Coral reefs are increasingly threatened by global and local anthropogenic stressors, such as rising seawater temperature and nutrient enrichment. These two stressors vary widely across the reef face and parsing out their influence on coral communities at reef system scales has been particularly challenging. Here, we investigate the influence of temperature and nutrients on coral community traits and life history strategies on lagoonal reefs across the Belize Mesoamerican Barrier Reef System (MBRS). A novel metric was developed using ultra-high-resolution sea surface temperatures (SST) to classify reefs as enduring low (lowTP), moderate (modTP), or extreme (extTP) temperature parameters over 10 years (2003 to 2012). Chlorophyll-a (chl a) records obtained for the same interval were employed as a proxy for bulk nutrients and these records were complemented with in situ measurements to "sea truth" nutrient content across the three reef types. Chl a concentrations were highest at extTP sites, medial at modTP sites and lowest at lowTP sites. Coral species richness, abundance, diversity, density, and percent cover were lower at extTP sites compared to lowTP and modTP sites, but these reef community traits did not differ between lowTP and modTP sites. Coral life history strategy analyses showed that extTP sites were dominated by hardy stress-tolerant and fast-growing weedy coral species, while lowTP and modTP sites consisted of competitive, generalist, weedy, and stress-tolerant coral species. These results suggest that differences in coral community traits and life history strategies between extTP and lowTP/modTP sites were driven primarily by temperature differences with differences in nutrients across site types playing a lesser role. Dominance of weedy and stress-tolerant genera at extTP sites suggests that corals utilizing these two life history strategies may be better suited to cope with warmer oceans and thus may warrant further protective status during this climate change interval.

Radiotherapy is commonly used to treat lung cancer. However, radiation induced damage to lung tissue is a major limiting factor to its use. To minimize normal tissue lung toxicity from conformal radiotherapy treatment planning, we investigated the use of Perfluoropropane(PFP)-enhanced MR imaging to assess and guide the sparing of functioning lung. Fluorine Enhanced MRI using Perfluoropropane(PFP) is a dynamic multi-breath steady state technique enabling quantitative and qualitative assessments of lung function(1).
Imaging data was obtained from studies previously acquired in the Duke Image Analysis Laboratory. All studies were approved by the Duke IRB. The data was de-identified for this project, which was also approved by the Duke IRB. Subjects performed several breath-holds at total lung capacity(TLC) interspersed with multiple tidal breaths(TB) of Perfluoropropane(PFP)/oxygen mixture. Additive wash-in intensity images were created through the summation of the wash-in phase breath-holds. Additionally, model based fitting was utilized to create parametric images of lung function(1).
Varian Eclipse treatment planning software was used for putative treatment planning. For each subject two plans were made, a standard plan, with no regional functional lung information considered other than current standard models. Another was created using functional information to spare functional lung while maintaining dose to the target lesion. Plans were optimized to a prescription dose of 60 Gy to the target over the course of 30 fractions.
A decrease in dose to functioning lung was observed when utilizing this functional information compared to the standard plan for all five subjects. PFP-enhanced MR imaging is a feasible method to assess ventilatory lung function and we have shown how this can be incorporated into treatment planning to potentially decrease the dose to normal tissue.

Critical bed shear stress for incipient motion has been determined for biogenic free-living coralline algae known as maërl. Maërl from three different sedimentary environments (beach, intertidal, and open marine) in Galway Bay, west of Ireland have been analysed in a rotating annular flume and linear flume. Velocity profile measurements of the benthic boundary layer, using an Acoustic Doppler Velocimeter, have been obtained in four different velocity experiments. The bed shear stress has been determined using three methods: Law of the Wall, Turbulent Kinetic Energy and Reynolds Stress. The critical Shields parameter has been estimated as a non-dimensional mobility number and the results have been compared with the Shields curve for natural sand. Maërl particles fall below this curve because its greater angularity allows grains to be mobilised easier than hydraulically equivalent particles. From previous work, the relationship between grain shape and the settling velocity of maërl suggests that the roughness is greatest for intertidal maërl particles. During critical shear stress determinations, beds of such rough particles exhibited the greatest critical shear stress probably because the particle thalli interlocked and resisted entrainment. The Turbulent Kinetic Energy methodology gives the most consistent results, agreeing with previous comparative studies. Rarely-documented maërl megaripples were observed in the rotating annular flume and are hypothesised to form at velocities ~10 cm s-1 higher than the critical threshold velocity, where tidal currents, oscillatory flow or combined-wave current interaction results in the preferential transport of maërl. A determination of the critical bed shear stress of maërl allows its mobility and rate of erosion and deposition to be evaluated spatially in subsequent applications to biological conservation management.

