Although suicidality is associated with mental illness in general and depression in particular, many depressed individuals do not attempt suicide and some individuals who attempt to or do die by suicide do not present depressive symptoms. This article aims to contribute to a more psychosocial approach to understanding suicide risk in nonclinical populations. In advocating a psychosocial perspective rather than a depression-focused approach, this article presents four diverse studies that demonstrate sampling and measurement invariance in findings across different populations and specific measures. Study 1 tests the mediation effects of 2 interpersonal variables, thwarted belongingness and perceived burdensomeness, in the association between depressive symptoms and recent suicidality. Studies 2 and 3 evaluate the contribution of hopelessness and psychache, beyond depressive symptoms, to suicidality. Study 4 tests the contribution of life events behind depressive symptoms, and other relevant sociodemographic and clinical variables, to the estimation of “future suicidality.” Overall, results demonstrate that depressive symptoms do not directly predict suicidality in nonclinical individuals, but that other psychosocial variables mediate the association between depressive symptoms and suicidality or predict suicidality when statistically controlling for depressive symptoms. The article contributes to understanding some of the nonpsychopathological factors that potentially link depressive symptoms to suicide risk and that might themselves contribute to suicidality, even when controlling for depressive symptoms.

Questo studio si concentra su tre direttrici fondamentali: la monomania religiosa, la medicina legale e il diritto penale nell'Italia postunitaria. Nell’ambito di un contesto temporale ben definito, i cui estremi cronologici coincidono con l’Unificazione italiana (1861) e con i primi anni del XX secolo, si sono approfondite le numerose implicazioni mediche e giuridiche della malattia mentale nella forma di monomania religiosa. In particolare, prendendo spunto da una serie di condotte criminali frutto di suggestione religiosa o di deliri ascetici in soggetti diagnosticati come ‛monomani’, si sono indagati i risvolti giuridico-processuali di alcune vicende fortemente rappresentative di tali percorsi. L'ampia documentazione d'archivio recuperata consente di verificare l'orientamento della magistratura all'indomani dell'entrata in vigore del Codice penale per il Regno d'Italia (1889). Nei casi esaminati, alla diagnosi di alienazione mentale degli imputati seguiva la dichiarazione di non imputabilità e l'affidamento degli stessi all'Autorità amministrativa che ne disponeva il ricovero in manicomio.

The Building Partnerships Program at the University of Queensland, Australia seeks to address the dual challenge of preparing doctors who are responsive to the community while providing a meaningful context for social sciences learning. Through partnerships with a diverse range of community agencies, the program offers students opportunities to gain non-clinical perspectives on health and illness through structured learning activities including: family visits; community agency visits and attachments; and interview training. Students learn first-hand about psychosocial influences on health and how people manage health problems on a day-to-day basis. They also gain insights into the work of community agencies and how they as future doctors might work in partnership with them to enhance patient care. We outline the main components of the program, identify challenges and successes from student and community agency perspectives, and consider areas that invite further development.

RESUMO: Num quadro de profundas transformações demográficas, o envelhecimento populacional coloca muitas questões à sociedade. A promoção da saúde e de comportamentos saudáveis desempenha um papel importante numa perspectiva de Envelhecimento Activo. Hoje em dia existem inúmeras evidências científicas de que a Actividade Física regular tem importantes benefícios, tanto a nível físico como psicológico, com um impacto significativo no bem-estar e na qualidade de vida das pessoas em todas as idades. Para fomentar a prática de Actividade Física há um envolvimento cada vez maior dos profissionais de saúde, visto serem figuras de referência para os pacientes. Desta forma, foi objectivo do nosso estudo verificar a existência de uma associação positiva entre a prática de Actividade Física e a Consultoria em Actividade Física, baseado nas seguintes variáveis: análise sócio-demográfica dos indivíduos; percepções e práticas de Actividade Física. Para tal estudámos uma amostra de conveniência, seleccionada a partir dos utentes de ambulatório das Unidades de Saúde Familiar de Benfica e Carnide, constituída por 5 pessoas, todas do sexo feminino, com idades compreendidas entre os 50 e os 65 anos, que se encontravam clinicamente estáveis. Os instrumentos de avaliação utilizados foram os seguintes: Entrevista Semi- Estruturada Inicial e Final, Questionário Internacional de Actividade Física – Versão Longa, e Questionário de Satisfação. Com os resultados obtidos podemos concluir que a Consultoria em Actividade Física contribuiu para o aumento da consciência das pessoas para a necessidade de movimentos e de Actividade Física para melhorar a saúde a nível físico, psicológico e social.-------------- ABSTRACT: In a context of profound demographic changes, population aging poses many questions to society. Health promotion and healthy behaviors play an important role in an Active Ageing perspective. Nowadays there are plenty of scientific evidence that regular physical activity has important benefits, both physically and psychologically, with significant impact on the wellbeing and quality of life for people of all ages. To encourage the practice of physical activity there is an increasing involvement of health professionals, because they are reference figures for the patients. Thus, the aim of our study was to verify the existence of a positive association between the practice of Physical Activity and Physical Activity Consulting, based on the following variables: social demographic features of individuals, perceptions and practices of Physical Activity. To this end we studied a convenience sample, selected from outpatients of Family Health Units in Benfica and Carnide, consisting of 5 persons, all female, aged between 50 and 65, who were clinically stable. The assessment instruments used were the following: Initial and Final Semi-Structured Interview, International Physical Activity Questionnaire - Long Version, and Satisfaction Questionnaire. With these results we can conclude that Physical Activity Consulting contributed to increase people's awareness of the need for movement and physical activity, in order to improve health at the physical, psychological and social level.

