Este estudo tem como objetivo principal analisar a oferta formativa a nível dos cursos profissionais e a sua adequação ao mercado de trabalho. Nesse sentido, analisou-se a evolução do ensino profissional em Portugal, nomeadamente em escolas públicas. A abertura das escolas secundárias da rede pública à educação e formação profissional tornou-se um facto incontornável, com a última revisão curricular do ensino secundário a permitir um arrojado salto no desenvolvimento, ao qual não é alheia a sua crescente valorização e consolidação no contexto da ação educativa. As sucessivas reformas do Ensino Secundário, também foram objeto de exploração deste trabalho. Ao longo das décadas, repercutiram-se várias reformas das políticas educativas que, entretanto, culminando na Reforma do Ensino Secundário, em 2004, assumiram a importância do ensino profissionalmente qualificante, com uma expressão no sistema educativo mais intensa que nunca. Neste estudo participaram alunos de um curso profissional, a Coordenadora do Agrupamento de Escolas e a Coordenadora da Associação Empresarial inquiridos sobre as representações que fazem sobre a oferta formativa disponibilizada e a da sua adequação ao mercado empresarial. O presente estudo permitiu perceber que a Escola não possui uma relação objetiva com o mercado empresarial, mas que os alunos têm vontade de se integrar numa empresa da sua região. O resultado do estudo aponta para a existência de uma oferta formativa que não parece estar adequada às necessidades do mercado de trabalho local.

PURPOSE: To present the long-term follow-up of 10 adolescents and young adults with documented cognitive and behavioral regression as children due to nonlesional focal, mainly frontal, epilepsy with continuous spike-waves during slow wave sleep (CSWS). METHODS: Past medical and electroencephalography (EEG) data were reviewed and neuropsychological tests exploring main cognitive functions were administered. KEY FINDINGS: After a mean duration of follow-up of 15.6 years (range, 8-23 years), none of the 10 patients had recovered fully, but four regained borderline to normal intelligence and were almost independent. Patients with prolonged global intellectual regression had the worst outcome, whereas those with more specific and short-lived deficits recovered best. The marked behavioral disorders resolved in all but one patient. Executive functions were neither severely nor homogenously affected. Three patients with a frontal syndrome during the active phase (AP) disclosed only mild residual executive and social cognition deficits. The main cognitive gains occurred shortly after the AP, but qualitative improvements continued to occur. Long-term outcome correlated best with duration of CSWS. SIGNIFICANCE: Our findings emphasize that cognitive recovery after cessation of CSWS depends on the severity and duration of the initial regression. None of our patients had major executive and social cognition deficits with preserved intelligence, as reported in adults with early destructive lesions of the frontal lobes. Early recognition of epilepsy with CSWS and rapid introduction of effective therapy are crucial for a best possible outcome.

OBJECTIVES: To investigate the development of the ureterovesical junction in rats. METHODS: A total of 110 albino rats (50 prenatal and 60 newborn) with a gestation of 21 days were studied at the age of 17 days after conception until 5 days after birth. The lower urinary tract was microdissected. Microphotography (110 animals), histologic examination (44 animals), and scanning electron microscopy (66 animals) of the ureterovesical junction were performed. Urea and creatinine from the amniotic fluid of 20 fetuses and from the urine of 10 neonates were measured. RESULTS: At day 17 after conception, separate penetration of the mesonephric duct and ureter into the wall of the urogenital sinus was observed. Continuity between the lumen of the ureter and the urogenital sinus was established on day 19 after conception. The straight passage of the intramural ureter into the urogenital sinus at day 17 after conception changed to the definitive L-shape with a vertical entry into the bladder on day 5 after birth. In the distal ureter, the change of the mesenchymal tissue into immature smooth muscle was first observed at birth, and the muscle became mature on the fifth postnatal day. At birth, Waldeyer's sheath was recognized. The creatinine and urea levels were stable prenatally (average 22.4 micromol/L and 6.88 mmol/L, respectively) and rose significantly postnatally (average 133 micromol/L and 32.65 mmol/L, respectively). CONCLUSIONS: The attachment of the ureter to the urogenital sinus and later to the bladder, the modification of its passage, and its mobility within Waldeyer's sheath may be essential in preventing vesicoureteral reflux. The production of urine and its flow does not seem to be the trigger of ureteral smooth muscle formation.

The physical-activity and sporting at the child and the teenager is probably, in these years 2000, in full change. In a paradoxical way, extremely sporting children or teenagers are living beside extremely sedentary school-boys, neglecting the majority of the physical-activities and preferring a home-lifestyle. In the evaluation of overload sporting lesion of at teenager, it is thus imperative to take into account not only the individual characteristics of the child: its sex, its age, its stage of growth, its psychology, the presence or not of preexistent pathologies or anatomical disorders. It is naturally necessary to wonder about the training methods of the activity, but it appears fundamental to me to evaluate the child from a sensitivo-motor point of view and this can be carried out by assessments physio-therapeutic or aptitude tests carried out by doctors of the sport.

