Introduction: Obesity is a risk factor for postoperative pulmonary complications. Overweight can alter respiratory mechanics and decrease lung volumes and capacities. Bariatric surgery (BS) has been used as treatment for morbid obesity, but this surgery can lead to respiratory complications. Objective: To evaluate pulmonary function in morbid obese patients undergoing bariatric surgery. Methods: The patients were submitted to pulmonary function testing, including slow and forced vital capacity (SVC and FVC), forced expiratory volume in the first second (FEV1), Tiffenau index (FEV1/FVC), forced expiratory flow 25-75% (FEF25-75%) and maximal inspiratory and expiratory pressure (MIP and MEP). Descriptive analysis was performed. Results: We evaluated 18 patients, mean age of 37.33 ± 11.23 years, height 1.65 ± 0.09 m, weight 125.92 ± 25.24 kg and BMI 46.34 ± 9 20 kg/m2 , SVC 100.64 ± 22.91%, FVC 102.04 ± 21.82%, FEV1 104.85 ± 22.16%, FEV1 /FVC 83.54 ± 6.63% and FEF25-75% 84.47 ± 25.74%, 99.75 ± 24.55 for MIP and MEP 102.12 ± 25.06 cmH2O. Conclusion: The bariatric surgery candidates had normal values for manovacuometry and spirometry.

Cataract is the leading cause of blindness in dogs. It is defined as an opacity in the lens and is independent of resulting visual deficit. Although there are a growing number of studies related to drug therapy of cataract, the treatment is still considered exclusively surgical. Among the techniques used to remove cataract, phacoemulsification is the best performer. Anesthesia in intraocular surgery has several peculiarities, which are essential to successfully conduct phacoemulsification in dogs. In these patients, a safe and effective anesthesia requires the maintenance of intraocular pressure close to normal, preventing the activation of the oculo-cardiac reflex and complete immobilization of the eye. The appropriate conditions for such surgical procedures can be established through the use of drugs from different pharmacological groups for premedication, induction and maintenance of anesthesia, making a balanced anesthesia. This work was compiled from a careful review of the literature on anesthesia in cataract surgery. Considerations on the pre-medication, induction and maintenance of anesthesia are discussed, aiming to contribute to the phacoemulsification becomes more safe and suitable for the anesthesiologist and the surgeon as well as comfortable for dogs undergoing surgery

Tricyclic antidepressants, such as amitriptyline, are inhibitors of serotonin and norepinephrine neuronal reuptake and this action has been implied in changes in pain threshold supporting its use to alleviate neuropathic pain. Although is known that 1 adrenoceptors participate in the antinociceptive effect of amitriptyline it is unclear which receptor subtype is the target for the increased synaptic levels of norepinephrine resultant from the inhibition of neuronal uptake. Paradoxically, several tricyclic antidepressants including amitriptyline also behave as antagonists of 1 adrenoceptors with different affinities for its subtypes: these drugs have 10 to 100-fold higher affinities for 1A than for 1B and 1D adrenoceptors. This work investigated the involvement of 1 adrenoceptors subtypes in the antinociceptive effect of the amitriptyline in a constriction of the sciatic nerve in rats by determining the effects of subtype selective 1 adrenoceptors antagonists. Fifteen days later, mechanical hyperalgesia was analyzed in a Randall-Selitto test. The 1A-selective antagonist RS100329 was the most potent antagonist of the contractions of the rat prostate, whereas the 1D-selective antagonist BMY 7378 (up to 100g/Kg) was unable to affect these contractions. The antagonist prazosin, BMY 7378 and 5-methyl urapidil inhibited the antinociceptive effect of the amitriptyline. However, the highly selective 1A adrenoceptor antagonist RS100329 was unable to affect the antinociception induced by amitriptyline. These results point out that 1B and/or 1D adrenoceptors, but not 1A, are involved in the antinociceptive effects of amitriptyline

