Background: Survival of patients with Acute Aortic Syndrome (AAS) may relate to the speed of diagnosis. Diagnostic delay is exacerbated by non classical presentations such as myocardial ischemia or acute heart failure (AHF). However little is known about clinical implications and pathophysiological mechanisms of Troponin T elevation and AHF in AAS. Methods and Results: Data were collected from a prospective metropolitan AAS registry (398 patients diagnosed between 2000 and 2013). Troponin T values (either standard or high sensitivity assay, HS) were available in 248 patients (60%) of the registry population; the overall frequency of troponin positivity was 28% (ranging from 16% to 54%, using standard or HS assay respectively, p = 0.001). Troponin positivity was associated with a twofold increased risk of long in-hospital diagnostic time (OR 1.92, 95% CI 1.05-3.52, p = 0.03), but not with in-hospital mortality. The combination of positive troponin and ACS-like ECG abnormalities resulted in a significantly increased risk of inappropriate therapy due to a misdiagnosis of ACS (OR 2.48, 95% CI 1.12-5.54, p = 0.02). Patients with AHF were identified by the presence of dyspnea as presentation symptom or radiological signs of pulmonary congestion or cardiogenic shock. The overall frequency of AHF was 28 % (32% type A vs. 20% type B AAS, p = 0.01). AHF was due to a variety of pathophysiological mechanisms including cardiac tamponade (26%), aortic regurgitation (25%), myocardial ischemia (17%), hypertensive crisis (10%). AHF was associated with increased surgical delay and with increased risk of in-hospital death (adjusted OR 1.97 95% CI1.13-3.37,p=0.01). Conclusions: Troponin positivity (particularly HS) was a frequent finding in AAS. Abnormal troponin values were strongly associated with ACS-like ECG findings, in-hospital diagnostic delay, and inappropriate therapy. AHF was associated with increased surgical delay and was an independent predictor of in-hospital mortality.

This doctoral thesis was focused on the investigation of enantiomeric and non-enantiomeric biogenic organic compound (BVOC) emissions from both leaf and canopy scales in different environments. In addition, the anthropogenic compounds benzene, toluene, ethylbenzene, and xylenes (BTEX) were studied. BVOCs are emitted into the lower troposphere in large quantities (ca. 1150 Tg C ·yr-1), approximately an order of magnitude greater than the anthropogenic VOCs. BVOCs are particularly important in tropospheric chemistry because of their impact on ozone production and secondary organic aerosol formation or growth. The BVOCs examined in this study were: isoprene, (-)/ (+)-α-pinene, (-)/ (+)-ß-pinene, Δ-3-carene, (-)/ (+)-limonene, myrcene, eucalyptol and camphor, as these were the most abundant BVOCs observed both in the leaf cuvette study and the ambient measurements. In the laboratory cuvette studies, the sensitivity of enantiomeric enrichment change from the leaf emission has been examined as a function of light (0-1600 PAR) and temperature (20-45°C). Three typical Mediterranean plant species (Quercus ilex L., Rosmarinus officinalis L., Pinus halepensis Mill.) with more than three individuals of each have been investigated using a dynamic enclosure cuvette. The terpenoid compound emission rates were found to be directly linked to either light and temperature (e.g. Quercus ilex L.) or mainly to temperature (e.g. Rosmarinus officinalis L., Pinus halepensis Mill.). However, the enantiomeric signature showed no clear trend in response to either the light or temperature; moreover a large variation of enantiomeric enrichment was found during the experiment. This enantiomeric signature was also used to distinguish chemotypes beyond the normal achiral chemical composition method. The results of nineteen Quercus ilex L. individuals, screened under standard conditions (30°C and 1000 PAR) showed four different chemotypes, whereas the traditional classification showed only two. An enclosure branch cuvette set-up was applied in the natural boreal forest environment from four chemotypes of Scots pine (Pinus sylvestris) and one chemotype of Norway spruce (Picea abies) and the direct emissions compared with ambient air measurements above the canopy during the HUMPPA-COPEC 2010 summer campaign. The chirality of a-pinene was dominated by (+)-enantiomers from Scots pine while for Norway spruce the chirality was found to be opposite (i.e. Abstract II (-)-enantiomer enriched) becoming increasingly enriched in the (-)-enantiomer with light. Field measurements over a Spanish stone pine forest were performed to examine the extent of seasonal changes in enantiomeric enrichment (DOMINO 2008). These showed clear differences in chirality of monoterpene emissions. In wintertime the monoterpene (-)-a-pinene was found to be in slight enantiomeric excess over (+)-a-pinene at night but by day the measured ratio was closer to one i.e. racemic. Samples taken the following summer in the same location showed much higher monoterpene mixing ratios and revealed a strong enantiomeric excess of (-)-a-pinene. This indicated a strong seasonal variance in the enantiomeric emission ratio which was not manifested in the day/night temperature cycles in wintertime. A clear diurnal cycle of enantiomeric enrichment in a-pinene was also found over a French oak forest and the boreal forest. However, while in the boreal forest (-)-a-pinene enrichment increased around the time of maximum light and temperature, the French forest showed the opposite tendency with (+)-a-pinene being favored. For the two field campaigns (DOMINO 2008 and HUMPPA-COPEC 2010), the BTEX were also investigated. For the DOMINO campaign, mixing ratios of the xylene isomers (meta- and para-) and ethylbenzene, which are all well resolved on the ß-cyclodextrin column, were exploited to estimate average OH radical exposures to VOCs from the Huelva industrial area. These were compared to empirical estimates of OH based on JNO2 measured at the site. The deficiencies of each estimation method are discussed. For HUMPPA-COPEC campaign, benzene and toluene mixing ratios can clearly define the air mass influenced by the biomass burning pollution plume from Russia.

