OBJECTIVE: to describe elderly mortality from cardiovascular and respiratory diseases and neoplasms in Marilia (SP). METHODS: this is a descriptive study of mortality from three diseases as defined by the 10th International Classification of Diseases, between 1998-2000 and 2005-2007. Mortality Information System records were used. Mortality rates by age and sex were calculated. RESULTS: circulatory diseases were the main causes of death among the elderly (39.25%). Neoplasm decline was noticed in both sexes and in those aged 60-69, particularly prostate cancer in men (-83.86%) and breast cancer (-70.96%) in this age group. Deaths from respiratory diseases increased in patients aged 80 and older: 39.31% in men and 57.92% in women. CONCLUSION: mortality from circulatory diseases and neoplasms among the elderly showed a decline, with increased mortality from respiratory system problems in patients 80 years of age and older.

Abusive consumption of alcohol leads to several negative consequences to health and quality of life, as it increases the frequency of diseases that cause death or functional disabilities. The rates of patients admitted to hospital due to physical problems stemming from alcohol abuse are high. This study aimed at identifying patients according to variables age, gender and education level as well as at evaluating the prevalence of CAGE-positive patients and morbidity due to CID-10 (International Classification of Diseases) by the World Health Organization. It is a cross-sectional, observational, exploratory, descriptive and quantitative study. It was conducted at the Botucatu School of Medicine University Hospital (HC) from July to September 2010, at the Internal Medicine ward. This ward aggregates 5 specialties, namely, Cardiology, Hematology, General Medicine, Nephrology and Gastroclinic. Presently, it has 36 beds, of which four are for Intensive Care Therapy (ICT). Three hundred and ten medical charts were analyzed for identification of morbidities and of variables age, gender and schooling. A closed semistructured questionnaire including, among other questions, the CAGE test was applied to each individual. The CAGE test is a questionnaire that comprises four questions and considers that one who answers two or more of such questions affirmatively is a suspected case of alcoholism. The total population studied included 310 patients. Indexes of 60% (186) male and of 40% (124) female patients were found. As regards age range, a population with a higher prevalence of individuals from 51 to 70 years old (41%) was observed. The main education indexes were: incomplete Elementary School: 23.22% (72), complete Elementary School: 21.61% (67), incomplete Secondary School: 18.40% (57) and Illiteracy: 17.74% (55). The purpose of the present study was to evaluate the incidence of dental anomalies in Brazilian patients with Down syndrome. A sample with 49 panoramic x-rays of syndromic patients aged 3 to 33 years (22 male and 27 female) was used. The characteristics of dental anomalies were observed in the panoramic radiographs in both the primary and permanent dentition, according to the ICD (International Classification of Diseases). The corresponding tables and percentile analysis were elaborated. There was a high incidence of syndromic patients with different types of anomalies, such as taurodontism (50%), proven anodontia (20.2%), suspected anodontia (10.7%), conic teeth (8.3%) and impacted teeth (5.9%). In conclusion, patients with Down syndrome presented a high incidence of dental anomalies and, in most cases, the same individual presented more than one dental anomaly.

Es werden Krankheitserleben und Behandlungsvoraussetzungen als Prädiktoren für den Erfolg stationärer Psychosomatischer Therapie anhand der vorliegenden Studie, die 125 Patienten einschliesst, überprüft. Dies geschieht mit Hilfe der Achse I der Operationalisierten Psychodynamischen Diagnostik (OPD).Die OPD wurde von der Arbeitsgruppe-OPD 1996 in Ergänzung zu der ICD-10 (International Classification of Diseases) herausgegeben und bietet für die Psyche betreffende Erkrankungen zusätzlich zu den rein deskriptiven Klassifikationen der ICD eine Klassifikationsmöglichkeit auf der Basis der Psychoanalyse und Psychodynamik an. Die erste der insgesamt fünf Achsen der OPD bezieht sich dabei auf das Krankheitserleben und die Behandlungsvoraussetzungen des Patienten.Der Erfolg der Therapie wird bis 1 Jahr nach dem Ende der Therapie zu drei verschiedenen Zeitpunkten durch Befragungen der Patienten erfasst. Die Ergebnisse zeigen für einzelne der Items der Achse I der OPD deutliche Zusammenhänge mit dem später eingetretenen Erfolg der Therapie. Dies gilt insbesondere für den Schweregrad des psychischen Befundes, den Leidensdruck, die Beeinträchtigung des Selbsterlebens, die Motivation zur Psychotherapie und die psychische Symptomdarbietung (Items 2, 3, 4, 11 und 15).Des weiteren werden zwei Faktorenlösungen erstellt, welche die 19 einzelnen Items der Achse I der OPD zu Faktoren zusammenfassen. Im Vergleich mit den bereits vorhandenen Faktorenlösungen zeigen sich aufgrund der Uneinheitlichkeit Probleme für eine mögliche Verwendung in der Praxis.