The aim of this work is to study a still little known technique in the Brazilian Veterinary Medicine, that is the use of the Platelet Rich Plasma (PRP), specifically in the small animals orthopedics, including the concept of PRP, why its use in orthopedics, its production method and application, its indications, vantages and disadvantages, and also evaluate the results achieved through the use of PRP both in Human Medicine as in Veterinary Medicine. Moreover, it aims to establish the feasibility of this technique for its use in veterinary orthopedic surgery, especially as an adjunct in fracture repair

Nos últimos anos vem aumentando o reconhecimento mundial sobre a relevância da nutrição como pilar básico para o desenvolvimento econômico e social. Milhares de crianças têm seu crescimento retardado pela má nutrição, no entanto poucos trabalhos relacionam a desnutrição proteica materna e o desenvolvimento do sistema reprodutor masculino. Dessa forma, o presente trabalho tem por objetivo investigar os possíveis efeitos da restrição proteica gestacional e durante a lactação sobre a morfogênese do complexo prostático, tendo em vista que a etiologia das doenças prostáticas relaciona-se com fatores epigenéticos, e dentre eles, fatores toxicológicos e nutricionais. Após o acasalamento e diagnóstico da prenhês, dois grupos de mães foram constituídos: Normoproteico (NP, 17% proteína, n=7) e Hipoproteico (LP, 6% proteína, n=7). No segundo dia pós-nascimento (DPN2), as ninhadas foram inspecionadas e reduzidas para no máximo oito animais (preferencialmente machos), a distância anogenital foi mensurada e, 2 machos por ninhada foram mortos por decapitação para coleta do broto prostático no dia do nascimento. Após o nascimento dos filhotes, um grupo de mães do grupo hipoproteico continuou recebendo a ração hipoproteica (n=3), enquanto as demais passaram a receber a ração normoproteica (n=3). No dia 22, dia do desmame, 2 machos de cada ninhada foram pesados, mortos por decapitação e o complexo prostático foi coletado para as análises morfológicas e imunoistoquímicas. Foram investigados a morfologia geral e o padrão de distribuição dos elementos celulares e da matriz extracelular por métodos citoquímicos e morfométricos, e a imunorreatividade para os seguintes marcadores: receptor de andrógeno (AR), Ki67, α-actina, EGF-R e P63. Os índices de proliferação e morte celular (TUNEL) foram calculados. A prole LP apresentou uma redução significativa do peso corporal e da distância anogenital no DPN2. ...

Radiotherapy is a branch of medical physics related to the treatment of malignant neoplasm, being an important instrument in the fight against cancer, when combined with the effort of a multidisciplinary team, composed of, physicians, physicists, nurses and technicians. Every year more than 3.5 million new cases of cancer are recorded in the world, being the prostate cancer responsible for approximately 25% of this amount (INCA and IARC, 2008). In this type of cancer, radiotherapy is a method indicated for treatement. The technological advance in this area over years has allowed a greater accuracy in the tumor location, more conformation of the radiation beam around the tumor, reducing the dose in healthy tissues and a consequent dose increase on treatment (Bedford et al., 1999). A radiotherapy planning, in which the physicist develops an important role, is composed of several steps, including choosing the best configuration of treatment beams. This choice has a close relationship with success of therapy and is critical to achieve the best distribution of dose inside the tumor and expose the least as possible the healthy tissue to radiation. In this work, two options for setting up camps in the first phase in a treatment of prostate cancer were simulated in computer planning: 4 fields orthogonal or “Box” with gantry angles in 00, 1800, 2700 e 90° and 4 fields angled or “X” (1350, 450, 3150 e 2250). The percentage of the rectal volume exposed to 40, 50, 60, 72 and 76 Gy should be limited to 60, 50, 25, 15 and 5% respectively (Greco et al., 2003). The femoral toxicity have limited dose by 70% of the total dose prescribed in a prostate treatment (Bedford et al., 1999). The planning of 27 patients with prostate adenocarcinoma submitted to 3D conformal radiotherapy were accompanied. As a result, it was assessed that the best TCP (tumor control probability)... Adequate testosterone levels are necessary for the development, growth and maintenance of the male reproductive system. Testosterone deficiency is common in men with diabetes in whom it may contribute to impaired performance, with consequent reduction of the activity of the androgen regulated organs, such as the prostate. However, little attention has been given to the plasma dihydrotestosterone (DHT) level, the most potent androgen, nor to the expression of the androgen receptor (AR), insulin-like growth factor type I (IGF-1) and receptor (IGF-1R) in target tissues. Here, we investigated the effect of type I diabetes mellitus on DHT plasma levels and on prostate AR, IGF-1 and IGF-1R expression during rat pubertal growth. Diabetes was induced in prepubertal male rats through administration of streptozotocin (STZ; 40 mg/kg). Diabetic, diabetic treated with insulin, and age-matched control animals were killed by overdoses of pentobarbital. The ventral prostatic lobe (VP) was dissected, weighed and processed for immunohistochemistry for AR, IGF-1 e IGF-1R; plasma T and DHT levels were also determined. Hyperglycemia at puberty reduced VP weight gain to about 50% and plasma T level to about 80% of the control levels. In contrast there were no changes in plasma DHT levels. Insulin replacement restored the VP weight gain, but not the plasma T levels, which remained 90% below the ones of controls. Immunohistochemistry showed that AR, IGF-1 and IGF-1R expression in the prostate epithelial cells did not change with hyperglycemia or insulin replacement. Thus, the AR expression in the prostate epithelial cells appears to be regulated by DHT, and to a minor extent it also controls glandular growth

