This paper estimates the marginal willingness-to-pay for attributes of a hypothetical HIV vaccine using discrete choice modeling. We use primary data from 326 respondents from Bangkok and Chiang Mai, Thailand, in 2008–2009, selected using purposive, venue-based sampling across two strata. Participants completed a structured questionnaire and full rank discrete choice modeling task administered using computer-assisted personal interviewing. The choice experiment was used to rank eight hypothetical HIV vaccine scenarios, with each scenario comprising seven attributes (including cost) each of which had two levels. The data were analyzed in two alternative specifications: (1) best-worst; and (2) full-rank, using logit likelihood functions estimated with custom routines in Gauss matrix programming language. In the full-rank specification, all vaccine attributes are significant predictors of probability of vaccine choice. The biomedical attributes of the hypothetical HIV vaccine (efficacy, absence of VISP, absence of side effects, and duration of effect) are the most important attributes for HIV vaccine choice. On average respondents are more than twice as likely to accept a vaccine with 99% efficacy, than a vaccine with 50% efficacy. This translates to a willingness to pay US$383 more for a high efficacy vaccine compared with the low efficacy vaccine. Knowledge of the relative importance of determinants of HIV vaccine acceptability is important to ensure the success of future vaccination programs. Future acceptability studies of hypothetical HIV vaccines should use more finely grained biomedical attributes, and could also improve the external validity of results by including more levels of the cost attribute.

Summary: We present a new R package, diveRsity, for the calculation of various diversity statistics, including common diversity partitioning statistics (?, G) and population differentiation statistics (D, GST ', ? test for population heterogeneity), among others. The package calculates these estimators along with their respective bootstrapped confidence intervals for loci, sample population pairwise and global levels. Various plotting tools are also provided for a visual evaluation of estimated values, allowing users to critically assess the validity and significance of statistical tests from a biological perspective. diveRsity has a set of unique features, which facilitate the use of an informed framework for assessing the validity of the use of traditional F-statistics for the inference of demography, with reference to specific marker types, particularly focusing on highly polymorphic microsatellite loci. However, the package can be readily used for other co-dominant marker types (e.g. allozymes, SNPs). Detailed examples of usage and descriptions of package capabilities are provided. The examples demonstrate useful strategies for the exploration of data and interpretation of results generated by diveRsity. Additional online resources for the package are also described, including a GUI web app version intended for those with more limited experience using R for statistical analysis. © 2013 British Ecological Society.

The aim of this paper is to investigate the mechanism of nanoscale fatigue using nano-impact and multiple-loading cycle nanoindentation tests, and compare it to previously reported findings of nanoscale fatigue using integrated stiffness and depth sensing approach. Two different film loading mechanism, loading history and indenter shapes are compared to comprehend the influence of test methodology on the nanoscale fatigue failure mechanisms of DLC film. An amorphous 100 nm thick DLC film was deposited on a 500 μm silicon substrate using sputtering of graphite target in pure argon atmosphere. Nano-impact and multiple-load cycle indentations were performed in the load range of 100 μN to 1000 μN and 0.1 mN to 100 mN, respectively. Both test types were conducted using conical and Berkovich indenters. Results indicate that for the case of conical indenter, the combination of nano-impact and multiple-loading cycle nanoindentation tests provide information on the life and failure mechanism of DLC film, which is comparable to the previously reported findings using the integrated stiffness and depth sensing approach. However, the comparison of results is sensitive to the applied load, loading mechanism, test-type and probe geometry. The loading mechanism and load history is therefore critical which also leads to two different definitions of film failure. The choice of exact test methodology, load and probe geometry should therefore be dictated by the in-service tribological conditions, and where necessary both test methodologies can be used to provide better insights of failure mechanism. Molecular dynamics (MD) simulations of the elastic response of nanoindentation is reported, which indicates that the elastic modulus of the film measured using MD simulation was higher than that experimentally measured. This difference is attributed to the factors related to the presence of material defects, crystal structure, residual stress, indenter geometry and loading/unloading rate differences between the MD and experimental results.

Background

An evidence-based approach to health care is recognized internationally as a key competency for healthcare practitioners. This overview systematically evaluated and organized evidence from systematic reviews on teaching evidence-based health care (EBHC).

