The human language-learning ability persists throughout life, indicating considerable flexibility at the cognitive and neural level. This ability spans from expanding the vocabulary in the mother tongue to acquisition of a new language with its lexicon and grammar. The present thesis consists of five studies that tap both of these aspects of adult language learning by using magnetoencephalography (MEG) and functional magnetic resonance imaging (fMRI) during language processing and language learning tasks. The thesis shows that learning novel phonological word forms, either in the native tongue or when exposed to a foreign phonology, activates the brain in similar ways. The results also show that novel native words readily become integrated in the mental lexicon. Several studies in the thesis highlight the left temporal cortex as an important brain region in learning and accessing phonological forms. Incidental learning of foreign phonological word forms was reflected in functionally distinct temporal lobe areas that, respectively, reflected short-term memory processes and more stable learning that persisted to the next day. In a study where explicitly trained items were tracked for ten months, it was found that enhanced naming-related temporal and frontal activation one week after learning was predictive of good long-term memory. The results suggest that memory maintenance is an active process that depends on mechanisms of reconsolidation, and that these process vary considerably between individuals. The thesis put special emphasis on studying language learning in the context of language production. The neural foundation of language production has been studied considerably less than that of perceptive language, especially on the sentence level. A well-known paradigm in language production studies is picture naming, also used as a clinical tool in neuropsychology. This thesis shows that accessing the meaning and phonological form of a depicted object are subserved by different neural implementations. Moreover, a comparison between action and object naming from identical images indicated that the grammatical class of the retrieved word (verb, noun) is less important than the visual content of the image. In the present thesis, the picture naming was further modified into a novel paradigm in order to probe sentence-level speech production in a newly learned miniature language. Neural activity related to grammatical processing did not differ between the novel language and the mother tongue, but stronger neural activation for the novel language was observed during the planning of the upcoming output, likely related to more demanding lexical retrieval and short-term memory. In sum, the thesis aimed at examining language learning by combining different linguistic domains, such as phonology, semantics, and grammar, in a dynamic description of language processing in the human brain.

Alzheimer`s disease (AD) is characterised neuropathologically by the presence of extracellular amyloid plaques, intraneuronal neurofibrillary tangles, and cerebral neuronal loss. The pathological changes in AD are believed to start even decades before clinical symptoms are detectable. AD gradually affects episodic memory, cognition, behaviour and the ability to perform everyday activities. Mild cognitive impairment (MCI) represents a transitional state between normal aging and dementia disorders, especially AD. The predictive accuracy of the current and commonly used MCI criteria devide this disorder into amnestic (aMCI) and non-amnestic (naMCI) MCI. It seems that many individuals with aMCI tend to convert to AD. However many MCI individuals will remain stable and some may even recover. At present, the principal drugs for the treatment of AD provide only symptomatic and palliative benefits. Safe and effective mechanism-based therapies are needed for this devastating neurodegenerative disease of later life. In conjunction with the development of new therapeutic drugs, tools for early detection of AD would be important. In future one of the challenges will be to detect at an early stage these MCI individuals who will convert to AD. Methods which can predict which MCI subjects will convert to AD will be much more important if the new drug candidates prove to have disease-arresting or even disease–slowing effects. These types of drugs are likely to have the best efficacy if administered in the early or even in the presymptomatic phase of the disease when the synaptic and neuronal loss has not become too widespread. There is no clinical method to determine with certainly which MCI individuals will progress to AD. However there are several methods which have been suggested as predictors of conversion to AD, e.g. increased [11C] PIB uptake, hippocampal atrophy in MRI, low CSF A beta 42 level, high CSF tau-protein level, apolipoprotein E (APOE) ε4 allele and impairment in episodic memory and executive functions. In the present study subjects with MCI appear to have significantly higher [11C] PIB uptake vs healthy elderly in several brain areas including frontal cortex, the posterior cingulate, the parietal and lateral temporal cortices, putamen and caudate. Also results from this PET study indicate that over time, MCI subjects who display increased [11C] PIB uptake appear to be significantly more likely to convert to AD than MCI subjects with negative [11C] PIB retention. Also hippocampal atrophy seems to increase in MCI individuals clearly during the conversion to AD. In this study [11C] PIB uptake increases early and changes relatively little during the AD process whereas there is progressive hippocampal atrophy during the disease. In addition to increased [11C] PIB retention and hippocampal atrophy, the status of APOE ε4 allele might contribute to the conversion from MCI to AD.

