Objective: To perform a long-term clinical evaluation of the periodontium of removable parti al denture (RPD) wearers, comparing the direct pillar teeth of tooth-supported and toothtissue supported RPDs. Method: Fifty patients with mean age of 45 years were enrolled in the study. The individuals were examined by a single examiner at the moment of denture installation and after 3, 6, 9 and 12 months. In each exam, the following parameters were verified: gingival recession (GR), probing depth (PD), plaque index (PI), gingival index (GI) e amount kerati nized mucosa (KM). All patients received oral hygiene instructions and prophylaxis and, when necessary, scaling and root planing. An analysis from the confidence interval was done to evaluate the endpoints regarding the type of denture in the direct pillar group. Results: The tooth-tissue supported dentures showed significantly higher GR, GI and PI values, and significantly lower KM values. Over time, neither of the types of denture presented statistically significant difference from the initial to the final examination for the parameters GR, PD, KM and GI, while the PI was significant only for the tooth-supported dentures. Conclusion: Pillar teeth adjacent to free ends presented a less favorable periodontal conditi on than the pillar teeth adjacent to intercalated spaces. However, the use of RPD did not aggravate the initial condition, after a follow-up period of 12 months. The findings of the study indicate that, within 1 year, there were no significant differences between the direct pillars of the toothsupported and tooth-ti ssue supported dentures, and suggest the need of professional follow up for a longer period.

The gingiva, part of the masticatory mucosa tissue that covers the alveolar process and surrounds the cervical portion of teeth, has a definitive shape and texture associated with the eruption of teeth. Therefore, it must be included in rehabilitative planning, as the absence of papillae induces problems with esthetics, phonetics and food-impaction. The purpose of the present study was to approach the indications, limitations and techniques for making a removable artificial gingiva made of acrylic resin, by reporting on a clinical case. The patient, a 29-year-old woman, with the sequelae of periodontal disease, presented loss of interdental papilla causing “black holes”. She was dissatisfied with the esthetic appearance of her teeth, and also complained of long teeth and air escaping when she spoke. As treatment, prosthetic reconstruction was proposed, by means of a removable artificial gingiva (gingival epithesis) to simulate the presence of interdental papillae, providing better esthetics and phonetics. Gingival epithesis is an easily made, inserted and hygienically cleaned appliance that provides a simple and safe alternative solution for cases in which surgical techniques are limited.

Angiogenesis and lymphangiogenesis are changes that occur due to gingival inflammation caused by microorganisms present in the biofilm, as well as the migration of immune cells and secretion of mediators in the aggressed site. This study aimed to research angiogenesis and lymphangiogenesis in 90 specimens of clinically healthy, with gingivitis and chronic periodontitis gingival tissue biopsies. The histological sections were evaluated by hematoxylin and eosin and the immunohistochemical technique through immunostaining for CD34 and podoplanin. To evaluate the angiogenic and lymphangiogenic indexes we performed a microvessel counting technique. The results showed that there is a correlation between the indexes (p = 0.030), however, we observed that periodontitis showed less lymphatic vessels than clinically healthy gingival tissue (p = 0.016). Podoplanin showed positive staining in the basal layers of the epithelium, and we observed a relationship between immunostaining intensity and the intensity of inflammatory infiltrate, with more intense staining in the presence of severe inflammatory infiltrate (p = 0.033). For this study, we concluded that there are fewer blood vessels in periodontitis compared with clinically healthy gingiva. The signaling present in the inflammatory process and the actual role of gingival blood and lymphatic vasculature are not fully understood, with further studies on angiogenesis and lymphangiogenesis being suggested.

