Background: The prevalence of Diabetes mellitus (DM) is on a rise in sub-Saharan Africa and will more than double by 2025. Cardiovascular disease (CVD) accounts for up to 2/3 of all deaths in the diabetic population. Of all the CVD deaths in DM, 3/4 occur in sub Saharan Africa (SSA). Non invasive identification of cardiac abnormalities, such as Left Ventricular Hypertrophy (LVH), diastolic and systolic dysfunction, is not part of diabetes complications surveillance programs in Uganda and there is limited data on this problem. This study sought to determine the prevalence, types and factors associated with echocardiographic abnormalities among newly diagnosed diabetic patients at Mulago National referral hospital in Uganda. Methods: In this cross sectional study conducted between June 2014 and December 2014, we recruited 202 newly diagnosed adult diabetic patients. Information on patients\' socio-demographics, bio-physical profile, biochemical testing and echocardiographic findings was obtained for all the participants using a pre-tested questionnaire. An abnormal echocardiogram in this study was defined as the presence of LVH, diastolic and/or systolic dysfunction and wall motion abnormality. Bivariate and multivariate logistic regression analyses were used to investigate the association of several parameters with echocardiographic abnormalities. Results: Of the 202 patients recruited, males were 102(50.5%) and the mean age was 46±15 years. Majority of patients had type 2 DM, 156(77.2%) and type 1 DM, 41(20.3%) with mean HbA1C of 13.9±5.3%. Mean duration of diabetes was 2 months. The prevalence of an abnormal echocardiogram was 67.8 % (95% CI 60%-74%). Diastolic dysfunction, systolic dysfunction, LVH and wall motion abnormalities were present in 55.0%, 21.8%, 19.3% and 4.0% of all the participants respectively. In bivariate logistic regression analysis, the factors associated with an abnormal echocardiogram were age (OR 1.09 [95% CI 1.06–1.12], P <0.0001), type 2 DM (OR 5.8[95% CI 2.77-12.07], P<0.0001), hypertension (OR 2.64[95% CI 1.44-4.85], P=0.002), obesity (OR 3.51[955 CI 1.25-9.84], P=0.017 and increased waist circumference (OR 1.02[95% CI 1.00-1.04], P=0.024. On Multiple logistic regression analysis, age was the only factor associated with an abnormal echocardiogram (OR 1.09[95%CI 1.05-1.15], P<0.0001). Conclusion: Echocardiographic abnormalities were common among newly diagnosed adults with DM. Traditional CVD risk factors were associated with an abnormal echocardiogram in this patient population. Due to a high prevalence of echocardiographic abnormalities among newly diagnosed diabetics, we recommend screening for cardiac disease especially in patients who present with traditional CVD risk factors. This will facilitate early diagnosis, management and hence better patient outcomes.

Background: Adherence to controller therapy in asthma is a major concern during the management of the disease. Objective: To determine the adherence rate and identify the predictors of low adherence to asthma controller therapy. Methods: A cross-sectional study including asthma patients was conducted from November 1, 2012 to May 31, 2013 in 4 chest clinics in Cameroon. The adherence to asthma treatment was rated using Morisky Medication Adherence Scale. A multivariate logistic regression analysis was performed for the identification of factors associated with adherence to asthma treatment. Results: Among the 201 asthma patients included, 133 (66.2%) were female. The mean age of participants was 41.2 years. Sixty-one (30.3%) of the patients did not visit the chest physician during the last year prior to the study. Asthma was well controlled in 118 patients (58.7%). The prevalence of low adherence rate to asthma controller therapy was 44.8% and the absence of any chest specialist visit within the last 12 months was the only factor associated with the low adherence rate to asthma treatment (OR 5.57 ; 95% CI 2.84–10.93). Conclusion: The adherence rate to asthma controller therapy in Cameroon is low and it could be improved if scheduled visits are respected by patients.

