OBJECTIVE To analyze the direct medical costs of HIV/AIDS in Portugal from the perspective of the National Health Service. METHODS A retrospective analysis of medical records was conducted for 150 patients from five specialized centers in Portugal in 2008. Data on utilization of medical resources during 12 months and patients’ characteristics were collected. A unit cost was applied to each care component using official sources and accounting data from National Health Service hospitals. RESULTS The average cost of treatment was 14,277 €/patient/year. The main cost-driver was antiretroviral treatment (€ 9,598), followed by hospitalization costs (€ 1,323). Treatment costs increased with the severity of disease from € 11,901 (> 500 CD4 cells/µl) to € 23,351 (CD4 count ≤ 50 cells/ µl). Cost progression was mainly due to the increase in hospitalization costs, while antiretroviral treatment costs remained stable over disease stages. CONCLUSIONS The high burden related to antiretroviral treatment is counterbalanced by relatively low hospitalization costs, which, however, increase with severity of disease. The relatively modest progression of total costs highlights that alternative public health strategies that do not affect transmission of disease may only have a limited impact on expenditure, since treatment costs are largely dominated by constant antiretroviral treatment costs.

OBJECTIVE To analyze if the nutritional status of children aged less than five years is related to the biological conditions of their mothers, environmental and socioeconomic factors, and access to health services and social programs.METHODS This cross-sectional population-based study analyzed 664 mothers and 790 children using canonical correlation analysis. Dependent variables were characteristics of the children (weight/age, height/age, BMI/age, hemoglobin, and retinol serum levels). Independent variables were those related to the mothers’ nutritional status (BMI, hemoglobin, and retinol serum levels), age, environmental and socioeconomic factors and access to health service and social programs. A < 0.05 significance level was adopted to select the interpreted canonical functions (CF) and ± 0.40 as canonical load value of the analyzed variables.RESULTS Three canonical functions were selected, concentrating 89.9% of the variability of the relationship among the groups. In the first canonical function, weight/age (-0.73) and height/age (-0.99) of the children were directly related to the mother’s height (-0.82), prenatal appointments (-0.43), geographical area of the residence (-0.41), and household incomeper capita (-0.42). Inverse relationship between the variables related to the children and people/room (0.44) showed that the larger the number of people/room, the poorer their nutritional status. Rural residents were found to have the worse nutritional conditions. In the second canonical function, the BMI of the mother (-0.48) was related to BMI/age and retinol of the children, indicating that as women gained weight so did their children. Underweight women tended to have children with vitamin A deficiency. In the third canonical function, hemoglobin (-0.72) and retinol serum levels (-0.40) of the children were directly related to the mother’s hemoglobin levels (-0.43).CONCLUSIONS Mothers and children were associated concerning anemia, vitamin A deficiency and anthropometric markers. Living in rural areas is a determining factor for the families health status.

ABSTRACT OBJECTIVE To identify the factors that interfere with the access of adolescents and young people to childbirth care for in the Northeast region of Brazil. METHODS Cross-sectional study with 3,014 adolescents and young people admitted to the selected maternity wards to give birth in the Northeast region of Brazil. The sample design was probabilistic, in two stages: the first corresponded to the health establishments and the second to women who had recently given birth and their babies. The data was collected by means of interviews and consulting the hospital records, from pre-tested electronic form. Descriptive statistics were used for the univariate analysis, Pearson’s Chi-square test for the bivariate analysis and multiple logistic regressions for the multivariate analysis. Sociodemographic variables, obstetrical history, and birth care were analyzed. RESULTS Half of the adolescents and young people interviewed had not been given guidance on the location that they should go to when in labor, and among those who had, 23.5% did not give birth in the indicated health service. Furthermore, one third (33.3%) had to travel in search of assisted birth, and the majority (66.7%) of the postpartum women came to maternity by their own means. In the bivariate analysis, the variables marital status, paid work, health insurance, number of previous pregnancies, parity, city location, and type of health establishment showed a significant association (p < 0.20) with inadequate access to childbirth care. The multivariate analysis showed that married adolescents and young people (p < 0.015), with no health insurance (p < 0.002) and from the countryside (p < 0.001) were more likely to have inadequate access to childbirth care. CONCLUSIONS Adolescents and young women, married, without health insurance, and from the countryside are more likely to have inadequate access to birth care. The articulation between outpatient care and birth care can improve this access and, consequently, minimize the maternal and fetal risks that arise from a lack of systematic hospitalization planning.

