In the field of psychiatry semi-structured interview is one of the central tools in assessing the psychiatric state of a patient. In semi-structured interview the interviewer participates in the interaction both by the prepared interview questions and by his or her own, unstructured turns. It has been stated that in the context of psychiatric assessment interviewers' unstructured turns help to get focused information but simultaneously may weaken the reliability of the data. This study examines the practices by which semi-structured psychiatric interviews are conducted. The method for the study is conversation analysis, which is both a theory of interaction and a methodology for its empirical, detailed analysis. Using data from 80 video-recorded psychiatric interviews with 16 patients and five interviewers it describes in detail both the structured and unstructured interviewing practices. In the analysis also psychotherapeutic concepts are used to describe phenomena that are characteristic for therapeutic discourse. The data was received from the Helsinki Psychotherapy Study (HPS). HPS is a randomized clinical trial comparing the effectiveness of four forms of psychotherapy in the treatment of depressive and anxiety disorders. A total of 326 patients were randomly assigned to one of three treatment groups: solution-focused therapy, short-term psychodynamic psychotherapy, and long-term psychodynamic psychotherapy. The patients assigned to the long-term psychodynamic psychotherapy group and 41 patients self-selected for psychoanalysis were included in a quasi-experimental design. The primary outcome measures were depressive and anxiety symptoms, while secondary measures included work ability, need for treatment, personality functions, social functioning, and life style. Cost-effectiveness was determined. The data were collected from interviews, questionnaires, psychological tests, and public health registers. The follow-up interviews were conducted five times during a 5-year follow-up. The study shows that interviewers pose elaborated questions that are formulated in a friendly and sensitive way and that make relevant patients' long and story-like responses. When receiving patients' answers interviewers use a wide variety of different interviewing practices by which they direct patients' talk or offer an understanding of the meaning of patients' response. The results of the study are two-fold. Firstly, the study shows that understanding the meaning of mental experiences requires interaction between interviewer and patient. It is stated that therefore semi-structured interview is both relevant and necessary method for collecting data in psychotherapy outcome study. Secondly, the study suggests that conversation analysis, enriched with psychotherapeutic concepts, offers methodological possibilities for psychotherapy process research, especially for process-outcome paradigm.

The present paper describes the development and evaluation of a standardized multi-component therapist training program in guided respiration mindfulness therapy (GRMT). GMRT is a manual-based, experimental clinical intervention involving concentrated focus on sustained self-regulation of breathing, application of mindfulness to emergent somatic experience and relaxation. Therapists (n = 61) new to the approach attended a 2-day experiential workshop and were evaluated pre-post workshop for change in intervention knowledge, as well as change in mindfulness. These trainees also participated in post-workshop focus group sessions to explore perception of the intervention. A subset of 40 therapists participated in a second training component, and 14 of these were rated for competent delivery of the intervention during participation in a clinical trial. During training, therapists personally received the treatment giving the opportunity to assess treatment session (n = 283) impact on sense of wellbeing. Results indicated a brief focused training program can equip therapists with basic knowledge and skills required to deliver the standardized manual-based treatment. Qualitative analysis of focus group sessions showed that therapists endorsed the intervention for clinical use and found it personally beneficial. This research provides a foundation for further evaluation of clinical effectiveness of the intervention.

