During the healthcare reform debate in the United States in 2009/2010, many health policy experts expressed a concern that expanding coverage would increase waiting times for patients to obtain care. Many complained that delays in obtaining care in turn would compromise the quality of healthcare in the United States. Using data from The Commonwealth Fund 2010 International Health Policy Survey in Eleven Countries, this study explored the relationship between wait times and quality of care, employing a wait time scale and several quality of care indicators present in the dataset. The impact of wait times on quality was assessed. Increased wait time was expected to reduce quality of care. However, this study found that wait times correlated with better health outcomes for some measures, and had no association with others. Since this is a pilot study and statistical significance was not achieved for any of the correlations, further research is needed to confirm and deepen the findings. However, if future studies confirm this finding, an emphasis on reducing wait times at the expense of other health system level performance variables may be inappropriate. ^

STATs play crucial roles in a wide variety of biological functions, including development, proliferation, differentiation, migration and in cancer development. In the present study, we examined the impact of Stat3 deletion or activation on behavior of keratinocytes, including keratinocyte stem cells (KSCs). Deletion of Stat3 specifically in the bulge region of the hair follicle using K15.CrePR1 X Stat3fl/fl mice led to decreased tumor development by altering survival of bulge region KSCs. To further understand the role of KSCs in skin tumorigenesis, K5.Stat3C transgenic (Tg) mice which express a constitutively active/dimerized form of Stat3 called Stat3C via the bovine keratin 5 (K5) promoter were studied. The number of CD34 and α6 integrin positive cells was significantly reduced in Tg mice as compared to non-transgenic (NTg) littermates. There was a concomitant increase in the progenitor populations (Lgr-6, Lrig-1 and Sca-1) in the Tg mice vs. the stem cell population (CD34 and Keratin15). To investigate the mechanism underlying the increase in the progenitor population at the expense of bulge region KSCs we examined if Stat3C expression was involved in inducing migration of the bulge region KSCs. There was altered β-catenin and α6-integrin expression in the hair follicles of Tg mice, which may have contributed to reduced adhesive interactions between the epithelial cells and the basement membrane facilitating migration out of the niche. To further study the effect of Stat3 on differentiation of keratinocytes we analyzed the epidermal keratinocytes in K5.Cre X Stat3fl/fl mice. There was an increase in the expression of epidermal differentiation markers in the Stat3 knockout mice. These data suggest that deletion of Stat3 in the epidermis and hair follicle induced differentiation in these cells. Preliminary studies done with the BK5.Stat3C mouse model suggests that multiple hair follicle stem/progenitor populations may be involved in skin tumor development and progression in this model of skin tumorigenesis. Overall, these data suggest that Stat3 plays an important role in differentiation as well as migration of keratinocytes and that these effects may play a role during epithelial carcinogenesis.

The intent of this research was to identify the level of risk methanol posed to a fetus during an ethanol co-exposure. This investigation was prompted by the known competitive inhibition properties of ethanol and the developmental toxicity of methanol. Integrated into this research was the practicality necessitated by regulatory processes, namely: does the risk justify the expense of additional research. To this end, the scope and nature of exposures were summarized to illustrate the ubiquity of these chemicals and the potential for dual exposure. Similarly, severity of outcome was evaluated by systematically reviewing the LOAELs, NOAELs, and statistical significance contained in methanol-induced developmental studies. Results. Blood methanol levels corresponding to developmental effects in laboratory studies were found to be substantially higher than the blood methanol levels predicted in high-risk methanol-ethanol exposure scenarios. This indicates that ethanol would not likely exacerbate methanol toxicity to the point of teratogenicity; however, it is important to note that the developmental toxicity of ethanol—an established human teratogen—was not included in the evaluation. Ethanol's contribution as a developmental toxicant rather than merely as an attenuator of methanol toxicity undermines the severity of effects possible from this chemical combination. Therefore further evaluation is needed to assess the developmental toxicities following dual exposures before rendering methanol and ethanol a high-priority mixture.^

