Objetivo: analizar cómo influye la inequidad y la posición socio económica como factores determinantes de la conceptualización de la actividad física, desde la concepción de los determinantes sociales, de forma tal que su revisión conceptual, ayude a constituir la re configuración del concepto de actividad física. Métodos: Exploración de la literatura científica desde la revisión conceptual de artículos seleccionados, utilizando las categorías inequidad y posición socioeconómica para establecer la influencia del concepto de la actividad física en entornos educativos. Y a partir de ella interpretar la re configuración del concepto de la actividad física, desde diferentes concepciones asociadas a los determinantes sociales. Resultados: Se hallaron concepciones de los diferentes autores respecto al concepto de la actividad física. (Peso, raza, índice de masa corporal, edad). Se interpretaron distintos niveles de influencia desde la inequidad y la posición socio económica asociados a la ocupación, el género, sitio de vivienda, nivel de estudio, y la estructura física del entorno. Posteriormente se reportaron resultados desde un análisis explicativo en el cual se interpretan las concepciones de los autores en cuanto al concepto de la actividad física, y como desde ella se establecen relaciones entre la inequidad, la posición socioeconómica y los entornos educativos que incluyen diferentes aspectos sociales para finalmente poder establecer un concepto claro de la re configuración social del concepto de la actividad física, concluyendo que este nuevo concepto social de la actividad física no posee una definición exacta, el análisis de los aspectos sociales de ésta, involucra innumerables concepciones que la influencian y la determinan, en consecuencia el concepto de la actividad física debe ser un concepto en constante evolución y reconfiguración debido a la naturaleza cambiante del ser humano, de las dinámicas sociales y las asociaciones de la salud con los aspectos sociales que se presentan en el diario vivir. Así mismo se evidencia que el concepto de la actividad física se fundamenta en estudios que dejan de lado los aspectos sociales que influencia el concepto y en consecuencia la práctica de la actividad física, esta investigación establece un punto de partida frente a la re configuración social del concepto de la actividad física. Conclusiones: Diferentes organizaciones en el mundo reconocen a la actividad física como una herramienta que favorece la socialización de las personas. Es así como la actividad física, desde el deporte en el campo competitivo y la educación física desde el campo educativo exploran la interacción social de los individuos, esto permite la construcción de valores y fortalece la sana convivencia dentro de las comunidades. Esta trasformación desde el concepto de la actividad física biomédica hasta la actividad física social se evidencia en los documentos revisados, en los cuales la actividad física es estudiada desde las categorías de inequidad y posición socio económica, De ahí que en su conceptualización se involucren diferentes conceptos asociados a los determinantes sociales de la salud, que en definitiva establecen un nuevo concepto de la re configuración social de la actividad física trascendiendo desde la actividad física biomédica o fisiológica hasta un concepto complejo que incluye intereses y particularidades de las personas como lo son la ocupación, la educación, el entorno, el medio ambiente y la estructura física.

A depressão é uma doença, é necessário reconhece-la como tal e procurar capacitar as pessoas que com ela convivem a procurar ajuda e precocemente iniciar o seu tratamento. Dos indivíduos que receberam tratamento para um episódio depressivo apresentaram recaídas, muitas vezes devido à não adesão ao tratamento. Nesse contexto, o tratamento medicamentoso combinado com intervenções psicossociais torna-se cada vez mais utilizado, sendo a Psicoeducação uma das intervenções mais eficazes. O desenvolvimento de um programa Psicoeducativo nesse sentido teve como base e metodologia o processo de relação de ajuda. Pretende-se criar as condições necessárias para que a própria pessoa reconheça a sua patologia, os sintomas que dela decorrem, a importância da adesão terapêutica e a necessidade de suporte por parte da família na pessoa com depressão. No final, procura-se desenvolver e refletir as competências inerentes ao enfermeiro especialista em enfermagem de Saúde Mental e Psiquiatria, no diagnostico, intervenção e avaliação na pessoa com sintomatologia depressiva; Abstract: Therapeutic approach to nursing depressive patient – the use of psychoeducation in the treatment of depressive symptomatology Depression is a disease, it is necessary to recognize it as such and seek to enable people to live with her to seek help early and start your treatment. Of individuals who received treatment for a depressive episode had relapses, often due to non-adherence to treatment. In this context, drug treatment combined with psychosocial interventions becomes increasingly used, the Psychoeducational one of the most effective interventions. The development of a program accordingly Psychoeducational was based on the methodology and process aid relationship. It is intended to create the necessary conditions so that the person recognize their disease, the symptoms resulting from it, the importance of adherence and the need for support by the family in person with depression. In the end, we seek to develop and reflect the skills inherent to the nurse specialist in nursing Mental Health and Psychiatry in the diagnosis, intervention and evaluation in people with depressive symptoms.

