Background There is a comprehensive literature on the academic outcomes (attrition and success) of students in traditional/baccalaureate nursing programs, but much less is known about the academic outcomes of students in accelerated nursing programs. The aim of this systematic review is to report on the attrition and success rates (either internal examination or NCLEX-RN) of accelerated students, compared to traditional students. Methods For the systematic review, the databases (Pubmed, Cinahl and PsychINFO) and Google Scholar were searched using the search terms ‘accelerated’ or ‘accreditation for prior learning’, ‘fast-track’ or ‘top up’ and ‘nursing’ with ‘attrition’ or ‘retention’ or ‘withdrawal’ or ‘success’ from 1994 to January 2016. All relevant articles were included, regardless of quality. Results The findings of 19 studies of attrition rates and/or success rates for accelerated students are reported. For international accelerated students, there were only three studies, which are heterogeneous, and have major limitations. One of three studies has lower attrition rates, and one has shown higher success rates, than traditional students. In contrast, another study has shown high attrition and low success for international accelerated students. For graduate accelerated students, most of the studies are high quality, and showed that they have rates similar or better than traditional students. Thus, five of six studies have shown similar or lower attrition rates. Four of these studies with graduate accelerated students and an additional seven studies of success rates only, have shown similar or better success rates, than traditional students. There are only three studies of non-university graduate accelerated students, and these had weaknesses, but were consistent in reporting higher attrition rates than traditional students. Conclusions The paucity and weakness of information available makes it unclear as to the attrition and/or success of international accelerated students in nursing programs. The good information available suggests that accelerated programs may be working reasonably well for the graduate students. However, the limited information available for non-university graduate students is weak, but consistent, in suggesting they may struggle in accelerated courses. Further studies are needed to determine the attrition and success rates of accelerated students, particularly for international and non-university graduate students.

This chapter is about essential nursing care. Because it is often referred to as basic nursing, nurses may not always perceive it as deserving of priority. Yet, how well patients are cared for has a direct effect on their sense of wellbeing and their recovery. ‘Interventional patient hygiene’ is a systematic, evidence-based approach to nursing actions designed to improve patient outcomes using a framework of hygiene, catheter care, skin care, mobility and oral care.1 This chapter focuses on the physical care, infection control, preventative therapies and transport of critically ill patients. The first two areas are closely linked: poor-quality physical care increases the risk of infection. The final areas are essential features of critical care nursing.

This chapter introduces the beginning perioperative nurse to the key concepts and principles informing perioperative practice within Australasia. It describes the patient care roles of the nurse as well as the perioperative context and culture that inform the delivery of care during the surgical patient's journey. Aspects of the regulatory environment are examined, such as advocacy, accountability, delegation and scope of practice. In addition, the chapter explores the role of professional associations and highlights the importance of practice standards for perioperative nursing. The role of evidence-based practice (EBP) is also acknowledged. As this dynamic nursing speciality continues to evolve, the chapter concludes with a discussion of emerging advanced-practice roles for perioperative nurses.