Background: Type 2 diabetes is linked to several complications which add to both physical and mental distress. Depression is a common co-morbidity of diabetes which can occur both as a cause and a consequence of type 2 diabetes. Depression has been shown to correlate with glucose regulation and treating depression might prove beneficial for glucose regulation as well as for mental well being. Another complication which might affect diabetes management is cognitive decline. Several risk factors and complications of diabetes might modify the risk for developing cognitive impairment, which is increased 1.5 times among subjects with type 2 diabetes. Type 2 diabetes, depression and impaired cognitive performance have all been linked to low birth weight. This thesis aimed to explore the effects and interactions of birth weight, depression and cognitive ability in relation to type 2 diabetes from a life course perspective. Subjects and methods: Studies I, II and V were part of the Helsinki Birth Cohort Study. 2003 subjects participated in an extensive clinical examination at an average age of 61 years. A standard glucose tolerance test (OGTT) was performed and depressive symptoms were assessed using the Beck Depression Inventory (BDI). In addition data was obtained from child welfare clinics and national registers. A subset of the cohort (n=1247) also performed a test on cognitive performance (CogState ®) at the average age of 64. Studies III and IV were randomised clinical trials where mildly depressed diabetic subjects were treated with paroxetine or placebo and the effect on metabolic parameters and quality of life was assessed. The first trial included 14 women and lasted 10 weeks, while the second trial included 43 subjects, both men and women, and lasted 6 months. Results: Type 2 diabetes was positively associated with the occurrence of depressive symptoms. Among diabetic subjects 23.6% had depressive symptoms, compared to 16.7% of subjects with normal glucose tolerance (OR = 1.77, p<0.001). Formal mediation analysis revealed that cardiovascular disease (CVD) is likely to act as a mediator in the association. Furthermore, low birth weight was found to modify the association between type 2 diabetes, CVD and depression. The association between BDI score and having type 2 diabetes or CVD was twice as strong in the subgroup with low birth weight (≤ 2500g) compared with the group with birth weight > 2500g (p for interaction 0.058). In the six months long randomised clinical trial (study IV) paroxetine had a transient beneficial effect on glycosylated haemoglobin A1c (GHbA1c) and quality of life when compared to placebo after three months of treatment. In study V we found that subjects with known diabetes had a consistently poorer level of cognitive performance than subjects with normal glucose tolerance in most of the tested cognitive domains. This effect was further amplified among those born with a small birth weight (p for interaction 0.002). Conclusions: Type 2 diabetes is associated with a higher occurrence of depressive symptoms compared to subjects with normal glucose tolerance. This association is especially strong among subjects with CVD and those born with a low birth weight. Treating depressed diabetic subjects with paroxetine has no long term effect on glucose regulation. Physicians should be aware of depression as an important co-morbidity of type 2 diabetes. Both depression and the cognitive decline often seen among diabetic subjects are increased if the subject is born with a low birth weight. Physicians should recognise low birth weight as an additional risk factor and modifier of diabetic complications.
Improving outcome of childhood bacterial meningitis by simplified treatment : Experience from Angola

Background Acute bacterial meningitis (BM) continues to be an important cause of childhood mortality and morbidity, especially in developing countries. Prognostic scales and the identification of risk factors for adverse outcome both aid in assessing disease severity. New antimicrobial agents or adjunctive treatments - except for oral glycerol - have essentially failed to improve BM prognosis. A retrospective observational analysis found paracetamol beneficial in adult bacteraemic patients, and some experts recommend slow β-lactam infusion. We examined these treatments in a prospective, double-blind, placebo-controlled clinical trial. Patients and methods A retrospective analysis included 555 children treated for BM in 2004 in the infectious disease ward of the Paediatric Hospital of Luanda, Angola. Our prospective study randomised 723 children into four groups, to receive a combination of cefotaxime infusion or boluses every 6 hours for the first 24 hours and oral paracetamol or placebo for 48 hours. The primary endpoints were 1) death or severe neurological sequelae (SeNeSe), and 2) deafness. Results In the retrospective study, the mortality of children with blood transfusion was 23% (30 of 128) vs. without blood transfusion 39% (109 of 282; p=0.004). In the prospective study, 272 (38%) of the children died. Of those 451 surviving, 68 (15%) showed SeNeSe, and 12% (45 of 374) were deaf. Whereas no difference between treatment groups was observable in primary endpoints, the early mortality in the infusion-paracetamol group was lower, with the difference (Fisher s exact test) from the other groups at 24, 48, and 72 hours being significant (p=0.041, 0.0005, and 0.005, respectively). Prognostic factors for adverse outcomes were impaired consciousness, dyspnoea, seizures, delayed presentation, and absence of electricity at home (Simple Luanda Scale, SLS); the Bayesian Luanda Scale (BLS) also included abnormally low or high blood glucose. Conclusions New studies concerning the possible beneficial effect of blood transfusion, and concerning longer treatment with cefotaxime infusion and oral paracetamol, and a study to validate our simple prognostic scales are warranted.