Methods/Findings

We searched for systematic reviews evaluating interventions for teaching EBHC to health professionals compared to no intervention or different strategies. Outcomes covered EBHC knowledge, skills, attitudes, practices and health outcomes. Comprehensive searches were conducted in April 2013. Two reviewers independently selected eligible reviews, extracted data and evaluated methodological quality. We included 16 systematic reviews, published between 1993 and 2013. There was considerable overlap across reviews. We found that 171 source studies included in the reviews related to 81 separate studies, of which 37 are in more than one review. Studies used various methodologies to evaluate educational interventions of varying content, format and duration in undergraduates, interns, residents and practicing health professionals. The evidence in the reviews showed that multifaceted, clinically integrated interventions, with assessment, led to improvements in knowledge, skills and attitudes. Interventions improved critical appraisal skills and integration of results into decisions, and improved knowledge, skills, attitudes and behaviour amongst practicing health professionals. Considering single interventions, EBHC knowledge and attitude were similar for lecture-based versus online teaching. Journal clubs appeared to increase clinical epidemiology and biostatistics knowledge and reading behavior, but not appraisal skills. EBHC courses improved appraisal skills and knowledge. Amongst practicing health professionals, interactive online courses with guided critical appraisal showed significant increase in knowledge and appraisal skills. A short workshop using problem-based approaches, compared to no intervention, increased knowledge but not appraisal skills.

Conclusions

EBHC teaching and learning strategies should focus on implementing multifaceted, clinically integrated approaches with assessment. Future rigorous research should evaluate minimum components for multifaceted interventions, assessment of medium to long-term outcomes, and implementation of these interventions.

Periodic monitoring of structures such as bridges is necessary as their condition can deteriorate due to environmental conditions and ageing, causing the bridge to become unsafe. This monitoring - so called Structural Health Monitoring (SHM) - can give an early warning if a bridge becomes unsafe. This paper investigates an alternative wavelet-based approach for the monitoring of bridge structures which consists of the use of a vehicle fitted with accelerometers on its axles. A simplified vehicle-bridge interaction model is used in theoretical simulations to examine the effectiveness of the approach in detecting damage in the bridge. The accelerations of the vehicle are processed using a continuous wavelet transform, allowing a time-frequency analysis to be performed. This enables the identification of both the existence and location of damage from the vehicle response. Based on this analysis, a damage index is established. A parametric study is carried out to investigate the effect of parameters such as the bridge span length, vehicle speed, vehicle mass, damage level, signal noise level and road surface roughness on the accuracy of results. In addition, a laboratory experiment is carried out to validate the results of the theoretical analysis and assess the ability of the approach to detect changes in the bridge response.

One of the main applications of serum proteomics is the identification of new biomarkers for animal disease or animal production. However, potential obstacles to these studies are the poor performance of affinity serum depletion methods based on human antigens when using animal samples, and loss of minor serum components bound to albumin and other proteins. In the present study, we have analyzed the efficiency and reproducibility of the ProteoMiner® beads with bovine and porcine serum samples, and compared to a traditional immunoaffinity-based albumin and IgG depletion system specific for human samples. The ProteoMiner kit is based on the use of a combinatorial peptide binding library and intends to enrich low-abundance proteins.

Objective
to systematically identify interventions that midwives could introduce to address post-traumatic stress in women following childbirth.

Methods
a search strategy was developed and relevant papers were identified from databases including Cinahl, Cochrane Library, EMBASE, Maternity and Infant Care, MEDLINE, PsycINFO, and Web of Science. Key search terms used were post-traumatic stress, post partum, intervention, controlled trial and review. Papers eligible for inclusion were primary studies and reviews of research published from 2002–2012, focusing on interventions which could be implemented by midwives for the prevention and/or management of PTSD. For primary studies, RCTs, controlled clinical trials, and cohort studies with a control group were eligible. Eligible reviews were those with a specified search strategy and inclusion/exclusion criteria. Methodological quality was assessed using recognised frameworks.