Il proposito principale di questo studio è valutare, attraverso un confronto tra tecnologia Nexfin e metodo invasivo, se l’attuazione di modelli fisiologici realizzata dal Monitor Nexfin, rende possibile un preciso e accurato monitoraggio dinamico della pressione arteriosa, garantendo in questo modo l’affidabilità della suddetta tecnologia in ambito clinico. È stato deciso di porre come termine di paragone il sistema invasivo in quanto rappresenta il gold standard per la rilevazione della pressione arteriosa. I capitoli sono stati disposti in modo da introdurre gradualmente il lettore all’interno del topic principale dell’elaborato, fornendo di volta in volta tutte le conoscenze essenziali per una comprensione ottimale degli argomenti trattati. Il primo capitolo fornisce inizialmente una breve visione d’insieme sull’apparato cardiocircolatorio, per poi concentrarsi sui fenomeni che concorrono alla definizione e alla caratterizzazione della pressione arteriosa. Il secondo capitolo presenta alcuni cenni storici sulle prime misurazioni di pressione effettuate, in seguito illustra i principali metodi di misurazione non invasivi, articolando il discorso anche attraverso l’analisi dei principali sfigmomanometri ad oggi disponibili. Il terzo capitolo guida il lettore attraverso la scoperta delle più recenti metodiche non invasive per la misurazione della pressione che consentono un monitoraggio dinamico, esponendo nel dettaglio i loro limiti e le loro peculiarità. Nella parte finale del capitolo si affronta anche l’evoluzione del “Volume Clamp Method” di Peñáz partendo dal progetto originale fino a giungere al notevole contributo di Wesseling e alla ricostruzione del segnale di pressione brachiale. Il quarto capitolo delinea brevemente le caratteristiche principali del metodo invasivo, ovvero il gold standard per quanto riguarda il monitoraggio della pressione sanguigna. Il quinto capitolo infine illustra in maniera completa le proprietà del Monitor Nexfin e analizza i risultati conseguiti, confrontando la tecnologia Nexfin con il sistema invasivo, durante lo studio condotto presso l’Ospedale Maurizio Bufalini dell’Azienda USL della Romagna – Sede operativa di Cesena. Lo studio è presentato seguendo una impostazione scientifica.