A reforma dos cuidados de saúde primários (CSP), iniciada em 2005, visa melhorar o desempenho dos centros de saúde através da reorganização dos serviços em várias unidades funcionais, no sentido de resolver os problemas tendo em conta as necessidades a satisfazer, complementando-se entre si e assumindo compromissos de acessibilidade e qualidade nos cuidados de saúde prestados. Ao mesmo tempo, são criados órgãos de gestão e governação clínica que nunca antes existiram nos CSP, envolvendo a participação da comunidade. A optimização da gestão e da governação clínica permitiu organizar os serviços de saúde em Agrupamentos de Centros de Saúde (AGES), dando-lhes poderes e responsabilidades para solucionarem problemas e tomarem as decisões acertadas e céleres, já que conhecem melhor as necessidades de saúde das populações. As relações burocráticas são substituídas por relações de contratualidade, orientadas para obter melhores resultados em saúde. Partindo destes pressupostos, o estudo realizado pretende analisar a percepção de profissionais de saúde quanto à política de humanização dos CSP, bem como, identificar/construir indicadores que avaliem essa política, sendo um estudo de carácter exploratório e descritivo, à luz de uma abordagem qualitativa. Participaram neste estudo cinco profissionais de saúde da Administração Regional de Saúde (ARS) do Norte, lP, do Departamento de Contratualização, Departamento de Estudos e Planeamento da ARS Norte e do AGES Tâmega 11 - Vale Sousa Sul, seleccionados por conveniência e inquiridos por entrevista semi-estruturada. Os dados foram tratados através da análise de conteúdo com o apoio informático NVivo9. Os resultados apresentados, com base nas entrevistas realizadas aos participantes no estudo, sustentam que os actuais indicadores quantitativos contratualizados com as unidades funcionais, expressam a política de humanização num serviço de saúde, não coincidindo totalmente com as definições internacionais expressas neste estudo.

The Janssen-Cilag proposal for a risk-sharing agreement regarding bortezomib received a welcome signal from NICE. The Office of Fair Trading report included risk-sharing agreements as an available tool for the National Health Service. Nonetheless, recent discussions have somewhat neglected the economic fundamentals underlying risk-sharing agreements. We argue here that risk-sharing agreements, although attractive due to the principle of paying by results, also entail risks. Too many patients may be put under treatment even with a low success probability. Prices are likely to be adjusted upward, in anticipation of future risk-sharing agreements between the pharmaceutical company and the third-party payer. An available instrument is a verification cost per patient treated, which allows obtaining the first-best allocation of patients to the new treatment, under the risk sharing agreement. Overall, the welfare effects of risk-sharing agreements are ambiguous, and care must be taken with their use.

A afluência desmedida aos Serviços de Urgência é uma questão que acarreta preocupações a nível financeiro. Contribui para este fato, a mentalidade da população, que acredita que este serviço oferece mais facilidades de acesso, dispõe de mais recursos e fornece melhores cuidados de saúde. Novas medidas foram preconizadas, como o aumento das taxas moderadoras, para tentar travar este fenómeno. No entanto, apesar da descida dos episódios de urgência em cerca de 10%, em Portugal, estudos apontam para valores na ordem dos 30-35% de episódios não urgentes. Assim, torna-se importante que não só se enfatizem as novas medidas, como se eduque a população com vista à correta utilização destes serviços, através de campanhas de sensibilização. Torna-se, assim, necessário que se chegue ao perfil do utilizador abusivo. Para a identificação de um perfil de abusividade, foram solicitados dados de episódios de urgência ocorridos durante um período de 6 meses no Hospital de São João, tendo depois sido estimado um modelo de regressão logística. A metodologia permite identificar quais as características que influenciam uma utilização abusiva do serviço e quantificar o impacto de cada uma destas características na probabilidade de um utente apresentar um comportamento abusivo. Concluiu-se que, uma mulher entre os 18-30 anos, que resida em Vila Nova de Gaia, recorra à urgência durante a noite tendo-lhe sido atribuída uma pulseira azul e seja abrangida pelo Serviço Nacional de Saúde, apresenta 91,92% de probabilidade de utilizar este serviço de forma abusiva. Contrariamente, um homem com mais de 60 anos, residente na Maia, que recorra ao serviço durante o dia, esteja isento do pagamento de taxas moderadoras e seja abrangido pela ADSE, e lhe seja atribuída uma pulseira laranja, apresenta apenas 39,93% de probabilidade de ter um comportamento abusivo. Estes resultados são importantes para definir campanhas de sensibilização que diminuam comportamentos abusivos.

