ABSTRACT – Background: According to the Report on Carcinogens, formaldehyde ranks 25th in the overall U.S. chemical production, with more than 5 million tons produced each year. Given its economic importance and widespread use, many people are exposed to formaldehyde environmentally and/or occupationally. Presently, the International Agency for Research on Cancer classifies formaldehyde as carcinogenic to humans (Group 1), based on sufficient evidence in humans and in experimental animals. Manyfold in vitro studies clearly indicated that formaldehyde can induce genotoxic effects in proliferating cultured mammalian cells. Furthermore, some in vivo studies have found changes in epithelial cells and in peripheral blood lymphocytes related to formaldehyde exposure. Methods: A study was carried out in Portugal, using 80 workers occupationally exposed to formaldehyde vapours: 30 workers from formaldehyde and formaldehyde-based resins production factory and 50 from 10 pathology and anatomy laboratories. A control group of 85 non-exposed subjects was considered. Exposure assessment was performed by applying simultaneously two techniques of air monitoring: NIOSH Method 2541 and Photo Ionization Detection equipment with simultaneously video recording. Evaluation of genotoxic effects was performed by application of micronucleus test in exfoliated epithelial cells from buccal mucosa and peripheral blood lymphocytes. Results: Time-weighted average concentrations not exceeded the reference value (0.75 ppm) in the two occupational settings studied. Ceiling concentrations, on the other hand, were higher than reference value (0.3 ppm) in both. The frequency of micronucleus in peripheral blood lymphocytes and in epithelial cells was significantly higher in both exposed groups than in the control group (p < 0.001). Moreover, the frequency of micronucleus in peripheral blood lymphocytes was significantly higher in the laboratories group than in the factory workers (p < 0.05). A moderate positive correlation was found between duration of occupational exposure to formaldehyde (years of exposure) and micronucleus frequency in peripheral blood lymphocytes (r = 0.401; p < 0.001) and in epithelial cells (r = 0.209; p < 0.01). Conclusions: The population studied is exposed to high peak concentrations of formaldehyde with a long-term exposure. These two aspects, cumulatively, can be the cause of the observed genotoxic endpoint effects. The association of these cytogenetic effects with formaldehyde exposure gives important information to risk assessment process and may also be used to assess health risks for exposed worker

This study explores (1) the reasons that lead Portuguese companies to use expatriates and (2) the motivations that led the repatriates to accepting an international assignment. Semistructured interviews were carried out on 44 individuals (14 responsible for organizational management of the expatriates and 30 repatriates), in seven international companies based in Portugal, and analysis of the thematic content was conducted for data processing. Results suggest that the reasons that lead Portuguese companies to expatriating employees are connected to business needs and the control of international operations, while individual motivations are linked to personal and professional ambition, the will to serve the company and organizational pressure. We concluded that Portuguese companies, like most European companies, seem to develop a strategy of International Human Resources Management (IHRM) according to an ethnocentric approach, characteristic to companies in early stages of internationalization. Similarly, the main motivations of Portuguese repatriates are identical to those of other expatriates, though the will to serve the company seems to be more valued by Portuguese repatriates than by expatriates from other countries.

The study seeks to identify the determinant factors of the repatriate’s decision to remain or leave the company after repatriation, in a convenience sample of 40 Portuguese returnees working in companies based in Portugal. The main results were as follows: (1) there are seven factor categories: (a) salaries and benefits; (b) possibility of promotion, development, professional development; (c) organizational support (during and after the international mission) recognition of work; (d) economic and social atmosphere of the company, (e) good relationship with leadership; (f) convenience and/or personal / family well-being and; (g) external alternatives; (2) the main factors leading to permanence are (a) possibility of promotion, development and professional development and; (b) the existence of personal and family well-being / convenience; (3) the main factors leading to abandonment are (a) lack of organizational support and recognition of work performed; (b) lack of possibility of promotion, development and professional development and; (c) lack of personal / family well-being / convenience. Globally, the study suggests that the factors leading to permanence are very similar to those that lead to abandonment, although in reverse.

