Nombreux individus vont expérimenter avec les drogues d’abus, mais peu vont devenir toxicomanes. Plusieurs facteurs sont impliqués dans la transition d’un usage récréatif à l’addiction. Les drogues, les conditionnements et les voies d’administration qui mènent à l’augmentation rapide du taux drogue dans le cerveau favorisent cette évolution. La raison est méconnue. Nous avons émis l’hypothèse que l’injection rapide de drogue promeut des changements dans le cerveau qui mènent à l’augmentation de la consommation et de la motivation à obtenir la drogue. Nous avons comparé la consommation lors de conditions à ratio fixe (FR) et à ratio progressif (PR) chez des rats s’auto-administrant la cocaïne administrée par voie intraveineuse (i.v.) en 5 ou 90 secondes (s). Tous les rats ont été entrainés à peser sur un levier afin de s’auto administrer des injections de cocaïne de 0.25 ou 0.5 mg/kg par voie intraveineuse injectée en 5 s sous FR avant d’être divisés en groupes s’auto administrant la cocaïne injectée en 5 ou 90 s pendant 1 heure (h)/session. Pour étudier les différences potentielles en consommation, l’accès à la cocaïne à été augmenté à 6 h/session. Les différences en motivation ont été détectées par l’auto administration de la cocaïne sous PR en fonction de la dose et de la vitesse d’infusion. L’accès à la drogue pendant 1 h/session n’a pas influencé la consommation. Lorsque l’accès a été prolongé à 6 h, tous les animaux ont augmenté leur consommation, mais l’augmentation était plus prononcée chez les rats s’injectant la cocaïne en 5 s. De plus, la vitesse d’injection a influencé la motivation pour obtenir la drogue. Lors de conditions à PR, la courbe dose-réponse pour le nombre d’infusions prises a été déplacée vers le haut pour les rats s’auto administrant des injections de cocaïne en 5 s versus 90 s. De plus, des différences qualitatives on été observées en PR. La consommation de cocaïne des rats s’injectant des infusions en 5 s était dépendante de la dose, tandis que les rats s’auto administrant la drogue en 90 s ont pris la même quantité de drogue, peu importe la dose. Finalement, les rats s’auto administrant des infusions de cocaïne 0.5 mg/kg en 5 s ont consommé plus de cocaïne que les rats prenant des infusions en 90 s, peu importe si elle était injectée en 5 ou 90 s le jour du test. Ainsi, nos résultats montrent que l’injection rapide de drogue dans le cerveau mène à l’augmentation de la consommation et de la motivation pour obtenir la cocaïne, deux symptômes qui caractérisent la toxicomanie.

Au cours de la grossesse et dans les premières années de vie de l’enfant, plusieurs facteurs externes, tels que le stress maternel ou le lien maternel, peuvent interférer avec son développement et avoir des conséquences à court et à long terme. Bien que le stress maternel périnatal ait souvent été étudié, plusieurs facteurs pré et postnataux potentiellement confondants (comme le stress paternel, la dépression, la prise d’antidépresseurs, etc.) n’ont pas systématiquement été pris en compte. Comparativement à l’attachement de l’enfant (à sa mère), moins de recherches se sont intéressées au lien maternel, en particulier chez des femmes ayant un diagnostic de dépression et/ou d’anxiété et exposées à des antidépresseurs. Notre thèse visait donc à étudier ces deux facteurs et leur association avec le développement du jeune enfant âgé d’un an, dans un contexte où la santé mentale maternelle (présence ou non de troubles psychologiques) et de traitements pharmacologiques associés étaient aussi considérés. Afin d’améliorer le dépistage de retards développementaux, nous avons également évalué l’administration au téléphone de deux instruments de pré-dépistage et dépistage actuellement utilisés. Notre thèse était constituée de quatre phases dont les objectifs étaient: la détermination des propriétés psychométriques de l’échelle de stress perçu de quatre items (en français et en anglais) au sein de la population de femmes enceintes (première étude); évaluer l’effet du stress maternel prénatal et parental postnatal sur le développement de l’enfant âgé d’un an (deuxième étude); estimer l’association entre le lien maternel et le développement de l’enfant âgé d’un an, en stratifiant sur la prise maternelle d’antidépresseurs (troisième étude); et évaluer l’administration au téléphone du ″Ages and Stages Questionnaire″ (ASQ, version de 12 mois) et du ″Revised Pre-screening Denver Questionnaire″(R-PDQ, version de 9-24 mois),deux questionnaires utilisés pour le pré-dépistage et le dépistage du développement infantile (quatrième étude). Ce projet faisait partie d’une plus grande étude, nommée le projet OTIS (The″Organization of Teratology Information Specialists″ Antidepressant in Pregnancy cohort study), qui visait à évaluer l’impact de l’arrêt versus la continuation de la prise