L’aménagement des maisons de naissance est actuellement en hausse sur le territoire québécois, pour répondre à la forte demande des femmes pour un service sage‐femme et également pour répondre au désir des femmes de donner naissance dans un milieu alternatif à l’hôpital ou à la maison. La problématique consiste à s’interroger sur la pertinence de l’environnement actuel des maisons de naissance, à savoir s’il soutient les besoins des femmes qui y donnent naissance, mais également si cet environnement affecte l’expérience d’accouchement des femmes, dans le but futur que les maisons de naissance soient parfaitement adaptées aux nécessités des femmes qui y accouchent. Cette problématique, axée sur la relation personne‐environnement avec point d’ancrage l’expérience sensible de l’usagère, est adressée en premier lieu par une analyse critique des maisons de naissance de Montréal, puis en analysant le discours des femmes, recueillis sous forme d’entretiens semi‐dirigés, pour en exprimer l’expérience d’accouchement en ces lieux. Se basant sur les principes d’études en relations personnes‐environnement, cette recherche vise ainsi à saisir les besoins relatifs au lieu de naissance à travers l’expérience sensible de la femme et des liens affectifs que tisse celle‐ci avec l’environnement lors de son accouchement, plus particulièrement au sein des maisons de naissance. Cette recherche vise également à saisir comment, à l’inverse, l’environnement de naissance affecte l’expérience de la femme lorsqu’elle donne naissance. Parmi les constats de cette recherche, nous réalisons que l’environnement affecte l’expérience d’accouchement dans une certaine mesure et qu’il aurait avantage à se faire des plus discret, afin de ne pas affecter la concentration des femmes durant leur travail. Cette « bulle » que les femmes se forment durant l’accouchement s’avère très importante dans le bon déroulement de la naissance; l’environnement devrait permettre et la mise en place de cette « bulle » et sa continuité jusqu’à la naissance de l’enfant. La poursuite des recherches sur le sujet pourrait entre autres mener à un modèle d’aménagement non rigide pour les maisons de naissance et pourrait également contribuer à l’amélioration des maisons de naissance existantes ainsi que celles à venir.

Problème: Ma thèse porte sur l’identité individuelle comme interrogation sur les enjeux personnels et sur ce qui constitue l’identification hybride à l’intérieur des notions concurrentielles en ce qui a trait à l’authenticité. Plus précisément, j’aborde le concept des identifications hybrides en tant que zones intermédiaires pour ce qui est de l’alternance de codes linguistiques et comme négociation des espaces continuels dans leur mouvement entre les cultures et les langues. Une telle négociation engendre des tensions et/ou apporte le lien créatif. Les tensions sont inhérentes à n’importe quelle construction d’identité où les lignes qui définissent des personnes ne sont pas spécifiques à une culture ou à une langue, où des notions de l’identité pure sont contestées et des codes communs de l’appartenance sont compromis. Le lien créatif se produit dans les exemples où l’alternance de code linguistique ou la négociation des espaces produit le mouvement ouvert et fluide entre les codes de concurrence des références et les différences à travers les discriminations raciales, la sexualité, la culture et la langue. Les travaux que j’ai sélectionnés représentent une section transversale de quelques auteurs migrants provenant de la minorité en Amérique du Nord qui alternent les codes linguistiques de cette manière. Les travaux détaillent le temps et l’espace dans leur traitement de l’identité et dans la façon dont ils cernent l’hybridité dans les textes suivants : The Woman Warrior de Maxine Hong Kingston (1975-76), Hunger of Memory de Richard Rodriguez (1982), Comment faire l’amour avec un nègre sans se fatiguer de Dany Laferrière (1985), Borderlands/La Frontera de Gloria Anzalduá (1987), Lost in Translation de Eva Hoffman (1989), Avril ou l’anti-passion de Antonio D’Alfonso (1990) et Chorus of Mushrooms de Hiromi Goto (1994). Enjeux/Questions La notion de l’identification hybride est provocante comme sujet. Elle met en question l’identité pure. C’est un sujet qui a suscité beaucoup de discussions tant en ce qui a trait à la littérature, à la politique, à la société, à la linguistique, aux communications, qu’au sein même des cercles philosophiques. Ce sujet est compliqué parce qu’il secoue la base des espaces fixes et structurés de l’identité dans sa signification culturelle et linguistique. Par exemple, la notion de patrie n’a pas les représentations exclusives du pays d’origine