It is being realized that the traditional closed-door and market driven approaches for drug discovery may not be the best suited model for the diseases of the developing world such as tuberculosis and malaria, because most patients suffering from these diseases have poor paying capacity. To ensure that new drugs are created for patients suffering from these diseases, it is necessary to formulate an alternate paradigm of drug discovery process. The current model constrained by limitations for collaboration and for sharing of resources with confidentiality hampers the opportunities for bringing expertise from diverse fields. These limitations hinder the possibilities of lowering the cost of drug discovery. The Open Source Drug Discovery project initiated by Council of Scientific and Industrial Research, India has adopted an open source model to power wide participation across geographical borders. Open Source Drug Discovery emphasizes integrative science through collaboration, open-sharing, taking up multi-faceted approaches and accruing benefits from advances on different fronts of new drug discovery. Because the open source model is based on community participation, it has the potential to self-sustain continuous development by generating a storehouse of alternatives towards continued pursuit for new drug discovery. Since the inventions are community generated, the new chemical entities developed by Open Source Drug Discovery will be taken up for clinical trial in a non-exclusive manner by participation of multiple companies with majority funding from Open Source Drug Discovery. This will ensure availability of drugs through a lower cost community driven drug discovery process for diseases afflicting people with poor paying capacity. Hopefully what LINUX the World Wide Web have done for the information technology, Open Source Drug Discovery will do for drug discovery. (C) 2011 Elsevier Ltd. All rights reserved.

Introduction: For over half a century now, the dopamine hypothesis has provided the most widely accepted heuristic model linking pathophysiology and treatment in schizophrenia. Despite dopaminergic drugs being available for six decades, this system continues to represent a key target in schizophrenia drug discovery. The present article reviews the scientific rationale for dopaminergic medications historically and the shift in our thinking since, which is clearly reflected in the investigational drugs detailed. Areas covered: We searched for investigational drugs using the key words `dopamine,' `schizophrenia,' and `Phase II' in American and European clinical trial registers (clinicaltrials. gov; clinicaltrialsregister.eu), published articles using National Library of Medicine's PubMed database, and supplemented results with a manual search of cross-references and conference abstracts. We provide a brief description of drugs targeting dopamine synthesis, release or metabolism, and receptors (agonists/partial agonists/antagonists). Expert opinion: There are prominent shifts in how we presently conceptualize schizophrenia and its treatment. Current efforts are not as much focused on developing better antipsychotics but, instead, on treatments that can improve other symptom domains, in particular cognitive and negative. This new era in the pharmacotherapy of schizophrenia moves us away from the older `magic bullet' approach toward a strategy fostering polypharmacy and a more individualized approach shaped by the individual's specific symptom profile.

Introduction: Antipsychotic drugs date back to the 1950s and chlorpromazine. Soon after, it was established that blockade of dopamine and, in particular, the D-2 receptor was central to this effect. Dopamine continues to represent a critical line of investigation, although much of the work now focuses on its potential in other symptom domains. Areas covered: A search was carried out for investigational drugs using the key words `dopamine', `schizophrenia' and `Phase III' in an American clinical trial registry (clinicaltrials.gov), published articles using the National Library of Medicine's PubMed database, and supplemented results with a manual search of cross-references and conference abstracts. Drugs were excluded that were already FDA approved. Expert opinion: There remains interest, albeit diminished, in developing better antipsychotic compounds. The greatest enthusiasm currently centres on dopamine's role in negative and cognitive symptom domains. With theories conceptualising hypodopaminergic activity as underlying these deficits, considerable effort is focused on drug strategies that will enhance dopamine activity. Finally, a small body of research is investigating dopaminergic compounds vis-a-vis side-effect treatments. In domains beyond psychosis, however, dopamine arguably is not seen as so central, reflected in considerable research following other lines of investigation.

