Clostridium perfringens est ubiquitaire dans l’environnement. Ce microorganisme peut être retrouvé dans la flore normale du tractus gastro-intestinal des mammifères et peut également causer une variété d’infections intestinales. Le phénotype de résistance à la bacitracine a déjà été rapporté chez C. perfringens mais les gènes associés n’ont pas été caractérisés. Dans cette étude, 24 des 99 isolats de C. perfringens aviaires testés ont démontré une résistance à la bacitracine. Les analyses ont révélé la présence d’un transporteur ABC ainsi que d’une undécaprénol kinase surproduite. Ces deux mécanismes semblent être codés par l’opéron bcrABDR. En amont et en aval des gènes bcr, un élément IS1216-like a été identifié, celui-ci pouvant jouer un rôle dans la dissémination de la résistance à la bacitracine. Des analyses d’hybridation sur ADN ont révélé que les gènes bcrABDR étaient localisés sur le chromosome. De plus, il a été démontré que les gènes bcr étaient exprimés en présence de bacitracine. Plusieurs études ont associé la tolérance aux antibiotiques et aux désinfectants à la formation de biofilm. Dans la littérature, peu d’informations sont disponibles sur le biofilm de C. perfringens. La majorité des isolats testés dans cette étude ont démontré la formation d’un biofilm. L’analyse de la matrice a démontré que celle-ci contenait des protéines, de l’ADN extracellulaire ainsi que des polysaccharides liés en bêta-1,4. Une meilleure survie des cellules en biofilm a été observée suite à une exposition à de fortes concentrations d’antibiotiques. Une exposition à de faibles doses de certains antibiotiques semblait diminuer le biofilm formé alors que pour d’autres, le biofilm semblait augmenter. Dans la présente étude, la susceptibilité des biofilms de C. perfringens à la désinfection a été également analysée. Les résultats ont démontré que la formation de biofilm protégeait les cellules de l’action du monopersulfate de potassium, des ammoniums quaternaires, du peroxyde d’hydrogène et du glutéraldéhyde. Toutefois, l’hypochlorite de sodium a été démontré comme étant efficace contre le biofilm de C. perfringens. Il a été démontré que les biofilms mixtes de C. perfringens cultivés en présence de Staphylococcus aureus ou d’Escherichia coli étaient plus résistants à la désinfection en comparaison aux biofilms simples de S. aureus ou d’E. coli. Toutefois, le biofilm simple de C. perfringens était plus résistant à la désinfection que les biofilms mixtes. Finalement, les profils de transcription entre les populations planctoniques et en biofilm ont été analysés par séquençage d’ARN. L’analyse transcriptomique du biofilm a identifié 238 gènes différentiellement exprimés entre les deux conditions. Les gènes négativement régulés sont impliqués dans la virulence, la production d’énergie, le métabolisme des sucres ainsi que dans la biosynthèse des acides gras et des acides aminés alors que les gènes induits sont impliqués dans la réponse au stress et au stress oxydatif, dans la biosynthèse d’acides gras et de phospholipides ainsi que dans la virulence. Cette étude décrit pour la première fois la découverte des gènes associés à la résistance à la bacitracine chez C. perfringens. Elle rapporte également de nouvelles données sur la matrice du biofilm, la tolérance aux antibiotiques et aux désinfectants ainsi que sur le transcriptome du biofilm de C. perfringens.

Poor bioavailability and poor pharmacokinetic characteristics are some of the leading causes of drug development failure. Therefore, poorly-soluble drugs, fragile proteins or nucleic acid products may benefit from their encapsulation in nanosized vehicles, providing enhanced solubilisation, protection against degradation, and increased access to pathological compartments. A key element for the success of drug-loaded nanocarriers (NC) is their ability to either cross biological barriers themselves or allow loaded drugs to traverse them to achieve optimal pharmacological action at pathological sites. Depending on the mode of administration, NC may have to cross different physiological barriers in their journey towards their target. In this review, the crossing of biological barriers by passive targeting strategies will be presented for intravenous delivery (vascular endothelial lining, particularly for tumour vasculature and blood-brain barrier targeting), oral administration (gastrointestinal lining) and upper airway administration (pulmonary epithelium). For each specific barrier, background information will be provided on the structure and biology of the tissues involved as well as available pathways for nano-objects or loaded drugs (diffusion and convection through fenestration, transcytosis, tight junction crossing, etc.). The determinants of passive targeting − size, shape, surface chemistry, surface patterning of nanovectors − will be discussed in light of current results. Perspectives on each mode of administration will be presented. The focus will be on polymeric nanoparticles and dendrimers although advances in liposome technology will be also reported as they represent the largest body in the drug delivery literature.

Polycyclic Aromatic Hydrocarbons and other toxic compounds from industrial effluents are noted for their high potency for skin, lung, bladder and gastrointestinal cancers. Increased industrialization, and population growth led to greater production of wastes, Pesticides and PAHs have received attention due to their carcinogenic effects. The main objectives of the study were; to collect base line data on the concentration of PAHs in seawater and sediment from the west coast of India, the concentration of PAHs in certain species of fishes, the comparative levels of PAHs in fish, the influence of sediment characteristics on the concentration of PAH in sediment, changes in PAH concentration in water, sediment and fish, to provide a base line concentration of trace metals in water, sediment and fish, the seasonal changes in content of selected trace metals in water, sediment and fish from the west coast of India. The present study revealed that a predominance of silt and clay at all stations in the off Cochin area. The study has provided comprehensive information available to date for PAHs in seawater, sediment and fishes from the west cost of India especially from the Quilon to Mangalore region.