ou du pays d’accueil. De même, les notions de race, d’appartenance ethnique, et d’espaces sexuels sont parfois négativement acceptées si elles proviennent des codes socialement admis et normalisés de l’extérieur. De tels codes de la signification sont souvent définis par l’étiquette d’identification hétérosexuelle et blanche. Dans l’environnement généralisé d’aujourd’hui, plus que jamais, une personne doit négocier qui elle est, au sens de son appartenance à soi, en tant qu’individu et ce, face aux modèles locaux, régionaux, nationaux, voire même globaux de la subjectivité. Nous pouvons interpréter ce mouvement comme une série de couches superposées de la signification. Quand nous rencontrons une personne pour la première fois, nous ne voyons que la couche supérieure. D’ailleurs, son soi intérieur est caché par de nombreuses couches superposées (voir Joseph D. Straubhaar). Toutefois, sous cette couche supérieure, on retrouve beaucoup d’autres couches et tout comme pour un oignon, on doit les enlever une par une pour que l’individualité complète d’une personne soit révélée et comprise. Le noyau d’une personne représente un point de départ crucial pour opposer qui elle était à la façon dont elle se transforme sans cesse. Sa base, ou son noyau, dépend du moment, et comprend, mais ne s’y limite pas, ses origines, son environnement et ses expériences d’enfance, son éducation, sa notion de famille, et ses amitiés. De plus, les notions d’amour-propre et d’amour pour les autres, d’altruisme, sont aussi des points importants. Il y a une relation réciproque entre le soi et l’autre qui établit notre degré d’estime de soi. En raison de la mondialisation, notre façon de comprendre la culture, en fait, comment on consomme et définit la culture, devient rapidement un phénomène de déplacement. À l’intérieur de cette arène de culture généralisée, la façon dont les personnes sont à l’origine chinoises, mexicaines, italiennes, ou autres, et poursuivent leur évolution culturelle, se définit plus aussi facilement qu’avant. Approche Ainsi, ma thèse explore la subjectivité hybride comme position des tensions et/ou des relations créatrices entre les cultures et les langues. Quoique je ne souhaite aucunement simplifier ni le processus, ni les questions de l’auto-identification, il m’apparaît que la subjectivité hybride est aujourd’hui une réalité croissante dans l’arène généralisée de la culture. Ce processus d’échange est particulièrement complexe chez les populations migrantes en conflit avec leur désir de s’intégrer dans les nouveaux espaces adoptés, c’est-à-dire leur pays d’accueil. Ce réel désir d’appartenance peut entrer en conflit avec celui de garder les espaces originels de la culture définie par son pays d’origine. Ainsi, les références antérieures de l’identification d’une personne, les fondements de son individualité, son noyau, peuvent toujours ne pas correspondre à, ou bien fonctionner harmonieusement avec, les références extérieures et les couches d’identification changeantes, celles qu’elle s’approprie du pays d’accueil. Puisque nos politiques, nos religions et nos établissements d’enseignement proviennent des représentations nationales de la culture et de la communauté, le processus d’identification et la création de son individualité extérieure sont formées par le contact avec ces établissements. La façon dont une personne va chercher l’identification entre les espaces personnels et les espaces publics détermine ainsi le degré de conflit et/ou de lien créatif éprouvé entre les modes et les codes des espaces culturels et linguistiques. Par conséquent, l’identification des populations migrantes suggère que la « community and culture will represent both a hybridization of home and host cultures » (Straubhaar 27). Il y a beaucoup d’écrits au sujet de l’hybridité et des questions de l’identité et de la patrie, toutefois cette thèse aborde la valeur créative de l’alternance de codes culturels et linguistiques. Ce que la littérature indiquera Par conséquent, la plate-forme à partir de laquelle j’explore mon sujet de l’hybridité flotte entre l’interprétation postcoloniale de Homi Bhabha concernant le troisième espace hybride; le modèle d’hétéroglossie de Mikhail Bakhtine qui englobent plusieurs de mes exemples; la représentation de Roland Barthes sur l’identité comme espace transgressif qui est un modèle de référence et la contribution de Chantal Zabus sur le palimpseste et l’alternance de codes africains. J’utilise aussi le modèle de Sherry Simon portant sur l’espace urbain hybride de Montréal qui établit un lien important avec la valeur des échanges culturels et linguistiques, et les analyses de Janet Paterson. En effet, la façon