The goal of this study was to examine the role of organizational causal attribution in understanding the relation of work stressors (work-role overload, excessive role responsibility, and unpleasant physical environment) and personal resources (social support and cognitive coping) to such organizational-attitudinal outcomes as work engagement, turnover intention, and organizational identification. In some analyses, cognitive coping was also treated as an organizational outcome. Causal attribution was conceptualized in terms of four dimensions: internality-externality, attributing the cause of one’s successes and failures to oneself, as opposed to external factors, stability (thinking that the cause of one’s successes and failures is stable over time), globality (perceiving the cause to be operative on many areas of one’s life), and controllability (believing that one can control the causes of one’s successes and failures). Several hypotheses were derived from Karasek’s (1989) Job Demands–Control (JD-C) model and from the Job Demands–Resources (JD-R) model (Demerouti, Bakker, Nachreiner & Schaufeli, 2001). Based on the JD-C model, a number of moderation effects were predicted, stating that the strength of the association of work stressors with the outcome variables (e.g. turnover intentions) varies as a function of the causal attribution; for example, unpleasant work environment is more strongly associated with turnover intention among those with an external locus of causality than among those with an internal locuse of causality. From the JD-R model, a number of hypotheses on the mediation model were derived. They were based on two processes posited by the model: an energy-draining process in which work stressors along with a mediating effect of causal attribution for failures deplete the nurses’ energy, leading to turnover intention, and a motivational process in which personal resources along with a mediating effect of causal attribution for successes foster the nurses’ engagement in their work, leading to higher organizational identification and to decreased intention to leave the nursing job. For instance, it was expected that the relationship between work stressors and turnover intention could be explained (mediated) by a tendency to attribute one’s work failures to stable causes. The data were collected from among Finnish hospital nurses using e-questionnaires. Overall 934 nurses responded the questionnaires. Work stressors and personal resources were measured by five scales derived from the Occupational Stress Inventory-Revised (Osipow, 1998). Causal attribution was measured using the Occupational Attributional Style Questionnaire (Furnham, 2004). Work engagement was assessed through the Utrecht Work Engagement Scale (Schaufeli & al., 2002), turnover intention by the Van Veldhoven & Meijman (1994) scale, and organizational identification by the Mael & Ashforth (1992) measure. The results provided support for the function of causal attribution in the overall work stress process. Findings related to the moderation model can be divided into three main findings. First, external locus of causality along with job level moderated the relationship between work overload and cognitive coping. Hence, this interaction was evidenced only among nurses in non-supervisory positions. Second, external locus of causality and job level together moderated the relationship between physical environment and turnover intention. An opposite pattern of interaction was found for this interaction: among nurses, externality exacerbated the effect of perceived unpleasantness of the physical environment on turnover intention, whereas among supervisors internality produced the same effect. Third, job level also disclosed a moderation effect for controllability attribution over the relationship between physical environment and cognitive coping. Findings related to the mediation model for the energetic process indicated that the partial model in which work stressors have also a direct effect on turnover intention fitted the data better. In the mediation model for the motivational process, an intermediate mediation effect in which the effects of personal resources on turnover intention went through two mediators (e.g., causal dimensions and organizational identification) fitted the data better. All dimensions of causal attribution appeared to follow a somewhat unique pattern of mediation effect not only for energetic but also for motivational processes. Overall findings on mediation models partly supported the two simultaneous underlying processes proposed by the JD-R model. While in the energetic process the dimension of externality mediated the relationship between stressors and turnover partially, all the dimensions of causal attribution appeared to entail significant mediator effects in the motivational process. The general findings supported the moderation effect and the mediation effect of causal attribution in the work stress process. The study contributes to several research traditions, including the interaction approach, the JD-C, and the JD-R models. However, many potential functions of organizational causal attribution are yet to be evaluated by relevant academic and organizational research. Physics teachers are in a key position to form the attitudes and conceptions of future generations toward science and technology, as well as to educate future generations of scientists. Therefore, good teacher education is one of the key areas of physics departments education program. This dissertation is a contribution to the research-based development of high quality physics teacher