Since the pioneering work of Charles Nicolle in 1909 [see Gross (1996) Proc Natl Acad Sci USA 93:10539-10540] most medical officers and scientists have assumed that body lice are the sole vectors of Rickettsia prowazekii, the aetiological agent of louse-borne epidemic typhus (LBET). Here we review the evidence for the axiom that head lice are not involved in epidemics of LBET. Laboratory experiments demonstrate the ability of head lice to transmit R. prowazekii, but evidence for this in the field has not been reported. However, the assumption that head lice do not transmit R. prowazekii has meant that head lice have not been examined for R. prowazekii during epidemics of LBET. The strong association between obvious (high) infestations of body lice and LBET has contributed to this perception, but this association does not preclude head lice as vectors of R. prowazekii. Indeed, where the prevalence and intensity of body louse infections may be high (e.g. during epidemics of LBET), the prevalence and intensity of head louse infestations is generally high as well. This review of the epidemiology of head louse and body louse infestations, and of LBET, indicates that head lice are potential vectors of R. prowazekii in the field. Simple observations in the field would reveal whether or not head lice are natural vectors of this major human pathogen.

A programme for the control of respiratory diseases in children was conceived for the State of S. Paulo, Brazil, in 1986. Its progress thereafter and the epidemiology of the diseases concerned are examined. Apart from an inquiry into the 64 existing State local health authorities, a sample of 18,255 cases of children assisted by the programme at different levels, including both in-patient and outpatient care, is analysed. Each case record included information about identification (child, doctor and health facility), reasons for calling, diagnoses made and outcome of treatment. Further data were also sought from hospitals and from State mortality records. The programme was found to be poorly implemented in the State but, where implemented, it showed itself capable of resolving problems (only 0.5% of the cases could not be handled) as also of changing ongoing trends (more than 50% reduction in hospital admission rates). Individual assessment of each item of the programme indicated its bottlenecks. Regarding the epidemiology of respiratory diseases, it is observed that the major burden to health services comes from children aged less than five, and that the most important diseases are wheezing illnesses and pneumonia. Morevoer, they were found to be significantly associated (p = 0.000) so that a child in the community presenting wheezing diseases is 5 times more likely to develop pneumonia than a child with any other respiratory diagnosis. Similarly, among the under five deaths it was found that the risk for pneumonia is 3 times greater for children who died presenting wheezing diseases than it is for children with any other sort of diagnosis. In conclusion, the programme is deemed to be efficient and effective but its efficacy is marred by administrative flaws. The successful control of respiratory problems in childhood is related to a proper appreciation of the importance of wheezing diseases.

A obesidade e a diabetes mellitus tipo 2 (DM2) são considerados dois grandes problemas de saúde pública. A má alimentação e a falta de atividade física encontram-se entre os principais desencadeadores de um crescente número de indivíduos obesos, diabéticos e com sensibilidade à insulina diminuída. Este aumento tem motivado a comunidade científica a investigar cada vez mais para o elevado contributo da herança genética associada aos fatores sociais e nutricionais. O gene dos recetores ativados por proliferadores do peroxissoma gama 2 (PPARγ2) desempenha um papel importante no metabolismo lipídico. Uma vez que o PPARγ2 é maioritariamente expresso no tecido adiposo, uma redução moderada da sua atividade tem influência na sensibilidade à insulina, diabetes, e outros parâmetros metabólicos. Vários estudos sugerem que tanto fatores genéticos como fatores ambientais (tais como a dieta), poderão estar envolvidos na formação de padrões associados ao polimorfismo Pro12Ala com a composição corporal em diferentes populações humanas. Os diversos estudos genéticos envolvendo o estudo do polimorfismo Pro12Ala do PPARγ2 na suscetibilidade de possuir risco de diabetes e obesidade em várias populações têm proposto conclusões diversas. Em alguns parece haver mais associações do que outros e, às vezes, não demonstram sequer associação. Desta forma, o presente trabalho teve como objectivo contribuir para a elucidação do impacto do polimorfismo Pro12Ala do PPARγ2 na resistência à insulina associada à DM2 e na obesidade, mediante estudo sistematizado da literatura existente até à data, através de meta análise. Do total de uma pesquisa de 63 publicações, foram incluídos 32 artigos no presente estudo, sendo que destes 25 foram incluídos na síntese qualitativa e 11 incluídos na sintese quantitativa. No presente trabalho pode-se concluir que existe evidência estatística que suporta a hipótese de que o polimorfismo Pro12Ala do PPARγ2 pode ser considerado um fator protetor para a DM2 [p <0,05 e OR (odds ratio) 0,702, com IC (intervalos de confiança) com valores que nunca incluem o 1]. No entanto, e mediante os mesmos pressupostos, o mesmo polimorfismo pode ser considerado um fator de risco ao desenvolvimento de obesidade, pela evidência estatística [p <0,05 e OR de 1,196, com IC com valores que nunca incluem o 1].