Growing scab-resistant apple cultivars on fully dwarfing rootstocks increases the feasibility for producing organically grown apples in the Midwest. However, in an organic orchard, fruit thinning to optimize crop load must be done by hand at a very high labor expense. The alternative is biennial bearing and inconsistent supply to meet consumer demands. Recently, sprays containing organicapproved materials such as liquid lime sulfur, fish, and various vegetable oils, salts, and kaolin have been tried alone or in combination for thinning apples with some degree of success. Lime sulfur alone or in combination with spray oil was recently labeled for use in Washington state orchards. This study was undertaken to evaluate the effectiveness of liquid lime sulfur alone and in combination with spray oil applied at various times during bloom on thinning three scab-resistant apple cultivars under Iowa conditions.

Analyses of rock clasts and of heavy minerals in upper Miocene coarse detrital units drilled along the East Sardinia passive-type continental margin (Sites 654, 653, 652, and 656) reveal that the stretched basement contains quite complex rock suites. Taking also into account previous sampling data, in moving from west to east across the margin, the nature of the basement changes drastically. To the west there are mostly Hercynian basement rocks with their cover, referable to the alpine foreland of the Corsica-Sardinia block. To the east, along the lower margin, where crustal thinning is quite severe, the basement contains rock suites referable to a pre-upper Tortonian orogenized zone with units constituting parts of the Alpine and Apenninic chains (presumably with thickened continental crust prior to stretching). Largest thinning and ocean forming occurred then, in a rather short time, mostly at the expense of unstable crust just thickened by orogenetic/tectogenetic processes.

The climate of Marine Isotope Stage (MIS) 11, the interglacial roughly 400,000 years ago, is investigated for four time slices, 416, 410, 400, and 394 ka. The overall picture is that MIS 11 was a relatively warm interglacial in comparison to preindustrial, with Northern Hemisphere (NH) summer temperatures early in MIS 11 (416-410 ka) warmer than preindustrial, though winters were cooler. Later in MIS 11, especially around 400 ka, conditions were cooler in the NH summer, mainly in the high latitudes. Climate changes simulated by the models were mainly driven by insolation changes, with the exception of two local feedbacks that amplify climate changes. Here, the NH high latitudes, where reductions in sea ice cover lead to a winter warming early in MIS 11, as well as the tropics, where monsoon changes lead to stronger climate variations than one would expect on the basis of latitudinal mean insolation change alone, are especially prominent. The results support a northward expansion of trees at the expense of grasses in the high northern latitudes early during MIS 11, especially in northern Asia and North America.

La ciudad de Córdoba no ha sido ajena a los procesos económicos-sociales de la “globalización", que implican una fuerte expansión y concentración del capital, con impactos en la esfera social y aumento de las desigualdades. La salida de la convertibilidad a fines del año 2001 y el consecuente incremento de la apertura externa del país impactó en algunos sectores económicos, agro-exportadores principalmente, que insertos en la dinámica comercial internacional logran obtener amplios márgenes de ganancias. Las ganancias se transfieren a otros sectores de la economía que continúan aumentando la rentabilidad de los agentes involucrados. Desde entonces se observa una marcada expansión de la actividad inmobiliaria y de la construcción, en barrios centrales de la misma y hacia la periferia, con la construcción de barrios privados. Esto exacerba el valor del suelo urbano y produce una revalorización inmobiliaria en las áreas mencionadas, que es demandada por grandes inversores. La configuración territorial de la ciudad de Córdoba en los últimos años ha estado comandada por el capital privado. Sin un proyecto urbano que garantice el ordenamiento de la ciudad; ha desembocado en altas densidades edificadas en detrimento de espacios públicos; en especial plazas, áreas recreativas y deportivas. A su vez se produce mayor densidad de habitantes por Km2 que demandan la utilización de estos espacios. Los vacíos urbanos que contiene la ciudad aún pueden revertir parte de esta situación y re-valorizar el “espacio verde público" como espacio de producción y reproducción socio-espacial.