education, designed to meet three central challenges of good teaching. The first challenge relates to the organization of physics content knowledge. The second challenge, connected to the first one, is to understand the role of experiments and models in (re)constructing the content knowledge of physics for purposes of teaching. The third challenge is to provide for pre-service physics teachers opportunities and resources for reflecting on or assessing their knowledge and experience about physics and physics education. This dissertation demonstrates how these challenges can be met when the content knowledge of physics, the relevant epistemological aspects of physics and the pedagogical knowledge of teaching and learning physics are combined. The theoretical part of this dissertation is concerned with designing two didactical reconstructions for purposes of physics teacher education: the didactical reconstruction of processes (DRoP) and the didactical reconstruction of structures (DRoS). This part starts with taking into account the required professional competencies of physics teachers, the pedagogical aspects of teaching and learning, and the benefits of the graphical ways of representing knowledge. Then it continues with the conceptual and philosophical analysis of physics, especially with the analysis of experiments and models role in constructing knowledge. This analysis is condensed in the form of the epistemological reconstruction of knowledge justification. Finally, these two parts are combined in the designing and production of the DRoP and DRoS. The DRoP captures the knowledge formation of physical concepts and laws in concise and simplified form while still retaining authenticity from the processes of how concepts have been formed. The DRoS is used for representing the structural knowledge of physics, the connections between physical concepts, quantities and laws, to varying extents. Both DRoP and DRoS are represented in graphical form by means of flow charts consisting of nodes and directed links connecting the nodes. The empirical part discusses two case studies that show how the three challenges are met through the use of DRoP and DRoS and how the outcomes of teaching solutions based on them are evaluated. The research approach is qualitative; it aims at the in-depth evaluation and understanding about the usefulness of the didactical reconstructions. The data, which were collected from the advanced course for prospective physics teachers during 2001ï€2006, consisted of DRoP and DRoS flow charts made by students and student interviews. The first case study discusses how student teachers used DRoP flow charts to understand the process of forming knowledge about the law of electromagnetic induction. The second case study discusses how student teachers learned to understand the development of physical quantities as related to the temperature concept by using DRoS flow charts. In both studies, the attention is focused on the use of DRoP and DRoS to organize knowledge and on the role of experiments and models in this organization process. The results show that students understanding about physics knowledge production improved and their knowledge became more organized and coherent. It is shown that the flow charts and the didactical reconstructions behind them had an important role in gaining these positive learning results. On the basis of the results reported here, the designed learning tools have been adopted as a standard part of the teaching solutions used in the physics teacher education courses in the Department of Physics, University of Helsinki.

The Scientific Forum on the Gulf of Mexico: The Islands in the Stream Concept took place in January 2008 in Sarasota, Florida. The purpose of the meeting was to bring together scientists and managers from around the Gulf of Mexico to discuss a range of topics on our knowledge of the Gulf of Mexico, from its geology to larger-scale connectivity to the Caribbean region, and their applications to the concept of a more integrated approach to area-based management. The forum included six panels of invited experts who spoke on the oceanographic and biological features in the Gulf of Mexico, including connections with Mexico and the Mesoamerican barrier reef system, and the legal and regulatory structure currently in place. The charge to the group was to share information, identify gaps in our knowledge, identify additional potential areas for protection, and discuss available science about connectivity and the potential value of establishing a marine protected area network in the Gulf of Mexico. Numerous problems are frequently observed when nursing competency assessment systems (NCAS) are implemented. How to effectively implement a nursing competency assessment system, according to academic and practical contributions, is poorly reported in the literature. The purpose of this paper is to present a set of recommendations for public hospitals and nursing management in order to facilitate the implementation of a NCAS. To achieve this objective we have revised the existing literature and conducted a Delphi study with nursing managers and human resource managers of the public hospitals of the Basque Health Service. The results are that the implementation of a NCAS requires a well-planned strategy that managers must consider before implementing any NCAS. This strategy must include, at minimum, the following aspects: communication, training, leadership, and content where the NCAS is concerned. The context of the organisations and the cultural dimensions may also influence the results of the application of the system.

