This invited editorial, reflecting on expectations of changing to graduate entry, eg enhanced maturity in the student cohort with greater self-sufficiency and taking of responsibility for learning in the context of adoption of a problem-based learning model, examines experiences of early post-change years and raises questions for contemplation by medical schools considering graduate entry.

“A memória não é um instrumento para explorar o passado, mas o seu teatro” (Walter Benjamin), cujos actores são pessoas comuns que recordam e recriam eventos passados através de diferentes formas de representação. Assim sendo, a memória é dinâmica e reconstrutiva e permite perpetuar a experiência, bem como criar e legitimar a identidade de cada um (Eber and Neal, 2001:37). O objectivo deste artigo é analisar os desenhos de Jack Sullivan sobre a comunidade das docas de Cardiff, conhecida como Butetown ou “Tiger Bay”, e a sua importância enquanto prática cultural na (re)construção de uma memória colectiva e imaginada. De que forma é que esta comunidade das docas é recordada e de que modo são as suas práticas quotidianas representadas através de desenhos? Quem e o que é que Sullivan representa? O que deixa na obscuridade? Como é que a memória individual se torna em memória colectiva? A este artigo subjaz a ideia de que os desenhos de Jack Sullivan, que estiveram na origem dos seus reconhecidos quadros compilados em Tramp Steamers, Seamen & Sailor Town, se apresentam como uma “forma de lembrança” de eventos passados, tanto para aqueles que viveram nesta área de Cardiff, como para aqueles que lhe eram espacial e temporalmente alheios. Estes desenhos constroem as memórias individuais e colectiva, tornando esta comunidade das docas acessível a todos.

Do Down ao UP… Percurso de sentimentos, emoções, procuras e certezas, por vezes, incertas mas alegre e esperançosamente desafiantes… O itinerário de investigação, e consequente formação, que foi encetado procurou, com o envolvimento familiar, as estratégias e dinâmicas mais eficazes para a promoção de aquisições relevantes e significativas para uma criança, filha do próprio investigador, com necessidades educativas especiais decorrentes do quadro de Trissomia 21. Através da procura objectiva de dados contextualizadores, foi delineado um programa de estimulação precoce aplicado nas rotinas familiares, as quais se constituíram como reais oportunidades/meios de um envolvimento familiar participante e corresponsável, facilitador e promotor de desenvolvimento das potencialidades e capacidades da própria criança. Com esta investigação pretendeu-se destacar a importância da intervenção/estimulação nas rotinas familiares constituídas ou a potencializar, como forma de obtenção de prometedores resultados em todas as áreas de desenvolvimento, veiculando os princípios da normalização, da funcionalidade, da contextualização e do envolvimento convergente de todo o agregado familiar.

O Brasil está em processo de convergência de sua contabilidade pública em relação aos padrões internacionais desenvolvidos pela Federação Internacional dos Contadores (Ifac). A implementação de sistemas de informação contábil é geralmente realizada por meio das abordagens top-down ou bottom-up. Assim, este estudo tem por objetivos: 1) identificar a abordagem adotada pelo governo federal brasileiro; 2) descrever o modelo de implementação do sistema de informação contábil público no Brasil; e 3) mapear o fluxo de informações e atores envolvidos no processo de convergência. A abordagem qualitativa foi adotada utilizando a pesquisa documental e análise de conteúdo de documentos disponíveis para operacionalizar a pesquisa. Foi identificado que o Brasil utiliza a abordagem middle-up-down, que favorece a interação entre múltiplos atores no processo, diferentemente da abordagem top-down, que segue o modelo internacional divulgado.