OBJECTIVE: To compare inpatient and outpatient care costs for pregnant/parturient women with diabetes and mild hyperglycemia. METHODS: A prospective observational quantitative study was conducted in the Perinatal Diabetes Center in the city of Botucatu, Southeastern Brazil, between 2007 and 2008. Direct and indirect costs and disease-specific costs (medications and tests) were estimated. Thirty diet-treated pregnant women with diabetes were followed up on an outpatient basis, and 20 who required insulin therapy were hospitalized. RESULTS: The cost of diabetes disease (prenatal and delivery care) was US$ 3,311.84 for inpatients and US$ 1,366.04 for outpatients. CONCLUSIONS: Direct and indirect costs as well as total prenatal care cost were higher for diabetic inpatients while delivery care costs and delivery-postpartum hospitalization were similar. Prenatal and delivery-postpartum care costs were higher for these patients compared to those paid by Brazilian National Health System.

Natural infection in Biomphalaria tenagophila with Schistosoma mansoni was observed for the first time in a small area, in Jaboticatubas, in the State of Minas Gerais, Brazil. In view of this finding, the Authors decided to carry out some detailed investigations, since B. glabrata has been known as the vector snail of schistosomiasis in that State. B. tenagophila was found naturally infected throughout the investigation period (15 months). Strains of the snail and of the parasite were isolated four times, and then maintained at the laboratory through B. tenagophila hamster passages. The possibilities of B. tenagophila to play a role in the epidemiology of the human disease in the State of Minas Gerais, in the future, are discussed.

Pathogenic Yersinia pestis isolates were collected during a plague outbreak at the Paraiba State in 1986. The Y. pestis isolates were investigated for the presence of virulence-associated factors and plasmid content. All strains analysed were proficient in the expression of the VW and fraction 1 antigens, pigment adsorption and pesticin-fibronolysin-coagulase production. A similar plasmid profile composed by four plasmid with molecular weight of 60, 44, 14.9, and 6.4 Megadaltons (MD) was found in all strains. DNA cleavage with EcoRI restriction enzyme further demonstrated the uniform plasmid content of the Y. pestis isolates. Seven additional Y. pestis strains, previously isolated in the same region but in an endemic state, showed the same plasmid fingerprint. The lack of any detectable difference between epidemic and endemic isolates as well as the value of plasmid fingerprints in epidemiology of Y. pestis is discussed.