Já na antiguidade o Homem se debruçava sobre a temática do envelhecimento, todavia os anos passaram e este fenómeno constitui-se como um dos assuntos da ordem do dia que nos abrange a todos, porque todos queremos vivenciar o prazer de envelhecer, à semelhança dos nossos antepassados. Porém estes procuravam o elixir de uma vida eterna e nós ambicionamos o elixir de uma longa vida com qualidade. Objectivo: Avaliar a qualidade de vida e aptidão física funcional dos idosos residentes do Concelho de Évora em instituições da rede solidária e nos seus domicílios com idade igual ou superior a 75 anos. Métodos: Após a selecção aleatória e tendo por base os critérios de exclusão (sem défice cognitivo avaliado pelo MMS e sem problemas físicos que impedissem de realizar os testes funcionais) aceitaram participar no estudo 396 idosos do Concelho de Évora dos quais 196 se encontram institucionalizados nos lares da rede solidária e 200 residem nos seus domicílios. Os instrumentos de avaliação utilizados foram o EASYcare e a bateria de testes Rikly Jones- Senior Fitness Test. Conclusões: Os idosos não institucionalizados apresentam melhor qualidade de vida e aptidão física funcional face aos idosos institucionalizados. Os níveis de incapacidade do grupo de idosos são baixos, o que revela que estamos perante um grupo maioritariamente independente. Por sua vez os homens apresentam melhor auto­ percepção da sua qualidade de vida e melhor desempenho nos testes físicos realizados. Com o processo de envelhecimento é notório uma avaliação inferior nas diferentes áreas da qualidade de vida por parte dos idosos assim como um pior desempenho nos testes físicos. Verificou-se também que existem correlações significativas entre as áreas da qualidade de vida avaliadas e os parâmetros de aptidão física funcional considerados, estando estes interrelacionados. ABSTRACT: Introduction: Already in the antiquity Man was dealing with the aging topic, however the years passed and this issue is as one of the topics on the agenda that covers everyone, because we all want to experience the pleasure of growing old, like of our ancestors. Although they sought the elixir of eternal life and we are aiming the elixir of long life with quality. Aim: To evaluate the quality of life and functional fitness of elderly aged over 75 years, residents of the Municipality of Évora in the network institutions in their households. Methods: After random selection and based on the exclusion criteria (no cognitive impairment assessed by MMS and no conditions that hamper them from performing functional tests) agreed to participate in the study 396 elderly from Évora's Municipality of which 196 are institutionalized in nursing homes network of solidarity and 200 reside in their homes. The assessment instruments used were the EASYcare and battery of tests Rikly Jones- Senior Fitness Test. Conclusions: The non-institutionalized elderly have a better quality of life and functional fitness in relation to the institutionalized elderly. The levels of disability in the elderly group are low, indicating that this is a group mostly independent. ln tum the men have better self-perception of their quality of life and better performance in physical tests performed. With the aging process is an assessment of less known in different areas of life quality for elderly people as well as a poorer performance in physical tests. There was also that there are significant correlations between the areas of quality of life and evaluated the parameters of functional fitness considered and these interrelated.

