Sirt3 is a mitochondrial NAD(+)-dependent deacetylase that governs mitochondrial metabolism and reactive oxygen species homeostasis. Sirt3 deficiency has been reported to accelerate the development of the metabolic syndrome. However, the role of Sirt3 in atherosclerosis remains enigmatic. We aimed to investigate whether Sirt3 deficiency affects atherosclerosis, plaque vulnerability, and metabolic homeostasis. Low-density lipoprotein receptor knockout (LDLR(-/-)) and LDLR/Sirt3 double-knockout (Sirt3(-/-)LDLR(-/-)) mice were fed a high-cholesterol diet (1.25 % w/w) for 12 weeks. Atherosclerosis was assessed en face in thoraco-abdominal aortae and in cross sections of aortic roots. Sirt3 deletion led to hepatic mitochondrial protein hyperacetylation. Unexpectedly, though plasma malondialdehyde levels were elevated in Sirt3-deficient mice, Sirt3 deletion affected neither plaque burden nor features of plaque vulnerability (i.e., fibrous cap thickness and necrotic core diameter). Likewise, plaque macrophage and T cell infiltration as well as endothelial activation remained unaltered. Electron microscopy of aortic walls revealed no difference in mitochondrial microarchitecture between both groups. Interestingly, loss of Sirt3 was associated with accelerated weight gain and an impaired capacity to cope with rapid changes in nutrient supply as assessed by indirect calorimetry. Serum lipid levels and glucose tolerance were unaffected by Sirt3 deletion in LDLR(-/-) mice. Sirt3 deficiency does not affect atherosclerosis in LDLR(-/-) mice. However, Sirt3 controls systemic levels of oxidative stress, limits expedited weight gain, and allows rapid metabolic adaptation. Thus, Sirt3 may contribute to postponing cardiovascular risk factor development.

A four compartment model of the cardiovascular system is developed. To allow for easy interpretation and to minimise the number of parameters, an effort was made to keep the model as simple as possible. A sensitivity analysis is first carried out to determine which are the most important model parameters to characterise the blood pressure signal. A four stage process is then described which accurately determines all parameter values. This process is applied to data from three patients and good agreement is shown in all cases.

Purpose: to assess the prevalence and trends of low cardiovascular risk factor (RF) profile in the Swiss population according to different definitions. Methods: Population-based cross-sectional study of 6170 subjects (3241 women) aged 35-75 years living in Lausanne, Switzerland. Trends were assessed using data from the Swiss MONICA population surveys conducted in 1984-6 (N = 3300), 1988-9 (N = 3331) and 1992-3 (N = 3133) and restricted to the same age group. Seven different definitions of low RF profile were used. Results: prevalence of low RF profile varied between 6.5% (95% confidence interval: 5.9-7.1) and 9.7% (9.0-10.5) depending on the definition used (see fig. 1). The prevalence was inversely related to the number of criteria used and higher than in other countries. Irrespective of the definition used, the prevalence of low RF profile was higher in women and in physically active participants, and decreased with increasing age or in the presence of a family history of cardiovascular disease. The prevalence of low RF profile increased from 3.8% (3.1- 4.5) in 1984-6 to 6.7% (6.1-7.3) in 2003-6; using another definition, the results were 5.9% (5.1-6.8) and 9.7% (9.0-10.5), respectively (see fig. 2). Conclusion: the prevalence of low RF profile varies according to the criteria used; this prevalence is relatively high and increasing in the Swiss population, which might partly explain the low and decreasing trend in cardiovascular mortality rates.

Este trabalho mostra as expectativas de estudantes de enfermagem quanto ao ensino de reabilitação durante o curso de graduação em enfermagem. Alunos do oitavo semestre e do segundo semestre do curso de Graduação em Enfermagem responderam um questionário específico para cada grupo e apontaram suas expectativas caracterizando uma proposta para o ensino de reabilitação em enfermagem.

