The principal constituent of cannabis, Delta(9)-tetrahydrocannabinol (THC), is moderately effective in treating nausea and vomiting, appetite loss, and acute and chronic pain. Oral THC (dronabinol) and the synthetic cannabinoid, nabilone, have been registered for medical use in the US and UK, but they have not been widely used because patients find it difficult to titrate doses of these drugs. Advocates for the medical use of cannabis argue that patients should be allowed to smoke cannabis to relieve these above-mentioned symptoms. Some US state governments have legislated to allow the medical prescription of cannabis, but the US federal government has tried to prevent patients from obtaining cannabis and threatened physicians who prescribe it with criminal prosecution or loss of their licence to practise. In the UK and Australia, committees of inquiry have recommended medical prescription (UK) and exemption from criminal prosecution (New South Wales, Australia), but governments have not accepted these recommendations. The Canadian government allows an exemption from criminal prosecution to patients with specified medical conditions. It has recently legislated to provide cannabis on medical prescription to registered patients, but this scheme so far has not been implemented. Some advocates argue that legalising cannabis is the only way to ensure that patients can use it for medical purposes. However, this would be contrary to international drug control treaties and is electorally unpopular. The best prospects for the medical use of cannabinoids lie in finding ways to deliver THC that do not involve smoking and in developing synthetic cannabinoids that produce therapeutic effects with a minimum of psychoactive effects. While awaiting these developments, patients with specified medical conditions could be given exemptions from criminal prosecution to grow cannabis for their own use, at their own risk.

Since the pioneering work of Charles Nicolle in 1909 [see Gross (1996) Proc Natl Acad Sci USA 93:10539-10540] most medical officers and scientists have assumed that body lice are the sole vectors of Rickettsia prowazekii, the aetiological agent of louse-borne epidemic typhus (LBET). Here we review the evidence for the axiom that head lice are not involved in epidemics of LBET. Laboratory experiments demonstrate the ability of head lice to transmit R. prowazekii, but evidence for this in the field has not been reported. However, the assumption that head lice do not transmit R. prowazekii has meant that head lice have not been examined for R. prowazekii during epidemics of LBET. The strong association between obvious (high) infestations of body lice and LBET has contributed to this perception, but this association does not preclude head lice as vectors of R. prowazekii. Indeed, where the prevalence and intensity of body louse infections may be high (e.g. during epidemics of LBET), the prevalence and intensity of head louse infestations is generally high as well. This review of the epidemiology of head louse and body louse infestations, and of LBET, indicates that head lice are potential vectors of R. prowazekii in the field. Simple observations in the field would reveal whether or not head lice are natural vectors of this major human pathogen.

The work of Michel Foucault sees modern penal technology its ann expression of power that operates through and is motivated by a dry instrumental reason. This article draws upon Durkheim and Bakhtin to advance a radically alternative approach. It is suggested that such technology is invested with sacred and profane symbolism and is understood via emotionally charged, dramatically compelling narrative frames. Tensions between official and unauthorized discourses can be understood through a center/periphery model of culture. In an extended case study of the guillotine, it is shown dial the apparatus was initially legitimated as an expression of a sacred revolutionary code. Such a discourse was subsequently destabilized by popular medical debates that raised the specter of pain after decapitation. While inconclusive, these new motifs mobilized Gothic and grotesque themes that confronted the rationalist aesthetics of the guillotine. A situation of Bakhtinian hetoroglossia eventuated. Uncertainty, the uncanny and fable entered a discursive field of increasing complexity.