Contamination of environmental water by pathogenic microorganisms and subsequent infections originated from such sources during different contact and non- contact recreational activities are a major public health problem worldwide particularly in developing countries. The main pathogen frequently associated with enteric infection in developing countries are Salmonella enterica serovar typhi and paratyphi. Although the natural habitat of Salmonella is the gastrointestinal tract of animals, it find its way into natural water through faecal contamination and are frequently identified from various aquatic environments (Baudart et al., 2000; Dionisio et al., 2000; Martinez -Urtaza et al., 2004., Abhirosh et al., 2008). Typhoid fever caused by S. enterica serotype typhi and paratyphi are a common infectious disease occurring in all the parts of the world with its highest endemicity in certain parts of Asia, Africa, Latin America and in the Indian subcontinent with an estimated incidence of 33 million cases each year with significant morbidity and mortality (Threlfall, 2002). In most cases the disease is transmitted by polluted water (Girard et al., 2006) because of the poor hygienic conditions, inadequate clean water supplies and sewage treatment facilities. However in developed countries the disease is mainly associated with food (Bell et al., 2002) especially shellfish (Heinitz et al., 2000

The primary habitat of Salmonella is the gastrointestinal tract of animals and they are discharged into the water bodies through the feces. Aquatic animals act as asymptomatic reservoirs of a wide range of Salmonella serotypes. The inevitable delay in the detection of Salmonella contamination and the low sensitivity of the conventional methods is a serious issue in the seafood industry. Due to the indiscriminate use, the antibiotics are finally accumulated in the aquatic environment which provides the required antibiotic stress for the emergence of more and more antibiotic resistant phenotypes ofSalmonella. Several genetic determinants like integrons, genomic islands etc. play their role in acquisition and reshuffling of antibiotic resistance genes. A large number of virulence determinants are required for Salmonella pathogenicity. The virulence potential of Salmonella is determined, to some extent, by the presence of phages or phage mediated genes in the bacterial genome. There is much intra-serotype polymorphism in Salmonella and epidemiological studies rely on genetic resemblance of the isolated strains. Proper identification of the strain employing the traditional and molecular techniques is a prerequisite for accurate epidemiological studies (Soto et al., 2000). In the course of the ‘Livestock Revolution’, extension and intensification of, among others, ruminant livestock production systems are current phenomena, with all their positive and negative side effects. Manure, one of the inevitable secondary products of livestock rearing, is a valuable source of plant nutrients and its skillful recycling to the soil-plant interface is essential for soil fertility, nutrient - and especially phosphorus - uses efficiency and the preservation or re-establishment of environmentally sustainable farming systems, for which organic farming systems are exemplarily. Against this background, the PhD research project presented here, which was embedded in the DFG-funded Research Training Group 1397 ‘Regulation of soil organic matter and nutrient turnover in organic agriculture ’ investigated possibilities to manipulate the diets of water buffalo (Bubalus bubalis L.) so as to produce manure of desired quality for organic vegetable production, without affecting the productivity of the animals used. Consisting of two major parts, the first study (chapter 2) tested the effects of diets differing in their ratios of carbon (C) to nitrogen (N) and of structural to non-structural carbohydrates on the quality of buffalo manure under subtropical conditions in Sohar, Sultanate of Oman. To this end, two trials were conducted with twelve water buffalo heifers each, using a full Latin Square design. One control and four tests diets were examined during three subsequent 7 day experimental periods preceded each by 21 days adaptation. Diets consisted of varying proportions of Rhodes grass hay, soybean meal, wheat bran, maize, dates, and a commercial concentrate to achieve a (1) high C/N and high NDF (neutral detergent fibre)/SC (soluble carbohydrate) ratio (HH), (2) low C/N and low NDF/SC ratio (LL); (3) high C/N and low NDF/SC ratio (HL) and (4) low C/N and high NDF/SC (LH) ratio. Effects of these diets, which were offered at 1.45 times maintenance requirements of metabolizable energy, and of individual diet characteristics, respectively, on the amount and quality of faeces excreted were determined and statistically analysed. The faeces produced from diets HH and LL were further tested in a companion PhD study (Mr. K. Siegfried) concerning their nutrient release in field experiments with radish and cabbage. The second study (chapter 3) focused on the effects of the above-described experimental diets on the rate of passage of feed particles through the gastrointestinal tract of four randomly chosen animals per treatment. To this end, an oral pulse dose of 683 mg fibre particles per kg live weight marked with Ytterbium (Yb; 14.5 mg Yb g-1 organic matter) was dosed at the start of the 7 day experimental period which followed 21 days of adaptation. During the first two days a sample for Yb determination was kept from each faecal excretion, during days 3 – 7 faecal samples were kept from the first morning and the first evening defecation only. Particle passage was modelled using a one-compartment age-dependent Gamma-2 model. In both studies individual feed intake and faecal excretion were quantified throughout the experimental periods and representative samples of feeds and faeces were subjected to proximate analysis following standard protocols. In the first study the organic matter (OM) intake and excretion of LL and LH buffaloes were significantly lower than of HH and HL animals, respectively. Digestibility of N was highest in LH (88%) and lowest in HH (74%). While NDF digestibility was also highest in LH (85%) it was lowest in LL (78%). Faecal N concentration was positively correlated (P≤0.001) with N intake, and was significantly higher in faeces excreted by LL than by HH animals. Concentrations of fibre and starch in faecal OM were positively affected by the respective dietary concentrations, with NDF being highest in HH (77%) and lowest in LL (63%). The faecal C/N ratio was positively related (P≤0.001) to NDF intake; C/N ratios were 12 and 7 for HH and LL (P≤0.001), while values for HL and LH were 11.5 and 10.6 (P>0.05). The results from the second