RESUMO: Com o objetivo de avaliar a produção e distribuição da biomassa da parte aérea de diferentes genótipos de pinhão-manso no semiárido brasileiro, foi implantado um experimento na Fazenda Gabriela, localizada no município de Santa Maria da Boa Vista, Pernambuco. O delineamento experimental adotado foi em blocos casualizados, com dez tratamentos (genótipos de pinhão-manso) e três repetições, em parcelas lineares de seis plantas, com bordadura simples e espaçamento de 3,0 x 2,0 m. Foram aplicadas uma adubação correspondente a 150 g de NPK (06:24:12) por cova no plantio e duas adubações de cobertura com 150 g.planta-1 de NPK (10:10:10) aos seis e aos doze meses de idade. As plantas foram irrigadas semanalmente por gotejamento com uma aplicação média de 20 litros de água por planta durante o período de estiagem da região. Aos 24 meses de idade, foi medida a altura total das plantas, o diâmetro médio das bifurcações a 1,30 m da superfície do solo e o número de bifurcações a 0,5 m de altura. Foram feitas 26 colheitas semanais de frutos/sementes. Os frutos foram colhidos maduros, antes de sua queda ao chão, durante sete meses. Os genótipos apresentaram elevada uniformidade agronômica, exceto para a variável número de bifurcações, onde o genótipo 1701 foi superior aos genótipos 1501, 1602, 1703 e 1601. A produção de biomassa dos genótipos em condições irrigadas no semiárido é elevada e a distribuição da biomassa dos genótipos nos diferentes componentes seguiu a ordem decrescente: raiz>frutos>galhos grossos>folhas>casca>galhos finos. ABSTRACT: In order to assess production and distribution of biomass shoots of different genotypes of Jatropha curcas under irrigation in the semiarid region of Pernambuco, Brazil, an experiment was established in Gabriela Farm, in the municipality of Santa Maria da Boa Vista-PE. The experimental design was randomized blocks with ten treatments (genotypes of Jatropha curcas), and three replications in row plots of six plants, with a single border and spacing of 3.0 x 2.0 m. Plants were fertilized with 150 g of NPK (06:24:12) at planting time, and a topdressing with 150 g.planta-1 NPK (10:10:10) applied at six and twelve months of age. The plants were irrigated weekly using a dripping system with an average water application of 20 l.plant-1 during the dry period of the region. At 24 months of age, the overall height of the plants, the average diameter of bifurcations at 1.30m from the soil level and the number of bifurcations at 0.5 m of height were evaluated. Twenty six fruit/ seed harvests were done weekly. Fruits were harvested ripe, before falling on the ground, for seven months. To determine dry biomass, the plants were cut at 0.30 m from soil level. The genotypes showed high agronomic uniformity, except for the variable number of bifurcations, where the genotype 1701 was superior to the genotypes 1501, 1602, 1703 and 1601. Biomass production of genotypes in irrigated conditions in the semiarid region is high and the distribution of biomass followed the decreasing order: root>fruit>thick branches>leaves>bark>thin branches.

A hipertensão Arterial Sistêmica (HAS) é considerada uma doença crônica, com maior prevalência no mundo moderno e é caracterizada pelo aumento da pressão arterial, tendo como causas a idade, sexo, antecedentes familiares, raça, obesidade, sedentarismo, o etilismo, o estresse, tabagismo e outras. Considera-se que, existe doença hipertensiva quando há uma insuficiência nos mecanismos que mantêm a pressão arterial dentro dos limites normais. Em media 10 a 20% da população adulta é portador de hipertensão arterial. Ela é causa direta ou indireta de elevado número de óbitos decorrentes de acidentes vasculares cerebrais insuficiência cardíaca, insuficiência renal e infarto do miocárdio. Tem como objetivo elaborar um plano de intervenção, para acompanhar os pacientes portadores de HAS, inscritos no PSF 04, em Martinho Campos-MG. A metodologia utilizada foi uma pesquisa de natureza qualitativa sobre o tema para subsidiar a elaboração do referencial teórico e o plano de ação. O problema priorizado foi acompanhamento deficiente aos portadores de HAS. Os nós críticos identificados foram o baixo nível de informação sobre a patologia, a falta de programação eficiente das atividades, a falta de equidade na distribuição de consultas e a não utilização de protocolo clínico. Pretende-se com o plano de ação aumentar o nível de informação sobre a hipertensão, elaborar agenda programada, utilizar os protocolos do Ministério da Saúde e Secretaria Estadual de Saúde de Minas Gerais. A elaboração deste plano de ação possibilitou ao PSF 04 reconhecer a importância de se utilizar um método de planejamento para organizar o processo de trabalho em prol de um acompanhamento mais qualificado aos portadores de hipertensão arterial sistêmica.