BACKGROUND: Increasing levels of physical inactivity and sedentariness are contributing to the current overweight and obesity epidemic. In this paper, the findings of two recent studies are used to explore the relationships between sitting time ( in transport, work and leisure), physical activity and body mass index (BMI) in two contrasting samples of adult Australians. METHODS: Data on sitting time, physical activity, BMI and a number of demographic characteristics were compared for participants in two studies-529 women who were participants in a preschool health promotion project ('mothers'), and 185 men and women who were involved in a workplace pedometer study ('workers'). Relationships between age, number of children, physical activity, sitting time, BMI, gender and work patterns were explored. Logistic regression was used to predict the likelihood of being overweight or obese, among participants with different physical activity, sitting time and work patterns. In modern magnetic resonance imaging (MRI), patients are exposed to strong, time-varying gradient magnetic fields that may be able to induce electric fields (E-fields)/currents in tissues approaching the level of physiological significance. In this work we present theoretical investigations into induced E-fields in the thorax, and evaluate their potential influence on cardiac electric activity under the assumption that the sites of maximum E-field correspond to the myocardial stimulation threshold (an abnormal circumstance). Whole-body cylindrical and planar gradient coils were included in the model. The calculations of the induced fields are based on an efficient, quasi-static, finite-difference scheme and an anatomically realistic, whole-body model. The potential for cardiac stimulation was evaluated using an electrical model of the heart. Twelve-lead electrocardiogram (ECG) signals were simulated and inspected for arrhythmias caused by the applied fields for both healthy and diseased hearts. The simulations show that the shape of the thorax and the conductive paths significantly influence induced E-fields. In healthy patients, these fields are not sufficient to elicit serious arrhythmias with the use of contemporary gradient sets. However, raising the strength and number of repeated switching episodes of gradients, as is certainly possible in local chest gradient sets, could expose patients to increased risk. For patients with cardiac disease, the risk factors are elevated. By the use of this model, the sensitivity of cardiac pathologies, such as abnormal conductive pathways, to the induced fields generated by an MRI sequence can be investigated. Objective: To examine the association between gain in motor and cognitive functional status with patient satisfaction 3-6 mo after rehabilitation discharge. Design: Patient satisfaction and changes in functional status were examined in 18,375 patients with stroke who received inpatient medical rehabilitation. Information was obtained from 144 hospitals and rehabilitation facilities contributing records to the Uniform Data System for Medical Rehabilitation and the National Follow-up Services. Results: Data analysis revealed significant (P < 0.05) differences in satisfaction responses based on whether information was collected from patient self-report or from a family member proxy, and the two subsets were analyzed separately. Logistic regression revealed the following significant predictors of satisfaction for data collected from stroke patients: cognitive and motor gain, rehospitalization, who the patient was living with at follow-up, age, and follow-up therapy. In the patient-reported data subset, compared with patients who showed improved cognitive or motor functional status, those with no change, respectively, had a 31% and 33% reduced risk of dissatisfaction. In addition, rehospitalized patients had a higher risk of dissatisfaction. For the proxy reported data subset, significant influences on satisfaction were health maintenance, rehospitalization, stroke type, ethnicity, cognitive FIM(TM) gain, length of stay, and follow-up therapy. Conclusions: Ratings of satisfaction with rehabilitation services were affected by change in functional status and whether the information was collected from patient rating or proxy response.

This invited editorial, reflecting on expectations of changing to graduate entry, eg enhanced maturity in the student cohort with greater self-sufficiency and taking of responsibility for learning in the context of adoption of a problem-based learning model, examines experiences of early post-change years and raises questions for contemplation by medical schools considering graduate entry.

The Commonwealth Government's Principles Based Review of the Law of Negligence recently recommended reforms aimed at limiting liability and damages arising from personal injury and death, in response to the growing perception that the current system of compensating personal injury had become financially unsustainable. Recent increases in medical liability and damages have eroded the confidence of doctors and their professional bodies, with fears of unprecedented desertion from and reduced recruitment into high risk areas, and one of the primary foci of the review concerned medical negligence. The article analyses proposals to redefine the principles necessary for the finding of negligence, against the terms of reference of the review. The article assumes that for the foreseeable future, Australia will persist with tort-based compensation for personal injury rather than developing a no-fault scheme. If the suggested changes to the fundamental principles of negligence are unlikely to reduce medical liability, greater attention might be given to the processes which come into play after the finding of negligence, where reform is more likely to benefit both plaintiffs and defendants.

