This article provides an overview of the relevance and import of the U.N. Convention on the Rights of the Child (CRC) to child health practice and pediatric bioethics. We discuss the four general principles of the CRC that apply to the implementation of all rights contained in the document, the right to health articulated in Article 24, and the important position ascribed to parents in fulfilling the rights of their children. We then examine how the CRC is implemented and monitored in law and practice. The CRC and associated principles of child rights provide strategies for rights-based approaches to clinical practice and health systems, as well as to policy design, professional training, and health services research. In light of the relevance of the CRC and principles of child rights to children’s health and child health practice, it follows that there is an intersection between child rights and pediatric bioethics. Pediatric bioethicists and child rights advocates should work together to define this intersection in all domains of pediatric practice.

Recent evidence suggests that - in addition to 17p deletion - TP53 mutation is an independent prognostic factor in chronic lymphocytic leukemia (CLL). Data from retrospective analyses and prospective clinical trials show that ∼5% of untreated CLL patients with treatment indication have a TP53 mutation in the absence of 17p deletion. These patients have a poor response and reduced progression-free survival and overall survival with standard treatment approaches. These data suggest that TP53 mutation testing warrants integration into current diagnostic work up of patients with CLL. There are a number of assays to detect TP53 mutations, which have respective advantages and shortcomings. Direct Sanger sequencing of exons 4-9 can be recommended as a suitable test to identify TP53 mutations for centers with limited experience with alternative screening methods. Recommendations are provided on standard operating procedures, quality control, reporting and interpretation. Patients with treatment indications should be investigated for TP53 mutations in addition to the work-up recommended by the International workshop on CLL guidelines. Patients with TP53 mutation may be considered for allogeneic stem cell transplantation in first remission. Alemtuzumab-based regimens can yield a substantial proportion of complete responses, although of short duration. Ideally, patients should be treated within clinical trials exploring new therapeutic agents.

This thesis examines the impact on child and adolescent psychotherapists within CAMHS of the introduction of routine outcome measures (ROMs) associated with the Children and Young People’s Improving access to Psychological Therapies programme (CYP-IAPT). All CAMHS therapists working within a particular NHS mental health Trust1 were required to trial CYP-IAPT ROMs as part of their everyday clinical practice from October 2013-September 2014. During this period considerable freedom was allowed as to which of the measures each therapist used and at what frequency. In order to assess the impact of CYP-IAPT ROMs on child psychotherapy, I conducted semi-structured interviews with eight psychotherapists within a particular CAMHS partnership within one NHS Trust. Each statement was coded and grouped according to whether it related to initial (generic) assessment, goal setting / monitoring, monitoring on-going progress, therapeutic alliance, or to issues concerning how data might be used or interpreted by managers and commissioners. Analysis of interviews revealed greatest concern about session-by session ROMs, as these are felt to impact most significantly on psychotherapy; therapists felt that session-by-session ROMs do not take account of negative transference relationships, they are overly repetitive and used to reward / punish the therapist. Measures used at assessment and review were viewed as most compatible with psychotherapy, although often experienced as excessively time consuming. The Goal Based Outcome Measure was generally experienced as compatible with psychotherapy so long as goals are formed collaboratively between therapist and young person. There was considerable anxiety about how data may be (mis)used and (mis)interpreted by managers and commissioners, for example to end treatment prematurely, trigger change of therapist in the face of negative ROMs data, or to damage psychotherapy. Use of ROMs for short term and generic work was experienced as less intrusive and contentious.