dont elle traite la figure de l’Autre dans les modèles littéraires au Québec fournisse un aperçu régional et national de l’identification hybride. Enfin, l’exploration du bilinguisme de Doris Sommer comme espace esthétique et même humoristique d’identification situe l’hybridité dans une espace de rencontre créative. Conséquence Mon approche dans cette thèse ne prétend pas résoudre les problèmes qui peuvent résulter des plates-formes de la subjectivité hybride. Pour cette raison, j’évite d’aborder toute approche politique ou nationaliste de l’identité qui réfute l’identification hybride. De la même façon, je n’amène pas de discussion approfondie sur les questions postcoloniales. Le but de cette thèse est de démontrer à quel point la subjectivité hybride peut être une zone de relation créatrice lorsque l’alternance de codes permet des échanges de communication plus intimes entre les cultures et les langues. C’est un espace qui devient créateur parce qu’il favorise une attitude plus ouverte vis-à-vis les différents champs qui passent par la culture, aussi bien la langue, que la sexualité, la politique ou la religion. Les zones hybrides de l’identification nous permettent de contester les traditions dépassées, les coutumes, les modes de communication et la non-acceptation, toutes choses dépassées qui emprisonnent le désir et empêchent d’explorer et d’adopter des codes en dehors des normes et des modèles de la culture contenus dans le discours blanc, dominant, de l’appartenance culturelle et linguistique mondialisée. Ainsi, il appert que ces zones des relations multi-ethniques exigent plus d’attention des cercles scolaires puisque la population des centres urbains à travers l’Amérique du Nord devient de plus en plus nourrie par d’autres types de populations. Donc, il existe un besoin réel d’établir une communication sincère qui permettrait à la population de bien comprendre les populations adoptées. C’est une invitation à stimuler une relation plus intime de l’un avec l’autre. Toutefois, il est évident qu’une communication efficace à travers les frontières des codes linguistiques, culturels, sexuels, religieux et politiques exige une négociation continuelle. Mais une telle négociation peut stimuler la compréhension plus juste des différences (culturelle ou linguistique) si des institutions académiques offrent des programmes d’études intégrant davantage les littératures migrantes. Ma thèse vise à illustrer (par son choix littéraire) l’identification hybride comme une réalité importante dans les cultures généralisées qui croissent toujours aujourd’hui. Les espaces géographiques nous gardent éloignés les uns des autres, mais notre consommation de produits exotiques, qu’ils soient culturels ou non, et même notre consommation de l’autre, s’est rétrécie sensiblement depuis les deux dernières décennies et les indicateurs suggèrent que ce processus n’est pas une tendance, mais plutôt une nouvelle manière d’éprouver la vie et de connaître les autres. Ainsi les marqueurs qui forment nos frontières externes, aussi bien que ces marqueurs qui nous définissent de l’intérieur, exigent un examen minutieux de ces enjeux inter(trans)culturels, surtout si nous souhaitons nous en tenir avec succès à des langues et des codes culturels présents, tout en favorisant la diversité culturelle et linguistique. Objectifs: Les études quantitatives ont documenté l’ampleur des problèmes d’observance en début de psychose et les conséquences cliniques qui en découlent. La compréhension du phénomène demeure toutefois limitée. Notre étude propose d’explorer, à partir de trois perspectives (celles des patients, des proches et des cliniciens), les raisons pour lesquelles les patients en début de psychose acceptent ou refusent les traitements. Méthode: La collecte des données s’est faite à l’aide d’entrevues individuelles semi-structurées auprès de 18 patients d’une clinique spécialisée en psychose débutante classifiés comme étant observants, ambivalents ou non observants, et auprès de 13 de leurs proches, ainsi qu’à l’aide d’un focus group réunissant 8 cliniciens de la clinique. Résultats: L’observance semble s’inscrire dans un processus marqué par une certaine ambivalence pour la plupart des sujets. Cette ambivalence est modulée par: des enjeux identitaires, des enjeux relationnels, la compréhension du diagnostic et la signification du traitement. Conclusion: L’ambivalence et l’inobservance seraient des étapes normales du processus au cours duquel le patient lutte pour reconstruire son identité. La présence d’un lien de confiance permet la résolution progressive de l’ambivalence, facilitant ainsi le processus de réappropriation du traitement par le patient.