Considerando el alto costo farmacológico de los tratamientos oncológicos actuales y de los anticuerpos monoclonales pautados para diferentes terapias que están apareciendo y que serán prácticamente lo único que se use en el futuro para diferentes diagnósticos, el Programa Oncológico Provincial cubre sin cargo alguno las necesidades de los pacientes atendidos en Hospitales Estatales, cumpliendo una función social, dispensando los medicamentos solicitados por el médico tratante, en forma confiable, segura, y con equidad para recibir el tratamiento adecuado en el momento oportuno. Los recursos que pueden ser destinados al gasto farmacéutico son limitados, por lo que es necesario racionalizar el consumo y priorizar en la asignación de estos recursos a las opciones que presentan mayores ventajas económicas, basada en mantener una mejor relación costo/beneficio, repercutiendo de manera directa en los enfermos y en la sociedad en general. En este trabajo, se analiza el sistema de adquisición de drogas oncológicas usado por el Programa Oncológico Provincial realizando una comparación entre el sistema de compras original y el adoptado por el Programa en el año 2005. Se evalúa el periodo 2001-2007, en base a diversos indicadores y datos estadísticos, para determinar si ha mejorado la calidad de las prestaciones, si ha disminuido la demanda contenida y si se ha logrado mejorar la relación costo / beneficio, controlando así el gasto sanitario global lo que constituiría una inversión rentable para el sistema de salud. Los resultados indican que el nuevo sistema adoptado por el Programa Provincial de Oncología ha mejorado la provisión de medicamentos y garantiza mayor calidad en las prestaciones, pero estos resultados pueden ser superados. También muestra la gravedad del problema de la utilización de los nuevos fármacos monoclonales.

El transporte del agua en las plantas es impulsado por diferencias de energía libre entre el suelo y la atmósfera, y está regulado por mecanismos biológicos evitadores, como el cierre estomático. La hidratación y la turgencia foliares resultan del equilibrio entre ΨL del apoplasto, el potencial osmótico del simplasto y la elasticidad de los tejidos. Sobre esta base se conjeturó que las interacciones de los mecanismos evitadores del estrés hídrico de la planta tienen un rol clave en la definición de su resistencia a déficit hídrico. Para probar esta hipótesis se construyó un modelo mecanístico basado en las leyes del flujo de savia de Van de Honert, de difusión de Fick, de elasticidad de Hooke, la ecuación de Gardner para el flujo del agua en la rizósfera y el modelo de conductancia estomática (gs) de Buckley. Mediante el modelo se demostró teóricamente que la hidratación y la turgencia foliares dependen de la oferta de agua edáfica (representada por el potencial hídrico del suelo) y de la demanda evaporativa de la atmósfera (representada por la radiación absorbida, la temperatura del aire, la velocidad del viento y el déficit de presión de vapor de la atmósfera). También que los mecanismos evitadores del estrés hídrico -i.e., conductancia hidráulica de la planta, conductancia estomática, elasticidad del tejido y potencial osmótico a turgencia máxima- son todos necesarios para determinar la hidratación y la turgencia foliares. El modelo también demostró que la conductancia hidráulica suelo-hoja (kL) depende de la fracción de agua edáfica transpirable (FTSW) con un patrón de decaimiento sigmoide, a medida que el suelo se seca. Esto implica que las variables que dependen en parte de kL (i.e., gs, transpiración, fotosíntesis y superficie foliar) también dependen de FTSW con el mismo patrón. El modelo se probó experimentalmente a distintos niveles de humedad edáfica (desde déficit hídrico nulo, hasta severo) en cinco variedades de vid y mostró un poder predictivo superior al 90%. En todas las variedades las gs se asociaron linealmente con las kL observadas, al considerar todas las situaciones de déficit hídrico en conjunto, si bien la pendiente de estas relaciones fueron distintas en cada variedad. La contrastación experimental mostró que, en una escala de tiempo de varios meses, las variedades más evitadoras -i.e., Grenache y Cereza- mantuvieron mayor kL, ajuste osmótico y rigidez de los tejidos y una menor pendiente de la relación de gs vs. kL, que las variedades menos evitadoras -i.e., Malbec y Syrah-. La menor pendiente de la relación entre gs y kL, en las variedades más evitadoras, estuvo asociada a una mayor cantidad de estomas, en relación con la cantidad de células epidérmicas. Los variedades más evitadoras bajo déficit hídrico moderado -i.e., con una fracción de agua edáfica transpirable entre 0,6 y 0,4- tuvieron mayor superficie foliar y produjeron más biomasa, favoreciendo raíces profundas y densas, y ahorrando agua. Chardonnay mantuvo una alta hidratación y turgencia a expensas de un alto gasto de agua debido a que privilegiaba una alta kL por sobre el ajuste estomático, por lo que no podría considerarse en forma estricta como muy evitadora.