I investigated the systematics, phylogeny and biogeographical history of Juncaginaceae, a small family of the early-diverging monocot order Alismatales which comprises about 30 species of annual and perennial herbs. A wide range of methods from classical taxonomy to molecular systematic and biogeographic approaches was used. rnrnIn Chapter 1, a phylogenetic analysis of the family and members of Alismatales was conducted to clarify the circumscription of Juncaginaceae and intrafamilial relationships. For the first time, all accepted genera and those associated with the family in the past were analysed together. Phylogenetic analysis of three molecular markers (rbcL, matK, and atpA) showed that Juncaginaceae are not monophyletic. As a consequence the family is re-circumscribed to exclude Maundia which is pro-posed to belong to a separate family Maundiaceae, reducing Juncaginaceae to include Tetroncium, Cycnogeton and Triglochin. Tetroncium is weakly supported as sister to the rest of the family. The reinstated Cycnogeton (formerly included in Triglochin) is highly supported as sister to Triglochin s.str. Lilaea is nested within Triglochin s. str. and highly supported as sister to the T. bulbosa complex. The results of the molecular analysis are discussed in combination with morphological characters, a key to the genera of the family is given, and several new combinations are made.rnrnIn Chapter 2, phylogenetic relationships in Triglochin were investigated. A species-level phylogeny was constructed based on molecular data obtained from nuclear (ITS, internal transcribed spacer) and chloroplast sequence data (psbA-trnH, matK). Based on the phylogeny of the group, divergence times were estimated and ancestral distribution areas reconstructed. The monophyly of Triglochin is confirmed and relationships between the major lineages of the genus were resolved. A clade comprising the Mediterranean/African T. bulbosa complex and the American T. scilloides (= Lilaea s.) is sister to the rest of the genus which contains two main clades. In the first, the widespread T. striata is sister to a clade comprising annual Triglochin species from Australia. The second clade comprises T. palustris as sister to the T. maritima complex, of which the latter is further divided into a Eurasian and an American subclade. Diversification in Triglochin began in the Miocene or Oligocene, and most disjunctions in Triglochin were dated to the Miocene. Taxonomic diversity in some clades is strongly linked to habitat shifts and can not be observed in old but ecologically invariable lineages such as the non-monophyletic T. maritima.rnrnChapter 3 is a collaborative revision of the Triglochin bulbosa complex, a monophyletic group from the Mediterranean region and Africa. One new species, Triglochin buchenaui, and two new subspecies, T. bulbosa subsp. calcicola and subsp. quarcicola, from South Africa were described. Furthermore, two taxa were elevated to species rank and two reinstated. Altogether, seven species and four subspecies are recognised. An identification key, detailed descriptions and accounts of the ecology and distribution of the taxa are provided. An IUCN conservation status is proposed for each taxon.rnrnChapter 4 deals with the monotypic Tetroncium from southern South America. Tetroncium magellanicum is the only dioecious species in the family. The taxonomic history of the species is described, type material is traced, and a lectotype for the name is designated. Based on an extensive study of herbarium specimens and literature, a detailed description of the species and notes on its ecology and conservation status are provided. A detailed map showing the known distribution area of T. magellanicum is presented. rnrnIn Chapter 5, the flower structure of the rare Australian endemic Maundia triglochinoides (Maundiaceae, see Chapter 1) was studied in a collaborative project. As the morphology of Maundia is poorly known and some characters were described differently in the literature, inflorescences, flowers and fruits were studied using serial mictrotome sections and scanning electron microscopy. The phylogenetic placement, affinities to other taxa, and the evolution of certain characters are discussed. As Maundia exhibits a mosaic of characters of other families of tepaloid core Alismatales, its segregation as a separate family seems plausible.

La tesi affronta il tema dello spazio collettivo come luogo del tempo libero attraverso il disegno di un progetto a Jesolo, nell'area definita dal masterplan di Kenzo Tange "Central Park". Il progetto affronta la scala territoriale e urbana con forte attenzione al paesaggio e al disegno del suolo. Partendo da un analisi del contesto ambientale e antropico il progetto si definisce a partire dalla logica del "cluster"e dello "stem", principi compositivi sviluppati dagli architetti Smithson e dagli architetti Candilis, Josic e Woods. L'area di approfondimento per il progetto di tesi viene selezionata a partire dal disegno territoriale e propone la realizzazione di due nuove architetture dedicate al tempo libero e al benessere della persona. La tesi alla base è l'interpretazione del tempo libero visto non come un prodotto ma come oggetto che necessita di riappropriazione individuale. Le riflessioni prendono spunto dalle tesi situazioniste e dal lavoro dell' architetto Cedric Price.