Le trouble déficitaire de l’attention avec ou sans hyperactivité (TDA/H) est une condition qui touche un nombre important d’enfants d’âge scolaire. Plusieurs d’entre eux sont également atteints d’un trouble concomitant, qu’il soit de nature anxieuse ou agressive. Les manifestations comportementales qui en découlent, ainsi que les conséquences qui y sont associées, ont une influence sur l’enfant, mais également sur son parent. Certaines études suggèrent que ces conditions sont associées chez les parents à un niveau de stress élevé, à un sentiment d’auto-efficacité (SAE) plus faible, à plus de symptômes dépressifs et à une satisfaction conjugale plus faible. D’autres résultats rapportent le contraire. Pourquoi ces résultats sont-il divergents? Cette étude a pour but, en premier lieu, d’examiner, dans un échantillon québécois de 110 parents d’enfants atteints de TDA/H, si les caractéristiques du TDA/H des enfants (sous-type de TDA/H et présence d’un trouble concomitant) influencent les caractéristiques personnelles de leur parent (stress parental, SAE, symptômes dépressifs et satisfaction conjugale). En deuxième lieu, nous examinerons si l’attachement adulte est lié à ces quatre caractéristiques personnelles parentales et s’il modère le lien entre les caractéristiques du TDA/H de l’enfant et celles du parent. Les résultats indiquent que le profil diagnostique de l’enfant est lié aux symptômes dépressifs des parents et que la présence d’un trouble concomitant chez l’enfant est liée à la satisfaction conjugale parentale. De plus, un effet d’interaction est observé entre le profil diagnostique et la présence d’un trouble concomitant chez l’enfant sur le niveau de stress du parent. Les résultats démontrent également un effet significatif du style d’attachement adulte sur le sentiment d’auto-efficacité des parents, les parents avec un attachement sécure-autonome ayant un sentiment d’auto-efficacité plus élevé que ceux avec un attachement de type insécure-ambivalent. Aucun effet modérateur de l’attachement adulte sur la relation entre les caractéristiques du TDA/H de l’enfant et les caractéristiques personnelles de son parent n’est observé.

Would a research assistant - who can search for ideas related to those you are working on, network with others (but only share the things you have chosen to share), doesn’t need coffee and who might even, one day, appear to be conscious - help you get your work done? Would it help your students learn? There is a body of work showing that digital learning assistants can be a benefit to learners. It has been suggested that adaptive, caring, agents are more beneficial. Would a conscious agent be more caring, more adaptive, and better able to deal with changes in its learning partner’s life? Allow the system to try to dynamically model the user, so that it can make predictions about what is needed next, and how effective a particular intervention will be. Now, given that the system is essentially doing the same things as the user, why don’t we design the system so that it can try to model itself in the same way? This should mimic a primitive self-awareness. People develop their personalities, their identities, through interacting with others. It takes years for a human to develop a full sense of self. Nobody should expect a prototypical conscious computer system to be able to develop any faster than that. How can we provide a computer system with enough social contact to enable it to learn about itself and others? We can make it part of a network. Not just chatting with other computers about computer ‘stuff’, but involved in real human activity. Exposed to ‘raw meaning’ – the developing folksonomies coming out of the learning activities of humans, whether they are traditional students or lifelong learners (a term which should encompass everyone). Humans have complex psyches, comprised of multiple strands of identity which reflect as different roles in the communities of which they are part – so why not design our system the same way? With multiple internal modes of operation, each capable of being reflected onto the outside world in the form of roles – as a mentor, a research assistant, maybe even as a friend. But in order to be able to work with a human for long enough to be able to have a chance of developing the sort of rich behaviours we associate with people, the system needs to be able to function in a practical and helpful role. Unfortunately, it is unlikely to get a free ride from many people (other than its developer!) – so it needs to be able to perform a useful role, and do so securely, respecting the privacy of its partner. Can we create a system which learns to be more human whilst helping people learn?

