as an innovative mechanism and to implement it will mean a deviation from the “old ways.” Thus there is a need for a “changed” mind set in order to implement sustained governance practices. Such an exercise has proven to be challenging so far, as there is “hidden” resistance from within the bureaucracy to change its ways. The inertia of such bureaucratic cultures forms a tension against the opportunity for the implementation of good governance. From this context an emergent finding is the existence of a ‘bureaucratic generation gap’ as an impeding variable to enhanced and more efficient implementation of governance systems. It was found that after the Asian financial crises the Indonesian government (both at national and regional level) drew upon a wider human resources pool to fill government positions – including entrants from academia, the private sector, international institutions, foreign nationals and new graduates. It suggested that this change in human capital within government is at the core of this ‘inter-generational divide.’ This divergence is exemplified, at one extreme, by [older] bureaucrats who have been in-position for long periods of time serving during the extended Soeharto regime. The “new” bureaucrats have only sat in their positions since the end of Asian financial crisis and did not serve during Soeharto’s regime. It is argued that the existence of this generation gap and associated aspects of organisational culture have significantly impeded modernising governance practices across regional Indonesia. This paper examines the experiences of government employees in five Indonesian regions: Solok, Padang, Gorontalo, Bali, and Jakarta. Each regional government is examined using a mixed methodology comprising of on-site observation, document analysis, and iterative semi-structured interviewing. Drawing from the experiences of five regional governments in implementing good governance this paper seeks to better understand the causal contexts of variable implementation governance practices and to suggest enhancements to the development of policies for sustainable inter-generational change in governance practice across regional government settings.

Data were collected from 269 Australian primary school children in grades 3 to 7. Self-report questionnaires measuring students' perceptions of the frequency of positive and negative statements directed to them by their teacher, their positive and negative self-talk; and their reading, mathematics and learning self-concepts were administered. Positive statements made by teachers were found to be directly related to positive self-talk and to maths and learning self-concepts. Teachers' positive statements were also indirectly related to reading self-concept through positive self-talk. Negative statements made by teachers were not predictive of self-talk or self-concepts for the total sample but were predictive of maths self-concept for girls and negative self-talk for boys. Implications for teachers and educational psychologists are discussed.

Two studies were conducted to investigate empirical support for two models relating to the development of self-concepts and self-esteem in upper-primary school children. The first study investigated the social learning model by examining the relationship between mothers' and fathers' self-reported self-concepts and self-esteem and the self-reported self-concepts and self-esteem of their children. The second study investigated the symbolic interaction model by examining the relationship between children's perception of the frequency of positive and negative statements made by parents and their self-reported self-concepts and self-esteem. The results of these studies suggested that what parents say to their children and how they interact with them is more closely related to their children's self-perceptions than the role of modelling parental attitudes and behaviours. The findings highlight the benefits of parents talking positively to their children.

Objectives. Intrusive memories of extreme trauma can disrupt a stepwise approach to imaginal exposure. Concurrent tasks that load the visuospatial sketchpad (VSSP) of working memory reduce the vividness of recalled images. This study tested whether relief of distress from competing VSSP tasks during imaginal exposure is at the cost of impaired desensitization . Design. This study examined repeated exposure to emotive memories using 18 unselected undergraduates and a within-subjects design with three exposure conditions (Eye Movement, Visual Noise, Exposure Alone) in random, counterbalanced order. Method. At baseline, participants recalled positive and negative experiences, and rated the vividness and emotiveness of each image. A different positive and negative recollection was then used for each condition. Vividness and emotiveness were rated after each of eight exposure trials. At a post-exposure session 1 week later, participants rated each image without any concurrent task. Results. Consistent with previous research, vividness and distress during imaging were lower during Eye Movements than in Exposure Alone, with passive visual interference giving intermediate results. A reduction in emotional responses from Baseline to Post was of similar size for the three conditions. Conclusion. Visuospatial tasks may offer a temporary response aid for imaginal exposure without affecting desensitization.
