Esta dissertação descreve o levantamento da gestão de uma unidade fabril no âmbito do engarrafamento de águas, para a implementação de um Sistema de Gestão Ambiental (SGA) recorrendo á Norma NP EN ISO 14001:2012 tendo como objetivo proporcionar a melhoria contínua dos processos implementados na empresa bem como todos os benefícios que este referencial pressupõe. Foi realizada, numa primeira fase, uma auditoria diagnóstico com uma visita á fabrica no âmbito da implementação da ISO 14001, e posteriormente realizado todo o trabalho de aplicação ao referencial. Este trabalho consistiu a abordagem a todos os requisitos da norma ISO 14001 com principal foco na fase de Planeamento de um SGA, procurando-se responder essencialmente ao requisito 4.3 da referida norma. Procedeu-se á identificação das atividades da empresa (entradas e saídas), dos aspetos ambientais associados (respetiva significância) e levantamento dos requisitos legais aplicáveis (obrigações). Os restantes requisitos foram abordados e fez-se uma sugestão para o seu cumprimento, para além de que muitos já se encontram em cumprimento por compatibilidade com a norma ISO 9001 da qual a empresa já se encontra certificada. Esta dissertação pretende discutir e apresentar propostas para a implementação do SGA, possíveis vantagens daí decorrentes e as barreiras a ultrapassar (aspetos ambientais significativos) de maneiras a por em prática os requisitos do referencial normativo que venha a ser adotado. Foi realizada uma revisão bibliográfica destas temáticas e, ainda que estas medidas propostas não tenham sido implementadas, assim que implementadas esperam-se ganhos na economia de recursos como também outros benefícios descritos ao longo da dissertação que permita, com implementação do SGA não só enquanto estratégia, ser inovadora e competitiva.

To improve surgical safety, and to reduce the mortality and surgical complications incidence, the World Health Organization (WHO) developed the Surgical Safety Checklist (SSC). The SSC is a support of information that aids health professionals to reduce the number of complications, induction of anaesthesia, period before skin incision and period before leaving the operating room (OR). The SSC was tested in several countries of the world and their results shown that after introduction of the SSC the incidence of patient complication lowered from 11.0% to 7.0% (P<0.001), the rate of death declined from 1.5% to 0.8% (P = 0.003) and the nurses recognized that patients identity was more often con rmed (81.6% to 94.2%, P<0.01) in many institutions. Recently the SSC was also implemented in Portuguese hospitals, which led us to its study in the real clinical environment. An observational study was performed: several health professionals were observed and interviewed, to understand the functioning of the SSC in an OR, during the clinical routine. The objective of this study was to understand the current use of the SSC, and how it may be improved in terms of usability, taking advantage of the technological advancements such as mobile applications. During two days were observed 14 surgeries, only 2 surgeries met the requirements for the three phases of the SSC, as de ned by the WHO. Of the remaining 12 observed surgeries, 9 surgeries completed the last phase at the correct time. It was also observed that only in 2 surgeries all the phases of the SSC were read aloud to the team and that, in 7 surgeries, several items were read aloud and answered but no one was checking the SSC, only after the end of the phase. The observational study results disclose that several health professionals do not meet with rules of the WHO manual. This study demonstrates that it is urgent to change the mindset of health professionals, and that di erent features in the SSC may be useful to make it more easy to use. With the results of the observational study, a SSC application proposal was developed with new functionalities to improve and aid the health professional in its use. In this application the user can chose between a SSC already created to a speci c surgery or to create a new SSC, adding and adapting some questions from the WHO standard. To create a new SSC, the application is connected to an online questionnaire builder (JotForm). The choice for this online questionnaire builder went through three essential characteristics: number of types of questions, mainly checkbox, radio button and text; the possibility of to create sections inside sections and the API. In addition, in this proposal the improvements are focused in forcing the user to focus in the work ow of the SSC and to save the input timestamps and any actions made by them. Therefore, the following features was implemented to achieve that goal: display one item of the SSC at a time; display the stage where the SSC is; do not allow going back to the previous step; do not allow going forward to the next item if the current is not lled; do not allow going forward to the next item if the time it took to ll the item was too short and log any action made by the user.