The need for continuous recording rain gauges makes it difficult to determine the rainfall erosivity factor (R-factor) of the (R)USLE model in areas without good temporal data coverage. In mainland Spain, the Nature Conservation Institute (ICONA) determined the R-factor at few selected pluviographs, so simple estimates of the R-factor are definitely of great interest. The objectives of this study were: (1) to identify a readily available estimate of the R-factor for mainland Spain; (2) to discuss the applicability of a single (global) estimate based on analysis of regional results; (3) to evaluate the effect of record length on estimate precision and accuracy; and (4) to validate an available regression model developed by ICONA. Four estimators based on monthly precipitation were computed at 74 rainfall stations throughout mainland Spain. The regression analysis conducted at a global level clearly showed that modified Fournier index (MFI) ranked first among all assessed indexes. Applicability of this preliminary global model across mainland Spain was evaluated by analyzing regression results obtained at a regional level. It was found that three contiguous regions of eastern Spain (Catalonia, Valencian Community and Murcia) could have a different rainfall erosivity pattern, so a new regression analysis was conducted by dividing mainland Spain into two areas: Eastern Spain and plateau-lowland area. A comparative analysis concluded that the bi-areal regression model based on MFI for a 10-year record length provided a simple, precise and accurate estimate of the R-factor in mainland Spain. Finally, validation of the regression model proposed by ICONA showed that R-ICONA index overpredicted the R-factor by approximately 19%.

Understanding the impact of extracellular matrix sub-types and mechanical stretch on cardiac fibroblast activity is required to help unravel the pathophysiology of myocardial fibrotic diseases. Therefore, the purpose of this study was to investigate pro-fibrotic responses of primary human cardiac fibroblast cells exposed to different extracellular matrix components, including collagen sub-types I, III, IV, VI and laminin. The impact of mechanical cyclical stretch and treatment with transforming growth factor beta 1 (TGFβ1) on collagen 1, collagen 3 and alpha smooth muscle actin mRNA expression on different matrices was assessed using quantitative real-time PCR. Our results revealed that all of the matrices studied not only affected the expression of pro-fibrotic genes in primary human cardiac fibroblast cells at rest but also affected their response to TGFβ1. In addition, differential cellular responses to mechanical cyclical stretch were observed depending on the type of matrix the cells were adhered to. These findings may give insight into the impact of selective pathological deposition of extracellular matrix proteins within different disease states and how these could impact the fibrotic environment.

Sjuksköterskor som arbetar på akutmottagningar utsätts regelbundet för stress. Det medför psykiska och fysiska besvär för sjuksköterskor och försämrad omvårdnad för patienten. Studien avser att utifrån vetenskaplig litteratur sammanställa de faktorer som leder till stress för sjuksköterskor som arbetar på akutmottagningar och hur vården påverkas av stressade sjuksköteskor. Syfte Syftet med litteraturstudien var att beskriva vilka faktorer på akutmottagningen som upplevdes bidra till stress för sjuksköterskor som arbetar på en akutmottagning och hur stressen påverkar sjuksköterskors möjligheter att erbjuda en god vård för patienter på akutmottagningen. Metod Studien har genomförts som en litteraturstudie. I studien användes 15 artiklar som bestod av både kvalitativ och kvantitativ ansats. Materialet hämtades i databaserna CINAHL och PubMed. Resultat Resultatet visar att det finns flera olika faktorer som bidrar till stress för sjuksköterskor på akutmottagningen. Stressande faktorer visade sig utifrån studierna i resultatet vara; hög arbetsbelastning och låg bemanning, avsaknad av tid för reflektion för sjuksköterskan, att vårda barn i stressade situationer, hot och våld på akutmottagningen, kommunikation samt smärta och lidande. Hur vården påverkas av sjuksköterskors stress på akutmottagningen var utifrån studierna att patientsäkerheten blev försämrad och att vårdrelationen påverkades. Slutsats Författarna drar slutsatsen att stress påverkar sjuksköteskors arbetsuppgifter på akutmottagningen. Patienters möjlighet till patientsäker och god vård på akutmottagningen påverkas negativt av stressade sjuksköterskor. För att komma till rätta med sjuksköterskebristen på akutmottagningar så är det av betydelse för sjukhusledningen att arbeta preventivt mot stressade sjuksköterskor.

