Objectives To investigate the effect of Nintendo Wii (TM)-based motor cognitive training versus balance exercise therapy on activities of daily living in patients with Parkinson's disease. Design Parallel, prospective, single-blind, randomised clinical trial. Setting Brazilian Parkinson Association. Participants Thirty-two patients with Parkinson's disease (Hoehn and Yahr stages 1 and 2). Interventions Fourteen training sessions consisting of 30 minutes of stretching, strengthening and axial mobility exercises, plus 30 minutes of balance training. The control group performed balance exercises without feedback or cognitive stimulation, and the experimental group performed 10 Wii Fit (TM) games. Main outcome measure Section II of the Unified Parkinson's Disease Rating Scale (UPDRS-II). Randomisation Participants were randomised into a control group (n = 16) and an experimental group (n = 16) through blinded drawing of names. Statistical analysis Repeated-measures analysis of variance (RM-ANOVA). Results Both groups showed improvement in the UPDRS-II with assessment effect (RM-ANOVA P < 0.001, observed power = 0.999). There was no difference between the control group and the experimental group before training {8.9 [standard deviation (SD) 2.9] vs 10.1 (SD 3.8)}, after training [7.6 (SD 2.9) vs 8.1 (SD 3.5)] or 60 days after training [8.1 (SD 3.2) vs 8.3 (SD 3.6)]. The mean difference of the whole group between before training and after training was -0.9 (SD 2.3, 95% confidence interval -1.7 to -0.6). Conclusion Patients with Parkinson's disease showed improved performance in activities of daily living after 14 sessions of balance training, with no additional advantages associated with the Wii-based motor and cognitive training. Aquaculture of filter-feeding bivalve mollusks involves the fruitful conversion of marine particulate organic matter into premium protein of high nutritive value. Culture performance of bivalves is largely dependent on hydrological conditions and directly affected by e. g. temperature and chlorophyll levels. Accordingly, these parameters may be related with seasonality but also with oceanographic features combined with climate events. Yields of Pacific cupped oyster (Crassostrea gigas) reared at commercial procedures in suspended structures (long-lines) in a sheltered bay in Southern Brazil (Santa Catarina State, 27S 43'; 48 W 30') were evaluated in relation to local environmental conditions: sea surface temperature, chlorophyll a concentration, and associate effects of cold fronts events and El Nino and La Nina periods. Outputs from four consecutive commercial crop years were analyzed (2005/06, 2006/07, 2007/08, 2008/09) in terms of oyster survival and development time during the following grow-out phases of the culture cycle: seed to juvenile, juvenile to adult, adult to marketable. Since culture management and genetics were standardized significant differences verified among crop performance could be mostly related to environmental effects. Time series of temperature and chlorophyll a (remote sensing data) from crop periods displayed significant seasonal and interannual variation. As expected, performance during initial grow-out stages (seed to juvenile) was critical for final crop yield. Temperature was the main factor affecting survival in these initial stages with a trend of negative correlation, though not statistically significant. On the other hand, oyster development rate was significantly and positively affected by chlorophyll a concentration. Chlorophyll a values could be increased by upwelled cold nutrient-rich South Atlantic Central Water (SACW, related to predominant Northern winds) though further dependent on occurrence of Southern winds (cold fronts) to assist seawater penetration into the sheltered farming area. Lower salinity nutrient-rich northward drifted waters from La Plata River discharge may also result in chlorophyll a rise in the farming area. The El Nino period (July 2006 to February 2007) coincided with lower chlorophyll a levels in the farming site that may be related to both decreased number of cold fronts as well as predominance of Northern winds that retain northward spreading of La Plata River discharge waters. In contrast, the La Nina period (August 2007 to June 2008) corresponded to higher chlorophyll a values in the farming area by both upwelling of SACW and penetration of La Plata River discharge water assisted by increased occurrence of Southern winds and cold fronts. The recognition of the potentially changing climate and effects upon the environment will be an important step in planning future development of bivalve aquaculture.