study showed that dietary N concentration was positively affecting faecal N concentration (P≤0.001), while there was a negative correlation with the faecal concentration of NDF (P≤0.05) and the faecal ratios of NDF/N and C/N (P≤0.001). Particle passage through the mixing compartment was lower (P≤0.05) for HL (0.033 h-1) than for LL (0.043 h-1) animals, while values of 0.034 h-1 and 0.038 h-1 were obtained for groups LH and HH. At 55.4 h, total tract mean retention time was significantly (P≤0.05) lower in group LL that in all other groups where these values varied between 71 h (HH) and 79 h (HL); this was probably due to the high dietary N concentration of diet LL which was negatively correlated with time of first marker appearance in faeces (r= 0.84, P≤0.001), while the dietary C concentration was negatively correlated with particle passage through the mixing compartment (r= 0.57, P≤0.05). The results suggest that manure quality of river buffalo heifers can be considerably influenced by diet composition. Despite the reportedly high fibre digestion capacity of buffalo, digestive processes did not suppress the expression of diet characteristics in the faeces. This is important when aiming at producing a specific manure quality for fertilization purposes in (organic) crop cultivation. Although there was a strong correlation between the ingestion and the faecal excretion of nitrogen, the correlation between diet and faecal C/N ratio was weak. To impact on manure mineralization, the dietary NDF and N concentrations seem to be the key control points, but modulating effects are achieved by the inclusion of starch into the diet. Within the boundaries defined by the animals’ metabolic and (re)productive requirements for energy and nutrients, diet formulation may thus take into account the abiotically and biotically determined manure turnover processes in the soil and the nutrient requirements of the crops to which the manure is applied, so as to increase nutrient use efficiency along the continuum of the feed, the animal, the soil and the crop in (organic) farming systems.

The regional population growth in West Africa, and especially its urban centers, will bring about new and critical challenges for urban development policy, especially in terms of ensuring food security and providing employment for the growing population. (Peri-) urban livestock and vegetable production systems, which can contribute significantly to these endeavours, are limited by various constraints, amongst them limited access to expensive production factors and their (in)efficient use. To achieve sustainable production systems with low consumer health risks, that can meet the urban increased demand, this doctoral thesis determined nutrient use efficiencies in representative (peri-) urban livestock production systems in three West African cities, and investigated potential health risks for consumers ensuing from there. The field study, which was conducted during July 2007 to December 2009, undertook a comparative analysis of (peri-) urban livestock production strategies across 210 livestock keeping households (HH) in the three West African cities of Kano/Nigeria (84 HH), Bobo Dioulasso/Burkina Faso (63 HH) and Sikasso/Mali (63 HH). These livestock enterprises were belonging to the following three farm types: commercial gardening plus field crops and livestock (cGCL; 88 HH), commercial livestock plus subsistence field cropping (cLsC; 109 HH) and commercial gardening plus semi-commercial livestock (cGscL; 13 HH) which had been classified in a preceding study; they represented the diversity of (peri-) urban livestock production systems in West Africa. In the study on the efficiency of ruminant livestock production, lactating cowsand sheep herd units were differentiated based on whether feed supplements were offered to the animals at the homestead (Go: grazing only; Gsf: mainly grazing plus some supplement feeding). Inflows and outflows of nutrients were quantified in these herds during 18 months, and the effects of seasonal variations in nutrient availability on animals’ productivity and reproductive performance was determined in Sikasso. To assess the safety of animal products and vegetables, contamination sources of irrigated lettuce and milk with microbiological contaminants, and of tomato and cabbage with pesticide residues in (peri-) urban agriculture systems of Bobo Dioulasso and Sikasso were characterized at three occasions in 2009. Samples of irrigation water, organic fertilizer and ix lettuce were collected in 6 gardens, and samples of cabbage and tomato in 12 gardens; raw and curdled milk were sampled in 6 dairy herds. Information on health risks for consumers of such foodstuffs was obtained from 11 health centers in Sikasso. In (peri-) urban livestock production systems, sheep and goats dominated (P<0.001) in Kano compared to Bobo Dioulasso and Sikasso, while cattle and poultry were more frequent (P<0.001) in Bobo Dioulasso and Sikasso than in Kano. Across cities, ruminant feeding relied on grazing and homestead supplementation with fresh grasses, crop residues, cereal brans and cotton seed cake; cereal grains and brans were the major ingredients of poultry feeds. There was little association of gardens and livestock; likewise field cropping and livestock were rarely integrated. No relation existed between the education of the HH head and the adoption of improved management practices (P>0.05), but the proportion of HH heads with a long-term experience in (peri-) urban agriculture was higher in Kano and in Bobo Dioulasso than in Sikasso (P<0.001). Cattle and sheep fetched highest market prices in Kano; unit prices for goats and chicken were highest in Sikasso. Animal inflow, outflow and dairy herd growth rates were significantly higher (P<0.05) in the Gsf than in the Go cattle herds. Maize bran and cottonseed expeller were the main feeds offered to Gsf cows as dry-season supplement, while Gsf sheep received maize bran, fresh grasses and cowpea pods. The short periodic transhumance of Go dairy cows help them maintaining their live weight, whereas Gsf cows lost weight during the dry season despite supplement feeding at a rate of 1506 g dry matter per cow and day, resulting in low productivity and reproductive performance. The daily live weight gains of calves and lambs, respectively, were low and not significantly different between the Go and the Gsf system. However, the average live weight gains of lambs were significantly higher in the dry season (P<0.05) than in the rainy season because of the high pressure of gastrointestinal parasites and of Trypanosoma sp. In consequence, 47% of the sheep leaving the Go and Gsf herds died due to diseases during the study period. Thermo-tolerant coliforms and Escherichia coli contamination levels of