Individuals with schizophrenia, particularly those with passivity symptoms, may not feel in control of their actions, believing them to be controlled by external agents. Cognitive operations that contribute to these symptoms may include abnormal processing in agency as well as body representations that deal with body schema and body image. However, these operations in schizophrenia are not fully understood, and the questions of general versus specific deficits in individuals with different symptom profiles remain unanswered. Using the projected-hand illusion (a digital video version of the rubber-hand illusion) with synchronous and asynchronous stroking (500 ms delay), and a hand laterality judgment task, we assessed sense of agency, body image, and body schema in 53 people with clinically stable schizophrenia (with a current, past, and no history of passivity symptoms) and 48 healthy controls. The results revealed a stable trait in schizophrenia with no difference between clinical subgroups (sense of agency) and some quantitative (specific) differences depending on the passivity symptom profile (body image and body schema). Specifically, a reduced sense of self-agency was a common feature of all clinical subgroups. However, subgroup comparisons showed that individuals with passivity symptoms (both current and past) had significantly greater deficits on tasks assessing body image and body schema, relative to the other groups. In addition, patients with current passivity symptoms failed to demonstrate the normal reduction in body illusion typically seen with a 500 ms delay in visual feedback (asynchronous condition), suggesting internal timing problems. Altogether, the results underscore self-abnormalities in schizophrenia, provide evidence for both trait abnormalities and state changes specific to passivity symptoms, and point to a role for internal timing deficits as a mechanistic explanation for external cues becoming a possible source of self-body input.

This article considers the ways in which British youth telefantasy Misfits (E4, 2009–13) takes up and makes strange urban spaces familiar from social-realist narratives. Filmed on the sprawling East London estate, Thamesmead, the programme chronicles a group of young offenders who are given powers by a freak storm, turning them into ‘ASBO superheroes’. Misfits depends on its British urban landscapes for the assertion of its ‘authenticity’ within British youth television, using spaces and landscapes familiar from urban youth exploitation cinema and television's narratives of the underclass. After situating the series within existing cultural discourses and recent developments in social-realist representations, the article explores how Misfits disrupts what have become signifiers for the ‘real’ – the brutalism of housing estates, the grey of the concrete and sky – by making them strange, turning them into telefantasy. The series presents the estate as an uncanny place: the domestic, social-realist world shifted into a fantastical space by the storm. Through close analysis, this article explores how the familiar spaces become skewed and unsettling to match our protagonists' isolation, shifting bodies and scrambled sense of self.

Bettelheim (2007), analyzing the infant psyche concluded that for the child to gain self-esteem and develop a balanced sense of self, need to learn to take certain decisions on a daily basis, which will be facilitated by identifying their problems, designed in the stories that are told (or read). Thus, it could find solutions and feel safer. Based on the reading of fairy tales, we dealt with this research, the resumption of fairy tales in homes and schools, in order to help parents and teachers to get parameters to work thinking of their children and students, from such stories, awakening the taste of children by reading and text production. The proposal considered different versions of the fairy tale Cinderella, noting as the moralizing process the messages each approach, and explores the plot and determines which versions would be appropriate for the psycho-cognitive development of children. We also investigated aspects pertinent to the narrative structure, based on literary theory, in order to work in comparative literature. From the discourse analysis, sought to address the marks left by the utterer, capable of denoting its cargo and its ideological worldview projected in the story, although he re-create the history and environment on real facts of a particular period (MARTINS, 2007)

The writing and defense of the dissertation serve both as demonstration one is able to do the work of a scholar and as a rite of initiation. In contrast to much academic writing, dissertations generally adhere to narrowly conceived notions of academic discourse. I explore this within the context of an academic community in which under-representation remains a serious issue. This dissertation is about women writing dissertations. I draw from conversations with fifteen women, in or beyond, the process; friends’ anecdotes; published accounts; and, autobiographically, my experience. I suggest the dissertation’s initiatory role is at least as important as its scholarly role; during the process one establishes a sense of self as scholar, writer, and researcher. Students come to the dissertation with some notion of self as writer and scholar – a culturally negotiated sense that is more, or less, congruent with the culturally established self required for successful completion of the dissertation. The degree of congruence (or alternatively, harmony and dissonance) shapes the process of doing a dissertation. I argue that both the community and the language in which dissertations must generally be written are gendered masculine. Negotiating