Findings
six primary studies and eight reviews were eligible for inclusion. The majority of included studies or reviews focused on debriefing and/or counselling interventions; however the results were not consistent due to significant variation in methodological quality and use of dissimilar interventions. Two of the reviews considered the general management of post partum PTSD and one broadly covered anxiety during pregnancy and the post partum, incorporating a section on PTSD. The majority of women reported that the opportunity to discuss their childbirth experience was subjectively beneficial.

Conclusions and implications for practice
no evidence-based midwifery interventions were identified from this systematic review that can be recommended for introduction into practice to address PTSD. It is recommended that future research in this area should incorporate standardised interventions with similar outcome measures to facilitate synthesis of results. Further research on interventions used in non-maternity populations is needed in order to confirm their usefulness in addressing post partum PTSD.

The use of an acid violet 7 (AV7) smart ink to assess the activity of photocatalytic paint is demonstrated. A linear correlation is established between the change in oxidized dye concentration, as measured by diffuse reflectance, and the change in the green component of the RGB color values, obtained using a portable hand-held scanner, suggesting that such tests can be monitored easily using an inexpensive piece of hand-held office equipment, as opposed to an expensive lab-based instrument, such as a diffuse reflectance UV/vis spectrophotometer. The bleaching of the AV7 follows first order kinetics, at a rate that is linearly dependent upon the UVA irradiance (0.30–3.26 mW cm–2). A comparison of relative rate of bleaching of the AV7 ink with the relative rate of removal of NOx, as determined using the ISO test (ISO 22197-1:2007), established a linear relationship between the two sets of results and the relevance of this correlation is discussed briefly.

Many of the bridges currently in use worldwide are approaching the end of their design lives. However, rehabilitating and extending the lives of these structures raises important safety issues. There is also a need for increased monitoring which has considerable cost implications for bridge management systems. Existing structural health monitoring (SHM) techniques include vibration-based approaches which typically involve direct instrumentation of the bridge and are important as they can indicate the deterioration of the bridge condition. However, they can be labour intensive and expensive. In the past decade, alternative indirect vibration-based approaches which utilise the response of a vehicle passing over a bridge have been developed. This paper investigates such an approach; a low-cost approach for the monitoring of bridge structures which consists of the use of a vehicle fitted with accelerometers on its axles. The approach aims to detect damage in the bridge while obviating the need for direct instrumentation of the bridge. Here, the effectiveness of the approach in detecting damage in a bridge is investigated using a simplified vehicle-bridge interaction (VBI) model in theoretical simulations and a scaled VBI model in a laboratory experiment. In order to identify the existence and location of damage, the vehicle accelerations are recorded and processed using a continuous Morlet wavelet transform and a damage index is established. A parametric study is carried out to investigate the effect of parameters such as the bridge span length, vehicle speed, vehicle mass, damage level and road surface roughness on the accuracy of results.

Structural and functional information encoded in DNA combined with unique properties of nanomaterials could be of use for the construction of novel biocomputational circuits and intelligent biomedical nanodevices. However, at present their practical applications are still limited by either low reproducibility of fabrication, modest sensitivity, or complicated handling procedures. Here, we demonstrate the construction of label-free and switchable molecular logic gates (AND, INHIBIT, and OR) that use specific conformation modulation of a guanine- and thymine-rich DNA, while the optical readout is enabled by the tunable metamaterials which serve as a substrate for surface enhanced Raman spectroscopy (MetaSERS). Our MetaSERS-based DNA logic is simple to operate, highly reproducible, and can be stimulated by ultra-low concentration of the external inputs, enabling an extremely sensitive detection of mercury ions down to 2×10-4 ppb, which is four orders of magnitude lower than the exposure limit allowed by United States Environmental Protection Agency