irrigation water significantly exceeded WHO recommendations for the unrestricted irrigation of vegetables consumed raw. Microbial contamination levels of lettuce at the farm gate and the market place in Bobo Dioulasso and at the farm gate in Sikasso were higher than at the market place in Sikasso (P<0.05). Pesticide residues were detected in only one cabbage and one tomato sample and were below the maximum residue limit for consumption. Counts of thermo-tolerant coliforms and Escherichia coli were higher in curdled than in raw milk (P<0.05). From 2006 to x 2009, cases of diarrhea/vomiting and typhoid fever had increased by 11% and 48%, respectively, in Sikasso. For ensuring economically successful and ecologically viable (peri-) urban livestock husbandry and food safety of (peri-) urban foodstuffs of animal and plant origin, the dissemination and adoption of improved feeding practices, livestock healthcare and dung management are key. In addition, measures fostering the safety of animal products and vegetables including the appropriate use of wastewater in (peri-) urban agriculture, restriction to approve vegetable pesticides and the respect of their latency periods, and passing and enforcement of safety laws is required. Finally, the incorporation of environmentally sound (peri-) urban agriculture in urban planning by policy makers, public and private extension agencies and the urban farmers themselves is of utmost importance. To enable an efficient (peri-) urban livestock production in the future, research should concentrate on cost-effective feeding systems that allow meeting the animals’ requirement for production and reproduction. Thereby focus should be laid on the use of crop-residues and leguminous forages. The improvement of the milk production potential through crossbreeding of local cattle breeds with exotic breeds known for their high milk yield might be an accompanying option, but it needs careful supervision to prevent the loss of the local trypanotolerant purebreds.

With Chinas rapid economic development during the last decades, the national demand for livestock products has quadrupled within the last 20 years. Most of that increase in demand has been answered by subsidized industrialized production systems, while million of smallholders, which still provide the larger share of livestock products in the country, have been neglected. Fostering those systems would help China to lower its strong urban migration streams, enhance the livelihood of poorer rural population and provide environmentally save livestock products which have a good chance to satisfy customers demand for ecological food. Despite their importance, China’s smallholder livestock keepers have not yet gained appropriate attention from governmental authorities and researchers. However, profound analysis of those systems is required so that adequate support can lead to a better resource utilization and productivity in the sector. To this aim, this pilot study analyzes smallholder livestock production systems in Xishuangbanna, located in southern China. The area is bordered by Lao and Myanmar and geographically counts as tropical region. Its climate is characterized by dry and temperate winters and hot summers with monsoon rains from May to October. While the regionis plain, at about 500 m asl above sea level in the south, outliers of the Himalaya mountains reach out into the north of Xishuangbanna, where the highest peak reaches 2400 m asl. Except of one larger city, Jinghong, Xishuangbanna mainly is covered by tropical rainforest, areas under agricultural cultivation and villages. The major income is generated through inner-Chinese tourism and agricultural production. Intensive rubber plantations are distinctive for the lowland plains while small-scaled traditional farms are scattered in the mountane regions. In order to determine the current state and possible future chances of smallholder livestock production in that region, this study analyzed the current status of the smallholder livestock sector in the Naban River National Nature Reserve (NRNNR), an area which is largely representative for the whole prefecture. It covers an area of about 50square kilometer and reaches from 470 up to 2400 m asl. About 5500 habitants of different ethnic origin are situated in 24 villages. All data have been collected between October 2007 and May 2010. Three major objectives have been addressed in the study: 1. Classifying existing pig production systems and exploring respective pathways for development 2. Quantifying the performance of pig breeding systemsto identify bottlenecks for production 3. Analyzing past and current buffalo utilization to determine the chances and opportunities of buffalo keeping in the future In order to classify the different pig production s ystems, a baseline survey (n=204, stratified cluster sampling) was carried out to gain data about livestock species, numbers, management practices, cultivated plant species and field sizes as well associo-economic characteristics. Sampling included two clusters at village level (altitude, ethnic affiliation), resulting in 13 clusters of which 13-17 farms were interviewed respectively. Categorical Principal Component Analysis (CatPCA) and a two-step clustering algorithm have been applied to identify determining farm characteristics and assort recorded households into classes of livestock production types. The variables keep_sow_yes/no, TLU_pig, TLU_buffalo, size_of_corn_fields, altitude_class, size_of_tea_plantationand size_of_rubber_fieldhave been found to be major determinants for the characterization of the recorded farms. All farms have extensive or semi-intensive livestock production, pigs and buffaloes are predominant livestock species while chicken and aquaculture are available but play subordinate roles for livelihoods. All pig raisers rely on a single local breed, which is known as Small Ear Pig (SMEP) in the region. Three major production systemshave been identified: Livestock-corn based LB; 41%), rubber based (RB; 39%) and pig based (PB;20%) systems. RB farms earn high income from rubber and fatten 1.9 ±1.80 pigs per household (HH), often using purchased pig feed at markets. PB farms own similar sized rubber plantations and raise 4.7 ±2.77 pigs per HH, with fodder mainly being cultivated and collected in theforest. LB farms grow corn, rice and tea and keep 4.6 ±3.32 pigs per HH, also fed with collected and cultivated fodder. Only 29% of all pigs were marketed (LB: 20%; RB: 42%; PB: 25%), average annual mortality was 4.0 ±4.52 pigs per farm (LB: 4.6 ±3.68; RB: 1.9 ±2.14; PB: 7.1 ±10.82). Pig feed mainly consists of banana pseudo stem, corn and rice hives and is prepared in batches about two to three times per week. Such fodder might be sufficient in energy content but lacks