Objectivos: O presente estudo avaliou a relação entre diversos aspectos objectivos e subjectivos inerentes às experiências de combate/guerra e sintomatologia associada à Perturbação Pós-Stress Traumático. Os instrumentos de auto-resposta utilizados na colheita dos dados foram analisados quanto às suas principais propriedades psicométricas. Métodos: A amostra foi composta por 105 combatentes da Guerra Colonial Portuguesa. Foram utilizados os seguintes instrumentos de avaliação: um questionário sóciodemográfico de auto-resposta para a caracterização da amostra, o Posttraumatic Stress Disorder Checklist (para militares), o Peritraumatic Dissociative Experiences Questionnaire, o Questionário de Experiências de Combate e duas escalas do Deployment Risk and Resilience Inventory, nomeadamente a Difficult Livings and Working Environment Scale e a Perceived Threat Scale. Resultados: O estudo das qualidades psicométricas dos instrumentos apresenta uma consistência interna superior a .86 e uma estabilidade temporal superior a .79. A análise factorial do Peritraumatic Dissociative Experiences Questionnaire apresenta bons valores na medida de Kaiser-Meyer-Olkin e no teste de esfericidade de Bartlett. Na rotação de Varimax é extraído um factor, predizendo a sua unidimensionalidade, que explica cerca de 70% da variância total do questionário. Os resultados relativos à validade divergentediscriminante indicam, por si só, que esta se apresenta de forma clara entre os instrumentos estudados (PCL-M, PDEQ, DLWES e PTS) no subgrupo com PTSD, com excepção da verificada entre o PCL-M e o PDEQ.. Quanto à validade de critério, constatam-se diferenças entre os subgrupos com e sem PTSD. Através do método da regressão linear múltipla, duas das seis variáveis têm, isoladamente, um peso preditivo significativo para a PTSD, nomeadamente a frequência de exposição a situações típicas e objectivas de combate/ guerra e os desconfortos diários durante a(s) comissão(ões). Ao considerarmos as variáveis no seu conjunto, aquelas que mais contribuem são a dissociação peritraumática e os desconfortos diários durante a(s) comissão(ões). Conclusão: Constata-se que os factores peritraumáticos em estudo desempenham um importante papel no desenvolvimento da sintomatologia inerente à PTSD. /

A pobreza infantil é reconhecida, na comunidade científico, como um problema social grave que viola os direitos das crianças, sendo este um fator favorável à vulnerabilização e à possibilidade de ocorrência de casos de negligência. Em consequência, esta fundamenta-se pela necessidade de intervenções, que a par de medidas de proteção social do Estado, em particular o Rendimento Social de Inserção (RSI), potenciem o desempenho de funções parentais protetoras. Por conseguinte, definiu-se como principal objetivo analisar a relação e/ou influência da medida do RSI sobre as funções parentais (des) protetoras no contexto da pobreza infantil em 15 crianças (dos 0 aos 6 anos), assim como compreender os domínios do seu bem-estar a partir dos Reports card`s da UNICEF. A investigação decorreu no concelho de Albergaria-a-Velha entre os meses de fevereiro a julho de 2013. A metodologia utilizada foi o método qualitativo de natureza exploratória cujos dados foram recolhidos através de dois inquéritos dirigidos às 11 famílias abrangidas pelo estudo e aos respetivos técnicos. A análise dos dados permitiu retirar as principais conclusões destacando-se que o Bem-estar Material e Habitacional das crianças encontram-se comprometidos, uma vez que, os rendimentos médios das famílias estudadas são inferiores ao salário mínimo nacional, o RSI a principal fonte de rendimentos, a maioria dos agregados beneficiam, para além deste, de apoios económicos, quatro famílias estão identificadas como pobreza intergeracional projeta-se para a maioria, a continuidade da “dependência” dos serviços de ação social. Perante estes dados as crianças podem vivenciar privações ou carências múltiplas. Constatou-se falta de privacidade e de conforto habitacional apesar das melhorias significativas nesta dimensão. Na dimensão do Bem-estar Saúde e Segurança