Background: Despite a number of programs aimed at the transfer of reproductive health information, adolescents in Zimbabwe still face unprecedented reproductive challenges. Objectives: The study sought to explore adolescent girls’ knowledge of their sexual and reproductive health; the factors that influence their sexual behaviors and to determine the extent to which adolescents had access to sexual and reproductive health information. Methods: The case study methodology was used for the study. The interpretive paradigm was used as the methodological theory and Grunig’s model of excellence in communication was used as the substantive theory. Data was obtained through the use of focus group discussions and indepth interviews. Results: Although adolescents knew the different types of sexually transmitted diseases and were aware of the consequences of engaging in risky sexual behaviors, they engaged in health behaviors which had potential for serious consequences. The study established that adolescents did not have adequate access to sexual and reproductive health information. Sexual issues were not adequately addressed both at school and at home. Conclusion: Adolescents lack adequate access to reproductive health information and there is need for effective communication programs that contribute towards the understanding of communicated messages by audiences and the understanding of audiences by communicators.

The purpose of this study was to investigate perceptions of health and health behaviors among Indigenous Australian men. Using a participatory action research (PAR) framework, we conducted two focus groups and 40 individual semi-structured interviews with men between the ages of 18 and 35 years in each of three locations across Australia. We used the health beliefs model to provide a framework for the analyses. Participants recognized that their Indigenous status placed them in a vulnerable position with regard to health, and that there might be serious consequences of failing to follow a good diet and engage in appropriate exercise. However, they delineated a number of barriers to engaging in such health behaviors. These perceived barriers require addressing at a range of policy levels within government, with a focus on social structures and institutionalized discrimination, as well as unemployment, poverty, dispossession, and cultural oppression.

BACKGROUND: Health literacy has become an important health policy and health promotion agenda item in recent years. It had been seen as a means to reduce health disparities and a critical empowerment strategy to increase people's control over their health. So far, most of health literacy studies mainly focus on adults with few studies investigating associations between child health literacy and health status. This study aimed to investigate the association between health literacy and body weight in Taiwan's sixth grade school children.

METHODS: Using a population-based survey, 162,209 sixth grade (11-12 years old) school children were assessed. The response rate at school level was 83%, with 70% of all students completing the survey. The Taiwan child health literacy assessment tool was applied and information on sex, ethnicity, self-reported health, and health behaviors were also collected. BMI was used to classify the children as underweight, normal, overweight, or obese. A multinomial logit model with robust estimation was used to explore associations between health literacy and the body weight with an adjustment for covariates.

RESULTS: The sample consisted of 48.9% girls, 3.8% were indigenous and the mean BMI was 19.55 (SD = 3.93). About 6% of children self-reported bad or very bad health. The mean child health literacy score was 24.03 (SD = 6.12, scale range from 0 to 32). The overall proportion of obese children was 15.2%. Children in the highest health literacy quartile were less likely to be obese (12.4%) compared with the lowest quartile (17.4%). After controlling for gender, ethnicity, self-rated health, and health behaviors, children with higher health literacy were less likely to be obese (Relative Risk Ratio (RRR) = 0.94, p < 0.001) and underweight (RRR = 0.83, p < 0.001). Those who did not have regular physical activity, or had sugar-sweetened beverage intake (RRR > 1.10, p < 0.0001) were more likely to report being overweight or obese.

CONCLUSIONS: This study demonstrates strong links between health literacy and obesity, even after adjusting for key potential confounders, and provides new insights into potential intervention points in school education for obesity prevention. Systematic approaches to integrating a health literacy curriculum into schools may mitigate the growing burden of disease due to obesity.

We evaluated sustainability of an intervention to reduce women’s cardiovascular risk factors, determined the influence of self-efficacy, and described women’s current health. We used a mixed method approach that utilized forced choice and open-ended questionnaire items about health status, habits, and self-efficacy. Sixty women, average age 61, returned questionnaires. Women in the original intervention group continued health behaviors intended to reduce cardiovascular disease (CVD) at a higher rate than the control group, supporting the feasibility of a targeted intervention built around women’s individual goals. The role of self-efficacy in behavior change is unclear. The original intervention group reported higher self-reported health.

In recent years a compelling body of knowledge has been accumulated to support the belief that physical activity and dietary behaviors carry important health consequences for young people. It has long been known that adequate nutrition and physical activity are essential for normal growth and development [1]. Recently, however, clear evidence has emerged that diet and physical activity during childhood and adolescence also affect an array of physiological factors associated with risk for developing chronic diseases; these factors include body composition (e.g., adiposity), blood lipid concentrations, blood pressure, and bone mineral density It also appears that physical activity and dietary behaviors and the physiological outcomes associated with them often track from childhood and adolescence into adulthood. Thus, risky health behaviors adopted early in life may negatively influence health in adulthood by having both a short-term effect on physiological risk factors and a long-term impact on health behavior.

