Objectives: Laryngo-hypopharyngeal sensitivity (LPS) as measured by thresholds to mechanostimulation and chemostimulation is important in the prevention of pulmonary aspiration. The presence of gastroesophageal reflux disease (GERD) increases thresholds to mechanostimulation. However, the effect of GERD on thresholds to chemostimulation remains unknown. The aim of this study was to compare laryngo-hypopharyngeal thresholds to chemostimulation in subjects with GERD with those of healthy subjects and to determine the relationship between thresholds to mechanostimulation and chemostimulation.

Deep vein thrombosis (DVT) presents a significant risk for women requiring Caesarean section.
Venous thromboembolism (VTE) remains the leading direct cause of maternal death in the
UK and is the second most common cause of maternal death worldwide, accounting for 11%
of maternal deaths (RCOG 2009). This article uses a reflective cycle to explore the role of the
preoperative practitioner in the prevention of DVT for women undergoing elective Caesarean
section.

Constipation is one of the most common digestive complaints. It is a symptom, not a disease. The subjectivity that this involves means that assessments of clinical epidemiology, socioeconomic costs and pharmacotherapy are difficult, since there is no definition of 'normal' bowel habit. Although constipation can affect all ages, the problem increases with age, and is of particular concern for those who are frail and in long term care. Cultural influences may affect the prevalence in older people. Drug therapy of constipation cannot be considered in isolation, since there are issues in the prevention of constipation and the principles of good management that also apply. Furthermore, some consideration of the pathophysiology and diagnosis is important. This is because a number of remediable causes can be identified, and the diagnostic process involves patient education, which in turn may be effective in reducing costs. It is the complaint of constipation which leads either to self-medication or to consultation with the medical profession. Both of these courses of action have a significant influence on utilisation of laxatives (cathartics), obtained both over-the-counter and by prescription. Although there are a large number of laxative preparations available, therapy has changed little in half a century. Costs may vary considerably, and with such a significant problem there is a need for comparative studies. However, study methodologies are difficult, and a significant placebo response may be found. Education and preventive measures have been shown to reduce laxative use and costs in institutions. Unfortunately, there are few comparative studies of individual laxatives and even fewer cost-effectiveness studies. Those that there are have been based in institutions, and so extrapolation to other situations may be difficult. In general, little attention is given to constipation. It is, however, an area with significant resource implications in which education and preventive measures have been shown to be beneficial. Even so, there is still a need for good comparative studies, particularly where cost effectiveness is concerned.

Objective: to identify non-invasive interventions in the perinatal period that could enable midwives to offer effective support to women within the area of maternal mental health and well-being.

Methods: a total of 9 databases were searched: MEDLINE, PubMed, EBSCO (CINAHL/British Nursing Index), MIDIRS Online Database, Web of Science, The Cochrane library, CRD (NHS EED/DARE/HTA), Joanne Briggs Institute and EconLit. A systematic search strategy was formulated using key MeSH terms and related text words for midwifery, study aim, study design and mental health. Inclusion criteria were articles published from 1999 onwards, English language publications and articles originating from economically developed countries, indicated by membership of the Organisation for Economic Co-operation and Development (OECD). Data were independently extracted using a data collection form, which recorded data on the number of papers reviewed, time frame of the review, objectives, key findings and recommendations. Summary data tables were set up outlining key data for each study and findings were organised into related groups. The methodological quality of the reviews was assessed based on predefined quality assessment criteria for reviews.

Findings: 32 reviews were identified as examining interventions that could be used or co-ordinated by midwives in relation to some aspect of maternal mental health and well-being from the antenatal to the postnatal period and met the inclusion criteria. The review highlighted that based on current systematic review evidence it would be premature to consider introducing any of the identified interventions into midwifery training or practice. However there were a number of examples of possible interventions worthy of further research including midwifery led models of care in the prevention of postpartum depression, psychological and psychosocial interventions for treating postpartum depression and facilitation/co-ordination of parent-training programmes. No reviews were identified that supported a specific midwifery role in maternal mental health and well-being in pregnancy, and yet, this is the point of most intensive contact.

