El objetivo de este estudio es analizar el impacto, en emisiones de CO2, de la demanda final de Cataluña en relación a los vínculos comerciales interregionales con el resto de España y el resto del mundo. Este proceso implica el análisis del balance en CO2 incorporado para Cataluña, lo que permitirá evaluar la responsabilidad de la economía catalana respecto a estas emisiones. Para este propósito se construye, para esta determinada desagregación regional, un modelo Multi-Regional Input-Output (MRIO) extendido al medioambiente con sectores verticalmente integrados. La incorporación de la técnica de la integración vertical nos permite un enfoque alternativo para el Balance Neto y un análisis más detallado de los vínculos interregionales entre los diversos sectores productivos, centrado en la responsabilidad última de la demanda final de cada sector en cada región. Hasta el momento, los estudios previos sobre los impactos medioambientales incorporados al comercio español se han centrado principalmente en el ámbito nacional. No obstante, por un lado el comercio interregional con el resto de España en términos monetarios representa cerca de la mitad del comercio exterior catalán. Por otro lado, los distintos metabolismos energéticos de ambas economías tienen como consecuencia una importante diferencia en la intensidad de emisión en la producción de bienes y servicios. Esta situación genera para Cataluña un déficit en el Balance Neto estimado con el resto de España, aún teniendo un importante superávit monetario. De esto se desprende la importancia de integrar el nivel interregional en los estudios de los impactos medioambientales incorporados en el comercio y, en consecuencia, en la planificación y formalización de políticas económicas y ambientales a nivel nacional.

It has been proposed that the number of tropical cyclones as a function of the energy they release is a decreasing power-law function, up to a characteristic energy cutoff determined by the spatial size of the ocean basin in which the storm occurs. This means that no characteristic scale exists for the energy of tropical cyclones, except for the finite-size effects induced by the boundaries of the basins. This has important implications for the physics of tropical cyclones. We discuss up to what point tropical cyclones are related to critical phenomena (in the same way as earthquakes, rainfall, etc.), providing a consistent picture of the energy balance in the system. Moreover, this perspective allows one to visualize more clearly the effects of global warming on tropical-cyclone occurrence.

BACKGROUND AND AIMS: Critically ill patients with complicated evolution are frequently hypermetabolic, catabolic, and at risk of underfeeding. The study aimed at assessing the relationship between energy balance and outcome in critically ill patients. METHODS: Prospective observational study conducted in consecutive patients staying > or = 5 days in the surgical ICU of a University hospital. Demographic data, time to feeding, route, energy delivery, and outcome were recorded. Energy balance was calculated as energy delivery minus target. Data in means+/-SD, linear regressions between energy balance and outcome variables. RESULTS: Forty eight patients aged 57+/-16 years were investigated; complete data are available in 669 days. Mechanical ventilation lasted 11+/-8 days, ICU stay 15+/-9 was days, and 30-days mortality was 38%. Time to feeding was 3.1+/-2.2 days. Enteral nutrition was the most frequent route with 433 days. Mean daily energy delivery was 1090+/-930 kcal. Combining enteral and parenteral nutrition achieved highest energy delivery. Cumulated energy balance was between -12,600+/-10,520 kcal, and correlated with complications (P < 0.001), already after 1 week. CONCLUSION: Negative energy balances were correlated with increasing number of complications, particularly infections. Energy debt appears as a promising tool for nutritional follow-up, which should be further tested. Delaying initiation of nutritional support exposes the patients to energy deficits that cannot be compensated later on.

This collection shows that even though the European Convention on Human Rights (ECHR) does not explicitly refer to older persons nor to age discrimination, the Strasbourg Court can play an important role as a guardian of the rights of older people. A major goal in the treatment of acute ischemia of a vascular territory is to restore blood flow to normal values, i.e. to "reperfuse" the ischemic vascular bed. However, reperfusion of ischemic tissues is associated with local and systemic leukocyte activation and trafficking, endothelial barrier dysfunction in postcapillary venules, enhanced production of inflammatory mediators and great lethality. This phenomenon has been referred to as "reperfusion injury" and several studies demonstrated that injury is dependent on neutrophil recruitment. Furthermore, ischemia and reperfusion injury is associated with the coordinated activation of a series of cytokines and adhesion molecules. Among the mediators of the inflammatory cascade released, TNF-alpha appears to play an essential role for the reperfusion-associated injury. On the other hand, the release of IL-10 modulates pro-inflammatory cytokine production and reperfusion-associated tissue injury. IL-1beta, PAF and bradykinin are mediators involved in ischemia and reperfusion injury by regulating the balance between TNF-alpha and IL-10 production. Strategies that enhance IL-10 and/or prevent TNF-alpha concentration may be useful as therapeutic adjuvants in the treatment of the tissue injury that follows ischemia and reperfusion.

Law and science have partnered together in the recent past to solve major public health issues, ranging from asbestos to averting the threat of a nuclear holocaust. This paper travels to a legal and health policy frontier where no one has gone before, examining the role of precautionary principles under international law as a matter of codified international jurisprudence by examining draft terminology from prominent sources including the Royal Commission on Environmental Pollution (UK), the Swiss Confederation, the USA (NIOSH) and the OECD. The research questions addressed are how can the benefits of nanotechnology be realized, while minimizing the risk of harm? What law, if any, applies to protect consumers (who comprise the general public, nanotechnology workers and their corporate social partners) and other stakeholders within civil society from liability? What law, if any, applies to prevent harm?

Every day, hospital doctors spend time at conducting ward rounds. Rounds are a core clinical activity during which doctors interact with patients, synthetise a whole set of informations and make many decisions. In addition, rounds can become a crucial teaching moment, when a trainee gets supervised by an attending physician. However, litterature on the topic of rounds is scarce. This paper summarizes the results of the few key studies focusing on ward rounds. The results are presented in four sections, each one being dedicated to one of the round stakeholders: the trainee or resident, the trainer, the patient and the nurse. An emphasis is put on ward rounds involving both a trainee and a trainer, since such rounds always mean striking a balance between care and teaching.