enquadra-se a negligência, deste modo, as praticas parentais de risco/negligência foram distribuídas pelos níveis “baixo, moderado e elevado”, apurou-se que em 6 agregados apenas um pratica uma parentalidade não protetora; para os restantes consideramos que o RSI contribui para atenuar a intensidade do stress parental face às privações económicas. / Child poverty is recognized in the scientific community as a serious social problem, which goes against the rights of children. It is a favorable factor to their increasing vulnerability and to the possibility of occurrence of negligence. It is based on the need of interventions that measures social protection of the Goverment through the Social Insertion Income (SII), potentiate the protective performance of parental functions. Thus, the main objective of this study was to analyze the relationship and / or influence of SII on parental protective functions in the context of child poverty in 15 children (aged 0 to 6 years), as well as understand the areas of their welfare according to the UNICEF Report Card’s. The research took place in the Municipality of Albergaria-a-Velha between the months February to July 2013. The used methodology was the qualitative method with exploratory contours and the data collected through two questionnaires surveys directed to the 11 families and Technicians. The information analysis led us to the main conclusions that the children’s Welfare Material and Housing are compromised, since the average yield of the families studied are lower than the minimum national wage, the RSI is the main source of income that most households benefit, in addition to this economic support the continuous "dependency" of social services is projected for the future and four families are identified as intergenerational poverty. Based on these facts the children can experience multiple disadvantage or deprivation. According to the housing conditions it was found lack of privacy and comfort, although exists significant improvements in this dimension. In the dimension of Wellbeing Health and Safety fits negligence, in this way the parental practice of risk / negligence were distributed by the levels of "low, moderate and high", it was found that only one of eight cases do not practice a protective parenting, for the remaining households we consider that the SII contributes to mitigate the intensity of the parental stress against the economic deprivation.

Le compartiment microvasculaire est une cible importante du stress oxydant qui est un facteur majeur de la dysfonction endothéliale, notamment au cours d’exposition aux rayonnements ionisants. L’altération de l’endothélium induite par le stress oxydant est impliquée dans la toxicité radio-induite des tissus sains. Limiter les dysfonctions endothéliales est donc un enjeu important des traitements radiothérapeutiques actuels. Cet objectif nécessite une meilleure caractérisation de la signalisation du stress oxydant dans les cellules endothéliales. La voie p38 MAPK est incontournable dans la réponse au stress oxydant mais reste encore insuffisamment caractérisée. Par une approche protéomique, nous avons identifié la nucléophosmine (NPM) comme nouveau partenaire de p38 dans le cytoplasme des cellules endothéliales. La phosphatase PP2a est aussi associée à ce complexe NPM/p38. Nos travaux montrent que le stress oxydant (H2O2, 500μM) régule la déphosphorylation de NPM via PP2a, entraine sa dissociation rapide du complexe et favorise sa translocation vers le noyau. De plus, nous montrons que la présence de NPM déphosphorylée au noyau altère la réponse des cellules aux dommages à l’ADN induits par le stress oxydant. Le céramide sphingolipide membranaire est également un facteur important des voies de stress, particulièrement dans les cellules endothéliales. Notre étude aborde donc l’implication de ce sphingolipide dans la régulation de la voie NPM/p38. Une meilleure caractérisation de la voie p38 et de ses acteurs permettra d’identifier de potentielles cibles afin de limiter les dysfonctions endothéliales et leurs conséquences délétères sur les tissus environnants.