Empirical approaches and, more recently, physical approaches, have grounded the establishment of logical connections between radiometric variables derived from remote data and biophysical variables derived from vegetation cover. This study was aimed at evaluating correlations of dendrometric and density data from canopies of Eucalyptus spp., as collected in Capao Bonito forest unit, with radiometric data from imagery acquired by the TM/Landsat-5 sensor on two orbital passages over the study site (dates close to field data collection). Results indicate that stronger correlations were identified between crown dimensions and canopy height with near-infrared spectral band data (rho(s)4), irrespective of the satellite passage date. Estimates of spatial distribution of dendrometric data and canopy density (D) using spectral characterization were consistent with the spatial distribution of tree ages during the study period. Statistical tests were applied to evaluate performance disparities of empirical models depending on which date data were acquired. Results indicated a significant difference between models based on distinct data acquisition dates.

A presença cada vez mais disseminada de organismos exóticos (muitos dos quais se tornam invasores) nas diferentes regiões do planeta levou ao surgimento de uma linha de pesquisa na ecologia voltada às invasões biológicas. E para permitir a comunicação entre autores também foi desenvolvido um arcabouço terminológico. Mas, apesar disso, a terminologia relativa às bioinvasões tem sido ignorada por boa parte dos botânicos no Brasil. Há uma boa dose de confusão entre botânicos sobre o que seja uma espécie exótica, naturalizada, invasora, daninha e ruderal, levando ao uso inconsistente da terminologia. Além disso, diferentes autores têm adotado posturas praticamente opostas ao lidar com espécies exóticas em suas áreas de estudo, seja na preparação de tratamentos taxonômicos, seja na publicação de levantamentos florísticos e fitossociológicos. Enquanto alguns pesquisadores incluem em floras mesmo espécies cultivadas que não se reproduzem, outros excluem plantas invasoras comuns e conspícuas. Nós apresentamos aqui, em português, os principais conceitos relativos ao tema da bioinvasão e chamamos a atenção dos autores brasileiros para a necessidade de utilizar de modo consistente o arcabouço terminológico já existente na literatura. Também propomos a adoção de rótulos claros para informar quais espécies são exóticas na área estudada, diferenciando-as das nativas, e sugerimos critérios para ajudar botânicos a decidirem quando uma planta exótica deve ou não ser incluída em tratamentos taxonômicos ou levantamentos de florística.

The National Institute for Clinical Excellence (NICE) guidelines recommend the use of bare-metal stents (BMS) in non-complex lesions with a low risk of restenosis (diameter a parts per thousand yen3 mm and lesion length a parts per thousand currency sign15 mm) and the use of drug-eluting stents (DES) in more complex lesions with a high risk of restenosis (diameter < 3.0 mm or lesion length > 15 mm). However, the guidelines were created based on studies evaluating BMS and DES only. We performed an analysis of patients undergoing non-urgent percutaneous coronary intervention with the novel endothelial cell capturing stent (ECS). The ECS is coated with CD34(+) antibodies that attract circulating endothelial progenitor cells to the stent surface, thereby accelerating the endothelialization of the stented area. We analyzed all patients enrolled in the worldwide e-HEALING registry that met the NICE criteria for either low-risk or high-risk lesions and were treated with a parts per thousand yen1 ECS. The main study outcome was target vessel failure (TVF) at 12-month follow-up, defined as the composite of cardiac death or MI and target vessel revascularization (TVR). A total of 4,241 patients were assessed in the current analysis. At 12-month follow-up, TVF occurred in 7.0% of the patients with low-risk lesions and in 8.8% of the patients with high-risk lesions (p = 0.045). When evaluating the diabetic patients versus the non-diabetic patients per risk group, no significant differences were found in TVF, MI or TVR in either risk group. The ECS shows good clinical outcomes in lesions carrying either a high or a low risk of restenosis according to the NICE guidelines with comparable rates of cardiac death, myocardial infarction, and stent thrombosis. The TVF rate with ECS was slightly higher in patients with high-risk lesions, driven by higher clinically driven TLR. The risk of restenosis with ECS in patients carrying high-risk lesions needs to be carefully considered relative to other risks associated with DES. Furthermore, the presence of diabetes mellitus did not influence the incidence of TVF in either risk group.