There is a bidirectional association between periodontal disease (PD) and diabetes mellitus, in which diabetes favors the development of PD and PD, if left untreated, can worsen the metabolic control of diabetes. Thus, periodontal disease should be treated to restore periodontal health and reduce the complications of diabetes. Therefore, the objective is assess the effect of full mouth periodontal therapy decontamination (Full Mouth Desinfection - FMD) in diabetic type II patients with chronic periodontitis during 12 months. Thirty-one patients in group one (G1) and 12 in group two (G2) were followed at baseline, 03, 06 09 and 12 months. There following clinical parameters were accessed: probing on bleeding (BOP), visible plaque index (PI), probing depth (PD), clinical attachment level (CAL) and gingival recession (GR). For diabetic patients, there were also made laboratory tests to evaluate blood parameters: fasting glucose and glycated hemoglobin. The results had been analyzed in two ways: all sites in the mouth and another with diseased sites. The Mann-Whitney, Friedman and Wilcoxon tests were used with 5% significance. Intergroup analysis of all sites it is clear that there was no significant difference over time concerning PD, BOP, PI, CAL and RG. However, when evaluating the diseased sites, we observed significant difference for CAL and PD, with higher values in G1. The intragroup analysis for all sites showed a statistically significant reduction at PD, PI and BOP in both groups. Intragroup analysis of periodontal affected sites showed a statistically significant reduction in PD, BOP and CAL in both groups. There was also a statistically significant increase in RG values. There was no significant change concerning glycated hemoglobin and fasting glucose in the G1. Therefore, it can be concluded that there were improvements in periodontal parameters over the 12 months of research, but without changes in glycemic levels of diabetic patients. Thus, periodontal therapy proved effective in maintaining oral health.

The coexistence of gingival recession (GR) with root coverage indication and non-carious cervical lesions (LCNC) generates the need for a protocol that respects and promotes health of dental and periodontal tissues and allows treatment predictability. The main objectives of this theses were: (1) verify, through clinical evaluations, the connective tissue graft for root coverage on direct and indirect restorations made of ceramic resin; (2) analyze the influence of the battery level of the LED curing unit in the composite resin characteristics; (3) assess the influence of restorative materials, composite resin and ceramics, on the viability of gingival fibroblasts from primary culture. Nine patients with good oral hygiene and occlusal stability diagnosed with LCNCs the anterior teeth including premolars associated with gingival recession (class I and II of Miller) and only gingival recession were selected. After initial clinical examination, occlusal adjustment was performed and the patients had their teeth randomized allocated on direct composite resin restoration of LCNC, polishing and GR treatment with connective tissue graft and advanced coronally flap CR group (n = 15); and indirect ceramic restoration of the LCNC's and GR treatment (CTG+CAF) Group C (n = 15). The GR presented teeth with no clinically formed LCNCs cavity were treated using (CTG+CAF) being the control group (n = 15). Sorption and solubility tests, analysis of the degree of conversion and diametral tensile strength were performed in composite resin samples (n = 10) photoactivated by 100, 50 and 10% battery charge LED unit. The viability of fibroblasts on composite resin, ceramics and dentin disks (n = 3) was examined. Clinical follow-up was performed for three months. The data obtained at different stages were tabulated and subjected to analysis for detection of normal distribution and homogeneity. The results showed that: the LED unit with 10% battery affects the characteristics of the composite resin; restorative materials present biocompatibility with gingival fibroblasts; and the association of surgical and restorative treatment of teeth affected by NCCL and GR presents successful results at 3-month follow-up.

Background: Several theories, such as the biological width formation, the inflammatory reactions due to the implant-abutment microgap contamination, and the periimplant stress/strain concentration causing bone microdamage accumulation, have been suggested to explain early periimplant bone loss. However, it is yet not well understood to which extent the implant-abutment connection type may influence the remodeling process around dental implants. Aim: to evaluate clinical, bacteriological, and biomechanical parameters related to periimplant bone loss at the crestal region, comparing external hexagon (EH) and Morse-taper (MT) connections. Materials and methods: Twelve patients with totally edentulous mandibles received four custom made Ø 3.8 x 13 mm implants in the interforaminal region of the mandible, with the same design, but different prosthetic connections (two of them EH or MT, randomly placed based on a split-mouth design), and a immediate implant- supported prosthesis. Clinical parameters (periimplant probing pocket depth, modified gingival index and mucosal thickness) were evaluated at 6 sites around the implants, at a 12 month follow-up. The distance from the top of the implant to the first bone-to-implant contact – IT-FBIC was evaluated on standardized digital peri-apical radiographs acquired at 1, 3, 6 and 12 months follow-up. Samples of the subgingival microbiota were collected 1, 3 and 6 months after implant loading. DNA were extracted and used for the quantification of Tanerella forsythia, Porphyromonas gingivalis, Aggragatibacter actinomycetemcomitans, Prevotella intermedia and Fusobacterium nucleatum. Comparison among multiple periods of observation were performed using repeated-measures Analysis of Variance (ANOVA), followed by a Tukey post-hoc test, while two-period based comparisons were made using paired t- test. Further, 36 computer-tomographic based finite element (FE) models were accomplished, simulating each patient in 3 loading conditions. The results for the peak EQV strain in periimplant bone were interpreted by means of a general linear model (ANOVA). Results: The variation in periimplant bone loss assessed by means of radiographs was significantly different between the connection types (P<0.001). Mean IT-FBIC was 1.17±0.44 mm for EH, and 0.17±0.54 mm for MT, considering all evaluated time periods. All clinical parameters presented not significant differences. No significant microbiological differences could be observed between both connection types. Most of the collected samples had very few pathogens, meaning that these regions were healthy from a microbiological point of view. In FE analysis, a significantly higher peak of EQV strain (P=0.005) was found for EH (mean 3438.65 µ∑) compared to MT (mean 840.98 µ∑) connection. Conclusions: Varying implant-abutment connection type will result in diverse periimplant bone remodeling, regardless of clinical and microbiological conditions. This fact is more likely attributed to the singular loading transmission through different implant-abutment connections to the periimplant bone. The present findings suggest that Morse-taper connection is more efficient to prevent periimplant bone loss, compared to an external hexagon connection.