a voice that is acceptable in a dissertation while maintain fidelity to a sense of who one is seems more problematic as one’s distance from the center of dominant culture increases. Believing that agency lies in altering the reiteration of such processes, I worked with my committee to find ways to alter the process yet still do a dissertation I write in a variety of voices – essay and poetry as well as analytical – play with visual qualities of text, and experiment with non-verbal interpretations. These don’t exhaust possibilities, but do give a sense of how the rich variety of expression found in academe cam be brought into the dissertation. I thus demonstrate that one need not reconstitute herself through characteristic academic discourse in order to be initiated into the community of scholars. I suggest both the desirability of encouraging flexibility in the language, form, and process, of dissertations, and the theoretical necessity for such flexibility if the academic community is to become diverse. The degree of congruence (or alternatively, harmony and dissonance) shapes the process of doing a dissertation. I argue that both the community and the language in which dissertations must generally be written are gendered masculine. Negotiating a voice that is acceptable in a dissertation while maintain fidelity to a sense of who one is seems more problematic as one’s distance from the center of dominant culture increases. Believing that agency lies in altering the reiteration of such processes, I worked with my committee to find ways to alter the process yet still do a dissertation I write in a variety of voices – essay and poetry as well as analytical – play with visual qualities of text, and experiment with non-verbal interpretations. These don’t exhaust possibilities, but do give a sense of how the rich variety of expression found in academe cam be brought into the dissertation. I thus demonstrate that one need not reconstitute herself through characteristic academic discourse in order to be initiated into the community of scholars. I suggest both the desirability of encouraging flexibility in the language, form, and process, of dissertations, and the theoretical necessity for such flexibility if the academic community is to become diverse. The writing and defense of the dissertation serve both as demonstration one is able to do the work of a scholar and as a rite of initiation. In contrast to much academic writing, dissertations generally adhere to narrowly conceived notions of academic discourse. I explore this within the context of an academic community in which under-representation remains a serious issue. This dissertation is about women writing dissertations. I draw from conversations with fifteen women, in or beyond, the process; friends’ anecdotes; published accounts; and, autobiographically, my experience. I suggest the dissertation’s initiatory role is at least as important as its scholarly role; during the process one establishes a sense of self as scholar, writer, and researcher Students come to the dissertation with some notion of self as writer and scholar – a culturally negotiated sense that is more, or less, congruent with the culturally established self required for successful completion of the dissertation. Although Great Britain is not normally credited with the achievement of having been the first nation state to implement measures characteristic of a welfare state (this honour goes to Germany and Bismarck's strategy of promoting social insurance in the 1880s) it nevertheless pioneered many models of welfare services in view of the early onset of industrialisation in that country and the subsequent social problems it created. Organisations like the Mutual Insurance and Friendly Societies, the Charity Organisation Society or the Settlement Movement characterised an early approach to welfare that is based on initiatives at the civil society level and express a sense of self-help or of self-organisation in such a way that it did not involve the state directly. The state, traditionally, dealt with matters of discipline and public order, and for this reason institutions like prisons and workhouses represented the other end of the scale of 'welfare' provisions.

BACKGROUND Bodily sensations are an important component of corporeal awareness. Spinal cord injury can leave affected body parts insentient and unmoving, leading to specific disturbances in the mental representation of one's own body and the sense of self. OBJECTIVE Here, we explored how illusions induced by multisensory stimulation influence immediate sensory signals and tactile awareness in patients with spinal cord injuries. METHODS The rubber hand illusion paradigm was applied to 2 patients with chronic and complete spinal cord injury of the sixth cervical spine, with severe somatosensory impairments in 2 of 5 fingers. RESULTS Both patients experienced a strong illusion of ownership of the rubber hand during synchronous, but not asynchronous, stroking. They also, spontaneously reported basic tactile sensations in their previously numb fingers. Tactile awareness from seeing the rubber hand was enhanced by progressively increasing the stimulation duration. CONCLUSIONS Multisensory illusions directly and specifically modulate the reemergence of sensory memories and enhance tactile sensation, despite (or as a result of) prior deafferentation. When sensory inputs are lost, and are later illusorily regained, the brain updates a coherent body image even several years after the body has become permanently unable to feel. This particular example of neural plasticity represents a significant opportunity to strengthen the sense of the self and the feelings of embodiment in patients with spinal cord injury.