The advent of novel genomic technologies that enable the evaluation of genomic alterations on a genome-wide scale has significantly altered the field of genomic marker research in solid tumors. Researchers have moved away from the traditional model of identifying a particular genomic alteration and evaluating the association between this finding and a clinical outcome measure to a new approach involving the identification and measurement of multiple genomic markers simultaneously within clinical studies. This in turn has presented additional challenges in considering the use of genomic markers in oncology, such as clinical study design, reproducibility and interpretation and reporting of results. This Review will explore these challenges, focusing on microarray-based gene-expression profiling, and highlights some common failings in study design that have impacted on the use of putative genomic markers in the clinic. Despite these rapid technological advances there is still a paucity of genomic markers in routine clinical use at present. A rational and focused approach to the evaluation and validation of genomic markers is needed, whereby analytically validated markers are investigated in clinical studies that are adequately powered and have pre-defined patient populations and study endpoints. Furthermore, novel adaptive clinical trial designs, incorporating putative genomic markers into prospective clinical trials, will enable the evaluation of these markers in a rigorous and timely fashion. Such approaches have the potential to facilitate the implementation of such markers into routine clinical practice and consequently enable the rational and tailored use of cancer therapies for individual patients. © 2010 Macmillan Publishers Limited. BACKGROUND: Falls and fall-related injuries are symptomatic of an aging population. This study aimed to design, develop, and deliver a novel method of balance training, using an interactive game-based system to promote engagement, with the inclusion of older adults at both high and low risk of experiencing a fall.

STUDY DESIGN: Eighty-two older adults (65 years of age and older) were recruited from sheltered accommodation and local activity groups. Forty volunteers were randomly selected and received 5 weeks of balance game training (5 males, 35 females; mean, 77.18 ± 6.59 years), whereas the remaining control participants recorded levels of physical activity (20 males, 22 females; mean, 76.62 ± 7.28 years). The effect of balance game training was measured on levels of functional balance and balance confidence in individuals with and without quantifiable balance impairments.

RESULTS: Balance game training had a significant effect on levels of functional balance and balance confidence (P < 0.05). This was further demonstrated in participants who were deemed at high risk of falls. The overall pattern of results suggests the training program is effective and suitable for individuals at all levels of ability and may therefore play a role in reducing the risk of falls.

CONCLUSIONS: Commercial hardware can be modified to deliver engaging methods of effective balance assessment and training for the older population.

O objectivo geral deste trabalho consistiu no desenvolvimento de novos sensores químicos de fibra óptica (OF) para análise de compostos orgânicos voláteis (VOCs) em ambientes industriais. A componente de detecção dos sensores desenvolvidos é constituída por uma pequena secção de fibra óptica revestida com um filme de polímero. A morfologia dos filmes poliméricos foi analisada e caracterizada por microscopia electrónica de varrimento (SEM), sendo a espessura dos filmes determinada por espectroscopia de retrodispersão de Rutherford (RBS, acrónimo do inglês Rutherford backscattering spectrometry). O desempenho analítico dos sensores de OF foi avaliado relativamente a diferentes parâmetros operacionais, tais como, concentração da solução de revestimento, técnica de deposição do filme polimérico, temperatura da célula de injecção, temperatura de cura do material polimérico, caudal do gás de arraste, comprimento de onda e frequência de funcionamento do laser, configurações estruturais da célula de injecção e do tubo analítico. Foram desenvolvidos dois sensores de OF a operar na região do infravermelho para determinação de diferentes classes de VOCs, nomeadamente hidrocarbonetos aromáticos, clorados e alifáticos, além de álcoois. Os sensores de OF desenvolvidos apresentaram adequadas características analíticas em termos de sensibilidade, linearidade, repetitibilidade e reprodutibilidade do sinal analítico, sendo o tempo de resposta de aproximadamente 30 segundos. Foi também desenvolvido um sensor de OF para especiação de benzeno, tolueno e o-xileno a operar na região do visível (635 - 650 nm), tendo sido aplicado à análise de amostras reais de ar de uma indústria de solventes. Relativamente à monitorização de VOCs em ambientes industriais, foi desenvolvido um sensor de OF para monitorização in situ e de forma remota (até uma distância máxima de 60 metros do local de amostragem) de benzeno, tolueno, etilbenzeno, p-xileno, m-xileno e o-xileno (BTEX), utilizando um díodo laser a 1550 nm. O desempenho analítico do sensor desenvolvido foi comparado, para a determinação de BTEX, com a cromatografia gasosa acoplada à detecção com ionização de chama (GC-FID). Foram ainda desenvolvidos dois detectores de fibra óptica acoplados a um cromatógrafo de gás para especiação de álcoois e hidrocarbonetos aromáticos. A metodologia desenvolvida baseada em cromatografia gasosa acoplada a um detector de fibra óptica (GC-OF) foi aplicada à análise de amostras reais de ar de uma indústria de solventes, comparando os respectivos resultados com os obtidos por GC-FID. Por fim foi efectuado um estudo visando a obtenção de um modelo geral para a resposta analítica dos sensores de fibra óptica desenvolvidos.