O sorriso não se baseia apenas em factores dentários relacionados com a cor, a forma ou o alinhamento dos dentes na arcada, mas implica também a presença de tecidos periodontais saudáveis e com um contorno gengival harmónico. Este trabalho tem como objectivo abordar as diversas técnicas de aumento de coroa clínica, enunciar as vantagens e indicações das mesmas, bem como comparar as técnicas cirúrgicas com as ortodônticas. Para tal foi realizada uma pesquisa bibliográfica recorrendo aos motores de busca da Pubmed e b-on, utilizando como palavras-chave: crown lengthening, biological width, crown lengthening AND surgery e crown lengthening AND orthodontic extrusion. Dos 539 artigos encontrados, foram seleccionados 28 que correspondiam aos critérios de inclusão por nós estabelecidos. Critérios de inclusão: meta-análises, ensaios clínicos randomizados e revisões sistemáticas publicadas em Português, Inglês e Espanhol nos últimos 12 anos. De acordo com a literatura, podemos verificar que o aumento de coroa clínica está indicado em várias situações clínicas tais como: cáries infra-gengivais, fracturas radiculares, resolução de alguns problemas estéticos, como o sorriso gengival, principalmente em casos de erupção passiva alterada e assimetrias das margens gengivais. Este aumento pode ser realizado por técnicas cirúrgicas (gengivectomia e retalho de reposicionamento apical), técnicas ortodônticas (extrusão ortodôntica com ou sem fibrotomia) ou através da combinação de ambas.

A doença periodontal é caracterizada como um conjunto de condições inflamatórias, de carater crônico ou agudo, e de origem bacteriana, que começa por afetar o tecido gengival e pode levar, com o tempo, à perda dos tecidos de suporte dos dentes. As reações inflamatórias e imunológicas à placa bacteriana representam as características predominantes da gengivite e da periodontite. A reação inflamatória é visível, microscópica e clinicamente, no periodonto afetado e representa a reação do hospedeiro à microbiota da placa e seus produtos. O processo de infecção no sulco periodontal leva, inicialmente, a formação de uma mucosite periodontal, que pode ser definida como uma inflamação dos tecidos moles periodontáis, sem ocasionar perda óssea, sendo reversível, se o seu diagnóstico for atempado. Os processos inflamatórios e imunológicos atuam nos tecidos gengivais para proteger contra o agressãoes microbianas, impedindo os microrganismos de se disseminarem ou invadirem os tecidos. Em alguns casos, essas reações de defesa do hospedeiro podem ser prejudiciais porque também são passíveis de danificar as células e estruturas vizinhas do tecido conjuntivo. Além disso, as reações inflamatórias e imunológicas cuja extensão alcança níveis mais profundos do tecido conjuntivo, além da base do sulco, podem envolver o osso alveolar nesse processo destrutivo. Assim, tais processos defensivos podem, paradoxalmente, ser os responsáveis pela maior parte da lesão tecidual observada na gengivite e na periodontite. O objectivo desse trabalho é fazer uma revisão de literatura específica sobre a etiologia da doença periodontal respectivamente. Serão descritos os principais agentes microbianos que estão relacionados com a doença periodontal e a forma como influenciam o desenvolvimento da doença, procurando desta forma contribuir para a procura de tratamentos mais eficientes.