Patients with chronic pain disorders frequently show nondermatomal somatosensory deficits (NDSDs) that are considered to be functional. Typically, NDSDs show quadratomal or hemibody distribution ipsilateral to the areas of chronic pain. According to the Diagnostic and Statistical Manual of Mental Disorders, 4th edition and the International Classification of Diseases, 10th revision, such functional somatosensory deficits are classified in the chapter "conversion disorder." Many publications also used the term "hysterical sensory loss." However, doubts are increasing about this one-sided psychiatric view. We aimed to better characterize the biopsychosocial factors associated with NDSDs. Therefore, we compared 2 groups of inpatients with chronic pain disorder, of whom 90 suffered from NDSDs and 90 did not. The patients with NDSDs all showed widespread somatosensory deficits with hemibody distribution. On logistic regression analysis, history of a prior physical trauma was positively predictive for patients with NDSDs. Personality disorder and adverse childhood experiences were positively predictive for the control group with chronic pain disorders without NDSDs. The frequencies of comorbid depression and anxiety disorder did not differ statistically between groups. In conclusion, pain patients with NDSDs are, psychopathologically, by no means more noticeable personalities than patients with chronic pain disorder without NDSDs. Similar to complex regional pain syndromes, we assume a multifactorial etiology of NDSDs, including stress. Based on our observations, terms like "hysteric" should not be applied any longer to patients with NDSDs who suffer from chronic pain.

The U.S. Air Force assesses Active Duty Air Force (ADAF) health annually using the Air Force Web-based Preventative Health Assessment (AF WebPHA). The assessment is based on a self-administered survey used to determine the overall Air Force health and readiness, as well as, the individual health of each airman. Individual survey responses as well as groups of responses generate further computer generated assessment and result in a classification of 'Critical', 'Priority', or 'Routine', depending on the need and urgency for further evaluation by a health care provider. The importance of the 'Priority' and 'Critical' classifications is to provide timely intervention to prevent or limit unfavorable outcomes that may threaten an airman. Though the USAF has been transitioning from a paper form to the online WebPHA survey for the last three years it was not made mandatory for all airmen until 2009. The survey covers many health aspects including family history, tobacco use, exercise, alcohol use, and mental health. ^ Military stressors such as deployment, change of station, and the trauma of war can aggravate and intensify the common baseline worries experienced by the general population and place airmen at additional risks for mental health concerns and illness. This study assesses the effectiveness of the AF WebPHA mental health screening questions in predicting a mental health disorder diagnosis according to International Classification of Diseases, 9th Revision, Clinical Modification (ICD-9-CM) codes generated by physicians or their surrogates. In order to assess the sensitivity, specificity, and positive predictive value of the AF WebPHA as a screening tool for mental health, survey results were compared to ascertain if they generated any mental health disorder related diagnosis for the period from January 1, 2009 to March 31, 2010. ^ Statistical analysis of the AF WebPHA mental health responses when compared with matching ICD-9-CM codes found that the sensitivity for 'Critical' or 'Priority' responses was only 3.4% and that it would correctly predict those who had the selected mental health diagnosis 9% of the time.^

Narcolepsy with cataplexy is a rare disease with an estimated prevalence of 0.02% in European populations. Narcolepsy shares many features of rare disorders, in particular the lack of awareness of the disease with serious consequences for healthcare supply. Similar to other rare diseases, only a few European countries have registered narcolepsy cases in databases of the International Classification of Diseases or in registries of the European health authorities. A promising approach to identify disease-specific adverse health effects and needs in healthcare delivery in the field of rare diseases is to establish a distributed expert network. A first and important step is to create a database that allows collection, storage and dissemination of data on narcolepsy in a comprehensive and systematic way. Here, the first prospective web-based European narcolepsy database hosted by the European Narcolepsy Network is introduced. The database structure, standardization of data acquisition and quality control procedures are described, and an overview provided of the first 1079 patients from 18 European specialized centres. Due to its standardization this continuously increasing data pool is most promising to provide a better insight into many unsolved aspects of narcolepsy and related disorders, including clear phenotype characterization of subtypes of narcolepsy, more precise epidemiological data and knowledge on the natural history of narcolepsy, expectations about treatment effects, identification of post-marketing medication side-effects, and will contribute to improve clinical trial designs and provide facilities to further develop phase III trials.

