A assistência psiquiátrica a indivíduos com sofrimento psíquico, em seu campo político, passou por um longo processo de discussão e formulação de estratégias a fim de garantir condição de humanidade aos indivíduos assistidos. A Reforma Psiquiátrica brasileira, como ficou conhecida, ocorreu concomitante ao processo de Reforma Sanitária e formulou leis para a garantia da integralidade e do acesso universal à saúde em território nacional sob a responsabilidade das esferas governamentais. No município de Cascavel PR, o processo de formulação das políticas assistenciais para o indivíduo com transtorno mental se inicia na década de 90, porém só é efetivamente estruturado a partir de 2003 com o fechamento do Hospital Psiquiátrico São Marcos, que forçou o município a agilizar o processo de implantação da rede assistencial e de serviços complementares com moldes psicossociais. Com isso o município estrutura a assistência ao portador de transtorno mental através da implantação de ambulatório especializado, serviço de urgência e centros de atenção psicossocial para adultos, crianças e usuários de álcool e outras drogas, todos estruturados com suporte assistencial realizado pela Unidade Básica de Saúde (UBS). Em cada UBS foi nomeado um profissional de referência para o acompanhamento desses usuários e de seus familiares com o intuito de fortalecer o vínculo e manter a relação entre os serviços complementares e atenção primária. O presente estudo é centrado na pesquisa bibliográfica e de campo, com caráter qualitativo, cujo ponto de partida é a coleta de dados por meio de aplicação de questionário semi-estruturado com a finalidade de conhecer o processo de formulação das políticas municipais de atenção ao doente mental através do questionamento sobre as práticas assistenciais desenvolvidas pela atenção primária à saúde. A população é composta por 10 (dez) profissionais das UBS, referências em saúde mental, e 10 (dez) familiares de usuários com sofrimento mental, assistidos pela atenção básica, mas inseridos em algum dos serviços ofertados da rede assistencial de saúde mental. A análise dos dados se deu por meio de análise de conteúdo, com estruturação de dois grandes focos de análise para melhor compreensão dos conteúdos (Análise de Conteúdo de Bardin). O resultado apontou dados positivos em relação à política de saúde mental municipal uma vez que, embora com apontamentos divergentes entre profissionais e usuários, percebe-se a intenção em assegurar aos indivíduos em sofrimento psíquico uma gama de procedimentos que são, inclusive, orientados por portarias ministeriais. Em relação à assistência prestada pela atenção básica de saúde, há convergência em relação às ações desenvolvidas pelos profissionais da UBS, embora os profissionais afirmem o desenvolvimento de atividades que não são confirmadas pelos usuários. Em relação às dificuldades encontradas para efetivação do tratamento, tanto profissionais quanto usuários apontam que há muito a avançar no campo da saúde mental para que efetivamente seja prestada assistência de forma equânime e integral.

Afirmar a sociedade brasileira nos quadros da modernidade foi o anseio de intelectuais de campos variados no contexto das décadas iniciais do século XX. Neste cenário, a educação era considerada uma via possível para promover mudanças de hábitos, conformando a população a partir dos referenciais modernos. Se a escola assumiu papel de destaque por ser um espaço a partir do qual seria possível educar a infância, a importância de outras instâncias educativas também era considerada, dentre as quais, destacamos a família, pensada como espaço fundamental de socialização. A educação ministrada no espaço privado deveria, no entanto, ser consoante com os ideais preconizados à época, para isso fazia-se necessário intervir sobre ele, educando os agentes do seu interior. Neste estudo, analisamos as ações encaminhadas pelo Serviço de Ortofrenia e Higiene Mental (SOHM), que funcionou no Rio de Janeiro, então Distrito Federal, no período de 1934 a 1939. Este Serviço, chefiado por Arthur Ramos, visava prevenir e corrigir os problemas dos escolares, considerando as relações culturais e sociais importantes para uma compreensão global destes indivíduos. Como as relações familiares e o espaço doméstico eram aspectos a serem conhecidos e modificados quando necessário e, como os pais eram chamados a colaborar com as ações do Serviço de formas diversas, buscamos analisar estratégias variadas destinadas a intervir no espaço doméstico, focalizando as prescrições voltadas a este e alguns aspectos da dinâmica estabelecida nas relações entre as famílias e a escola, através do SOHM.