appropriate content of protein. Pigs therefore suffer from malnutrition, which becomes most critical in the time before harvest season around October. Farmers reported high occurrences of gastrointestinal parasites in carcasses and often pig stables were wet and filled with manure. Deficits in nutritional and hygienic management are major limits for development and should be the first issues addressed to improve productivity. SME pork was found to be known and referred by local customers in town and by richer lowland farmers. However, high prices and lacking availability of SME pork at local wet-markets were the reasons which limited purchase. If major management constraints are overcome, pig breeders (PB and LB farms) could increase the share of marketed pigs for town markets and provide fatteners to richer RB farmers. RB farmers are interested in fattening pigs for home consumption but do not show any motivation for commercial pig raising. To determine the productivity of input factors in pig production, eproductive performance, feed quality and quantity as well as weight development of pigs under current management were recorded. The data collection included a progeny history survey covering 184 sows and 437 farrows, bi-weekly weighing of 114 pigs during a 16-months time-span on 21 farms (10 LB and 11 PB) as well as the daily recording of feed quality and quantity given to a defined number of pigs on the same 21 farms. Feed samples of all recorded ingredients were analyzed for their respective nutrient content. Since no literature values on thedigestibility of banana pseudo stem – which is a major ingredient of traditional pig feed in NRNNR – were found, a cross-sectional digestibility trial with 2x4 pigs has been conducted on a station in the research area. With the aid of PRY Herd Life Model, all data have been utilized to determine thesystems’ current (Status Quo = SQ) output and the productivity of the input factor “feed” in terms of saleable life weight per kg DM feed intake and monetary value of output per kg DM feed intake.Two improvement scenarios were simulated, assuming 1) that farmers adopt a culling managementthat generates the highest output per unit input (Scenario 1; SC I) and 2) that through improved feeding, selected parameters of reproduction are improved by 30% (SC II). Daily weight gain averaged 55 ± 56 g per day between day 200 and 600. The average feed energy content of traditional feed mix was 14.92 MJ ME. Age at first farrowing averaged 14.5 ± 4.34 months, subsequent inter-farrowing interval was 11.4 ± 2.73 months. Littersize was 5.8 piglets and weaning age was 4.3 ± 0.99 months. 18% of piglets died before weaning. Simulating pig production at actualstatus, it has been show that monetary returns on inputs (ROI) is negative (1:0.67), but improved (1:1.2) when culling management was optimized so that highest output is gained per unit feed input. If in addition better feeding, controlled mating and better resale prices at fixed dates were simulated, ROI further increased to 1:2.45, 1:2.69, 1:2.7 and 1:3.15 for four respective grower groups. Those findings show the potential of pork production, if basic measures of improvement are applied. Futureexploration of the environment, including climate, market-season and culture is required before implementing the recommended measures to ensure a sustainable development of a more effective and resource conserving pork production in the future. The two studies have shown that the production of local SME pigs plays an important role in traditional farms in NRNNR but basic constraints are limiting their productivity. However, relatively easy approaches are sufficient for reaching a notable improvement. Also there is a demand for more SME pork on local markets and, if basic constraints have been overcome, pig farmers could turn into more commercial producers and provide pork to local markets. By that, environmentally safe meat can be offered to sensitive consumers while farmers increase their income and lower the risk of external shocks through a more diverse income generating strategy. Buffaloes have been found to be the second important livestock species on NRNNR farms. While they have been a core resource of mixed smallholderfarms in the past, the expansion of rubber tree plantations and agricultural mechanization are reasons for decreased swamp buffalo numbers today. The third study seeks to predict future utilization of buffaloes on different farm types in NRNNR by analyzing the dynamics of its buffalo population and land use changes over time and calculating labor which is required for keeping buffaloes in view of the traction power which can be utilized for field preparation. The use of buffaloes for field work and the recent development of the egional buffalo population were analyzed through interviews with 184 farmers in 2007/2008 and discussions with 62 buffalo keepers in 2009. While pig based farms (PB; n=37) have abandoned buffalo keeping, 11% of the rubber based farms (RB; n=71) and 100% of the livestock-corn based farms (LB; n=76) kept buffaloes in 2008. Herd size was 2.5 ±1.80 (n=84) buffaloes in early 2008 and 2.2 ±1.69 (n=62) in 2009. Field work on own land was the main reason forkeeping buffaloes (87.3%), but lending work buffaloes to neighbors (79.0%) was also important. Other purposes were transport of goods (16.1%), buffalo trade (11.3%) and meat consumption(6.4%). Buffalo care required 6.2 ±3.00 working hours daily, while annual working time of abuffalo was 294 ±216.6 hours. The area ploughed with buffaloes remained constant during the past 10 years despite an expansion of land cropped per farm. Further rapid replacement of buffaloes by tractors is expected in the near future. While the work economy is drastically improved by the use of tractors, buffaloes still can provide cheap work force and serve as buffer for economic shocks on poorer farms. Especially poor farms, which lack alternative assets that could quickly be liquidizedin times of urgent need for cash, should not abandon buffalo keeping. Livestock has been found to be a major part of small mixed farms in NRNNR. The general productivity was low in both analyzed species, buffaloes and pigs. Productivity of pigs can be improved through basic adjustments in feeding, reproductive and hygienic management, and with external support pig production could further be commercialized to provide pork and weaners to local markets and fattening farms. Buffalo production is relatively time intensive, and only will be of importance in the future to very poor farms and such farms that cultivate very small terraces on steep slopes. These should be encouraged to further keep buffaloes. With such measures, livestock production in NRNNR has good chances to stay competitive in the future.