Introdução: O stress condiciona o comportamento e desenvolvimento pessoal, podendo ter consequências preocupantes ao nível individual e organizacional. Os professores são uma classe reconhecida como estando sujeita a elevados níveis de stress, fator que prejudica a qualidade do ensino em geral. Objetivos: Avaliar a vulnerabilidade ao stress; identificar os principais fatores de stress e avaliar a correlação entre autoestima e stress, em professores do 1.º, 2.º e 3.º ciclos do ensino básico. Metodologia: Estudo correlacional, com uma amostra constituída por 60 professores do 1.º, 2.º e 3.º ciclos de escolaridade, de escolas do concelho de Leiria. Para a análise das variáveis em estudo foram aplicados: um questionário sociodemográfico; a Rosenberg Self-Esteem Scale (RSES); o Questionário de stress nos professores: ensino básico e secundário (QSPEBS) e a Escala de vulnerabilidade ao stress (23 QVS). Resultados: O estudo revela que 65% dos professores percecionam a profissão como stressante e que 30% são vulneráveis ao stress. O principal fator de stress identificado foi comportamentos inadequados/indisciplina dos alunos. Verificou-se uma correlação negativa entre a autoestima e a vulnerabilidade ao stress dos professores. Conclusão: Conclui-se que, quanto mais elevada é a auto-estima, menor é a vulnerabilidade dos professores ao stress e que, apesar de não se registarem níveis significativos de vulnerabilidade ao stress na população docente, a maioria dos professores perceciona a sua atividade profissional como geradora de elevados níveis de stress, sendo os comportamentos inadequados/indisciplina dos alunos o fator que mais contribui para o stress nos docentes. / Introduction: Stress affects behavior and personal development, and may have severe consequences either at the individual or organizational level. Teachers are identified as a class exposed to high levels of stress, factor that affects the quality of education in general. Objectives: To assess stress vulnerability; identify key stressors and assess the correlation between self-esteem and stress in primary and secondary school teachers. Methodology: A sociodemographic questionnaire and three self-report measures were administered to a sample of 60 primary and secondary teachers from Leiria district: the Rosenberg Self-Esteem Scale (RSES); a teachers stress questionnaire for primary, secondary and high school (QSPEBS) and a scale to access stress vulnerability (23 QVS). Results: The study reveals that 65% of teachers perceive the profession as stressful and that 30% are vulnerable to stress. The main stressor identified is inappropriate/disruptive behavior of students. There is a negative correlation between self-esteem and vulnerability to stress of teachers. Conclusion: We conclude that a higher self-esteem leads to less stress vulnerability in teachers and that, despite the non-significant level of vulnerability determined in this study, most teachers perceive their activity as stressful, being inappropriate/disruptive behavior of students the factor that most contributes for that.

Stainless steels were developed in the early 20th century and are used where both the mechanical properties of steels and corrosion resistance are required. There is continuous research to allow stainless steel components to be produced in a more economical way and be used in more harsh environments. A necessary component in this effort is to correlate the service performance with the production processes. The central theme of this thesis is the mechanical grinding process. This is commonly used for producing stainless steel components, and results in varied surface properties that will strongly affect their service life. The influence of grinding parameters including abrasive grit size, machine power and grinding lubricant were studied for 304L austenitic stainless steel (Paper II) and 2304 duplex stainless steel (Paper I). Surface integrity was proved to vary significantly with different grinding parameters. Abrasive grit size was found to have the largest influence. Surface defects (deep grooves, smearing, adhesive/cold welding chips and indentations), a highly deformed surface layer up to a few microns in thickness and the generation of high level tensile residual stresses in the surface layer along the grinding direction were observed as the main types of damage when grinding stainless steels. A large degree of residual stress anisotropy is interpreted as being due to mechanical effects dominating over thermal effects. The effect of grinding on stress corrosion cracking behaviour of 304L austenitic stainless steel in a chloride environment was also investigated (Paper III). Depending on the surface conditions, the actual loading by four-point bend was found to deviate from the calculated value using the formula according to ASTM G39 by different amounts. Grinding-induced surface tensile residual stress was suggested as the main factor to cause micro-cracks initiation on the ground surfaces. Grinding along the loading direction was proved to increase the susceptibility to chloride-induced SCC, while grinding perpendicular to the loading direction improved SCC resistance. The knowledge obtained from this work can provide a reference for choosing appropriate grinding parameters when fabricating stainless steel components; and can also be used to help understanding the failure mechanism of ground stainless steel components during service.