Background: Little is known about how sitting time, alone or in combination with markers of physical activity (PA), influences mental well-being and work productivity. Given the need to develop workplace PA interventions that target employees' health related efficiency outcomes; this study examined the associations between self-reported sitting time, PA, mental well-being and work productivity in office employees. Methods: Descriptive cross-sectional study. Spanish university office employees (n = 557) completed a survey measuring socio-demographics, total and domain specific (work and travel) self-reported sitting time, PA (International Physical Activity Questionnaire short version), mental well-being (Warwick-Edinburg Mental Well-Being Scale) and work productivity (Work Limitations Questionnaire). Multivariate linear regression analyses determined associations between the main variables adjusted for gender, age, body mass index and occupation. PA levels (low, moderate and high) were introduced into the model to examine interactive associations. Results: Higher volumes of PA were related to higher mental well-being, work productivity and spending less time sitting at work, throughout the working day and travelling during the week, including the weekends (p < 0.05). Greater levels of sitting during weekends was associated with lower mental well-being (p < 0.05). Similarly, more sitting while travelling at weekends was linked to lower work productivity (p < 0.05). In highly active employees, higher sitting times on work days and occupational sitting were associated with decreased mental well-being (p < 0.05). Higher sitting times while travelling on weekend days was also linked to lower work productivity in the highly active (p < 0.05). No significant associations were observed in low active employees. Conclusions: Employees' PA levels exerts different influences on the associations between sitting time, mental well-being and work productivity. The specific associations and the broad sweep of evidence in the current study suggest that workplace PA strategies to improve the mental well-being and productivity of all employees should focus on reducing sitting time alongside efforts to increase PA.

Em todo o mundo é alta a prevalência dos transtornos depressivos e ansiosos, em especial no nível primário. Uma das estratégias terapêuticas é o atendimento em grupo que propicia bons resultados e cuidado adequado, incluindo melhor aproveitamento do tempo e maior abrangência no atendimento segundo experiências relatadas em diversos países.O objeto deste estudo é o discurso de mulheres com transtornos depressivos e ansiosos atendidas em grupo na atenção primária. O objetivo é conhecer representações e questões sobre o processo de adoecimento de mulheres atendidas na Estratégia de Saúde da Família (ESF) diagnosticadas com transtornos depressivos e ansiosos. Os grupos terapêuticos ocorreram em Postos de Saúde da Família, no município de Petrópolis, Rio de Janeiro, realizados pelos profissionais da ESF (médicos e enfermeiros) e supervisionados por especialistas em saúde mental. A pesquisa se deu com base análise de material de observação de dois grupo em comunidades distintas. Foi utilizado o método qualitativo, com analise das falas que foram organizadas em categorias segundo o método análise de conteúdo. Resultados: Transtornos depressivos e ansiosos são retratados no discurso das mulheres nos grupos como um processo solitário, sem apoio, compreensão e ajuda de terceiros. São agravantes: dificuldades financeiras, falta de lazer, sobrecarga de obrigações e cobranças.O álcool é retratado como apaziguador do sofrimento e da solidão. Relatam relacionamentos insatisfatórios e ocupam posições de submissão, sentindo-se desempoderadas, sem ver saída para sua situação, perpetuando pensamentos e ações, retroalimentando o sofrimento emocional. Valorizam o grupo enquanto espaço terapeutico. A possibilidade de verem retratado nas colegas situações similares encoraja a ajuda mútua. Estes são aspectos que auxiliam na autopercepção e no empoderamento. A intervenção se mostrou aplicável na atenção primária, sendo factível a não especialistas de saúde mental.

A pesquisa tem como tema a prática de cuidado no matriciamento em saúde mental na Estratégia da Saúde da Família. Matriciamento é uma metodologia de prática de cuidado em saúde, constituído por uma equipe de profissionais especialistas, que possui o objetivo de oferecer apoio e retaguarda à equipe de profissionais da Estratégia Saúde da Família. A hipótese é de que a prática de matriciamento em saúde mental é um dispositivo de transformação do imaginário social sobre a loucura. Trata-se de uma pesquisa de abordagem psicossocial de referencial teórico na sociologia e na antropologia para discutir os conceitos de cultura, imaginário social, práticas de cuidado e rede social, por entender que a prática de cuidado no matriciamento em saúde mental opera em rede e produz transformação no imaginário social sobre a loucura. O estudo tem, inicialmente, como base metodológica a sociologia para a análise documental da Lei 10.216/2001 Reforma Psiquiátrica brasileira - e da Portaria 154/2008 Núcleo de Apoio à Saúde da Família NASF. Em seguida, apoiada pela antropologia, a pesquisa utiliza o método etnográfico para a realização de entrevista com quatro profissionais matriciadores, tendo a análise do discurso como caminho para discutir e problematizar os dados coletados. Conclui-se, então, que o matriciamento é um dispositivo de rede e produção de cuidado que pode favorecer a transformação do imaginário social sobre a loucura. Porém, é de fundamental importância que a rede tenha funcionalidade para que isto possa ocorrer de fato, pois o matriciamento é um dispositivo de rede e de cuidado em saúde.