A diabetes é a doença do seculo XXI, atinge mais de um milhão de portugueses cada vez mais jovens em idades trabalhadoras, e já custa mais de 1% do PIB, além dos enormes danos que pode causar às pessoas e à sociedade. Segundo o Relatório Anual do Observatório Nacional da Diabetes, em 2011, cerca de um quarto da população portuguesa integrada no escalão etário dos 60 aos 79 tem Diabetes. Um estudo realizado pelo Lisbon Internet and Networks Institute (LINI), em 2010, diz-nos que “a internet é um recurso para a informação e educação para a saúde”. Em 2010 49% dos lares portugueses possuem acesso à internet e 44% da população é utilizadora (acréscimo significativo relativamente aos 29% em 2003). Destes, dois terços têm entre os 15 e os 24 anos. Os idosos representam 1,6%, dos quais os reformados e pensionistas representam 5%, as domésticas representam 11% e os trabalhadores manuais 22%, sendo estes os menos utilizadores. Um terço da restante população procura informação sobre saúde semanalmente, cerca de 16%. Face à atual conjuntura económica, com restrições orçamentais, nomeadamente na área da saúde, devem-se encontrar meios para prevenir e lidar com a Diabetes numa perspetiva de custo-benefício, isto porque a Diabetes pode reduzir as oportunidades de emprego e de aprendizagem. O atual diretor da Organização para a Cooperação e Desenvolvimento Económico (OCDE) Yyes Leterme diz que “Prevenir e tratar a Diabetes e as suas complicações custa cerca de 90 mil milhões de euros anualmente na Europa”. Atualmente, não é muito utilizado nem explorado o potencial das tecnologias de informação e as ferramentas web ao serviço da saúde, quer por profissionais de saúde, quer por utentes na gestão ao regime terapêutico na doença crónica, mais precisamente na gestão adequada da Diabetes. Potenciar uma visão integrada dos diferentes recursos de comunicação e a sua utilização conjugada com a promoção da saúde e prevenção da doença poderá enfatizar em termos de eficácia e eficiência a minimização de recursos das organizações de saúde e promover a gestão adequada da Diabetes. Tendo por base esta problemática, este estudo pretende abordar e refletir o possível contributo das tecnologias de informação e das ferramentas web na gestão adequada ao regime terapêutico da Diabetes.

Objective Patient-centredness has become an important aspect of health service delivery; however, there are a limited number of studies that focus on this concept in the domain of hearing healthcare. The objective of this study was to examine and compare audiologists’ preferences for patient-centredness in Portugal, India and Iran. Design The study used a cross-sectional survey design with audiologists recruited from three different countries. Participants A total of 191 fully-completed responses were included in the analysis (55 from Portugal, 78 from India and 58 from Iran). Main outcome measure The Patient–Practitioner Orientation Scale (PPOS). Results PPOS mean scores suggest that audiologists have a preference for patient-centredness (ie, mean of 3.6 in a 5-point scale). However, marked differences were observed between specific PPOS items suggesting these preferences vary across clinical situations. A significant level of difference (p<0.001) was found between audiologists’ preferences for patient-centredness in three countries. Audiologists in Portugal had a greater preference for patient-centredness when compared to audiologists in India and Iran, although no significant differences were found in terms of age and duration of experience among these sample populations. Conclusions There are differences and similarities in audiologists’ preferences for patient-centredness among countries. These findings may have implications for the training of professionals and also for clinical practice in terms of optimising hearing healthcare across countries.

