We report the case of a 37-year-old woman who developed critical upper limb ischemia caused by a cervical rib. Because the malformation was initially undiagnosed, a vascular bypass was performed, and failure occurred. Following a 6-month therapy with sildenafil, revascularization of the arm was successful and amputation was avoided. A 6-year follow-up shows a rich collateral network at the compression site and normal values of digital plethysmography. Because hand surgeons often see patients with digital ulcerations and other manifestations of peripheral vascular pathology, therapy of ischemia with sildenafil could be an effective treatment option in patients not responding to classic drugs.

Plants are photoautotrophic sessile organisms that use environmental cues to optimize multiple facets of growth and development. A classic example is phototropism - in shoots this is typically positive, leading to growth towards the light, while roots frequently show negative phototropism triggering growth away from the light. Shoot phototropism optimizes light capture of leaves in low light environments and hence increases photosynthetic productivity. Phototropins are plasma-membrane-associated UV-A/blue-light activated kinases that trigger phototropic growth. Light perception liberates their protein kinase domain from the inhibitory action of the amino-terminal photosensory portion of the photoreceptor. Following a series of still poorly understood events, phototropin activation leads to the formation of a gradient of the growth hormone auxin across the photo-stimulated stem. The greater auxin concentration on the shaded compared with the lit side of the stem enables growth reorientation towards the light. In this Minireview, we briefly summarize the signaling steps starting from photoreceptor activation until the establishment of a lateral auxin gradient, ultimately leading to phototropic growth in shoots.

Cyclic esotropia is characterized by a 24-hour period of straight eye position followed by 24 hours of large-angle esotropia. Possible mechanisms include notably progressive loss of compensation of a latent strabismus. The classic treatment is surgical correction of the angle measured on the days with manifest deviation. We report the first case of cyclic esotropia successfully treated by prismatic correction of the latent strabismus present on "straight" days.

Työ liittyy uuden sukupolven monitelakalanterin tuotekehitysprojektiin. Työssä tutkittiin uuden monitelakalanterin tuoterakennetta, esisuunnitteluaineistoa ja mahdol- lisuuksia parametriseen 3D-suunnitteluun. Työssä ei suoritettu teknistä laskentaa. Uuden monitelakalanterin modulaarinen tuoterakenne ja esisuunnitteluaineisto pohjautuu markkinoilla olevaan OptiLoad-monitelakalanteriin. Tuoterakenteella ja esisuunnitteluaineistolla ohjataan monitelakalantereiden asiakastoimitusprojekteissa eri kokoluokkien kustannuksia, suunnittelua ja tuotantoa. Modulaarisella tuoterakenteella, jolla on selkeät mitoitusperusteet vaikutetaan oleellisesti asiakastoimitusten luotettavuuteen. Huomioimalla tuotekehitysprojektissa asiakaskohtaiset muutokset modulaarisessa tuoterakenteessa voidaan tehdä alustava esisuunnitteluaineisto. Parametrinen 3D-suunnittelu sekä lopullinen esisuunnitteluaineisto vaatii uuden monitelakalanterin rakenteiden tuotteistusta, standardointia ja elementtimenetelmään perustuvaa lujuuslaskentaa.

Classic semiquantitative proteomic methods have shown that all organisms respond to a mild heat shock by an apparent massive accumulation of a small set of proteins, named heat-shock proteins (HSPs) and a concomitant slowing down in the synthesis of the other proteins. Yet unexplained, the increased levels of HSP messenger RNAs (mRNAs) may exceed 100 times the ensuing relative levels of HSP proteins. We used here high-throughput quantitative proteomics and targeted mRNA quantification to estimate in human cell cultures the mass and copy numbers of the most abundant proteins that become significantly accumulated, depleted, or unchanged during and following 4 h at 41 °C, which we define as mild heat shock. This treatment caused a minor across-the-board mass loss in many housekeeping proteins, which was matched by a mass gain in a few HSPs, predominantly cytosolic HSPCs (HSP90s) and HSPA8 (HSC70). As the mRNAs of the heat-depleted proteins were not significantly degraded and less ribosomes were recruited by excess new HSP mRNAs, the mild depletion of the many housekeeping proteins during heat shock was attributed to their slower replenishment. This differential protein expression pattern was reproduced by isothermal treatments with Hsp90 inhibitors. Unexpectedly, heat-treated cells accumulated 55 times more new molecules of HSPA8 (HSC70) than of the acknowledged heat-inducible isoform HSPA1A (HSP70), implying that when expressed as net copy number differences, rather than as mere "fold change" ratios, new biologically relevant information can be extracted from quantitative proteomic data. Tässä työssä käsitellään muutoksia, joita EGPRS-standardi aiheuttaa matkapuhelimen fyysiselle signalointiprotokollatasolle. EGPRS on laajennus normaaliin GPRS-verkkoon ja se mahdollistaa yhteistoiminnan Yhdysvalloissa käytetyn TDMA-matkapuhelinstandardin kanssa. EGPRS-puhelimessa puhelut hoidetaan TDMA-verkon kautta ja datan siirto GPRS-verkon kautta. EGPRS operoi GSM850 ja GSM1900 taajuusalueilla. EGPRS:n merkittävimpiä uusia ominaisuuksia ovat modulointimenetelmä EDGE ja kompaktit kontrollikanavat. Uudella modulointimenetelmällä saavutetaan kolminkertainen tiedonsiirtonopeus vanhaan modulointimenetelmään verrattuna. Kompaktien kontrollikanavien avulla voidaan tukiaseman taajuuskaistan tarvetta pienentää huomattavasti. Tukiasema voi käyttää vanhaa ja uutta modulointimenetelmää yhtä aikaa. EGPRS-verkko voi sisältää sekä kompakteja että normaaleja tukiasemia.

