Este relatório é o resultado descritivo e reflexivo de um Projeto de Educação e Intervenção Social, "Viver mais... Pensar o passado, viver o presente e sonhar o futuro", desenvolvido com um grupo de idosos do Centro de Caridade Nossa Senhora do Perpetuo Socorro. O projeto de investigação e de intervenção social seguiu as linhas metodológicas da Investigação Ação Participativa, em que os sujeitos são atores participativos e interventivos. Para o desenvolvimento deste projeto foi necessário recorrer a algumas técnicas de investigação, como a observação participante, conversas intencionais e análise documental, que facilitaram a recolha de informações, a análise da realidade e a intervenção desenvolvida. A partir da análise da realidade emergiram alguns problemas e necessidades que foram priorizados pelos sujeitos, daí surgiu o projeto de intervenção que aqui se apresenta com a finalidade de “Melhorar a qualidade de vida dos idosos do Centro de Dia do Centro de Caridade Nossa Senhora do Perpetuo Socorro, através da valorização das suas histórias de vida e melhoria das relações interpessoais”. Este projeto teve três grandes focos importantes que foi a recolha e construção das narrativas de vida de alguns participantes, a realização de vários encontros intergeracionais entre os idosos do Centro de Dia e as crianças do Jardim de Infância e o processo de consciencialização por parte dos idosos sobre a importância do grupo para o bem-estar pessoal, a gestão de conflitos e a melhoria das dinâmicas relacionais. Os resultados do projeto foram relevantes porque foram o produto de um processo reflexivo e de partilha entre os indivíduos, que favoreceu o desenvolvimento de um sentimento de utilidade e de valorização pessoal nos sujeitos, melhorando e fortalecendo assim as relações interpessoais entre eles.

Esta investigação surgiu da identificação de dificuldades ao nível da interação e gestão de conflitos entre pares, que a investigadora teve a oportunidade de observar no grupo de crianças com quem desenvolvia a sua prática educativa. Este estudo tem como objetivo compreender de que forma a transformação da praxis do educador influencia os processos de prevenção e a mediação dos conflitos entre pares no jardim-de-infância. Tendo em conta as formas e as características das relações entre pares na infância bem como o desenvolvimento moral das crianças em idade préescolar, a investigadora levou a cabo um projeto de investigação-ação com um grupo de vinte e quatro crianças de cinco anos. Ao longo do projeto, a investigadora procurou conhecer e experimentar estratégias de prevenção e mediação de conflitos interpares, monitorizando esse processo através da reflexão sobre a ação, assim como compreender os efeitos dessa experimentação no desenvolvimento profissional e na aprendizagem das crianças. Os processos do projeto são apresentados através de uma descrição densa dos eixos centrais da ação: construção do quadro de regras, organização do tempo e do grupo, organização do espaço e dos materiais, experimentação de instrumentos de pilotagem, reunião de grande grupo e estratégias de resolução de conflitos. Os dados apresentados resultam da interpretação das categorias emergentes da análise das entrevistas e das notas de campo. Através dessa interpretação, foi possível constatar os efeitos positivos do projeto, não apenas ao nível das competências de escuta e de negociação das crianças, mas também para a prática profissional da investigadora, que desenvolveu competências de reflexão e de autossupervisão.

BACKGROUND AND OBJECTIVES: Human Bocavirus (HBoV) has been described since 2005 as an etiological agent of respiratory virus infections. From 2001 to 2008 we investigated the etiology of HBoV among adults and children in different groups at risk of presenting complications arising from acute respiratory infection, the investigation was carried out in a tertiary hospital health care system in Brazil. METHODS: HBoV DNA was assayed in 598 respiratory samples from community and hospitalized patients by PCR. RESULTS: Of the 598 tested samples, 2.44% (8/328) of children, including five children with heart disease, and 0.4% (1/270) of adult bone-marrow-transplant were HBoV positive. CONCLUSIONS: These data suggested lower HBoV frequency among different at-risk patients and highlights the need to better understand the real role of HBoV among acute respiratory symptomatic patients.

This study aimed to determine the existence of blood vessels within ganglia of the myenteric plexus of the human esophagus and colon. At necropsy, 15 stillborns, newborns and children up to two years of age, with no gastrointestinal disorders, were examined. Rings of the esophagus and colon were analyzed and then fixed in formalin and processed for paraffin. Histological sections were stained by hematoxylin-eosin, Giemsa and immunohistochemistry for the characterization of endothelial cells, using antibodies for anti-factor VIII and CD31. Blood vessels were identified within the ganglia of the myenteric plexus of the esophagus, and no blood vessels were found in any ganglia of the colon. It was concluded that the ganglia of the myenteric plexus of the esophagus are vascularized, while the ganglia of the colon are avascular. Vascularization within the esophageal ganglia could facilitate the entrance of infectious agents, as well as the development of inflammatory responses (ganglionitis) and denervation, as found in Chagas disease and idiopathic achalasia. This could explain the higher frequency of megaesophagus compared with megacolon.