Glucose monitoring in vivo is a crucial issue for gaining new understanding of diabetes. Glucose binding protein (GBP) fused to two fluorescent indicator proteins (FLIP) was used in the present study such as FLIP-glu- 3.2 mM. Recombinant Escherichia coli whole-cells containing genetically encoded nanosensors as well as cell-free extracts were immobilized either on inner epidermis of onion bulb scale or on 96-well microtiter plates in the presence of glutaraldehyde. Glucose monitoring was carried out by Förster Resonance Energy Transfer (FRET) analysis due the cyano and yellow fluorescent proteins (ECFP and EYFP) immobilized in both these supports. The recovery of these immobilized FLIP nanosensors compared with the free whole-cells and cell-free extract was in the range of 50–90%. Moreover, the data revealed that these FLIP nanosensors can be immobilized in such solid supports with retention of their biological activity. Glucose assay was devised by FRET analysis by using these nanosensors in real samples which detected glucose in the linear range of 0–24 mM with a limit of detection of 0.11 mM glucose. On the other hand, storage and operational stability studies revealed that they are very stable and can be re-used several times (i.e. at least 20 times) without any significant loss of FRET signal. To author's knowledge, this is the first report on the use of such immobilization supports for whole-cells and cell-free extract containing FLIP nanosensor for glucose assay. On the other hand, this is a novel and cheap high throughput method for glucose assay.

Some epidemiological and immunological characteristics and the methodology of diagnosis of 44 cases of histoplasmosis (HP); 36 (27 males and 9 women) associated with AIDS (HP+AIDS) and 8 (7 males and 1 female) with other predisposing factors (HP+non AIDS), diagnosed in the Muñiz Hospital (MH) during 1994, were retrospectively studied. The median age (MA) of HP+AIDS patients was 28 years; 25.5 (22-40) in the women and 28.5 (20-42) in the men and 50 (22-58) years in the HP+non AIDS patients. The more frequent risk factors for HIV infection were intravenous drug addiction (55%) and homo/bisexuality (19%). The MA of these groups were 28 (20-39) and 41 (26-42) years, respectively. Tobaccoism was a predisposing factor in 83% of HP+non AIDS patients. The muco-cutaneous lesions scraping and blood-cultures established the initial diagnosis in 53% and 36% of HP+AIDS patients, respectively and the muco-cutaneous lesions biopsies in 75% of HP+non AIDS cases. At time of diagnosis, all HP+AIDS patients had <200 while HP+non AIDS patients had > 200 CD4 + lymphocytes/µl. Seventy two per cent of HP+AIDS patients were born in Buenos Aires (Bs As) city and 62% of HP+non AIDS patients were born in provinces of Argentina other than Bs As. At moment of diagnosis, 87.5% of HP+AIDS and 62.5% of HP+non AIDS patients lived in Bs As city and Bs As outskirts.

Pinworm infection was prospectively studied during one year in 469 children attending three day care centers. Each child was examined at six months intervals using up to three perianal swabs with adhesive tape. Those found infected were treated with mebendazole. At the beginning of the study we found a prevalence of 28% that dropped to 13% and 12% in the following study periods. The reinfection rate was twice the incidence rate in both study periods. We also found a small percentage (10%) of the children reinfected in most or all study periods. There was a high correlation between reinfection and perianal itching. Our results add further knowledge to the epidemiology of intestinal parasites in day care centers.