Purpose To test the effects of a community-based physical activity intervention designed to increase physical activity and to conduct an extensive process evaluation of the intervention. Design Quasi-experimental. Setting Two rural communities in South Carolina. One community received the intervention, and the other served as the comparison. Subjects Public school students who were in fifth grade at the start of the study (558 at baseline) were eligible to participate. A total of 436 students participated over the course of the study. Intervention The intervention included after-school and summer physical activity programs and home, school, and community components designed to increase physical activity in youth. The intervention took place over an 18-month period. Measures. Students reported after-school physical activity at three data collection points (prior to, during, and following the intervention) using the Previous Day Physical Activity Recall (PDPAR). They also completed a questionnaire designed to measure hypothesized psychosocial and environmental determinants of physical activity behavior The process evaluation used meeting records, documentation of program activities, interviews, focus groups, and heart rate monitoring to evaluate the planning and implementation of the intervention. Results There were no significant differences in the physical activity variables and few significant differences in the psychosocial variables between the intervention and comparison groups. The process evaluation indicated that the after-school and summer physical activity component of the intervention was implemented as planned, but because of resource and time limitations, the home, school, and community components were not implemented as planned. Conclusions The intervention did not have a significant effect on physical activity in the target population of children in the intervention community. This outcome is similar to that reported in other studies of community-based physical activity intervention.

We developed a theoretical framework to organize obesity prevention interventions by their likely impact on the socioeconomic gradient of weight. The degree to which an intervention involves individual agency versus structural change influences socioeconomic inequalities in weight. Agentic interventions, such as standalone social marketing, increase socioeconomic inequalities. Structural interventions, such as food procurement policies and restrictions on unhealthy foods in schools, show equal or greater benefit for lower socioeconomic groups. Many obesity prevention interventions belong to the agento–structural types of interventions, and account for the environment in which health behaviors occur, but they require a level of individual agency for behavioral change, including workplace design to encourage exercise and fiscal regulation of unhealthy foods or beverages. Obesity prevention interventions differ in their effectiveness across socioeconomic groups. Limiting further increases in socioeconomic inequalities in obesity requires implementation of structural interventions. Further empirical evaluation, especially of agento–structural type interventions, remains crucial.