Clinical responses to anticancer therapies are often restricted to a subset of patients. In some cases, mutated cancer genes are potent biomarkers for responses to targeted agents. Here, to uncover new biomarkers of sensitivity and resistance to cancer therapeutics, we screened a panel of several hundred cancer cell lines--which represent much of the tissue-type and genetic diversity of human cancers--with 130 drugs under clinical and preclinical investigation. In aggregate, we found that mutated cancer genes were associated with cellular response to most currently available cancer drugs. Classic oncogene addiction paradigms were modified by additional tissue-specific or expression biomarkers, and some frequently mutated genes were associated with sensitivity to a broad range of therapeutic agents. Unexpected relationships were revealed, including the marked sensitivity of Ewing's sarcoma cells harbouring the EWS (also known as EWSR1)-FLI1 gene translocation to poly(ADP-ribose) polymerase (PARP) inhibitors. By linking drug activity to the functional complexity of cancer genomes, systematic pharmacogenomic profiling in cancer cell lines provides a powerful biomarker discovery platform to guide rational cancer therapeutic strategies.

OBJECTIVES: Pediatric resuscitation is an intense, stressful, and challenging process. The aim of this study was to review the life-threatening pediatric (LTP) emergencies admitted in a Swiss university hospital with regards to patients' demographics, reason for admission, diagnosis, treatment, significant events, critical incidents, and outcomes. METHODS: A retrospective observational cohort study of prospectively collected data was conducted, including all LTP emergencies admitted over a period of 2 years in the resuscitation room (RR). Variables, including indication for transfer, mode of prehospital transportation, diagnosis, and time spent in RR, were recorded. RESULTS: Of the 60,939 pediatric emergencies treated in our university hospital over 2 years, a total of 277 LTP emergencies (0.46%) were admitted in the RR. They included 160 boys and 117 girls, aged 6 days to 15.95 years (mean, 6.69 years; median, 5.06). A medical problem was identified in 55.9% (n = 155) of the children. Of the 122 children treated for a surgical problem, 35 (28.3%) went directly from the RR to the operating room. Hemodynamic instability was noted in 19.5% of all LTP emergencies, of which 1.1% benefited from O negative transfusion. Admission to the intensive care unit was necessary for 61.6% of the children transferred from another hospital. The average time spent in the RR was 46 minutes. The overall mortality rate was 7.2%. CONCLUSIONS: The LTP emergencies accounted for a small proportion of all pediatric emergencies. They were more medical than surgical cases and resuscitation measures because of hemodynamic instability were the most frequent treatment.
Augmentation cystoplasty using pedicled and de-epithelialized gastric patches in the mini-pig model.

PURPOSE: The most common methods of bladder augmentation are gastrocystoplasty and enterocystoplasty. Gastrocystoplasty is advantageous due to minimal mucous secretion and a well developed muscular wall as well as good urodynamic properties of the patch. However, the permanent contact of urine with the gastric mucosa is not free of complications. We report the urodynamic, macroscopic and histological outcomes of a pedicled de-epithelialized gastric patch incorporated in the bladder. We compared the results to those of our previous study, which sought to analyze these techniques of patch coverage using sigmoid patches. MATERIALS AND METHODS: We performed 20 augmentation cystoplasties in the mini-pig model using a pedicled de-epithelialized gastric patch and 5 techniques of patch coverage. RESULTS: Three months after surgery all bladders had an increase in volume except those in which the auto-augmentation technique was used. However, all gastric patches were smaller compared to preoperative size. Many had irregular fibrosed inner surfaces and histological evaluation revealed a fibrosed newly formed submucosal layer with a complete urothelial coverage in every patch. No gastric mucosal remnant was found. CONCLUSIONS: De-epithelialized gastrocystoplasty is an attractive procedure that can increase bladder capacity as well as provide a complete urothelial lining without mucosal remnants. However, the success of this procedure seems to be limited by increased morbidity and fibrotic changes, and decreased surface of the patch.

BACKGROUND: The writing of prescriptions is an important aspect of medical practice. This activity presents some specific problems given a danger of misinterpretation and dispensing errors in community pharmacies. The objective of this study was to determine the evolution of the prescription practice and writing quality in the outpatient clinics of our paediatric university hospital.¦METHODS: Copies of prescriptions written by physicians were collected from community pharmacies in the region of our hospital for a two-month period in 2005 and 2010. They were analysed according to standard criteria, including both formal and pharmaceutical aspects.¦RESULTS: A total of 597 handwritten prescriptions were reviewed in 2005 and 633 in 2010. They contained 1,456 drug prescriptions in 2005 and 1,348 in 2010. Fifteen drugs accounted for 80% of all prescriptions and the most common drugs were paracetamol and ibuprofen. A higher proportion of drugs were prescribed as International Nonproprietary Names (INN) or generics in 2010 (24.7%) compared with 2005 (20.9%). Of the drug prescriptions examined, 55.5% were incomplete in 2005 and 69.2% in 2010. Moreover in 2005, 3.2% were legible only with difficulty, 22.9% were ambiguous, and 3.0% contained an error. These proportions rose respectively to 5.2%, 27.8%, and 6.8% in 2010.¦CONCLUSION: This study showed that fifteen different drugs represented the majority of prescriptions, and a quarter of them were prescribed as INN or generics in 2010; and that handwritten prescriptions contained numerous omissions and preventable errors. In our hospital computerised prescribing coupled with advanced decision support is eagerly awaited.

