O presente relatório teve como base o estágio curricular no Hospital Veterinário Central, sob a orientação da Professora Cristina Queiroga, e coorientação do Dr. Nuno Paixão. Na primeira parte deste documento são relatadas as atividades desenvolvidas durante o estágio curricular, abrangendo várias áreas de especialidade clínica, em medicina de animais de companhia. A segunda parte do documento consiste numa revisão bibliográfica sobre transfusões sanguíneas, onde são abordados vários pontos, seguida da apresentação e discussão de um estudo sobre as transfusões sanguíneas realizadas no Hospital Veterinário Central, no período de 1 de janeiro de 2012 a 28 de fevereiro de 2013. O estudo tem como objetivos a avaliação da variação do hematócrito, frequência cardíaca e respiratória, coloração das mucosas, temperatura retal, pressões sanguíneas e atitude, em intervalos de tempo estipulados. Outro objetivo do presente estudo, é determinar a prevalência de reações transfusionais adversas e a taxa de sobrevivência dos animais submetidos a transfusão, e avaliar, através de testes estatísticos, se existe associação entre as várias variáveis em estudo; ### ABSTRACT: Blood transfusions in dogs and cats: indications and transfusion reactions This report is based on the internship accomplished in Hospital Veterinário Central, under the orientation of Dr. Cristina Queiroga and co-orientation of Dr. Nuno Paixão. The first part of this report describes the activities undertaken during a traineeship in several areas of clinical specialty, in small animal medicine. The second part of this document consists of a literature review on blood transfusions, which addresses several points, followed by a presentation and discussion of a study of blood transfusions performed at Hospital Veterinário Central, during the period of 1 January 2012 and 28 February 2013. The study aims at assessing the changes in hematocrit, heart and respiratory rate, mucous membrane coloration, retal temperature, blood pressures and attitude, at prescribed time intervals. Another objective of the present study is to determine the prevalence of adverse transfusion reactions and the survival rate of animals subjected to transfusion, and evaluate, through statistical tests, whether an association exists between the different variables under study.

Introdução: O envolvimento respiratório é a principal causa de morbilidade e mortalidade na Fibrose Quística (FQ). Dados pediátricos sobre atividade física (AF), saturação periférica da oxi-hemoglobina (SpO2) e pico do fluxo da tosse (PFT) são escassos e não padronizados. Objetivos: Avaliar a função pulmonar (FP), AF, SpO2 e PFT, em crianças e adolescentes com FQ, no estado basal e em agudização (AR) e, na fase estável, avaliar a correlação entre as variáveis. Métodos: Realizou-se um estudo observacional prospetivo, com análise de espirometria, podometria, oximetria noturna e PFT, em condições basais. Na AR reavaliaram-se os mesmos parâmetros às 24-48 horas, 7, 15 e 30 dias, excetuando a AF aos 7 dias. Resultados: Avaliaram-se 8 doentes dos quais dois apresentaram um comprometimento ligeiro da FP e um moderado. A SpO2 foi de 96,2% [95,6; 96,6] e o número médio de passos/dia (NMP) foi de 6369 [4431; 10588]. Todos apresentaram valores do PFT inferiores ao percentil 5 para o género e idade (265 L/min [210; 290]). Apesar de não estatisticamente significativa, a correlação foi moderada entre FEV1 e SpO2 nocturna (rs =0,61; p=0,11); entre PFT e idade (rs=0,69; p=0,06); e entre PFT e capacidade vital forçada (CVF) (rs=0,54; p=0,17). Não se verificou correlação entre FEV1 e idade, NMP e PFT; e entre NMP e idade. No único caso de AR, à exceção da frequência respiratória, verificou-se a diminuição das variáveis às 24-48h; após 1 mês, a maioria das variáveis aproximou-se ou igualou os valores basais. Conclusão: Os resultados sugerem uma tendência para melhores valores de FEV1 corresponderem a melhores SpO2 noturnas e que, quanto maior a idade e a CVF, maior é o PFT. Não foi possível avaliar o impacto da AR por ter ocorrido apenas um caso.