In Spain a significant number of individuals die from atherosclerotic disease of the coronary and carotid arteries without having classic risk factors and prodromal symptoms. The diagonal ear lobe crease (DELC) has been characterized in the medical literature as a surrogate marker which can identify high risk patients having occult atherosclerosis. This topic however has not been examined in either the medical or dental literature emanating from Spain. The majority of clinical, angiography and postmortem reports support the premise that DELC is a valuable extravascular physical sign able to distinguish some patients at risk of succumbing to atherosclerosis of the coronary arteries. A minority of studies have however failed to support this hypothesis. More recently reports using B mode ultrasound have also linked DELC to atherosclerosis of the carotid artery and another report has related DELC to the presence of calcified carotid artery atheromas on panoramic radiographs. DELC is readily visible during head and neck cancer screening examinations. In conjunction with the patient"s medical history, vital signs, and panoramic radiograph, the DELC may assist in atherosclerotic risk assessment

Elizabeth Russell sosté que les utopies escrites per dones són més transgressores «perquè desconstrueixen el concepte de perfecció» (2007: 21). En aquesta línia de crítica social i necessitat de reformes, quant al gènere femení, neixen, amb cinc segles de diferència, les dues obres estudiades aquí, Le livre de la cité des dames (1405), de Christine de Pisan, i Herland (1915), de Charlotte Perkins Gilman. Amb notables excepcions com La mística de la feminitat (1965), de Betty Friedan, i El segon sexe (1968), de Simone de Beauvoir, a casa nostra els discursos androcèntrics es ben cuidaren de negar l’entrada d’aires subversius que, per raons òbvies, poguessin esverar aquell àngel de la llar amansit i silenciat, per dir-ho a la manera woolfiana. Charlotte Perkins Gilman arribà per primera vegada a Catalunya i a la península ibèrica el 1982 amb la traducció al català de Montserrat Abelló per a l’editorial La Sal d’El paper de paret groc. En els mateixos anys vuitanta i per a la mateixa editorial Helena Valentí traduí al castellà El país de ellas (1987), obra que el 2002 traslladà al català Jordi Vidal Tubau en una edició a cura d’Eulàlia Lledó que porta per títol Terra d’elles. Poc després, el 1990, La ciutat de les dames, de Christine de Pisan, es pogué llegir en llengua catalana amb una edició i traducció de Mercè Otero per a la col·lecció «Espai de Dones» d’Edicions de l’Eixample. Aquest article presenta la recepció a Catalunya de dos clàssics del gènere de les utopies feministes, La ciutat de les dames (1990), de Christine de Pisan, i Terra d’elles (2002), de Charlotte Perkins Gilman, amb notes sobre la seva difusió al castellà i al gallec.