São conhecidos os benefícios da aplicabilidade da Classificação Internacional para a Prática de Enfermagem, nos cuidados de enfermagem. Tivemos como objetivo, otimizar o SClinico na consulta de saúde materna da Maternidade Dr. Francisco Feitinha da Unidade Local de Saúde do Norte Alentejano, EPE. Para diagnóstico da situação foi aplicada uma entrevista que permitiu identificar as necessidades da população. Foram propostas intervenções de diagnóstico frequentes, necessárias para promover a melhoria dos cuidados prestados e a qualidade dos mesmos. Para avaliação da intervenção aplicamos um questionário. Constatou-se que a maioria dos enfermeiros considera os registos eletrónicos de enfermagem importantes e de muita utilidade, para a atividade profissional e constituem uma fonte de informação essencial que permite a comunicação entre os profissionais de saúde garantindo continuidade e qualidade dos cuidados de enfermagem; ABSTRACT: The benefits of the application of the International Classification for Nursing Practice in nursing care are known. Our purpose was to optimize the Clinical System and Nursing health consultation Maternity Dr. Francisco Feitinha Unit of the North Alentejo Health, EPE. For the diagnosis of the situation, it was applied an interview to identify the needs of the population. It was proposed more frequent diagnosis of interventions necessary to promote the improvement of care and its own quality. For evaluation of the intervention, we applied a questionnaire. Thus, we concluded that most nurses consider important electronic nursing records and that they are very useful for the professional activity. They are seen as an essential source of information and enable communication between health professionals ensuring continuity of quality care.

OBJECTIVE The aim of this research project was to obtain an understanding of the barriers to and facilitators of providing palliative care in neonatal nursing. This article reports the first phase of this research: to develop and administer an instrument to measure the attitudes of neonatal nurses to palliative care. METHODS The instrument developed for this research (the Neonatal Palliative Care Attitude Scale) underwent face and content validity testing with an expert panel and was pilot tested to establish temporal stability. It was then administered to a population sample of 1285 neonatal nurses in Australian NICUs, with a response rate of 50% (N 645). Exploratory factor-analysis techniques were conducted to identify scales and subscales of the instrument. RESULTS Data-reduction techniques using principal components analysis were used. Using the criteria of eigenvalues being 1, the items in the Neonatal Palliative Care Attitude Scale extracted 6 factors, which accounted for 48.1% of the variance among the items. By further examining the questions within each factor and the Cronbach’s of items loading on each factor, factors were accepted or rejected. This resulted in acceptance of 3 factors indicating the barriers to and facilitators of palliative care practice. The constructs represented by these factors indicated barriers to and facilitators of palliative care practice relating to (1) the organization in which the nurse practices, (2) the available resources to support a palliative model of care, and (3) the technological imperatives and parental demands. CONCLUSIONS The subscales identified by this analysis identified items that measured both barriers to and facilitators of palliative care practice in neonatal nursing. While establishing preliminary reliability of the instrument by using exploratory factor-analysis techniques, further testing of this instrument with different samples of neonatal nurses is necessary using a confirmatory factor-analysis approach.

We came from professions that themselves are poles apart, and many of the challenges we faced entering the profession were the same: juggling full-time work, part time study, and family commitmemts, taking a pay cut, and loss of social life. But over a short period of time we both transitioned to our new profession successfully. so what make our successful transition possible?

