The viability of boundary layer ingesting (BLI) engines for future aircraft propulsion is dependent on the ability to design robust, efficient engine fan systems for operation with continuously distorted inlet flow. A key step in this process is to develop an understanding of the specific mechanisms by which an inlet distortion affects the performance of a fan stage. In this paper, detailed full-annulus experimental measurements of the flow field within a low-speed fan stage operating with a continuous 60 deg inlet stagnation pressure distortion are presented. These results are used to describe the three-dimensional fluid mechanics governing the interaction between the fan and the distortion and to make a quantitative assessment of the impact on loss generation within the fan. A 5.3 percentage point reduction in stage total-to-total efficiency is observed as a result of the inlet distortion. The reduction in performance is shown to be dominated by increased loss generation in the rotor due to off-design incidence values at its leading edge, an effect that occurs throughout the annulus despite the localized nature of the inlet distortion. Increased loss in the stator row is also observed due to flow separations that are shown to be caused by whirl angle distortion at rotor exit. By addressing these losses, it should be possible to achieve improved efficiency in BLI fan systems. © 2013 by ASME.

Purpose Obstructive sleep apnoea (OSA) patients effectively treated by and compliant with continuous positive air pressure (CPAP) occasionally miss a night’s treatment. The purpose of this study was to use a real car interactive driving simulator to assess the effects of such an occurrence on the next day’s driving, including the extent to which these drivers are aware of increased sleepiness. Methods Eleven long-term compliant CPAP-treated 50–75-year-old male OSA participants completed a 2-h afternoon, simulated, realistic monotonous drive in an instrumented car, twice, following one night: (1) normal sleep with CPAP and (2) nil CPAP. Drifting out of road lane (‘incidents’), subjective sleepiness every 200 s and continuous electroencephalogram (EEG) activities indicative of sleepiness and compensatory effort were monitored. Results Withdrawal of CPAP markedly increased sleep disturbance and led to significantly more incidents, a shorter ‘safe’ driving duration, increased alpha and theta EEG power and greater subjective sleepiness. However, increased EEG beta activity indicated that more compensatory effort was being applied. Importantly, under both conditions, there was a highly significant correlation between subjective and EEG measures of sleepiness, to the extent that participants were well aware of the effects of nil CPAP. Conclusions Patients should be aware that compliance with treatment every night is crucial for safe driving.

Introduction Sleep restriction and missing 1 night’s continuous positive air pressure (CPAP) treatment are scenarios faced by obstructive sleep apnoea (OSA) patients, who must then assess their own fitness to drive. This study aims to assess the impact of this on driving performance. Method 11 CPAP treated participants (50–75 yrs), drove an interactive car simulator under monotonous motorway conditions for 2 hours on 3 afternoons, following;(i)normal night’s sleep (average 8.2 h) with CPAP (ii) sleep restriction (5 h), with CPAP (iii)normal length of sleep, without CPAP. Driving incidents were noted if the car came out of the designated driving lane. EEG was recorded continually and KSS reported every 200 seconds. Results Driving incidents: Incidents were more prevalent following CPAP withdrawal during hour 1, demonstrating a significant condition time interaction [F(6,60) = 3.40, p = 0.006]. KSS: At the start of driving participants felt sleepiest following CPAP withdrawal, by the end of the task KSS levels were similar following CPAP withdrawal and sleep restriction, demonstrating a significant condition, time interaction [F(3.94,39.41) = 3.39, p = 0.018]. EEG: There was a non significant trend for combined alpha and theta activity to be highest throughout the drive following CPAP withdrawal. Discussion CPAP withdrawal impairs driving simulator performance sooner than restricting sleep to 5 h with CPAP. Participants had insight into this increased sleepiness reflected by the higher KSS reported following CPAP withdrawal. In the practical terms of driving any one incident could be fatal. The earlier impairment reported here demonstrates the potential danger of missing CPAP treatment and highlights the benefit of CPAP treatment even when sleep time is short.