Livestock production contributes substantially to the livelihoods of poor rural farmers in Pakistan; strengthening pastoral communities plays an imperative role in the country’s thrive for poverty alleviation. Intestinal helminths constitute a major threat for pastoral livestock keepers in the whole country because chronic infestation leads to distinct losses in livestock productivity, particularly the growth of young animals. Synthetic anthelmintics have long been considered the only effective way of controlling this problem but high prices, side effects and chemical residues/toxicity problems, or development of resistance, lead to their very limited use in many pastoral systems. Additionally, poor pastoralists in remote areas of Pakistan hardly have access to appropriate anthelmintic drugs, which are also relatively expensive due to the long routes of transportation. The search for new and more sustainable ways of supporting livestock keepers in remote areas has given rise to studies of ethno-botanicals or traditional plant-based remedies to be used in livestock health care. Plant-based remedies are cheap or free of cost, environmentally safe and generally create no problem of drug resistance; they thus might substitute allopathic drugs. Furthermore, these remedies are easily available in remote areas and simple to prepare and/or administer. Cholistan desert is a quite poor region of Pakistan and the majority of its inhabitants are practicing a nomadic life. The region’s total livestock population (1.29 million heads) is almost twice that of the human population. Livestock husbandry is the primordial occupation of the communities and traditionally wealth assessment was based on the number of animals, especially goats and sheep, owned by an individual. Fortunately, about 60% of this desert region is richly endowed with highly adapted grasses, shrubs and trees. This natural flora has a rich heritage of scientifically unexplored botanical pharmacopoeia. Against this background, the present research project that was conducted under the umbrella of the International Center for Development and Decent Work at Kassel University, focused on a development aspect: in the Cholistan desert region it was firstly examined how pastoralists manage their livestock, which major health problems they face for the different animal species, and which of the naturally occurring plants they use for the treatment of animal diseases (Chapter 2). For this purpose, a baseline survey was carried out across five locations in Cholistan, using a structured questionnaire to collect data from 100 livestock farmers (LF) and 20 local healers (LH). Most of LF and LH were illiterate (66%; 70%). On average, LH had larger herds (109 animals) than LF (85 animals) and were more experienced in livestock husbandry and management. On average LF spent about 163 Euro per year on the treatment of their livestock, with a huge variability in expenditures. Eighty-six traditional remedies based on 64 plants belonging to 43 families were used. Capparaceae was the botanical family with the largest number of species used (4), followed by Chenopodiaceae, Poaceae, Solanaceae and Zygophyllaceae (3). The plants Capparis decidua (n=55 mentions), Salsola foetida (n=52), Suaeda fruticosa (n=46), Haloxylon salicornicum (n=42) and Haloxylon recurvum (n=39) were said to be most effective against the infestations with gastrointestinal parasites. Aerial parts (43%), leaves (26%), fruits (9%), seeds and seed oils (9%) were the plant parts frequently used for preparation of remedies, while flowers, roots, bulbs and pods were less frequently used (<5%). Common preparations were decoction, jaggery and ball drench; oral drug administration was very common. There was some variation in the doses used for different animal species depending on age, size and physical condition of the animal and severity of the disease. In a second step the regionally most prevalent gastrointestinal parasites of sheep and goats were determined (Chapter 3) in 500 animals per species randomly chosen from pastoral herds across the previously studied five localities. Standard parasitological techniques were applied to identify the parasites in faecal samples manually collected at the rectum. Overall helminth prevalence was 78.1% across the 1000 animals; pure nematode infestations were most prevalent (37.5%), followed by pure trematode (7.9%), pure cestode (2.6%) and pure protozoa infestations (0.8%). Mixed infestations with nematodes and trematodes occurred in 6.4% of all animals, mixed nematode-cestode infestations in 3.8%, and all three groups were found in 19.1% of the sheep and goats. In goats more males (81.1%) than females (77.0%) were infested, the opposite was found in sheep (73.6% males, 79.5% females). Parasites were especially prevalent in suckling goats (85.2%) and sheep (88.5%) and to a lesser extent in young (goats 80.6%, sheep 79.3%) and adult animals (goats 72.8%, sheep 73.8%). Haemonchus contortus, Trichuris ovis and Paramphistomum cervi were the most prevalent helminths. In a third step the in vitro anthelmintic activity of C. decidua, S. foetida, S. fruticosa, H. salicornicum and H. recurvum (Chapter 2) was investigated against adult worms of H. contortus, T. ovis and P. cervi (Chapter 3) via adult motility assay (Chapter 4). Various concentrations ranging from 7.8 to 500 mg dry matter/ml of three types of extracts of each plant, i.e. aqueous, methanol, and aqueous-methanol (30:70), were used at different time intervals to access their anthelmintic activity. Levamisol (0.55 mg/ml) and oxyclozanide (30 mg/ml) served as positive and phosphate-buffered saline as negative control. All extracts exhibited minimum and maximum activity at 2 h and 12 h after parasite exposure; the 500 mg/ml extract concentrations were most effective. Plant species (P<0.05), extract type (P<0.01), parasite species (P<0.01), extract concentration (P<0.01), time of exposure (P<0.01) and their interactions (P<0.01) had significant effects on the number of immobile/dead helminths. From the comparison of LC50 values it appeared that the aqueous extract of C. decidua was more potent against H. contortus and T. ovis, while the aqueous extract of S. foetida was effective against P. cervi. The methanol extracts of H. recurvum were most potent against all three types of parasites, and its aqueous-methanol extract was also very effective against T. ovis and P. cervi. Based on these result it is concluded that the aqueous extract of C. decidua, as well as the methanol and aqueous-methanol extract of H. recurvum have the potential to be developed into plant-based drugs for treatment against H. contortus, T. ovis and P. cervi infestations. Further studies are now needed to investigate the in vivo anthelmintic activity of these plants and plant extracts, respectively, in order to develop effective, cheap and locally available anthelmintics for pastoralists in Cholistan and neighboring desert regions. This will allow developing tangible recommendations for plant-based anthelminthic treatment of sheep and goat herds, and by this enable pastoralists to maintain healthy and productive flocks at low costs and probably even manufacture herbal drugs for marketing on a regional scale.

