Patients and methods: Clinical data from all patients admitted with acute respiratory failure due to novel viral H1N1 infection were reviewed. Lung tissue was submitted for viral and bacteriological analyses by real-time RT-PCR, and autopsy was conducted on all patients who died. Results: Eight patients were admitted, with ages ranging from 55 to 65 years old. There were five patients with solid organ tumors (62.5%) and three with hematological malignancies (37.5%). Five patients required mechanical ventilation and all died. Four patients had bacterial bronchopneumonia. All deaths occurred due to multiple organ failure. A milder form of lung disease was present in the three cases who survived. Lung tissue analysis was performed in all patients and showed diffuse alveolar damage in most patients. Other lung findings were necrotizing bronchiolitis or extensive hemorrhage. Conclusions: H1N1 viral infection in patients with cancer can cause severe illness, resulting in acute respiratory distress syndrome and death. More data are needed to identify predictors of unfavorable evolution in these patients.

Objectives The aim of the present paper is to assess the influence of demographic, muscle enzymes, JDM scores and treatment on non-adjuvanted influenza A H1N1/2009 vaccine immunogenicity in juvenile dermatomyositis (JDM) patients. Methods Thirty JDM patients and 81 healthy age-matched controls were vaccinated. All participants were evaluated pre- and 21 days post-vaccination and serology for anti-HI NI was performed by haemagglutination inhibition assay. Muscle enzymes, JDM scores and treatment were evaluated before and after vaccination. Adverse events were reported. Results After immunisation seroconversion rates were significantly lower in JDM patients compared to age-matched controls (86.7 vs. 97.5%, p=0.044), whereas seropmtection (p=0.121), geometric mean titres (GMT) (p=0.992) and factor increase (FI) in GMT (p=0.827) were similar in both groups. Clinical and labomtorial evaluations revealed that JDM scores and muscle enzymes remained stable throughout the study (p>0.05). A higher frequency of chronic course was observed in non-seroconverted compared to seroconverted (100% vs. 27%, p=0.012). Regarding treatment, a lower rate of seroconversion was observed in patients under prednisone>20mg/day (50% vs. 4%, p=0.039), and in those treated with a combination of prednisone, methotrexate and cyclosporine (50% vs. 4%, p=0.039). Local and systemic vaccine adverse events were mild and similar in patients and controls (p>0.05). Conclusion This study identified that chronic course and immunosuppressive therapy are the major factors hampering seroconversion was JDM, suggesting that a specific protocol may be required for this subgroup of patients. In spite of that, a single dose of non-adjuvanted influenza A/H1N1 2009 vaccine was generally seroprotective in this disease with no evident deleterious effect in disease itself (ClinicalTrials.gov, no. Methods We conducted a phase I, multicenter, randomized, double-blind, placebo-controlled, multi-arm (10) parallel study involving healthy adults to evaluate the safety and immunogenicity of influenza A (H1N1) 2009 non-adjuvanted and adjuvanted candidate vaccines. Subjects received two intramuscular injections of one of the candidate vaccines administered 21 days apart. Antibody responses were measured by means of hemagglutination-inhibition assay before and 21 days after each vaccination. The three co-primary immunogenicity end points were the proportion of seroprotection >70%, seroconversion >40%, and the factor increase in the geometric mean titer >2.5. Results A total of 266 participants were enrolled into the study. No deaths or serious adverse events were reported. The most commonly solicited local and systemic adverse events were injection-site pain and headache, respectively. Only three subjects (1.1%) reported severe injection-site pain. Four 2009 influenza A (H1N1) inactivated monovalent candidate vaccines that met the three requirements to evaluate influenza protection, after a single dose, were identified: 15 μg of hemagglutinin antigen without adjuvant; 7.5 μg of hemagglutinin antigen with aluminum hydroxide, MPL and squalene; 3.75 μg of hemagglutinin antigen with aluminum hydroxide and MPL; and 3.75 μg of hemagglutinin antigen with aluminum hydroxide and squalene. Conclusions Adjuvant systems can be safely used in influenza vaccines, including the adjuvant monophosphoryl lipid A (MPL) derived from Bordetella pertussis with squalene and aluminum hydroxide, MPL with aluminum hydroxide, and squalene and aluminum hydroxide.