Antecedentes: Existe la percepción de que los pacientes con trastorno mental severo (TMS) presentan una mayor prevalencia de factores de riesgo cardiovascular (FRCV) y un menor control de los mismos en comparación con la población general. Objetivos: Conocer la prevalencia de los FRCV y hábitos tóxicos en pacientes con TMS y evaluar el seguimiento por la Atención Primaria (AP) en los que presentan un riesgo vascular elevado. Diseño y metodología: Se realizó un estudio descriptivo de prevalencia y de seguimiento en una muestra representativa de los pacientes con diagnóstico de TMS seguidos en el Centro de salud mental de Mataró. Se registraron los parámetros antropométricos, los FRCV, hábitos tóxicos, perfil lipídico y tratamientos farmacológicos. Se calculó el RCV mediante las tablas de REGICOR. Los pacientes con un RCV & 10% fueron derivados a AP para valorar la estrategia terapéutica. Se evaluó el seguimiento por parte de los especialistas a los 4 meses y en Atención Primaria a los 6-9 meses. Resultados: Se realizaron 112 entrevistas en el centro de Salud Mental de Mataró; 62% eran hombres y 38% mujeres, con una edad media de 43 ± 11,9 años. La obesidad abdominal fue el FRCV más prevalente (62%), le siguió el tabaquismo en un 58,9%, el sedentarismo con un 48,2% y la dislipemia 42%. El 66,8% de los pacientes eran portadores de tratamiento con antipsicóticos atípicos, lo que se asocia con un mayor RCV. Un total de 102 pacientes (91% de la muestra) presentaban uno o más FRCV, 98 de los mismos tenían un RCV & 10 y 4 presentaron un RCV & 10. Se derivaron 6 pacientes a la AP para control de los FRCV, de los que 3 nunca habían sido atendidos en AP. A los 9 meses fueron visitados el 50% de los derivados a AP, y se consiguió reducir el RCV & 10 en un 50% de los que se le hizo la recomendación de seguimiento. Conclusión: Los pacientes con TMS acuden habitualmente a las consultas de AP para el control de los FRCV. La prevalencia de FRCV en pacientes con TMS es superior a la población general. El porcentaje de pacientes con RCV elevado según el REGICOR es bajo.

Este artigo tem como objetivo divulgar a experiência de um grupo de profissionais envolvidos na reabilitação e na reabilitação vocal dos laringectomizados que participam do GARPO- Laringectomizados. São descritas as diferentes possibilidades de reabilitação vocal para o laringectomizado como também, os pressupostos que permeiam todas as atividades e etapas do processo. Destaca-se as características da pessoas laringectomizadas, as conseqüências da cirurgia, o significado de ser laringectomizado que direcionam a assistência do enfermeiro no processo, em integração com o fonoaudiólogo.

ABSTRACT: BACKGROUND: The ability of different obesity indices to predict cardiovascular risk is still debated in youth and few data are available in sub Saharan Africa. We compared the associations between several indices of obesity and cardiovascular risk factors (CVRFs) in late adolescence in the Seychelles. METHODS: We measured body mass index (BMI), waist circumference, waist/hip ratio (WHiR), waist/height ratio (WHtR) and percent fat mass (by bioimpedance) and 6 CVRFs (blood pressure, LDL-cholesterol, HDL-cholesterol, triglycerides, fasting blood glucose and uric acid) in 423 youths aged 19-20 years from the general population. RESULTS: The prevalence of overweight/obesity and several CVRFs was high, with substantial sex differences. Except for glucose in males and LDL-cholesterol in females, all obesity indices were associated with CVRFs. BMI consistently predicted CVRFs at least as well as the other indices. Linear regression on BMI had standardized regression coefficients of 0.25-0.36 for most CVRFs (p<0.01) and ROC analysis had an AUC between 60%-75% for most CVRFs. The quasi-ubiquitous distribution of vitamin D receptors in the human tissues and the high prevalence of vitamin D deficiency worldwide have generated much enthusiasm about the opportunity of cardiovascular disease prevention through vitamin D supplementation. However, reported associations between vitamin D and cardiovascular disease present important limitations and are prone to confounding and reverse causation. Results from ongoing randomized clinical trials testing the efficacy of vitamin D supplementation to reduce cardiovascular events will not be available before year 2015. This article reviews the epidemiology of vitamin D and provides a brief overview on the relationship between vitamin D and cardiovascular disease.