Introduction. Selective embolization of the left-gastric artery (LGA) reduces levels of ghrelin and achieves significant short-term weight loss. However, embolization of the LGA would prevent the performance of bariatric procedures because the high-risk leakage area (gastroesophageal junction [GEJ]) would be devascularized. Aim. To assess an alternative vascular approach to the modulation of ghrelin levels and generate a blood flow manipulation, consequently increasing the vascular supply to the GEJ. Materials and methods. A total of 6 pigs underwent a laparoscopic clipping of the left gastroepiploic artery. Preoperative and postoperative CT angiographies were performed. Ghrelin levels were assessed perioperatively and then once per week for 3 weeks. Reactive oxygen species (ROS; expressed as ROS/mg of dry weight [DW]), mitochondria respiratory rate, and capillary lactates were assessed before and 1 hour after clipping (T0 and T1) and after 3 weeks of survival (T2), on seromuscular biopsies. A celiac trunk angiography was performed at 3 weeks. Results. Mean (±standard deviation) ghrelin levels were significantly reduced 1 hour after clipping (1902 ± 307.8 pg/mL vs 1084 ± 680.0; P = .04) and at 3 weeks (954.5 ± 473.2 pg/mL; P = .01). Mean ROS levels were statistically significantly decreased at the cardia at T2 when compared with T0 (0.018 ± 0.006 mg/DW vs 0.02957 ± 0.0096 mg/DW; P = .01) and T1 (0.0376 ± 0.008mg/DW; P = .007). Capillary lactates were significantly decreased after 3 weeks, and the mitochondria respiratory rate remained constant over time at the cardia and pylorus, showing significant regional differences. Conclusions. Manipulation of the gastric flow targeting the gastroepiploic arcade induces ghrelin reduction. An endovascular approach is currently under evaluation.

IMPORTANCE: The clinical benefit of adding a macrolide to a β-lactam for empirical treatment of moderately severe community-acquired pneumonia remains controversial. OBJECTIVE: To test noninferiority of a β-lactam alone compared with a β-lactam and macrolide combination in moderately severe community-acquired pneumonia. DESIGN, SETTING, AND PARTICIPANTS: Open-label, multicenter, noninferiority, randomized trial conducted from January 13, 2009, through January 31, 2013, in 580 immunocompetent adult patients hospitalized in 6 acute care hospitals in Switzerland for moderately severe community-acquired pneumonia. Follow-up extended to 90 days. Outcome assessors were masked to treatment allocation. INTERVENTIONS: Patients were treated with a β-lactam and a macrolide (combination arm) or with a β-lactam alone (monotherapy arm). Legionella pneumophila infection was systematically searched and treated by addition of a macrolide to the monotherapy arm. MAIN OUTCOMES AND MEASURES: Proportion of patients not reaching clinical stability (heart rate <100/min, systolic blood pressure >90 mm Hg, temperature <38.0°C, respiratory rate <24/min, and oxygen saturation >90% on room air) at day 7. RESULTS: After 7 days of treatment, 120 of 291 patients (41.2%) in the monotherapy arm vs 97 of 289 (33.6%) in the combination arm had not reached clinical stability (7.6% difference, P = .07). The upper limit of the 1-sided 90% CI was 13.0%, exceeding the predefined noninferiority boundary of 8%. Patients infected with atypical pathogens (hazard ratio [HR], 0.33; 95% CI, 0.13-0.85) or with Pneumonia Severity Index (PSI) category IV pneumonia (HR, 0.81; 95% CI, 0.59-1.10) were less likely to reach clinical stability with monotherapy, whereas patients not infected with atypical pathogens (HR, 0.99; 95% CI, 0.80-1.22) or with PSI category I to III pneumonia (HR, 1.06; 95% CI, 0.82-1.36) had equivalent outcomes in the 2 arms. There were more 30-day readmissions in the monotherapy arm (7.9% vs 3.1%, P = .01). Mortality, intensive care unit admission, complications, length of stay, and recurrence of pneumonia within 90 days did not differ between the 2 arms. CONCLUSIONS AND RELEVANCE: We did not find noninferiority of β-lactam monotherapy in patients hospitalized for moderately severe community-acquired pneumonia. Chez les animaux de laboratoire, même si les anesthésiques par inhalation sont généralement plus sécuritaires que les injectables, leur utilité est souvent restreinte lorsqu’un protocole expérimental exige une autre approche. Des combinaisons d’anesthésiques contenant de la kétamine sont considérées comme l’option de choix pour les anesthésies injectables chez les rats. Le vieillissement entraîne des changements dégénératifs au niveau de la structure et la fonction des organes, modifiant souvent à la pharmacocinétique des drogues. Ce projet porte sur l’évaluation des changements pharmacodynamiques (physiologiques, biochimiques et histologiques) et pharmacocinétiques, lors d’une combinaison anesthésique de kétamine-­‐xylazine chez le rat Sprague-­‐Dawley vieillissant. Une anesthésie à la kétamine-­‐xylazine fut induite chez des rats Sprague-­‐Dawley de différents âges. Afin d’évaluer l’effet du vieillissement sur le métabolisme des deux drogues, des prélèvements sanguins périodiques pour l’analyse de la pharmacocinétique ainsi que des mesures des paramètres physiologiques, biochimiques et une histopathologie furent effectués. Le vieillissement a causé certaines modifications notamment en produisant une diminution de la saturation d’oxygène, une baisse marquée de la fréquence cardiaque et respiratoire, une hypoalbuminémie ainsi qu’une augmentation de la durée d’anesthésie. Les paramètres pharmacocinétiques de la kétamine et de la xylazine furent grandement affectés par le vieillissement causant une augmentation progressive significative de l’aire sous la courbe (AUC) et du temps de demi-­‐vie, ainsi qu’une diminution de la clairance. À la lumière de ces résultats, les doses de kétamine et de xylazine doivent être adaptées chez les rats vieillissants pour permettre une anesthésie de durée raisonnable et un réveil sans complications.