With the improvement of laparoscopic techniques, endoscopic liver surgery has become feasible. While laparoscopic wedge liver resection are performed more frequently, laparoscopic (anatomical or nonanatomical) liver resection are still at an early stage of development and are somewhat controversial. We reporte laparoscopic hepatic resection without use of sophisticated laparoscopic instruments. A 47-year-old woman underwent radical mastectomy for adenocarcinoma in 1995. 1n the postoperative follow-up presented, a lesion in the left hepatic lobe and, after laparoscopic approach, left lateral segmentectomy was performed. The hepatic resection elapsed without complications. The surgical time was 4 hours and the blood loss was minimal, without transfusion being necessary.The abdominal drain was removed in 24 hours and the patient was discharged in the second postoperative day. Compared to the classic approach by laparotomy, this method was less traumatic, required a shorter hospital stay, and followed by faster recovery.

Tuberculous involvement of the liver is usually a diffuse process, associated with miliary tuberculosis. However localized tuberculosis of the liver producing a macronodular tuberculoma or an abscess is rare. The authors present a case of pseudotumoral hepatic tuberculosis in a 34-year old woman. This patient presented a 2 month history of fever weight loss of 4Kg and right upper quadrant abdominal pain. She denied jaundice, choluria, or acholia. Laboratory investigation, including renal and liver function tests. revealed normal levels. Chest X-ray was normal. Abdominal ultrasonography demonstrated a hypoechoic nodule in the right hepatic lobe. CT scan showed hypodense areas in the same place and no retroperitoneal lymphadenopathy. Due to the inespecificity of the signs, symptoms and image findings, a diagnostic laparoscopy was performed, it was however inconclusive. Then, the patient was submitted to a laparotomy with ressection of the lesion. Histological examination revealed a tuberculoid granulomatous lesion with caseous necrosis. Postoperatively, the patient was placed on antituberculous chemotherapy with rifampin, isoniazid and pyrazinamide. Eight months later the patient is asymptomatic.

The authors present a case of a female. 43-year-old patient. with a neck mass suggestive of a thyroglossal duct cyst. During the operation. a hard and adherent mass was found and the pathological findings demonstrated a metastatic follicular carcinoma. Despite of the negative image, total thyroidectomy was performed and a small primary tumor of 0,2cm was found in the left inferior lobe. There were only three of these cases reported.

Os autores pesquisam o efeito do Dextran 40 sobre a formação de aderências pós-operatórias em um modelo experimental em ratos. Vinte ratos Wistar foram divididos de forma aleatória em dois grupos. Ambos os grupos foram submetidos a laparotomia mediana e realizada escarificação serosa do peritônio visceral do intestino grosso e peritônio parietal adjacente. O grupo I (controle), formado por dez animais que não receberam tratamento complementar, e o grupo 2, formado por dez animais nos quais administrou-se Dextran 40 na cavidade peritoneal. No vigésimo dia de pós-operatório, os animais foram mortos e submetidos a nova laparotomia mediana e retirada dos segmentos intestinais previamente escarificados. A análise histológica das peças operatórias demonstrou menor formação de fibrose no grupo de animais nos quais foi utilizado Dextran 40 (grupo 2), quando comparados ao grupo controle (p<0,05). Os autores concluem que o Dextran 40 interfere no processo de fibrinogênese reduzindo as aderências intra-abdominais pós-operatórias.

Hepatic lymphangioma developed from a malformation that blocks the lymphatic system of a given area of the liver is a rare benign tumor observed most often in children and adolescents. The authors report and discuss a case of cystic lymphangioma of the liver in a 65 years old female patient, from a region where hydatid disease is endemic, with a complaint of epigastric pain, nausea, and vomiting. The patient presented a non-pulsatile mass, with severe pain at palpation in the epigastric region, The diagnostic investigation revealed a large cystic lesion in the left lobe of the liver; and she was submitted to bisegmentectomy II and III, which showed a hepatic lymphangioma. Considering the progressive character of this lesion, it should be carefully evaluated, since the removal of the lymphangioma is a safe, curative procedure.