BACKGROUND: Furniture companies can analyze their safety status using quantitative measures. However, the data needed are not always available and the number of accidents is under-reported. Safety climate scales may be an alternative. However, there are no validated Portuguese scales that account for the specific attributes of the furniture sector. OBJECTIVE: The current study aims to develop and validate an instrument that uses a multilevel structure to measure the safety climate of the Portuguese furniture industry. METHODS: The Safety Climate in Wood Industries (SCWI) model was developed and applied to the safety climate analysis using three different scales: organizational, group and individual. A multilevel exploratory factor analysis was performed to analyze the factorial structure. The studied companies’ safety conditions were also analyzed. RESULTS: Different factorial structures were found between and within levels. In general, the results show the presence of a group-level safety climate. The scores of safety climates are directly and positively related to companies’ safety conditions; the organizational scale is the one that best reflects the actual safety conditions. CONCLUSIONS: The SCWI instrument allows for the identification of different safety climates in groups that comprise the same furniture company and it seems to reflect those groups’ safety conditions. The study also demonstrates the need for a multilevel analysis of the studied instrument.

O crescimento dos sistemas de informação e a sua utilização massiva criou uma nova realidade no acesso a experiências remotas que se encontram geograficamente distribuídas. Nestes últimos tempos, a temática dos laboratórios remotos apareceu nos mais diversos campos como o do ensino ou o de sistemas industriais de controlo e monitorização. Como o acesso aos laboratórios é efectuado através de um meio permissivo como é o caso da Internet, a informação pode estar à mercê de qualquer atacante. Assim, é necessário garantir a segurança do acesso, de forma a criar condições para que não se verifique a adulteração dos valores obtidos, bem como a existência de acessos não permitidos. Os mecanismos de segurança adoptados devem ter em consideração a necessidade de autenticação e autorização, sendo estes pontos críticos no que respeita à segurança, pois estes laboratórios podem estar a controlar equipamentos sensíveis e dispendiosos, podendo até eventualmente comprometer em certos casos o controlo e a monotorização de sistemas industriais. Este trabalho teve como objectivo a análise da segurança em redes, tendo sido realizado um estudo sobre os vários conceitos e mecanismos de segurança necessários para garantir a segurança nas comunicações entre laboratórios remotos. Dele resultam as três soluções apresentadas de comunicação segura para laboratórios remotos distribuídos geograficamente, recorrendo às tecnologias IPSec, OpenVPN e PPTP. De forma a minimizar custos, toda a implementação foi assente em software de código aberto e na utilização de um computador de baixo custo. No que respeita à criação das VPNs, estas foram configuradas de modo a permitir obter os resultados pretendidos na criação de uma ligação segura para laboratórios remotos. O pfSense mostrou-se a escolha acertada visto que suporta nativamente quaisquer das tecnologias que foram estudadas e implementadas, sem necessidade de usar recursos físicos muito caros, permitindo o uso de tecnologias de código aberto sem comprometer a segurança no funcionamento das soluções que suportam a segurança nas comunicações dos laboratórios remotos.

Analisando numa perspectiva mais ampla, esta Tese/Dissertação deverá ser uma ferramenta de apoio a quem necessite de implementar e integrar sistemas de gestão normalizados. Seguindo uma abordagem empresarial, visa ajudar os praticantes de uma filosofia Kaizen, a melhorar continuamente, acrescentando valor às suas organizações, através de um conjunto de estratégias e técnicas de carácter prático. Em termos mais específicos, o objectivo foi apresentar um caso prático onde se aplicassem essas mesmas ferramentas. O case-study incidiu na análise da implementação da certificação integrada da Qualidade, Ambiente e Segurança na Divisão de Equipamento Industrial da Toyota Caetano Portugal. Esta tese/dissertação retrata a implementação teórica e prática do SIG QAS, aplicando os requisitos definidos pela TMHE (Toyota Material Handeling Europe), entre os quais o TPS (Toyota Production System), Kaizen, Toyota Way, Elimination of Waste, Gemba, JIT, Genchi Genbutsu, etc. Os resultados obtidos foram positivos por várias razões. Por ter conseguido elaborar um documento simples de entender, com sugestões de fácil aplicação futura e porque as propostas resultantes do case-study permitirão à TCAP - DEI a promoção da qualidade, a eliminação do desperdício, a completa satisfação dos clientes pela análise, antecipação das suas necessidades e expectativas e pelo cumprimento dos requisitos legais Ambientais e de Segurança aplicáveis, adoptando boas práticas ambientais e de prevenção de acidentes. Temos assim o comprometimento de que esta Tese/Dissertação contribuirá de forma inabalável para o principal desígnio, de assegurar a melhoria contínua do desempenho, garantindo a sustentabilidade da Organização.

