Objective Genetic sonography following first-trimester combined screening appears to increase substantially detection rates for Down syndrome but it relies on the unproved assumption of independence between these tests. In this study we aimed to investigate the relationship between first-trimester nuchal translucency (NT) and a series of second-trimester soft markers and structural defects in unaffected pregnancies. Methods NT measurement in the first trimester was followed by second-trimester scan (18 to 23 + 6 weeks) including examination for three categorical markers (intracardiac echogenic foci, hyperechogenic bowel and structural defects) and measurement of nasal bone length, nuchal-fold thickness, femur length, humerus length, renal pelvis diameter and prenasal thickness. All continuous variables were expressed in multiples of the median (MoM) for gestation and correlation coefficients between log-transformed NT and second-trimester variables were calculated. In addition, frequencies of soft markers and structural defects in cases with increased NT were compared to those with normal NT, using MoM cut-offs. Results In a dataset of 1970 cases, NT was significantly correlated (P < 0.05) with all second-trimester continuous variables, the correlation being strongest for nuchal-fold thickness (r = 0.10). There was a higher frequency of cases with second-trimester nuchal-fold thickness above the 97.5th centile (10.7 vs. 2.2%) and hyperechogenic bowel (2.4 vs. 0.1%) in cases with increased NT. Conclusions Straightforward reassessment of risk using likelihood ratios derived from the second-trimester genetic sonogram might lead to inaccurate estimates. Multivariate models using continuous second-trimester variables might be preferable in sequential screening strategies. Copyright. 2012 ISUOG. Published by John Wiley & Sons, Ltd.

Topoisomerase 2 alpha (), HER-2/ and are genes that lie on chromosome 17 and correlate with the prognosis and prediction of target-driven therapy against tumors. In a previous study, we showed that TOP2A transcripts levels were significantly higher in soft tissue sarcomas (STS) than in benign tumors and desmoid-type fibromatoses (FM). Because these genes have been insufficiently examined in STS, we aimed to identify alterations in TOP2A and HER-2 expression by fluorescent in situ hybridization and immunohistochemistry, as well as that of survivin, and correlate them with clinicopathologic findings to assess their prognostic value. Eighteen FM and 244 STS were included. Fluorescent in situ hybridization and immunohistochemistry were performed on a tissue microarray. TOP2A and survivin were more highly expressed in sarcomas than in FM. TOP2A was an independent predictor of an unfavorable prognosis; it was combined with formerly established prognostic factors (primarily histologic grade and tumor size at diagnosis) to create a prognostic index that evaluated overall survival. Gene amplification/polysomy (13%) did not correlate with protein overexpression. Survivin and HER-2 expression were not associated with patient outcomes. These findings might become valuable in the management of patients with STS and possibly in the prospective evaluation of responses to new target-driven therapies.

The aim of this study was to evaluate the short-and long-term treatment effects of rapid maxillary expansion (RME) on the soft tissue facial profile of subjects treated with a modified acrylic-hyrax device. The sample comprised 10 males and 10 females in the mixed dentition. Their average age was 9.3 years +/- 10 months pre-treatment (T1), with a narrow maxilla and posterior crossbite, treated with a modified fixed maxillary expander with an occlusal splint. Lateral cephalometric radiographs obtained at T1, immediately post-expansion (T2), and after retention (T3) were used to determine possible changes in the soft tissue facial profile. The means and standard deviations for linear and angular cephalometric measurements were analysed statistically using analysis of variance and Tukey's test (alpha = 0.05). The measurements at T2 differed significantly from those at T1 and T3. However, RME did not produce any statistically significant alteration (P > 0.05) in the soft tissue profile for any of the cephalometric landmarks evaluated when compared at T1 and T3. The use of a fixed expander associated with an occlusal splint did not cause significant alterations in the soft tissue facial profile at T3. This modified device is effective for preventing the adverse vertical effects of RME such as an increase anterior face height in patients with a crossbite.