It has been established that Wingate-based high-intensity training (HIT) consisting of 4 to 6 x 30-s all-out sprints interspersed with 4-min recovery is an effective training paradigm. Despite the increased utilisation of Wingate-based HIT to bring about training adaptations, the majority of previous studies have been conducted over a relatively short timeframe (2 to 6 weeks). However, activity during recovery period, intervention duration or sprint length have been overlooked. In study 1, the dose response of recovery intensity on performance during typical Wingate-based HIT (4 x 30-s cycle all-out sprints separated by 4-min recovery) was examined and active recovery (cycling at 20 to 40% of V̇O2peak) has been shown to improve sprint performance with successive sprints by 6 to 12% compared to passive recovery (remained still), while increasing aerobic contribution to sprint performance by ~15%. In the following study, 5 to 7% greater endurance performance adaptations were achieved with active recovery (40%V̇O2peak) following 2 weeks of Wingate-based HIT. In the final study, shorter sprint protocol (4 to 6 x 15-s sprints interspersed with 2 min of recovery) has been shown to be as effective as typical 30-s Wingate-based HIT in improving cardiorespiratory function and endurance performance over 9 weeks with the improvements in V̇O2peak being completed within 3 weeks, whereas exercise capacity (time to exhaustion) being increased throughout 9 weeks. In conclusion, the studies demonstrate that active recovery at 40% V̇O2peak significantly enhances endurance adaptations to HIT. Further, the duration of the sprint does not seem to be a driving factor in the magnitude of change with 15 sec sprints providing similar adaptations to 30 sec sprints. Taken together, this suggests that the arrangement of recovery mode should be considered to ensure maximal adaptation to HIT, and the practicality of the training would be enhanced via the reduction in sprint duration without diminishing overall training adaptations.

Sub lethal (0.2 ppm) mercuric chloride induced stress related histopathological alterations in the epithelial linings of foot (podium) of the edible freshwater mussel Lamellidens marginalis (Lamarck) were studied using histochemical techniques up to 60 days of exposure. The histomorphological changes were manifested only slowly and its intensity was somewhat proportional to the duration of exposure. The immediate response of the exposed mussels was the altered mucous secretion. There was a progressive incorporation of sulphated glycoproteins into the secretory contents of the mucous cells especially in the first half of the experiment. Marked histopathological changes including necrosis, appearance of pyknotic nuclei, sloughing of epithelial cells and appearance of non-tissue spaces, etc., started appearing during the later half of the experiment. The fag end of the experiment, which witnessed prominent histomorphological changes, was accompanied by highly decreased mucous secretion. Chronic kidney disease (CKD) is associated with increased cardiovascular risk in comparison with the general population. This can be observed even in the early stages of CKD, and rises in proportion to the degree of renal impairment. Not only is cardiovascular disease (CVD) more prevalent in CKD, but its nature differs too, with an excess of morbidity and mortality associated with congestive cardiac failure, arrhythmia and sudden death, as well as the accelerated atherosclerosis which is also observed. Conventional cardiovascular risk factors such as hypertension, dyslipidaemia, obesity, glycaemia and smoking, are highly prevalent amongst patients with CKD, although in many of these examples the interaction between risk factor and disease differs from that which exists in normal renal function. Nevertheless, the extent of CVD cannot be fully explained by these conventional risk factors, and non-conventional factors specific to CKD are now recognised to contribute to the burden of CVD. Oxidative stress is a state characterised by excessive production of reactive oxygen species (ROS) and other radical species, a reduction in the capacity of antioxidant systems, and disturbance in normal redox homeostasis with depletion of protective vascular signalling molecules such as nitric oxide (NO). This results in oxidative damage to macromolecules such as lipids, proteins and DNA which can alter their functionality. Moreover, many enzymes are sensitive to redox regulation such