The anti-HIV drug abacavir is associated with idiosyncratic hypersensitivity reactions and cardiotoxicity. Although the mechanism underlying abacavir-toxicity is not fully understood, drug bioactivation to reactive metabolites may be involved. This work was aimed at identifying abacavir-protein adducts in the hemoglobin of HIV patients as biomarkers of abacavir bioactivation and protein modification. The protocol received prior approval from the Hospital Ethics Committee, patients gave their written informed consent and adherence was controlled through a questionnaire. Abacavir-derived Edman adducts with the N-terminal valine of hemoglobin were analyzed by an established liquid chromatography-electrospray ionization-tandem mass spectrometry method. Abacavir-valine adducts were detected in three out of ten patients. This work represents the first evidence of abacavir-protein adduct formation in humans. The data confirm the ability of abacavir to modify self-proteins and suggest that the molecular mechanism(s) of some abacavir-induced adverse reactions may require bioactivation.

RESUMO - A variação clínica indesejada é um problema global que atinge os diversos sistemas de saúde. Vários autores relacionaram as instituições de saúde e suas características (a oferta) com as decisões clínicas, originando variação clínica entre prestadores de cuidados de saúde. Este estudo procurou identificar a existência de variação clínica indesejada nos nascimentos assistidos no SNS entre 2002 e 2009. É conhecido que uma taxa elevada de cesarianas é prejudicial para as mães e crianças. Neste sentido, procurou analisar-se a variação na percentagem de cesarianas realizadas por hospital do SNS e a influência do número de profissionais nestes valores. A metodologia utilizada foi a análise de fontes de informação que incluíram a caracterização dos internamentos e o número e especialidade dos profissionais de saúde no SNS português. Os resultados permitem afirmar que existe de variação clínica indesejada nos nascimentos no SNS, nomeadamente: (1) a percentagem de cesarianas realizada por hospital varia entre 19,78% e 40,09%; (2) o número de médicos obstetras varia entre os hospitais do SNS, entre 2,1 e 31,1 por 1000 partos; (3) o número de enfermeiros obstetras varia entre 3,8 e 50,7 por 1000 partos; (4) o número médio de dias internamento da mulher é 1,54 dias mais curto nos partos vaginais, que nos partos por cesariana, e 1 dia para o tempo mediano; (5) o tempo mediano de internamento da mulher submetida a cesariana é mais curto nos hospitais que realizam mais este procedimento; (6) não existe relação entre a idade da mãe e a percentagem de cesarianas; (7) nem do número de profissionais de saúde ajustado por 1000 partos; (8) não é possível identificar alterações significativas na percentagem de cesarianas entre hospitais universitários e não universitários.

Allergy affects at least one-quarter of European schoolchildren, it reduces quality of life and may impair school performance; there is a risk of severe reactions and, in rare cases, death. Allergy is a multi-system disorder, and children often have several co-existing diseases, i.e. allergic rhinitis, asthma, eczema and food allergy. Severe food allergy reactions may occur for the first time at school, and overall 20% of food allergy reactions occur in schools. Up to two-thirds of schools have at least one child at risk of anaphylaxis but many are poorly prepared. A cooperative partnership between doctors, community and school nurses, school staff, parents and the child is necessary to ensure allergic children are protected. Schools and doctors should adopt a comprehensive approach to allergy training, ensuring that all staff can prevent, recognize and initiate treatment of allergic reactions.