RESUMO: O aumento da longevidade humana é um facto prevalente na actualidade mas, a requerer grandes esforços na promoção da saúde e prevenção de doenças, estando, ainda, as pessoas idosas sob riscos elevados de desenvolver doenças à medida que envelhecem. Assim, é importante o contributo dos próprios indivíduos no sentido minorar estes riscos e retardar a evolução e agudizações das doenças uma vez manifestadas. Nesse sentido, o objectivo geral deste estudo é descrever de que modo os conhecimentos do diabético, podem estar relacionados com a presença de complicações da doença. Realizámos um estudo exploratório descritivo, no Hospital Nossa Senhora do Rosário E.P.E, em pessoas idosas residentes no concelho do Barreiro que frequentaram o Hospital Dia de Diabetologia, após as consultas médicas e de enfermagem. O método utilizado para a selecção da amostra foi a amostragem não probabilística por conveniência (n=50). O instrumento utilizado na colheita de dados foi um questionário, aplicado sob a forma de inquérito, para colheita de dados sócio-demográficos, saúde em geral, estado mental, dados sobre a diabetes e comorbilidades. Os dados foram tratados informaticamente, recorrendo ao programa estatístico S.P.S.S. e a análise de conteúdo. Os resultados obtidos demonstraram que os participantes possuíam conhecimentos sobre a sua doença. Verificámos que os inquiridos que já tiveram complicações devido à diabetes responderam correctamente à questão acerca da medicação ser mais importante que a dieta e o exercício físico para controlar a diabetes, do que os que nunca tiveram complicações devido à diabetes, que responderam incorrectamente ou desconheciam a resposta à questão.----------- ABSTRACT: The increase in human longevity is a fact prevalent nowadays, but to require great efforts on health promotion and disease prevention and is also in the elderly high risk of developing illnesses as they age. It is therefore important contribution of the individuals themselves towards mitigating these risks and delaying the development and exacerbations of disease once manifested. Thus, the general purpose of this study is to describe how the knowledge of diabetes, may be related to complications of the disease. We conducted an exploratory study in the Hospital Nossa Senhora do Rosario EPE, in elderly people living in the county of Barry, who attended the hospital Diabetes Day, after the medical and nursing care. The method used for the sampling was not probabilistic by convenience sampling (n = 50). The instrument used for data collection was a questionnaire applied in the form of survey to collect socio-demographic data, general health, mental state, data on diabetes and comorbidities. The data were processed by computer, using the SPSS and content analysis. The results showed that participants had knowledge about their disease. We found that respondents who have had complications due to diabetes responded correctly to questions about the medication is more important than diet and exercise to control diabetes than those who never had complications due to diabetes, who answered incorrectly or did not know the answer the question.

The antigenic relationship of 9 flaviviruses, Yellow fever (YF) , Wesselsbron (WSL) , Uganda S (UGS) , Potiskum (POT), West Nile (WN) , Banzi (BAN) , Zika (ZK) , Dengue type 1 (DEN-1) and Dengue type 2 (DEN-2), was assessed by cross-haemagglutination-inhibition (Cross-HI) and cross-complement fixation (Cross-CF) reactions between each of the viruses and their homologous immune mouse ascitic fluids. Titre ratios were calculated using the heterologous and homologous titres. Cross-CF reactions revealed wider antigenic variations among viruses than Cross-HI reactions. There was no significant antigenic variation between WSL, POT and YF viruses using either of those methods. However, definite differences in antigenicity were observed between them and UGS, BAN and ZK viruses. There were no significant differences between UGS, BAN and ZK or between DEN-1 and DEN-2. The serological relationship among flaviviruses is important in establishing diagnosis and epidemiology of these infections in Africa.