Esta tese descreve diversas estratégias preparativas assim como estudos de caracterização de nanocompósitos e outras nanoestruturas, para a análise em espectroscopia de Raman com intensificação por superfície (SERS). Em particular foi estudada a incorporação de nanopartículas (NPs) metálicas de Ag e Au em matrizes poliméricas visando avaliar o potencial destes materiais como novos substratos para SERS na deteção de moléculas. O primeiro capítulo consiste numa revisão bibliográfica, onde é destacado o desenvolvimento de novos substratos para SERS utilizando nomeadamente nanopartículas de Ag, Au e Cu. Numa primeira fase, esta secção apresenta uma breve descrição sobre as propriedades plasmónicas dos metais utilizados e alguns conceitos básicos de espectroscopia de difusão de Raman. Posteriormente, descreve-se em mais detalhe o efeito de SERS, revendo-se sobretudo a química de materiais descrita em trabalhos recentes tendo em conta a sua utilização como novas plataformas para análise química por SERS. O capítulo 2 descreve a síntese e caracterização de nanocompósitos de poli(acrilato de butilo) obtidos através de polimerização in situ por miniemulsão na presença de nanopartículas de Ag. Os nanocompósitos Ag/PtBA foram investigados como novos substratos de SERS visando o desenvolvimento de novas plataformas versáteis para deteção molecular. Estudos como o efeito da temperatura, pH e pressão foram investigados, visando a compreensão do efeito da matriz polimérica na difusão/adsorção do analito (ácido tiossalicílico) na superfície metálica. No capítulo 3, os nanocompósitos de Ag/PtBA descritos anteriormente foram investigados detalhadamente como substratos para bio-deteção em SERS, usando a adenina como analito modelo. Os nanocompósitos foram submetidos a vários tratamentos pré-analíticos para a bio-deteção da adenina. Foram realizadas experiências nos sistemas Ag/PtBA de modo a obter informação sobre o efeito do pH na deteção deste analito em soluções aquosas diluídas. Os nanocompósitos poliméricos obtidos apresentam a vantagem de poderem ser processados utilizando tecnologia disponível. Pelo que o estudo das suas propriedades térmicas é especialmente relevante. Assim, a influência da incorporação de NPs inorgânicas na temperatura de transição vítrea (Tg) do polímero PtBA foi investigada por meio de calorimetria diferencial de varrimento (DSC) e os resultados são apresentados no capítulo 4. Estes estudos descrevem efeitos na Tg do PtBA quando analisado em nanocompósitos obtidos por diferentes métodos (métodos ex situ e in situ), contendo nanopartículas metálicas com diferentes distribuições de tamanho e presentes em quantidade variável na matriz polimérica. Estes estudos possibilitaram relacionar a influência das NPs metálicas na mobilidade das cadeias poliméricas com as propriedades térmicas observadas. O capítulo 5 descreve a preparação e caracterização de materiais compósitos de base polimérica (PtBA) e NPs de Au com morfologia de bastonetes (NRs). Estes materiais foram investigados como substratos para SERS tendo originado um bom sinal de SERS na análise de 2-2’-ditiodipiridina. Investigouse igualmente o efeito da variação da morfologia das NPs metálicas (esferas e bastonetes), a razão de aspecto (R.A.) dos bastonetes e o tipo de matriz polimérica (PtBA e PnBA) no sinal de SERS. No capítulo 6 é descrita a utilização da técnica de SERS como método alternativo para a monitorização de alterações morfológicas de coloides de NRs de Au. Os NRs de Au foram recolhidos em diferentes fases de oxidação promovida pela presença de K2S2O8 e a sua sensibilidade como substratos de SERS foi avaliada utilizando o anião dietilditiocarbamato (DTC) como analito modelo. Os estudos foram realizados utilizando as linhas de excitação a 1064 nm e 633 nm. Este estudo demonstrou que a sensibilidade dos NRs de Au como substratos de SERS diminui à medida que a sua R.A. diminui devido à competitividade do CTAB (estabilizante) e o DTC pela superfície dos NRs. É de salientar que este processo é acompanhado por um diferente comportamento em termos de adsorção dos dois tautómeros do DTC à superfície do metal. O capítulo 7 introduz um novo tipo de compósitos para SERS, utilizando matrizes biopoliméricas. Assim, descreve-se a preparação e caracterização de nanocompósitos de carragenano e NPs de Ag. Nesta secção é avaliada a utilização destes materiais como novos substratos para a análise em SERS, utilizando a 2-2’-ditiodipiridina como molécula modelo. Descrevem-se estudos pioneiros que procuram relacionar a dependência do sinal de SERS com a força do gel. Para tal, realizou-se um estudo sistemático aos nanobiocompósitos usados como substratos de SERS em diferentes condições analíticas e investigaram-se as suas propriedades reológicas. No capitulo 8 é descrita a investigação de nanocompósitos de Ag/Gelatina como substratos para SERS, utilizando o anião dietilditiocarbamato como analito modelo. Realizaram-se várias experiências para correlacionar a variação da força do gel com o aumento do sinal de SERS bem como a diferente adsorção dos dois tautómeros do DTC à superfície do metal. Ao longo desta dissertação são apresentados metodologias distintas para a preparação e obtenção de nanocompósitos com base em polímeros (sintéticos ou naturais) e NPs metálicas (Ag e Au). Esta investigação não só permitiu a síntese e estudo de novos substratos para SERS mas também a compreensão do efeito matriz/NPs metálicas no sinal de SERS e na formação de “hot spots”. Este trabalho contribui para o enriquecimento na área da Nanociência e Nanotecnologia demonstrando a eficácia e reprodutibilidade de nanocompósitos com base em polímeros como novos substratos para SERS. Embora as propriedades óticas apresentadas por estes materiais serem aqui direcionadas para a deteção molecular pela técnica de SERS, estes materiais podem ser investigados em outras área tecnológicas.