The environmental interpretation of the 13C/12C variations in the skeletons of massive corals is still a matter of debate. A 19-year seasonal skeletal 13C/12C record of a shallow-water Pontes coral from the northern Red Sea (Gulf of Aqaba) documents interannual events of extraordinarily large plankton blooms, indicated by anomalous 13C depletions in the coral skeleton. These blooms are caused by deep vertical water mass mixing, convectively driven in colder winters, which results in increased supplies of nutrients to the surface waters. The deep vertical mixings can sometimes be driven by the cooling occurring throughout the Middle East after large tropical volcanic eruptions. We therefore have evidence in our coral skeletal 13C/12C record for an indirect volcanic signal of the eruptions of El Chichón (1982) and Mount Pinatubo (1991). Deep mixing induced 13C/12C variations of the dissolved inorganic carbon in the surface waters can be neglected at this location. We therefore suggest that the 13C skeletal depletions can be best explained by changes in the coral's autotrophy-heterotrophy diet, through increased heterotrophic feeding on Zooplankton during the blooms. Increased feeding on 13C-depleted Zooplankton or increased heterotrophy at the expense of autotrophy can both result in a 13C-depleted coral skeleton. However, this suggestion requires more testing. If our conclusions are substantiated, seasonal skeletal 13C/12C records of corals which change from autotrophy under normal conditions to increased heterotrophy during bloom events may be used as indicators of ocean paleoproductivity at interannual resolution, available from no other source.

The silicate fractions of recent pelagic sediments in the central north Pacific Ocean are dominated by eolian dust derived from central Asia. An 11 Myr sedimentary record at ODP Sites 885/886 at 44.7°N, 168.3°W allows the evaluation of how such dust and its sources have changed in response to late Cenozoic climate and tectonics. The extracted eolian fraction contains variable amounts (>70%) of clay minerals with subordinate quartz and plagioclase. Uniform Nd isotopic compositions (epsilon-Nd =38.6 to 310.5) and Sm/Nd ratios (0.170-0.192) for most of the 11 Myr record demonstrate a well-mixed provenance in the basins north of the Tibetan Plateau and the Gobi Desert that was a source of dust long before the oldest preserved Asian loess formed. epsilon-Nd values of up to 36.5 for samples 62.9 Ma indicate <=35 wt% admixture of a young, Kamchatka-like volcanic arc component. The coherence of Pb and Nd in the erosional cycle allows us to constrain the Pb isotopic composition of Asian loess devoid of anthropogenic contamination to 206Pb/204Pb =18.97 +/- 0.06, 207Pb/204Pb =15.67 +/- 0.02, 208Pb/204Pb =39.19 +/- 0.11. 87Sr/86Sr (0.711-0.721) and Rb/Sr ratios (0.39-1.1) vary with dust mineralogy and provide an age indication of ~250 Ma. 40Ar/39Ar ages of six dust samples are uniform around 200 Ma and match the K-Ar ages of modern dust deposited on Hawaii. These data reflect the weighted age average of illite formation. Changes from illite- smectite with significant kaolinite to illite- and chlorite-rich, kaolinite-free assemblages since the late Pliocene document changes in the intensity of chemical weathering in the source region. Such weathering evidently did not disturb the K-Ar systematics, and only induced scatter in the Rb-Sr data. We propose that when smectite forms at the expense of illite, K and Ar are quantitatively lost from what becomes smectite, but are quantitatively retained in adjacent illite layers. 40Ar/39Ar age data, therefore, are insensitive to smectite formation during chemical weathering but date the diagenetic growth of illite, the major K-bearing phase in the dust. Over the past 12 Myr, the dust flux to the north Pacific increased by more than an order of magnitude, documenting a substantial drying of central Asia. This climatic change, however, did not alter the ultimate source of the dust, and neoformational products of chemical weathering always remained subordinate to assemblages reworked by mechanical erosion in dust deposited in eastern Asia and the Pacific Ocean.

