Co-morbidity burden in Parkinson’s disease : Comparison with controls and its influence on prognosis

Cerebrovascular amyloid beta-protein (Abeta) deposition is a pathological feature of several related disorders including Alzheimer disease and hereditary cerebral hemorrhage with amyloidosis Dutch-type (HCHWA-D). HCHWA-D is caused by a point mutation in the gene that encodes the Abeta precursor and results in a Glu --> Gln substitution at position 22 of Abeta. In comparison to Alzheimer disease, the cerebrovascular Abeta deposition in HCHWA-D is generally more severe, often resulting in intracerebral hemorrhage when patients reach 50 years of age. We recently reported that Abeta(1-42), but not the shorter Abeta(1-40) induces pathologic responses in cultured human leptomeningeal smooth muscle cells including cellular degeneration that is accompanied by a marked increase in the levels of cellular Abeta precursor and soluble Abeta peptide. In the present study, we show that the HCHWA-D mutation converts the normally nonpathologic Abeta(1-40) into a highly pathologic form of the peptide for cultured human leptomeningeal smooth muscle cells. These findings suggest that these altered functional properties of HCHWA-D mutated Abeta may contribute to the early and often severe cerebrovascular pathology that is the hallmark of this disorder.

Thrombin receptor activation was explored in human epidermal keratinocytes and human dermal fibroblasts, cells that are actively involved in skin tissue repair. The effects of thrombin, trypsin, and the receptor agonist peptides SFLLRN and TFRIFD were assessed in inositolphospholipid hydrolysis and calcium mobilization studies. Thrombin and SFLLRN stimulated fibroblasts in both assays to a similar extent, whereas TFRIFD was less potent. Trypsin demonstrated weak efficacy in these assays in comparison with thrombin. Results in fibroblasts were consistent with human platelet thrombin receptor activation. Keratinocytes, however, exhibited a distinct profile, with trypsin being a far better activator of inositolphospholipid hydrolysis and calcium mobilization than thrombin. Furthermore, SFLLRN was more efficacious than thrombin, whereas no response was observed with TFRIFD. Since our data indicated that keratinocytes possess a trypsin-sensitive receptor, we addressed the possibility that these cells express the human homologue of the newly described murine protease-activated receptor, PAR-2 [Nystedt, S., Emilsson, K., Wahlestedt, C. & Sundelin, J. (1994) Proc. Natl. Acad. Sci. USA 91, 9208-9212]. PAR-2 is activated by nanomolar concentrations of trypsin and possesses the tethered ligand sequence SLIGRL. SLIGRL was found to be equipotent with SFLLRN in activating keratinocyte inositolphospholipid hydrolysis and calcium mobilization. Desensitization studies indicated that SFLLRN, SLIGRL, and trypsin activate a common receptor, PAR-2. Northern blot analyses detected a transcript of PAR-2 in total RNA from keratinocytes but not fibroblasts. Levels of thrombin receptor message were equivalent in the two cell types. Our results indicate that human keratinocytes possess PAR-2, suggesting a potential role for this receptor in tissue repair and/or skin-related disorders.