Paciente de sexo femenino, de 59 años, educadora; llevada a institución hospitalaria por alteración súbita del estado de conciencia. Antecedente de hipotiroidismo en manejo con levotiroxina, 50 microgramos al día. Al examen físico de ingreso se encontraba en mal estado general, FC: 88 x min., TA 170/110, FR: 8 x min., temp.: 35 Cº, Glasgow: 6/15; estuporosa, con apertura ocular al estímulo doloroso, sin respuesta verbal, movimiento de retirada en hemicuerpo izquierdo al estímulo doloroso. Plejia de miembro superior derecho, paresia 2/5 en miembro inferior derecho y Babinski bilateral. Las imágenes diagnósticas confirmaron evento cerebrovascular tipo hemorragia intraparenquimatosa, de extensión parietotemporal e insular del hemisferio cerebral izquierdo. Se le realizó cirugía de drenaje de hematoma intraparenquimatoso e intraventricular, sin complicaciones. En el primer día postoperatorio la paciente cursó con hipokalemia, que persistió a pesar de reposición de potasio y presentó acidemia metabólica sin causa aparente que empeoró a pesar de manejo médico.

This work investigated the personal exposure to indoor particulate matters using the intake fraction metric and provided a possible way to trace the particle inhaled from an indoor particle source. A turbulence model validated by the particle measurements in a room with underfloor air distribution (UFAD) system was used to predict the indoor particle concentrations. Inhalation intake fraction of indoor particles was defined and evaluated in two rooms equipped with the UFAD, i.e., the experimental room and a small office. According to the exposure characteristics and a typical respiratory rate, the intake fraction was determined in two rooms with a continuous and episodic (human cough) source of particles, respectively. The findings showed that the well-mixing assumption of indoor air failed to give an accurate estimation of inhalation exposure and the average concentration at return outlet or within the overall room could not relate well the intake fraction to the amount of particle emitted from an indoor source.

It is known that slow breathing (<10 breaths min(-1)) reduces blood pressure ( BP), but the mechanisms involved in this phenomenon are not completely clear. The aim of this study was to evaluate the acute responses of the muscle sympathetic nerve activity, BP and heart rate (HR), using device-guided slow breathing ( breathe with interactive music (BIM)) or calm music. In all, 27 treated mild hypertensives were enrolled. Muscle sympathetic nerve activity, BP and HR were measured for 5min before the use of the device (n=14) or while subjects listened to calm music (n=13), it was measured again for 15 min while in use and finally, 5min after the interventions. BIM device reduced respiratory rate from 16 +/- 3 beats per minute (b.p.m) to 5.5 +/- 1.8 b.p.m (P<0.05), calm music did not affect this variable. Both interventions reduced systolic (-6 and -4mmHg for both) and diastolic BPs (-4mmHg and -3mmHg, respectively) and did not affect the HR (-1 and -2 b.p.m respectively). Only the BIM device reduced the sympathetic nerve activity of the sample (-8bursts min(-1)). In conclusion, both device-guided slow breathing and listening to calm music have decreased BP but only the device-guided slow breathing was able to reduce the peripheral sympathetic nerve activity. This study evaluated the analgesia effects of the epidural administration of 0.1 mg/kg bodyweight (BW) of morphine or 5 mu g/kg BW of buprenorphine in ponies with radiocarpal joint synovitis. Six ponies were submitted to 3 epidural treatments: the control group (C) received 0.15 mL/kg BW of a 0.9% sodium chloride (NaCl) solution; group M was administered 0.1 mg/kg BW of morphine; and group B was administered 5 mu g/kg BW of buprenorphine, both diluted in 0.9% NaCl to a total volume of 0.15 mL/kg BW administered epidurally at 10 s/mL. The synovitis model was induced by injecting 0.5 ng of lipopolysaccharide (LPS) in the left or right radiocarpal joint. An epidural catheter was later introduced in the lumbosacral space and advanced up to the thoracolumbar level. The treatment started 6 h after synovitis induction. Lameness, maximum angle of carpal flexion, heart rate, systolic arterial pressure, respiratory rate, temperature, and intestinal motility were evaluated before LPS injection (baseline), 6 h after LPS injection (time 0), and 0.5, 1, 2, 4, 6, 8, 10, 12, 16, 20, and 24 h after treatments. Although the model of synovitis produced clear clinical signs of inflammation, the lameness scores in group C were different from the baseline for only up to 12 h. Both morphine and buprenorphine showed a reduction in the degree of lameness starting at 0.5 and 6 h, respectively. Reduced intestinal motility was observed at 0.5 h in group M and at 0.5 to 1 h in group B. Epidural morphine was a more effective analgesic that lasted for more than 12 h and without side effects. It was concluded that morphine would be a valuable analgesic option to alleviate joint pain in the thoracic limbs in ponies.