d’antidépresseurs au cours de la grossesse sur les comportements maternels et sur le développement de l’enfant âgé de un à trois ans. Ainsi, entre 2006 et 2010, la cohorte prospective OTIS de femmes enceintes a été construite et suivie jusqu’à trois ans postpartum. À partir de 2008, l’évaluation du stress et du lien maternel (thématiques de ce projet de thèse) a été incorporée au projet initial. Aussi, à partir de là, toute femme nouvellement recrutée pour la cohorte OTIS pouvait faire partie des études de cette thèse. Notre population source était composée des femmes enceintes ayant eu recours à un service nord-américain d’information sur les tératogènes ou ayant été suivies à la clinique d'obstétrique et de gynécologie du CHU Ste Justine. Pour être admissibles, les femmes devaient 1) être âgées d’au moins 18 ans, 2) être enceinte d’au maximum 14 semaines (le début de la grossesse étant défini comme étant le premier jour des dernières règles); 3) être exposées à un antidépresseur depuis au moins le premier jour de grossesse (pour le groupe exposé à un antidépresseur), 4) savoir lire et comprendre l’anglais ou le français. Pour les études deux, trois et quatre, les femmes devaient habiter dans un rayon de 250 km autour de Montréal pour recevoir notre visite à domicile, nécessaire à l’évaluation psychométrique du développement infantile. Les pères des enfants ont été recrutés à deux mois postpartum pour évaluer leur stress. Les données ont été collectées par entrevue téléphonique, auto-administration de questionnaires, et évaluation psychométrique du développement infantile. Dans le cadre de cette thèse, les femmes étaient suivies depuis le premier trimestre de grossesse jusqu’à un an postpartum. Les résultats de nos travaux démontrent que le recours à l’échelle de stress perçu de quatre items est une mesure fiable et valide pour mesurer le stress au cours de la grossesse (en recherche ou en clinique). Ensuite, le stress postnatal maternel et paternel serait défavorable au développement moteur et socio-émotionnel de l’enfant d’un an, respectivement. Par contre le stress maternel prénatal favoriserait le développement moteur. Un lien maternel optimal est associé à un meilleur développement socio-émotionnel à un an chez l’enfant, en particulier pour les femmes exposées à des antidépresseurs. Enfin, l’administration au téléphone de l’ASQ et des sous-échelles de langage, de motricité fine et motricité globale du R-PDQ serait une alternative possible au mode auto-administré pour dépister et pré-dépister les retards de développement chez les enfants.

Dopamine D2 receptors are involved in ethanol self- administration behavior and also suggested to mediate the onset and offset of ethanol drinking. In the present study, we investigated dopamine (DA) content and Dopamine D2 (DA D2) receptors in the hypothalamus and corpus striatum of ethanol treated rats and aldehyde dehydrogenase (ALDH) activity in the liver and plasma of ethanol treated rats and in vitro hepatocyte cultures. Hypothalamic and corpus striatal DA content decreased significantly (P\0.05, P\0.001 respectively) and homovanillic acid/ dopamine (HVA/DA) ratio increased significantly (P\0.001) in ethanol treated rats when compared to control. Scatchard analysis of [3H] YM-09151-2 binding to DA D2 receptors in hypothalamus showed a significant increase (P\0.001) in Bmax without any change in Kd in ethanol treated rats compared to control. The Kd of DA D2 receptors significantly decreased (P\0.05) in the corpus striatum of ethanol treated rats when compared to control. DA D2 receptor affinity in the hypothalamus and corpus striatum of control and ethanol treated rats fitted to a single site model with unity as Hill slope value. The in vitro studies on hepatocyte cultures showed that 10-5 M and 10-7 M DA can reverse the increased ALDH activity in 10% ethanol treated cells to near control level. Sulpiride, an antagonist of DA D2, reversed the effect of dopamine on 10% ethanol induced ALDH activity in hepatocytes. Our results showed a decreased dopamine concentration with enhanced DA D2 receptors in the hypothalamus and corpus striatum of ethanol treated rats. Also, increased ALDH was observed in the plasma and liver of ethanol treated rats and in vitro hepatocyte cultures with 10% ethanol as a compensatory mechanism for increased aldehyde production due to increased dopamine metabolism. A decrease in dopamine concentration in major brain regions is coupled with an increase in ALDH activity in liver and plasma, which contributes to the tendency for alcoholism. Since the administration of 10-5 M and 10-7 M DA can reverse the increased ALDH activity in ethanol treated cells to near control level, this has therapeutic application to correct ethanol addicts from addiction due to allergic reaction observed in aldehyde accumulation.