Structural and electronic properties of the PtnTM55-n (TM = Co, Rh, Au) nanoalloys are investigated using density functional theory within the generalized gradient approximation and employing the all-electron projected augmented wave method. For TM = Co and Rh, the excess energy, which measures the relative energy stability of the nanoalloys, is negative for all Pt compositions. We found that the excess energy has similar values for a wide range of Pt compositions, i.e., n = 20-42 and n = 28-42 for Co and Rh, respectively, with the core shell icosahedron-like configuration (n = 42) being slightly more stable for both Co and Rh systems because of the larger release of the strain energy due to the smaller atomic size of the Co and Rh atoms. For TM = Au, the excess energy is positive for all compositions, except for n = 13, which is energetically favorable due to the formation of the core-shell structure (Pt in the core and Au atoms at the surface). Thus, our calculations confirm that the formation of core-shell structures plays an important role to increase the stability of nanoalloys. The center of gravity of the occupied d-states changes almost linearly as a function of the Pt composition, and hence, based on the d-band model, the magnitude of the adsorption energy of an adsorbate can be tuned by changing the Pt composition. The magnetic moments of PtnCo55-n decrease almost linearly as a function of the Pt composition; however, the same does not hold for PtRh and PtAu. We found an enhancement of the magnetic moments of PtRh by a few times by increasing Pt composition, which we explain by the compression effects induced by the large size of the Pt atoms compared with the Rh atoms.

Health safety during trips is based on previous counseling, vaccination and prevention of infections, previous diseases or specific problems related to the destination. Our aim was to assess two aspects, incidence of health problems related to travel and the traveler’s awareness of health safety. To this end we phone-interviewed faculty members of a large public University, randomly selected from humanities, engineering and health schools. Out of 520 attempts, we were able to contact 67 (12.9%) and 46 (68.6%) agreed to participate in the study. There was a large male proportion (37/44, 84.1%), mature adults mostly in their forties and fifties (32/44, 72.7%), all of them with higher education, as you would expect of faculty members. Most described themselves as being sedentary or as taking occasional exercise, with only 15.9% (7/44) taking regular exercise. Preexisting diseases were reported by 15 travelers. Most trips lasted usually one week or less. Duration of the travel was related to the destination, with (12h) or longer trips being taken by 68.2% (30/44) of travelers, and the others taking shorter (3h) domestic trips. Most travelling was made by air (41/44) and only 31.8% (14/44) of the trips were motivated by leisure. Field research trips were not reported. Specific health counseling previous to travel was reported only by two (4.5%). Twenty seven of them (61.4%) reported updated immunization, but 11/30 reported unchecked immunizations. 30% (9/30) reported travel without any health insurance coverage. As a whole group, 6 (13.6%) travelers reported at least one health problem attributed to the trip. All of them were males travelling abroad. Five presented respiratory infections, such as influenza and common cold, one neurological, one orthopedic, one social and one hypertension. There were no gender differences regarding age groups, destination, type of transport, previous health counseling, leisure travel motivation or pre-existing diseases. Interestingly, the two cases of previous health counseling were made by domestic travelers. Our data clearly shows that despite a significant number of travel related health problems, these highly educated faculty members, had a low awareness of those risks, and a significant number of travels are made without prior counseling or health insurance. A counseling program conducted by a tourism and health professional must be implemented for faculty members in order to increase the awareness of travel related health problems.

O presente artigo discute aspectos que interferem na garantia do direito à saúde e sua relação com o princípio da integralidade. Para tanto, são descritas questões históricas e culturais que contextualizam esta realidade. Também são apontados os limites e as possibilidades para garantia da integralidade como parte do direito à saúde, com foco no SUS. A abrangência do conteúdo do direito à saúde e a limitação daquilo que é efetivamente alcançado demonstram a dificuldade para sua garantia plena, aspecto igualmente observado em outros países. O direito compartimentado vivenciado na prática do SUS é antagônico ao princípio da integralidade em seus diferentes sentidos. Apesar dos desafios para efetivação da integralidade, diversas iniciativas foram identificadas e têm sido utilizadas na perspectiva de superar as dificuldades e alcançar o direito à saúde em sua plenitude.