Background: Recently eye effects of Diabetes Mellitus (DM) are an important concern due to increase in its trend especially in developing countries. Objectives: To assess the awareness related to eye effects of DM and its prevention practices among people with diabetes. Methods: This cross sectional study was conducted from January 2013 to April 2013 in Villupuram district of Tamil Nadu, India. All 105 people with diabetes from the service area of two sub-centres were included. Data on socio demographic details, history of DM, awareness on systemic complications of DM, effects of DM on eyes, practice on regular blood check-up, eye examination and source of information were collected by interview technique using a structured questionnaire. Univariate and multiple logistic regression analysis were done to assess the association of awareness of eye examination with socio-demographic variables. Results: Mean age of the study population was 56.7 years. About 93 people with diabetes (88.6%) tested their blood sugar at least once in every 3 months. About 80 people with diabetes (76.2%) were aware of at least one systemic complication of DM. Although 78 (74.3%) people with diabetes were aware that DM could affect the eyes, majority of this group (68, 87.2%) did not know the specific effects of DM on eyes. In this group, about 28(35.9%) people with diabetes were not aware of the reasons for eye effects, while others mentioned that persistent high blood sugar level (n=26, 33.3%), longer duration of DM (n=14, 17.9%) and lifestyle (n=10, 12.8%) were the reasons for the eye effects of DM. Only 31 (29.5%) of them knew that their eyes must be regularly examined. People with diabetes who had post-secondary and above (>10th standard) level of education had significantly higher awareness on examination of eye (Adjusted OR=19.63). Conclusion: Although awareness of people with diabetes on systemic effects of DM was more, their awareness on specific eye effects and need for regular screening was low. Systematic efforts are required to increase awareness on eye effects and importance of regular screening in this population.

Background: Prevalence of H. pylori infection varies greatly between populations in different countries. This study was conducted to determine the magnitude of H.pylori among adult patients with dyspepsia attending the gastroenterology unit at Bugando medical centre. Methods: A cross sectional study involving 202 dyspeptic patients was conducted between June and July 2014. A Standardized data collection tool was used to collect socio-demographic characteristics. H.pylori antibodies were detected using rapid immunochromatographic tests according to manufacturer’s instructions. Results: The median age of study population was 42 (IQR: 33-54). Females 105 (51.9%) formed majority of the population studied. Of 202 participants; 119 (58.9%) were from rural areas. Seroprevalence of H.pylori infection was found to be 79/202 (39.1%, 95% CI: 32.3 -45.7). As the age increased the risk of having H.pylori infection also increased (OR: 1.02 95% CI: 1-1.04, P=0.02). On multivariate logistic regression analysis untreated drinking water was found to predict H.pylori seropositivity (OR: 2.33, CI: 1.09-4.96, p=0.028). Conclusion: The seroprevalence of H.pylori among dyspeptic patients is high in this setting. Therefore the community in Mwanza should be educated on the use of safe drinking water in order to minimize H. pylori infections.

Nowadays entrepreneurship is one of the main key objects in internal policy of each country. More and more women start doing their own business and thus become integral participants of entrepreneurial activities. However, despite of the abundance of various scientific publications, female entrepreneurship is poorly understood phenomenon, which is needed to be carefully scrutinized. The general purpose of this work is to describe and analyse such phenomenon as female entrepreneurship generally in the world and separately and mainly in Belarus. Indeed, it intends to determine the factors that drive women's entrepreneurship in Belarus. The findings are supported by literature, gathered from different scientific researches and actual statistical data. The data used in the empirical part was collected from World Bank Enterprise Surveys and comprises the responses of representatives of 360 companies selected randomly from the population of the Belarus companies. With the help of descriptive statistics and the application of logistic regression simple models to determine which economic, social, fiscal and legal environmental factors impact on female entrepreneurial activity was possible to understand the female involvement in business activities and society of the country.

A prepayment scheme for health through the National Health Insurance Scheme (NHIS) was commenced in Nigeria about ten years ago. Nigeria operates a federal system of government. Sub- national levels possess a high degree of autonomy in a number of sectors including health. It is important to assess the level of coverage of the scheme among the formal sector workers in Nigeria as a proxy to gauge the extent of coverage of the scheme and derive suitable lessons that could be used in its expansion. This is a cross-sectional, descriptive survey carried out among formal sector workers in Ilorin Kwara State, Nigeria. A stratified sampling technique was used to select study participants. A self-administered questionnaire was used to collect data from respondents. Data was analysed with the SPSS. Ethical approval to conduct the study was obtained from the Bowen University Teaching Hospital Research Ethics Committee. A total of 370 people participated in the study. Majority, (78.9%) of the respondents were aware of the NHIS, however only 13.5 % paid for health care services through the NHIS. Logistic regression analysis shows that respondents with post-secondary education (OR = 9.032, CI = 2.562 – 31.847, p = 0.001) and in federal civil service (OR = 2.679, CI = 1.036 – 6.929, p = 0.042) were over nine and three times more likely to be aware of the scheme than others. Coverage of the scheme among the respondents was unimpressive. A lot still need to be done to fast-track the expansion of the scheme among this sector of the population.