OBJETIVO: Analisar as opiniões de pais e profissionais sobre a sexualidade de pessoas com Síndrome de Down (SD) e identificar como essas pessoas percebem a própria sexualidade. MÉTODOS: Trata-se de pesquisa que utiliza análises quantitativa e qualitativa. Foram aplicados 809 questionários aos pais e aos profissionais (348). Foram realizados 12 grupos focais, com pais, profissionais e pessoas com SD. Foi realizado o teste qui-quadrado para análise das questões formuladas. RESULTADOS: Os pais tratam os filhos num padrão infantil de comportamento, pois temem assumir as conseqüências de um relacionamento sexual que pode resultar numa gravidez com risco de reincidência da síndrome. Os profissionais sentem-se despreparados para orientar sexualmente a pessoa com SD e harmonizar as atitudes dos pais aos desejos sexuais dos filhos. CONCLUSÕES: A sexualidade das pessoas com SD estrutura-se como nos demais seres humanos, embora seja vivenciada com restrições percebidas por elas mesmas a depender do contexto social no qual estão inseridas.

Background: A common task in analyzing microarray data is to determine which genes are differentially expressed across two (or more) kind of tissue samples or samples submitted under experimental conditions. Several statistical methods have been proposed to accomplish this goal, generally based on measures of distance between classes. It is well known that biological samples are heterogeneous because of factors such as molecular subtypes or genetic background that are often unknown to the experimenter. For instance, in experiments which involve molecular classification of tumors it is important to identify significant subtypes of cancer. Bimodal or multimodal distributions often reflect the presence of subsamples mixtures. Consequently, there can be genes differentially expressed on sample subgroups which are missed if usual statistical approaches are used. In this paper we propose a new graphical tool which not only identifies genes with up and down regulations, but also genes with differential expression in different subclasses, that are usually missed if current statistical methods are used. This tool is based on two measures of distance between samples, namely the overlapping coefficient (OVL) between two densities and the area under the receiver operating characteristic (ROC) curve. The methodology proposed here was implemented in the open-source R software. Results: This method was applied to a publicly available dataset, as well as to a simulated dataset. We compared our results with the ones obtained using some of the standard methods for detecting differentially expressed genes, namely Welch t-statistic, fold change (FC), rank products (RP), average difference (AD), weighted average difference (WAD), moderated t-statistic (modT), intensity-based moderated t-statistic (ibmT), significance analysis of microarrays (samT) and area under the ROC curve (AUC). On both datasets all differentially expressed genes with bimodal or multimodal distributions were not selected by all standard selection procedures. We also compared our results with (i) area between ROC curve and rising area (ABCR) and (ii) the test for not proper ROC curves (TNRC). We found our methodology more comprehensive, because it detects both bimodal and multimodal distributions and different variances can be considered on both samples. Another advantage of our method is that we can analyze graphically the behavior of different kinds of differentially expressed genes. Conclusion: Our results indicate that the arrow plot represents a new flexible and useful tool for the analysis of gene expression profiles from microarrays.

OBJECTIVE: To analyze whether previously identified risk factors for sudden death syndrome have a significant impact in a developing country. METHODS: Retrospective longitudinal case-control study carried out in Porto Alegre, Southern Brazil. Cases (N=39) were infants born between 1996 and 2000 who died suddenly and unexpectedly at home during sleep and were diagnosed with sudden death syndrome. Controls (N=117) were infants matched by age and sex who died in hospitals due to other conditions. Data were collected from postmortem examination records and questionnaires answers. A conditional logistic model was used to identify factors associated with the outcome. RESULTS: Mean age at death of cases was 3.2 months. The frequencies of infants regarding gestational age, breastfeeding and regular medical visits were similar in both groups. Sleeping position for most cases and controls was the lateral one. Supine sleeping position was found for few infants in both groups. Maternal variables, age below 20 years (OR=2, 95% CI: 1.1; 5.1) and smoking of more than 10 cigarettes per day during pregnancy (OR=3, 95% CI: 1.3; 6.4), significantly increased the risk for the syndrome. Socioeconomic characteristics were similar in both groups and did not affect risk. CONCLUSIONS: Infant-maternal and socioeconomic profiles of cases in a developing country closely resembled the profile described in the literature, and risk factors were similar as well. However, individual characteristics were identified as risks in the population studied, such as smoking during pregnancy and maternal age below 20 years.