[ES] Teniendo en cuenta la importancia de una gestión ética de los recursos humanos, el objetivo del presente trabajo consiste en determinar si las empresas aplican diferentes modelos de gestión. Para ello, se ha tratado de comprobar si las empresas aplican una dirección de recursos humanos (DRRHH) soft o hard y sobre la base de qué perspectivas éticas lo hacen, tratando de contrastar los cuatro modelos de DRRHH propuestos por Greenwood (2004). El estudio empírico realizado a una muestra de 189 empresas con sede en la Comunidad Autónoma de Canarias ha permitido contrastar estos cuatro modelos, así como identificar la existencia de dos nuevos. Estos resultados nos permiten abrir una nueva línea de investigación en la que se tenga en cuenta los aspectos éticos en la gestión de los recursos humanos.

[EN]During the last decade, researchers have verified that clothing can provide information for gender recognition. However, before extracting features, it is necessary to segment the clothing region. We introduce a new clothes segmentation method based on the application of the GrabCut technique over a trixel mesh, obtaining very promising results for a close to real time system. Finally, the clothing features are combined with facial and head context information to outperform previous results in gender recognition with a public database.

In case of severe osteoarthritis at the knee causing pain, deformity, and loss of stability and mobility, the clinicians consider that the substitution of these surfaces by means of joint prostheses. The objectives to be pursued by this surgery are: complete pain elimination, restoration of the normal physiological mobility and joint stability, correction of all deformities and, thus, of limping. The knee surgical navigation systems have bee developed in computer-aided surgery in order to improve the surgical final outcome in total knee arthroplasty. These systems provide the surgeon with quantitative and real-time information about each surgical action, like bone cut executions and prosthesis component alignment, by mean of tracking tools rigidly fixed onto the femur and the tibia. Nevertheless, there is still a margin of error due to the incorrect surgical procedures and to the still limited number of kinematic information provided by the current systems. Particularly, patello-femoral joint kinematics is not considered in knee surgical navigation. It is also unclear and, thus, a source of misunderstanding, what the most appropriate methodology is to study the patellar motion. In addition, also the knee ligamentous apparatus is superficially considered in navigated total knee arthroplasty, without taking into account how their physiological behavior is altered by this surgery. The aim of the present research work was to provide new functional and biomechanical assessments for the improvement of the surgical navigation systems for joint replacement in the human lower limb. This was mainly realized by means of the identification and development of new techniques that allow a thorough comprehension of the functioning of the knee joint, with particular attention to the patello-femoral joint and to the main knee soft tissues. A knee surgical navigation system with active markers was used in all research activities presented in this research work. Particularly, preliminary test were performed in order to assess the system accuracy and the robustness of a number of navigation procedures. Four studies were performed in-vivo on patients requiring total knee arthroplasty and randomly implanted by means of traditional and navigated procedures in order to check for the real efficacy of the latter with respect to the former. In order to cope with assessment of patello-femoral joint kinematics in the intact and replaced knees, twenty in-vitro tests were performed by using a prototypal tracking tool also for the patella. In addition to standard anatomical and articular recommendations, original proposals for defining the patellar anatomical-based reference frame and for studying the patello-femoral joint kinematics were reported and used in these tests. These definitions were applied to two further in-vitro tests in which, for the first time, also the implant of patellar component insert was fully navigated. In addition, an original technique to analyze the main knee soft tissues by means of anatomical-based fiber mappings was also reported and used in the same tests. The preliminary instrumental tests revealed a system accuracy within the millimeter and a good inter- and intra-observer repeatability in defining all anatomical reference frames. In in-vivo studies, the general alignments of femoral and tibial prosthesis components and of the lower limb mechanical axis, as measured on radiographs, was more satisfactory, i.e. within ±3°, in those patient in which total knee arthroplasty was performed by navigated procedures. As for in-vitro tests, consistent patello-femoral joint kinematic patterns were observed over specimens throughout the knee flexion arc. Generally, the physiological intact knee patellar motion was not restored after the implant. This restoration was successfully achieved in the two further tests where all component implants, included the patellar insert, were fully navigated, i.e. by means of intra-operative assessment of also patellar component positioning and general tibio-femoral and patello-femoral joint assessment. The tests for assessing the behavior of the main knee ligaments revealed the complexity of the latter and the different functional roles played by the several sub-bundles compounding each ligament. Also in this case, total knee arthroplasty altered the physiological behavior of these knee soft tissues. These results reveal in-vitro the relevance and the feasibility of the applications of new techniques for accurate knee soft tissues monitoring, patellar tracking assessment and navigated patellar resurfacing intra-operatively in the contest of the most modern operative techniques. This present research work gives a contribution to the much controversial knowledge on the normal and replaced of knee kinematics by testing the reported new methodologies. The consistence of these results provides fundamental information for the comprehension and improvements of knee orthopedic treatments. In the future, the reported new techniques can be safely applied in-vivo and also adopted in other joint replacements.