Objetivo: Determinar la prevalencia de colonización vagino-rectal materna por Estreptococo Hemolítico del grupo B (SßG) entre la semana 35 y 37 de gestación, en la población obstétrica de la Clínica Universitaria Colombia, Bogotá, Colombia. Diseño: Estudio descriptivo de prevalencia Métodos: Entre abril y septiembre de 2008 se tomó la totalidad de pacientes obstétricas con parto atendido en la Clínica Universitaria Colombia y con tamizaje de colonización por SßG entre las semanas 35 y 37 de gestación, empleando cultivo vaginal y/o rectal. Se discriminó el sitio de toma del cultivo, se determinó la prevalencia de colonización vagino-rectal materna por SßG y se evaluó la presencia de infección neonatal temprana en la población estudio. Resultados: Se estudiaron 928 pacientes, encontrándose una prevalencia de colonización por SGB entre la semana 35 y 37 de gestación de 7,8%. Dentro de las pacientes colonizadas el 12,5% fueron detectadas por cultivo vaginal y el 87,5 % por cultivo vaginal – rectal. Conclusiones: La prevalencia de colonización vagino-rectal por SGB entre la semana 35 y 37 de gestación en las pacientes de la Clínica Universitaria Colombia, se encuentra en el rango reportado en Norteamérica y otras poblaciones latinas. La colonización por Streptococcus agalactiae encontrada con cultivo vaginal fue menor a la colonización encontrada por cultivo vaginal y rectal; lo cual está en relación con la importancia en la realización de cultivo vaginal y rectal reportada en la literatura, ya que el tracto gastrointestinal es un reservorio mayor de SGB.

Antecedentes: la Translocación Bacteriana (TB) describe el paso de bacterias residentes en el tracto gastrointestinal a tejidos normalmente estériles como los ganglios linfáticos mesentéricos (GLMs) y a otros órganos internos. Hasta el momento no ha sido demostrada la asociación de infección posoperatoria y TB en pacientes con trauma. Métodos: Para detectar la TB se extrajeron y cultivaron GLMs de 36 pacientes llevados a laparotomía por trauma. Se registraron y documentaron las complicaciones infecciosas posoperatorias. Se definió como infección posoperatoria cualquier cultivo positivo en el periodo posoperatorio. Por medio de un análisis de regresión logística multivariado se establecieron asociaciones entre las variables clínicas preoperatorias, operatorias y la infección posoperatoria. Se realizo genotipificación de los gérmenes que coincidían en el análisis microbiológico entre los hallados en los GLM y los focos infecciosos PO. Resultados: se detectó TB en 33.3% (n=12) de los pacientes. Se presentaron complicaciones infecciosas en el 22.2% (n=8) de los pacientes. Se encontró una diferencia estadísticamente significativa (P=0.047) entre los pacientes con evidencia de TB y el desarrollo de infección en el posoperatorio (41.6%; 5/8), comparada con los pacientes sin evidencia de TB y desarrollo de infección posoperatoria (12.5%; 3/24). El germen responsable de la infección clínica coincidió con el cultivado en el GLM en el 40% de los casos (n=2/5). Cuando realizamos genotipificación de dos gérmenes aislados en un GLM y sitio de infección coincidió un microorganismo en la secuenciación, estableciendo relación de causalidad a nivel molecular. Conclusiones: la TB se asocia con un incremento significativo de aparición de infección posoperatoria en pacientes sometidos a laparotomía por trauma abdominal. Se demostró relación de causalidad a nivel molecular entre el organismo identificado en GLM y el encontrado en sitio de infección posoperatorio.

La evaluación del trauma penetrante de cuello es un tema controversia. Algunas instituciones continúan manejando en forma agresiva el trauma de cuello, llevando a exploración quirúrgica a todo paciente. La selección de los métodos diagnostico más apropiado es un problema polémico. La tendencia actual en la literatura está dirigida hacia la racionalización del uso de los métodos de diagnóstico, indicándolos selectivamente de acuerdo con los hallazgos clínicos al ingreso. Objetivo: determinar el valor de signos blandos al ingreso de los pacientes con trauma penetrante de cuello para definir la necesidad de realizar estudios de diagnóstico adicionales. Métodos: es un estudio observacional de prueba diagnóstica en Hospital Occidente de Kennedy, durante los meses de agosto de 2009 hasta junio de 2010. Se incluyeron 207 pacientes con herida penetrante de cuello. A todos se les realizo una exploración física enfocándose en los signos blandos de lesión vascular, vía aérea y tracto gastrointestinal superior. Se analizaron la sensibilidad, especificidad, valor predictivo positivo y negativo de los signos blandos. Resultados: los signos blandos de lesión vascular tienen una sensibilidad de 95.65%, especificidad 47.83%, VPP de 18.64 %; VPN de 98.88%. Los signos blandos de lesión de vía aérea tienen una sensibilidad 94.12%, especificidad 91.05%, VVP 48.48%; VPN 99.43%. Conclusiones: los pacientes asintomáticos no requieren de otras pruebas diagnosticas para excluir lesiones que ameriten reparo quirúrgico, siendo el seguimiento clínico la conducta apropiada a seguir.