Tutkielman tavoitteena oli selvittää: - Miten franchisingvalmennus eroaa klassisesta yrittäjäkoulutuksesta? - Mitä mieltä franchisingyrittäjä-valmennuksen läpikäyneet ovat valmennuksesta? - Mikä on franchisingin tila Suomessa? Yrittäjyyttä tarkasteltaessa huomiota kiinnitettiin käsitteen määrittämiseen yrittäjätypologioiden, motivaatioteorioiden ja toimintaympäristön kautta. Tutkielmassa selvitettiin myös yrittäjäksi ryhtymiseen vaikuttavia tekijöitä ja Suomen yrittäjyysaktiivisuuden tasoa. Franchisingin osalta määriteltiin eri toimintamuodot ja keskityttiin Business Format Franchising-toimintamalliin. Työssä selvitettiin myös franchisingyrittäjyyden eroja klassiseen yrittäjyyteen ja tarkasteltiin franchisingin tilaa Suomessa. Koulutuksen osalta keskityttiin tarkastelemaan aikuiskoulutusta ja vielä tarkemmin työvoimapoliittista yrittäjäkoulutusta sekä koulutuksen arviointia. Työssä tarkasteltiin myös franchisingyrittäjä-valmennusta ja sen tavoitteet käytiin myös läpi. Empiirinen tutkimus koostui 118:ta franchisingyrittäjä-valmennukseen vuosina 1998–2000 osallistuneesta henkilöstä. Tutkimus suoritettiin postikyselynä. Tutkimusaineistoa analysoitiin SPSS for Windows-tietojenkäsittelyohjelmalla ja tutkimusjoukon kuvailuun käytettiin frekvenssejä, prosenttilukuja ja ristiintaulukointia. Franchisingvalmennus ja klassinen yrittäjäkoulutus erosivat siinä vaiheessa, kun tarkasteltiin franchisingantajan ja – ottajan näkökulmaa. Franchisingyrittäjä-valmennuksessa käsiteltiin hyvinkin tarkasti sekä franchisingantajan, että – ottajan roolit, velvollisuudet ja oikeudet. Franchisingyrittäjä-valmennuksessa ei myöskään käsitelty lainkaan tuotekehitystä ja lanseerausta. Franchisingyrittäjä-valmennus oli suurimmalle osalle koulutuksen läpäisseistä positiivinen kokemus. Yhdeksän kymmenestä oli myös valmiita suosittelemaan valmennusta franchisingyrittäjyydestä kiinnostuneille henkilöille. Suomessa franchisingin kehitys on siirtymässä suvantovaiheesta uuden kasvun vaiheeseen. Uusia franchisingketjuja perustetaan ja vanhat ketjut lisäävät toimipaikkojensa määrää. Jatkossa voidaan odottaa naisyrittäjien määrän lisääntyvän, sillä uusimmat Suomeen perustetut ketjut ovat terveys- ja hoiva-alalla, jotka ovat perinteisesti naisvaltaisia aloja.

The application of contrast media in post-mortem radiology differs from clinical approaches in living patients. Post-mortem changes in the vascular system and the absence of blood flow lead to specific problems that have to be considered for the performance of post-mortem angiography. In addition, interpreting the images is challenging due to technique-related and post-mortem artefacts that have to be known and that are specific for each applied technique. Although the idea of injecting contrast media is old, classic methods are not simply transferable to modern radiological techniques in forensic medicine, as they are mostly dedicated to single-organ studies or applicable only shortly after death. With the introduction of modern imaging techniques, such as post-mortem computed tomography (PMCT) and post-mortem magnetic resonance (PMMR), to forensic death investigations, intensive research started to explore their advantages and limitations compared to conventional autopsy. PMCT has already become a routine investigation in several centres, and different techniques have been developed to better visualise the vascular system and organ parenchyma in PMCT. In contrast, the use of PMMR is still limited due to practical issues, and research is now starting in the field of PMMR angiography. This article gives an overview of the problems in post-mortem contrast media application, the various classic and modern techniques, and the issues to consider by using different media.

Accurate prediction of mortality following burns is useful as an audit tool, and for providing treatment plan and resource allocation criteria. Common burn formulae (Ryan Score, Abbreviated Burn Severity Index (ABSI), classic and revised Baux) have not been compared with the standard Acute Physiology and Chronic Health Evaluation II (APACHEII) or re-validated in a severely (≥20% total burn surface area) burned population. Furthermore, the revised Baux (R-Baux) has been externally validated thoroughly only once and the pediatric Baux (P-Baux) has yet to be. Using 522 severely burned patients, we show that burn formulae (ABSI, Baux, revised Baux) outperform APACHEII among adults (AUROC increase p<0.001 adults; p>0.5 children). The Ryan Score performs well especially among the most at-risk populations (estimated mortality [90% CI] original versus current study: 33% [26-41%] versus 30.18% [24.25-36.86%] for Ryan Score 2; 87% [78-93%] versus 66.48% [51.31-78.87%] for Ryan Score 3). The R-Baux shows accurate discrimination (AUROC 0.908 [0.869-0.947]) and is well-calibrated. However, the ABSI and P-Baux, although showing high measures of discrimination (AUROC 0.826 [0.737-0.916] and 0.848 [0.758-0.938]) in children), exceedingly overestimates mortality, indicating poor calibration. We highlight challenges in designing and employing scores that are applicable to a wide range of populations.