RESUMO: Os vírus respiratórios continuam a ocupar um papel relevante na morbilidade e mortalidade infantil, tendo na última década sido alargado o espectro de vírus potencialmente causadores das infeções respiratórias. O diagnóstico destas infeções pode ser efetuado por várias metodologias, sendo as técnicas de biologia molecular consideradas as mais sensíveis para este fim. No âmbito do Projeto Ambiente e Saúde em Creches e Infantários (ENVIRH) foi efetuada uma comparação da prevalência dos principais vírus respiratórios em crianças em idade pré-escolar, com critérios de infeção respiratória, recorrendo a técnicas de biologia molecular, em duas populações: crianças que se encontravam na escola/domicilio e crianças que recorreram a uma urgência hospitalar. O estudo decorreu em dois períodos, de Fevereiro a Maio de 2011 e de Outubro de 2011 a Abril de 2012. Foram efetuadas duas colheitas de zaragatoas, uma nasal e outra orofaríngea. A metodologia utilizada para a identificação viral nas amostras foi a PCR e RT-PCR multiplex em tempo real. Os vírus pesquisados foram: Influenza A e B, Parainfluenza 1-4, Metapneumovirus humano, Vírus Sincicial respiratório (VSR), Rinovírus, Enterovírus, Coronavírus e Bocavirus. Foram realizadas 100 colheitas em crianças com idades compreendidas entre os 5 meses e os 5 anos. Foram obtidas 64 amostras dos infantários/domicílios, das quais 47 foram positivas. Da urgência Hospitalar obtiveram-se 36 amostras, em que 32 foram positivas. O vírus da gripe A (H3) foi o mais frequentemente detetado nas duas populações, mas apenas durante o surto de 2012. O VSR e os adenovírus foram mais frequentes nas crianças que recorreram ao hospital, ao contrário dos enterovirus e dos coronavírus, que não foram detetados nesta população. Os bocavirus nunca foram detetados isoladamente. Este estudo reforça a importância de se utilizarem técnicas de biologia molecular para o diagnóstico etiológico das infeções respiratórias, devido à elevada sensibilidade das mesmas, o que se reflete na elevada percentagem de amostras positivas. O facto de se utilizarem técnicas “multiplex”, que permitem a pesquisa simultânea de vários vírus, facilita a deteção de um maior espectro destes agentes. A elevada prevalência de Influenza A H3N2 deveu-se ao facto de grande parte do estudo ter coincidido com um período de surto por este vírus. O sistema de alerta montado durante o projeto ENVIRH pareceu promissor para uma eventual utilização futura em períodos de atividade gripal.--------------ABSTRACT: In the last decade, as respiratory viruses keep representing a relevant factor in child morbidity and mortality, the spectrum of viruses that may potentially cause respiratory infections has been widened. Within the several methodologies that may be applied in the diagnosis of these types of infections, the ones that use molecular biology are considered to be the most sensitive. The Environment and Health in Daycares and Nurseries Project (ENVIRH) arranged for a study, by means of molecular biology techniques, on the main respiratory viruses' influence in pre-school aged children with respiratory infection symptoms. This study compared children in two different populations: children at school or at home and children that were taken to a hospital emergency service. The study was conducted in two different time periods, one from February to May 2011 and the other from October 2011 to April 2012. During this time, two swab collections were held, one nasal and one oropharyngeal. PCR and RT-PCR multiplex in real time techniques were used for viral identification of the samples, searching for the viruses Influenza A and B, Parainfluenza 1-4, human Metapneumovirus, Respiratory Sincytial Virus (RSV), Rhinovirus, Enterovirus, Coronavirus and Bocavirus. One hundred (100) collections were held in children between the ages of 5 months and 5 years, sixty-four (64) at home/school and thirty-six (36) at the hospital's emergency service. From a total of seventy-nine (79) positive samples, forty-seven (47) were obtained at home/school and thirty-two (32) at the hospital. The virus detected the most in both populations was the Influenza A (H3), but only during the outbreak of 2012. Unlike the enteroviruses and coronaviruses, that were not detected within this population, the RSV and the adenoviruses were most common within the children at the hospital. Bocaviruses were never detected isolated from other viruses. The high percentage of positive samples reinforces the significance of using molecular biology techniques for the etiological diagnosis of respiratory infections. The use of multiplex techniques, that make the simultaneous search for multiple viruses possible, enhances the detection of a larger spectrum of such agents. Most of the study coincided with an outbreak of the Influenza A H3N2 virus, thus explaining the high number of its cases identified. The alert system set up during the ENVIRH project looked promising enough for eventual periods of flu activity in the future.

We retrospectively analyzed a series of 151 cases of cutaneous leishmaniasis treated between 1967 and 1982. One-hundred-and-thirty-nine (92%) patients presented with active lesions and were treated with daily doses of meglumine antimoniate: 81 adults received a 5-ml vial IM and 58 children received 1 to 5ml. Forty-five (32.4%) patients underwent continuous treatment with meglumine antimoniate for 25 to 116 days without rest intervals, and 94 (67.6%) intermittent treatment with 2 to 5 series of meglumine antimoniate. Intermittent series could include schedules of daily IM applications for 10 to 25 days each and intervals varying from 10 to 60 days. Antimony dose was calculated for 66 (47.5%) patients and ranged from 3.9 to 28.7 Sb5+/kg/day. Of these, 35 patients received >10mg and 31 patients <10mg Sb5+/kg/day. Median time of healing was longer for lesions on the legs and feet - 67.5 days versus 48.7 days (p < 0.001) for other sites. However, there were no significant differences in the median time of healing between adults and children, intermittent and continuous regimens or high and low antimony doses. Fifty-one patients were reassessed 5 to 14 years after treatment and showed no evidence of disease. These results support further investigation (clinical trials) on treatment using low doses of antimony.