BACKGROUND: Diabetes mellitus (DM) increases tuberculosis risk while tuberculosis, as an infectious disease, leads to hyperglycemia. We compared hyperglycemia screening strategies in controls and patients with tuberculosis in Dar es Salaam, Tanzania. METHODS: Consecutive adults with tuberculosis and sex- and age-matched volunteers were included in a case-control study between July 2012 and June 2014. All underwent DM screening tests (fasting capillary glucose [FCG] level, 2-hour CG [2-hCG] level, and glycated hemoglobin A1c [HbA1c] level) at enrollment, and cases were tested again after receipt of tuberculosis treatment. Association of tuberculosis and its outcome with hyperglycemia was assessed using logistic regression analysis adjusted for sex, age, body mass index, human immunodeficiency virus infection status, and socioeconomic status. Patients with tuberculosis and newly diagnosed DM were not treated for hyperglycemia. RESULTS: At enrollment, DM prevalence was significantly higher among patients with tuberculosis (n = 539; FCG level > 7 mmol/L, 4.5% of patients, 2-hCG level > 11 mmol/L, 6.8%; and HbA1c level > 6.5%, 9.3%), compared with controls (n = 496; 1.2%, 3.1%, and 2.2%, respectively). The association between hyperglycemia and tuberculosis disappeared after tuberculosis treatment (adjusted odds ratio [aOR] for the FCG level: 9.6 [95% confidence interval {CI}, 3.7-24.7] at enrollment vs 2.4 [95% CI, .7-8.7] at follow-up; aOR for the 2-hCG level: 6.6 [95% CI, 4.0-11.1] vs 1.6 [95% CI, .8-2.9]; and aOR for the HbA1c level, 4.2 [95% CI, 2.9-6.0] vs 1.4 [95% CI, .9-2.0]). Hyperglycemia, based on the FCG level, at enrollment was associated with tuberculosis treatment failure or death (aOR, 3.3; 95% CI, 1.2-9.3). CONCLUSIONS: Transient hyperglycemia is frequent during tuberculosis, and DM needs confirmation after tuberculosis treatment. Performance of DM screening at tuberculosis diagnosis gives the opportunity to detect patients at risk of adverse outcome.

The aim of this study is to shed light on what makes women decide whether or not to continue with legal proceedings for intimate partner violence once they have commenced. Legal professionals, members of the police force, and women in Spain were interviewed to help draft a questionnaire that was applied to a sample of 345 women who had undertaken legal proceedings against their (ex)partners. Socio-demographic, emotional, and psychological variables were considered as possible predictor variables and included in a logistic regression analysis. Results show that the best equation for predicting disengagement from legal procedures includes the level of support received by the victim, contact with the aggressor, thoughts about going back with the aggressor, and a feeling of guilt. The essential role of the psychological support during the legal process is emphasized in conclusions.

It is important to identify groups of people vulnerable to a disease condition. Aim: To determine the association between social vulnerability to caries and caries status of children in Ile-Ife, Nigeria. Methods: A composite vulnerability index for caries was developed using data generated for 992 children. Wilks’ Lambda test to verify relationship between vulnerability and its variables. Logistic regression analysis was conducted to determine if the social vulnerability for caries index was a good predictor for caries status. Results: The social vulnerability to caries index could not predict caries status. The study found that sex, age and number of siblings were the significant predictors of caries status in the study population. Females (AOR: 1.63; 95%CI: 1.08 – 2.46; p=0.02) and children with more than two siblings had higher odds of having caries (AOR: 2.61; 95%CI: 1.61 – 4.24; p<0.001) while children below 5 years had lower odds of having caries (AOR: 0.62; 95%CI: 0.39 – 1.00; p=0.05) Conclusions: The social vulnerability index for caries could not predict the caries status of children in the study population. Sensitive tools to identify children with caries in the study population should be developed.

O objetivo deste estudo foi analisar associações entre a qualidade do sono, a atividade física e o funcionamento físico (aptidão física e atividades da vida diária (AVD)) de pessoas idosas independentes. Metodologia: A amostra incluiu 437 pessoas idosas independentes (143 homens e 294 mulheres; 65-103 anos). A qualidade do sono e as AVD foram avaliadas através de questionário, a atividade física através de acelerometria e a aptidão física através do Senior Fitness Test. Resultados: A análise da regressão logística tendo como variável dependente a qualidade do sono e como variáveis independentes a atividade física, a aptidão física e as AVD, revelou que as AVD foi a única variável explicativa da discriminação entre má e boa qualidade do sono. O aumento de um ponto nas AVD correspondeu a uma diminuição de 91,4% na probabilidade de ter uma má qualidade de sono. Os resultados não foram alterados quando se incluiu no modelo o género, a idade ou o escalão etário. Conclusões: Um melhor funcionamento físico parece estar associado a uma melhor qualidade do sono em pessoas idosas. A obtenção de 19 pontos nas AVD revelou ser discriminatória da qualidade do sono de pessoas idosas.

