The use of medicinal plants to cure and treat various diseases is a common practice in the world and in Brazil. In several regions of the Brazil´s Northeast, the cactus Cereus jamacaru, known as mandacaru, is used popularly as a treatment to many diseases, including those related to heart respiratory diseases, gastric ulcers, scurvy, and kidney diseases. However, there is a scarcity in the scientific literature that proves scientifically the popular application of this cactus. Like other plants, Cereus jamacaru synthesizes several potentially bioactive molecules, like as polysaccharides. In this work, three polysaccharides-rich aqueous extracts, MCA80, MPM and MCP60, were obtained from this plant and analyzed chemically, as well as their cytotoxic and antioxidant potential. The data showed that all extracts consist mainly of polysaccharides (89.42 to 95.76%), but also protein (> 2%) and phenolic (3 to 8.87%) contaminants were detected. All extracts are rich in galactose, glucose and mannose. In addition, glucuronic acid was found in MCA80 and MCP60. The extracts showed total antioxidant capacity ranged from 55.21 to 68.13 of ascorbic acid equivalents (AAE). Besides, they exhibited reducer power and cupric chelation in a dose-dependent manner. None of the extracts inhibited the MTT reduction in the presence of prostate tumor cells (PC-3). However, MCP60 was the most effective extract by preventing the reduction of MTT by about 80% in the presence of cells 786. Nuclear fragmentation tests showed that this extract induces cell death. The data indicated that mandacaru synthesizes bioactive polysaccharides with potential as antioxidant and antitumor agents. For future studies, it is intended to purify and characterize these polysaccharides and its antioxidant and antitumor mechanisms

Background: There is a high prevalence of gastro-duodenal disease in sub-Saharan Africa. Peptic ulcer disease in dyspeptic patients, 24.5%, was comparable to prevalence of gastro-duodenal disease among symptomatic individuals in developed countries (12 – 25%). Limited data exists regarding its associated risk factors despite accumulating evidence indicating that gastroduodenal disease is common in Ghana. Objectives: This study investigates risk factors associated with gastro-duodenal disease at the Korle-Bu Teaching Hospital, Accra, Ghana. Methods: This study utilized a cross-sectional design to consecutively recruit patients referred with upper gastro-intestinal symptoms for endoscopy. The study questionnaire was administered to study participants. Helicobacter pylori infection was confirmed by rapid-urease examination at endoscopy. Results: Of 242 patients sampled; 64 had duodenal ulcer, 66 gastric ulcer, 27gastric cancer and 64 non-ulcer dyspepsia. Nineteen (19) had duodenal and gastric ulcer while 2 had gastric ulcer and cancer. A third (32.6%) of patients had history of NSAIDuse. H. pylori was associated with gastric ulcer (p=0.033) and duodenal ulcer (p=0.001). There was an increased prevalence of duodenal ulcer in H. pylori-infected patients taking NSAIDs, P=0.003. Conclusion: H. pylori was a major risk factor for peptic ulcer disease. However, NSAID-related gastro-duodenal injury has been shown to be common in H. pylori infected patients. It highlights the need for awareness of the adverse gastro-intestinal effects in a H. pylori endemic area.

Behçet's disease (BD) is a multi-systemic vascular disorder characterized by oral and genital ulcers, cutaneous, ocular, arthritic, vascular, central nervous system and gastrointestinal involvement. It usually affects young adults and the etiopathogenesis is unknown. A 21-year-old girl, Caucasian, with diagnostic BD, presented with rheumatoid arthritis, genital lesions and multiple recurrent ulcers inside the mouth, with an erythematous halo, covered by yellowish exudates exacerbated during menstrual periods, and in situations of stress and anxiety. The application of low power laser in ulcers was considered in order to decrease the inflammatory symptoms and pain, beyond getting the healing process accelerated. The proposed therapy was able to promote pain relief, increase local microcirculation and repair ulcerated lesions, eliminating the need for administration of systemic or topical medications, leading to improved quality of life.