The role of genetic factors in the pathogenesis of Alzheimer’s disease (AD) is not completely understood. In order to improve this understanding, the cerebral glucose metabolism of seven monozygotic and nine dizygotic twin pairs discordant for AD was compared to that of 13 unrelated controls using positron emission tomography (PET). Traditional region of interest analysis revealed no differences between the non-demented dizygotic co-twins and controls. In contrast, in voxel-level and automated region of interest analyses, the non-demented monozygotic co-twins displayed a lower metabolic rate in temporal and parietal cortices as well as in subcortical grey matter structures when compared to controls. Again, no reductions were seen in the non-demented dizygotic co-twins. The reductions seen in the non-demented monozygotic co-twins may indicate a higher genetically mediated risk of AD or genetically mediated hypometabolism possibly rendering them more vulnerable to AD pathogenesis. With no disease modifying treatment available for AD, prevention of dementia is of the utmost importance. A total of 2 165 at least 65 years old twins of the Finnish Twin Cohort with questionnaire data from 1981 participated in a validated telephone interview assessing cognitive function between 1999 and 2007. Those subjects reporting heavy alcohol drinking in 1981 had an elevated cognitive impairment risk over 20 years later compared to light drinkers. In addition, binge drinking was associated with an increased risk even when total alcohol consumption was controlled for, suggesting that binge drinking is an independent risk factor for cognitive impairment. When compared to light drinkers, also non-drinkers had an increased risk of cognitive impairment. Midlife hypertension, obesity and low leisure time physical activity but not hypercholesterolemia were significant risk factors for cognitive impairment. The accumulation of risk factors increased cognitive impairment risk in an additive manner. A previously postulated dementia risk score based on midlife demographic and cardiovascular factors was validated. The risk score was found to well predict cognitive impairment risk, and cognitive impairment risk increased significantly as the score became higher. However, the risk score is not accurate enough for use in the clinic without further testing.

OBJETIVO: Apresentar os resultados iniciais com o uso da técnica de abdominoplastia longitudinal anterior por plicatura parietal descrita por Furness et al (1998) no Hospital Municipal Jesus. MÉTODO: Descrevemos as características e resultados dos primeiros pacientes tratados em nosso serviço, inclusive duas modificações adotadas com relação à técnica descrita, a saber, a exploração retroperitoneal sistemática para cura da criptorquia bilateral simultânea à abdominoplastia e a celiotomia "de segurança" para todos os pacientes operados. RESULTADOS: O seguimento dos pacientes foi de 1,5 ano, 15 meses e 10 meses, respectivamente. Foi possível demonstrar que a técnica adotada foi de fácil execução e pequena morbidade, com resultados estéticos satisfatórios, grande melhora das condições psicológicas e do convívio social para os pacientes desde o pós-operatório imediato, melhora da constipação crônica, capacidade aeróbica e postura. Quanto às repercussões urodinâmicas, aguardamos reavaliação a longo prazo, para conclusões mais efetivas. CONCLUSÃO: A abdominoplastia longitudinal anterior é uma técnica de fácil execução e segura para o tratamento da síndrome de prune belly e melhora a capacidade funcional muscular abdominal. Tem conseqüências urodinâmicas, que necessitam de avaliação após longo prazo.