Este estudo teve como objetivos analisar as concepções teóricas e práticas docentes de enfermagem no cuidado à saúde da mulher a partir da ideia de integralidade e discutir as estratégias utilizadas pelos docentes para inserir o conteúdo da integralidade no ensino de enfermagem na saúde da mulher. A polÃtica atual de atenção integral à saúde da mulher propõe a incorporação do princÃpio da integralidade como eixo norteador que articule o mundo do ensino ao mundo do trabalho e da realidade social. Neste contexto, muitos sentidos se combinam e se conflitam na formação da ideia de integralidade no cuidado à saúde da mulher. A metodologia envolveu a abordagem qualitativa realizada nas Instituições Públicas de Ensino Superior do Estado do Rio de Janeiro que oferecem o Curso de Graduação em Enfermagem. Utilizou-se como técnica de coleta de dados a entrevista com dezessete docentes de enfermagem da área de saúde da mulher de acordo com os critérios de inclusão selecionados pela pesquisa. Da análise do material produzido surgiram quatro categorias, a saber: Concepções de integralidade no cuidado à saúde da mulher; Integralidade do cuidado no ensino da saúde da mulher; Estratégias utilizadas para inserir a integralidade no ensino de enfermagem na saúde da mulher; Dificuldades para implantar a integralidade no cuidado à saúde da mulher. Identificou-se que foram muitos os avanços do Sistema Único de Saúde na última década. Contudo, no que diz respeito à sua consolidação como sistema público de saúde, ainda estamos diante de grandes desafios, entre os quais se destaca o relativo à incorporação efetiva dos princÃpios e valores do SUS nos processos de trabalho, bem como nos processos formativos para a enfermagem na área da saúde da mulher. Desta maneira, faz-se necessário construir novas formas de trabalhar melhor com a assistência, perceber como efetivas as polÃticas publicas na área da saúde da mulher, considerando as necessidades e demandas locorregionais no paÃs. É evidente a dificuldade em seguir os princÃpios aqui defendidos, porém a integralidade no cuidado à saúde da mulher, só será possÃvel quando houver compromisso ético com as ações e relações necessárias para sua efetivação.

Este estudo trata dos processos de mudança na formação em saúde, especialmente das questões polÃtico estruturais da articulação do ensino e da extensão para a construção de estratégias de transformação dos cursos da área da saúde. Tem por objetivos analisar as perspectiva dos sujeitos envolvidos na transformação curricular no que diz respeito à articulação entre práticas de ensino, extensão universitária e saúde, a constituição de novos cenários de prática e sua relação com a extensão universitária, assim como examinar os sentidos atribuÃdos pelos sujeitos aos processos polÃticos internos e externos à s instituições de ensino. Parte-se da compreensão de que há em andamento um grande movimento de lutas pela mudança na formação em saúde e de algumas das ideias que têm sido discutidas e difundidas na defesa da reorientação da formação em saúde baseada em princÃpios e diretrizes consoantes com a polÃtica de saúde e com a defesa de direitos de cidadania. Baseia-se em concepções de espaço cotidiano das instituições de formação superior em saúde e dos serviços de saúde e outros cenários de prática. Reflete sobre a responsabilidade polÃtica das instituições formadoras e da extensão universitária como lugar de tornar públicas as posições e valores defendidos para as práticas do cuidado e da relação social e polÃtica da universidade. Para a operacionalização do estudo, foram escolhidas duas instituições universitárias que se apresentaram e que foram selecionadas no programa do Pro-Saúde. Foram realizadas entrevistas com roteiro semi-estruturado com protagonistas de processos de mudança na formação nestas instituições, em três profissões da área da saúde, enfermagem, medicina e odontologia. Fez-se analise hermenêutica a partir dos sentidos que foram atribuÃdos a estas experiências concretas pelos atores que as empreenderam. Definiu-se três categorias de análise, a dimensão das polÃticas governamentais de incentivo a mudanças na formação em saúde e os efeitos das mesmas para as instituições de ensino; a ampliação de cenários de prática, as estratégias de aproximação entre os mundos do ensino e do trabalho e se significaram novas práticas de cuidado; relação existente entre os processos de transformação curricular e a extensão universitária. A extensão vem adquirindo protagonismo na construção de dispositivos de enfrentamento e superação das dificuldades e resistências nos processos de transformação curricular, além de criar com relativa liberdade novas possibilidades espaciais e conceituais para o cuidado em saúde, porém pelos próprios sentidos que assume desde a invisibilidade até a redenção da universidade sua baixa institucionalidade e reflexão impede que sua potencialidade seja tomada como aliada nestes processos de modo mais sistemático e impactante.