RESUMO - O Inquérito Nacional de Saúde é um instrumento de observação em saúde promovido pelo Ministério da Saúde desde 1987, em resposta às necessidades crescentes de informação de saúde ligadas ao estabelecimento e às exigências de planeamento e avaliação decorrentes da instituição do Serviço Nacional de Saúde. O Inquérito Nacional de Saúde conta, até agora, com quatro edições. Os dados provêm através do estudo transversal de amostras probabilísticas da população portuguesa, através de entrevistas no domicílio, utilizando instrumentos e métodos válidos e estáveis. Os indicadores gerados descrevem o estado de saúde, utilização de cuidados de saúde e determinantes de saúde da população portuguesa residente em unidades de alojamento familiar e tem representatividade a nível nacional e regional. Além de útil para a investigação dos padrões e tendências na área da saúde da população, esta informação é importante como elemento de planeamento, na identificação de necessidades em saúde e na avaliação dos eventuais efeitos e impactes das intervenções realizadas na saúde da população. A evolução do Inquérito Nacional de Saúde depende da definição das necessidades de informação epidemiológica por parte do Estado e da sua integração no sistema europeu de inquéritos de saúde. --------------------------ABSTRACT – The National Health Interview Survey is an instrument of observation in health promoted by the Ministry of Health since 1987, in response to the growing demand for health information, derived from the institution of the National Health Service, as well as its planning and evaluation activities. The National Health Interview Survey has had four editions so far. Data are obtained through the cross-sectional study of probabilistic samples of the Portuguese population, through interviews at home, using valid and stable instruments and methods. Obtained indicators describe the state of health, use of health care and health determinants of the Portuguese population living in familiar dwellings, and are representative at national and regional levels. Besides its usefulness for the study of patterns and trends, this information is important for planning purposes, in the identification of health needs and for the evaluation of the eventual effects of the planned health interventions. The future of the National Health Interview Survey depends on the State’s definition of the needs of epidemiological information and also on its participation in the European system of health surveys. RESUMO - Através da investigação teórica analisa-se o processo de implementação da contratualização em Portugal e procura- -se perspectivar o seu desenvolvimento. Questionam-se ainda os factores críticos que no contexto português condicionam a implementação de políticas de mudança no sector da saúde e descortina-se o desenvolvimento deste importante instrumento de mudança à luz dos ensinamentos obtidos, quer na revisão bibliográfica quer nos casos Inglês e Espanhol. --------------------------ABSTRACT – Through a theoretical research, the implementation of the contracting process in Portugal is analysed and its development is envisioned. Conflicts of interest were potentially great but they were minimized by the great conviction from both Doctors and Health Ministry that something had to be done to improve data on perinatal health. To decrease the number of hospitals where deliveries took place, to concentrate doctors, nurses and equipment, to define staff and to acquire equipment and to train nurses and paediatricians was the way. One the point of view of cost-effectiveness, centralization of expensive technologies, and development of expertise concentrating cases in a same centre - Surgery, VLBW, etc- and lowering mortality rates and get better outcomes were clear health gains. In 1989 after the political decision of closing small maternities the committee return to villages and cities to explain to political local power and people, the decision, which kind of care they will have in the future, why and expected gains. Level I hospitals and Health Centers stop to have deliveries; Health Centers were given a great responsibility: the follow up of the most part of the normal pregnancies by GP. There was no economic pressure because the National Health Service is free, there are no economic incentives for obstetrical or neonatal care, hospitals are financed through ICD, hospital level is defined according to both delivery and newborn care. In 1989 the rule was “No results can be obtained without the interested and responsible participation of all – institutions and people”. At that time the emphasis was on training. There are geographic influences on regionalization for example for islands and inner and far geographic areas. Also we would like to emphasize the influence of demographics on regionalization. As birth rate continues to decrease the hospitals left open 20 years ago with more than 1500 deliveries have to be closed now because the number of deliveries decreased. It was much more difficult and unacceptable to close some few maternities now than 20 years ago. All the difference was that at that time reasons were explained and now it was a Minister order. Other fearful events are the opening of private hospitals, the lowering gross national income, the economic difficulties and financial problems.

Toxocariasis is a worldwide public-health problem that poses major risks to children who may accidentally ingest embryonated eggs of Toxocara. The objectives of this study were to investigate the occurrence of anti-Toxocara spp. antibodies in children and adolescents and the variables that may be involved, as well as environmental contamination by Toxocara spp. eggs, in urban recreation areas of north central mesoregion, Paraná State, Brazil. From June 2005 to March 2007. a total of 376 blood samples were collected by the Public Health Service from children and adolescents one to 12 years old, of both genders. Samples were analyzed by the indirect ELISA method for detection of anti-Toxocara antibodies. Serum samples were previously absorbed with Ascaris suum antigens, and considered positive with a reagent reactivity index >1. Soil samples from all of the public squares and schools located in the four evaluated municipalities that had sand surfaces (n = 19) or lawns (n = 15) were analyzed. Of the 376 serum samples, 194 (51.6%) were positive. The seroprevalence rate was substantially higher among children aging one to five years (p = 0.001) and six to eight years (p = 0.022). The clinical signs and symptoms investigated did not show a statistical difference between seropositive and seronegative individuals (p > 0.05). In 76.5% of the investigated recreation places, eggs of Toxocara were detected in at least one of the five collected samples. Recreation areas from public schools were 2.8 times more contaminated than from public squares. It is important to institute educational programs to inform families and educators, as well as to improve sanitary control of animals and cleaning of the areas intended for recreation in order to prevent toxocariasis.

Foi objectivo deste trabalho obter uma panorâmica sobre amputações de membros efectuadas em Portugal nos Hospitais do Serviço Nacional de Saúde num período de quatro anos (1990 a 1993), analisando-se a sua distribuição por etiologias e sexo, idade média, demora média de internamento, nível de amputação e taxa de mortalidade. Os autores destacam ainda os níveis de amputação mais favoráveis para uma reabilitação eficaz.