Tuberculosis continues to kill 1.4 million people annually. During the past 5 years, an alarming increase in the number of patients with multidrug-resistant tuberculosis and extensively drug-resistant tuberculosis has been noted, particularly in eastern Europe, Asia, and southern Africa. Treatment outcomes with available treatment regimens for drug-resistant tuberculosis are poor. Although substantial progress in drug development for tuberculosis has been made, scientific progress towards development of interventions for prevention and improvement of drug treatment outcomes have lagged behind. Innovative interventions are therefore needed to combat the growing pandemic of multidrug-resistant and extensively drug-resistant tuberculosis. Novel adjunct treatments are needed to accomplish improved cure rates for multidrug-resistant and extensively drug-resistant tuberculosis. A novel, safe, widely applicable, and more effective vaccine against tuberculosis is also desperately sought to achieve disease control. The quest to develop a universally protective vaccine for tuberculosis continues. So far, research and development of tuberculosis vaccines has resulted in almost 20 candidates at different stages of the clinical trial pipeline. Host-directed therapies are now being developed to refocus the anti-Mycobacterium tuberculosis-directed immune responses towards the host; a strategy that could be especially beneficial for patients with multidrug-resistant tuberculosis or extensively drug-resistant tuberculosis. As we are running short of canonical tuberculosis drugs, more attention should be given to host-directed preventive and therapeutic intervention measures.

Multidrug resistance is a major therapeutic challenge faced in the conventional chemotherapy. Nanocarriers are beneficial in the transport of chemotherapeutics by their ability to bypass the P-gp efflux in cancers. Most of the P-gp inhibitors under phase II clinical trial are facing failures and hence there is a need to develop a suitable carrier to address P-gp efflux in cancer therapy. Herein, we prepared novel protamine and carboxymethyl cellulose polyelectrolyte multi-layered nanocapsules modified with Fe3O4 nanoparticles for the delivery of doxorubicin against highly drug resistant HeLa cells. The experimental results revealed that improved cellular uptake, enhanced drug intensity profile with greater percentage of apoptotic cells was attained when doxorubicin loaded magnetic nanocapsules were used in the presence of external magnetic field. Hence, we conclude that this magnetic field assisted nanocapsule system can be used for delivery of chemotherapeutics for potential therapeutic efficacy at minimal dose in multidrug resistant cancers. From the Clinical Editor: Many cancer drugs fail when cancer cells become drug resistant. Indeed, multidrug resistance (MDR) is a major therapeutic challenge. One way that tumor cells attain MDR is by over expression of molecular pumps comprising of P-glycoprotein (P-gp) and multidrug resistant proteins (MRP), which can expel chemotherapeutic drugs out of the cells. In this study, the authors prepared novel protamine and carboxymethyl cellulose polyelectrolyte multi-layered nanocapsules modified with Fe3O4 nanoparticles for the delivery of doxorubicin. Estudo de natureza qualitativa, descritiva e exploratória, que teve como objeto o raciocínio clínico elaborado pelos enfermeiros ao cuidarem de feridas em clientes com afecções oncológicas. Os objetivos traçados para o estudo foram: identificar as estratégias cognitivas que os enfermeiros com especialização em área oncológica consideram adotar para o estabelecimento de um julgamento clínico na avaliação de feridas em clientes com afecções oncológicas e caracterizar as etapas de elaboração mental para construção do raciocínio clínico que os enfermeiros consideram percorrer quando da avaliação de feridas em cliente acometido por afecções oncológicas. O campo de pesquisa foi o Instituto Nacional do Câncer, no qual os cenários de coleta foi a unidade HC-I nas Seções de Oncologia Clínica, Neurocirurgia, Abdominopélvica, Centro de Tratamento Intensivo e Cirurgia de Cabeça e Pescoço. Os sujeitos do estudo foram treze enfermeiros com especialização em oncologia, que assistiam clientes com afecções oncológicas, há pelo menos cinco anos. A coleta dos dados aconteceu nos meses de junho e julho de 2009, sendo utilizado um roteiro de entrevista semi-estruturada para captar as informações. A análise dos dados foi realizada com base no método de análise de conteúdo, que ao ser aplicado possibilitou a apreensão de quatro categorias: (1) a afecção oncológica como fator expressivo na elaboração mental diagnóstica do enfermeiro; (2) a relevância do conhecimento teórico-prático avançado para a elaboração mental avaliativa do enfermeiro; (3) a construção da elaboração mental para o raciocínio clínico diagnóstico do enfermeiro; (4) a importância da interação humana no contexto avaliativo. Concluiu-se que, este estudo identificou, na discussão das categorias, as quatro principais estratégias cognitivas que os enfermeiros com especialização em área oncológica consideram adotar para o estabelecimento de um julgamento clínico na avaliação de feridas em clientes com afecções oncológicas e caracterizou as etapas de elaboração mental para construção do raciocínio clínico diagnóstico do tipo hipotético-dedutivo e intuitivo. Compreendeu-se que os sujeitos da pesquisa detêm qualidades intelectuais específicas e avançadas, quando elaboram diagnósticos e intervenções baseadas nas respostas humanas em situação de avaliação de feridas nos clientes com doença oncológica, e foi considerado que o ensino pode impulsionar o desenvolvimento das competências cognitivas no sentido de formar profissionais capazes de avaliar o próprio conhecimento, bem como a fomentação de novas pesquisas relativas a essa temática imprescindível para uma assistência de enfermagem qualificada.