Trata-se de estudo qualitativo abordando as relações entre saúde e trabalho de professores de uma escola da rede pública na cidade de Vitória, capital do estado do Espírito Santo. Seu objetivo foi cartografar as vivências de prazer e sofrimento dos docentes, principalmente as estratégias criadas para defenderem-se das nocividades do ambiente de trabalho e produzirem saúde. Afirma a indissociabilidade entre modos de trabalhar e modos de subjetivar, e constata que a deterioração do sistema público de ensino tem produzido seus efeitos no trabalho do professorado, comprometendo a saúde desses profissionais e gerando, muitas vezes, seu afastamento das funções por meio de licenças médicas ou mesmo abandono da profissão. Procura dar visibilidade às ações que apontam para a recriação do trabalho, mesmo dentro de condições adversas.

O artigo trata a questão da esquizofrenia, tomada como um signo negativo quando se instala como patologia no campo das psicoses. Afirma que o discurso médico psiquiátrico celebrou seu advento e comemorou tais possibilidades que o pensamento "freudo-bleuleriano" abriu e desvelou. Considera que a carreira da esquizofrenia, atravessando o movimento político da antipsiquiatria, confrontou-se com outra sorte de experiências, sendo registrada segundo uma nova lógica que, sob as condições impostas pelo acontecimento freudiano, transformou o saber no século XX. Afirma que Gilles Deleuze compreendeu esse aturdimento, recolheu a parte que lhe cabia e documentou-a, em particular em sua obra Lógica do Sentido quando inverteu a polaridade dos signos, positivando a esquizofrenia.

O objetivo desta tese é analisar que práticas de tradução educacional foram feitas em relação às crianças e jovens excluídos da/na escola da Primeira República (1889-1930) no Estado do Espírito Santo. Período este em que a cultura ocidental, mais especificamente a europeia, exercia forte influência nas políticas de regulação e de emancipação do Brasil. Assim constando, na área educacional, muitas das experiências e teorias defendidas e socializadas na Europa foram traduzidas para o Brasil, e por extensão, para o Espírito Santo. Para compreender as traduções na área educacional, situada no contexto da modernidade é que se reportou a Boaventura de Sousa Santos, à luz das ferramentas disponíveis pela Sociologia das Ausências, pela Sociologia das Emergências, bem como o Trabalho de Tradução. A proposta de pesquisa está ancorada numa perspectiva qualitativa de base hermenêutica e, para construir a investigação, foram utilizadas fontes históricas bibliográficas e documentais. As fontes de investigação permitiram explicar parte das possíveis razões para as exclusões da escola ou defesas de segregação (exclusão na escola). Estas exclusões se concentram em torno de cinco tempos: 1909, 1917, 1923, 1924 e 1929. Nos anos de 1909 e 1924, disserta-se pela via educacional, sobre as práticas da área jurídica em relação aos órfãos. A Escola Normal do Espírito Santo, por sua vez, reproduzia a ideia de ordem ao atrelar a Pedagogia à educação cívica. Em 1917, a partir do episódio relativo a uma jovem, associada a um possível caso de loucura/alienação mental, é abordada a influência da área médica na área educacional, no Estado do Espírito Santo. Quanto à década de 20, existiu todo um discurso sobre o estudo da criança, na formação de educadores, como tentativa de apreender a totalidade do ser humano. Algumas práticas de tradução construtivas na área educacional foram evidenciadas, como por exemplo, quando o professor Elpídio Pimentel sugere que os educadores trabalhem com o que o aluno sabe ou poderá aprender. Ao final do trabalho, percebe-se que, muitas exclusões poderiam ter sido evitadas, mas para isso, a sociedade, os educadores teriam que ter adotado uma outra postura em relação às práticas de tradução: talvez uma menos idealizada e que valorizasse o que emergia de potencial nas realidades locais.