Nervous system involvement in Lyme disease often mimics other conditions and thus represents a diagnostic challenge, especially in an emergency department setting. We report a case of a female teenager presenting with sudden-onset aphasia and transient right-sided faciobrachial hemiplegia, along with headache and agitation. Ischemia, vasculitis, or another structural lesion was excluded by brain imaging. Toxicologic evaluation results were negative. Cerebral perfusion computed tomography and electroencephalography showed left parietotemporal brain dysfunction. Lumbar puncture result, although atypical, suggested bacterial infection and intravenous ceftriaxone was initiated. Finally, microbiological cerebrospinal fluid analysis revealed Lyme neuroborreliosis, showing specific intrathecal antibody production and high level of C-X-C motif chemokine 13. The patient rapidly recovered. To our knowledge, this report for the first time illustrates that acute-onset language and motor symptoms may be directly related to Lyme neuroborreliosis. Neuroborreliosis may mimic other acute neurologic events such as stroke and should be taken into diagnostic consideration even in the absence of classic symptoms and evolution.

PURPOSE OF REVIEW: To provide an overview of available evidence of the potential role of epigenetics in the pathogenesis of hypertension and vascular dysfunction. RECENT FINDINGS: Arterial hypertension is a highly heritable condition. Surprisingly, however, genetic variants only explain a tiny fraction of the phenotypic variation and the term 'missing heritability' has been coined to describe this phenomenon. Recent evidence suggests that phenotypic alteration that is unrelated to changes in DNA sequence (thereby escaping detection by classic genetic methodology) offers a potential explanation. Here, we present some basic information on epigenetics and review recent work consistent with the hypothesis of epigenetically induced arterial hypertension. SUMMARY: New technologies that enable the rigorous assessment of epigenetic changes and their phenotypic consequences may provide the basis for explaining the missing heritability of arterial hypertension and offer new possibilities for treatment and/or prevention.

AIMS: c-Met is an emerging biomarker in pancreatic ductal adenocarcinoma (PDAC); there is no consensus regarding the immunostaining scoring method for this marker. We aimed to assess the prognostic value of c-Met overexpression in resected PDAC, and to elaborate a robust and reproducible scoring method for c-Met immunostaining in this setting. METHODS AND RESULTS: c-Met immunostaining was graded according to the validated MetMab score, a classic visual scale combining surface and intensity (SI score), or a simplified score (high c-Met: ≥20% of tumour cells with strong membranous staining), in stage I-II PDAC. A computer-assisted classification method (Aperio software) was developed. Clinicopathological parameters were correlated with disease-free survival (DFS) and overall survival(OS). One hundred and forty-nine patients were analysed retrospectively in a two-step process. Thirty-seven samples (whole slides) were analysed as a pre-run test. Reproducibility values were optimal with the simplified score (kappa = 0.773); high c-Met expression (7/37) was associated with shorter DFS [hazard ratio (HR) 3.456, P = 0.0036] and OS (HR 4.257, P = 0.0004). c-Met expression was concordant on whole slides and tissue microarrays in 87.9% of samples, and quantifiable with a specific computer-assisted algorithm. In the whole cohort (n = 131), patients with c-Met(high) tumours (36/131) had significantly shorter DFS (9.3 versus 20.0 months, HR 2.165, P = 0.0005) and OS (18.2 versus 35.0 months, HR 1.832, P = 0.0098) in univariate and multivariate analysis. CONCLUSIONS: Simplified c-Met expression is an independent prognostic marker in stage I-II PDAC that may help to identify patients with a high risk of tumour relapse and poor survival.

BACKGROUND/AIMS: The determinants of a healthy diet have not been studied in Switzerland. This study aimed at assessing the individual and behavioural factors associated with a healthy diet in a Swiss city. METHODS: Cross-sectional, population-based study conducted between 2009 and 2013 (n = 4,439, 2,383 women, mean age 57.5 ± 10.3 years) in Lausanne. Food consumption was assessed using a validated food frequency questionnaire. Two Mediterranean diet scores (classic score and specific for Switzerland) and the Harvard School of Public Health alternate healthy eating index were computed. RESULTS: For all three dietary scores considered, living in couple or having a high education were associated with a healthier diet. An unhealthy lifestyle (smoking, sedentary behaviour) or a high body mass index were associated with an unhealthier diet. Participants born in Italy, Portugal and Spain had healthier diets than participants born in France or Switzerland. Women and elderly participants had healthier diets than men and young participants according to 2 scores, while no differences were found for the Swiss-specific Mediterranean score. CONCLUSIONS: In Switzerland, healthy eating is associated with high education, a healthy lifestyle, marital status and country of origin. The associations with gender and age depend on the dietary score considered.