Erasmus Mundus Masters (EMM) are programs with a strong component of interculturality. Our study aimed at understanding the level of cultural intelligence (CQ) of EMM students and alumni, as well as some of the characteristics associated with higher levels of CQ. The study included 626 EMM students and alumni from 109 different countries that encompasses 6 continents. Ang and Van Dyne’s (2006) cultural intelligence scale was used; closed and open ended questions were used to describe the sample’s sociodemographic characteristics and experiences regarding interculturality. After validating and assessing the scale’s psychometric properties, relations between different variables were explored using Pearson’s correlation, ANOVA, t Tests, and GLM procedures. We then analysed the open ended responses to gain further insight on our results. Differences among respondents are mainly equated with international experience rather than nationality or training. Respondents’ open ended replies provided us with a deeper insight on why training seems to be so ineffective in developing CQ. This is a transversal study that uses self-reporting measures; also, questionnaires were conducted in English, which was not the mother tongue of most of the respondents. This work is consistent with the CQ literature, however we argue that training mentioned by respondents systematically fails to meet some of literature’s foremost conditions for effective CQ trainings and provide clues for the implementation of more successful initiatives. With an exceptionally diverse sample, this study contributes towards the understanding of mechanisms of developing CQ among EMM and international Students. Results can be useful for selection processes, training/development of CQ and reducing dropout/turnover.

Evidence indicates that exposure to high levels of noise adversely affects human health, and these effects are dependent upon various factors. In hospitals, there are many sources of noise, and high levels exert an impact on patients and staff, increasing both recovery time and stress, respectively. The goal of this pilot study was to develop, implement and evaluate the effectiveness of a training program (TP) on noise reduction in a Neonatal Intensive Care Units (NICU) by comparing the noise levels before and after the implementation of the program. A total of 79 health professionals participated in the study. The measurements of sound pressure levels took into account the layout of the unit and location of the main sources of noise. General results indicated that LAeq levels before implementation of the training program were often excessive, ranging from 48.7 ± 2.94 dBA to 71.7 ± 4.74 dBA, exceeding international guidelines. Similarly following implementation of the training program noise levels remained unchanged (54.5 ± 0.49 dBA to 63.9 ± 4.37 dBA), despite a decrease in some locations. There was no significant difference before and after the implementation of TP. However a significant difference was found for Lp, Cpeak, before and after training staff, suggesting greater care by healthcare professionals performing their tasks. Even recognizing that a TP is quite important to change behaviors, this needs to be considered in a broader context to effectively control noise in the NICU.

We study the effects of entry of a foreign firm on domestic welfare in the presence of licensing, when the entrant is technologically inferior to the incumbent. We show that foreign entry increases domestic welfare for intermediate (respectively, sufficiently large) technological differences between the firms under licensing with fixed fee (respectively, output royalty).

The aim of this study was to analyze the efficacy of cognitive-motor dual-task training compared with single-task training on balance and executive functions in individuals with Parkinson's disease. Fifteen subjects, aged between 39 and 75 years old, were randomly assigned to the dual-task training group (n = 8) and single-task training group (n = 7). The training was run twice a week for 6 weeks. The single-task group received balance training and the dual-task group performed cognitive tasks simultaneously with the balance training. There were no significant differences between the two groups at baseline. After the intervention, the results for mediolateral sway with eyes closed were significantly better for the dual-task group and anteroposterior sway with eyes closed was significantly better for the single-task group. The results suggest superior outcomes for the dual-task training compared to the single-task training for static postural control, except in anteroposterior sway with eyes closed.

BACKGROUND: Metabolic syndrome (MS) is associated with increased incidence of diabetes and atherosclerotic complications. The new definition of the International Diabetes Federation (IDF) increases the population with this entity, compared to the NCEP ATP III definition. OBJECTIVES: To study the prevalence of coronary artery disease (CAD) and carotid intima-media thickness (IMT) in patients with and without MS, according to the NCEP ATP III and IDF definitions, and the predictive ability of carotid IMT for CAD. METHODS: We studied 270 consecutive patients admitted for elective coronary angiography due to suspicion of CAD. All patients underwent ultrasound study of the carotid arteries to measure IMT (the highest value between the right and left common carotid arteries was used in the analysis). Coronary stenosis of > or =70% (or 50% for the left main coronary artery) was considered significant. RESULTS: By the ATP III definition, 14% of the patients had MS, and these patients had a higher prevalence of CAD (87% vs. 63%, p = 0.004), but no significant difference was found for carotid IMT (1.03 +/- 0.36 mm vs. 0.95 +/- 0.35 mm, p=NS). With the IDF definition, 61% of the patients had MS; this group was slightly older and included more women. There were no differences in terms of CAD (68% vs. 63%) or carotid IMT (0.97 +/- 0.34 vs. 0.96 +/- 0.39 mm). On multivariate analysis, the ATP III definition of MS predicts CAD (OR 4.76, 95% CI 1.71-13.25, p = 0.003), but the IDF definition does not (OR 1.29, 95% CI 0.74-2.27, p = 0.37). On ROC curve analysis, an IMT of > or = 0.95 mm predicts CAD (AUC 0.66, p < 0.001), with a sensitivity of 52% and specificity of 75%. CONCLUSIONS: The new IDF definition increases the population with MS, decreasing the capacity to predict the presence of CAD. In our population, neither the ATP III nor the IDF definition showed differences in terms of carotid IMT. Carotid IMT can predict CAD, but with only modest sensitivity.