Introdução: O uso da pressão expiratória positiva na via aérea (EPAP) não é sugerido como técnica de desmame. O EPAP pode previnir o colapso das vias aéreas durante a expiração. Objetivo: O objetivo deste trabalho é verificar se a utilização da pressão expiratória positiva na via aérea (EPAP) apresenta benefícios na redução da hiperinsuflação dinâmica com redução do trabalho respiratório e melhora da oxigenação em pacientes submetidos ao processo de desmame da ventilação mecânica. Material e Métodos: Quarenta pacientes submetidos à ventilação mecânica por um período maior que 48 horas em 2 unidades de terapia intensiva foram avaliados prospectivamente em um estudo randomizado controlado cruzado. Todos os pacientes foram submetidos métodos de ventilação de pressão de suporte (PSV), tubo-t e EPAP, durante 30 minutos, com um período de descanso de 30 minutos entre cada método. Os pacientes foram monitorizados pelo VenTrack (Novametrix, EUA). As variáveis estudadas, mensuradas no minuto 1, 15 e 30, foram: PEEP intínseca (PEEPi), trabalho respiratório (WOBtotal), frequência respiratória (f), volume de ar corrente (Vt) e saturação periférica de oxigênio (SaO2). A amostra geral foi analisada e dividida em subgrupos DPOC (n= 14) e não-DPOC (n=26), traqueostomizados (n=15) e não-traqueostomizados (n=25). As comparações foram feitas pela Análise de Variância (ANOVA) e teste-t. O nível de significância foi de 95%. Resultados: PEEPi DPOC e não-DPOC minuto 1 (0,014 + 0,03 versus 0,17 + 0,38 cmH2O) e minuto 15 (0,042 + 0,13 versus 0,41 + 0,78 cmH2O) (p<0,05). No subgrupo não-traqueo, nos métodos de PSV15 (0,26 + 0,5 cm H2O) e EPAP15 (0,02 + 0,07 cm H2O), assim como PSV 30 (0,21 + 0,4 cm H2O) e EPAP 30 (0,02 + 0,1 cm H2O) (p<0,05). Para traqueo vs não-traqueo, no método EPAP minuto 1 (PEEPi traqueo 0,58 + 0,94 cm H2O; PEEPi não-traqueo 0,08 + 0,28 cmH2O) e minuto 15 (PEEPi traqueo 0,91 + 2,06 cm H2O; PEEPi não-traqueo 0,02 + 0,07 cmH2O) (p<0,05). Em relação ao WOBtotal houve um aumento significativo no método EPAP em relação ao tubo-t na análise geral da amostra (p<0,05). A f mostrou-se maior no método EPAP para o subgrupo não-DPOC e não-traqueo (minutos 1, 15 e 30). A SaO2 foi maior no subgrupo PSV quando comparada com tubo-t na análise geral da amostra, (p<0,05) Conclusões: A EPAP não demonstrou redução na PEEPi na análise geral da amostra, subgrupo DPOC, não-DPOC e traqueostomizados. Houve redução na PEEPi no grupo não-traqueostomizados. Houve aumento do WOBtotal com o uso da EPAP. Neste estudo a EPAP não demonstrou vantagens em relação aos outros métodos.

Introduction: The snoring is produced by the vibration of the soft palatum and/or other oropharyngeal tissues, during the air passage, in the breathing function. OSAHS is defined as the closing of 30%, at least, of the nasal/ buccal airway for 10 seconds or more, in spite of existing inspiratory effort, accompanied by oxyhemoglobin de- saturation of 4% or more. Objectives: To evaluate the available scientific evidence about the use of mandibular advancement intraoral appliances in the treatment of the snoring and/or OSAHS. Methodology: Electronic search strategy using predefined key-words and criteria was realized including studies published until October of 2008. It was also used the qualitative evaluation of the articles methodology. Results: Although a significant number of articles has studied this subject, only 7 articles showed methodological quality to be included in this systematic review. Conclusions: The intraoral appliances are widely prescribed for the treatment of snoring and OSAHS as primary therapy and as an alternative to patients who are unable to tolerate the positive airway pressure therapy.

This review is intended to gather together recent studies that explore the effects of different types of noninvasive ventilation (NIV) on the autonomic nervous system, assessed through heart rate variability (HRV). A search for papers was conducted in the PubMed, PEDro, SciELO and Lilacs databases with the following descriptors: noninvasive ventilation, CPAP ventilation, intermittent positive pressure breathing and autonomic nervous system, for the period between 2008 and 2012. After eliminating papers not addressing the topic, we selected six studies, of which five applied NIV in CPAP mode and one used biphasic positive airway pressure. In general, the findings suggest that NIV promotes changes in autonomic modulation that are dependent on the conditions of the subjects analyzed and the time when these rates are assessed, meaning acute or long-term effects.

