The reactivation of Chagas disease in HIV infected patients presents high mortality and morbidity. We present the case of a female patient with confirmed Chagasic meningoencephalitis as AIDS-defining illness. Interestingly, her TCD4+ lymphocyte cell count was 318 cells/mm3. After two months of induction therapy, one year of maintenance with benznidazol, and early introduction of highly active antiretroviral therapy (HAART), the patient had good clinical, parasitological and radiological evolution. We used a qualitative polymerase chain reaction for the monitoring of T. cruzi parasitemia during and after the treatment. We emphasize the potential value of molecular techniques along with clinical and radiological parameters in the follow-up of patients with Chagas disease and HIV infection. Early introduction of HAART, prolonged induction and maintenance of antiparasitic therapy, and its discontinuation are feasible, in the current management of reactivation of Chagas disease.

Publications are often used as a measure of research work success. Human T-lymphotropic virus (HTLV) type 1 and 2 are human retroviruses, which were discovered in the early 1980s, and it is estimated that 15-20 million people are infected worldwide. This article describes a bibliometric review and a coauthorship network analysis of literature on HTLV indexed in PubMed in a 24-year period. A total of 7,564 documents were retrieved, showing a decrease in the number of documents from 1996 to 2007. HTLV manuscripts were published in 1,074 journals. Japan and USA were the countries with the highest contribution in this field (61%) followed by France (8%). Production ranking changed when the number of publications was normalized by population (Dominican Republic and Japan), by gross domestic product (Guinea-Bissau and Gambia), and by gross national income per capita (Brazil and Japan). The present study has shed light on some of the defining features of scientific collaboration performed by HTLV research community, such as the existence of core researchers responsible for articulating the development of research in the area, facilitating wider collaborative relationships and the integration of new authors in the research groups.

Candida albicans is an opportunistic human pathogen that is capable of causing superficial and systemic infections in immunocompromised patients. Extracts of Sapindus saponaria have been used as antimicrobial agents against various organisms. In the present study, we used a combination of two-dimensional polyacrylamide gel electrophoresis (2D-PAGE) and matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) to identify the changes in protein abundance of C. albicans after exposure to the minimal inhibitory concentration (MIC) and sub-minimal inhibitory concentration (sub-MIC) of the butanolic extract (BUTE) of S. saponaria and also to fluconazole. A total of six different proteins with greater than 1.5 fold induction or repression relative to the untreated control cells were identified among the three treatments. In general, proteins/enzymes involved with the glycolysis (GPM1, ENO1, FBA1), amino acid metabolism (ILV5, PDC11) and protein synthesis (ASC1) pathways were detected. In conclusion, our findings reveal antifungal-induced changes in protein abundance of C. albicans. By using the previously identified components of the BUTE of S. saponaria(e.g., saponins and sesquiterpene oligoglycosides), it will be possible to compare the behavior of compounds with unknown mechanisms of action, and this knowledge will help to focus the subsequent biochemical work aimed at defining the effects of these compounds.

Introdução: Os doentes infectados pelo Vírus da Imunodeficiência Humana têm um risco elevado de desenvolver diferentes tipos de Neoplasias. Com a introdução da terapêutica anti-retroviral de alta potência, e consequente aumento da sobrevida, assistimos a uma mudança do espectro das patologias relacionadas com a infecção, nomeadamente das doenças Oncológicas, com aumento das Neoplasias Não Definidoras em deterimento das Definidoras de SIDA. Material e Métodos: Caracterização dos doentes com infecção Vírus da Imunodeficiência Humana e diagnóstico de Neoplasias Não Definidoras acompanhados ao longo de 16 anos na Consulta de Medicina/Imunodeficiência do Hospital de São José, através da consulta dos processos clínicos e avaliação retrospectiva dos aspectos demográficos, epidemiológicos, clínico-laboratoriais, tratamento e sobrevida. Resultados: Nos 1042 doentes avaliados, foram identificados 34 casos de Neoplasias Não Definidoras, principalmente em homens(78%) e com idade mediana de 55 anos. As neoplasias mais frequentes foram: pulmão (20,6%), bexiga (17,6%), próstata (8,8%) e canal anal (5,9%), sendo o tempo médio entre o diagnóstico da infecção pelo Vírus da Imunodeficiência Humana e da Neoplasias Não Definidoras de 6,8 ± 4 anos. Na altura do diagnóstico da Neoplasias Não Definidoras a maioria dos doentes (78,8%) estava sob terapêutica anti-retroviral de alta potência, em média desde há 5,7 ± 3 anos, encontrando-se imunovirologicamente controlada. No total verificaram-se 45,5% óbitos, sobretudo em doentes com Neoplasia do pulmão (20%). Conclusão: Perante o risco de desenvolvimento de Neoplasias Não Definidoras nos doentes infectados pelo Vírus da Imunodeficiência Humana, torna-se fundamental o investimento em estratégias de prevenção, promoção de cessação tabágica e vacinação, bem como aplicação de protocolos de rastreio ajustados a esta população.

In a liver transplant (LT) center, treatments with Prometheus were evaluated. The main outcome considered was 1 and 6 months survival. Methods. During the study period, 74 patients underwent treatment with Prometheus; 64 were enrolled,with a mean age of 51 13 years; 47men underwent 212 treatments (mean, 3.02 per patient). The parameters evaluated were age, sex, laboratorial (liver enzymes, ammonia) and clinical (model for end-stage liver disease and Child-Turcotte-Pugh score) data. Results. Death was verified in 23 patients (35.9%) during the hospitalization period, 20 patients (31.3%) were submitted to liver transplantation, and 21 were discharged. LT was performed in 4 patients with acute liver failure (ALF, 23.7%), in 7 patients with acute on chronic liver failure (AoCLF, 43.7%), and in 6 patients with liver disease after LT (30%). Seven patients who underwent LT died (35%). In the multivariate analysis, older age (P ¼ .015), higher international normalized ratio (INR) (P ¼ .019), and acute liver failure (P ¼ .039) were independently associated with an adverse 1-month clinical outcome. On the other hand, older age (P ¼ .011) and acute kidney injury (P ¼ .031) at presentation were both related to worse 6-month outcome. For patients with ALF and AoCLF we did not observe the same differences. Conclusions. In this cohort, older age was the most important parameter defining 1- and 6-month survival, although higher INR and presence of ALF were important for 1-month survival and AKI for 6-month survival. No difference was observed between patients who underwent LT or did not have LT.