Resumo - As doenças crónicas não transmissíveis são uma ameaça crescente à Saúde Pública em Portugal. As principais causas de mortalidade e morbilidade são doenças relacionadas com os estilos de vida, hábitos alimentares e de actividade física. Os Cuidados de Saúde Primários estão na linha da frente para dar resposta a estas patologias. Os profissionais de saúde, nomeadamente médicos e enfermeiros, sentem dificuldades para as tratar, como a falta de tempo, de conhecimentos e de confiança para o fazer, bem como uma descrença na efectividade das suas intervenções no âmbito da mudança comportamental destes pacientes. A dificuldade em referenciar estes pacientes a outros profissionais, especializados, como os nutricionistas e os fisiologistas do exercício, implica dotar médicos e enfermeiros com as competências básicas de aconselhamento alimentar e de actividade física, bem como serem capazes de assumirem uma atitude centrada no paciente e motivadora da mudança comportamental. O objectivo deste estudo é avaliar os conhecimentos, atitudes e práticas no tratamento da obesidade e sua associação com o nível de actividade física reportado por médicos e enfermeiros. Este é um estudo observacional, transversal, que recorre à aplicação de um questionário de resposta directa. --------Abstract - Non communicable chronic diseases are increasingly relevant public health threats. The main causes of mortality and morbidity in Portugal are lifestyle, food and exercise habits, related diseases. Primary health care services are in the front line to adress this pathologies. Health care professionals, namely physicians and nurses, face numerous barriers like reduced consultation time, knowledge and confidence to deal with this problems, as well as a disbelief in the efectiviness of their intervention in patients health behaviour change. The inhability to reference this patients to nutrition and exercise specialists, increases the need to give physicians and nurses the adequate nutrition and exercise basic counselling skills, as well as promoting a patient centred attitude that enables them to increase patients motivation to health behaviour change. Hypoglycemia is a potentially serious complication of insulin therapy. Some insulin-dependent diabetic patients can benefit from continuous subcutaneous insulin infusion therapy (an "insulin pump"), which in most case improves glycemia control and decreases the occurrence of hypoglycemic episodes. However, such events may occur, particularly during initial treatment phases or pregnancy. Severe hypoglycemia is mainly managed by stopping the insulin pump and insuring an adequate carbohydrate intake. Patients with insulin pumps and their entourage should receive specific instruction in the adjustment of pump flow in the presence of dysglycemia-inducing circumstances (illness, physical exertion), as well as in anticipation of high-risk situations, such as motor-vehicle driving.

Background: This study explores significant ones' implication before and after transplantation. Methods: Longitudinal semi-structured interviews were conducted in 64 patients awaiting all-organ transplantation. Among them, 58 patients spontaneously discussed the importance of their significant other in their daily support. Discourse analysis was applied. Findings: During the pre-transplantation period renal patients reported that significant others took part in dialysis treatment and participated to regimen adherence. After transplantation, quality of life improved and the couple dynamics returned to normal. Patients awaiting lung or heart transplantation were more heavily impaired. Significant others had to take over abandoned roles. After transplantation resuming normal life became gradually possible, but after one year either transplantation health benefits relieved physical, emotional and social loads, or complications maintained the level of stress on significant others. Discussion: Patients reported that significant others had to take over various responsibilities and were concerned about long-term stress that should be adequately supported.

La medicina occidental és la imperant a tot el planeta. Malgrat tot, a la Xina la medicina tradicional hi té un pes important. La convivència entre totes dues visions de la salut i la malaltia -l'occidental i l'oriental- no és senzilla. En aquest treball es fa un recorregut pel sistema de salut xinès per a fer una aproximació a la situació de la medicina xinesa a la Xina contemporània.