O uso de primers autocondicionantes tem sido proposto como uma alternativa para a redução de passos clínicos. O objetivo deste estudo clínico aleatório e controlado foi avaliar a performance de um sistema autocondicionante (Transbond Plus Self-Etching Primer, 3M Unitek - SEP) comparado a um sistema multipasso convencional (Transbond XT, 3M Unitek - TBXT) de colagem ortodôntica, durante um período de 12 meses. Vinte e oito pacientes participaram deste estudo, sendo estes alocados aos grupos SEP ou TBXT de forma aleatória, através de randomização em bloco. Um total de 548 bráquetes metálicos (Micro-Arch, prescrição Alexander, GAC International, Bohemia, NY) foram colados com uso da pasta adesiva Transbond XT (3M Unitek), sendo todos os produtos manuseados de acordo com as recomendações dos fabricantes. Foram totalizados 276 bráquetes no grupo controle e 272 no segundo. Curvas de sobrevivência Kaplan-Meier e o teste log-rank (p<0,05) foram utilizados para comparar o percentual de falhas adesivas para as duas técnicas. Ao final do período foram verificadas trinta e duas falhas adesivas (bráquetes descolados), sendo 19 (6,98%) falhas quando utilizado o primer autocondicionante (SEP) e 13 (4,71%) quando usado o primer convencional (TBXT). Não houve diferença significante entre a sobrevivência dos bráquetes entre os dois grupos avaliados (log-rank test, p=0,311). Quando a influência de gênero do paciente, arco dentário e tipo dentário (anterior ou posterior) foram analisados, somente o tipo dentário foi achado significante. Bráquetes de dentes posteriores apresentaram uma maior probabilidade de falha adesiva que os colados em dentes anteriores (p=0,013) Os autores concluem que o primer autocondicionante pode ser utilizado para colagem direta de bráquetes ortodônticos sem que sua sobrevivência clínica seja afetada.