Objetivo: Avaliar a eficácia da fisioterapia respiratória como tratamento adjuvante em pacientes pediátricos com pneumonia adquirida na comunidade. Delineamento: Ensaio clínico randomizado Local do estudo: Hospital da Criança Santo Antônio – Complexo Hospitalar Santa Casa, Porto Alegre, Brasil. Participantes e métodos: Foram arroladas crianças com idade entre 1 e 12 anos, com diagnóstico clínico e radiológico confirmado de pneumonia, hospitalizadas no período de setembro de 2001 a setembro de 2002. Os pacientes que preencheram os critérios de inclusão foram randomizados para receber fisioterapia respiratória três vezes ao dia (grupo intervenção) ou para receber, uma vez ao dia, orientações para respirar profundamente, expectorar a secreção e manter preferencialmente o decúbito lateral (grupo controle). As variáveis analisadas na linha de base, no primeiro e no segundo períodos de seguimento e no dia da alta hospitalar foram: escore de gravidade (composto pela freqüência respiratória anormal para a idade, tiragem supra-esternal, intercostal, e subcostal, febre, saturação de oxigênio da hemoglobina e raio-x de tórax), duração da hospitalização, freqüência respiratória, temperatura e saturação do oxigênio. Resultados: Setenta e dois pacientes foram randomizados para os grupos intervenção ou controle. Destes, sete foram retirados devido a complicações como atelectasia ou drenagem pleural. Dentre os 65 pacientes estudados no primeiro seguimento (terceiro dia), a febre foi mais prevalente no grupo intervenção (34,4%) do que no grupo controle (12,5%), bem como o escore de gravidade 9,63 ± 1,62 e 8,71 ± 0,86 pontos, respectivamente. No segundo seguimento, entre o quarto e sexto dia, a diferença entre os grupos teve tendência à significância apenas para febre, 31,6% no grupo intervenção e 6,7% no grupo controle (P= 0,07). A duração média da hospitalização foi de 7,41 ± 6,58 dias para o grupo intervenção e 4,52 ± 2,21 dias para o controle. Conclusão: Neste ensaio clínico, a fisioterapia prolongou a hospitalização e a duração da febre nos pacientes pediátricos com pneumonia adquirida na comunidade. Nestes pacientes, a fisioterapia é prejudicial e não deveria ser prescrita até que evidências de benefício estejam disponíveis.