RESUMO - A diabetes mellitus é considerada um flagelo à escala mundial, não apenas por atingir milhões de pessoas, mas também porque se tem verificado um aumento da sua prevalência e, consequente-mente, das suas complicações crónicas, que se traduzem no aumento dos gastos em saúde e, sobretudo, em sofrimento das pessoas com diabetes, seus familiares e comunidade. A prevenção das complicações crónicas da diabetes reside, sobretudo, na adopção de comporta-mentos saudáveis que, no dia-a-dia, se traduzem em competência no autocuidado. Assim, será importante conhecer em que medida o sentido de coerência – característica pessoal entendida como influenciadora das estratégias utilizadas face aos acontecimentos originadores de stress, com repercussões na saúde mental, bem como na saúde física – pode determinar a escolha de comportamentos de saúde e, por tal, possa considerar-se como um aspecto sobre o qual os pro-fissionais de saúde poderão investir em prole do seu fortalecimento e, consequentemente, garantir a facilitação do processo de adopção de comportamentos saudáveis. De acordo com o estudo transversal exploratório retrospectivo proposto ambiciona-se conhecer a relação entre o nível de sentido de coerência e o nível de autocuidado e de controlo metabólico, das pessoas com diabetes de tipo 2. Para tal investigação reconhece-se a pertinência da aplica-ção da escala de avaliação do sentido de coerência – Questionário Orientação para Viver (Nunes, 1999), da escala de avaliação do autocuidado com a diabetes – Escala de Avaliação das Activida-des de Autocuidado com a Diabetes (Bastos, Severo e Lopes, 2007) e do Formulário de Controlo Metabólico na Diabetes (European Society of Cardiology e European Association for the Study of Diabetes cit. por Direcção-Geral da Saúde, 2007), numa amostra constituída por 391 pessoas com diabetes de tipo 2, seleccionadas por amostragem aleatória estratificada proporcional, de entre os utentes inscritos nas 33 instituições de Cuidados de Saúde Primários, do distrito de Viana do Castelo --------------------------- ABSTRACT - Diabetes mellitus is considered a global scourge, not only for reaching millions of people, but also because there has been an increase in its prevalence and, consequently, its chronic complications, which translate into increased spending on health and especially in the suffering of people with diabetes, their families and community. The prevention of chronic complications of diabetes includes adoption healthy behaviors that on the day to day, translate into competence in self-care. Therefore it is important to know how far the sense of coherence – understood as influential personal characteristic of the strategies used by the events originators of stress, affecting the mental health and physical health – may determine the choice of health behaviors and, as such, can be considered as an aspect about which health pro-fessionals will invest in strengthening their offspring and thus ensure the facilitation of the process of adopting healthy behaviors. According to the proposed retrospective exploratory cross-sectional study aims to discover the relationship between the level of sense of coherence and level of self-care and metabolic control on people with type 2 diabetes. For this research recognizes the appropriateness of applying the rating scale of sense of coherence – Questionário Orientação para Viver (Nunes, 1999), the rating scale of self-care in diabetes – Escala de Avaliação das Actividades de Autocuidado com a Di-abetes (Bastos, Severo and Lopes, 2007) and the Formulário de Controlo Metabólico na Diabetes (European Society of Cardiology and European Association for the Study of Diabetes cit. por Di-recção-Geral da Saúde, 2007), a sample of 391 people with type 2 diabetes, selected by propor-tional stratified random sampling from among registered users in 33 institutions of Primary Health Care, on district of Viana do Castelo.

This study was carried out in order to obtain base-line data concerning the epidemiology of American Visceral Leishmaniasis and Chagas Disease in an indigenous population with whom the government is starting a dwelling improvement programme. Information was collected from 242 dwellings (1,440 people), by means of house to house interviews about socio-economic and environmental factors associated with Leishmania chagasi and Trypanosoma cruzi transmission risk. A leishmanin skin test was applied to 385 people and 454 blood samples were collected on filter paper in order to detect L. chagasi antibodies by ELISA and IFAT and T. cruzi antibodies by ELISA. T. cruzi seroprevalence was 8.7% by ELISA, L. chagasi was 4.6% and 5.1% by IFAT and ELISA, respectively. ELISA sensitivity and specificity for L. chagasi antibodies were 57% and 97.5% respectively, as compared to the IFAT. Leishmanin skin test positivity was 19%. L. chagasi infection prevalence, being defined as a positive result in the three-immunodiagnostic tests, was 17.1%. Additionally, 2.7% of the population studied was positive to both L. chagasi and T. cruzi, showing a possible cross-reaction. L. chagasi and T. cruzi seropositivity increased with age, while no association with gender was observed. Age (p<0.007), number of inhabitants (p<0.05), floor material (p<0.03) and recognition of vector (p<0.01) were associated with T. cruzi infection, whilst age ( p<0.007) and dwelling improvement (p<0.02) were associated with L. chagasi infection. It is necessary to evaluate the long-term impact of the dwelling improvement programme on these parasitic infections in this community.