Os profissionais de Saúde Oral, nomeadamente os Médicos Dentistas, têm vindo a enfrentar variados riscos decorrentes da sua atividade profissional. Apesar de haver um esforço constante no sentido de melhorar os equipamentos e materiais dentários, através dos avanços tecnológicos, estes não conseguiram, ainda, colmatar significativamente os problemas Músculo-Esqueléticos dos Médicos Dentistas. Este tipo de problemas surge enquanto estudantes, durante a prática clínica, muitas vezes devido às condições de trabalho, à inexperiência inerente, mas, principalmente, devido às postura e aos hábitos de trabalho errados que adquirem e que, consequentemente, persistem ao longo da sua vida profissional. A presente dissertação, aliada a uma vasta literatura relacionada, pretende alertar os profissionais de Saúde Oral, com foco nos Médicos Dentistas, para as patologias decorrentes das posturas erradas no exercício da Medicina Dentária, denominadas de Desordens Músculo-Esqueléticas relacionadas com o Trabalho, identificando-as, assim como aos fatores de risco que influenciam o seu aparecimento. A elaboração desta dissertação, pretende, ainda, propor exercícios de Ginástica Laboral a realizar entre consultas, como estratégia de prevenção para o surgimento de Lesões MúsculoEsqueléticas Relacionadas com o Trabalho nos Médicos Dentistas e alertar para a importância da Ergonomia na conceção de um Consultório Dentário. Para a presente revisão bibliográfica, foi realizada uma pesquisa bibliográfica com recurso a livros e artigos publicados em revistas, e que foram consultados nas Bibliotecas da Faculdade de Medicina Dentária da Universidade do Porto e da Universidade Fernando Pessoa. Procedeu-se à pesquisa por recurso aos motores de busca na internet, tais como, PubMed, Scielo, B-On e Medline, utilizando as seguintes palavras-chave, em conjunto ou individualmente: “Lesões por esforços repetitivos”; “Lesões Músculo-Esqueléticas”; “Ergonomia”; “Prevenção”; “Fatores de Risco”; “Ginástica Laboral”; “Work related musculoskeletal disorders”; “Dentistry”; “Ergonomics”; “Pain”; “Prevention”; “Risk factors”. Foram selecionados artigos entre 1987 e 2015, com relevância para o presente trabalho de dissertação de Mestrado, escritos em Português e Inglês. Desta forma, a pesquisa bibliográfica permitiu verificar a prevalência de Lesões Músculo-Esqueléticas Relacionadas com o Trabalho, no âmbito da Medicina Dentária, assim como os fatores de risco associados ao seu aparecimento. Foram ainda tecidas algumas recomendações de forma a contribuir para o bem-estar dos Médicos Dentistas, elucidando-os para a necessidade de adotar posturas corretas e usando a Ergonomia como base para a organização e conceção de um Consultório Dentário. Recorreu-se, ainda, à ilustração de um programa de Ginástica Laboral, através de dois cartazes elucidativos, com o objetivo de prevenir, corrigir e compensar este tipo de patologias do Médico Dentista, desde a sua prática clínica. Após a elaboração desta dissertação, acredita-se ser de extrema importância a aplicação de diretrizes ergonómicas na conceção de um Consultório Dentário, na organização de tarefas, no procedimento clínico, na adoção de posturas, no posto de trabalho, na localização do equipamento e na escolha dos instrumentos. Este método, para além de minimizar o risco do aparecimento de doenças profissionais, também simplifica tarefas, proporciona a correta comunicação entre o Médico Dentista e o assistente e melhora a qualidade e produtividade laboral, através da redução da fadiga física e mental e do aumento da confiança e bem-estar dos Médicos Dentistas.