A periodontite é uma patologia inflamatória, causada pela interação entre o biofilme bacteriano e a resposta imune do hospedeiro e é caracterizada pela degradação de fibras colágenas do periodonto. Os avanços científicos possibilitaram o desenvolvimento de novas técnicas que possibilitem o diagnóstico precoce da doença, ou seja, prevenir a perda de inserção e progressão da periodontite. O diagnóstico da doença periodontal baseia-se em parâmetros clínicos como profundidade de sondagem, perda de inserção, hemorragia a sondagem e exames radiográficos. A análise dos biomarcadores presentes no fluido crevicular gengival (FCG) e saliva tem sido alvo de inúmeros artigos, uma vez que o fluido crevicular gengival (FCG) é composto de uma gama de biomarcadores celulares e moleculares. O presente trabalho teve como objetivo fazer uma revisão narrativa da literatura publicada a cerca dos métodos utilizados para diagnóstico precoce e prognóstico dos processos patológicos que podem estar presentes no periodonto. Foi realizada uma pesquisa bibliográfica online entre Dezembro de 2015 e Maio de 2016, com o objetivo de fazer uma revisão bibliográfica acerca do tema. Não foi estabelecido restrição quanto a data de publicação. Através da identificação da concentração de determinadas proteínas presentes no FCG é possível avaliar a atividade da doença periodontal e o prognóstico. Um importante número de marcadores como a elastase neutrofílica, prostaglandina E2, catepsinas, osteocalcina, -glucoronidase, colagenase, fosfatase alcalina, aspartato aminotransferase são citados (Genco et al., 1992). De acordo com a literatura publicada pode-se concluir que há alguns marcadores promissores para o diagnóstico precoce e prognóstico da doença periodontal, entretanto, não há ainda um marcador que possa identificar a futura perda de inserção ou a suscetibilidade à doença periodontal. São necessários estudos longitudinais para melhor entendimento do papel dos biomarcadores do FCG.

Existe uma grande quantidade de fármacos que provocam reacções adversas na mucosa oral. Os efeitos que causam são variados, nomeadamente angioedema, língua pilosa nigra, bruxismo, síndrome da boca ardente, candidose, queilite, xerostomia, eritema multiforme, hiperplasia gengival, glossite, halitose, hiperpigmentação das mucosas, hipersalivação, reações liquenóides, lupóides, penfigóides e tipo pênfigo, osteonecrose, tumefação das glandulas salivares, alterações do paladar, ulcerações, mucosite e lesões potencialmente malignas, entre outras. Com o presente trabalho pretende-se criar uma base de dados que facilite a pesquisa dos diferentes efeitos adversos e relação com a terapêutica prescrita ao paciente, de modo a auxiliar a vida ao Médico Dentista no contexto da prestação de serviços aos seus Utentes.

El abordaje terapéutico de las recesiones gingivales requiere un plan de tratamiento que involucre terapia básica, la cual tendrá como objetivo la resolución de la/las etiologías de la/las mismas y una terapia de cirugía plástica periodontal adecuada al caso específico. Los procedimientos quirúrgicos con injertos de tejido conjuntivo tomados principalmente del paladar son el gold standard en el tratamiento de las recesiones gingivales. Uno de los principales éxitos en prostodoncia es la exactitud de la restauración final que garantice la sobrevida de las prótesis, la reproducción precisa de márgenes de la preparación en una impresión es un requisito necesario para lograr una buena calidad marginal. Es por eso que la técnica y material de impresión, así como el manejo de los tejidos periodontales resultan claves en la consecución del éxito. Entender las propiedades de cada uno de los materiales utilizados durante el proceso de toma de impresión así como el diagnóstico oportuno de la condición periodontal previa (salud, enfermedad, biotipo periodontal) y las técnicas disponibles para la retracción o desplazamiento de los tejidos gingivales, proporcionan al clínico las herramientas necesarias para la toma de decisiones y la consecución de los mejores resultados estéticos y funcionales en prótesis fija. El propósito de esta revisión de tema es proponer una serie de recomendaciones para el clínico restaurador desde el punto de vista periodontal, que le permita establecer protocolos en el diagnóstico, planificación y ejecución de sus casos clínicos, conociendo las actuales técnicas de desplazamiento o retracción gingival y toma de impresión y sus efectos sobre el periodonto.
