OBJECTIVE: To identify factors that lead people to visit a doctor in Brazil and assess differences between socioeconomic groups. METHODS: A cross-sectional study comprising 1,260 subjects aged 15 or more was carried out in southern Brazil. Demographic, socioeconomic, health needs and regular source of care data were analyzed concerning visits to a doctor within two months from the interview. Adjusted prevalence ratios and 95% confidence intervals were calculated using Poisson regression. RESULTS: Adjusted PR showed that women having stressful life events, health insurance, and a regular doctor increased the outcome. A dose-related response was found with self-reported health, and the probability of visiting a doctor increased with health needs. Analysis in the chronic disease group revealed that uneducated lower income subjects had a 62% reduction in the chance of visiting a doctor compared to uneducated higher income ones. However, as it was seen a significant interaction between income and education, years of schooling increased utilization in this group. CONCLUSIONS: Results suggest the existence of health inequity in the poorest group that could be overcome with education. Specific measures reinforcing the importance of having a regular doctor may also improve access in the underserved group.

Neste trabalho é apresentado um Sistema de Informação (SI) cujo objetivo é contribuir para a colaboração entre VE possibilitando a sua entrada no mercado de energia elétrica, permiitindo deste modo que o proprietário do VE seja compensado. Esta compensação incidiria em medidas de minimização do investimento inicial na compra do VE e na criação de um modelo de armazenamento de energia mais sustentável. A este SI chamou-se de Broker Colaborativo ou simplesmente Broker e deverá permitir ao proprietário do VE criar um perfil com a sua informação pessoal, com informação do VE que possui e com informação relativa à colaboração que pretende efetuar, ou seja, locais e percentagem da bateria que pretende usar na colaboração. Para assegurar a entrada no mercado de energia torna-se necessário agrupar e filtrar um número significativo de VE, sendo essa responsabilidade atribuída ao Broker. Sendo a otimização do consumo de energia um fator importante neste trabalho, a implementação do Broker foi efetuada sobre o paradigma de Cloud Computing (será utilizada a designação abreviada de Cloud) onde os recursos energéticos são partilhados. A Clod possuiu uma grande capacidade de escalonamento onde se poderão criar os recursos à medida da necessidade. A utilização da Cloud permite que as especificações iniciais das máquinas virtuais, necessárias para alojar a base de dados ou os ficheiros do Broker, possam ser configuradas conforme o número de utilizadores do sistema. Assim, não se desperdiçam recursos que não seriam utilizados. Devido a estes aspetos dinâmicos, que estão intrínsecos ao fator humano, o Broker foi implementado utilizando um módulo de integração com a rede social Facebook esperando-se desta forma, que a colaboração entre diferentes VE seja difundida através dos amigos e conhecidos de cada utilizador da colaboração. Sendo assim, este trabalho pretende criar um sistema de informação onde os proprietários de VE possam juntar-se e criar uma colaboração possibilitando a sua entrada no mercado da energia elétrica. Essa colaboração será alargada recorrendo a uma rede social e compensada através de créditos. É abordado o tema das plataformas de Cloud como uma plataforma sustentável e vantajosa para o desenvolvimento de sistemas com grande potencial de crescimento.

A presente dissertação pretende analisar a existência de associação entre as entidades detentoras de planos de pensões e os elementos que caracterizam os planos subscritos por tais entidades, sendo objecto deste estudo os planos de pensões de benefícios definidos. A partir de uma abordagem baseada na análise de conteúdo, e tendo como suporte documental as Demonstrações Financeiras inseridas no Relatório e Contas Consolidadas, a amostra deste estudo integra as entidades com valores mobiliários admitidos à negociação na Euronext Lisboa incluídas no índice PSI-20, durante o período compreendido entre 2005 e 2009. Da revisão de literatura efectuada, foram definidas hipóteses de associação significativa entre determinados factores que caracterizam as referidas entidades, nomeadamente, os elementos do Balanço e da Demonstração dos Resultados relativos à dimensão, ao endividamento e à rendibilidade, e os elementos caracterizadores dos planos de pensões subscritos por tais entidades, nomeadamente, o activo total do plano, a variação anual de tais activos e o défice do plano. Os dados recolhidos foram posteriormente submetidos a técnicas de análise univariada e bivariada, através de medidas estatísticas descritivas e do coeficiente de correlação de Spearman, respectivamente. Os resultados encontrados sugerem a existência de uma associação significativa entre o factor relativo à dimensão e os elementos caracterizadores dos planos de pensões seleccionados para este estudo. A referida associação, no entanto, não se verifica no que diz respeito aos factores relativos ao endividamento e à rendibilidade das entidades analisadas nesta dissertação.