Introdução: Com uma consciencialização cada vez maior das populações para a preservação dos dentes naturais, a Endodontia tem assumido uma importância crescente, tendo por objectivo principal a manutenção de dentes funcionais, sem prejudicar a saúde dos pacientes. Estes devem beneficiar de um tratamento segundo o “standard of care”, proporcionado por profissionais competentes. Vários estudos demonstram que a Endodontia é considerada uma área difícil e stressante para os estudantes, exibindo um sentimento de menor confiança na prática clínica, sobretudo no que diz respeito a procedimentos mais complexos, nomeadamente, no tratamento de dentes posteriores. Em 2010, na tentativa de homogeneizar as competências adquiridas pelos Médicos Dentistas (MD), a Associação para a Educação Dentária Europeia (AEDE) definiu critérios para a sua formação pré-graduada, indicando que o MD recém-graduado deve ter adquirido competências e capacidades técnicas que lhe permita começar a sua prática clínica de forma independente. Objectivos: Analisar os conhecimentos adquiridos e as dificuldades sentidas na área da Endodontia pelos alunos do 4º e 5º anos do Mestrado Integrado em Medicina Dentária da Faculdade de Ciências de Saúde (FCS) da Universidade Fernando Pessoa (UFP), fazendo, igualmente, uma abordagem ao ensino ministrado na área da Endodontia face aos resultados obtidos. Materiais e Métodos: Este trabalho está dividido em duas partes: revisão bibliográfica e investigação científica. A revisão bibliográfica do tema engloba o ensino graduado em Medicina Dentária, o uso de isolamento absoluto (IA) na prática clínica de Endodontia, a instrumentação manual versus rotatória, a influência no sucesso do tratamento endodôntico (TE) da sua execução por estudantes do 4º e 5º ano, as Guidelines para o TE e o ensino de Endodontia na UFP. A pesquisa bibliográfica foi efectuada através da base de dados PubMed, tendo sido utilizadas, em diferentes combinações, as seguintes palavras-chave: ”Endodontics”, “teaching”; “pre-clinical”; “undergraduate”; “Clinical”; “treatment” e “Europe”. O trabalho de investigação consistiu na elaboração, aprovação pela Comissão de Ética da UFP e posterior aplicação de um questionário destinado aos alunos do 4º e 5º ano do Mestrado Integrado em Medicina Dentária da FCS-UFP. A população estimada de alunos do 4º ano foi de 190 alunos e a de 5º ano de 150 alunos, o que perfaz um total de 340 alunos. Dos inquéritos respondidos, apenas foram considerados 338, uma vez que dois foram anulados pois continham respostas inválidas. Resultados: A “condição socioeconómica do paciente” foi considerada a causa menos relevante para avaliar um caso endodôntico, tendo sido apontada por 34,3% dos alunos. Assinala-se que 92% dos alunos de 4º ano e 93,3% dos alunos de 5º ano consideram que os molares superiores são os dentes mais difíceis de tratar, sendo a visibilidade uma das principais razões para esta opinião. O grau de dificuldade para a colocação do IA é definido como “Elevado” para a maioria dos alunos de 4º ano. A maior parte dos alunos de 5º ano considera que o grau de dificuldade para a “Determinação do tipo de reconstrução/prótese fixa mais indicada” é “Elevado”. O passo do TE onde os alunos do 4º ano se sentem mais confiantes é no “Diagnóstico de cárie”, sendo que os alunos do 5º ano se sentem mais confiantes ao realizar o “TE em dentes com 1 ou 2 canais”. O “Conteúdo leccionado nas aulas teóricas” foi considerado o principal aspecto positivo do ensino endodôntico. Por outro lado, o “Número de actos clínicos realizados” foi considerado o principal aspecto negativo, tanto para alunos de 4º como de 5º ano. Em ambos os anos, os principais pontos que os alunos acham que devem ser melhorados são o “Número de pacientes nas aulas clínicas” e a “Aprendizagem da técnica de instrumentação mecanizada/obturação termoplástica”. As principais preocupações referidas, tanto por alunos de 4º como de 5º anos, foram a “Insegurança na prática clínica” e a “Dificuldade em encontrar ofertas profissionais”. Relativamente à qualidade dos seus TE existe um número significativo de alunos do 5º ano que já se auto-avaliam como “Bons” (33,6%), por comparação com os do 4º ano (21,2%), sendo a auto-avaliação de “Razoável” dominante em ambos os anos de formação. Conclusões: Conclui-se que na disciplina de Endodontia na FCS-UFP, são seguidas as Guidelines da European Society of Endodontology (ESE), sendo que estas indicam o protocolo mais correcto a seguir durante o Tratamento Endodôntico Não-Cirúrgico (TENC). Contudo, o reduzido número de actos clínicos e a consequente falta de prática faz com que os alunos se sintam pouco confiantes ao iniciar a sua actividade profissional.

The exponential increase in clinical research has profoundly changed medical sciences. Evidence that has accumulated in the past three decades from clinical trials has led to the proposal that clinical care should not be based solely on clinical expertise and patient values, and should integrate robust data from systematic research. As a consequence, clinical research has become more complex and methods have become more rigorous, and evidence is usually not easily translated into clinical practice. Therefore, the instruction of clinical research methods for scientists and clinicians must adapt to this new reality. To address this challenge, a global distance-learning clinical research-training program was developed, based on collaborative learning, the pedagogical goal of which was to develop critical thinking skills in clinical research. We describe and analyze the challenges and possible solutions of this course after 5 years of experience (2008-2012) with this program. Through evaluation by students and faculty, we identified and reviewed the following challenges of our program: 1) student engagement and motivation, 2) impact of heterogeneous audience on learning, 3) learning in large groups, 4) enhancing group learning, 5) enhancing social presence, 6) dropouts, 7) quality control, and 8) course management. We discuss these issues and potential alternatives with regard to our research and background.