In the current work, three studies about non-aqueous dispersions of particles were carried out by using an amphiphilic block copolymer poly(isoprene)-block-poly(methyl methacrylate) (PI-b-PMMA) as stabilizer:rn1. Dispersions of polyurethane and polyurea porous particles for polymer compositesrn2. Dispersions of PMMA and PU particles with PDI dye for study of Single Molecule Spectroscopy Detectionrn3. Dispersions of graphene nanosheets for polymer compositesrnrnIn the first study, highly porous polyurethane and polyurea particles were prepared in a non-aqueous emulsion. The preparation of porous particles consisted of two parts: At first, a system was developed where the emulsion had high stability for the polymerization among diisocyanate, diol and water. In the second part, porous particles were prepared by using two methods fission/fusion and combination by which highly porous particles were obtained. In this study, the applications of porous particles were also investigated where polyurethane particles were tested as filling material for polymer composites and as catalyst carrier for polyethylene polymerization. rnrnIn the second study, PMMA and PU particles from one non-aqueous emulsion were investigated via single molecule fluorescence detection. At first the particles were loaded with PDI dye, which were detected by fluorescence microscopy. The distribution and orientation of the PDI molecules in the particles were successfully observed by Single Molecule Fluorescence Detection. The molecules were homogenously distributed inside of the particles. In addition they had random orientation, meaning that no aggregations of dye molecules were formed. With the results, it could be supposed that the polymer chains were also homogenously distributed in the particles, and that the conformation was relatively flexible. rnrnIn the third part of the study, graphene nanosheets with high surface area were dispersed in an organic solvent with low boiling point and low toxicity, THF, stabilized with a block copolymer PI-b-PMMA. The dispersion was used to prepare polymer composites. It was shown that the modified graphene nanosheets had good compatibility with the PS and PMMA matrices. rn

Il lavoro di tesi ha preso spunto da un fenomeno franoso avvenuto a Porretta Terme (provincia di Bologna), nell’autunno 2008, a causa di abbondanti precipitazioni, in tale occasione si innescò una colata rapida di detriti che invase la strada statale n. 64 e lambì la ferrovia che collega l’Emilia Romagna e la Toscana. I detriti colpirono anche un’abitazione, senza per fortuna causare perdite umane ne danni ingenti, ma l’evento riscosse numerose attenzioni da parte della popolazione locale, preoccupata che fenomeni del genere potessero ripetersi anche in altre zone e senza alcun preavviso. L’evento allertò anche la Protezione Civile regionale che da subito si interessò all’applicazione di metodi in grado di prevedere la suscettività areale da frane di questo tipo. Metodi previsionali per colate rapide sono stati sviluppati negli ultimi anni in diverse università sia in Europa sia negli Stati Uniti d’America. Lo scopo è quello di prevedere le possibili zone di innesco e individuare le soglie di precipitazione critica in modo da realizzare carte di suscettività a scopo di pianificazione territoriale e mitigazione del rischio. Il lavoro, durato diversi mesi, è passato attraverso diverse fasi. In primis si è proceduto a sopralluoghi di campo al fine di inquadrare l’area oggetto di studio dal punto di vista geologico e geomorfologico. Successivamente è stata svolta una fase di campionamento sul terreno; tramite una trivella manuale sono stati effettuati numerosi carotaggi (circa 60) sparsi sull’intero versante oggetto di studio, al fine di ricavare un dato relativo allo spessore del suolo. Inoltre sono stati raccolti dei campioni di terreno, analizzati successivamente in laboratorio al fine di ricavarne le curve granulometriche e alcuni dati caratterizzanti. Terminata la fase sul terreno, si è provveduto ad effettuare le prove di laboratorio (curve granulometriche, limiti di Attemberg, etc). Grazie ai dati raccolti, è stato possibile applicare un modello previsionale di stabilità superficiale, TRIGRS 2.0 basato sull’infiltrazione in un mezzo non saturo di una precipitazione di durata finita. Particolare attenzione è stata rivolta ai metodi per la previsione dello spessore del suolo (Z Model, S Model, Sexp Model) e alla loro influenza nella suscettività da frana superficiale.