Livestock production contributes substantially to the livelihoods of poor rural farmers in Pakistan; strengthening pastoral communities plays an imperative role in the country’s thrive for poverty alleviation. Intestinal helminths constitute a major threat for pastoral livestock keepers in the whole country because chronic infestation leads to distinct losses in livestock productivity, particularly the growth of young animals. Synthetic anthelmintics have long been considered the only effective way of controlling this problem but high prices, side effects and chemical residues/toxicity problems, or development of resistance, lead to their very limited use in many pastoral systems. Additionally, poor pastoralists in remote areas of Pakistan hardly have access to appropriate anthelmintic drugs, which are also relatively expensive due to the long routes of transportation. The search for new and more sustainable ways of supporting livestock keepers in remote areas has given rise to studies of ethno-botanicals or traditional plant-based remedies to be used in livestock health care. Plant-based remedies are cheap or free of cost, environmentally safe and generally create no problem of drug resistance; they thus might substitute allopathic drugs. Furthermore, these remedies are easily available in remote areas and simple to prepare and/or administer. Cholistan desert is a quite poor region of Pakistan and the majority of its inhabitants are practicing a nomadic life. The region’s total livestock population (1.29 million heads) is almost twice that of the human population. Livestock husbandry is the primordial occupation of the communities and traditionally wealth assessment was based on the number of animals, especially goats and sheep, owned by an individual. Fortunately, about 60% of this desert region is richly endowed with highly adapted grasses, shrubs and trees. This natural flora has a rich heritage of scientifically unexplored botanical pharmacopoeia. Against this background, the present research project that was conducted under the umbrella of the International Center for Development and Decent Work at Kassel University, focused on a development aspect: in the Cholistan desert region it was firstly examined how pastoralists manage their livestock, which major health problems they face for the different animal species, and which of the naturally occurring plants they use for the treatment of animal diseases (Chapter 2). For this purpose, a baseline survey was carried out across five locations in Cholistan, using a structured questionnaire to collect data from 100 livestock farmers (LF) and 20 local healers (LH). Most of LF and LH were illiterate (66%; 70%). On average, LH had larger herds (109 animals) than LF (85 animals) and were more experienced in livestock husbandry and management. On average LF spent about 163 Euro per year on the treatment of their livestock, with a huge variability in expenditures. Eighty-six traditional remedies based on 64 plants belonging to 43 families were used. Capparaceae was the botanical family with the largest number of species used (4), followed by Chenopodiaceae, Poaceae, Solanaceae and Zygophyllaceae (3). The plants Capparis decidua (n=55 mentions), Salsola foetida (n=52), Suaeda fruticosa (n=46), Haloxylon salicornicum (n=42) and Haloxylon recurvum (n=39) were said to be most effective against the infestations with gastrointestinal parasites. Aerial parts (43%), leaves (26%), fruits (9%), seeds and seed oils (9%) were the plant parts frequently used for preparation of remedies, while flowers, roots, bulbs and pods were less frequently used (<5%). Common preparations were decoction, jaggery and ball drench; oral drug administration was very common. There was some variation in the doses used for different animal species depending on age, size and physical condition of the animal and severity of the disease. In a second step the regionally most prevalent gastrointestinal parasites of sheep and goats were determined (Chapter 3) in 500 animals per species randomly chosen from pastoral herds across the previously studied five localities. Standard parasitological techniques were applied to identify the parasites in faecal samples manually collected at the rectum. Overall helminth prevalence was 78.1% across the 1000 animals; pure nematode infestations were most prevalent (37.5%), followed by pure