The aim of this study was to undertake a comparative analysis of the practices and information behaviour of European information users who visit information units specialising in European information in Portugal and Spain. The study used a quantitative methodology based on a questionnaire containing closed questions and one open question. The questions covered the general sociological profile of the respondents and their use of European Document Centres, in addition to analysing aspects associated with information behaviour relating to European themes. The study therefore examined data on the preferred means and sources for accessing European information, types of documents and the subjects investigated most. The use of European databases and the Internet to access material on Europe was also studied, together with the reasons which users considered made it easy or difficult to access European information, and the aspects they valued most in accessing this information. The questionnaire was administered in European Document Centres in 2008 and 2010.

RESUMO:O objectivo deste estudo foi analisar a relação entre mobilidade funcional, risco de queda, nível de actividade física e percepção de saúde de 34 indivíduos praticantes (n=18) e não praticantes (n=16) de exercício físico duas ou mais vezes por semana durante pelo menos 45 minutos, residentes na comunidade e ambulatórios. Tipo de estudo: transversal exploratório-descritivo. Metodologia: foi feito um levantamento das variáveis de caracterização (idade, sexo, habilitações literárias, situação sócio-económica, situação familiar) e do estado cognitivo e estado emocional/depressão (Mini-Mental State Examination e Geriatric Depression Scale). As variáveis em análise foram: a mobilidade funcional avaliada através do Timed up and GoTest, o risco de queda medido com o Funtional Reach Test, o nível de actividade física avaliado através do Questionário Internacional de Actividade Física (IPAQ) e a percepção de saúde medida através do SF-6D. Foi também questionada a prática de alguma modalidade de exercício físico, da sua frequência e duração. Os dados foram analisados através de estatística descritiva, foi realizada uma regressão linear múltipla e uma análise bivariada das correlações, utilizando o coeficiente de correlação linear de Pearson (p ≤ 0,05).Resultados: verificou-se que, na amostra global, a maioria dos indivíduos apresentou uma mobilidade funcional considerada normal (TUG<10 segundos), e um risco de queda moderado (FRT entre 15,24 e 25,40 cm), embora sem diferenças entre os grupos em análise. A actividade física apresentou uma duração média de 685,88±540,16 minutos por semana, sendo que 18 indivíduos praticavam exercício físico pelo menos 45 minutos e duas ou mais vezes por semana.A percepção do estado de saúde foi bastante satisfatória, sendo a pontuação média do SF-6D de 0,915±0,067. A análise entre grupos demonstrou que o grupo que praticava exercício físico apresentava um maior número de indivíduos na faixa etária dos 65-74 anos, tinha mais escolarização e melhor estado cognitivo. Estes indivíduos eram fisicamente mais activos e faziam-o, na sua maioria, com uma frequência bissemanal, apenas um desempenhando uma modalidade de intensidade vigorosa. A análise estatística demonstrou que: a mobilidade funcional e o risco de queda eram mais desfavoráveis nos indivíduos com mais idade; o estado cognitivo estava associado a maior mobilidade funcional; uma boa mobilidade funcional correspondeu a um risco de queda reduzido, a mais prática de actividade física, a melhor percepção do estado de saúde e a manutenção do estado cognitivo. Os indivíduos com menor risco de queda apresentaram melhor estado cognitivo e emocional. E este último correspondeu a uma melhor percepção do estado de saúde e a um melhor estado cognitivo. Conclusão: a manutenção da mobilidade funcional reduz o risco de queda aumenta a prática de actividade física e melhora a percepção de saúde de indivíduos com 65 ou mais anos residentes na comunidade.--------- ABSTRACT: Objective: the aim of this study was to analyze the relationship between functional mobility, falls risk, level of physical activity and health perception in a sample of 34 subjects, 18 that practice exercise two or more times a week for at least 45 minutes and 16 that don’t practice exercise, residents and community. Designs: cross-sectional exploratory-descriptive survey. Methods: descriptive variables are age, sex, education, socio-economic level, family status, cognitive status (Mini-Mental State Examination) and emotional status/depression (Geriatric Depression Scale). We analyze the functional mobility with the Timed up and Go Test, the falls risk with Functional Reach Test, the level of physical activity with the International Physical Activity Questionnaire (IPAQ) and health perception with SF-6D. We also questioned the practice of exercise, their frequency and duration. Data were analyzed using descriptive statistics, a multiple linear regression analysis and bivariate correlations, using the linear correlation coefficient of Pearson (p ≤ 0.05). Results: we found that, in the total sample, most individuals had considered a normal functional mobility (TUG <10 seconds), and a moderate falls risk (FRT between 15.24 and 25.40 cm), but no difference between groups. Physical activity showed an mean of 685.88 ± 540.16 minutes per week, with 18 individuals pratice physical exercise at least 45 minutes and two or more times per week. The mean score of the SF-6D was 0.915 ± 0.067 and the perception of health was satisfactory. The analysis between groups showed that the group that practice physical exercise had a greater number of individuals aged 65-74 years, had more schooling and better cognitive status. These subjects were more physically active and mostly did it two times a weak and only one playing a kind of vigorous intensity. The multiple linear regression and correlations, using the linear correlation coefficient of Pearson (p≤0.05) showed that: functional mobility and fall risk decrease with age increase. The cognitive status was associated with greater functional mobility, a good functional mobility corresponded to a reduced falls risk, more physical activity, a better perception of health status and maintenance of cognitive status. Subjects with lower falls risk had better cognitive and emotional state. And subjects with a better emotional status have a better health perception and better cognitive status. Conclusion: the maintenance of functional mobility reduces falls risk, increase physical activity and improves health perception of individuals with 65 years or older living in the community.