A conjuntura atual referente à política de saúde mental Brasileira tem suas ações, serviços e estratégias de intervenção direcionadas pelos princípios do projeto da Reforma Psiquiátrica. Logo, há no Brasil uma série de serviços substitutivos ao hospício que foram criados ao longo dos últimos trinta anos para substituir em sua totalidade a perspectiva da psiquiatria clássica, que pensa ser o isolamento social a única maneira de cuidar e tratar o sujeito em sofrimento psíquico. O presente trabalho tem por objetivo realizar uma reflexão acerca de uma experiência política no campo da Reforma Psiquiátrica Brasileira. O objeto de estudo se designa ao conhecimento do consórcio em saúde mental existente entre os municípios de Quatis e Porto Real situados na região Médio-Paraíba interior do Estado do Rio de Janeiro. Utilizamos uma abordagem qualitativa, pois acreditamos que a mesma pode nos oferecer instrumentos de conhecimento da realidade que desejamos alcançar que são mais apropriados para desvendar os significados das ações em saúde mental e seus impactos na vida dos sujeitos envolvidos nesse processo. Para tal optamos por realizar uma observação participante no campo de pesquisa que se dá no CAPS Sonho Real e também a realização de dois grupos focais, um com usuários e outro com profissionais. Acreditamos que conhecer essa experiência se constitui como tarefa de sua importância para o fortalecimento e consolidação do compromisso político assumido por esses dois municípios no que tange a modalidade de consórcio de serviços de saúde, sendo dois territórios que se referenciam a um único dispositivo de saúde.

Drug calculations are an essential skill for nurses. The clinical skill of performing a drug calculation has come under recent scrutiny,resulting in the development of essential skills clusters inwhich pre-registration nurses must be competent before qualifying (Nursing and Midwifery Council 2007). The focuson drug calculation skills places renewed emphasis on how these skills are taught in higher education institutions and how they are learned by students theoretically and in clinical practice.

• Summary: During the last decade increasing attention has been paid to the impact of the Troubles1 on social work in Northern Ireland. In this paper, the authors describe the first survey used to test some of the assumptions which exist in the literature. An 87-item questionnaire was applied to a range of social work staff currently working in, or associated with, mental health settings. One hundred and one questionnaires were returned: it is estimated that this represented over 70 per cent of mental health social workers in Northern Ireland. • Findings: The design of the questionnaire elicited both qualitative and quantitative data. The findings reveal a workforce with complex religious and national identities and many of the respondents have experienced relatively high levels of Troubles-related incidents whilst carrying out their duties in a variety of organizational and geographical settings. High proportions of respondents received minimal agency support and training to equip them to deal with Troubles-related problems faced by them during this period. • Applications: The authors conclude that the profession and employing agencies should pay greater attention to past and present effects of the Troubles on social work practice and develop appropriate strategies for supporting, training and resourcing staff in this neglected area.

In mental health services over recent decades, the positive move away from hospital-based care to community-based services has entailed that people with higher levels of need are being supported by community mental health services. This paper begins by reviewing the literature on coercion in the field of community-based mental health care and treatment. It is argued that the lack of a critical understanding of the concept and how it is used by practitioners and agencies can have serious repercussions for the rights of service users. Using a quasi-experimental, longitudinal design, the authors then seek to test some of the ideas about coercion by comparing the activities of assertive outreach and community mental health teams in Northern Ireland, particularly the key ideas of perceived coercion, workers’ strategies and engagement with services. Key findings were that assertive outreach appeared to be more successful at reducing perceived coercion, minimizing the need for coercive strategies, engaging high-risk clients and reducing inpatient bed use. These findings are compared with other studies in this area. The authors also argue that there is a need for greater transparency in the way that practitioners use coercive measures and more explicit guidance is required in this crucial area of mental health practice.