trematode (7.9%), pure cestode (2.6%) and pure protozoa infestations (0.8%). Mixed infestations with nematodes and trematodes occurred in 6.4% of all animals, mixed nematode-cestode infestations in 3.8%, and all three groups were found in 19.1% of the sheep and goats. In goats more males (81.1%) than females (77.0%) were infested, the opposite was found in sheep (73.6% males, 79.5% females). Parasites were especially prevalent in suckling goats (85.2%) and sheep (88.5%) and to a lesser extent in young (goats 80.6%, sheep 79.3%) and adult animals (goats 72.8%, sheep 73.8%). Haemonchus contortus, Trichuris ovis and Paramphistomum cervi were the most prevalent helminths. In a third step the in vitro anthelmintic activity of C. decidua, S. foetida, S. fruticosa, H. salicornicum and H. recurvum (Chapter 2) was investigated against adult worms of H. contortus, T. ovis and P. cervi (Chapter 3) via adult motility assay (Chapter 4). Various concentrations ranging from 7.8 to 500 mg dry matter/ml of three types of extracts of each plant, i.e. aqueous, methanol, and aqueous-methanol (30:70), were used at different time intervals to access their anthelmintic activity. Levamisol (0.55 mg/ml) and oxyclozanide (30 mg/ml) served as positive and phosphate-buffered saline as negative control. All extracts exhibited minimum and maximum activity at 2 h and 12 h after parasite exposure; the 500 mg/ml extract concentrations were most effective. Plant species (P<0.05), extract type (P<0.01), parasite species (P<0.01), extract concentration (P<0.01), time of exposure (P<0.01) and their interactions (P<0.01) had significant effects on the number of immobile/dead helminths. From the comparison of LC50 values it appeared that the aqueous extract of C. decidua was more potent against H. contortus and T. ovis, while the aqueous extract of S. foetida was effective against P. cervi. The methanol extracts of H. recurvum were most potent against all three types of parasites, and its aqueous-methanol extract was also very effective against T. ovis and P. cervi. Based on these result it is concluded that the aqueous extract of C. decidua, as well as the methanol and aqueous-methanol extract of H. recurvum have the potential to be developed into plant-based drugs for treatment against H. contortus, T. ovis and P. cervi infestations. Further studies are now needed to investigate the in vivo anthelmintic activity of these plants and plant extracts, respectively, in order to develop effective, cheap and locally available anthelmintics for pastoralists in Cholistan and neighboring desert regions. This will allow developing tangible recommendations for plant-based anthelminthic treatment of sheep and goat herds, and by this enable pastoralists to maintain healthy and productive flocks at low costs and probably even manufacture herbal drugs for marketing on a regional scale.

Drugs that alter brain serotonin (5-HT) function can modulate the behavioral effects of cocaine, but the underlying receptor mechanisms are poorly understood. The present study examined the effects of the selective 5-HT1A receptor agonist (+/-)-8-hydroxy-2-(di-n-propylamino)tetralin (8-OH-DPAT, 0.01-0.1 mg/kg, i.v.) on cocaine self-administration in the context of a choice procedure. Five adult male cynomolgus monkeys self-administered cocaine (saline, 0.003-0.03 mg/kg per injection) under a concurrent fixed-ratio 50 schedule of food (1-g banana-flavored pellets) and cocaine presentation. Allocation of responses to the cocaine-associated lever (cocaine choice) increased in a dose-related manner from < or =20% of total responses when saline or 0.003 mg/kg per injection cocaine was the alternative to food to > or =75% when 0.03 mg/kg per injection cocaine was available. In four of five monkeys, when choice was between a low cocaine dose and food, 0.01 mg/kg 8-OH-DPAT increased injection-lever responding. At cocaine doses which occasioned > or =75% cocaine choice, 8-OH-DPAT did not alter response allocation. In the fifth monkey, 8-OH-DPAT only decreased injection-lever responding. When choice was between saline and food, 8-OH-DPAT did not reliably shift responding to the injection lever, except at doses that disrupted operant performance. These results suggest that a 5-HT1A receptor agonist can increase the reinforcing strength of a low cocaine dose relative to a concurrently available non-drug reinforcer.