This case series reports the correlation between extravascular lung water (EVLW) and the partial arterial oxygen pressure/fractional inspiratory oxygen (PaO(2)/FiO(2)) ratio in three patients with severe influenza A (H1N1)-induced respiratory failure. All patients suffered from grave hypoxia (PaO(2), 26-42 mmHg) and were mechanically ventilated using biphasic airway pressure (PEEP, 12-15 mmHg; FiO(2), 0.8-1) in combination with prone positioning at 12 hourly intervals. All patients were monitored using the PICCO system for 8-11 days. During mechanical ventilation, a total of 62 simultaneous determinations of the PaO(2)/FiO(2) ratio and EVLW were performed. A significant correlation between EVLW and the PaO(2)/FiO(2) ratio (Spearman-rho correlation coefficient, -0.852; p < 0.001) was observed. In all patients, a decrease in EVLW was accompanied by an improvement in oxygenation. Serum lactate dehydrogenase levels were elevated in all patients and significantly correlated with EVLW during the intensive care unit stay (Spearman-rho correlation coefficient, 0.786; p < 0.001). In conclusion, EVLW seems increased in patients with severe H1N1-induced respiratory failure and appears to be closely correlated with impairments of oxygenatory function.

BACKGROUND To systematically assess the literature published on the clinical impact of Influenza A(H1N1)pdm09 on cystic fibrosis (CF) patients. METHODS An online search in PUBMED database was conducted. Original articles on CF patients with Influenza A(H1N1)pdm09 infection were included. We analyzed incidence, symptoms, clinical course and treatment. RESULTS Four surveys with a total of 202 CF patients infected by Influenza A(H1N1)pdm09 were included. The meta-analysis showed that hospitalisation rates were higher in CF patients compared to the general population. While general disease symptoms were comparable, the clinical course was more severe and case fatality rate (CFR) was higher in CF patients compared to asthmatics and the general population. CONCLUSIONS Evidence so far suggests that CF patients infected with Influenza A(H1N1)pdm09 show increased morbidity and a higher CFR compared to patients with other chronic respiratory diseases and healthy controls. Particularly, CF patients with advanced stage disease seem to be more susceptible to severe lung disease. Accordingly, early antiviral and antibiotic treatment strategies are essential in CF patients. Preventive measures, including vaccination as well as hygiene measures during the influenza season, should be reinforced and improved in CF patients.

El compromiso hepático en la infección por virus Influenza A (H1N1) es muy infrecuente. Presentamos un caso de un paciente varón de 21 años, sano, vacunado, que consulta con un cuadro clásico de gripe. En el examen físico no se palpa hepatoesplenomegalia ni se observa ictericia. Se diagnostica gripe por virus de la influenza A (H1N1) y se inicia tratamiento con oseltamivir oral. Los exámenes de laboratorio revelaron elevación moderada de enzimas hepáticas. Los anticuerpos para virus de la hepatitis A, B y C, virus Epstein-Barr y citomegalovirus (CMV) fueron negativos. El hisopado nasofaríngeo fue positivo para influenza A (H1N1) con prueba de reacción en cadena de polimerasa en tiempo real (PCRRT). Se detectó hepatomegalia homogénea por ecografía abdominal. El cuadro clínico se resolvió en una semana, permaneciendo elevadas las enzimas hepáticas por 21 días. Discutimos los probables mecanismos de la injuria hepática en este caso.

Introducción: Considerado como grupo de riesgo específico en la estrategia de inmunización contra la gripe A (H1N1), el colectivo de trabajadores sanitarios ha sido objeto de este estudio desde la perspectiva de sus actitudes y creencias hacia la inmunización, con especial énfasis en la influencia de las fuentes de información para tomar la decisión de vacunarse. Métodos: Estudio observacional de carácter transversal dirigido a trabajadores sanitarios en activo de la provincia de Alicante y realizado mediante cuestionario cara a cara a una muestra aleatoria por cuotas según categoría profesional en trabajadores de hospitales y centros de salud. Resultados: Las fuentes de información difieren entre subgrupos: los médicos utilizaron revistas científicas y/o congresos, las enfermeras la obtuvieron a través de Sanidad y otras enfermeras, el resto de trabajadores optaron por la televisión y/o el médico de familia. De los 3 colectivos estudiados, los médicos son los que menos sensación de gravedad han percibido frente a la gripe A (H1N1) (59,4%), son los que más confían en la vacuna (42,3%), los que más la recomiendan (44,4%), los que mejor han seguido las recomendaciones para evitar el contagio (93%) y los más vacunados (18,3%). El 75,5% de los sanitarios valoró la información recibida como regular, mala o muy mala. La totalidad admitió que se creó alarma social. Discusión: El éxito de futuras campañas de inmunización contra la gripe en personal sanitario podría incrementarse si fueran diseñadas actividades informativas segmentadas y orientadas a cada subgrupo del colectivo, adecuando la estrategia y mejorando la calidad de la información.