Background: Vaspin is a newly-identified adipocytokine associated with insulin resistance (IR). Objective: The aim of this study was to investigate the correlation between plasma vaspin concentrations and IR and determine whether this association is affected by body composition, physical activity and pubertal stage in adolescents. Methods: Were studied 484 Brazilian adolescents aged 10-14 years whose anthropometric, clinical, biochemical, and lifestyle measurements were analized. We evaluated the correlation between vaspin and risk factors for IR in adolescents with normal and high body fat percentage (%BF) and did a logistic regression to calculate the odds ratio for IR according to vaspin quartiles sex specific for the sample. Results: Vaspin was positively correlated with IR in adolescents with high %BF (r = 0.23, p = 0.003). The logistic regression analysis adjusted for sex, age, BMI, and pubertal stage showed that adolescents in the 2nd (OR = 0.43, 95% CI = 0.23-0.80, p = 0.008) and 3rd (OR = 0.46, 95% CI = 0.25-0.85, p = 0.014) quartile of vaspin concentration had a lower risk for IR. When the model was adjusted for %BF and physical activity, the association remained statically significant only for adolescents in the 2nd quartile. Conclusion: Vaspin was correlated positively with risk factors associated with insulin metabolism in adolescents with high %BF. Vaspin was associated with a reduced risk of IR independently of BMI and pubertal stage and the association was influenced by body fat and physical activity in these adolescents.

Purpose: To assess the compliance of Daily Disposable Contact Lenses (DDCLs) wearers with replacing lenses at a manufacturer-recommended replacement frequency. To evaluate the ability of two different Health Behavioural Theories (HBT), The Health Belief Model (HBM) and The Theory of Planned Behaviour (TPB), in predicting compliance. Method: A multi-centre survey was conducted using a questionnaire completed anonymously by contact lens wearers during the purchase of DDCLs. Results: Three hundred and fifty-four questionnaires were returned. The survey comprised 58.5% females and 41.5% males (mean age 34. ±. 12. years). Twenty-three percent of respondents were non-compliant with manufacturer-recommended replacement frequency (re-using DDCLs at least once). The main reason for re-using DDCLs was "to save money" (35%). Predictions of compliance behaviour (past behaviour or future intentions) on the basis of the two HBT was investigated through logistic regression analysis: both TPB factors (subjective norms and perceived behavioural control) were significant (p. <. 0.01); HBM was less predictive with only the severity (past behaviour and future intentions) and perceived benefit (only for past behaviour) as significant factors (p. <. 0.05). Conclusions: Non-compliance with DDCLs replacement is widespread, affecting 1 out of 4 Italian wearers. Results from the TPB model show that the involvement of persons socially close to the wearers (subjective norms) and the improvement of the procedure of behavioural control of daily replacement (behavioural control) are of paramount importance in improving compliance. With reference to the HBM, it is important to warn DDCLs wearers of the severity of a contact-lens-related eye infection, and to underline the possibility of its prevention.

Toxoplasmosis is an important parasitic zoonosis with a worldwide distribution, being the parasitic disease with the highest occurrence in Europe. Wild boar has an important role in the epidemiological cycle of Toxoplasma gondii as an intermediate host, that can potentially infect humans when the meat is consumed raw or undercooked. The purpose of this work was to determine the presence of antibodies to T. gondii in serum of hunted wild boar. During the hunting season 2011/2012, sera samples were collected from 97 wild boar and tested for IgG antibodies to T. gondii, using the modified agglutination test. Twenty out of the 97 wild boar (20.6%) were seropositive for T. gondii IgG antibodies. Multivariate logistic regression analysis showed that males and older animals were associated with T. gondii seropositivity. These results show that T. gondii has an important presence in wild boar population from Portugal, suggesting a potential zoonotic risk for humans when wild boar meat or meat products are consumed raw or undercooked.

