Of 156 cases of histoplasmosis observed in the State of Rio Grande do Sul (Brazil), during a 21-year period (1978-1999) 137 were included in this study. Sixty-seven per cent of the patients had hematogeneous disseminated histoplasmosis, 24% had a self-limited syndrome (acute pulmonary histoplasmosis, histoplasmoma or primary pulmonary lymph node complex), and 9 per cent had chronic pulmonary histoplasmosis. Clinical, mycological, and epidemiological data were reviewed and commented.

The authors report a confirmed case of hantavirus pulmonary syndrome in the rural area of the municipality of Anajatuba, state of Maranhão. Two other suspected cases from the same region are also described. The confirmed case involved a previously healthy young woman who died with signs and symptoms of acute respiratory insufficiency 5 days after presenting fever, myalgia and a dry cough. The patient was a student who was helping her parents with work in the fields; it was a habit of the family to store rice inside the house. The suspected cases involved two first-degree relatives working as field hands who died of acute respiratory insufficiency 24 and 48 hours, respectively, after presenting fever, myalgia and a dry cough. Both stored rice and corn inside their home. People living in the region reported massive infestations with rats in the woods and fields.

O modelo de sociedade em que vivemos depende em grande medida de uma fonte de energia eléctrica ininterrupta, estável e capaz. Tanto no sector industrial, comercial como residencial, a contínua disponibilidade de energia tem um papel de extrema importância na segurança e conforto das pessoas. Assim, e nos momentos de interrupção da alimentação de energia pela rede de distribuição principal, quer por motivos de falha, manutenção ou outros, um grupo eletrogéneo de emergência apresenta-se, muitas das vezes, como a solução para o problema. Torna-se portanto vital o correto dimensionamento e instalação do equipamento de forma a garantir a longevidade do mesmo, bem como a fiabilidade e estabilidade desta fonte de energia. Iremos abordar neste artigo, de forma muito sucinta, quais os principais critérios que deverão ser tomados em conta no dimensionamento de um grupo eletrogéneo.

A presente dissertação de mestrado tem como primeiro e fundamental objetivo salientar a importância das levadas da ilha da Madeira como património cultural e sublinhar a pertinência da sua candidatura a Património da Humanidade. Debruçando-se sobre um enquadramento conceitual e teórico acerca do património cultural e das levadas da ilha da Madeira, esta investigação pretende conhecer qual a relação existente entre ambos, existindo, também, o intuito de compreender qual o interesse da valorização, preservação e conservação destes aquedutos. O estudo desenvolveu-se, portanto, no sentido de responder à questão “As levadas da ilha da Madeira e o património cultural: que relação?”. Para dar resposta à mesma procedeu-se à explanação do conceito de património cultural e a sua legislação e, ainda, à história das levadas, que se cruza com a da própria ilha, recorrendo à consulta, leitura e análise de documentos relacionados com estes assuntos bem como ao trabalho de campo, percorrendo diversas levadas e registando observações in loco. Neste sentido, chegou-se à conclusão de que as levadas, que, no século XV, surgiram da necessidade de levar a água até aos terrenos agrícolas, são hoje a prova do esforço, luta e triunfo do povo madeirense. É na perspetiva de herança e cultura que encarar as levadas como património cultural faz todo o sentido, na medida em que apresentam, de facto, uma autenticidade ímpar, relativamente à sua forma e conceção, construção e materiais utilizados, tradições e técnicas de gestão de águas, localização, enquadramento e amplitude, termos linguísticos, lendas e contos a que deram origem e, ainda, a todo um património material e imaterial consigo relacionados.

Twenty-two cases of nocardial infections were diagnosed in our city between 1977- 1998. All patients whose clinical specimens showed Nocardia spp. at Gram stain, which were further confirmed by culture, were selected to be included in the study. Data from patients who were cured were compared with those from patients who died by statistical tests using EPIINFO version 6.04 software. Six isolates were identified as Nocardia asteroides complex, one as Nocardia asteroides sensu stricto and other as Nocardia brasiliensis. We had 17 cases of lung nocardiosis, being one out of them also a systemic disease. Other four cases of systemic nocardiosis were diagnosed: nocardial brain abscesses (one); nocardiosis of the jejunum (one); multiple cutaneous abscesses (one); and a case of infective nocardial endocarditis of prosthetic aortic valve. One patient had a mycetoma by N. brasiliensis. Fifteen (68.2%) out of 22 patients were immunosuppressed, being most (93.3%) by high-doses corticotherapy. Mortality by nocardial infection was 41%; mortality of systemic nocardiosis was 60%. Nocardiosis has a bad prognosis in immunosuppressed patients and also in non-immunosuppressed patients if the diagnosis is delayed. We propose that the delay in diagnosis should be examined in larger series to document its influence in the prognosis of the disease.