Power systems operation in a liberalized environment requires that market players have access to adequate decision support tool, allowing them to consider all the business opportunities and take strategic decisions. Ancillary services represent a good negotiation opportunity that must be considered by market players. For this, decision support tools must include ancillary market simulation. This paper deals with ancillary services negotiation in electricity markets. The proposed concepts and methodologies are implemented in MASCEM, a multi-agent based electricity market simulator. A test case concerning the dispatch of ancillary services using two different methods (Linear Programming and Genetic Algorithm approaches) is included in the paper.

Distributed generation unlike centralized electrical generation aims to generate electrical energy on small scale as near as possible to load centers, interchanging electric power with the network. This work presents a probabilistic methodology conceived to assist the electric system planning engineers in the selection of the distributed generation location, taking into account the hourly load changes or the daily load cycle. The hourly load centers, for each of the different hourly load scenarios, are calculated deterministically. These location points, properly weighted according to their load magnitude, are used to calculate the best fit probability distribution. This distribution is used to determine the maximum likelihood perimeter of the area where each source distributed generation point should preferably be located by the planning engineers. This takes into account, for example, the availability and the cost of the land lots, which are factors of special relevance in urban areas, as well as several obstacles important for the final selection of the candidates of the distributed generation points. The proposed methodology has been applied to a real case, assuming three different bivariate probability distributions: the Gaussian distribution, a bivariate version of Freund’s exponential distribution and the Weibull probability distribution. The methodology algorithm has been programmed in MATLAB. Results are presented and discussed for the application of the methodology to a realistic case and demonstrate the ability of the proposed methodology for efficiently handling the determination of the best location of the distributed generation and their corresponding distribution networks.

OBJECTIVE: To assess the effects of individual, household and healthcare system factors on poor children's use of vaccination after the reform of the Colombian health system. METHODS: A household survey was carried out in a random sample of insured poor population in Bogota, in 1999. The conceptual and analytical framework was based on the Andersen's Behavioral Model of Health Services Utilization. It considers two units of analysis for studying vaccination use and its determinants: the insured poor population, including the children and their families characteristics; and the health care system. Statistical analysis were carried out by chi-square test with 95% confidence intervals, multivariate regression models and Cronbach's alpha coefficient. RESULTS: The logistic regression analysis showed that vaccination use was related not only to population characteristics such as family size (OR=4.3), living area (OR=1.7), child's age (OR=0.7) and head-of-household's years of schooling (OR=0.5), but also strongly related to health care system features, such as having a regular health provider (OR=6.0) and information on providers' schedules and requirements for obtaining care services (OR=2.1). CONCLUSIONS: The low vaccination use and the relevant relationships to health care delivery systems characteristics show that there are barriers in the healthcare system, which should be assessed and eliminated. Non-availability of regular healthcare and deficient information to the population are factors that can limit service utilization.

OBJECTIVE: To analyze the prevalence of physiotherapy utilization and to explore the variables associated to its utilization. METHODS: A population-based cross-sectional study, including 3,100 subjects aged 20 years or more living in the urban area of Pelotas, southern Brazil, was carried out. The sample was selected following a multiple-stage protocol; the census tracts delimited by the Instituto Brasileiro de Geografia e Estatística (Brazilian Institute of Geography and Statistics) were the primary sample units. Following descriptive and crude analyses, Poisson regression models taking the clustering of the sample into account were carried out. Data were collected through face-to-face interviews using a standardized and pre-tested questionnaire. RESULTS: The lifetime utilization of physiotherapy was 30.2%; and physiotherapy utilization in the 12 months prior to the interview was reported by 4.9%. Women, elderly subjects, and those from higher socioeconomic levels were more likely to use physiotherapy. Restricting analysis to subjects who attended physiotherapy, 66% used public health services, 25% used insurance health services and 9% had private sessions. CONCLUSIONS: This is the first population-based study on physiotherapy utilization carried out in Brazil. Utilization of physio therapy was lower than reported in both developed and developing countries. The study findings might help public health authorities to organize healthcare service in terms of this important demand.