Penetration of fractional flow reserve (FFR) in clinical practice varies extensively, and the applicability of results from randomized trials is understudied. We describe the extent to which the information gained from routine FFR affects patient management strategy and clinical outcome. METHODS AND RESULTS: Nonselected patients undergoing coronary angiography, in which at least 1 lesion was interrogated by FFR, were prospectively enrolled in a multicenter registry. FFR-driven change in management strategy (medical therapy, revascularization, or additional stress imaging) was assessed per-lesion and per-patient, and the agreement between final and initial strategies was recorded. Cardiovascular death, myocardial infarction, or unplanned revascularization (MACE) at 1 year was recorded. A total of 1293 lesions were evaluated in 918 patients (mean FFR, 0.81±0.1). Management plan changed in 406 patients (44.2%) and 584 lesions (45.2%). One-year MACE was 6.9%; patients in whom all lesions were deferred had a lower MACE rate (5.3%) than those with at least 1 lesion revascularized (7.3%) or left untreated despite FFR≤0.80 (13.6%; log-rank P=0.014). At the lesion level, deferral of those with an FFR≤0.80 was associated with a 3.1-fold increase in the hazard of cardiovascular death/myocardial infarction/target lesion revascularization (P=0.012). Independent predictors of target lesion revascularization in the deferred lesions were proximal location of the lesion, B2/C type and FFR. CONCLUSIONS: Routine FFR assessment of coronary lesions safely changes management strategy in almost half of the cases. Also, it accurately identifies patients and lesions with a low likelihood of events, in which revascularization can be safely deferred, as opposed to those at high risk when ischemic lesions are left untreated, thus confirming results from randomized trials.

Résumé : Les réanimateurs ont recours à des interventions à la fois médicales et chirurgicales en contexte de choc traumatique. Le rôle des vasopresseurs dans cette prise en charge est controversé. Alors que les lignes directrices américaines considèrent que les vasopresseurs sont contre-indiqués, certains experts européens en encouragent l’utilisation pour diminuer le recours aux liquides intraveineux. Avant d’élaborer un essai clinique, il importe de comprendre la pratique actuelle à laquelle se comparera une intervention expérimentale, ainsi que de connaître le niveau d’incertitude dans la littérature entourant la question de recherche. Le Chapitre 2 de ce travail présente une étude observationnelle effectuée dans un centre régional de traumatologie québécois. Cette étude documente les pratiques de réanimation adoptées par les équipes de traumatologie en 2013, particulièrement le recours aux liquides intraveineux et aux vasopresseurs. Les résultats démontrent que les vasopresseurs ont été utilisés chez plus de 40% des patients, particulièrement les victimes de traumatismes crâniens (RC 10.2, IC 95% 2.7-38.5). De plus, les vasopresseurs ont été administrés dans les phases précoces de la réanimation, soit avant l’administration d’un volume important de liquides. Le Chapitre 3 présente une revue systématique portant sur l’utilisation précoce de vasopresseurs en traumatologie. Les bases de données MEDLINE, EMBASE, CENTRAL et ClinicalTrials.gov ont été interrogées, ainsi que les abrégés présentés dans les conférences majeures en traumatologie depuis 2005. La sélection des études et l’extraction des données ont été effectuées en duplicata. Aucune donnée interprétable n’a pu être extraite des études observationnelles et le seul essai clinique identifié n’avait pas une puissance suffisante (RR de mortalité avec vasopresseurs 1.24, IC 95 % 0.64-2.43). Cette synthèse met en lumière l’incertitude scientifique sur le rôle des vasopresseurs en traumatologie. Les vasopresseurs ont des bénéfices potentiels importants, puisqu’ils permettent entre autres de supporter étroitement l’hémodynamie des patients. En revanche, ils présentent aussi un fort potentiel de dangerosité. Ils sont utilisés fréquemment, malgré l’absence de données sur leurs risques et bénéfices. Ces trouvailles établissent clairement la pertinence clinique et le bien-fondé éthique d’un essai clinique sur le rôle des vasopresseurs dans la prise en charge précoce des victimes de traumatismes.