A elevada magnitude e ampla distribuição geográfica das geohelmintíases, aliadas às repercussões negativas que podem causar no organismo humano, têm conferido a essas infecções uma posição relevante entre os principais problemas de saúde da população. Como estratégia de redução da carga de infecção pelos geohelmintos, o Ministério da Saúde propõe a implantação do tratamento quimioprofilático em escolares do ensino público na faixa etária entre 5 a 14 anos. Assim, este trabalho objetivou desenvolver um programa educativo como forma de intervenção para os escolares participantes da campanha nacional de geohelmintíases em uma escola do município de Marechal Deodoro - AL. A elaboração da proposta de intervenção foi baseada no Planejamento Estratégico Situacional (PES). Fez-se a pesquisa bibliográfica na base de dados do SciELO na biblioteca virtual do NESCON (Núcleo de Educação em Saúde Coletiva da Faculdade de Medicina da Universidade Federal de Minas Gerais). Na escola, a ação ocorreu no período de 4 a 8 de Agosto de 2014. 297 escolares entre 5 a 14 anos de idade receberam o tratamento através da ingestão do Albendazol 400mg. Observou-se que os escolares do sexo masculino realizaram a ingestão da medicação em 53,8%; enquanto que do sexo feminino 46,2%. Portanto, a atuação do Programa de Saúde na Escola (PSE) é de suma importância para minimizar agravos, prevenir doenças e promover ações voltadas para a saúde dos escolares, visando assim, um cuidado de forma humanizada, proporcionando um melhor rendimento escolar e uma melhor qualidade de vida.
Hipertensão Arterial: intervenção sobre os fatores de risco e promoção de hábitos de vida saudáveis.

A Hipertensão Arterial Sistêmica é conceituada pela V Diretrizes Brasileiras de Hipertensão Arterial como uma síndrome caracterizada pela presença de níveis tensionais elevados associados a alterações metabólicas, hormonais e a fenômenos tróficos, que consistem na hipertrofia cardíaca e vascular. Trata-se de uma síndrome de origem multifatorial, sendo considerados os valores, a partir de 139 mmhg para a Pressão Arterial Sistólica e 89 mmhg para a Pressão Arterial Diastólica nos indivíduos adultos. Certas circunstâncias aumentam a probabilidade de ter hipertensão, que são conhecidas como fatores de risco modificáveis e não modificáveis. Destacam-se como fatores de risco, excesso de peso ou obesidade, predisposição hereditária, sedentarismo, idade avançada, dieta rica em sal e gorduras, raça negra, dieta pobre em fruta fresca e vegetais, sexo masculino, consumo excessivo de álcool. Considerando que o principal problema é a prevalência de HAS, torna-se necessário conhecê-lo de forma abrangente, suas causas e suas consequências. O Programa de Saúde da Família se operacionaliza na Unidade Básica de Saúde Alcides Peixoto 2, no município de Crato-Ceará. A área tem uma população de 1091 pacientes, destes 446 são do sexo masculino e 645 do sexo feminino, dos quais 108 (9,9%) têm diagnóstico de hipertensão arterial. Acredita-se que deve haver um subregistro, levando-se em conta que esta doença não raramente cursa de forma silenciosa. Portanto, o problema prioritário neste território é a alta prevalência de HAS não controlada. Urge pensar-se em estratégias relacionadas a protocolos e diretrizes bem estabelecidos. Devendo ser comtemplados a assistência e o controle de medicamentos, redimensionamento nas atitudes da equipe multiprofissional e na educação em saúde dos pacientes com uma sistematização de planejamento, discussão e avaliação. Este estudo é justificado pela alta prevalência de hipertensão arterial na comunidade do Parque Grangeiro 2, no município, bem como pelo grande número de pacientes com fatores de riscos como: maus hábitos dietéticos, sedentarismo, tabagismo e uso de bebidas alcoólicas que foram encontrados durante o processo de cadastramento na área de abrangência.