A produção de soja no Brasil alcançou destaque na última década. Atualmente, o país configura-se como maior exportador e segundo maior produtor de soja no mundo. Porém, a expansão desse cultivo está sendo associada diretamente ao desmatamento da Floresta Amazônica. Sua cultura iniciou-se no sul do país e avançou para a região central, sobre o bioma do cerrado, expandindo-se, gradativamente, ao norte do Brasil, principalmente por meio de latifúndios monocultores e controlados por grandes empresas transnacionais. A área de avanço agrícola ao norte tem substituído o bioma local: a Floresta Amazônica, o que tem preocupado entidades públicas e privadas, em face do desmatamento exagerado e perda da biodiversidade. Este estudo analisa o avanço da cultura da soja no norte do estado de Mato Grosso, no período entre 1984 e 2009, por meio da interpretação de imagens de satélite, e sua relação com o desmatamento da Floresta Amazônica. Como resultado, apresentam-se três mapas de uso do solo da área de estudo. A análise dos mapas permitiu verificar que o desmatamento recente da floresta está sendo promovido pela atividade pecuária, e os solos descampados e erodidos do pasto têm sido usados para a cultura de soja. Ou seja, na área analisada, a soja estabelece-se em áreas antes degradadas pelo gado, e não diretamente sobre as áreas de floresta desmatada.

INTRODUÇÃO: Os estudos com stents farmacológicos têm avaliado predominantemente populações masculinas de descendência europeia. O estudo de braço único SPIRIT Women avalia o stent eluidor de everolimus XIENCE TM V em lesões de novo complexas em uma população feminina do mundo real, incluindo pacientes latino-americanas. Esta análise permite compreender como essa população responde ao implante de stent, comparativamente a pacientes não-latino-americanas. MÉTODOS: Das 1.572 pacientes matriculadas em 73 locais fora dos Estados Unidos, 138 (9%) foram recrutadas na Argentina, no Brasil e na Venezuela. RESULTADOS: As lesões-alvo tinham diâmetro de referência do vaso entre 2,25 mm e 4 mm e extensão da lesão ≤ 28 mm. As características basais foram semelhantes entre os grupos, com exceção de maior prevalência de hipertensão arterial, infarto do miocárdio (IM) de parede anterior e história familiar de doença arterial coronária na coorte latino-americana. As lesões tendiam a ser mais complexas em mulheres latino-americanas, com menor diâmetro de referência do vaso-alvo, maior extensão da lesão, maior excentricidade e angulação e mais lesões tipo B2/C. Os eventos foram adjudicados de acordo com as definições do Academic Research Consortium. Em um ano, o desfecho combinado de morte por todas as causas, IM e revascularização do vaso-alvo (RVA) foi de 12,1% na população não-latino-americana e de 10,1% na população latino-americana (P = 0,58). CONCLUSÕES: Em um ano, os baixos índices de eventos cardíacos adversos, incluindo trombose do stent, falha da lesão-alvo, morte cardíaca, IM e RVA nas mulheres latino-americanas foram comparáveis aos das mulheres não-latino-americanas, apesar da maior complexidade das lesões. Esses resultados demonstram a segurança e a eficácia do stent XIENCE TM V nessa pequena coorte de pacientes latino-americanas, à semelhança do que é observado com populações maiores e mais variadas.