Introducción: Actualmente existe un reconocimiento creciente de que el hogar desempeña un papel importante en varias cuestiones de higiene y salud pública. El ambiente del hogar ha sido implicado como una fuente importante de propagación de enfermedades infecciosas, y la intervención de las medidas de higiene, implican una reducción de la incidencia, especialmente en los países menos desarrollados y en poblaciones vulnerables como las gestantes. Objetivo: Evaluar la asociación entre la práctica de hábitos higiénicos de las gestantes estrato 1 y 2 de las localidades de Usaquén y Kennedy en relación a sus factores socioeconómicos. Métodos: Estudio Analítico de Corte transversal. Se realizó en las gestantes de los estratos 1 y 2 de las localidades de Usaquén y Kennedy en la ciudad de Bogotá. Se recolectaron datos referentes a factores socioeconómicos y hábitos de higiene de 141 gestantes a través de la aplicación de una encuesta. Los datos obtenidos de las variables de interés fueron procesados a través de análisis multivariado y regresión logística paramétrica y no paramétrica, con el fin de establecer si existía asociación o no entre las mismas. Resultados: Existe asociación entre el número de Nacidos vivos y la presencia de plagas (p=0.034 y Coeficiente de correlación: -1.253). Así mismo se encontró asociación habitar en cuartos rentados y la limpieza de casa general (p=0.008 y Coeficiente de correlación: 0.480). Existe una asociación entre la variable edad y el lavado de frutas (p=0.041 y Coeficiente de correlación: 0.384). Conclusiones: Existe relación entre los hábitos higiénicos y los factores socioeconómicos de las gestantes estudiadas. Existe un mayor hábito de lavado de frutas antes de ser consumidas en gestantes de mayor edad. Adicional a esto se evidencia a mayor número de hijos hay menor presencia de plagas en el hogar y mayor limpieza del hogar. Solo en un pequeño porcentaje de los hogares se evidencio una óptima limpieza, por lo cual se deben plantear más políticas para mejorar la higiene de los mismos ya que los datos reportados permanecen subóptimos en la población seleccionada.

La prevalencia de sintomatología osteomuscular en los trabajadores va en aumento. La influencia de factores propios de la labor como la postura, los movimientos repetitivos, el tipo de remuneración y los factores sociodemográficos como la edad, el sexo y el índice de masa corporal pueden influir en la aparición temprana de sintomatología osteomuscular. Objetivo: Determinar la prevalencia de sintomatología musculoesquelética en trabajadores de una empresa Outsourcing operativa en la ciudad de Bogotá Colombia en el año 2016. Métodos: Se realizó un estudio de corte transversal para la estimación de la prevalencia de sintomatología musculoesquelética para lo cual se utilizó una fuente de datos secundaria de 323 trabajadores de una empresa Outsourcing operativa. Se tuvieron en cuenta las variables sociodemográficas y laborales. El análisis descriptivo incluyó el cálculo de la media y los porcentajes. El análisis comparativo se realizó por medio del Test Chi² con una significancia estadística p <0.05 para un intervalo de confianza del 95%. Por último se realizó un análisis de regresión logístico. Resultados: Del total de la población estudiada, la cual fue de 323 personas, el 55.4% de los trabajadores corresponde al sexo femenino y el 44.6% corresponde al sexo masculino. El promedio de edad fue de 30.34 años. El tiempo en el cargo que presentó mayor prevalencia fue entre 13 a 60 meses con un porcentaje de 60,7%. Para el estudio se incluyó la variable de tipo de remuneración con un porcentaje de 58.2% de pago por salario mínimo legal mensual vigente (SMLMV), mientras que el 41.8% de la población recibió pago por destajo. La prevalencia de sintomatología por segmentos en la población fue de 17.3% para espalda baja, el 12,1% para mano y muñeca, el 10.2% para piernas, el 7.7% para espalda alta, el 7,4% para cuello, el 7,1% para hombro, el 6.2% para brazos y antebrazos y el 2,5% para dedos. Se aplicó un modelo de regresión logístico, analizando variables de confusión e interacción, estas últimas no aportaron al modelo. Con la variable tipo de remuneración, se encontró una asociación estadística significativa (P= 0.005) con la sintomatología. Para las demás variables sociodemográficas y extra-laborales no se obtuvieron resultados estadísticamente significativos Conclusión: Teniendo en cuenta los resultados obtenidos en el estudio se encontró asociación estadísticamente significativa entre la sintomatología de los segmentos cuello y manos con la variable sociodemográfica (sexo) y la variable laboral (tipo de remuneración: Destajo). Para las demás variables no se encontró asociación. Se sugiere realizar programas de vigilancia epidemiológica para hacerle seguimiento a esta población y que permitan la prevención de enfermedades de origen laboral.