As empresas no século XXI têm necessidade de reduzir os seus custos de manutenção para serem competitivas num mercado cada vez mais global, implementando a Manutenção Lean no setor diretamente responsável pela Manutenção e também estender a toda a organização. Este trabalho foi realizado no âmbito do estágio efetuado na empresa Lameirinho, empresa líder de Têxtil-Lar, localizada na zona de Guimarães e que possui um Departamento de Manutenção onde a fiabilidade e disponibilidade dos equipamentos é um aspeto importante bem como a sua manutenção a custos reduzidos. Assim, tendo como objetivo a melhoria do Departamento de Manutenção da empresa Lameirinho foi efetuada uma auditoria de Manutenção Lean com o intuito de determinar o estado atual, foram identificados problemas e oportunidades de melhoria bem como foram apresentadas soluções para os mesmos. Os benefícios que se obtêm com a implementação das soluções são, entre outros: melhor planeamento das intervenções de Manutenção com alocação no momento necessário dos recursos humanos e a disponibilização de peças assim como as intervenções de manutenção serem efetuadas quando a Produção menos necessita do equipamento produtivo; redução e eliminação das intervenções de Manutenção Corretiva de Emergência; redução dos custos de Manutenção na globalidade da empresa através dos benefícios obtidos e com impacto direto nos resultados económicos da empresa.

In this study, we report on the safety and skin delayed-type hypersensitivity (DTH), responses of the Leishmania donovani whole cell sonicate antigen delivered in conjunction with alum-BCG (AlBCG), Montanide ISA 720 (MISA) or Monophosphoryl lipid A (MPLA) in groups of vervet monkeys. Following three intradermal injections of the inoculums on days 0, 28 and 42, safety and DTH responses were assessed. Preliminary tumor necrosis factor alpha (TNF-α) and interferon gamma (IFN-γ) levels were also measured and these were compared with DTH. Only those animals immunized with alum-BCG reacted adversely to the inoculum by producing ulcerative erythematous skin indurations. Non-parametric analysis of variance followed by a post-test showed significantly higher DTH responses in the MISA+Ag group compared with other immunized groups (p < 0.001). The MPLA+Ag group indicated significantly lower DTH responses to the sonicate antigen compared with the AlBCG+Ag group. There was a significant correlation between the DTH and cytokine responses (p < 0.0001). Based on this study we conclude that Leishmania donovani sonicate antigen containing MISA 720 is safe and is associated with a strong DTH reaction following immunization.

Foodborne diseases represent operational risks in industrial restaurants. We described an outbreak of nine clustered cases of acute illness resembling acute toxoplasmosis in an industrial plant with 2300 employees. These patients and another 36 similar asymptomatic employees were diagnosed with anti-T. gondii IgG titer and avidity by ELISA. We excluded 14 patients based on high IgG avidity and chronic toxoplasmosis: 13 from controls and one from acute disease other than T. gondii infection. We also identified another three asymptomatic employees with T.gondii acute infection and also anti-T. gondii IgM positive as remaining acute cases. Case control study was conducted by interview in 11 acute infections and 20 negative controls. The ingestion of green vegetables, but not meat or water, was observed to be associated with the incidence of acute disease. These data reinforce the importance of sanitation control in industrial restaurants and also demonstrate the need for improvement in quality control regarding vegetables at risk for T. gondii oocyst contamination. We emphasized the accurate diagnosis of indexed cases and the detection of asymptomatic infections to determine the extent of the toxoplasmosis outbreak.