BACKGROUND: We sought to investigate the relationship between infarct and dyssynchrony post- myocardial infarct (MI), in a porcine model. Mechanical dyssynchrony post-MI is associated with left ventricular (LV) remodeling and increased mortality. METHODS: Cine, gadolinium-contrast, and tagged cardiovascular magnetic resonance (CMR) were performed pre-MI, 9 ± 2 days (early post-MI), and 33 ± 10 days (late post-MI) post-MI in 6 pigs to characterize cardiac morphology, location and extent of MI, and regional mechanics. LV mechanics were assessed by circumferential strain (eC). Electro-anatomic mapping (EAM) was performed within 24 hrs of CMR and prior to sacrifice. RESULTS: Mean infarct size was 21 ± 4% of LV volume with evidence of post-MI remodeling. Global eC significantly decreased post MI (-27 ± 1.6% vs. -18 ± 2.5% (early) and -17 ± 2.7% (late), p < 0.0001) with no significant change in peri-MI and MI segments between early and late time-points. Time to peak strain (TTP) was significantly longer in MI, compared to normal and peri-MI segments, both early (440 ± 40 ms vs. 329 ± 40 ms and 332 ± 36 ms, respectively; p = 0.0002) and late post-MI (442 ± 63 ms vs. 321 ± 40 ms and 355 ± 61 ms, respectively; p = 0.012). The standard deviation of TTP in 16 segments (SD16) significantly increased post-MI: 28 ± 7 ms to 50 ± 10 ms (early, p = 0.012) to 54 ± 19 ms (late, p = 0.004), with no change between early and late post-MI time-points (p = 0.56). TTP was not related to reduction of segmental contractility. EAM revealed late electrical activation and greatly diminished conduction velocity in the infarct (5.7 ± 2.4 cm/s), when compared to peri-infarct (18.7 ± 10.3 cm/s) and remote myocardium (39 ± 20.5 cm/s). CONCLUSIONS: Mechanical dyssynchrony occurs early after MI and is the result of delayed electrical and mechanical activation in the infarct.

La coronariopatia diabètica podria relacionar-se amb el dèficit de vitamina D. El nostre estudi compara les concentracions sanguínies de la vitamina i dues proves de detecció d’isquèmia silent en pacients diabètics tipus1 asimptomàtics respecte un grup control. Els diabètics i els fumadors actius presentaren un major dèficit de vitamina respecte els controls i els no fumadors. No es detectaren diferències entre casos i controls ni entre deficitaris i no deficitaris en els resultats de les proves cardiovasculars. Concloem que els diabètics tipus1 presenten major dèficit de vitamina D sense associar-se a una major proporció de coronariopatia silent.

O traumatismo raquimedular é uma agressão à medula espinhal que pode ocasionar danos neurológicos, tais como alteração das funções motora, sensitiva e autônoma. Este artigo tem como propósito relatar detalhadamente as principais complicações clínicas resultantes desse tipo de lesão, e apresentar as intervenções assistenciais de enfermagem que possam auxiliar na promoção do bem estar e na melhoria da qualidade de vida dos pacientes, seja em caráter de acometimento já instalado ou profilático.

O estudo identificou os tipos de suporte social oferecido pelo parceiro sexual da mulher com câncer de mama e verificou como este apoio é percebido por eles. Participaram do estudo nove parceiros de mulheres nessa condição. Os dados foram coletados por meio de entrevistas e submetidos à análise de conteúdo. Os parceiros se perceberam como importantes elementos de suporte social para as suas esposas oferecendo afeto, estímulo ao auto cuidado e auxílio nos afazeres domésticos. Revelaram dificuldades enfrentadas ao oferecer suporte social as quais estavam relacionadas à esfera sexual, aos canais de comunicação, à sensação de impotência e insegurança para lidar com as implicações do diagnóstico e reorganizar as atividades domésticas.

Este estudo objetivou compreender o significado do trabalho em equipe dos profissionais de reabilitação em anomalias craniofaciais. Realizou-se análise fenomenológica por contemplar a compreensão e interpretação do sentido considerando o sujeito. Entrevistou-se 12 profissionais de diferentes áreas, norteadas pela questão: O que significa para você trabalhar em equipe na reabilitação de anomalias craniofaciais? Resgatando-se os temas: Capacitação para o trabalho, Dificuldade para trabalho em equipe, Relação com paciente e família, Condições de trabalho e A inserção do profissional na equipe. A análise visou refletir o fenômeno engendrando convergências e divergências destacando a explicitação das diferenças e aprendizado contínuo.
