Esta pesquisa tem como objeto de estudo a análise do processo de acompanhamento dos clientes portadores de feridas na atenção primária do município de Angra dos Reis. Os objetivos são: verificar a existência de processo de acompanhamento de clientes portadores de feridas nas unidades de atenção primária do município de Angra dos Reis; analisar as dificuldades inerentes ao processo de acompanhamento de clientes portadores de feridas nas unidades de atenção primária do município de Angra dos Reis. O método foi descritivo, o tipo é de levantamento e de natureza quantitativa. O instrumento de coleta de dados foi o formulário, tendo como sujeitos do estudo a população de enfermeiros atuantes na atenção primária do município de Angra dos Reis e que assinaram o Termo de Consentimento Livre e Esclarecido, conforme disposto na Resolução 196/96 do Conselho Nacional de saúde. O cenário utilizado foi o município de Angra dos Reis. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa do Instituto de Medicina Social da UERJ, através da Plataforma Brasil em 28/06/2013. A análise demonstrou que mais de 93% dos enfermeiros realizam curativos durante sua atuação profissional na atenção primária, destes, 80% realizam algum tipo de acompanhamento de clientes portadores de feridas. Este acompanhamento nem sempre é contínuo, por contada dificuldade técnica do próprio profissional, da interrupção do fornecimento de materiais por parte do almoxarifado, da baixa adesão do cliente e da inexistência de uma rotina institucionalizada. A insuficiência de produtos disponíveis no município para a realização de curativos também foi um fator descrito pelos sujeitos como prejudicador no processo de acompanhamento destes pacientes. Foi verificado que a utilização de produtos de segunda geração para realização de curativos, quando indicados de forma correta e respeitando o prazo de troca, proporciona uma economia nos fastos do município no que se refere ao tratamento tópico de feridas, economia esta que pode chegar à metade dos gastos. Este estudo foi relevante para a otimização do processo de acompanhamento de clientes portadores de feridas no município, bem como reorganização defluxos, rotinas e do cuidado prestado.

A aplicação tópica de doadores de óxido nítrico é conhecida pelos seus efeitos benéficos no reparo tecidual cutâneo. O objetivo deste trabalho foi avaliar os efeitos da associação de biomateriais com doadores de óxido nítrico no reparo tecidual cutâneo de camundongos. Camundongos swiss machos foram submetidos a lesões excisionais por punch de biópsia de 8 mm no dorso. Os animais foram separados em 4 grupos (n=6 em cada grupo) de acordo com a aplicação do curativo de polivinil álcool e hidrogel sobre a lesão de punch: a) grupo polivinil álcool com grupos tiol e Pluronic F-127 (PVA-SH/F127) - tratado com hidrogel sem S- nitrosoglutationa (GSNO) e filme sem óxido nítrico; b) grupo polivinil álcool S-nitrosado (PVA-SNO/F127-GSNO) - tratado com hidrogel contendo GSNO e filme com óxido nítrico; c) grupo PVA-SNO - tratado apenas com filme óxido nítrico e d) grupo PVA-SNO/ F127 - tratado com hidrogel sem GSNO e filme com óxido nítrico. Os animais foram tratados por 7 dias consecutivos com aplicação diária de curativos com seus respectivos biomateriais. Após 7 dias de tratamento, foram retirados os curativos e as lesões foram deixadas cicatrizar por segunda intenção. O grupo tratado com filme de PVA-SNO (d) associado ao hidrogel F127, comparado com os demais grupos descritos acima, apresentou melhora no reparo tecidual, melhora da contração da lesão, diminuição do gap epitelial e densidade celular, aceleração da fase inflamatória, aumento da diferenciação miofibroblástica e aumento da expressão de colágeno do tipo III (p<0,05, ao menos). Com base nesses dados, a combinação de filmes PVA liberadores de óxido nítrico com F-127 pode representar uma nova abordagem para o tratamento de lesões cutâneas.

L’étude de la guérison des plaies à l’aide de substituts produits par génie tissulaire est un domaine en plein essor. Dans ces travaux, les effets de pansements biologiques produits en laboratoire à partir de cellules souches/stromales du tissu adipeux (CSTA) différenciées ou non en adipocytes ont été évalués sur des plaies cutanées in vivo. Un modèle de souris possédant un épiderme fluorescent a permis de démontrer que les plaies traitées avec les pansements biologiques guérissent plus rapidement que les plaies non traitées, et ce, de manière indépendante de la réépithélialisation. Une augmentation de la formation du tissu de granulation et une angiogenèse accrue ont également été observées dans les groupes traités. Ces résultats établissent que les substituts contenant des CSTA ou des adipocytes fonctionnels favorisent la réparation tissulaire. À terme, ces travaux pourraient mener au développement de nouvelles indications cliniques pour le traitement des ulcères cutanés.