OBJETIVO: Tendo como perspectiva a prática da integralidade, o objetivo do estudo foi analisar os fatores relacionados à atenção odontológica recebida por crianças e adolescentes com síndrome de Down. MÉTODOS: Foi realizado um estudo transversal com 112 pares de mães com filhos sindrômicos de 3 a 18 anos, recrutados em ambulatório de genética de um hospital público, sem atendimento odontológico local, no Rio de Janeiro (RJ), 2006. Os dados foram coletados por meio de um questionário aplicado às mães e do exame bucal dos filhos. Para análise dos dados utilizou-se a regressão logística múltipla. Analisou-se a "atenção odontológica da criança ou adolescente com síndrome de Down", em função de características demográficas, socioeconômicas e comportamentais. RESULTADOS: A maioria dos sindrômicos (79,5%) já tinha ido pelo menos uma vez ao dentista (IC 90%: 72,3; 87,8). A experiência odontológica das crianças foi associada às variáveis: mães que afirmaram receber orientação de algum profissional, que assiste seu filho, para que o levasse ao dentista (OR=6,1 [2,5; 15,1]), crianças/adolescentes com história prévia de cirurgia (OR=2,5 [0,9; 7,1]) e idade entre 12 e 18 anos (OR=13,1 [2,0; 86,9]). CONCLUSÕES: A atenção odontológica recebida pelas crianças e adolescentes com síndrome de Down foi relacionada à orientação dos profissionais de saúde que os assistem, caracterizando um atendimento integral por parte da equipe de saúde.

Existing work in the context of energy management for real-time systems often ignores the substantial cost of making DVFS and sleep state decisions in terms of time and energy and/or assume very simple models. Within this paper we attempt to explore the parameter space for such decisions and possible constraints faced.

OBJECTIVE To analyze if metabolic syndrome and its altered components are associated with demographic, socioeconomic and behavioral factors in fixed-shift workers.METHODS A cross-sectional study was conducted on a sample of 902 shift workers of both sexes in a poultry processing plant in Southern Brazil in 2010. The diagnosis of metabolic syndrome was determined according to the recommendations from Harmonizing the Metabolic Syndrome. Its frequency was evaluated according to the demographic (sex, skin color, age and marital status), socioeconomic (educational level, income and work shift), and behavioral characteristics (smoking, alcohol intake, leisure time physical activity, number of meals and sleep duration) of the sample. The multivariate analysis followed a theoretical framework for identifying metabolic syndrome in fixed-shift workers.RESULTS The prevalence of metabolic syndrome in the sample was 9.3% (95%CI 7.4;11.2). The most frequently altered component was waist circumference (PR 48.4%; 95%CI 45.5;51.2), followed by high-density lipoprotein. Work shift was not associated with metabolic syndrome and its altered components. After adjustment, the prevalence of metabolic syndrome was positively associated with women (PR 2.16; 95%CI 1.28;3.64), workers aged over 40 years (PR 3.90; 95%CI 1.78;8.93) and those who reported sleeping five hours or less per day (PR 1.70; 95%CI 1.09;2.24). On the other hand, metabolic syndrome was inversely associated with educational level and having more than three meals per day (PR 0.43; 95%CI 0.26;0.73).CONCLUSIONS Being female, older and deprived of sleep are probable risk factors for metabolic syndrome, whereas higher educational level and higher number of meals per day are protective factors for metabolic syndrome in fixed-shift workers.

ABSTRACT OBJECTIVE : To analyze if the demographic and socioeconomic variables, as well as percutaneous coronary intervention are associated with the use of medicines for secondary prevention of acute coronary syndrome. METHODS : In this cohort study, we included 138 patients with acute coronary syndrome, aged 30 years or more and of both sexes. The data were collected at the time of hospital discharge, and after six and twelve months. The outcome of the study was the simultaneous use of medicines recommended for secondary prevention of acute coronary syndrome: platelet antiaggregant, beta-blockers, statins and angiotensin-converting-enzyme inhibitor or angiotensin receptor blocker. The independent variables were: sex, age, education in years of attending, monthly income in tertiles and percutaneous coronary intervention. We described the prevalence of use of each group of medicines with their 95% confidence intervals, as well as the simultaneous use of the four medicines, in all analyzed periods. In the crude analysis, we verified the outcome with the independent variables for each period through the Chi-square test. The adjusted analysis was carried out using Poisson Regression. RESULTS : More than a third of patients (36.2%; 95%CI 28.2;44.3) had the four medicines prescribed at the same time, at the moment of discharge. We did not observe any differences in the prevalence of use in comparison with the two follow-up periods. The most prescribed class of medicines during discharge was platelet antiaggregant (91.3%). In the crude analysis, the demographic and socioeconomic variables were not associated to the outcome in any of the three periods. CONCLUSIONS : The prevalence of simultaneous use of medicines at discharge and in the follow-ups pointed to the under-utilization of this therapy in clinical practice. Intervention strategies are needed to improve the quality of care given to patients that extend beyond the hospital discharge, a critical point of transition in care.