Il disegno del verde acquisisce un ruolo imprescindibile nell’intera area soggetta a studio. Se in prossimità delle zone edificate di progetto esso assume una dialettica propria, derivata dallo studio dei tracciati urbani sopra citati, in adiacenza di Via Carlo Tosi, prospiciente all’ospedale San Carlo, va ad assumere un carattere affine alla griglia ortogonale sopra descritta, rispondendo non più alle giaciture del costruito esistente ma all’inclinazione dei campi risalenti al 1878, epoca a cui corrisponde la carta di Manovra.

Dati climatici ad alta risoluzione sono attualmente molto richiesti essendo indispensabili per la valutazione degli impatti dei cambiamenti climatici alla scala locale in svariati campi d'applicazione. Per aumentare l'offerta di tali dati per il territorio italiano viene presentata in questo studio la realizzazione di un data-set con risoluzione di trenta secondi d'arco, per le temperature massime e minime giornaliere per il Trentino Alto Adige, per il periodo che va dal 1951 al 2014. La metodologia utilizzata per proiettare i dati meteorologici di un set di stazioni su di un grigliato ad alta risoluzione si basa sull'assunzione che la struttura spazio-temporale del campo di una variabile meteorologica su una determinata area possa essere descritta dalla sovrapposizione di due campi:i valori normali relativi e un periodo standard, ovvero la climatologia,e le deviazioni da questi, ovvero le anomalie. La climatologia mensile verrà interpolata sull'intero dominio tramite una regressione lineare pesata della temperatura rispetto alla quota,stimata separatamente per ogni nodo del grigliato,con pesi legati alla topografia del territorio,in modo da attribuire di volta in volta la massima importanza alle stazioni con caratteristiche più simili a quella del punto di griglia considerato. Da questa sarà possibile tramite la sovrapposizione con le anomalie mensili ricostruite sul medesimo grigliato, ottenute mediante un'interpolazione basata su una media pesata,ottenere un grigliato a 30 secondi d'arco, di serie temporali mensili in valori assoluti. Combinando poi l'interpolazione dei rapporti delle anomalie giornaliere relative alla media mensile per un set di stazioni con i campi mensili precedentemente stimati,sarà possibile costruire il data-set a risoluzione giornaliera. Prima di quest'ultima fase sarà necessario effettuare un'operazione di sincronizzazione dei dati giornalieri per assicurarsi che non vi siano sfasamenti nelle serie utilizzate. I risultati confermano l'efficacia nell'utilizzo di tale metodo su regioni orograficamente complesse, sia nel confronto diretto con i casi di studio,nei quali si nota bene la discriminazione spaziale effettuata dal modello, che nella valutazione dell'accuratezza e della precisione dei risultati. I dati ottenuti non sono affetti da errori sistematici,mentre l'errore medio assoluto risulta pari od inferiore ai $2^{\circ}$C, in linea con precedenti studi realizzati su altre aree alpine. Il metodo e i risultati risultano soddisfacenti ma ulteriormente migliorabili, sia tramite un ulteriore ottimizzazione del modello usato, che con un aumento nella qualità dei dati sui quali è stato svolto lo studio.

The aim of this study involving 170 patients suffering from non-specific low back pain was to test the validity of the spinal function sort (SFS) in a European rehabilitation setting. The SFS, a picture-based questionnaire, assesses perceived functional ability of work tasks involving the spine. All measurements were taken by a blinded research assistant; work status was assessed with questionnaires. Our study demonstrated a high internal consistency shown by a Cronbach's alpha of 0.98, reasonable evidence for unidimensionality, spearman correlations of >0.6 with work activities, and discriminating power for work status at 3 and 12 months by ROC curve analysis (area under curve = 0.760 (95% CI 0.689-0.822), respectively, 0.801 (95% CI 0.731-0.859). The standardised response mean within the two treatment groups was 0.18 and -0.31. As a result, we conclude that the perceived functional ability for work tasks can be validly assessed with the SFS in a European rehabilitation setting in patients with non-specific low back pain, and is predictive for future work status.