Introdução A presença de mulheres no transporte rodoviário de cargas tem sido cada vez mais crescente e as repercussões do trabalho na vida das motoristas de caminhão ainda são desconhecidas pela comunidade científica. Objetivo - Caracterizar e analisar o trabalho de mulheres motoristas de caminhão e suas repercussões sobre sua saúde, a partir do relato de homens e mulheres motoristas de caminhão. Metodologia - O estudo com abordagem qualitativa utilizou a técnica do grupo focal, entrevistas individuais e observação não participante. Os grupos focais foram realizados em uma empresa transportadora localizada no estado de São Paulo e as entrevistas individuais em evento realizado na cidade de Itupeva/SP. Em oito encontros, grupos de motoristas de caminhão, discutiram a temática trabalho e saúde conduzida por meio de questões semiabertas. As mesmas questões foram utilizadas para as entrevistas individuais. Os relatos foram gravados, sendo o conteúdo das gravações transcrito e analisado por meio da metodologia Análise de Conteúdo de Bardin. A partir dos dados obtidos, construíram-se as seguintes categorias: A trajetória profissional de motoristas de caminhão; As mulheres no Transporte Rodoviário de Cargas; O trabalho; Um momento inesquecível na profissão e A saúde das mulheres motoristas na estrada. Resultados Tornar-se motorista de caminhão, para a maioria das mulheres, ocorreu por acaso, ou devido à uma necessidade financeira, ou a falta de perspectiva de emprego. Para as mulheres no Transporte Rodoviário de Cargas, o cotidiano de trabalho das profissionais está atravessado por aspectos como: a força física; dúvidas e preconceitos quanto à sua orientação sexual; o desafio em conciliar a vida dentro e fora do caminhão; conflitos na vida conjugal decorrentes da vida profissional; a discriminação sexual e a necessidade do reconhecimento no trabalho, bem como a falta de infraestrutura dedicada às trabalhadoras nas empresas e postos de parada nas rodovias brasileiras. O momento inesquecível nesta profissão, considerado por homens e mulheres, foi a primeira viagem. Os impactos do trabalho sobre a saúde das trabalhadoras recaíram sobre dores lombares e na coluna; problemas relacionados ao sono; necessidade de recorrer ao uso de drogas como anfetaminas e cocaína para manterem-se acordadas durante o trabalho; estresse; infecção urinária e uso ininterrupto de anticoncepcionais. Conclusões - A inserção das mulheres no transporte rodoviário de cargas desafia empresas e a infraestrutura existente nas rodovias do país a acompanharem as transformações sociais no mundo do trabalho, incluindo as demandas de um novo perfil de trabalhadoras. Visual symptoms are relatively common in Parkinson's disease (PD) and optical coherence tomography has indicated possible retinal thinning. Accumulation of aggregated α-synuclein is thought to be a central pathogenic event in the PD brain but there have not as yet been reports of retinal synucleinopathy. Retinal wholemounts were prepared from subjects with a primary clinicopathological diagnosis of PD (N = 9), dementia with Lewy bodies (DLB; N = 3), Alzheimer's disease (N = 3), progressive supranuclear palsy (N = 2) as well as elderly normal control subjects (N = 4). These were immunohistochemically stained with an antibody against α-synuclein phosphorylated at serine 129, which is a specific molecular marker of synucleinopathy. Phosphorylated α-synuclein-immunoreactive (p-syn IR) nerve fibers were present in 7/9 PD subjects and in 1/3 DLB subjects; these were sparsely distributed and superficially located near or at the inner retinal surface. The fibers were either long and straight or branching, often with multiple en-passant varicosities along their length. The straight fibers most often had an orientation that was radial with respect to the optic disk. Together, these features are suggestive of either retinopetal/centrifugal fibers or of ganglion cell axons. In one PD subject there were sparse p-syn IR neuronal cell bodies with dendritic morphology suggestive of G19 retinal ganglion cells or intrinsically photosensitive ganglion cells. There were no stained nerve fibers or other specific staining in any of the non-PD or non-DLB subjects. It is possible that at least some of the observed visual function impairments in PD subjects might be due to α-synucleinopathy.

Ce mémoire présente les résultats d’une synthèse systématique (SS) des écrits traitant des instruments d’évaluation multidimensionnelle des troubles concomitants qui peuvent être présentés par les adolescent(e)s. La SS a permis d’identifier 11 instruments en mesure d’évaluer les troubles comorbides de l’Axe I du DSM-IV, incluant chaque fois les troubles liés à l’utilisation de substances psychoactives (TUS). Une fois les instruments répertoriés, une seconde recherche fut effectuée afin identifier les études les ayant mis à l’épreuve du point de vue de leur validité et de leur fidélité diagnostique : 57 études furent identifiées. La robustesse méthodologique de ces études fut analysée à l’aide de la grille du QUADAS-2 et 47 études furent retenues pour l’échantillon final. Les résultats sont présentés par diagnostics (troubles liés à l’utilisation des substances (TUS) (obligatoire), trouble d’anxiété généralisée (TAG), épisode dépressif majeur (ÉDM), troubles des conduites (TC), trouble du déficit de l’attention /hyperactivité (TDA/H), état de stress post-traumatique (ÉSPT) et par instrument retenu. Suite à l’analyse des données recueillies, il s’avère difficile de comparer les instruments les uns aux autres, étant donnée la très grande diversité des échelles qu’ils contiennent, ainsi que les devis fort différents des études qui les ont mis à l’épreuve. Par contre, deux instruments se distinguent par la robustesse méthodologique des études à leur sujet, ainsi que leur excellente performance globale. Il s’agit du ChIPS et du K-SADS.