A gravidez é o período durante o qual toda mulher na idade fértil tem um feto no seu corpo e vai desde a concepção até o parto; baseado em isto se realizou um estudo descritivo, onde analisei o conhecimento e percepção dos riscos das grávidas de nossa UBS Dr. Raimundo Gomes, com a identificação das adolescentes e as maiores de 35 anos; que pertencem ao grupo que tem maiores riscos durante a gestação, tais quais riscos maternos e riscos fetais; e fizemos uma analise da literatura sobre o tema. Serão entrevistadas as setenta grávidas que tivemos durante os oito primeiros meses do ano e analisados seus prontuários; os dados obtidos servirão para criar um plano de intervenção que nos permita que toda mulher na idade fértil conheça as características da gravidez, saiba identificar seus riscos antes e durante a gestação, e a sua vez lograr a melhoria na atenção primaria à saúde, especificamente na atenção pré-natal em nosso posto e no município com o trabalho de todos os integrantes da equipe de saúde.

BACKGROUND: The model for end-stage liver disease (MELD) was developed to predict short-term mortality in patients with cirrhosis. There are few reports studying the correlation between MELD and long-term posttransplantation survival. AIM: To assess the value of pretransplant MELD in the prediction of posttransplant survival. METHODS: The adult patients (age >18 years) who underwent liver transplantation were examined in a retrospective longitudinal cohort of patients, through the prospective data base. We excluded acute liver failure, retransplantation and reduced or split-livers. The liver donors were evaluated according to: age, sex, weight, creatinine, bilirubin, sodium, aspartate aminotransferase, personal antecedents, brain death cause, steatosis, expanded criteria donor number and index donor risk. The recipients' data were: sex, age, weight, chronic hepatic disease, Child-Turcotte-Pugh points, pretransplant and initial MELD score, pretransplant creatinine clearance, sodium, cold and warm ischemia times, hospital length of stay, blood requirements, and alanine aminotransferase (ALT >1,000 UI/L = liver dysfunction). The Kaplan-Meier method with the log-rank test was used for the univariable analyses of posttransplant patient survival. For the multivariable analyses the Cox proportional hazard regression method with the stepwise procedure was used with stratifying sodium and MELD as variables. ROC curve was used to define area under the curve for MELD and Child-Turcotte-Pugh. RESULTS: A total of 232 patients with 10 years follow up were available. The MELD cutoff was 20 and Child-Turcotte-Pugh cutoff was 11.5. For MELD score > 20, the risk factors for death were: red cell requirements, liver dysfunction and donor's sodium. For the patients with hyponatremia the risk factors were: negative delta-MELD score, red cell requirements, liver dysfunction and donor's sodium. The regression univariated analyses came up with the following risk factors for death: score MELD > 25, blood requirements, recipient creatinine clearance pretransplant and age donor >50. After stepwise analyses, only red cell requirement was predictive. Patients with MELD score < 25 had a 68.86%, 50,44% and 41,50% chance for 1, 5 and 10-year survival and > 25 were 39.13%, 29.81% and 22.36% respectively. Patients without hyponatremia were 65.16%, 50.28% and 41,98% and with hyponatremia 44.44%, 34.28% and 28.57% respectively. Patients with IDR > 1.7 showed 53.7%, 27.71% and 13.85% and index donor risk <1.7 was 63.62%, 51.4% and 44.08%, respectively. Age donor > 50 years showed 38.4%, 26.21% and 13.1% and age donor <50 years showed 65.58%, 26.21% and 13.1%. Association with delta-MELD score did not show any significant difference. Expanded criteria donors were associated with primary non-function and severe liver dysfunction. Predictive factors for death were blood requirements, hyponatremia, liver dysfunction and donor's sodium. CONCLUSION: In conclusion MELD over 25, recipient's hyponatremia, blood requirements, donor's sodium were associated with poor survival.

CONTEXT: Intestinal constipation - a common symptom among the general population - is more frequent in women. It may be secondary to an improper diet or organic or functional disturbances, such as dyskinesia of the pelvic floor. This is basically characterized by the absence of relaxation or paradoxical contraction of the pelvic floor and anal sphincter during evacuation. OBJECTIVE: To analyze, by manometric data, the anal pressure variation at rest, during evacuation effort by using the Valsalva maneuver and forced post-expiratory apnea in subjects with secondary constipation. METHODS: Twenty-one patients (19 females - 90.4%) with a mean age of 47.5 years old (23-72) were studied. The diagnosis was performed using anorectal manometry, with a catheter containing eight channels disposed at the axial axis, measuring the proximal (1) and distal (2) portions of the anal orifice. The elevation of the pressure values in relation to the resting with the evacuation effort was present in all patients. The Agachan score was used for clinical evaluation of constipation. The variables studied were: mean anal pressure of the anal orifice for 20 seconds at rest, the effort of evacuation using Valsalva maneuver and the effort of evacuation during apnea after forced expiration, as well as the area under the curve of the manometric tracing at moments Valsalva and apnea. RESULTS: The analysis of the mean values of the anal pressure variation at rest evidenced difference between proximal and distal channels (P = 0.007), independent of the moment and tendency to differ during moments Valsalva and apnea (P = 0.06). The mean of values of the area under the manometric tracing curve showed differences between moments Valsalva and apnea (P = 0.0008), either at the proximal portion or at the distal portion of the anal orifice. CONCLUSION: The effort of evacuation associated with postexpiratory apnea, when compared with the effort associated with the Valsalva maneuver, provides lower elevation of anal pressure at rest by the parameter area under the curve.