A Síndrome de Brown (SB) é uma síndrome anatómica e restritiva. Segundo Brown (1942), esta síndrome é classificada como uma ausência de elevação em adução, uma síndrome restritiva do grande oblíquo. Pode ser congénita, adquirida e iatrogénica e divide-se como ligeira, severa ou profunda. O Pequeno Oblíquo (PO) tem como acções a exciclodução, elevação e abdução. Ao estar afectado, observa-se nos movimentos oculares limitação da elevação e adução, que induz uma paralisia deste músculo. Assim, as duas apresentam ausência de elevação em adução, sendo importante, a realização de diagnóstico diferencial entre elas. Objectivos: Identificar as diversas características motoras e sensoriais da paralisia do PO comparando-as com as características da SB, especificando os testes de ortóptica adequados. Apresentar o diagnóstico diferencial a realizar, explicitando as características principais dos exames coordimétricos e do teste das ducções forçadas.

ABSTRACT OBJECTIVE To determine the prevalence of metabolic syndrome and its components in Brazilian adolescents. METHODS We evaluated 37,504 adolescents who were participants in the Study of Cardiovascular Risks in Adolescents (ERICA), a cross-sectional, school-based, national study. The adolescents, aged from 12 to 17 years, lived in cities with populations greater than 100,000 inhabitants. The sample was stratified and clustered into schools and classes. The criteria set out by the International Diabetes Federation were used to define metabolic syndrome. Prevalences of metabolic syndrome were estimated according to sex, age group, school type and nutritional status. RESULTS Of the 37,504 adolescents who were evaluated: 50.2% were female; 54.3% were aged from 15 to 17 years, and 73.3% were from public schools. The prevalence of metabolic syndrome was 2.6% (95%CI 2.3-2.9), slightly higher in males and in those aged from 15 to 17 years in most macro-regions. The prevalence was the highest in residents from the South macro-region, in the younger female adolescents and in the older male adolescents. The prevalence was higher in public schools (2.8% [95%CI 2.4-3.2]), when compared with private schools (1.9% [95%CI 1.4-2.4]) and higher in obese adolescents when compared with nonobese ones. The most common combinations of components, referring to 3/4 of combinations, were: enlarged waist circumference (WC), low HDL-cholesterol (HDL-c) and high blood pressure; followed by enlarged WC, low HDL-c and high triglycerides; and enlarged WC, low HDL-c, high triglycerides and blood pressure. Low HDL was the second most frequent component, but the highest prevalence of metabolic syndrome (26.8%) was observed in the presence of high triglycerides. CONCLUSIONS ERICA is the first Brazilian nation-wide study to present the prevalence of metabolic syndrome and describe the role of its components. Despite the prevalence of Metabolic Syndrome being low, the high prevalences of some components and participation of others in the syndrome composition shows the importance of early diagnosis of this changes, even if not grouped within the metabolic syndrome.

Stool samples of 157 patients with AIDS, living in the county of São Paulo, were submitted to several techniques in the search for Cryptosporidium sp.. Among the various techniques tested for slide preparation (direct smear, spontaneous sedimentation method, and formol-ether concentration), the latter, formol-ether concentration, offered the best results, clearly outdoing all the others. Nineteen samples out of 157 prepared by this technique, after dyeing by the Kinyoun method or by carbol fuchsin dimethyl sulfoxide, were found to be positive for Cryptosporidium sp..