Old timber structures may show significant variation in the cross section geometry along the same element, as a result of both construction methods and deterioration. As consequence, the definition of the geometric parameters in situ may be both time consuming and costly. This work presents the results of inspections carried out in different timber structures. Based on the obtained results, different simplified geometric models are proposed in order to efficiently model the geometry variations found. Probabilistic modelling techniques are also used to define safety parameters of existing timber structures, when subjected to dead and live loads, namely self-weight and wind actions. The parameters of the models have been defined as probabilistic variables, and safety of a selected case study was assessed using the Monte Carlo simulation technique. Assuming a target reliability index, a model was defined for both the residual cross section and the time dependent deterioration evolution. As a consequence, it was possible to compute probabilities of failure and reliability indices, as well as, time evolution deterioration curves for this structure. The results obtained provide a proposal for definition of the cross section geometric parameters of existing timber structures with different levels of decay, using a simplified probabilistic geometry model and considering a remaining capacity factor for the decayed areas. This model can be used for assessing the safety of the structure at present and for predicting future performance.

Introduction: Skin diseases in paediatric age are often distressing conditions with significant impact in children’s psychosocial development. Additionally, systemic therapeutic options are often limited in childhood, due to its potential toxicity in this vulnerable group. Phototherapy is therefore an endorsed option for photo-responsive dermatological conditions. Objective and Methods:This observational retrospective study aims to access efficacy and safety of Phototherapy in our paediatric population. Relevant clinical data from 1996 to present concerning patients aged 18 years or less was collected. Results: 78 patients were included, of which64,1%was female. Mean age was 12,9 years (range 2-18). Distribution according to diagnosis was:47,4%psoriasis, 34,6% alopecia areata, 9,0% vitiligo, 9,0% other diagnosis. Mean number of cycles was 1,5 (range 1-7), with an average of 16,3 treatments per cycle and mean cumulative dose 134 J/cm2. 70,5% was treated with one single cycle. Topic and systemic PUVA were the first choice in 37,2% and 39,7%, respectively, while UVB TL01 and broadband UVB were used in 11,5% each. On the first cycle 67,5% improved, 14,3% showed no sustained clinical response and 19,5% were lost to follow-up. Psoriasis patients had the best response rates (81,8%), followed by alopecia areata (59,3%). Side effects occurred in 21%, being erythema the most common (12%). None led to therapeutic interruption. Discussion: Phototherapy is a safe and effective option in childhood, yet the withdraw rate might be an important limitation.

PURPOSE: The Genous™ stent (GS) is designed to accelerate endothelization, which is potentially useful in the pro-thrombotic environment of ST-elevation acute myocardial infarction (STEMI). We aimed to evaluate the safety and effectiveness of the GS in the first year following primary percutaneous coronary intervention (PCI) and to compare our results with the few previously published studies. METHODS AND MATERIALS: All patients admitted to a single center due to STEMI that underwent primary PCI using exclusively GS, between May 2006 and January 2012, were enrolled. The primary study endpoints were major adverse cardiac events (MACEs), defined as the composite of cardiac death, acute myocardial infarction and target vessel revascularization, at one and 12months. RESULTS: In the cohort of 109 patients (73.4% male, 59 ±12years), 24.8% were diabetic. PCI was performed in 116 lesions with angiographic success in 99.1%, using 148 GS with median diameter of 3.00mm (2.50-4.00) and median length of 15mm (9-33). Cumulative MACEs were 2.8% at one month and 6.4% at 12months. Three stent thromboses (2.8%), all subacute, and one stent restenosis (0.9%) occurred. These accounted for the four target vessel revascularizations (3.7%). At 12months, 33.9% of patients were not on dual antiplatelet therapy. CONCLUSIONS: GS was safe and effective in the first year following primary PCI in STEMI, with an apparently safer profile comparing with the previously published data. SUMMARY: We report the safety and effectiveness of the Genous™ stent (GS) in the first year following primary percutaneous coronary intervention in ST-elevation acute myocardial infarction. A comprehensive review of the few studies that have been published on this subject was included and some suggest a less safe profile of the GS. Our results and the critical review included may add information and reinforce the safety and effectiveness of the GS in ST-elevation in acute myocardial infarction.