This article provides an overview of the literature on the impact of ‘the Troubles’ on mental health in Nor thern Ireland. It identifies three main phases of professional and policy response from concerns about the effects of the violence in the early 1970s, through many years of collective denial and neglect, until acknowledgment, following the Good Friday Agreement in 1998 (Nor thern Ireland Office, 1998), of high levels of trauma and unmet need. The issues of inequality and stigma are also considered and it is argued that peace is necessary but insufficient for promoting mental health. The development of mental health services in Nor thern Ireland and the relatively recent focus on promoting mental health are also outlined and examined. It is suggested that attempts to address the needs arising as a result of ‘the Troubles’ and more general mental health promotion strategies have, to some extent, developed in parallel and that it may be impor tant to integrate these effor ts. The relative under-development of mental health services, the comprehensive Bamford Review of Mental Health and Learning Disability (2005; 2006) and the positive approach of the Public Health Agency mean that, even in the current economic climate, there are great oppor tunities for progress. Routine screening, in primary care and mental health services for trauma, including Troubles-related trauma, is recommended to identify and address these issues on an individual level. It is also argued, however, that more substantial political change is needed to effectively address societal division, inequality and stigma to the benefit of all.

Evidence for acquiescence (yea-saying) in interviews with people who have mental retardation is reviewed and the different ways it has been assessed are discussed. We argue that acquiescence is caused by many factors, each of which is detected differentially by these methods. Evidence on the likely causes of acquiescence is reviewed, and we suggest that although researchers often stress a desire to please or increased submissiveness as the must important factor, acquiescence should also be seen as a response to questions that are too complex, either grammatically or in the type of judgments they request. Strategies to reduce acquiescence in interviews are reviewed and measures that can be taken to increase the inclusiveness of interviews and self-report scales in this population suggested.

This longitudinal study sought to identify developmental changes
in strategy use between 5 and 7 years of age when solving exact
calculation problems. Four mathematics and reading achievement
subtypes were examined at four time points. Five strategies were
considered: finger counting, verbal counting, delayed retrieval,
automatic retrieval, and derived fact retrieval. Results provided
unique insights into children’s strategic development in exact calculation
at this early stage. Group analysis revealed relationships
between mathematical and/or reading difficulties and strategy
choice, shift, and adaptiveness. Use of derived fact retrieval by
7 years of age distinguished children with mathematical difficulties
from other achievement subtypes. Analysis of individual differences
revealed marked heterogeneity within all subtypes,
suggesting (inter alia) no marked qualitative distinction between
our two mathematical difficulty subtypes.

Aim: This paper reports a study on how men cope with the side-effects of radiotherapy and neo-adjuvant androgen deprivation for prostate cancer up to 1 year after treatment.

Background: With early detection and improved treatments, prostate cancer survivors are living longer with the disease and the side-effects of treatment. How they cope affects their long-term physical and mental health.

Design: A prospective, longitudinal, exploratory design using both qualitative and quantitative methods was used in this study.

Method: Between September 2006–September 2007 149 men who were about to undergo radical radiotherapy ± androgen deprivation for localized prostate cancer in Northern Ireland were recruited to the study. They completed the Brief Cope scale at four time-points.

Results: Acceptance, positive reframing, emotional support, planning and, just getting on with it, were the most common ways of coping. Fewer men used coping strategies less at 6 months and 1 year after radiotherapy in comparison to pre-treatment and 4–6 weeks after radiotherapy. Interviews with these men demonstrated that men adapted to a new norm, with the support of their wives/partners and did not readily seek professional help. A minority of men used alcohol, behavioural disengagement and self blame as ways of coping.

Conclusion: Men used a variety of ways of coping to help them deal with radiotherapy and neo-adjuvant androgen deprivation for up to 12 months after radiotherapy. Interventions need to be developed to take account of the specific needs of partners of men with prostate cancer and single men who have prostate cancer.