Seat belts are effective safety devices used to protect car occupants from severe injuries and fatalities during road vehicle accidents. Despite the proven effectiveness of seat belts, seat belt use rates are quite low, especially in developing countries, such as Turkey. The general aim of the present study was to investigate a large variety of factors related to seat belt use among Turkish car occupants using different perspectives and methods and therefore, to contribute to the design of effective seat belt use interventions for increasing seat belt use rates in Turkey. Five sub-studies were conducted within the present study. In the first sub-study, environmental (e.g., road type) and psycho-social factors (e.g., belt use by other car occupants) related to the seat belt use of front-seat occupants were investigated using observation techniques. Being male, of a young age, and traveling on city roads were the main factors negatively related to seat belt use. Furthermore, seat belt use by the drivers and front-seat passengers was highly correlated and a significant predictors of each other. In the second sub-study, the motivations of the car occupants for seat belt use and non-use were investigated using interview techniques. Situational conditions, such as traveling on city roads and for short distances, and not believing in the effectiveness and relevance of seat belt use for safety, were the most frequently reported reasons for not using a seat belt. Safety, habit and avoiding punishment were among the most frequently reported reasons for using a seat belt. In the third sub-study, the Theory of Planned Behavior (TPB) and the Health Belief Model (HBM) were applied to seat belt use using Structural Equation Modeling techniques. The TPB model showed a good fit to the data, whereas the HBM showed a poor fit to the data. Within the TPB model, attitude and subjective norm were significant predictors of intentions to use a seat belt on both urban and rural roads. In the fourth sub-study, seat belt use frequency and motivations for seat belt use among taxi drivers were investigated and compared between free-time and work-time driving using a survey. The results showed that taxi drivers used seat belts more when driving a private car in their free-times compared to when driving a taxi during their work-times. The lack of a legal obligation to use a seat belt in city traffic and fear of being attacked or robbed by the passengers were found as two specific reasons for not using a seat belt when driving a taxi. Lastly, in the fifth sub-study, the relationship of seat belt use to driver and health behaviors was investigated using a survey. Although seat belt use was related both to health and driver behaviors, factor analysis results showed that it grouped with driver behaviors. Based on the results of the sub-studies, a tentative empirical model showing different predictors of seat belt use was proposed. According to the model, safety and normative motivations and perceived physical barriers related to seat belt use are the three important predictors of seat belt use. Varhaislapsuuden karies ja sen ehkäisy kehittyvän terveydenhuollon maassa Varhaislapsuuden karies on merkittävä kansanterveysongelma varsinkin lapsirikkaissa maissa ja väestöissä. Karieksen hoitaminen vie paljon voimavaroja ja aiheuttaa mittavia taloudellisia seuraamuksia. Karies voi ilmaantua lapselle jo vauvaikäisenä, pian ensimmäisten maitohampaiden puhjettua suuhun. Alle 3-vuotiaiden karieksesta on kuitenkin niukasti tilastotietoja. Maailman terveysjärjestökin suosittaa tietojen keräämistä vasta 3-vuotiaiden ikäryhmästä. Heistä kariesta sairastaa Suomessa 16 %, Yhdysvalloissa 25 %, Englannissa 30 %, Iranissa 46 % ja Saudi-Arabiassa 61 %. Tämä väitöstutkimus selvitti karieksen esiintymistä ja sen vaaratekijöitä 1─3-vuotiailla Teheranissa. Lisäksi tutkimus arvioi perusterveydenhuoltoon sisällytetyn karieksen ehkäisyn tuloksellisuutta. Tutkimuskohteiksi arvottiin Teheranista 18 neuvolaa. Jokaisessa oltiin 4 päivää, jolloin kaikkia rokotuksiin tulleita 1─3-vuotiaita äiteineen pyydettiin osallistumaan tutkimukseen. Kahta lukuun ottamatta kaikki äidit suostuivat, ja aineistoon tuli kaikkiaan 504 lasta äiteineen. Kaikki 1-vuotiaat, 242 lasta äiteineen, valittiin karieksen ehkäisykokeiluun. Sitä varten neuvolat jaettiin kolmeen ryhmään, joista kaksi (A ja B) oli koeryhmiä ja yksi (C) oli vertailuryhmä. Tutkimus alkoi äidin haastattelulla. Siinä selvitettiin perheen koulutus- ja tulotaso sekä lapsen ruokinnasta imetyksen kesto, yösyötöt ja päiväaikaan nautitut makeat. Vielä kysyttiin lapsen ja äidin suuhygieniatavoista ja äidin kokemuksista lapsen suun puhdistamisessa. Sitten hammaslääkäri tutki lapsen suun ja kirjasi karieksen ja hammasplakin esiintymät. Suun tutkimuksen jälkeen äiti ja lapsi siirtyivät rokotushuoneeseen. Koeryhmissä (A ja B) äidit saivat terveydenhoitajalta suunterveyttä koskevan esitteen ja kehotuksen lukea se huolellisesti. Lisäksi ryhmässä A terveydenhoitaja kertoi suun ja hampaiden terveydenhoidosta saman esitteen avulla, ja neuvolan henkilökunta muistutti suunhoidon tärkeydestä puhelimitse kahdesti seuraavan puolen vuoden kuluessa. Vertailuryhmässä äideille ei annettu suunhoidon ohjeita. Kaikissa ryhmissä äitejä muistutettiin seuraavan rokotuskerran ajankohdasta, muttei mainittu tulevaa toista hammastarkastusta. Varhaislapsuuden kariesta sairasti ikäryhmästä riippuen 3─26 % tutkituista 1─3-vuotiaista, ja 65─76 %:lla oli hammasplakkia. Äideistä 68 % harjasi hampaansa päivittäin ja 39 % puhdisti lapsensa suun päivittäin. Mitä useammin äiti harjasi omat hampaansa, sitä paremmin hän huolehti lapsen suun puhtaudesta. Rintaruokinta oli yleistä eikä lisännyt kariesvaaraa. Yöllä pullomaitoa saavilla karies oli 5 kertaa yleisempää kuin muilla. Neuvolassa saatu ohjeistus ehkäisi selvästi karieksen syntyä puolen vuoden kokeessa.