Introduction The purpose of this study was to develop, implement and evaluate the impact of an educational intervention, comprising an innovative model of clinical decisionmaking and educational delivery strategy for facilitating nursing students‘ learning and development of competence in paediatric physical assessment practices. Background of the study Nursing students have an undergraduate education that aims to produce graduates of a generalist nature who demonstrate entry level competence for providing nursing care in a variety of health settings. Consistent with population morbidity and health care roles, paediatric nursing concepts typically form a comparatively small part of undergraduate curricula and students‘ exposure to paediatric physical assessment concepts and principles are brief. However, the nursing shortage has changed traditional nursing employment patterns and new graduates form the majority of the recruitment pool for paediatric nursing speciality staff. Paediatric nursing is a popular career choice for graduates and anecdotal evidence suggests that nursing students who select a clinical placement in their final year intend to seek employment in paediatrics upon graduation. Although concepts of paediatric nursing are included within undergraduate curriculum, students‘ ability to develop the required habits of mind to practice in what is still regarded as a speciality area of practice is somewhat limited. One of the areas of practice where this particularly impacts is in paediatric nursing physical assessment. Physical assessment is a fundamental component of nursing practice and competence in this area of practice is central to nursing students‘ development of clinical capability for practice as a registered nurse. Timely recognition of physiologic deterioration of patients is a key outcome of nurses‘ competent use of physical assessment strategies, regardless of the practice context. In paediatric nursing contexts children‘s physical assessment practices must specifically accommodate the child‘s different physiological composition, function and pattern of clinical deterioration (Hockenberry & Barrera, 2007). Thus, to effectively manage physical assessment of patients within the paediatric practice setting nursing students need to integrate paediatric nursing theory into their practice. This requires significant information processing and it is in this process where students are frequently challenged. The provision of rules or models can guide practice and assist novice-level nurses to develop their capabilities (Benner, 1984; Benner, Hooper-Kyriakidis & Stannard, 1999). Nursing practice models are cognitive tools that represent simplified patterns of expert analysis employing concepts that suit the limited reasoning of the inexperienced, and can represent the =rules‘ referred to by Benner (1984). Without a practice model of physical assessment students are likely to be uncertain about how to proceed with data collection, the interpretation of paediatric clinical findings and the appraisal of findings. These circumstances can result in ad hoc and unreliable nursing physical assessment that forms a poor basis for nursing decisions. The educational intervention developed as part of this study sought to resolve this problem and support nursing students‘ development of competence in paediatric physical assessment. Methods This study utilised the Context Input Process Product (CIPP) Model by Stufflebeam (2004) as the theoretical framework that underpinned the research design and evaluation methodology. Each of the four elements in the CIPP model were utilised to guide discrete stages of this study. The Context element informed design of the clinical decision-making process, the Paediatric Nursing Physical Assessment model. The Input element was utilised in appraising relevant literature, identifying an appropriate instructional methodology to facilitate learning and educational intervention delivery to undergraduate nursing students, and development of program content (the CD-ROM kit). Study One employed the Process element and used expert panel approaches to review and refine instructional methods, identifying potential barriers to obtaining an effective evaluation outcome. The Product element guided design and implementation of Study Two, which was conducted in two phases. Phase One employed a quasiexperimental between-subjects methodology to evaluate the impact of the educational intervention on nursing students‘ clinical performance and selfappraisal of practices in paediatric physical assessment. Phase Two employed a thematic analysis and explored the experiences and perspectives of a sample subgroup of nursing students who used the PNPA CD-ROM kit as preparation for paediatric clinical placement. Results Results from the Process review in Study One indicated that the prototype CDROM kit containing the PNPA model met the predetermined benchmarks for face validity and the impact evaluation instrumentation had adequate content validity in comparison with predetermined benchmarks. In the first phase of Study Two the educational intervention did not result in statistically significant differences in measures of student performance or self-appraisal of practice. However, in Phase Two qualitative commentary from students, and from the expert panel who reviewed the prototype CD-ROM kit (Study One, Phase One), strongly endorsed the quality of the intervention and its potential for supporting learning. This raises questions regarding transfer of learning and it is likely that, within this study, several factors have influenced students‘ transfer of learning from the educational intervention to the clinical practice environment, where outcomes were measured. Conclusion In summary, the educational intervention employed in this study provides insights into the potential e-learning approaches offer for delivering authentic learning experiences to undergraduate nursing students. Findings in this study raise important questions regarding possible pedagogical influences on learning outcomes, issues within the transfer of theory to practice and factors that may have influenced findings within the context of this study. This study makes a unique contribution to nursing education, specifically with respect to progressing an understanding of the challenges faced in employing instructive methods to impact upon nursing students‘ development of competence. The important contribution transfer of learning processes make to students‘ transition into the professional practice context and to their development of competence within the context of speciality practice is also highlighted. This study contributes to a greater awareness of the complexity of translating theoretical learning at undergraduate level into clinical practice, particularly within speciality contexts.