El artículo analiza las principales características del modelo tecnológico del agronegocio en la Argentina, discute sus principales problemas y propone una reflexión que enmarque la problemática en contextos económicos y políticos más amplios. Esta tecnología es descripta como technological fix y se presentan tres atributos principales: instantaneidad, transitoriedad y recurrencia. La supuesta eficiencia del modelo productivo ocurre a expensas de la dilapidación del capital natural y de los costos que internalizan otros actores sociales, ya sea vía acumulación por desposesión o a través de la socialización y del diferimiento temporal de sus externalidades negativas. Su fortaleza radica en que su poder trasciende largamente la esfera tecnológica. Poner en dudas al actual modelo implica no sólo cuestionar a su cabeza visible (i.e., el agronegocio), sino también objetar a las instituciones (científicas, educativas, legales y administrativas) y a las estructuras políticas que lo sostienen. Finalmente, el artículo discute algunas alternativas y propone desarrollar una agronomía política para Latinoamérica.

A partir de la fundación de la ciudad de Buenos Aires en el siglo XVI, la ocupación del espacio provincial se realizó a expensas de las tribus autóctonas que habitaban las regiones pampeana y patagónica. Desde fines del siglo XVIII, y durante todo el XIX, la población hispano-criolla realizó sucesivos avances para incorporar estas tierras a la estructura productiva que se vinculaba al mercado internacional. En este trabajo analizamos la cuestión de la ocupación sin títulos de la tierra como un problema persistente en esa frontera que se expandía. Para ello hemos focalizado nuestro estudio en dos momentos y lugares diferentes: Chascomús (1780-1838) y Bragado (1846-1860). Estos casos nos permiten demostrar la permanencia de los establecimientos productivos sin respaldo legal a medida que se fueron cerrando las posibilidades de acceder a la propiedad plena de la tierra, insistiendo en que la primitiva ocupación y la posterior legalización del dominio fueron dos procesos permanentes, complementarios y conflictivos hasta fines del siglo XIX

Antipatharia are a diverse group of corals with many species found in deep water. Many Antipatharia are habitat for associates, have extreme longevity and some species can occur beyond 8500 m depth. As they are major constituents of 'coral gardens', which are Vulnerable Marine Ecosystems (VMEs), knowledge of their distribution and environmental requirements is an important pre-requisite for informed conservation planning particularly where the expense and difficulty of deep-sea sampling prohibits comprehensive surveys. This study uses a global database of Antipatharia distribution data to perform habitat suitability modelling using the Maxent methodology to estimate the global extent of black coral habitat suitability. The model of habitat suitability is driven by temperature but there is notable influence from other variables of topography, surface productivity and oxygen levels. This model can be used to predict areas of suitable habitat, which can be useful for conservation planning. The global distribution of Antipatharia habitat suitability shows a marked contrast with the distribution of specimen observations, indicating that many potentially suitable areas have not been sampled, and that sampling effort has been disproportionate to shallow, accessible areas inside marine protected areas (MPAs). Although 25% of Antipatharia observations are located in MPAs, only 7-8% of predicted suitable habitat is protected, which is short of the Convention on Biological Diversity target to protect 10% of ocean habitats by 2020.