Abstract : Providing high-quality clinical experiences to prepare students for the complexities of the current health-care system has become a challenge for nurse educators. Additionally, there are concerns that the current model of clinical practice is suboptimal. Consequently, nursing programs have explored the partial replacement of traditional in-hospital clinical experiences with a simulated clinical experience. Despite research demonstrating numerous benefits to students following participation in simulation activities, insufficient research conducted within Québec exists to convince the governing bodies (Ordre des infirmières et des infirmiers du Québec, OIIQ; Ministère de L’Éducation supérieur, de la Recherche, de la Science et de la Technologie) to fully embrace simulation as part of nurse training. The purpose of this study was to examine the use of a simulated clinical experience (SCE) as a viable, partial pedagogical substitute for traditional clinical experience by examining the effects of a SCE on CEGEP nursing students’ perceptions of self-efficacy (confidence), and their ability to achieve course objectives. The findings will contribute new information to the current body of research in simulation. The specific case of obstetrical practice was examined. Based on two sections of the Nursing III-Health and Illness (180-30K-AB) course, the sample was comprised of 65 students (thirty-one students from section 0001 and thirty-four students from section 0002) whose mean age was 24.8 years. With two sections of the course available, the opportunity for comparison was possible. A triangulation mixed method design was used. An adapted version of Ravert’s (2004) Nursing Skills for Evaluation tool was utilized to collect data regarding students’ perceptions of confidence related to the nursing skills required for care of mothers and their newborns. Students’ performance and achievement of course objectives was measured through an Objective Structured Clinical Examination (OSCE) consisting of three marked stations designed to test the theoretical and clinical aspects of course content. The OSCE was administered at the end of the semester following completion of the traditional clinical experience. Students’ qualitative comments on the post -test survey, along with journal entries served to support the quantitative scale evaluation. Two of the twelve days (15 hours) allocated for obstetrical clinical experience were replaced by a SCE (17%) over the course of the semester. Students participated in various simulation activities developed to address a range of cognitive, psychomotor and critical thinking skills. Scenarios incorporating the use of human patient simulators, and designed using the Jeffries Framework (2005), exposed students to the care of families and infants during the perinatal period to both reflect and build upon class and course content in achievement of course objectives and program competencies. Active participation in all simulation activities exposed students to Bandura’s four main sources of experience (mastery experiences, vicarious experiences, social persuasion, and physiologic/emotional responses) to enhance the development of students’ self-efficacy. Results of the pre-test and post-test summative scores revealed a statistically significant increase in student confidence in performing skills related to maternal and newborn care (p < .0001) following participation in the SCE. Confidence pre-test and post-test scores were not affected by the students’ section. Skills related to the care of the post-partum mother following vaginal or Caesarean section delivery showed the greatest change in confidence ratings. OSCE results showed a mean total class score (both sections) of 57.4 (70.0 %) with normal distribution. Mean scores were 56.5 (68.9%) for section 0001 and 58.3 (71.1%) for section 0002. Total scores were similar between sections (p =0.342) based on pairwise comparison. Analysis of OSCE scores as compared to students’ final course grade revealed similar distributions. Finally, qualitative analysis identified how students’ perceived the SCE. Students cited gains in knowledge, development of psychomotor skills and improved clinical judgement following participation in simulation activities. These were attributed to the « hands on » practice obtained from working in small groups, a safe and authentic learning environment and one in which students could make mistakes and correct errors as having the greatest impact on learning through simulation.

As perturbações psiquiátricas e os problemas de saúde mental infantojuvenil têm vindo a merecer especial atenção por parte da comunidade científica, não só pelo facto de se estimar que 10 a 20% das crianças apresentem um ou mais problemas de saúde mental, não só por estes valores terem tendência a aumentar, mas também pelo impacto humano e financeiro que acarretam para a sociedade. Em Portugal, o Plano Nacional da Saúde Mental 2007-2016 é categórico ao afirmar a necessidade de promover a saúde mental infantojuvenil junto da população e ao salientar a importância do envolvimento e articulação com outras estruturas comunitárias ligadas à saúde, educação e direito de menores. Este trabalho vai de encontro a estas linhas de orientação, na medida em que procura identificar, desenvolver e implementar diversos aspetos inerentes à vertente comunitária da saúde mental infantojuvenil. Numa primeira fase é realizado um levantamento das reais necessidades da população infantojuvenil do concelho de Odemira e é definida uma rede de articulação entre os serviços de saúde, escolas e outras entidades locais com competência nesta área. De forma a melhorar a prestação dos cuidados e a garantir respostas adequadas às necessidades identificadas, também se procurou o individual desenvolvimento de competências especializadas na área da saúde mental infantojuvenil. Por último, atendendo à importância da implementação atempada de medidas preventivas de determinados fenómenos psicopatológicos, procurou-se treinar e desenvolver a capacidade de sensibilização e reforço de competências de uma bolsa populacional específica: a comunidade escolar. De um modo geral, os resultados obtidos demonstram aplicabilidade na prática clínica dos cuidados, contribuindo para a melhoria dos mesmos. Algumas das estratégias utilizadas constituem-se como ponto de partida para projetos futuros, podendo ser replicadas e adequadas a outros contextos de intervenção; ABSTRACT: LINK HEALTH-SCHOOL: the intervention in the community context Psychiatric disorders and problems of youth mental health have been given special attention by the scientific community, not only because it is estimated that 10-20% of children present one or more mental health problems, not only because these values have tendency to increase, but also the human and financial impact to society. In Portugal, the Mental Health National Plan 2007-2016 is emphatic in affirming the need to promote the mental youth health within the population and to emphasize the importance of the involvement and coordination with other community entities related to health, education and minors rights. This paper follows these guidelines, it tries to identify, develop and implement various aspects related to the Community part of youth health. In the first phase is carried out a survey of the real needs of youth population of the Odemira County and defined a network between health services, schools and other local organizations with expertise in this area. In order to improve the delivery of health care and ensure appropriate responses to the identified needs, also sought the individual development of expertise in the area of youth mental health. Finally, given the value of timely implementation of preventive measures of certain psychopathological phenomena, it sought to train and develop the awareness and skills improvement of a specific population: the school community. In general, the results prove the applicability of the procedures in clinical practice, contributing to a practice improvement. Some of the strategies used are the Foundation for future projects, they can be replicated and adapted to other contexts of intervention.