INTRODUZIONE: L’integrazione mente-corpo applicata ad un ambito patologico predominante in questi tempi, come il cancro, è il nucleo di questa tesi. Il background teorico entro cui è inserita, è quello della Psiconeuroendocrinoimmunologia (Bottaccioli, 1995) e Psico-Oncologia. Sono state identificate, nella letteratura scientifica, le connessioni tra stati psicologici (mente) e condizioni fisiologiche (corpo). Le variabili emerse come potenzialmente protettive in pazienti che si trovano ad affrontare il cancro sono: il supporto sociale, l’immagine corporea, il coping e la Qualità della Vita, insieme all’indice fisiologico Heart Rate Variability (HRV; Shaffer & Venner, 2013). Il potenziale meccanismo della connessione tra queste variabili potrebbe essere spiegato dall’azione del Nervo Vago, come esposto nella Teoria Polivagale di Stephen Porges (2007; 2009). OBIETTIVI: Gli obiettivi principali di questo studio sono: 1. Valutare l’adattamento psicologico alla patologia in termini di supporto sociale percepito, immagine corporea, coping prevalente e qualità della vita in donne con cancro ovarico; 2. Valutare i valori di base HRV in queste donne; 3. Osservare se livelli più elevati di HRV sono associati ad un migliore adattamento psicologico alla patologia; 4. Osservare se una peggiore percezione dell’immagine corporea e l’utilizzo di strategie di coping disadattive sono associate ad una Qualità della Vita più scarsa. METODO: 38 donne affette da cancro ovarico, al momento della valutazione libere da patologia, sono state reclutate presso la clinica oncologica del reparto di Ginecologia dell’Azienda Ospedaliero-Universitaria di Parma, Italia. Ad ogni partecipante è stato chiesto di compilare una batteria di test composta da: MSPSS, per la valutazione del supporto sociale percepito; DAS-59, per la valutazione dell’immagine corporea; MAC, per la valutazione delle strategie di coping prevalenti utilizzate verso il cancro; EORTC-QLQ30, per la valutazione della Qualità della Vita. Per ogni partecipante è stato registrato HRV di base utilizzando lo strumento emWave (HeartMath). RISULTATI PRINCIPALI: Rispondendo agli obiettivi 1 e 2, in queste donne si è rilevato una alto tasso di supporto sociale percepito, in particolare ricevuto dalla persona di riferimento. L’area rivelatasi più critica nel supporto sociale è quella degli amici. Per quanto riguarda l’immagine corporea, la porzione di campione dai 30 ai 61 anni, ha delle preoccupazioni globali legate all’immagine corporea paragonabili ai dati provenienti dalla popolazione generale con preoccupazioni riguardo l’aspetto corporeo. Invece, nella porzione di campione dai 61 anni in su, il pattern di disagio verso l’aspetto fisico sembra decisamente peggiorare. Inoltre, in questo campione, si è rilevato un disagio globale verso l’immagine corporea significativamente più alto rispetto ai valori normativi presenti in letteratura riferiti a donne con cancro al seno con o senza mastectomia (rispettivamente t(94)= -4.78; p<0.000001; t(110)= -6.81;p<0.000001). La strategia di coping più utilizzata da queste donne è lo spirito combattivo, seguito dal fatalismo. Questo campione riporta, inoltre, una Qualità della Vita complessivamente soddisfacente, con un buon livello di funzionamento sociale. L’area di funzionalità più critica risulta essere il funzionamento emotivo. Considerando i sintomi prevalenti, i più riferiti sono affaticamento, disturbi del sonno e dolore. Per definire, invece, il pattern HRV, sono stati confrontati i dati del campione con quelli presenti in letteratura, riguardanti donne con cancro ovarico. Il campione valutato in questo studio, ha un HRV SDNN (Me=28.2ms) significativamente più alto dell’altro gruppo. Tuttavia, confrontando il valore medio di questo campione con i dati normativi sulla popolazione sana (Me=50ms), i nostri valori risultano drasticamente più bassi. In ultimo, donne che hanno ricevuto diagnosi di cancro ovarico in età fertile, sembrano avere maggiore HRV, migliore funzionamento emotivo e minore sintomatologia rispetto alle donne che hanno ricevuto diagnosi non in età fertile. Focalizzando l’attenzione sulla ricerca di relazioni significative tra le variabili in esame (obiettivo 3 e 4) sono state trovate numerose correlazioni significative tra: l’età e HRV, supporto percepito , Qualità della Vita; Qualità della Vita e immagine corporea, supporto sociale, strategie di coping; strategie di coping e immagine corporea, supporto sociale; immagine corporea