Objetivo: Avaliar o efeito da intervenção dietética individualizada sobre o diagnóstico nutricional e controle metabólico em diabéticos tipo 2 sedentários Casuística e Métodos: Trata-se de um ensaio clínico controlado e prospectivo com 80 adultos, de ambos dos sexos, com Diabetes Mellitus tipo 2 divididos em GI (grupo intervenção: 40 indivíduos submetidos à intervenção dietética e a utilização de hipoglicemiante) e GC (grupo controle: 40 indivíduos submetidos à medicação hipoglicemiante). Foi realizada intervenção dietética individualizada por três meses baseando-se nas recomendações da American Diabetes Association (2002). Foram analisadas as variáveis antropométricas: massa corporal total (MCT), estatura com determinação do Índice de Massa Corporal (IMC) e perímetro da cintura (PC); as variáveis bioquímicas glicemia, colesterol total, LDL-colesterol, HDL-colesterol, triglicerídeos (TG) e hemoglobina glicada (HbA1c) e as variáveis dietéticas energia, proteínas, carboidratos, lipídeos, colesterol e fibras alimentares. Para estatística inferencial foi utilizado o Anova two-way com nível de significância de 95%. Resultados: Na análise intergrupos, o GC apresentou aumento nas variáveis: MCT (Δ%=0,78; p=0,014), IMC (Δ%=0,76; p=0,012), PC (Δ%=0,75; p=0,019) enquanto que o GI apresentou redução nas variáveis: MCT (Δ%=-3,71; p<0,001), IMC (Δ%=-3,77; p<0,001), PC (Δ%=-3,98; p<0,001). Na comparação da média do IR intergrupos, observou-se diferença nas variáveis: energia (p<0,001), lipídeos (p=0,012), gorduras saturadas (p<0,001); colesterol dietético (p=0,006); fibras alimentares (p=0,001); glicemia (p<0,001), colesterol total (p<0,001), LDL-colesterol (p<0,001) e HbA1c (p<0,001).Conclusão: A intervenção dietética foi eficiente em melhorar o perfil antropométrico e o controle metabólico dos diabéticos tipo 2 sedentários.

O estudo acompanha a rotina de um ensaio clínico de vacinas experimentais anti-HIV/AIDS desenvolvido no Projeto Praça Onze, da UFRJ, em colaboração com a rede mundial de pesquisas de vacinas anti-HIV/AIDS, HVTN (HIV Vaccine Trials Network) e apoio financeiro dos NIH (National Institutes of Health), dos EUA. Focaliza os processos de recrutamento, seleção e seguimento dos voluntários, articulando os aspectos mais gerais às particularidades e demandas locais deste estudo realizado no Rio de Janeiro, Brasil. Do ponto de vista teórico-conceitual utiliza recursos oriundos das ciências sociais - história, sociologia da ciência e da corrente interacionista da sociologia. O estudo mostra que, como todo ensaio clínico, o experimento do Projeto Praça Onze é uma atividade coletiva, sustentada sobre múltiplos atores e instâncias, com diversas racionalidades, o que gera uma permanente tensão. Estuda a tensão que se coloca, principalmente entre a expectativa dos profissionais envolvidos (aconselhadores, recrutadores, médicos, enfermeiros, farmacêuticos, gerentes e coordenadores de estudos) e a experiência vivenciada pelos voluntários, assim como as possíveis negociações propostas durante o desenvolvimento do protocolo, a partir do contexto sociopolítico onde o mesmo está inserido. Os profissionais, apesar de suas especificidades, articulam-se para incorporar do universo dos colegas suas linguagens e seus modos de agir, num processo de hibridização dos conhecimentos e práticas. Os voluntários do ensaio, por sua vez, submetem-se, por um lado a uma rotina de disciplina e compromisso, que implica na entrega de seu corpo à medicina, implícita nesse papel. Por outro, apropriam-se da lógica do ensaio clínico para, em sua ótica, transformar-se em um coadjuvante no descobrimento de uma nova biotecnologia, que neste caso, se coloca como uma questão de vida ou morte, para ele e para uma ampla comunidade de infectados pelo HIV. Para o voluntário a entrega do corpo à ciência faz parte da construção de uma bioidentidade e de uma biopolítica contemporânea, onde não se pode desconsiderar a constante inter-relação entre ciência, sociedade, técnica e política.