Bartonellosis (Carrion's Disease) during pregnancy is associated with high rates of maternal and perinatal mortality. We report the immunological patterns in two cases of human bartonellosis during pregnancy. One patient had an uncomplicated course while the second patient developed life threatening anasarca and cardiac tamponade. The patient with a complicated course had a Th1 response with a higher elevation of IL-10. This elevation has been associated with poor outcome pregnancies during bacterial infections.

No vector transmitted cases of Chagas disease had been notified in the state of São Paulo since the 1970s. However, in March, 2006, the death of a six-year-old boy from the municipality of Itaporanga was notified to the Center for Epidemiological Survey of the São Paulo State Health Secretariat: an autochthonous case of acute Chagas disease. The postmortem histopathological examination performed in the Hospital das Clínicas of the Botucatu School of Medicine confirmed the diagnosis. Reference to hospital records, consultation with the health professionals involved in the case and interviews with members of the patient's family supplied the basis for this study. We investigated parasite route of transmission, probable local reservoirs and vectors. No further human cases of acute Chagas disease were diagnosed. No locally captured vectors or reservoirs were found infected with Trypanosoma cruzi. Alternative transmission hypotheses - such as the possible ingestion of foods contaminated with vector excreta - are discussed, as well as the need to keep previously endemic regions and infested houses under close surveillance. Clinicians should give due attention to such signs as uni- or bilateral palpebral edema, cardiac failure, myocarditis, pericarditis, anasarca and atypical signs of nephrotic syndrome or nephritis and consider the diagnostic hypothesis of Chagas disease.

Speech interfaces for Assistive Technologies are not common and are usually replaced by others. The market they are targeting is not considered attractive and speech technologies are still not well spread. Industry still thinks they present some performance risks, especially Speech Recognition systems. As speech is the most elemental and natural way for communication, it has strong potential for enhancing inclusion and quality of life for broader groups of users with special needs, such as people with cerebral palsy and elderly staying at their homes. This work is a position paper in which the authors argue for the need to make speech become the basic interface in assistive technologies. Among the main arguments, we can state: speech is the easiest way to interact with machines; there is a growing market for embedded speech in assistive technologies, since the number of disabled and elderly people is expanding; speech technology is already mature to be used but needs adaptation to people with special needs; there is still a lot of R&D to be done in this area, especially when thinking about the Portuguese market. The main challenges are presented and future directions are proposed.

During the earlier stages of visceral leishmaniasis transmission in Posadas City, Misiones, both the night activity and attraction to humans of Lutzomyia longipalpis were assessed, in order to provide preliminary recommendations. The impact of peridomestic deltamethrin spraying performed by local officials was also evaluated. Although Lu. longipalpis were found in traps located over a dog the entire night, 90% of the females were captured from 20.30h to 1.30h, and only landed on a human when he was at a distance of 1.5 m from the dog. Peridomestic spraying of deltamethrin (25 mg/m²) reduced the sand fly capture up to seven days post-intervention without dispersion in the border of the sprayed areas. These results support the recommendations about time-space focus of the protection measures: first half of the night, in the backyard, with pets and domestic animals kept at least 5 m from humans. The deltamethrin as it was used did not seem very effective in this scenario; neither did the eventual use of bed nets, at least in adults, due to the place/hour of sand fly higher activity. This study strengthens the need for a multidisciplinary approach to develop prevention strategies based both on biological and anthropological studies.

Human Bartonellosis has an acute phase characterized by fever and hemolytic anemia, and a chronic phase with bacillary angiomatosis-like lesions. This cross-sectional pilot study evaluated the immunology patterns using pre- and post-treatment samples in patients with Human Bartonellosis. Patients between five and 60 years of age, from endemic areas in Peru, in the acute or chronic phases were included. In patients in the acute phase of Bartonellosis a state of immune peripheral tolerance should be established for persistence of the infection. Our findings were that elevation of the anti-inflammatory cytokine IL-10 and numeric abnormalities of CD4+ and CD8+ T-Lymphocyte counts correlated significantly with an unfavorable immune state. During the chronic phase, the elevated levels of IFN-γ and IL-4 observed in our series correlated with previous findings of endothelial invasion of B. henselae in animal models.

Non-H. pylori helicobacters (NHPH) have been demonstrated as gastric spiral-shaped bacteria in specimens obtained from dogs; however, their roles in the pathogenesis of upper gastrointestinal disease have not yet been clearly established. The purpose of this study was to evaluate the prevalence of NHPH DNA in the gastric mucosa of dogs and its association with histopathology. Helicobacter was detected through histopathological techniques, PCR, and FISH analysis from fundic biopsies of twenty dogs with or without signs of gastrointestinal disease. PCR and FISH were based on partial 16S rRNA gene sequences. Nineteen dogs showed mild to marked gastritis in the fundus, and only one dog had a healthy gastric mucosa. NHPH DNA was detected in 18 dogs with gastritis and one with normal gastric mucosa. However, there was no significant correlation between the presence of NHPH DNA and the degree of gastritis. These results show a high prevalence of NHPH DNA in the gastric mucosa of dogs from Venezuela. Further studies are necessary to determine a possible association between a specific NHPH species and the degree of gastritis.