INTRODUCTION: The case definition of influenza-like illness (ILI) is a powerful epidemiological tool during influenza epidemics. METHODS: A prospective cohort study was conducted to evaluate the impact of two definitions used as epidemiological tools, in adults and children, during the influenza A H1N1 epidemic. Patients were included if they had upper respiratory samples tested for influenza by real-time reverse transcriptase polymerase chain reaction during two periods, using the ILI definition (coughing + temperature > 38ºC) in period 1, and the definition of severe acute respiratory infection (ARS) (coughing + temperature > 38ºC and dyspnoea) in period 2. RESULTS: The study included 366 adults and 147 children, covering 243 cases of ILI and 270 cases of ARS. Laboratory confirmed cases of influenza were higher in adults (50%) than in children (21.6%) ( p < 0.0001) and influenza infection was more prevalent in the ILI definition (53%) than ARS (24.4%) (p < 0.0001). Adults reported more chills and myalgia than children (p = 0.0001). Oseltamivir was administered in 58% and 46% of adults and children with influenza A H1N1, respectively. The influenza A H1N1 case fatality rate was 7% in adults and 8.3% in children. The mean time from onset of illness until antiviral administration was 4 days. CONCLUSIONS: The modification of ILI to ARS definition resulted in less accuracy in influenza diagnosis and did not improve the appropriate time and use of antiviral medication.

INTRODUCTION: This study aimed to determine the epidemiology of the three most common nosocomial infections (NI), namely, sepsis, pneumonia, and urinary tract infection (UTI), in a pediatric intensive care unit (PICU) in a developing country and to define the risk factors associated with NI. METHODS: We performed a prospective study on the incidence of NI in a single PICU, between August 2009 and August 2010. Active surveillance by National Healthcare Safety Network (NHSN) was conducted in the unit and children with NI (cases) were compared with a group (matched controls) in a case-control fashion. RESULTS: We analyzed 172 patients; 22.1% had NI, 71.1% of whom acquired it in the unit. The incidence densities of sepsis, pneumonia, and UTI per 1,000 patients/day were 17.9, 11.4, and 4.3, respectively. The most common agents in sepsis were Enterococcus faecalis and Escherichia coli (18% each); Staphylococcus epidermidis was isolated in 13% of cases. In pneumonias Staphylococcus aureus was the most common cause (3.2%), and in UTI the most frequent agents were yeasts (33.3%). The presence of NI was associated with a long period of hospitalization, use of invasive devices (central venous catheter, nasogastric tube), and use of antibiotics. The last two were independent factors for NI. CONCLUSIONS: The incidence of NI acquired in this unit was high and was associated with extrinsic factors.

INTRODUCTION: The main route of human immunodeficiency virus (HIV) infection in children is from mother to child. The preventive measures established for the Aids Clinical Trial Group protocol 076 (ACTG 076) significantly reduces HIV vertical transmission rates. This study aims to evaluate the implementation of the ACTG 076 protocol in the maternity units of State of Sergipe, Brazilian northeast. METHODS: This is a descriptive, retrospective study with a quantitative approach, with HIV positive women and children exposed, attending a Maternity reference for high-risk pregnancies. Data were obtained from patient records registered in the years 1994 to 2010. RESULTS: Amongst the 110 pregnant women and exposed newborns, the ACTG 076 protocol was fully utilized in only 31.8% of the participants. During the prenatal period, zidovudine (ZDV) was taken by 79.1% of the pregnant women. Only 49.1% of HIV seropositive patients used ZDV during delivery. Two (1.8%) children were considered infected and 50 (45.5%) do not have a conclusive diagnosis to date. CONCLUSIONS: There were significant deficiencies in the prevention of mother-to-child transmission of HIV, including lack of compliance with the three phases of the ACTG 076 protocol; inadequacies in prenatal care; inappropriate mode of delivery and lack of adequate follow up of exposed children.

We aimed to assess and synthesize the information available in the literature regarding the treatment of American tegumentary leishmaniasis in special populations. We searched MEDLINE (via PubMed), EMBASE, LILACS, SciELO, Scopus, Cochrane Library and mRCT databases to identify clinical trials and observational studies that assessed the pharmacological treatment of the following groups of patients: pregnant women, nursing mothers, children, the elderly, individuals with chronic diseases and individuals with suppressed immune systems. The quality of evidence was assessed using the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approach. The available evidence suggests that the treatments of choice for each population or disease entity are as follows: nursing mothers and children (meglumine antimoniate or pentamidine), patients with renal disease (amphotericin B or miltefosine), patients with heart disease (amphotericin B, miltefosine or pentamidine), immunosuppressed patients (liposomal amphotericin), the elderly (meglumine antimoniate), pregnant women (amphotericin B) and patients with liver disease (no evidence available). The quality of evidence is low or very low for all groups. Accurate controlled studies are required to fill in the gaps in evidence for treatment in special populations. Post-marketing surveillance programs could also collect relevant information to guide treatment decision-making.