Aunque el manejo nutricional de los pacientes con Pancreatitis Aguda Severa ha sido bien establecido por la evidencia disponible, el inicio de la vía oral en Pancreatitis Leve no ha sido igualmente estudiado. El objetivo de este estudio es evaluar el efecto del inicio temprano de la nutrición por vía oral en estos pacientes. Métodos: Realizamos un descriptivo serie de comparación de casos en los cuales comparamos la evolución y resultados del manejo de los pacientes con pancreatitis aguda antes y después de iniciar un protocolo de inicio temprano de nutrición por vía oral. Resultados: La serie incluyó 13 pacientes manejados con restitución convencional y 9 pacientes a quienes se restituyó tempranamente la nutrición oral. No existieron diferencias estadísticamente significativas en cuanto las características clínicas, etiológicas y demográficas entre los grupos. No se presentaron casos de progresión a enfermedad severa en ninguno de los grupos. La aparición de síntomas gastrointestinales como vomito y dolor después del inicio de la dieta fue similar en los dos grupos. Conclusiones: La restitución de la nutrición por vía oral basada criterios preestablecidos (la intensidad del dolor menor de 4 y un periodo mayor a 6 horas desde el último episodio emético) es segura y disminuye el periodo de ayuno inicial mas no el de estancia hospitalaria.

Introducción: El Síndrome de intestino irritable (SII) es de las patologías gastrointestinales más frecuentes a nivel mundial y de difícil manejo y control, con una prevalencia actual del 14% de acuerdo con los criterios ROMA II en Colombia, con tendencia al aumento. Su origen es multifactorial y no se ha logrado establecer un origen común, se ha encontrado asociación entre el SII y la presencia de ansiedad y depresión. Metodología: Estudio de corte transversal con componente exploratorio de la asociación de ansiedad y depresión con SII; muestra consecutiva de 315 pacientes que se calculó mediante stat calc epi info tomando una prevalencia del 14%, con un número de pacientes esperados de 1200 al mes. Se seleccionaron los pacientes que asisten a consulta de gastroenterología de la IPS CAFAM entre el 8 y el 29 de noviembre de 2010, se les aplicó la encuesta para medir factores sociodemográficos, establecer diagnóstico de SII mediante criterios Roma III y escala ZUNG para depresión y ansiedad. El análisis estadístico se realizó utilizando los programas SPSS versión 18.0 y EPI INFO versión 3.5.1 Resultados: La prevalencia del SII en los pacientes de la consulta de gastroenterología fue del 23% (n 63), con IC 95% (18%-28%). Se hallo una asociación significativa entre el género femenino (p:0.035),el grupo de edad de 31 a 55 (p: 0.043), y una fuerte asociación entre depresión severa y el síndrome de intestino irritable (p: 0.08). No se encontró asociación entre el SII y la ansiedad. Discusión: Existen factores de riesgo asociados a la presencia SII, como el género, la edad y la depresión severa documentados en la literatura (22). Se observó que la depresión severa está asociada significativamente con el SII, no así la ansiedad en ninguna de sus presentaciones; es necesario realizar estudios posteriores, que contribuyan a verificar la existencia y fuerza de dicha asociación y así aportar mayor evidencia que permita incentivar y fomentar el manejo multidisciplinario de la depresión en los pacientes con SII. La prevalencia de coledocolitiasis es de un 10 a 20%. 10-20% tienen coledocolitiasis gigante, es decir presencia de cálculos mayores de 15 mm, aumentando la morbimortalidad por complicaciones. El objetivo principal fue determinar la frecuencia de coledocolitiasis gigante, la presencia de factores predictores del éxito o fracaso del manejo endoscópico. El éxito en el manejo endoscópico está entre 80 y 90%, un 20% requieren cirugía de exploración biliar. Se realizó la búsqueda de las variables utilizando el instrumento para la recolección de la información. Se realizó un análisis univariado y bivariado de las variables medidas y se utilizo STATA versión 10. Como principal resultado, se encontró que la frecuencia de coledocolitiasis gigante en nuestra población fue del 10%, el éxito del manejo endoscopio fue del 89.23% y el factor predictor mas fuerte para el éxito fue el diámetro del cálculo, siendo mayor para cálculos de menos de 19.09 mm. Como conclusión, en nuestro estudio, la frecuencia de coledocolitiasis gigante es cercana a la conocida en la literatura mundial. El manejo endoscópico en nuestro estudio es el pilar en estos casos, teniendo probabilidad de éxito en el manejo que es igual a la publicada en los estudios mundiales, que existe la probabilidad que el tamaño del cálculo mayor a 19 mm de diámetro indique mayor tasa de fracaso y requerimiento de técnicas endoscópicas avanzadas para su éxito. Se requieren estudios, con mayor número de pacientes para determinar la validez estadística de estos resultados.