Introduction & Objectives: Several factors may influence the decision to pursue nonsurgical modalities for the treatment of non-melanoma skin cancer. Topical photodynamic therapy (PDT) is a non-invasive alternative treatment reported to have a high efficacy when using standardized protocols in Bowen’s disease (BD), superficial basal cell carcinoma (BCC) and in thin nodular BCC. However, long-term recurrence studies are lacking. The aim of this study was to evaluate the long-term efficacy of PDT with topical methylaminolevulinate (MAL) for the treatment of BD and BCC in a dermato-oncology department. Materials & Methods: All patients with the diagnosis of BD or BCC, treated with MAL-PDT from the years 2004 to 2008, were enrolled. Treatment protocol included two MAL-PDT sessions one week apart repeated at three months when incomplete response, using a red light dose of 37-40 J/cm2 and an exposure time of 8’20’’. Clinical records were retrospectively reviewed, and data regarding age, sex, tumour location, size, treatment outcomes and recurrence were registered. Descriptive analysis was performed using chi square tests, followed by survival analysis with the Kaplan-Meier and Cox regression models. Results: Sixty-eight patients (median age 71.0 years, P25;P75=30;92) with a total of 78 tumours (31 BD, 45 superficial BCC, 2 nodular BCC) and a median tumour size of 5 cm2 were treated. Overall, the median follow-up period was 43.5 months (P25;P75=0;100), and a total recurrence rate of 33.8% was observed (24.4 % for BCC vs. 45.2% for BD). Estimated recurrence rates for BCC and BD were 5.0% vs. 7.4% at 6 months, 23.4% vs. 27.9% at 12 months, and 30.0% vs. 72.4% at 60 months. Both age and diagnosis were independent prognostic factors for recurrence, with significantly higher estimated recurrence rates in patients with BD (p=0.0036) or younger than 58 years old (p=0.039). The risk of recurrence (hazard ratio) was 2.4 times higher in patients with BD compared to superficial BCC (95% CI:1.1-5.3; p=0.033), and 2.8 times higher in patients younger than 58 years old (95% CI:1.2-6.5; p=0.02). Conclusions: In the studied population, estimated recurrence rates are higher than those expected from available literature, possibly due to a longer follow-up period. To the authors’ knowledge there is only one other study with a similar follow-up period, regarding BCC solely. BD, as an in situ squamous cell carcinoma, has a higher tendency to recur than superficial BCC. Despite greater cosmesis, PDT might no be the best treatment option for young patients considering their higher risk of recurrence.