Key conclusions and implications for practice: This systematic review of systematic reviews provides a valuable overview of the current strengths and gaps in relation to maternal mental health interventions in the perinatal period. While there was little evidence identified to inform the current role of midwives in maternal mental health, the review provides the opportunity to reflect on what is achievable by midwives now and in the future and the need for high quality randomised controlled trials to inform a strategic approach to promoting maternal mental health in midwifery.

The IDS (Intrusion Detection System) is a common means of protecting networked systems from attack or malicious misuse. The development and rollout of an IDS can take many different forms in terms of equipment, protocols, connectivity, cost and automation. This is particularly true of WIDS (Wireless Intrusion Detection Systems) which have many more opportunities and challenges associated with data transmission through an open, shared medium.
The operation of a WIDS is a multistep process from origination of an attack through to human readable evaluation. Attention to the performance of each of the processes in the chain from attack detection to evaluation is imperative if an optimum solution is to be sought. At present, research focuses very much on each discrete aspect of a WIDS with little consideration to the operation of the whole system. Taking a holistic view of the technology shows the interconnectivity and inter-dependence between stages, leading to improvements and novel research areas for investigation.
This chapter will outline the general structure of Wireless Intrusion Detection Systems and briefly describe the functions of each development stage, categorised into the following 6 areas:
• Threat Identification,
• Architecture,
• Data Collection,
Intrusion Detection,
• Alert Correlation,
• Evaluation.
These topics will be considered in broad terms designed for those new to the area. Focus will be placed on ensuring the readers are aware of the impact of choices made at early stages in WIDS development on future stages.

Heparin is ubiquitously employed in surgery in the prevention and treatment of thromboembolic disorders. A small but significant number of patients receiving heparin develop thrombocytopenia and are at risk of both serious thromboembolic disease and haemorrhage. A case is reported where a young woman developed life-threatening postoperative haemorrhage associated with heparin induced thrombocytopenia (HIT).

Biomaterial-related infections are a persistent burden on patient health, recovery, mortality and healthcare budgets. Self-assembled antimicrobial peptides have evolved from the area of antimicrobial peptides. Peptides serve as important weapons in nature, and increasingly medicine, for combating microbial infection and biofilms. Self-assembled peptides harness a “bottom-up” approach, whereby the primary peptide sequence may be modified with natural and unnatural amino acids to produce an inherently antimicrobial hydrogel. Gelation may be tailored to occur in the presence of physiological and infective indicators (e.g. pH, enzymes) and therefore allow local, targeted antimicrobial therapy at the site of infection. Peptides demonstrate inherent biocompatibility, antimicrobial activity, biodegradability and numerous functional groups. They are therefore prime candidates for the production of polymeric molecules that have the potential to be conjugated to biomaterials with precision. Non-native chemistries and functional groups are easily incorporated into the peptide backbone allowing peptide hydrogels to be tailored to specific functional requirements. This article reviews an area of increasing interest, namely self-assembled peptides and their potential therapeutic applications as innovative hydrogels and biomaterials in the prevention of biofilm-related infection.

OBJECTIVE: To investigate the impact of tooth replacement on the nutritional status of partially dentate older patients, and, to compare two different tooth replacement strategies; conventional treatment using removable partial dentures and functionally orientated treatment based on the shortened dental arch.

BACKGROUND: Amongst older patients, diet plays a key role in disease prevention, as poor diets have been linked to numerous illnesses. Poor oral health and loss of teeth can have very significant negative effects on dietary intake and nutritional status for elderly patients. There is evidence that good oral health generally, has positive effects on the nutritional intake of older adults.

MATERIALS AND METHODS: A randomised, controlled clinical trial was designed to investigate the impact of tooth replacement on the nutritional status of partially dentate elders. Forty-four patients aged over 65 years completed the trial, with 21 allocated to conventional treatment and 23 allocated to functionally orientated treatment. Nutritional status was accessed at baseline and after treatment using the Mini Nutritional Assessment (MNA) and a range of haematological markers.