We have developed novel composites by grafting caffeic acid (CA) onto the P(3HB)-EC based material and laccase from Trametes versicolor was used for grafting purposes. The resulting composites were designated as CA-g-P(3HB)-EC i.e., P(3HB)-EC (control), 5CA-g-P(3HB)-EC, 10CA-g-P(3HB)-EC, 15CA-g-P(3HB)-EC and 20CA-g-P(3HB)-EC. An FT-IR (Fourier-transform infrared spectroscopy) was used to examine the functional and elemental groups of the control and laccase-assisted graft composites. Evidently, 15CA-g-P(3HB)-EC composite exhibited resilient antibacterial activity against Gram-positive and Gram-negative bacterial strains, respectively. Moreover, a significant level of biocompatibility and biodegradability of the CA-g-P(3HB)-EC composites was also achieved with the human keratinocytes-like HaCaT cells and soil burial evaluation, respectively. In conclusion, the newly developed novel composites with multi characteristics could well represent the new wave of biomaterials for medical applications, and more specifically have promising future in the infection free would dressings, burn and/or skin regeneration field due to their sophisticated characteristics.

CASCO (Canada Starch Company) began operations in 1982 and was officially opened in May of 1983. Premier William Davis was in attendance. CASCO is a company whose roots can be traced back to 1858 when it was founded by W.T. Benson in Cardinal, Ontario. The company grew as corn uses were developed. Corn derived products now include: corn oil, liquid sweetener and feed for dairy and cattle. A grande diversidade de apósitos disponível para a recuperação da integridade cutânea torna complexa e difícil a sua escolha. Torna-se pois fundamental, aprofundar o conhecimento sobre o impacto específico dos diferentes materiais de penso. O presente estudo pretende ilustrar o impacto da oclusão na recuperação da função de “barreira” da pele e envolveu 8 mulheres saudáveis ( = 22,6±1,1) em cujos antebraços foram marcados 3 sítios experimentais sujeitos a biopsia superficial cutânea (BSC) com cianoacrilato, mantendo integro um quarto local, assim utilizado como controlo negativo (CN). Dois dos locais experimentais sujeitos a BSC foram aleatoriamente ocludidos com o apósito de hidroxipoliuretano (PermaFoam®, Hartmann)(sitio A) ou com parafilm (sitio B) e o terceiro local (sitio C) deixado sem oclusão, funcionando como controlo positivo (CP). As variáveis em estudo foram a Perda Transepidérmica de água (PTEA, Tewameter TM300), o eritrema (Minolta CR3000) e a microcirculação local (LDF Periflux). Os resultados sugerem uma mais rápida recuperação da integridade cutânea nos sítios ocludidos, quando comparados com o respectivo controle, salientando-se nestes, o local tratado com o apósito de hidroxipoliuretano. Conclui-se que, nas presentes condições experimentais, a oclusão determina uma maior e mais rápida recuperação da integridade cutânea in vivo.

There has been a recent surge in the use of silver as an antimicrobial agent in a wide range of domestic and clinical products, intended to prevent or treat bacterial infections and reduce bacterial colonization of surfaces. It has been reported that the antibacterial and cytotoxic properties of silver are affected by the assay conditions, particularly the type of growth media used in vitro. The toxicity of Ag+ to bacterial cells is comparable to that of human cells. We demonstrate that biologically relevant compounds such as glutathione, cysteine and human blood components significantly reduce the toxicity of silver ions to clinically relevant pathogenic bacteria and primary human dermal fibroblasts (skin cells). Bacteria are able to grow normally in the presence of silver nitrate at >20-fold the minimum inhibitory concentration (MIC) if Ag+ and thiols are added in a 1:1 ratio because the reaction of Ag+ with extracellular thiols prevents silver ions from interacting with cells. Extracellular thiols and human serum also significantly reduce the antimicrobial activity of silver wound dressings Aquacel-Ag (Convatec) and Acticoat (Smith & Nephew) to Staphylococcus aureus, Pseudomonas aeruginosa and Escherichia coli in vitro. These results have important implications for the deployment of silver as an antimicrobial agent in environments exposed to biological tissue or secretions. Significant amounts of money and effort have been directed at the development of silver-coated medical devices (e.g. dressings, catheters, implants). We believe our findings are essential for the effective design and testing of antimicrobial silver coatings.

Hydrogels have become very popular due to their unique properties such as high water content, softness, flexibility and biocompatibility. Natural and synthetic hydrophilic polymers can be physically or chemically cross-linked in order to produce hydrogels. Their resemblance to living tissue opens up many opportunities for applications in biomedical areas. Currently, hydrogels are used for manufacturing contact lenses, hygiene products, tissue engineering scaffolds, drug delivery systems and wound dressings. This review provides an analysis of their main characteristics and biomedical applications. From Wichterle’s pioneering work to the most recent hydrogel-based inventions and products on the market, it provides the reader with a detailed introduction to the topic and perspective on further potential developments.