Management of acute heart failure is an important consideration in critical care. Mechanical support of the failing heart is crucial for improving health outcomes. The most common Australasian application of intraaortic balloon counterpulsation (IABP) is in the setting of cardiogenic shock. High end users of IABP (>37/annum) demonstrate significantly lower mortality for cardiogenic shock managed with IABP (p <0.001) in contrast to hospitals which employ limited IABP (<4/annum). This underscores the importance of proficiency in managing patient receiving IABP support. Nurses play a crucial role in carding for patients with acute heart failure. This paper summarises care considerations for management of the IABP.

Objectives: The Nurse Researcher Project (NRP) was initiated to support development of a nursing research and evidence based practice culture in Cancer Care Services (CCS) in a large tertiary hospital in Australia. The position was established and evaluated to inform future directions in the organisation.---------- Background: The demand for quality cancer care has been expanding over the past decades. Nurses are well placed to make an impact on improving health outcomes of people affected by cancer. At the same time, there is a robust body of literature documenting the barriers to undertaking and utilising research by and for nurses and nursing. A number of strategies have been implemented to address these barriers including a range of staff researcher positions but there is scant attention to evaluating the outcomes of these strategies. The role of nurse researcher has been documented in the literature with the aim to provide support to nurses in the clinical setting. There is, to date, little information in relation to the design, implementation and evaluation of this role.---------- Design: The Donabedian’s model of program evaluation was used to implement and evaluate this initiative.---------- Methods: The ‘NRP’ outlined the steps needed to implement the nurse researcher role in a clinical setting. The steps involved the design of the role, planning for the support system for the role, and evaluation of outcomes of the role over two years.---------- Discussion: This paper proposes an innovative and feasible model to support clinical nursing research which would be relevant to a range of service areas.---------- Conclusion: Nurse researchers are able to play a crucial role in advancing nursing knowledge and facilitating evidence based practice, especially when placed to support a specialised team of nurses at a service level. This role can be implemented through appropriate planning of the position, building a support system and incorporating an evaluation plan.

Background: Children having chemotherapy for cancer are prone to developing influenza infections. Influenza virus infection may lead to hospitalization/prolonged hospitalization, interruption of treatment, and other severe adverse outcomes such as death. Although clinical guidelines recommend children who are being treated for cancer be vaccinated against influenza, evidence supporting this recommendation is unclear.--------- Objectives: The objectives of this review were to (1) assess the efficacy of influenza vaccination in stimulating immunologic response in children with cancer receiving chemotherapy, compared with other control groups; (2) assess the efficacy of influenza vaccination in preventing influenza infection; and (3) establish any adverse effects associated with influenza vaccines in children with cancer.

There has been limited research into the scope or standards of specialist palliative care nursing practice in an Australian context. This study sought to develop a competency framework that described the core domains of specialist palliative care nursing. This article explores one key domain of specialist palliative care nursing practice - therapeutic relationships - that was identified as underpinning other domains of practice. A mixed method was used, involving a literature review, a survey including practice exemplars and an interview of specialist palliative care nurses. Seventy-four registered nurses working in designated specialist palliative care nursing roles from each Australian state and mainland territory were involved. The nurses represented metropolitan, regional, rural and remote communities, various inpatient facilities and community practice settings. Five core domains of specialist palliative care nursing practice were identified: complex supportive care, collaborative practice, leadership, improving practice and therapeutic relationships. Therapeutic relationships were identified as the central domain of specialist palliative care nursing practice to which all other domains were inextricably linked.