Introdução: O uso da pressão expiratória positiva na via aérea (EPAP) não é sugerido como técnica de desmame. O EPAP pode previnir o colapso das vias aéreas durante a expiração. Objetivo: O objetivo deste trabalho é verificar se a utilização da pressão expiratória positiva na via aérea (EPAP) apresenta benefícios na redução da hiperinsuflação dinâmica com redução do trabalho respiratório e melhora da oxigenação em pacientes submetidos ao processo de desmame da ventilação mecânica. Material e Métodos: Quarenta pacientes submetidos à ventilação mecânica por um período maior que 48 horas em 2 unidades de terapia intensiva foram avaliados prospectivamente em um estudo randomizado controlado cruzado. Todos os pacientes foram submetidos métodos de ventilação de pressão de suporte (PSV), tubo-t e EPAP, durante 30 minutos, com um período de descanso de 30 minutos entre cada método. Os pacientes foram monitorizados pelo VenTrack (Novametrix, EUA). As variáveis estudadas, mensuradas no minuto 1, 15 e 30, foram: PEEP intínseca (PEEPi), trabalho respiratório (WOBtotal), frequência respiratória (f), volume de ar corrente (Vt) e saturação periférica de oxigênio (SaO2). A amostra geral foi analisada e dividida em subgrupos DPOC (n= 14) e não-DPOC (n=26), traqueostomizados (n=15) e não-traqueostomizados (n=25). As comparações foram feitas pela Análise de Variância (ANOVA) e teste-t. O nível de significância foi de 95%. Resultados: PEEPi DPOC e não-DPOC minuto 1 (0,014 + 0,03 versus 0,17 + 0,38 cmH2O) e minuto 15 (0,042 + 0,13 versus 0,41 + 0,78 cmH2O) (p<0,05). No subgrupo não-traqueo, nos métodos de PSV15 (0,26 + 0,5 cm H2O) e EPAP15 (0,02 + 0,07 cm H2O), assim como PSV 30 (0,21 + 0,4 cm H2O) e EPAP 30 (0,02 + 0,1 cm H2O) (p<0,05). Para traqueo vs não-traqueo, no método EPAP minuto 1 (PEEPi traqueo 0,58 + 0,94 cm H2O; PEEPi não-traqueo 0,08 + 0,28 cmH2O) e minuto 15 (PEEPi traqueo 0,91 + 2,06 cm H2O; PEEPi não-traqueo 0,02 + 0,07 cmH2O) (p<0,05). Em relação ao WOBtotal houve um aumento significativo no método EPAP em relação ao tubo-t na análise geral da amostra (p<0,05). A f mostrou-se maior no método EPAP para o subgrupo não-DPOC e não-traqueo (minutos 1, 15 e 30). A SaO2 foi maior no subgrupo PSV quando comparada com tubo-t na análise geral da amostra, (p<0,05) Conclusões: A EPAP não demonstrou redução na PEEPi na análise geral da amostra, subgrupo DPOC, não-DPOC e traqueostomizados. Houve redução na PEEPi no grupo não-traqueostomizados. Houve aumento do WOBtotal com o uso da EPAP. Neste estudo a EPAP não demonstrou vantagens em relação aos outros métodos.

The objective of this work was to evaluate the effect of environmental variables on dairy buffaloes physiology in two different places after milking, shaded plus artificial ventilation and another one non-shaded, in Ribeira Valley, São Paulo State, Brazil. Data on the respiratory rate (RR) and the surface temperature (ST) at udder, neckmiddle, forehead, back middle and rump were collected in 12 dairy buffaloes at autumn. In the same way, it were recorded the black globe temperature in the sun (GTS) and in the shade (GTNS), air temperature and wind speed at padronized height of 1.60 meters. All data were collected at 10:30am and 1:30pm. The results showed statistical difference among black globe temperature, wind speed, RR and ST (P<0.01) in two treatments.The results showed the necessity of protection against the solar radiation in the buffaloes, even in periods of warm climates.

Objective: To determine the cardiovascular effects of desflurane in dogs following acute hemorrhage.Design: Experimental study.Animals: Eight mix breed dogs.Interventions: Hemorrhage was induced by withdrawal of blood until mean arterial pressure (MAP) dropped to 60 mmHg in conscious dogs. Blood pressure was maintained at 60 mmHg for 1 hour by further removal or replacement of blood. Desflurane was delivered by facemask until endotracheal intubation could be performed and a desflurane expiratory end-tidal concentration of 10.5 V% was maintained.Measurements and main results: Systolic, diastolic, and mean arterial blood pressure (SAP, DAP and MAP), central venous pressure (CVP), cardiac output (CO), stroke volume (SV), cardiac index (0), systemic vascular resistance (SVR), heart rate (HR), respiratory rate (RR), partial pressure of carbon dioxide in arterial blood (PaCO2), and arterial pH were recorded before and 60 minutes after hemorrhage, and 5, 15, 30, 45 and 60 minutes after intubation. Sixty minutes after hemorrhage, SAP, DAP, MAP, CVP, CO, Cl, SV, PaCO2, and arterial pH decreased, and HR and RR increased when compared with baselines values. Immediately after intubation, MAP and arterial pH decreased, and PaCO2 increased. Fifteen minutes after intubation SAP, DAP, MAP, arterial pH, and SVR decreased. At 30 and 45 minutes, MAP and DAP remained decreased and PaCO2 increased, compared with values measured after hemorrhage. Arterial pH increased after 30 minutes of desflurane administration compared with values measured 5 minutes after intubation.Conclusions: Desflurane induced significant changes in blood pressure and arterial pH when administered to dogs following acute hemorrhage.