In this paper, we report the synthesis and healing ability of a non-cytotoxic supramolecular polyurethane network whose mechanical properties can be recovered efficiently (> 99%) at the temperature of the human body (37 ºC). Rheological analysis revealed an acceleration in the drop of the storage modulus above 37 ºC, on account of the dissociation of the supramolecular polyurethane network, and this decrease in viscosity enables the efficient recovery of the mechanical properties. Microscopic and mechanical characterisation has shown that this material is able to recover mechanical properties across a damage site with minimal contact required between the interfaces and also demonstrated that the mechanical properties improved when compared to other low temperature healing elastomers or gel-like materials. The supramolecular polyurethane was found to be non-toxic in a cytotoxicity assay carried out in human skin fibroblasts (cell viability > 94% and non-significantly different compared to the untreated control). This supramolecular network material also exhibited excellent adhesion to pig skin and could be healed completely in situ post damage indicating that biomedical applications could be targeted, such as artificial skin or wound dressings with supramolecular materials of this type.

A venous ulcer is the most serious clinical manifestation of chronic venous failure, and the most frequent in lower limbs, accounting for 70% of all ulcers. Patients may suffer from this infirmity for several years without healing of the lesion if treatment is inadequate. The aim of this investigation was to verify the effectiveness of decongestive physical therapy in the healing of venous ulcers. This is a quasi-experimental, interventionist study, with paired, non-probabilistic sampling, composed of 50 patients divided into two groups: control and intervention, each composed of 25 patients. Both groups were identically treated for six months with daily dressings and the latter also underwent complex physical therapy consisting of a combination of the following techniques: manual lymphatic drainage, compression bandaging, lower limb elevation, myolymphokinetic exercises and skin care. The study was approved (Protocol no. 59/2007) by the Ethics Committee of the State University of Southeast Bahia. The Mann-Whitney and Chi-square tests were applied for data analysis. After statistical analysis the patients who underwent therapy showed a statistically significant difference with respect to wound contraction starting in the second month of treatment. Compared to the control, the intervention group showed a greater reduction in both pain and edema starting in the third and fourth month of therapy, respectively. To reinforce these findings, the mean percentage of tissue present at the base of the ulcer (granulation/fibrin ratio) was calculated. The intervention group showed greater granulation at the base of the ulcer compared to the control, significant from the second month of treatment on. It was observed in this study that lymphotherapy, when compared between the intervention and control groups, accelerated the healing process, and reduced pain and edema in the affected limbs. It is expected, therefore, that these results widen scientific knowledge and we suggest that this therapy be used 78 not only to reduce lymphedema, but also as a treatment option for venous ulcers, given its easy application and low cost. The data, therefore, demonstrated the importance of basic care on the part of a multiprofessional and transdisciplinary health team involved in the healing process of these infirmities, thereby contributing to better quality of life in these individuals

Venous ulcers (VU), recurrent chronic wounds resulting from Chronic Venous Insufficiency (CVI), affect different age groups and would severely affect ambulation of patients. The lesions require treatment lasting and complex and are responsible for significant morbidity and mortality. Thus, this study aims to identify the important aspects covered in the scientific literature protocol for assisting patients with venous ulcers, identifying the issues to be proposed by the judges of the study (nurses, doctors and physiotherapists) to the protocol of care provided to patients venous ulcers and present the structure of protocol proposed by the judges of the study to assist patients with venous ulcers treated at a referral hospital of Rio Grande do Norte. This is a descriptive study using a quantitative approach, carried out at the dressings, located in the outpatient surgical clinic of the Hospital University Onofre Lopes (HUOL), located in East Sanitary District, Natal-RN. The sample consisted of 39 professionals, 30 nurses, seven doctors and two physical therapists, team members HUOL surgical clinic and other public and private institutions of Rio Grande do Norte and Jequié/Bahia. These professionals were the judges responsible for selecting the guidelines already proposed in the literature on VU protocols. Approved by the Ethics in Research HUOL (Report n.o 081/07), began the first stage of the study which consisted of reviewing the scientific literature about the relevant aspects to be included in a protocol for assisting patients with VU. These aspects were organized into a proposed questionnaire to the judges of the study. Following examination, held on the content validation with application of the Kappa (K), accepting a score higher than 0.80 and the Likert Scale, whereas rates from 4.0 to 5.0. The data collected were organized in Microsoft Excel and exported into Statistical Package for Social Sciences (SPSS) 15.0. The literature review included national and international scientific articles, thesis, dissertation and institutional protocols. Regarding the characterization of professional nurses predominated (76.1%), between 34 and 45 years (41.0%), female (79.5%), married/consensual union (46.2%), with specialization in VU care (61.5%), working in the hospital network (46.1%), with up to 5 years experience in VU (69.2%) and claiming to feel prepared to care for these injuries (92.3 %). With regard to aspects that had very good agreement (K ≥ 0.81), remained the items found in the literature with some modifications. In the analysis of the proposed evaluation items had very important, ranging from 4.1 (drug treatment) to 4.9 (patient assessment and care of the injury and the injured and perilesional skin).