The goal of this study is to characterize the clinical outline of patients in postoperative of bariatric surgery attended in an Intensive Care Unit and identify the nursing diagnosis presented by these patients in accordance to Taxonomy II of North American Nursing Diagnosis Association (NANDA). This is a transversal, descriptive and prospective study that was desenvolved at the Intensive Care Unit of the Clinical Hospital of Botucatu Medical School – UNESP. The population was composed by all the patients who were submitted to the bariatric surgery and were attended at the Intensive Care Unit in the period between June and August of 2010, totalizing 13 patients. The data were collected based on the Nursing History that is composed by the Anamnesis, Physical Examination and Complementary Exams. The results were worked out: a) among the 13 patients studied, 10 are women (76,9%) and 3 men (23,1%); the majority is between 30 and 40 years old; 11 people of 13 are morbid obese, which means that they have Body Mass Index higher than 40 kg/m2; b) It was identified 22 nursing diagnosis and 14 of these were found in 100% of the patients. They belong to the dominions of Activity/Rest, Comfort, Elimination and Changing, Security and Protection, Functions and Relationships. Therefore, in relation to the factors, it was conclude that nursing diagnosis help nurses to understand the unsettled human answers or the risks to disturbance in an individual way, which contributes to plan specific interventions to these patients. It was possible to verify that the stage of the nursing diagnosis that is part of the nursing process should be executed by the nurses systematically, because it has been realized by the nurses’ clinical idea, but not in a systematical way

The study was performed to verify if there are changes in salivary pH in patients with hyperhidrosis who were treated by Videothoracoscopic Sympathectomy. Twelve patients underwent sympathectomy with clamping the sympathetic chain along the vertebrae T2, T3 and T4, under inhalation and intravenous general anesthesia and with drainage of pneumothorax through probe N° 16. These patients had their saliva collected 1 day before and 7 days after surgery for measurement of pH by the colorimetric method with indicator paper. All patients showed no sign or symptom of hyperhidrosis in the immediate postoperative period, which demonstrates the success of sympathetic denervation on these levels. No patient had compensatory sweating and there was no post-operatively surgical complication. No drug or substance has been administered to patients for the study. The pH results obtained were analyzed using test ´´t´´ Student. Although the average pH in samples obtained after surgery was lower than the preoperative, the test revealed no statistically significant difference (p = 0.181). It is concluded that sympathectomy, an excellent therapeutic and definitive method for hyperhidrosis, does not alter the salivary pH, and so, did not influence the cariogenic process