Detrital zircon and igneous zircon U-Pb ages are reported from Proterozoic metamorphic rocks in northern New Mexico. These data give new insight into the provenance and depositional age of a >3-km-thick metasedimentary succession and help resolve the timing of orogenesis within an area of overlapping accretionary orogens and thermal events related to the Proterozoic tectonic evolution of southwest Laurentia. Three samples from the Paleoproterozoic Vadito Group yield narrow, unimodal detrital zircon age spectra with peak ages near 1710 Ma. Igneous rocks that intrude the Vadito Group include the Cerro Alto metadacite, the Picuris Pueblo granite, and the Penasco quartz monzonite and yield crystallization ages of 1710 +/- 10 Ma, 1699 +/- 3 Ma, and 1450 +/- 10 Ma, respectively. Within the overlying Hondo Group, a metamorphosed tuff layer from the Pilar Formation yields an age of 1488 +/- 6 Ma and represents the first direct depositional age constraint on any part of the Proterozoic metasedimentary succession in northern New Mexico. Detrital zircon from the overlying Piedra Lumbre Formation yield a minimum age peak of 1475 Ma, and similar to 60 grains (similar to 25%) yield ages between 1500 Ma and 1600 Ma, possibly representing non-Laurentian detritus originating from Australia and/or Antarctica. Detrital zircons from the basal metaconglomerate and the middle quartzite member of the Marquenas Formation yield minimum age peaks of 1472 Ma and 1471 Ma, consistent with earlier results. We interpret the onset of ca. 1490-1450 Ma deposition followed by tectonic burial, regional Al2SiO5 triple-point metamorphism, and ductile deformation at depths of 12-18 km to reflect a Mesoproterozoic contractional orogenic event, possibly related to the final suturing of the Mazatzal crustal province to the southern margin of Laurentia. We propose to call this event the Picuris orogeny.

Maternal effects are a mother¿s non-genetic contributions to development that alter phenotypic traits in offspring. Maternal effects can take the form of prenatal allocation of resources, such as the deposition of androgens into egg yolks. For example, elevated yolk testosterone increases male sexual behaviors such as copulation solicitation and courtship displays in some avian species, in addition to aggressive behaviors like pecks and intimidating postures towards same-sex competitors. However, the mechanism connecting in ovo testosterone exposure with changes in sexual and aggressive behaviors has yet to be elucidated. While testosterone released by the gonads is important in the activation of sexual behaviors, it must undergo conversion to estrogen by the enzyme aromatase in the pre-optic area (POA) of the avian brain for full expression of sexual activity. POA aromatase is also necessary for the activation of aggressive behaviors in male birds. This experiment tested the hypothesis that elevated yolk testosterone leads to changes in POA aromatase activity and levels of gonadal testosterone, as these two endocrine parameters may mediate the effect of yolk testosterone on the frequency of sexual and aggressive behaviors. The effect of elevated yolk testosterone on gonadal testosterone levels and aromatase activity in the POA of 3-day-old domestic chickens Gallus gallus domesticus was investigated. Unincubated eggs were injected with either 10 ng testosterone in 50 ¿L sesame oil (¿T chicks¿) or 50 ¿L sesame oil (¿C chicks¿). At 3 days post-hatch, gonadal testosterone content was measured after steroid extraction using an EIA, and aromatase activity in the POA was quantified by measuring the production of tritiated water from [1ß-3H]-androstenedione. I predicted that gonadal testosterone levels and brain aromatase activity would be higher in T chicks, however found no difference between treatments. Though juvenile T production peaks at 3 days post-hatch, it is possible that the reproductive systems, including the testes and POA, are not fully developed at this time.