Position in the social hierarchy can influence brain dopamine function and cocaine reinforcement in nonhuman primates during early cocaine exposure. With prolonged exposure, however, initial differences in rates of cocaine self-administration between dominant and subordinate monkeys dissipate. The present studies used a choice procedure to assess the relative reinforcing strength of cocaine in group-housed male cynomolgus monkeys with extensive cocaine self-administration histories. Responding was maintained under a concurrent fixed-ratio 50 schedule of food and cocaine (0.003-0.1 mg/kg per injection) presentation. Responding on the cocaine-associated lever increased as a function of cocaine dose in all monkeys. Although response distribution was similar across social rank when saline or relatively low or high cocaine doses were the alternative to food, planned t tests indicated that cocaine choice was significantly greater in subordinate monkeys when choice was between an intermediate dose (0.01 mg/kg) and food. When a between-session progressive-ratio procedure was used to increase response requirements for the preferred reinforcer (either cocaine or food), choice of that reinforcer decreased in all monkeys. The average response requirement that produced a shift in response allocation from the cocaine-associated lever to the food-associated lever was higher in subordinates across cocaine doses, an effect that trended toward significance (p = 0.053). These data indicate that despite an extensive history of cocaine self-administration, most subordinate monkeys were more sensitive to the relative reinforcing strength of cocaine than dominant monkeys.

A maioria dos estudos pré-clínicos e clínicos aponta a nicotina como o principal agente responsável pelo desenvolvimento da dependência ao tabaco. Muitos trabalhos têm demonstrado que as bases neurais da dependência à nicotina são semelhantes àquelas das outras drogas de abuso. A nicotina induz preferência condicionada por lugar e auto-administração e, portanto, atua como reforçador positivo, esse efeito parece ser mediado pelo sistema dopaminérgico mesolímbico. A nicotina também induz à sensibilização comportamental que é provavelmente resultante de alterações da expressão gênica do núcleo acumbens induzidas pela exposição prolongada a essa substância. A suspensão do uso de nicotina resulta em síndrome de abstinência. As evidências indicam que esses sinais e sintomas sejam mediados por receptores colinérgicos nicotínicos centrais e periféricos. Outros neurotransmissores, como por exemplo a serotonina e os peptídeos opióides, também podem estar envolvidos na mediação da dependência e síndrome de abstinência à nicotina. A revisão da literatura mostra a complexidade dos efeitos da nicotina no organismo. A integração entre as abordagens comportamental, neuroquímica e molecular possibilitará a compreensão dos mecanismos neurais da dependência ao tabaco e fornecerá as bases para o desenvolvimento racional de agentes terapêuticos que possam ser utilizados para o tratamento da dependência e síndrome de abstinência ao tabaco.

Population aging is one of the greatest challenges to contemporary public health and, in this perspective, the functional capacity emerges as an important feature in geriatric assessment. The oral health of elderly, in turn, deserves special attention because, historically, in the dental services, this population group was not considered a priority for attention, which is verified by high rates of edentulism found even among these individuals. The present study proposes to examine the relationship between oral health status and functional capacity in an elderly population. To this end, intra-oral epidemiological examination was performed to assess the degree of dental caries, periodontal status, use and need of prosthesis and the presence of lesions. Functional capacity was assessed by the Independence in Activities of Daily Living, which considers the independence or not in the performance of six self-care functions. Socioeconomic and demographic characteristics and general health status were also investigated, in view of the possibility of intervention of these variables in the investigated relation. An factor analysis of the principal components was conducted which resulted four indicators of oral health conditions, representative of the population studied. 441 seniors were enrolled with mean age of 71.7 (± 8.7) years, the majority being female (68%). Functional capacity was dichotomized into completely independent individuals (89.6%) and dependent on at least one of the functions considered (10.4%). There was an association between functional capacity and the indicators related to the presence of many teeth and dental caries, and to that associated with the use and need of prostheses. These associations in turn, lost statistical significance when adjusting for confounding variables, combined in separate models for each indicator. Some of these variables, however, remained associated with functional capacity. It is considered that the study of oral health status of elderly, associeted with the search for an association with functional capacity is important in the construction of indicators necessary for planning preventive and therapeutic interventions that reduce the risk for loss of ability in daily physical functions and their consequences, as the harm in the oral self-care

The professional exposure of dental surgeons to mercury has attracted a great deal of attention from governmental organs and institutions in different countries. Considering that the normal ratio is 10 μg/1000 ml of urine, it was verified from tests that 72.7% of the professionals working in the dental practice in the city of Araraquara (SP) Brazil, show mercury levels above this limit. These results demonstrated the almost complete unawareness of many dentists to mercury and, consequently, precautionary measures were almost nonexistent.