A autoavaliação do estado de saúde (AAS) é um indicador de saúde amplamente utilizado e influenciado por uma grande variedade de fatores. Em particular, existem evidências crescentes de que a discriminação racial é um importante fator de risco para eventos mórbidos em saúde e seu impacto na saúde da população brasileira ainda é pouco explorado. No primeiro artigo, o objetivo principal é investigar a associação entre AAS e fatores sociodemográficos, comportamentais e de morbidade. No segundo artigo, o objetivo é estimar a associação entre discriminação racial e diferentes desfechos em saúde, a saber, AAS, morbidade física e depressão ajustando por variáveis sociodemográficas, comportamentos relacionados à saúde e Índice de Massa Corporal, na população de pretos e pardos. O presente estudo possui delineamento seccional, baseado nos dados do inquérito de abrangência nacional Pesquisa Dimensão Social das Desigualdades. Os entrevistados responderam a questionários estruturados e suas medidas antropométricas foram aferidas. No primeiro artigo, foram avaliados 12.324 indivíduos, entre chefes de família e cônjuges, com idade maior ou igual a 20 anos. No segundo artigo, foram avaliados 3.863 chefes de família que responderam a pergunta sobre discriminação racial e que se classificaram como pretos e pardos. AAS foi avaliada por meio de pergunta obtida do instrumento de qualidade de vida SF-36 e, para o primeiro artigo, foi analisada de forma dicotômica em AAS boa (categorias de resposta excelente, muito boa e boa) e AAS ruim (categorias de resposta razoável e ruim). No segundo artigo, esse desfecho foi analisado utilizando-se as 5 categorias de resposta. As análises foram realizadas utilizando-se modelos de regressão logística uni e multivariados, para dados binários (artigo 1) ou ordinais (artigo 2). Os resultados foram apresentados na forma de Odds Ratios com os respectivos intervalos de 95% de confiança. Maior faixa etária, analfabetismo, tabagismo, obesidade e doenças crônicas estiveram associados a maior chance de AAS ruim. Para cada incremento na faixa de renda, observou-se uma redução de 20% na chance de relatar AAS ruim. Atividade física esteve associada a menor chance de AAS ruim. No segundo artigo, exposição à discriminação racial esteve associada com aumento na chance de relato de pior AAS, de morbidade física e de depressão. O presente estudo identificou a influência de diversos fatores sociais, demográficos, comportamentos relacionados à saúde e morbidade física na AAS. O estudo demonstrou ainda que a discriminação racial está associada negativamente aos três desfechos em saúde avaliados (AAS, morbidade física e depressão). Esses resultados podem traçar um perfil de subgrupos populacionais mais vulneráveis, ou seja, com maior risco de contrair doenças ou de procurar o serviço de saúde por uma doença já existente, auxiliando na definição de populações-alvo para o adequado planejamento de políticas e de programas de promoção de saúde.

Este trabalho refere-se ao educar/cuidar/pesquisar centrado na orientação de enfermagem para o autocuidado da pessoa idosa, com diabetes, visando seu bem-estar; visto a evidência do crescimento demográfico da população idosa em todo o mundo e, em especial, no Brasil. Tem como objetivo aplicar, no contexto de um curso de autocuidado ministrado em oficina sociopoética, o Diagrama de Nola Pender como instrumento de produção de dados sobre a construção de ações e apresentação de propostas de comportamentos de saúde, visando o bem-estar de pessoas. O marco teórico enfoca a Teoria de promoção da saúde de Nola Pender, que defende o processo de capacitação da comunidade para buscar sua qualidade de vida e saúde, incluindo maior participação e controle deste processo. O marco teórico metodológico é a sociopoética, que fundamenta um estudo qualitativo, descritivo, desenvolvido em 2012, após aprovação do Comitê de Ética da Universidade do Estado do Rio de Janeiro, mediante a instituição do Grupo Pesquisador, dispositivo analítico, deste método, composto por 10 sujeitos, aplicando-se as técnicas de pesquisa de sensibilidade e artísticas. São resultados da técnica Vivência de Lugares Geomíticos a delimitação das categorias temáticas, a saber: Medo do desconhecido; Transcendendo a convivência com a diabetes através da aceitação; O autocuidado como resolução dos problemas a serem enfrentados; Busca da cura da diabetes através de uma vida saudável; e Expectativas para o futuro. Na técnica Corpo como território mínimo emergiram os temas: Insegurança; Fragilidade; Dificuldade no controle da glicemia; Controle; Autocuidado; Otimismo; Perseverança; Dificuldade no controle da alimentação; Tranquilidade; Dependência; Conformação; Revolta; Equilíbrio; Desânimo; Autoestima; Autoimagem e Descuidado. Aplicando o Diagrama de Nola Pender constatou-se entre os membros do Grupo Pesquisador (GP) os fatores pessoais que influenciam a conduta prévia à adoção do autocuidado, os benefícios de ações percebidas durante o ensino do autocuidado, e as demandas de competência para assumi-lo, visando à promoção da saúde. Conclui-se, que a conduta de promoção de saúde é a variável que conduz a um olhar através de ações dirigidas que tenham resultados positivos para o bem-estar e o equilíbrio das dimensões corporais. Entre as condutas de promoção da saúde adotadas pelo GP estão: ter compromisso na realização de atividades físicas; alimentação saudável, uso de medicação diariamente e regularmente, além do autocuidado com o corpo e mente, através de práticas de atividade física e de lazer.