RESULTS: At baseline, relationships were observed between the number of occluding tooth contacts and some measures of nutritional status. As the number of contacts increased, MNA scores (R = 0.16), in addition to vitamin B12 (R = 0.21), serum folate (R = 0.32) and total lymphocyte count (R = 0.35), also increased. After treatment intervention, the only measure of nutritional status that showed a statistically significant improvement for both treatment groups was MNA score (p = 0.03). No significant between group differences were observed from analysis of the haematological data.

CONCLUSION: In this study, prosthodontic rehabilitation with both conventional treatment and functionally orientated treatment resulted in an improvement in MNA score. Haematological markers did not illustrate a clear picture of improvement in nutritional status for either treatment group.

Consumer studies and market reports show an increase in consumption of ready-to-eat (RTE) foods. Although conventional processing technologies can in most cases produce safe products, they can also lead to the degradation of nutritional compounds and negatively affect quality characteristics. Consumers strongly prefer food that is minimally processed with the maximum amount of health-promoting substances. Novel processing technologies as pre- or post-treatment decontamination methods or as substitutes of conventional technologies have the potential to produce foods that are safe, rich in nutrient content and with superior organoleptic properties. Combining novel with conventional processes can eliminate potential drawbacks of novel technologies. This review examines available scientific information and critically evaluates the suitability and efficiency of various novel thermal and nonthermal technologies in terms of microbial safety, quality as well as nutrient content on the production of RTE meals, meats and pumpable products.

The topic "nutrition and the eye" cannot adequately be covered in a single review article; indeed, dozens of books and hundreds of articles have been written on the subject. This review concentrates on three areas in which specific nutrients are known or theorized to have a major impact on vision and the visual system: vitamin A deficiency; antioxidants and their proposed role in the prevention of age-related cataract and macular degeneration; and nutritional optic neuropathies, including those of the recent Cuban epidemic. In addition, this article touches on nutritional treatments that have been suggested for several less common eye diseases and, finally, considers several less prevalent conditions in which deficiency of or excess exposure to a particular nutrient has been associated with ocular pathology.

Desde a Pré-História que a escolha de materiais esteve relacionada com a Arte. Mais tarde, durante a Idade Moderna vai ganhando uma importância cada vez maior. Atingida que foi a Idade Contemporânea, nomeadamente após a Revolução Industrial e durante a Segunda Guerra Mundial, devido ao aumento do número de materiais disponíveis, é que se pode falar de uma verdadeira seleção de materiais. É também após a Revolução Industrial que se clarificam as relações entre a evolução dos materiais e os movimentos e correntes das Artes Plásticas. Neste contexto, estudaram-se as interligações entre o processo de design e as metodologias de seleção, assim como as diversas tipologias de ferramentas existentes para esse efeito. Deste estudo, consideradas as respetivas vantagens e limitações, foi possível identificar bases de dados essencialmente técnicas, ou ao invés, ferramentas para inspiração com muitas imagens e pouca informação sobre as propriedades dos materiais. Para completar este levantamento crítico sobre processos e ferramentas de seleção, inquiriram-se cinquenta e três profissionais que trabalhavam em diferentes gabinetes de design portugueses. As perguntas dirigidas aos designers portugueses versaram sobre problemas relacionados com a escolha de materiais, abrangendo o tipo de matériasprimas empregues, processos utilizados e a qualidade da informação obtida. Na sequência deste estudo, verificou-se a existência de diversas lacunas relativamente aos meios disponíveis, rotinas de seleção, qualidade da informação existente e metodologias utilizadas. Foi neste contexto que se iniciou o projeto de criação de uma nova metodologia suportada por uma ferramenta digital. Os principais aspetos inovadores são: uma melhor interligação entre a metodologia de design e o processo de seleção de materiais e a sua sincronização; a informação necessária em cada etapa e o destaque dos fatores catalisadores da seleção de materiais. Outro elemento inovador foi a conjugação de três formas deferentes de seleção de materiais numa só ferramenta (a geral, a visual e a específica) e a hipótese de aceder a diferentes graus de informação. A metodologia, no contexto dos recursos disponíveis, foi materializada sob a forma de ferramenta digital (ptmaterials.com). O protótipo foi aferido com testes de usabilidade de cariz heurístico, com a participação de dezanove utilizadores. Foram detetadas diversas falhas de interação que condicionaram a liberdade e o controlo da navegação no seio da interface. Os utilizadores também mencionaram a existência de lacunas na prevenção de erros e a ligação do sistema à lógica habitual de outras aplicações já existentes. No entanto, também constituiu um estímulo a circunstância da maioria dos designers avaliarem o sistema como eficaz, eficiente, satisfatório e confirmarem o interesse da existência dos três tipos de seleção. Posteriormente, ao analisar os restantes resultados dos testes de usabilidade, também foram evidenciadas as vantagens dos diferentes tipos de informação disponibilizada e a utilidade de uma ferramenta desta natureza para a Indústria e Economia Nacionais. Esta ferramenta é apenas um ponto de partida, existindo espaço para melhorar a proposta, apesar da concretização de uma ferramenta digital ser um trabalho de grande complexidade. Não obstante se tratar de um protótipo, esta ferramenta está adequada aos dias de hoje e é passível de evoluir no futuro, tendo também a possibilidade de vir a ser preferencialmente utilizada por outros países de língua portuguesa.