3D video-fluoroscopy is an accurate but cumbersome technique to estimate natural or prosthetic human joint kinematics. This dissertation proposes innovative methodologies to improve the 3D fluoroscopic analysis reliability and usability. Being based on direct radiographic imaging of the joint, and avoiding soft tissue artefact that limits the accuracy of skin marker based techniques, the fluoroscopic analysis has a potential accuracy of the order of mm/deg or better. It can provide fundamental informations for clinical and methodological applications, but, notwithstanding the number of methodological protocols proposed in the literature, time consuming user interaction is exploited to obtain consistent results. The user-dependency prevented a reliable quantification of the actual accuracy and precision of the methods, and, consequently, slowed down the translation to the clinical practice. The objective of the present work was to speed up this process introducing methodological improvements in the analysis. In the thesis, the fluoroscopic analysis was characterized in depth, in order to evaluate its pros and cons, and to provide reliable solutions to overcome its limitations. To this aim, an analytical approach was followed. The major sources of error were isolated with in-silico preliminary studies as: (a) geometric distortion and calibration errors, (b) 2D images and 3D models resolutions, (c) incorrect contour extraction, (d) bone model symmetries, (e) optimization algorithm limitations, (f) user errors. The effect of each criticality was quantified, and verified with an in-vivo preliminary study on the elbow joint. The dominant source of error was identified in the limited extent of the convergence domain for the local optimization algorithms, which forced the user to manually specify the starting pose for the estimating process. To solve this problem, two different approaches were followed: to increase the optimal pose convergence basin, the local approach used sequential alignments of the 6 degrees of freedom in order of sensitivity, or a geometrical feature-based estimation of the initial conditions for the optimization; the global approach used an unsupervised memetic algorithm to optimally explore the search domain. The performances of the technique were evaluated with a series of in-silico studies and validated in-vitro with a phantom based comparison with a radiostereometric gold-standard. The accuracy of the method is joint-dependent, and for the intact knee joint, the new unsupervised algorithm guaranteed a maximum error lower than 0.5 mm for in-plane translations, 10 mm for out-of-plane translation, and of 3 deg for rotations in a mono-planar setup; and lower than 0.5 mm for translations and 1 deg for rotations in a bi-planar setups. The bi-planar setup is best suited when accurate results are needed, such as for methodological research studies. The mono-planar analysis may be enough for clinical application when the analysis time and cost may be an issue. A further reduction of the user interaction was obtained for prosthetic joints kinematics. A mixed region-growing and level-set segmentation method was proposed and halved the analysis time, delegating the computational burden to the machine. In-silico and in-vivo studies demonstrated that the reliability of the new semiautomatic method was comparable to a user defined manual gold-standard. The improved fluoroscopic analysis was finally applied to a first in-vivo methodological study on the foot kinematics. Preliminary evaluations showed that the presented methodology represents a feasible gold-standard for the validation of skin marker based foot kinematics protocols.

This dissertation is devoted to the experimental exploration of the propagation of elastic waves in soft mesoscopic structures with submicrometer dimensions. A strong motivation of this work is the large technological relevance and the fundamental importance of the subject. Elastic waves are accompanied by time-dependent fluctuations of local stress and strain fields in the medium. As such, the propagation phase velocities are intimately related to the elastic moduli. Knowledge of the elastic wave propagation directly provides information about the mechanical properties of the probed mesoscopic structures, which are not readily accessible experimentally. On the other hand, elastic waves, when propagating in an inhomogeneous medium with spatial inhomogeneities comparable to their wavelength, exhibit rather rich behavior, including the appearance of novel physical phenomena, such as phononic bandgap formation. So far, the experimental work has been restricted to macroscopic structures, which limit wave propagation below the KHz range. It was anticipated that an experimental approach capable of probing the interplay of the wave propagation with the controlled mesoscopic structures would contribute to deeper insights into the fundamental problem of elastic wave propagation in inhomogeneous systems. The mesoscopic nature of the structures to be studied precludes the use of traditional methods, such as sound transmission, for the study of elastic wave propagation. In this work, an optical method utilizing the inelastic scattering of photons by GHz frequency thermally excited elastic waves, known as Brillouin light scattering spectroscopy (BLS), was employed. Two important classes of soft structures were investigated: thin films and colloidal crystals. For the former, the main interest was the effect of the one-dimensional (1D) confinement on the wave propagation due to the presence of the free-surface or interface of the layer and the utilization of these waves to extract relevant material parameters. For the second system, the primary interest was the interaction of the elastic wave and the strong scattering medium with local resonance units in a three-dimensional (3D) periodic arrangement.