La infección por el virus de la influenza es un problema de salud pública por su rápida diseminación y alta morbilidad. Los casos graves de infección por éste virus presentan hipercitocinemia, la cual se ha asociado a una respuesta inmune adquirida desfavorable y un mal pronóstico para el paciente. Se han realizado hasta ahora experimentos en suero de pacientes o en tejido pulmonar en modelos de animales, sin embargo, no se tienen reportes del microambiente en el pulmón de pacientes fallecidos por el virus de la influenza. Objetivo: Determinar las subpoblaciones de macrófagos y linfocitos T y su correlación con el daño tisular en pulmón de pacientes fallecidos por influenza A H1N1 y otras enfermedades respiratorias. Material y Métodos: Se identificó y cuantificó el virus de influenza A H1N1 (pdm)09 e influenza A estacional por qRT-PCR, así como se determinó los niveles de citocinas y marcadores de macrófagos por qRT-PCR (IL-2, IL-4,IL-6, IL- 10, IL-12, IL-17, IL-23, IFN-, TGFβ, TNFα, Arg1, Retnlb e iNOS). Se realizaron tinciones de HyE para la determinación del daño tisular. Por otra parte, se realizaron tinciones de inmunohistoquímica para analizar las poblaciones celulares CD14+, CD206+, CD4+, CD8+, FOXP3+ y citocinas (IL-4, IL-10, IL-17 e IFN-) in situ. Resultados: Se determinó la presencia del virus de la influenza A en 10 muestras, de las cuales 4 fueron H1N1 (pdm)09. Además, se obtuvieron 5 muestras de neumonía por Coccidioides spp., y 6 de neumonías de origen bacteriano. No existe diferencia en el daño causado por el virus de la influenza A H1N1 (pdm)09 e influenza A estacional a nivel histopatológico. Las células CD14+ y CD4+ se encontraron aumentadas para todos los grupos de neumonías sin importar el agente etiológico, excepto CD4 para las neumonías bacterianas. Las células que expresaron Foxp3 solo se encontraron aumentadas en el grupo de coccidioidomicosis y neumonías bacterianas. No se encontró diferencia significativa entre los grupos de estudio para las células CD8+, excepto para las neumonías bacterianas. La expresión génica relativa de IL-6 se encontró aumentada 3000 veces la expresión génica en el grupo de influenza pandémica A H1N1 (pdm)09, mientras en influenza estacional se encontró una disminución de 0.08 veces de su expresión. INOS se encontró con expresión disminuida 0.5 veces para los grupos de influenza pandémica, influenza estacional y coccidioidomicosis. La expresión génica de IL-10 se encontró solamente para el grupo de influenza A H1N1 (pdm)09 (P=0.01). Resistin Like Beta se encontró con expresión disminuida solo para el grupo de influenza A H1N1 (pdm)09. Existe un aumento de células positivas para IL4 en todos los grupos, excepto neumonías bacterianas. No se encontró diferencia significativa de células positivas para IL-10 en los grupos de estudio. Existe un aumento de células positivas para IL-17 en influenza A H1N1p/2009, influenza A y neumonías bacterianas. IFN se encontró aumentado en los grupos de neumonía comparados con el control. Conclusiones: Ambos grupos de neumonías por influenza A se caracterizan por daño tisular y un exacerbado ambiente inflamatorio; solamente CD206 es capaz de diferenciar entre influenza A H1N1(pdm)09 e influenza estacional

Because of the advent of a new influenza A H1N1. strain, many countries have begun mass immunisation programmes. Awareness of the background rates of possible adverse events will be a crucial part of assessment of possible vaccine safety concerns and will help to separate legitimate safety concerns from events that are temporally associated with but not caused by vaccination. We identified background rates of selected medical events for several countries. Rates of disease events varied by age, sex, method of ascertainment, and geography. Highly visible health conditions, such as Guillain-Barre syndrome, spontaneous abortion, or even death, will occur in coincident temporal association with novel influenza vaccination. On the basis of the reviewed data, if a cohort of 10 million individuals was vaccinated in the UK, 21.5 cases of Guillain-Barre syndrome and 5.75 cases of sudden death would be expected to occur within 6 weeks of vaccination as coincident background cases. In female vaccinees in the USA, 86.3 cases of optic neuritis per 10 million population would be expected within 6 weeks of vaccination. 397 per 1 million vaccinated pregnant women would be predicted to have a spontaneous abortion within 1 day of vaccination.