Background: The increasing resistance of Gram-negative bacteria isolated from nosocomial infections and chronic wounds, such as diabetic foot ulcers has renewed research interests in the use of polymyxins in the treatment of multidrug resistant infections. The added resistance conferred by biofilm development in such infections and the absence of novel antibiotics presuppose that polymyxins are the likely drugs of choice in spite of their nephrotoxicity. The effects of PMB and PMBN have been previously assessed on planktonic bacteria isolated from various infections. Methods: This current study assessed the synergy between a PMB/PMBN and two antibiotics (ceftazidime and levofloxacin) in an attempt to develop a strategy for biofilm disruption using the Minimum Biofilm Eradication Concentration Physiology and Genetic assay (MBEC™ P & G, Innovotech Inc, Edmonton, Alberta, Canada) according to manufacturer’s instructions. Klebsiella pneumoniae (K. pneumoniae) and Proteus mirabilis (P. mirabilis) biofilms of initial broth suspensions of 108 colony forming units per mL, cultivated on the pegs of the MBEC device were challenged with 5120 µg/mL of both ceftazidime and levofloxacin in a ten-fold dilution assay and in the presence of 100 and 500 µg/mL PMB and PMBN. Results: From table of results (Table 1), it can be deduced that both ceftazidime and levofloxacin are very effective in inhibiting biofilm development (as shown by percentage inhibition (PI)) when augmented with PMB and PMBN. This is about 100-fold increase in efficacy when compared to the antibiotics used on their own. The percentage reduction (PR) in biofilm was also increased considerably when PMB and PMBN concentrations were increased to 500 µg/mL. PMB was more effective than its less antibacterial derivative PMBN. Levofloxacin was also found to be more effective than ceftazidime when combined with both PMB and PMBN due to its enhanced cell-membrane permeability and as an anti-DNA replication uncoupling agent. Conclusion: The above results indicate that the synergy between antibiotics and cell membrane permeabilising agents may provide alternate strategies towards biofilm eradication