We conducted an open, add-on study with topiramate (TPM) as adjunctive therapy in Lennox-Gastaut syndrome (LGS), to assess the long-term efficacy and safety and to evaluate quality of life (QL) measurements in the chronic use of TPM. We studied 19 patients (11 male; age ranging from 4 to 14 years) with uncontrolled seizures receiving 2-3 anti-epileptic drugs. Patients were followed up to 36 months of treatment. A questionnaire was used to query parents about QL. Seven patients completed the study at 36 months and seizure frequency was reduced > 75% in 4, and < 50% in 3 patients. Two children became seizure free for more than 24 months. Most side effects were CNS related, with the most frequent being somnolence and anorexia. These were generally transient. One patient dropped-out due to powder in the urine. None of the patients required hospitalization. At 36 months, patients' alertness (2/7), interaction with environment (5/7), ability to perform daily activities (5/7), and verbal performance (6/7) improved on TPM. We conclude that TPM may be useful as adjunctive therapy in the treatment of LGS. The efficacy of TPM was maintained in long-term treatment in more than 40% of patients, long term safety was confirmed and QL improved on TPM.

Video-polygraphic-EEG studies were performed in the first 24 life-hours of 26 healthy full-term newborns without perinatal injuries. The neurological examination and cranial ultrasonography were normal. The newborns were divided into two groups: one, with full-term appropriate - birth weight 11 newborns (control group ) and the other with full-term low-birth weight 15 newborns. Thirteen newborns of the second group had video-polygraphic-EEG study abnormalities. The most frequent abnormalities were found in 11 cases, as far as sleep architecture is concerned. Also, when compared with the control group, 8 cases of an excessive amount of startles and 2 cases of low behavior activities were found. The results demonstrate the usefulness of video-polygraphic-EEG study in the full-term newborns with intra-uterine growth retard. This examination was sensitive to detect behavior, sleep architecture and EEG standard differences in the low birth-weight newborns as to the control group.

As a way to reduce water losses in furrow irrigation systems, used in fresh market tomato production, farmers are improperly distributing water into the field using plastic hose. The objective of this work was to study the suitability of using quick coupler sprinkler as hose connectors for water distribution in tomato plantation. The first step of the study was to assess the current hose field operation for tomato growers. Subsequently, four models of quick couplers sprinklers available in the market were tested in laboratory to determine the coefficient of resistance, the equivalent tube length, the head loss curve and the linking efficiency. As result, a structural design for hose connectors was presented using the model of coupling with the best hydraulic performance. Additionally, some technical recommendations on its use in irrigation water distribution are highlighted. Despite the requirement for additional field trials, the proposed system has potential to optimize the water use efficiency, to improve workers ergonomic conditions, and ensure good profitability to the producer.

The aim of this study was to analyze the prevalence of hypertension and control practices among the elderly. The survey analyzed data from 872 elderly people in São Paulo, Brazil, through a cluster sampling, stratified according to education and income. A Poisson multiple regression model checked for the existence of factors associated with hypertension. The prevalence of self-reported hypertension among the elderly was 46.9%. Variables associated with hypertension were self-rated health, alcohol consumption, gender, and hospitalization in the last year, regardless of age. The three most common measures taken to control hypertension, but only rarely, are oral medication, routine salt-free diet and physical activity. Lifestyle and socioeconomic status did not affect the practice of control, but knowledge about the importance of physical activity was higher among those older people with higher education and greater income. The research suggests that health policies that focus on primary care to encourage lifestyle changes among the elderly are necessary.