Delayed lipoprotein clearance is associated with atherosclerosis. This study examined whether chronic intermittent hypoxia (CIH), a hallmark of obstructive sleep apnoea (OSA), can lead to hyperlipidaemia by inhibiting clearance of triglyceride rich lipoproteins (TRLP). Male C57BL/6J mice on high-cholesterol diet were exposed to 4 weeks of CIH or chronic intermittent air (control). FIO2 was decreased to 6.5 once per minute during the 12 h light phase in the CIH group. After the exposure, we measured fasting lipid profile. TRLP clearance was assessed by oral gavage of retinyl palmitate followed by serum retinyl esters (REs) measurements at 0, 1, 2, 4, 10, and 24 h. Activity of lipoprotein lipase (LpL), a key enzyme of lipoprotein clearance, and levels of angiopoietin-like protein 4 (Angptl4), a potent inhibitor of the LpL activity, were determined in the epididymal fat pads, skeletal muscles, and heart. Chronic intermittent hypoxia induced significant increases in levels of total cholesterol and triglycerides, which occurred in TRLP and LDL fractions (P 0.05 for each comparison). Compared with control mice, animals exposed to CIH showed increases in REs throughout first 10 h after oral gavage of retinyl palmitate (P 0.05), indicating that CIH inhibited TRLP clearance. CIH induced a 5-fold decrease in LpL activity (P 0.01) and an 80 increase in Angptl4 mRNA and protein levels in the epididymal fat, but not in the skeletal muscle or heart. CIH decreases TRLP clearance and inhibits LpL activity in adipose tissue, which may contribute to atherogenesis observed in OSA.

Inhaled nitric oxide (iNO) improves gas exchange in about 60% of patients with acute respiratory distress syndrome (ARDS). Recruitment of atelectatic lung areas may improve responsiveness and preservation of spontaneous breathing (SB) may cause recruitment. Accordingly, preservation of SB may improve effectiveness of iNO. To test this hypothesis, iNO was evaluated in experimental acute lung injury (ALI) during SB. In 24 pigs with ALI, effects of 10 ppm iNO were evaluated during controlled mechanical ventilation (CMV) and SB in random order. Preservation of SB was provided by 4 different modes: Unassisted SB was enabled by biphasic positive airway pressure (BIPAP), moderate inspiratory assist was provided by pressure support (PS) and volume-assured pressure support (VAPS), maximum assist was ensured by assist control (A/C). Statistical analysis did not reveal gas exchange improvements due to SB alone. Significant gas exchange improvements due to iNO were only achieved during unassisted SB with BIPAP (P <.05) but not during CMV or assisted SB. The authors conclude that effectiveness of iNO may be improved by unassisted SB during BIPAP but not by assisted SB. Thus combined iNO and unassisted SB is possibly most effective to improve gas exchange in severe hypoxemic ARDS.

Obstructive Sleep Apnea Syndrome (OSAS) is a debilitating condition stemming from disruption to the respiratory system during sleep. At present, the nature of the relationship between OSAS and mood, specifically depression and anxiety, is still unclear. The purpose of this paper is to shed some light on this relationship. PsycINFO was used to locate relevant papers on this topic. This literature search formed the basis of our investigation. Results showed that the anxiety and depression methodology is weak. It is now clear that there is an urgent need to better understand the roles of anxiety and depression in OSAS. For example, the research literature suggests that depression and anxiety covary with OSAS. However, because of methodological issues, such as difficulties involved in diagnosis and the use of inappropriate instruments, this conclusion remains tenuous. Future directions are discussed. B-type natriuretic peptide (BNP) is the first biomarker of proven value in screening for left ventricular dysfunction. The availability of point-of-care testing has escalated clinical interest and the resultant research is defining a role for BNP in the investigation and treatment of critically ill patients. This review was undertaken with the aim of collecting and assimilating current evidence regarding the use of BNP assay in the evaluation of myocardial dysfunction in critically ill humans. The information is presented in a format based upon organ system and disease category. BNP assay has been studied in a spectrum of clinical conditions ranging from acute dyspnoea to subarachnoid haemorrhage. Its role in diagnosis, assessment of disease severity, risk stratification and prognostic evaluation of cardiac dysfunction appears promising, but requires further elaboration. The heterogeneity of the critically ill population appears to warrant a range of cut-off values. Research addressing progressive changes in BNP concentration is hindered by infrequent assay and appears unlikely to reflect the critically ill patient's rapidly changing haemodynamics. Multi-marker strategies may prove valuable in prognostication and evaluation of therapy in a greater variety of illnesses. Scant data exist regarding the use of BNP assay to alter therapy or outcome. It appears that BNP assay offers complementary information to conventional approaches for the evaluation of cardiac dysfunction. Continued research should augment the validity of BNP assay in the evaluation of myocardial function in patients with life-threatening illness.