The fundamental objective for health research is to determine whether changes should be made to clinical decisions. Decisions made by veterinary surgeons in the light of new research evidence are known to be influenced by their prior beliefs, especially their initial opinions about the plausibility of possible results. In this paper, clinical trial results for a bovine mastitis control plan were evaluated within a Bayesian context, to incorporate a community of prior distributions that represented a spectrum of clinical prior beliefs. The aim was to quantify the effect of veterinary surgeons’ initial viewpoints on the interpretation of the trial results. A Bayesian analysis was conducted using Markov chain Monte Carlo procedures. Stochastic models included a financial cost attributed to a change in clinical mastitis following implementation of the control plan. Prior distributions were incorporated that covered a realistic range of possible clinical viewpoints, including scepticism, enthusiasm and uncertainty. Posterior distributions revealed important differences in the financial gain that clinicians with different starting viewpoints would anticipate from the mastitis control plan, given the actual research results. For example, a severe sceptic would ascribe a probability of 0.50 for a return of <£5 per cow in an average herd that implemented the plan, whereas an enthusiast would ascribe this probability for a return of >£20 per cow. Simulations using increased trial sizes indicated that if the original study was four times as large, an initial sceptic would be more convinced about the efficacy of the control plan but would still anticipate less financial return than an initial enthusiast would anticipate after the original study. In conclusion, it is possible to estimate how clinicians’ prior beliefs influence their interpretation of research evidence. Further research on the extent to which different interpretations of evidence result in changes to clinical practice would be worthwhile.

Maladies fréquentes, l’asthme touche 8,4% de la population canadienne âgée de 12 ans et plus et la maladie pulmonaire obstructive chronique (MPOC) touche de 5 à 15% de la population âgée entre 35 et 79 ans. L’asthme et la MPOC peuvent coexister chez un patient. Ce phénomène appelé syndrome de chevauchement de l’asthme et de la MPOC (ACOS) toucherait environ 10% à 55% des patients MPOC. Afin de mieux caractériser l’ACOS et les effets indésirables des médicaments utilisés pour traiter la MPOC, deux études ont été mises en place. Une première étude réalisée auprès de pneumologues a permis de décrire les méthodes de diagnostic, de traitement et d’évaluation de la maitrise de l’ACOS dans la pratique clinique. Les pneumologues rapportent que le diagnostic devrait être basé sur les caractéristiques cliniques, les tests de fonction pulmonaire et l'intuition clinique du médecin. De plus, un corticostéroïde inhalé en combinaison et un bronchodilatateur inhalé à longue durée d’action devraient être introduits rapidement dans le plan de traitement. La deuxième étude a permis d’évaluer la fréquence des effets indésirables chez les patients MPOC/ACOS traités avec un bronchodilatateur inhalé à longue durée d’action. Cette étude démontre que les effets indésirables sont fréquents chez les patients MPOC/ACOS et que la sécheresse buccale et la gorge sèche sont les plus rapportés. Ces résultats démontrent que la mise en place de lignes directrices pour l’ACOS ainsi qu’une meilleure connaissance du profil de tolérance des bronchodilatateurs inhalés à longue durée d’action seraient bénéfiques pour les patients