O presente trabalho resulta do estudo que procurou analisar a reconfiguração das práticas educativas na prevenção do stresse na infância. O principal objetivo foi o de analisar de que forma os educadores podem desenvolver práticas educativas, estruturadoras e estruturantes do bem-estar da criança, investindo intencionalmente no desenvolvimento de atividades educativas, de modo a prevenir os índices de stresse das crianças, na educação pré-escolar. Foram realizados cinco estudos complementares, de natureza quantitativa e qualitativa. O estudo 1: contextualização e caracterização de indicadores sociofamiliares de crianças que frequentam a educação pré-escolar; Estudo 2: adaptação/avaliação do PKBS-2 de Merrell, para identificação das aptidões sociais e os problemas de comportamento em crianças dos 3 aos 6 anos numa amostra portuguesa (N=150) e estudos comparativos Portugal/Brasil (N=300) e Portugal/Cabo-Verde (N=150); Estudo 3: validação da Escala Comportamental para crianças em idade Pré-Escolar – PKBSpt, versão portuguesa do PKBS-2 em crianças dos 2 aos 7 anos (N=581); Estudo 4: contributos para a reconfiguração das práticas pedagógicas e o stresse na infância sob o olhar dos educadores. No âmbito deste estudo 4, desenvolvemos e validamos o Protocolo para a Avaliação do Stresse na Infância–PASI, constituído por três subescalas que aplicamos a educadores ou equiparados (N=188): ESISI, EPELSI e ECPLSI; Estudo 5: análise das competências dos profissionais da educação ao nível da ansiedade (IAB de Beck) e estratégias de coping; (EC de Gomes & Pereira). Para análise estatística dos dados utilizamos o programa SPSS e o Excel. Os resultados evidenciaram a validade e fidelidade dos instrumentos: ESISI, EPELSI, ECPLSI e PKBSpt, bem como a validação para a língua portuguesa deste último. A variável género apresentou diferenças estatisticamente significativas nas aptidões sociais e problemas de comportamento, no entanto não estão correlacionadas com os problemas de ansiedade. Os Educadores focalizam a sua praxis educativa na observação, identificação e definição de estratégias orientadas para o stresse na infância e, ainda, na cooperação escola/família e na prevenção das situações indutoras de stresse. Contudo apontam algumas fragilidades na formação básica e contínua, para lidarem com o stresse na primeira infância. As causas psicossociais são preditoras das situações indutoras de stresse na infância e são explicadas pelas causas externas de componente escolar, ou seja, as perturbações de ansiedade na infância poderão ter repercussões no contexto escolar. Importa que a comunidade educativa perspetive particular atenção às crianças que estão em situação de maior vulnerabilidade, de modo a prevenir e a intervir nas situações de stresse na educação pré-escolar. São referidas implicações psicopedagógicas deste estudo.