1) Executive Summary
Legislation (Autism Act NI, 2011), a cross-departmental strategy (Autism Strategy 2013-2020) and a first action plan (2013-2016) have been developed in Northern Ireland in order to support individuals and families affected by Autism Spectrum Disorder (ASD) without a prior thorough baseline assessment of need. At the same time, there are large existing data sets about the population in NI that had never been subjected to a secondary data analysis with regards to data on ASD. This report covers the first comprehensive secondary data analysis and thereby aims to inform future policy and practice.
Following a search of all existing, large-scale, regional or national data sets that were relevant to the lives of individuals and families affected by Autism Spectrum Disorder (ASD) in Northern Ireland, extensive secondary data analyses were carried out. The focus of these secondary data analyses was to distill any ASD related data from larger generic data sets. The findings are reported for each data set and follow a lifespan perspective, i.e., data related to children is reported first before data related to adults.
Key findings:
Autism Prevalence:
Of children born in 2000 in the UK,
• 0.9% (1:109) were reported to have ASD, when they were 5-year old in 2005;
• 1.8% (1:55) were reported to have ASD, when they were 7-years old in 2007;
• 3.5% (1:29) were reported to have ASD, when they were 11-year old in 2011.
In mainstream schools in Northern Ireland
• 1.2% of the children were reported to have ASD in 2006/07;
• 1.8% of the children were reported to have ASD in 2012/13.

Economic Deprivation:
• Families of children with autism (CWA) were 9%-18% worse off per week than families of children not on the autism spectrum (COA).
• Between 2006-2013 deprivation of CWA compared to COA nearly doubled as measured by eligibility for free school meals (from near 20 % to 37%)
• In 2006, CWA and COA experienced similar levels of deprivation (approx. 20%), by 2013, a considerable deprivation gap had developed, with CWA experienced 6% more deprivation than COA.
• Nearly 1/3 of primary school CWA lived in the most deprived areas in Northern Ireland.
• Nearly ½ of children with Asperger’s Syndrome who attended special school lived in the most deprived areas.

Unemployment:
• Mothers of CWA were 6% less likely to be employed than mothers of COA.
• Mothers of CWA earned 35%-56% less than mothers of COA.
• CWA were 9% less likely to live in two income families than COA.

Health:
• Pre-diagnosis, CWA were more likely than COA to have physical health problems, including walking on level ground, speech and language, hearing, eyesight, and asthma.
• Aged 3 years of age CWA experienced poorer emotional and social health than COA, this difference increased significantly by the time they were 7 years of age.
• Mothers of young CWA had lower levels of life satisfaction and poorer mental health than mothers of young COA.
Education:
• In mainstream education, children with ASD aged 11-16 years reported less satisfaction with their social relationships than COA.
• Younger children with ASD (aged 5 and 7 years) were less likely to enjoy school, were bullied more, and were more reluctant to attend school than COA.
• CWA attended school 2-3 weeks less than COA .
• Children with Asperger’s Syndrome in special schools missed the equivalent of 8-13 school days more than children with Asperger’s Syndrome in mainstream schools.
• Children with ASD attending mainstream schooling were less likely to gain 5+ GCSEs A*-C or subsequently attend university.



Further and Higher Education:
• Enrolment rates for students with ASD have risen in Further Education (FE), from 0% to 0.7%.
• Enrolment rates for students with ASD have risen in Higher Education (HE), from 0.28% to 0.45%.
• Students with ASD chose to study different subjects than students without ASD, although other factors, e.g., gender, age etc. may have played a part in subject selection.
• Students with ASD from NI were more likely than students without ASD to choose Northern Irish HE Institutions rather than study outside NI.

Participation in adult life and employment:
• A small number of adults with ASD (n=99) have benefitted from DES employment provision over the past 12 years.
• It is unknown how many adults with ASD have received employment support elsewhere (e.g. Steps to Work).

Awareness and Attitudes in the General Population:
• In both the 2003 and 2012 NI Life and Times Survey (NILTS), NI public reported positive attitudes towards the inclusion of children with ASD in mainstream education (see also BASE Project Vol. 2).

Gap Analysis Recommendations:
This was the first comprehensive secondary analysis with regards to ASD of existing large-scale data sets in Northern Ireland. Data gaps were identified and further replications would benefit from the following data inclusion:
• ASD should be recorded routinely in the following datasets:
o Census;
o Northern Ireland Survey of Activity Limitation (NISALD);
o Training for Success/Steps to work; Steps to Success;
o Travel survey;
o Hate crime; and
o Labour Force Survey.
• Data should be collected on the destinations/qualifications of special school leavers.
• NILT Survey autism module should be repeated in 5 years time (2017) (see full report of 1st NILT Survey autism module 2012 in BASE Project Report Volume 2).
• General public attitudes and awareness should be assessed for children and young people, using the Young Life and Times Survey (YLT) and the Kids Life and Times Survey (KLT); (this work is underway, Dillenburger, McKerr, Schubolz, & Lloyd, 2014-2015).