OBJETIVO: Avaliar os efeitos do sulfato de bário na cavidade pleural de ratos. MÉTODO: Foram avaliados, experimentalmente, os efeitos do sulfato de bário a 100% na cavidade pleural de 43 ratos. Sob anestesia inalatória com éter, foi realizada injeção de contraste radiológico (1ml) na cavidade pleural direita após punção com agulha romba pela via subxifóide. Os ratos, divididos em três grupos, foram mortos em câmara fechada com éter, após 24h (13 ratos), 48h (16 ratos) e 21 dias (14 ratos), respectivamente. Através de esternotomia longitudinal e laparotomia alta, foram retiradas a pleura parietal e visceral, juntas com o gradil costal e o pulmão direito. No grupo-controle, de 22 ratos, foi injetado 1ml de soro fisiológico a 0,9% na cavidade pleural direita. RESULTADOS: Não houve mortes entre os 43 ratos em que foi injetado sulfato de bário e no grupo-controle. As alterações encontradas na cavidade pleural dos grupos injetados com sulfato de bário e mortos com 24h e 48h foram semelhantes: leve e difusa hiperemia na pleura parietal, sulfato de bário livre, derrame pleural inflamatório com predomínio de polimorfonucleares; macrófagos na pleura fagocitando sulfato de bário e pleura com infiltrado predominantemente polimorfonuclear. Com 21 dias, o sulfato de bário estava localizado e bloqueado na região retroesternal e havia formação de sínfises pleurais intensas. No exame histopatológico das pleuras havia grande quantidade de macrófagos repletos de sulfato de bário, raros pigmentos de sulfato de bário no meio extracelular, importante proliferação fibroblástica em 13/14 (92%) casos e não ocorreu formação de granulomas. No grupo-controle (22 ratos), o exame histopatológico foi normal em todas as fases do experimento. CONCLUSÕES: a) o sulfato de bário causou derrame pleural inflamatório em todos os casos; b) com 21 dias ocorreu formação de sínfises pleurais em 100% dos casos; c) não houve formação de granuloma; d) em todas as fases do experimento não ocorreram óbitos.

The authors present a rare case of inflammatory pseudotumor of the liver whose differential diagnosis with hepatocellular carcinoma - in some cases - is only possible with the histological examination. In this case, a female patient, 38 years-old was suffering from abdominal pain, enlarged liver, thinning, tiredness and fever; alpha-fetoprotein, the liver function tests, amylase and lipase were normal. Abdominal ultrasound and computed tomography showed a node in the right liver lobe. The patient was submitted to a right hepatectomy for hepatocellular carcinoma. Histological examination showed an inflammatory pseudotumor of the liver. The patient was discharged from the hospital on the seventh postoperative day; she is asymptomatic for three years.

We report a case of a ten year old boy with an inflammatory pseudotumor in the right upper lobe. Surgical excision was undertaken. Inflammatory pseudotumors of the lung often occur in children. We recommend complete resection for diagnosis and cure. This tumor can mimic malignant neoplastic lesions and can recur after surgical treatment.

Our objective is to report a case of a patient, with a thoraco-abdominal gunshot wound with right hemothorax and liver lesion in the right lobe. The liver and the diafragm were sutured and the chest was drained. On the 9th post operative day the patient had hematemesis, jaundice and pain in the right upper quadrant of the abdomen. The abdominal ultrasound image with Doppler, revealed arteriobiliary fistulae. The diagnosis of hemobilia was made and the patient underwent embolization of the fistulae by liver arteriography.

An isolated hepatic caudate lobectomy was performed in a fifty-seven-year-old white woman presenting a colo-rectal metastasis to hepatic segment I. The resection was performed under total liver vascular exclusion due to the proximity to hepatic veins. The patient presented an uneventful hospitalization being discharged on day seven.

OBJETIVO: Avaliar se o enxerto autógeno de peritônio permanece vivo e que tipo de tecido conjuntivo predomina no local. MÉTODO: Foram utilizados 30 ratos machos para o enxerto de um fragmento de peritônio parietal do segmento ântero-superior do abdome, no interior do canal inguinal, fixado com monofilamento. Após 20 dias os animais foram sacrificados, a área doadora reparada e a área do enxerto foram retiradas para estudo histológico á microscopia óptica de luz. Em cinco animais a parede abdominal e o canal inguinal contralateral, sem enxerto, foram utilizados para o estudo de sua anatomia microscópica. RESULTADOS: Houve um predomínio de fibroblastos (células jovens) no enxerto, em relação ao receptor e de colágeno denso não modelado em toda a área da intervenção, predominando no receptor, com índice de significância p < 0,01. CONCLUSÕES: O enxerto permanece vivo. O padrão histológico do tecido conjuntivo predominante no local é o tecido conjuntivo denso não modelado.