This aim of this work was to carry out an epidemiological study on acetabular fractures in the city of Campinas and surrounds, in view of the few published papers on this subject. Medical files with a diagnosis of acetabular fracture between the years 2004 and 2008 that were made available by the Medical Archiving Service of Hospital das Clínicas, State University of Campinas (UNICAMP) were analyzed by six observers. Data on patients' ages, sex, side affected by the fracture, mechanism of injury, material used for synthesis, complications of the operation, associated fractures, length of hospitalization before and after the surgery, time of total internment and number of physiotherapy sessions before and after the surgery were gathered. It was observed in this population that the left side was more affected; the mechanism of injury that most often caused this type of fracture was automobile accidents; injuries to the sciatic nerve were the commonest surgical complications; and the synthesis material most used was reconstruction plates.

OBJECTIVE: Evaluate the impact that trochanteric hip fractures produce on life quality of elderly patients, treated surgically, during one year of monitoring. METHODS: Were selected 73 elderly with transtrochanteric femur fracture, aged equal or over than 65 years and of both sexes. All participants underwent a standardized questionnaire which obtained information on lifestyle habits, functionality, physical activity, ambulation and cognitive state. Were excluded patients who died, not walking, with neurological diseases and pathological fractures. RESULTS: The average age was 80.17 ± 7.2 years and 75% was female. When comparing the summation of activities of daily living (p = 0.04) and instrumental daily living (p = 0.004) obtained before and after fracture, the patients become more dependent after fracture. Activities of daily living that worsened after fracture were bathing (p = 0.04), toileting (p = 0.02) and dressing (p = 0.04). All instrumental activities of daily living showed a significant difference, with functional dependence after fracture, increased need for walking aid (p = 0.00002), aged (p = 0.01) and not doing housework (p = 0.01). The low score on the test Minimental was associated with a greater dependence to perform activities of daily living before (p = 0.00002) and after (p = 0.01) fracture. CONCLUSION: After a year, activities of daily living that depended on the lower limbs worsened significantly, all instrumental activities of daily living were significantly worse in more than 50% of patients and more than half of patients who walked without support lost this ability.

The aim of this study was to test fear, anxiety and control related to dental treatment. The subjects were 364 children with ages between 7 and 13 years. Three questionnaires with multiple choice questions were applied in groups of 10 children. The first instrument was the 15-item dental subscale from the Childrens Fear Survey Schedule9. The subjects rated their level of fear on a 5-point scale. The second survey instrument was the 20-item subscale from the State Trait Anxiety Inventory for Children16. This measure was used to capture how anxious the child was, in general. The third instrument was the Child Dental Control Assessment19. It contained 20 items to assess perceived control and 20 items to assess desired control. The results of the survey indicated that dental fear and anxiety were slightly higher for females when compared with male subjects (P < 0.05). Older children (11 to 13 years old) obtained higher fear scores than younger ones (7 to 9 years old). Concerning perceived control, the results indicate that younger children perceive more control than older ones. For desired control, the results indicate that younger children reported higher percentages than older ones. In this study, patients who had undergone anesthesia during treatment revealed higher fear scores when compared with those who had not. Dental fear etiology seems to be related to a procedure that may involve pain or lack of control.

INTRODUCTION: Armies from all over the world acknowledge the importance of good physical fitness for the performance of military duties. The Military Exercise Training (MET) attempts to provide assistance to this search for better physical fitness and performance. OBJECTIVE: To verifying the effect on the body composition and physical performance of the students at ESPCEX (Military School for Preparation of Army Cadets) after 13 weeks of MET. METHODS: The sample was formed by 287 male students from the ESPCEX, whose average age was 18.33 ±1.26. Such students accomplished a boarding school routine, having defined schedules, meals and activities from which they were only released during the weekends. The MET was accomplished five days a week and it comprised both aerobic and resistance training. Measurement of body mass, height, skinfold (triceps, abdominal and suprailiac) was accomplished during pre and post training periods, and the following tests were performed: 12-minutes-run, oblique sit up, arm push up and pull up. Fat percentage, fat-free body mass and fat body mass were calculated using the anthropometric data based on the Guedes 3 skinfold protocol. RESULTS: Significant reduction in fat body mass, fat percentage and in triceptal and abdominal skinfold, as well as increase in suprailiac skinfold and fat-free body mass was observed when anthropometric and body composition data were compared, during the initial and the final periods of training. Significant improvement also occurred in all prformed physical tests, in which better performance was achieved. CONCLUSION: The acquired data suggest that performance of MET 5 days a week brought significantly improved body composition as well as physical performance