Objective: The aim of the present study was to investigate if somatoform disorders (SFD) are associated with changes in the normal serum levels of important interleukins, and further, to establish if these changes are related to the presence and severity of alexithymia in patients with SFD. Methods: Twenty-four unmedicated patients who met the International Classification of Diseases (ICD-10) diagnostic criteria for SFD completed the psychological questionnaire to assess alexithymia (Toronto Alexithymia Scale), symptom reporting (SCL-90-R) and diagnostic criteria for SFD (Screening for Somatoform Symptoms scale). Serum concentrations of soluble interleukin 2 receptor α (sIL-2 Rα), IL-4, IL-6, IL-10 and IL-12 were determined in patients with SFD and in 9 healthy subjects. Results: In patients with SFD, serum levels of IL-6 (p < 0.001), IL-10 (p = 0.047) and immunoglobulin E (p = 0.045) were significantly increased in comparison with healthy controls. Additionally, a negative correlation was observed between the level of alexithymia ('total' Toronto Alexithymia Scale score) and the serum levels of sIL-2 Rα (r = -0.538) in SFD. Conclusions: Taken together, these results suggest that SFD, with clinically significant alexithymia, are associated with a reduction in Th1-mediated immune function and an increase in the activation of the Th2 immune function, indicated by the augmented serum levels of IL-6 and IL-10 and elevated immunoglobulin E. Background.  The optimum approach for infectious complication surveillance for cardiac implantable electronic device (CIED) procedures is unclear. We created an automated surveillance tool for infectious complications after CIED procedures. Methods.  Adults having CIED procedures between January 1, 2005 and December 31, 2011 at Duke University Hospital were identified retrospectively using International Classification of Diseases, 9th revision (ICD-9) procedure codes. Potential infections were identified with combinations of ICD-9 diagnosis codes and microbiology data for 365 days postprocedure. All microbiology-identified and a subset of ICD-9 code-identified possible cases, as well as a subset of procedures without microbiology or ICD-9 codes, were reviewed. Test performance characteristics for specific queries were calculated. Results.  Overall, 6097 patients had 7137 procedures. Of these, 1686 procedures with potential infectious complications were identified: 174 by both ICD-9 code and microbiology, 14 only by microbiology, and 1498 only by ICD-9 criteria. We reviewed 558 potential cases, including all 188 microbiology-identified cases, 250 randomly selected ICD-9 cases, and 120 with neither. Overall, 65 unique infections were identified, including 5 of 250 reviewed cases identified only by ICD-9 codes. Queries that included microbiology data and ICD-9 code 996.61 had good overall test performance, with sensitivities of approximately 90% and specificities of approximately 80%. Queries with ICD-9 codes alone had poor specificity. Extrapolation of reviewed infectious rates to nonreviewed cases yields an estimated rate of infection of 1.3%. Conclusions.  Electronic queries with combinations of ICD-9 codes and microbiologic data can be created and have good test performance characteristics for identifying likely infectious complications of CIED procedures.

Being able to accurately predict the risk of falling is crucial in patients with Parkinson’s dis- ease (PD). This is due to the unfavorable effect of falls, which can lower the quality of life as well as directly impact on survival. Three methods considered for predicting falls are decision trees (DT), Bayesian networks (BN), and support vector machines (SVM). Data on a 1-year prospective study conducted at IHBI, Australia, for 51 people with PD are used. Data processing are conducted using rpart and e1071 packages in R for DT and SVM, con- secutively; and Bayes Server 5.5 for the BN. The results show that BN and SVM produce consistently higher accuracy over the 12 months evaluation time points (average sensitivity and specificity > 92%) than DT (average sensitivity 88%, average specificity 72%). DT is prone to imbalanced data so needs to adjust for the misclassification cost. However, DT provides a straightforward, interpretable result and thus is appealing for helping to identify important items related to falls and to generate fallers’ profiles.