The aim of the present study was to determine effects of lactation on basal LH and IGF-1 concentrations and on the LH response to a GnRH-analogue at different stages of the oestrous cycle in mares. A total of 17 cyclic Haflinger mares were included in the study. Experiments were performed on lactating mares in first postpartum oestrus, the subsequent early luteal phase, and second postpartum oestrus. Non-lactating mares were used in oestrus and early luteal phase. Blood samples were taken for 1 h at 15 min intervals. Mares were then injected with the GnRH-analogue buserelin (GnRHa; 5 microg i.v.) and blood samples were drawn every 15 min for further 2 h. LH in all samples and basal IGF-1-concentrations were determined by RIA. In lactating mares, basal LH concentrations during the early luteal phase tended to be lower (p = 0.07) and the LH response to GnRHa, calculated as area under the curve, was significantly less pronounced compared to non-lactating mares (p < 0.01). As well in lactating mares, the basal LH concentration between first early luteal phase and second oestrus differed significantly (p < 0.05) and the net response to GnRHa was significantly lower between first oestrus as well as second oestrus and first early luteal phase (p < 0.05) but not between first and second oestrous postpartum. Within the group of non-lactating mares, the LH response to GnRHa was as well significantly lower during oestrus than during early luteal phase (p < 0.01). IGF-1 concentrations differed neither between groups nor stages of the cycle within groups. In conclusion, basal and GnRHa-stimulated LH release in lactating mares is lower than in non-lactating mares. This difference, however, occurs only in the early luteal phase. In lactating mares, concentrations of LH appear adequate to allow ovulation to occur.

We prospectively investigated urinary iodine concentration (UIC) in pregnant women and in female, non-pregnant controls in the canton of Berne, Switzerland, in 1992. Mean UIC of pregnant women [205 +/- 151 microg iodine/g creatinine (microg l/g Cr); no. = 153] steadily decreased from the first (236 +/- 180 microg l/g Cr; no. = 31) to the third trimester (183 +/- 111 microg l/g Cr, p < 0.0001; no. = 66) and differed significantly from that of the control group (91 +/- 37 microg l/g Cr, p < 0.0001; no. = 119). UIC increased 2.6-fold from levels indicating mild iodine deficiency in controls to the first trimester, demonstrating that high UIC during early gestation does not necessarily reflect a sufficient iodine supply to the overall population. Pregnancy is accompanied by important alterations in the regulation of thyroid function and iodine metabolism. Increased renal iodine clearance during pregnancy may explain increased UIC during early gestation, whereas increased thyroidal iodine clearance as well as the iodine shift from the maternal circulation to the growing fetal-placental unit, which both tend to lower the circulating serum levels of inorganic iodide, probably are the causes of the continuous decrease of UIC over the course of pregnancy. Mean UIC in our control group, as well as in one parallel and several consecutive investigations in the same region in the 1990s, was found to be below the actually recommended threshold, indicating a new tendency towards mild to moderate iodine deficiency. As salt is the main source of dietary iodine in Switzerland, its iodine concentration was therefore increased nationwide in 1998 for the fourth time, following increases in 1922, 1965 and 1980.

BACKGROUND: Elderly individuals who provide care to a spouse suffering from dementia bear an increased risk of coronary heart disease (CHD). OBJECTIVE: To test the hypothesis that the Framingham CHD Risk Score would be higher in dementia caregivers relative to non-caregiving controls. METHODS: We investigated 64 caregivers providing in-home care for their spouse with Alzheimer's disease and 41 gender-matched non-caregiving controls. All subjects (mean age 70 +/- 8 years, 75% women, 93% Caucasian) had a negative history of CHD and cerebrovascular disease. The original Framingham CHD Risk Score was computed adding up categorical scores for age, blood lipids, blood pressure, diabetes, and smoking with adjustment made for sex. RESULTS: The average CHD risk score was higher in caregivers than in controls even when co-varying for socioeconomic status, health habits, medication, and psychological distress (8.0 +/- 2.9 vs. 6.3 +/- 3.0 points, p = 0.013). The difference showed a medium effect size (Cohen's d = 0.57). A relatively higher blood pressure in caregivers than in controls made the greatest contribution to this difference. The probability (area under the receiver operator curve) that a randomly selected caregiver had a greater CHD risk score than a randomly selected non-caregiver was 65.5%. CONCLUSIONS: Based on the Framingham CHD Risk Score, the potential to develop overt CHD in the following 10 years was predicted to be greater in dementia caregivers than in non-caregiving controls. The magnitude of the difference in the CHD risk between caregivers and controls appears to be clinically relevant. Clinicians may want to monitor caregiving status as a routine part of standard evaluation of their elderly patients' cardiovascular risk.