Die Entdeckung einer Transmembranform des Proteoglykans Agrin führte zu der Hypothese, dass TM-Agrin ein Rezeptor oder Korezeptor mit signaltransduzierenden Eigenschaften sein könnte. Die durch Antikörper simulierte Ligandenbindung an TM-Agrin resultiert in der Bildung von zahlreichen Ausläufern sowohl auf ZNS als auch auf PNS Axonen und Dendriten und unterstützt diese Hypothese. Übereinstimmend mit diesen Ergebnissen führt die Überexpression von TM-Agrin in ZNS Neuronen ebenfalls zur Bildung von zahlreichen Ausläufern, wohingegen die Unterdrückung der TM-Agrin Expression durch siRNA die Anzahl der Ausläufer stark reduziert. Der Mechanismus der durch TM-Agrin induzierten Bildung von Ausläufern ist jedoch unbekannt. Ziel der Dissertation war es deshalb, die durch TM-Agrin induzierte Signalkaskade zu untersuchen. Mit Hilfe von biochemischen, immunozytochemischen und pharmakologischen Unter-suchungen konnte gezeigt werden, dass sich TM-Agrin auf auswachsenden Axonen und nach Überexpression in nicht-neuronalen Zellen in lipid rafts anreichert und das lipid rafts sowohl für die Bildung als auch für die Aufrechterhaltung der Ausläufer notwendig sind. Die durch polyklonale Antikörper induzierte Bildung von Ausläufern kann sowohl durch die Inhibierung der Src-Kinase Fyn, welche in lipid rafts lokalisiert ist, als auch der 44/42 MAPK gehemmt werden. Zudem führt die Aggregation von TM-Agrin durch polyklonale Antikörper zu einer erhöhten Aktivität der Fyn- und 44/42 MAP-Kinase. Im Gegensatz dazu hat eine Inhibierung der PI3-Kinase keine Auswirkungen auf die Bildung der Ausläufer. Der intrazelluläre, sekundäre Botenstoff cGMP, nicht aber cAMP, fördert die Bildung von Ausläufern. Die konzentrationsabhängige Blockierung des Coxsackie-Adenovirus-Rezeptors (CAR) führt zu einer Hemmung der Ausläuferbildung. Ferner interagiert CAR im Zentralnervensystem während der Phase des aktiven axonalen Wachstums mit TM-Agrin und induziert nach Überexpression in neuronalen und nicht-neuronalen Zellen die Bildung von Ausläufern. In der vorliegenden Arbeit konnten somit Teilabschnitte der durch TM-Agrin induzierten Signalkaskade entschlüsselt werden.

Non-invasive molecular-imaging technologies are playing a key role in drug discovery, development and delivery. Positron Emission Tomography (PET) is such a molecular imaging technology and a powerful tool for the observation of various deceases in vivo. However, it is limited by the availability of vectors with high selectivity to the target and radionuclides with a physical half-life which matches the biological half-life of the observed process. The 68Ge/68Ga radionuclide generator makes the PET-nuclide anywhere available without an on-site cyclotron. Besides the perfect availability 68Ga shows well suited nuclide properties for PET, but it has to be co-ordinated by a chelator to introduce it in a radiopharmaceuticals.rnHowever, the physical half-life of 68Ga (67.7 min) might limit the spectrum of clinical applications of 68Ga-labelled radiodiagnostics. Furthermore, 68Ga-labelled analogues of endoradiotherapeuticals of longer biological half-live such as 90Y- or 177Lu-labeled peptides and proteins cannot be used to determine individual radiation dosimetry directly. rnThus, radionuclide generator systems providing positron emitting daughters of extended physical half-life are of renewed interest. In this context, generator-derived positron emitters with longer physical half-life are needed, such as 72As (T½ = 26 h) from the 72Se/72As generator, or 44Sc (T½ = 3.97 h) from the 44Ti/44Sc generator.rnIn this thesis the implementation of radioactive gallium-68 and scandium-44 for molecular imaging and nuclear medical diagnosis, beginning with chemical separation and purification of 44Ti as a radionuclide mother, investigation of pilot generators with different elution mode, building a prototype generator, development and investigation of post-processing of the generator eluate, its concentration and further purification, the labeling chemistry under different conditions, in vitro and in vivo studies of labeled compounds and, finally, in vivo imaging experiments are described.