e supporto sociale; HRV e supporto sociale, Qualità della Vita. Per verificare la possibile connessione causale tra le variabili considerate, sono state applicate regressioni lineari semplici e multiple per verificare la bontà del modello teorico. Si è rilevato che HRV è significativamente positivamente influenzata dal supporto percepito dalla figura di riferimento, dal funzionamento di ruolo, dall’immagine corporea totale. Invece risulta negativamente influenzata dal supporto percepito dagli amici e dall’uso di strategie di coping evitanti . La qualità della vita è positivamente influenzata da: l’immagine corporea globale e l’utilizzo del fatalismo come strategia di coping prevalente. Il funzionamento emotivo è influenzato dal supporto percepito dalla figura di riferimento e dal fatalismo. DISCUSSIONI E CONCLUSIONI: Il campione Italiano valutato, sembra essere a metà strada nell’adattamento dello stato psicologico e dell’equilibrio neurovegetativo al cancro. Sicuramente queste donne vivono una vita accettabile, in quanto sopravvissute al cancro, ma sembra anche che portino con sé preoccupazioni e difficoltà, in particolare legate all’accettazione della loro condizione di sopravvissute. Infatti, il migliore adattamento si riscontra nelle donne che hanno avuto peggiori condizioni in partenza: stadio del cancro avanzato, più giovani, con diagnosi ricevuta in età fertile. Pertanto, è possibile suggerire che queste condizioni critiche forzino queste donne ad affrontare apertamente il cancro e la loro situazione di sopravvissute al cancro, portandole ad “andare avanti” piuttosto che “tornare indietro”. Facendo riferimento alle connessioni tra variabili psicologiche e fisiologiche in queste donne, si è evidenziato che HRV è influenzata dalla presenza di figure significative ma, in particolare, è presumibile che sia influenzata da un’appropriata condivisione emotiva con queste figure. Si è anche evidenziato che poter continuare ad essere efficaci nel proprio contesto personale si riflette in un maggiore HRV, probabilmente in quanto permette di preservare il senso di sé, riducendo in questo modo lo stress derivante dall’esperienza cancro. Pertanto, HRV in queste donne risulta associato con un migliore adattamento psicologico. Inoltre, si è evidenziato che in queste donne la Qualità della Vita è profondamente influenzata dalla percezione dell’immagine corporea. Si tratta di un aspetto innovativo che è stato rilevato in questo campione e che, invece, nei precedenti studi non è stato indagato. In ultimo, la strategia di coping fatalismo sembra essere protettiva e sembra facilitare il processo di accettazione del cancro. Si spera sinceramente che le ricerche future possano superare i limiti del presente studio, come la scarsa numerosità e l’uso di strumenti di valutazione che, per alcuni aspetti come la scala Evitamento nel MAC, non centrano totalmente il target di indagine. Le traiettorie future di questo studio sono: aumentare il numero di osservazioni, reclutando donne in diversi centri specialistici in diverse zone d’Italia; utilizzare strumenti più specifici per valutare i costrutti in esame; valutare se un intervento di supporto centrato sul miglioramento di HRV (come HRV Biofeedback) può avere una ricaduta positiva sull’adattamento emotivo e la Qualità della Vita.

The purpose of the current paper is to examine the racial identity development of Japanese American biracial adolescents. A review of the literature revealed critical factors as it relates to Japanese American biracial adolescents and their identity formation. Ethnic identity, adolescent developmental stage, environmental ethnic socialization processes used to facilitate identity development, and the resulting psychological adjustment were the major issues identified in the literature. Educators, counselors and other mental health professionals must begin to understand their development if the needs of this population are to be met in the future. A case study of a biracial Japanese American teen was included to heighten the understanding of the identity development of biracial individuals. Findings indicate that biracial children who are raised to identify with both sides of their ethnic heritage are not suffering from negative psychological consequences (e.g. guilt, anger, isolation) but, instead, gain a "healthier" sense of self. Implications for clinicians are discussed in terms of how therapists can be responsive to the unique needs of biracial adolescents.