that oxidative modification to cysteine thiol groups results in activation of signalling cascades which result in adverse cardiovascular effects such as vascular and endothelial dysfunction. Endothelial dysfunction and oxidative stress are present in association with many conventional cardiovascular risk factors, and can be observed even prior to the development of overt, clinical, vascular pathology, suggesting that these phenomena represent the earliest stages of CVD. In the presence of CKD, there is increased ROS production due to upregulated NADPH oxidase (NOX), increase in a circulating asymmetric dimethylarginine (ADMA), uncoupling of endothelial nitric oxide synthase (eNOS) as well as other mechanisms. There is also depletion in exogenous antioxidants such as ascorbic acid and tocopherol, and a reduction in activity of endogenous antioxidant systems regulated by the master gene regulator Nrf-2. In previous studies, circulating markers of oxidative stress have been shown to be increased in CKD, together with a reduction in endothelial function in a stepwise fashion relating to the severity of renal impairment. Not only is CVD linked to oxidative stress, but the progression of CKD itself is also in part dependent on redox sensitive mechanisms. For example, administration of the ROS scavenger tempol attenuates renal injury and reduces renal fibrosis seen on biopsy in a mouse model of CKD, whilst conversely, supplementation with the NOS inhibitor L-NAME causes proteinuria and renal impairment. Previous human studies examining the effect of antioxidant administration on vascular and renal function have been conflicting however. The work contained in this thesis therefore examines the effect of antioxidant administration on vascular and endothelial function in CKD. Firstly, 30 patients with CKD stages 3 – 5, and 20 matched hypertensive controls were recruited. Participants with CKD had lower ascorbic acid, higher TAP and ADMA, together with higher augmentation index and pulse wave velocity. There was no difference in baseline flow mediated dilatation (FMD) between groups. Intravenous ascorbic acid increased TAP and O2-, and reduced central BP and augmentation index in both groups, and lowered ADMA in the CKD group only. No effect on FMD was observed. The effects of ascorbic acid on kidney function was then investigated, however this was hindered by the inherent drawbacks of existing methods of non-invasively measuring kidney function. Arterial spin labelling MRI is an emerging imaging technique which allows measurement of renal perfusion without administration of an exogenous contrast agent. The technique relies upon application of an inversion pulse to blood within the vasculature proximal to the kidneys, which magnetically labels protons allowing measurement upon transit to the kidney. At the outset of this project local experience using ASL MRI was limited and there ensued a prolonged pre-clinical phase of testing with the aim of optimising imaging strategy. A study was then designed to investigate the repeatability of ASL MRI in a group of 12 healthy volunteers with normal renal function. The measured T1 longitudinal relaxation times and ASL MRI perfusion values were in keeping with those found in the literature; T1 time was 1376 ms in the cortex and 1491 ms in the whole kidney ROI, whilst perfusion was 321 mL/min/100g in the cortex, and 228 mL/min/100g in the whole kidney ROI. There was good reproducibility demonstrated on Bland Altman analysis, with a CVws was 9.2% for cortical perfusion and 7.1% for whole kidney perfusion. Subsequently, in a study of 17 patients with CKD and 24 healthy volunteers, the effects of ascorbic acid on renal perfusion was investigated. Although no change in renal perfusion was found following ascorbic acid, it was found that ASL MRI demonstrated significant differences between those with normal renal function and participants with CKD stages 3 – 5, with increased cortical and whole kidney T1, and reduced cortical and whole kidney perfusion. Interestingly, absolute perfusion showed a weak but significant correlation with progression of kidney disease over the preceding year. Ascorbic acid was therefore shown to have a significant effect on vascular biology both in CKD and in those with normal renal function, and to reduce ADMA only in patients with CKD. ASL MRI has shown promise as a non-invasive investigation of renal function and as a biomarker to identify individuals at high risk of progressive renal impairment.