A síndrome da apneia e hipopneia obstrutiva do sono (SAHOS) é uma patologia que atinge cerca de 4% da população adulta, é uma doença crónica, progressiva, com alta morbilidade e mortalidade. A apneia do sono e o ronco têm despertado a atenção da área da saúde devido às inúmeras comorbilidades associadas. Esta síndrome é caracterizada por obstrução parcial ou total das vias aéreas superiores durante o sono e manifesta-se como uma redução (hipopneia) ou cessação (apneia) do fluxo aéreo. A queixa habitual dos pacientes além dos problemas sociais como ronco e a hipersonolência diurna, são a preocupação com outros quadros que esta patologia pode desencadear, como insuficiência cardíaca e hipertensão pulmonar. O tratamento da apneia obstrutiva do sono é multidisciplinar e indispensável, podendo ser realizado de diversas formas, tanto conservadoras como cirúrgicas, dependendo de inúmeros fatores como a gravidade da doença, as alterações anatómicas das vias aéreas superiores, idade e condições sistémicas do paciente. Os aparelhos intra orais têm sido usados durante anos, no tratamento da SAHOS como controle da obstrução das vias aéreas superiores. A terapia com aparelhos intraorais é simples e reversível. O sucesso do tratamento com aparelhos intra orais para o ronco e SAHOS leve e moderada é comprovado pela grande maioria dos estudos. A facilidade de uso, manutenção e conforto quando comparados a outros tratamentos faz com que os aparelhos intrabucais sejam considerados de fácil aceitação pelos pacientes. Através de uma revisão de literatura discutiu-se o uso de aparelhos intrabucais para o tratamento dessa patologia, destacando a eficácia e as limitações dessa terapia, os principais sintomas clínicos, os principais efeitos colaterais oclusais, o grau de colaboração e o índice de satisfação dos pacientes. Foi possível concluir que aparelhos intrabucais de avanço mandibular estão amplamente indicados para o tratamento do ronco e da SAHOS como terapia principal, ou alternativa naqueles pacientes que não podem suportar a terapia da pressão positiva das vias aéreas e que para o sucesso do tratamento é fundamental uma equipe multidisciplinar, incluindo um Médico Dentista e a colaboração do paciente.

PURPOSE:To assess the hemodynamic changes and bispectral index (BIS) following administration of a continuous rate infusion (CRI) of butorphanol in isoflurane-anesthetized calves.METHODS: Eight calves weighing 110 ± 12 kg were included in the study. Anesthesia was induced with 5% isoflurane in O2 delivered via face mask and maintained with end-tidal concentration of 1.4%. IPPV was set to a peak inspiratory airway pressure of 15 cmH2O and respiratory rate of six breaths minute-1. Forty minutes after the start of anesthetic maintenance, 0.1 mg kg-1butorphanol was administered intravenously, followed by a CRI of 20 µg kg-1 minute-1. Hemodynamic variables and BIS were recorded before butorphanol administration (T0), and at 10, 20, 40 and 80 minutes following the CRI. Anesthesia was discontinued after the last recording and the calves were allowed to recover. The time to sternal recumbency (SRE) and standing (ST) were evaluated.RESULTS: There were no significant differences between the moments in all hemodynamic variables and BIS. The time to SRE and ST was 9 ± 5 and 14 ± 7 minutes, respectively.CONCLUSION: The continuous rate infusion did not produce clinically relevant changes in hemodynamic or bispectral index values compared to baseline in mechanically ventilated and unstimulated calves anesthetized at 1.4% isoflurane.

Objective: To investigate the use of nasal intermittent positive pressure ventilation (NIPPV) in level three neonatal intensive care units (NICU) in northeastern Brazil. Methods: This observational cross-sectional survey was conducted from March 2009 to January 2010 in all level three NICUs in northeastern Brazil that are registered in the Brazilian Registry of Health Establishments (Cadastro Nacional de Estabelecimentos de Saude, CNES) of the Ministry of Health. Questionnaires about the use of NIPPV were sent to the NICU directors in each institution. Statistical analysis was conducted using the software Epi-Info 6.04 and double data entry. A chi-square test was used to compare variables, and the level of statistical significance was set at p <= 0.05. Results: This study identified 93 level three NICUs in northeastern Brazil registered in CNES, and 87% answered the study questionnaire. Most classified themselves as private institutions (30.7%); 98.7% used NIPPV; 92.8 % adapted mechanical ventilators for NIPPV and used short binasal prongs as the interface (94.2%). Only 17.3% of the units had a protocol for the use of NIPPV. Mean positive inspiratory pressure and positive end-expiratory pressure were 20.0 cmH(2)O (standard deviation [SD]: 4.47) and 5.0 cmH(2)O (SD: 0.84). Conclusion: NICUs in northeastern Brazil use nasal intermittent positive pressure ventilation, but indications and ventilation settings are not the same in the different institutions.