Lo scopo della tesi è quello di affrontare la progettazione con un approccio,quanto più attuale e per certi versi avanguardista, chiamato Parametric design (progettazione parametrica), accoppiato efficacemente col concetto di Arte generativa (in questo caso Architettura). Già nel 1957 Luigi Moretti affrontò il tema dell’architettura parametrico-generativa fondando l’IRMOU (Istituto per la Ricerca Matematica e Operativa applicata all'Urbanistica) e oggi è una mentalità molto diffusa nei più grandi studi del mondo. Il tema non è solo tecnologico o informatico strumentale, ma è proprio un modo di pensare e immaginare il possibile, costruito o naturale che sia. E’ un modo di vivere la propria creatività. L’aggettivo “generativa” è legato al fatto che l’arte in esame è generata seguendo regole preimpostate e ben definite dal progettista, coerentemente agli obiettivi e alle finalità del progetto. L’evoluzione delle stesse, seguendo relazioni molto semplici, può dar vita a risultati sorprendenti e inaspettati, dotati di una notevole complessità che però, se letta nell’insieme, è perfettamente in armonia con l’idea progettuale di partenza. Il fascino di questa materia è il legame entusiasmante che crea tra architettura, ingegneria, poesia, filosofia, matematica, biologia, fisica, pittura ecc ecc. Questo perché i concetti di evoluzione, di relazione e di generazione appartengono a tutto ciò che ci circonda, e quindi alla concezione umana di vita. E’ possibile in questo modo permeare il costrutto progettuale con principi e regole oggettivamente riconoscibili e apprezzabili dallo spettatore perché instrisi di una forte veridicità processuale. Il titolo "Oxymoron" è la traduzione inglese della figura retorica ossimoro,la quale è strettamente connessa all’ispirazione progettuale: proviene dall’indagine approfondita di processi evolutivi (distruttivi in questo caso) caratterizzanti realtà naturali che, esplorate con sempre più accuratezza, determinano morfologie e forme aventi profonde radici strutturali. La distruzione che crea lo spazio. La genesi stessa della forma segue predominanti algoritmi matematici governati e corretti da variabili di diversa natura che definiscono l'enviroment di influenze interagenti ed agenti sul campione di studio. In questo caso la ricerca è focalizzata su processi erosivi fisici e chimici, di agenti esterni (quali vento e sali rispettivamente) ,di cui materiali inorganici, quali minerali e aggregati degli stessi (rocce), sono soggetti. In particolare, l’interesse è approfondito su fenomeni apparentemente emergenti dei tafoni e dei cosiddetti Micro canyon. A tal scopo si sfrutterà un metodo di soft kill option (SKO) di ottimizzazione topologica (optimization topology) attraverso gli strumenti informatici più idonei quali software di modellazione parametrica e di calcolo computazionale. La sperimentazione sta proprio nell'utilizzare uno strumento concepito per uno scopo, con un'ottica strettamente ingegneristica, per un'altra meta, ossia ricavare e ottenere se possibile un metodo di lavoro o anche solo un processo generativo tale da riprodurre o simulare casi e situazioni riscontrabili in natura negli eventi soggetti a erosione. Il tutto coerente con le regole che stanno alla base della genesi degli stessi. Il parallelismo tra singolarità naturale e architettura risiede nella generazione degli spazi e nella combinazione di questi. L’ambizioso obiettivo è quello di innescare un ciclo generativo, che messo in comunicazione diretta con un contesto variegato ed eterogeneo, dia vita a una soluzione progettuale dall'alto contenuto morfologico e spaziale.