[Es]Introducción: El manejo óptimo de la vía aérea extrahospitalaria es todavía incierto. Los dispositivos supraglóticos y la intubación endotraqueal han sido utilizados en los últimos años por los servicios de emergencia, pero no se conoce aún si el uso de los nuevos dispositivos supraglóticos mejora la supervivencia. Objetivo: Determinar la supervivencia a corto plazo (igual o menor a 1 mes) entre los dispositivos supraglóticos y la intubación endotraqueal en la parada cardíaca extrahospitalaria. Metodología: Se realizó una revisión bibliográfica en las bases de datos de Cochrane, Pubmed, MEDES, Scopus, CINAHL, Science Direct e IBECS y una búsqueda manual en las revistas Emergencias, Prehospital Emergency Care y Annals Emergency Medicine de estudios comprendidos entre los años 2004- 2014 que comparasen la supervivencia en la parada cardíaca extrahospitalaria del adulto entre los dispositivos supraglóticos y la intubación endotraqueal. Resultados: Se identificaron 9 estudios elegibles: 2 revisiones sistemáticas (una con metaanálisis), 1 ensayo clínico aleatorizado y 6 estudios de cohortes. 6 de los estudios mostraron mejores resultados en la intubación endotraqueal, 2 en los que no hubo diferencias y uno de ellos mostró mejores resultados en los dispositivos supraglóticos. Conclusiones: La intubación endotraqueal proporciona mayor supervivencia que los dispositivos supraglóticos en la parada cardíaca extrahospitalaria en adultos. Los dispositivos supraglóticos deberían utilizarse por parte de personal con poca experiencia en el uso de la intubación endotraqueal o como alternativa a la intubación fallida o con dificultad.

Introducción y objetivo: El collarín cervical es un dispositivo que tiene como objetivo disminuir el movimiento del cuello para evitar lesiones secundarias en el manejo del paciente traumático en el ámbito prehospitalario. Mediante la realización de esta revisión sistemática se ha pretendido evaluar si la colocación del collarín cervical disminuye la movilidad del cuello en el paciente traumático, así como determinar si puede producir o evitar lesiones durante su manejo. Metodología: Revisión sistemática en base a las disposiciones PRISMA. Se elaboró un protocolo de búsqueda que se utilizó en cuatro bases de datos (Medline, Scopus, CINAHL y Web of Science) y se incluyeron ensayos clínicos y estudios observacionales publicados entre enero de 1995 y diciembre de 2014. Resultados: La revisión se realizó a partir de 10 ensayos clínicos no aleatorizados de modesta calidad metodológica: en 6 se utilizaron cadáveres con lesión cervical y en los otros 4 voluntarios sanos sin lesión cervical y un ensayo clínico aleatorizado de muestra pequeña realizado sobre cadáveres con lesión cervical. En los estudios realizados en pacientes sanos sin lesión cervical se observó que el collarín disminuía de forma significativa la movilidad del cuello frente a la no inmovilización. Por el contrario, en los estudios en los que participaban cadáveres con lesión cervical se determinó que el collarín cervical no disminuía la movilidad del cuello. Además en tres estudios se detectó un aumento de la separación intervertebral y en uno, un aumento de la presión venosa yugular. Conclusiones: Si bien la inmovilización cervical reduce la movilidad del cuello en pacientes sin lesión, este efecto no se produce en quienes presentan lesiones cervicales.