Introduction : Les fractures du membre inférieur (MI) de l'enfant traitées par immobilisation plâtrée engendrent une modification significative de la mobilité exacerbée en cas d'obésité. L'accéléromètre est un outil d'évaluation du degré d'activité physique (AP) de l'enfant scientifiquement validé. Il n'a jamais fait l'objet d'étude chez un enfant ayant souffert d'une fracture du MI. Le but de ce travail était d'identifier les problèmes dans l'utilisation d'un accéléromètre comme moyen de mesure de l'AP après fracture nécessitant une décharge du MI. Une adaptation de la réhabilitation post-traumatique en fonction du BMI pourrait alors être proposée. Méthode : Identification d'enfants âgés de 8 et 15 ans, victimes d'une fracture du membre inférieur, consultant aux urgences de l'Hôpital de l'Enfance d'octobre 2013 à mai 2014 et nécessitant une décharge post-traumatique. Etaient exclus les enfants polytraumatisés ou souffrants d'un déficit mental. Données pré-requises des patients: âge, poids, taille, sexe, mécanisme de l'accident, type de fracture et traitement. Proposition de port d'un Actiwatch® Spectrum au poignet et cheville pour la période de remobilisation en décharge. Identification des avantages et problèmes liés à l'usage de l'appareil durant les premiers 30 jours de la période de réhabilitation. Importance : L'absence totale d'étude sur la mobilité post-fracture, la complexité des problèmes liés à la marche en décharge, les contraintes de l'immobilisation plâtrée et la prévalence grandissante de l'obésité pédiatrique justifient la recherche d'un moyen fiable pour quantifier la mobilité d'un enfant en décharge après traumatisme du MI. Résultats : Sur 43 fractures du MI traitées à l'HEL durant la période de l'étude, 13 enfants identifiés, dont 1 exclu pour maladie psychiatrique, 1 refus de participation, 2 transferts immédiats, 2 non inclus pour causes pratiques. Sept garçons âgés de 11 à 16 ans ont accepté le port de l'Actiwatch® pour une durée variant entre 7 et 27 jours (moyenne 15). Nombre d'activités (NA) médians de 5 enfants: 171,79 ±105,37 [cpm]* à J1 et 219,48 ±145,52 [cpm] à J5. NA totales médianes sur 24h : 114'072±44'791 [cpm] à J1 et 234'452 ±134'775 [cpm] à J5. Une dynamique de regain de mobilité est mise en évidence avec intensités maximales et minimales du nombre d'activités pour chacun. La médiane du temps de sommeil des 5 enfants était de 716± 45,5 [mn]. Les problèmes rencontrés ont été d'ordre mécanique (Un Actiwatch® fut défectueux), d'ordre pratique (un perdu et rendu tardivement, un port intermittent, une réaction allergique au bracelet à 4j de port). Conclusions La compliance à l'utilisation de l'Actiwatch® sur toute la durée de la décharge n'était pas optimale. La mobilité moyenne des enfants était objectivable de par leur dynamique, leur intensité maximale et minimale et comparables vis-à-vis de certaines études. Une différence avec les sujets en surpoids est observable. La durée de sommeil de chaque enfant suggère que l'antalgie administrée en cours de traitement est suffisante. Utiliser ce capteur de manière prolongée et sur un grand collectif d'enfants serait un moyen fiable et simple d'objectiver la dynamique de reprise de l'activité physique chez ces patients. Conditional heteroskedasticity is an important feature of many macroeconomic and financial time series. Standard residual-based bootstrap procedures for dynamic regression models treat the regression error as i.i.d. These procedures are invalid in the presence of conditional heteroskedasticity. We establish the asymptotic validity of three easy-to-implement alternative bootstrap proposals for stationary autoregressive processes with m.d.s. errors subject to possible conditional heteroskedasticity of unknown form. These proposals are the fixed-design wild bootstrap, the recursive-design wild bootstrap and the pairwise bootstrap. In a simulation study all three procedures tend to be more accurate in small samples than the conventional large-sample approximation based on robust standard errors. In contrast, standard residual-based bootstrap methods for models with i.i.d. errors may be very inaccurate if the i.i.d. assumption is violated. We conclude that in many empirical applications the proposed robust bootstrap procedures should routinely replace conventional bootstrap procedures for autoregressions based on the i.i.d. error assumption.