Desde o final do século XX, o tema saúde é um das mais procurados na internet para diversos fins. As pessoas que convivem com HIV/AIDS não estão afastadas dessa tendência, formando inclusive um dos grupos de usuários que mais acessam a internet. Um grupo com um passado e presente de produção de movimento social que muito contribuiu para a reconhecida política HIV/AIDS brasileira. O objetivo desta tese é identificar e analisar os padrões de busca e interação com o conteúdo em saúde na internet no cotidiano das pessoas com HIV/AIDS, em particular nos potenciais desdobramentos em processos de medicalização, tomada de decisão sobre condutas em saúde e relação com movimento social. A metodologia se baseou em análise de conteúdo de entrevistas e realização de etnografia virtual de uma página fechada no Facebook. As discussões sobre o material pesquisado foram divididas em categorias analíticas, cuja análise gerou os seguintes resultados: a sociabilidade produzida na internet contribui para diminuir o sofrimento em relação ao preconceito, tanto em relação ao HIV/AIDS, quanto à homossexualidade; há uma carência de espaços de acolhimento virtual em detrimento a uma maior oferta de espaços para discussão sobre políticas públicas; a medicalização na rede produz a chance de se obter condutas não recomendadas, no entanto, pessoas vinculadas a grupos virtuais possuem mais estímulos a não abandonar a medicação; a confiabilidade nos conteúdos da internet em geral possui um padrão de acesso a sites recomendados pelos órgãos oficiais do setor saúde; é comum pesquisar antes ou depois da consulta médica, no entanto a negociação se dá em cyberespaços de acolhimento; muitos ativistas do HIV/AIDS foram estimulados a participar do ativismo político através da internet. Há necessidade de se ampliar espaços virtuais de acolhimento através de políticas públicas incentivadoras; a formação médica precisa contemplar questões relacionadas à internet e saúde sobre sociabilidade, adesão, e terapêutica digital, prescrição de sites, blogs e redes sociais, devendo-se ponderar com questões de medicalização e prevenção quaternária.

OBJECTIVES: To assess the prevalence of musculoskeletal symptoms and their association with sociodemographic risk factors among female garment factory workers in Sri Lanka. METHODS: 1058 randomly selected female garment factory workers employed in the free trade zone of Kogalla, Sri Lanka were recruited to complete two interviewer-administered questionnaires assessing musculoskeletal symptoms and health behaviors. DISCUSSION: Musculoskeletal complaints among female garment workers in the FTZ of Kogalla are less common than expected. Sociocultural factors may have resulted in underreporting and similarly contribute to the low rates of healthcare utilization by these women. RESULTS: 164 (15.5%) of workers reported musculoskeletal symptoms occurring more than 3 times or lasting a week or more during the previous 12-month period. Back (57.3%) and knee (31.7%) were the most common sites of pain. Although most symptomatic women reported that their problems interfered with work and leisure activities, very few missed work as a result of their pain. Prevalence correlated positively with increased age and industry tenure of less than 12 months. Job type, body mass index, and education were not significant predictors of musculoskeletal symptoms.

Shame has been shown to predict sexual HIV transmission risk behavior, medication non-adherence, symptomatic HIV or AIDS, and symptoms of depression and PTSD. However, there remains a dearth of tools to measure the specific constructs of HIV-related and sexual abuse-related shame. To ameliorate this gap, we present a 31-item measure that assesses HIV and sexual abuse-related shame, and the impact of shame on HIV-related health behaviors. A diverse sample of 271 HIV-positive men and women who were sexually abused as children completed the HIV and Abuse Related Shame Inventory (HARSI) among other measures. An exploratory factor analysis supported the retention of three-factors, explaining 56.7% of the sample variance. These internally consistent factors showed good test-retest reliability, and sound convergent and divergent validity using eight well-established HIV specific and general psychosocial criterion measures. Unlike stigma or discrimination, shame is potentially alterable through individually-focused interventions, making the measurement of shame clinically meaningful.