In Andalusia, Spain, the pandemic influenza A(H1N1)v virus has spread throughout the community, being the dominant influenza strain in the season so far. The current objective of the Andalusia Health Service is focussed on the mitigation of the health and social impact by appropriate care of the patients at home or in health centres. The 2009-10 seasonal influenza epidemic started early compared with to previous seasons. This article analyses the influenza A(H1N1)v situation in Andalusia until the week 39/2009.

Background During the 2009 influenza pandemic, a change in the type of patients most often affected by influenza was observed. The objective of this study was to assess the role of individual and social determinants in hospitalizations due to influenza A (H1N1) 2009 infection. Methods We studied hospitalized patients (cases) and outpatients (controls) with confirmed influenza A (H1N1) 2009 infection. A standardized questionnaire was used to collect data. Variables that might be related to the hospitalization of influenza cases were compared by estimation of the odds ratio (OR) and 95% confidence intervals (CI) and the variables entered into binomial logistic regression models. Results Hospitalization due to pandemic A (H1N1) 2009 influenza virus infections was associated with non-Caucasian ethnicity (OR: 2.18, 95% CI 1.17 − 4.08), overcrowding (OR: 2.84, 95% CI 1.20 − 6.72), comorbidity and the lack of previous preventive information (OR: 2.69, 95% CI: 1.50 − 4.83). Secondary or higher education was associated with a lower risk of hospitalization (OR 0.56, 95% CI: 0.36 − 0.87) Conclusions In addition to individual factors such as comorbidity, other factors such as educational level, ethnicity or overcrowding were associated with hospitalization due to A (H1N1) 2009 influenza virus infections.

Objective. Describe acceptability of pandemic A(H1N1) influenza vaccination by Essential Community Workers (ECWs) from Alicante province (Spain) in January 2010. Evaluate the correlation with attitudes, beliefs, professional advice and information broadcasted by media. Method. In this cross-sectional study, face-to-face interviews were conducted with 742 ECWs to assess their attitudes towards vaccination against the pandemic influenza strain. A multivariable regression model was made to adjust the Odds Ratios (ORs). Results. Some ECWs reported having been vaccinated with seasonal vaccine, 21.5% (95%IC 18.6–24.9); only 15.4% (95%IC 12.8–18.4) with the pandemic one. ECWs vaccinated regularly against seasonal flu (OR 5.1; 95%IC 2.9–9.1), those who considered pandemic influenza as a severe or more serious disease than seasonal flu (OR 3.8; 95%IC 2.1–6.7) and those who never had doubts about vaccine safety (OR 3.7; 95%IC2.1–6.7) had a better acceptance of pandemic vaccine. Finally, 78.7% (95%IC 75.1–81.4) had doubts about pandemic vaccine's effectiveness. Conclusion. The vast amount of information provided by the media did not seem to be decisive to prevent doubts or to improve the acceptability of the vaccine in ECWs. Professional advice should be the focus of interest in future influenza vaccination campaigns. These results should be taken into account by health authorities.

The 2009 pandemic influenza A (H1N1) caused significant morbidity and mortality. Acute lung injury is the hallmark of the disease, but multiple organ system dysfunction can develop and lead to death. Therefore, we sought to investigate whether there was postmortem evidence of H1N1 presence and virus-induced organ injury in autopsy specimens. Five cases in which patients died of influenza A (H1N1) virus infection were studied. The lungs of all patients showed macroscopic and microscopic findings already described for H1N1 (consolidation, edema, hemorrhage, alveolar damage, hyaline membrane, and inflammation), and H1N1 viruses were present in alveolar cells in immunochemical studies. Acute tubular necrosis was present in all cases, but there was no evidence of direct virus-induced kidney injury. Nevertheless, H1N1 viruses were found in the cytoplasm of glomerular macrophages in the kidneys of 4 patients. Therefore, our data provide strong evidence that H1N1 presence is not restricted to the lungs.