Behçet's disease (BD) is a multi-systemic vascular disorder characterized by oral and genital ulcers, as well as cutaneous, ocular, arthritic, vascular, central nervous system and gastrointestinal involvement. It usually affects young adults, and its pathological origin is unknown. The case of a 47-year-old woman with recurrent ulcers in the oral cavity is presented. She linked the pain with sitting and during the sexual act, with vaginal and oral cavity pain, due to the lesions present at those sites, as well as swelling and pain in the knees, making walk painful. The patient was kept under observation and underwent multidisciplinary treatment with prescription of topical and systemic drugs to improve quality of life. Dentists should be aware of BD and the need of multidisciplinary treatment to increase the patient's quality of life.

Introduction - Learning about ageing and the appropriate management of older patients is important for all doctors. This survey set out to evaluate what medical undergraduates in the UK are taught about ageing and geriatric medicine and how this teaching is delivered. Methods – An electronic questionnaire was developed and sent to the 28/31 UK medical schools which agreed to participate. Results – Full responses were received from 17 schools. 8/21 learning objectives were recorded as taught, and none were examined, across every school surveyed. Elder abuse and terminology and classification of health were taught in only 8/17 and 2/17 schools respectively. Pressure ulcers were taught about in 14/17 schools but taught formally in only 7 of these and examined in only 9. With regard to bio- and socio- gerontology, only 9/17 schools reported teaching in social ageing, 7/17 in cellular ageing and 9/17 in the physiology of ageing. Discussion – Even allowing for the suboptimal response rate, this study presents significant cause for concern with UK undergraduate education related to ageing. The failure to teach comprehensively on elder abuse and pressure sores, in particular, may be significantly to the detriment of older patients.

Localised cutaneous leishmaniasis (LCL) is the most common form of cutaneous leishmaniasis characterised by single or multiple painless chronic ulcers, which commonly presents with secondary bacterial infection. Previous culture- based studies have found staphylococci, streptococci, and opportunistic pathogenic bacteria in LCL lesions, but there have been no comparisons to normal skin. In addition, this approach has strong bias for determining bacterial composition. The present study tested the hypothesis that bacterial communities in LCL lesions differ from those found on healthy skin (HS). Using a high throughput amplicon sequencing approach, which allows for better populational evaluation due to greater depth coverage and the Quantitative Insights Into Microbial Ecology pipeline, we compared the microbiological signature of LCL lesions with that of contralateral HS from the same individuals. Streptococcus , Staphylococcus , Fusobacterium and other strict or facultative anaerobic bacteria composed the LCL microbiome. Aerobic and facultative anaerobic bacteria found in HS, including environmental bacteria, were significantly decreased in LCL lesions (p < 0.01). This paper presents the first comprehensive microbiome identification from LCL lesions with next generation sequence methodology and shows a marked reduction of bacterial diversity in the lesions.

Para garantir a qualidade nos cuidados de saúde é necessário conhecer as principais componentes do conceito de qualidade, elaborar um programa de garantia da qualidade, avaliar de uma forma sistemática a execução do programa e definir o modelo conceptual a aplicar. A prevenção das úlceras de pressão é uma preocupação dos profissionais de saúde que prestam cuidados aos idosos dependentes de cuidadores informais, sendo a sua prevenção um desafio para a equipa de enfermagem, uma vez que a incidência de úlcera de pressão é frequente nestes. A aposta na prevenção e tratamento da UP terá um efeito positivo na qualidade de cuidados prestados. Tendo em conta esta problemática, foi feito o diagnóstico de situação com base na observação dos registos de enfermagem, bem como os procedimentos inerentes à prevenção e tratamento de úlceras de pressão, com a finalidade de contribuir para a implementação de um programa de melhoria contínua da qualidade dos cuidados a idosos dependentes de cuidadores informais com risco de úlceras de pressão. Após esta fase definiram-se algumas estratégias que consideramos pertinentes implementar. Com as atividades desenvolvidas neste trabalho, esperamos melhorar a informação produzida conseguindo obter dados que permitem melhorar os cuidados de enfermagem aos idosos/família com risco de UP, assim como, contribuir para a identificação de problemas e definições de estratégias de melhoria no futuro.

O cuidador informal é aquela pessoa sobre quem recai a responsabilidade pela prestação da maioria dos cuidados ligados ao doente dependente, sem que para isso tenha sido preparado. Cabe-nos a nós técnicos de saúde orientar, no sentido de apoio e ajuda a estes cuidadores informais que têm a seu cargo um doente dependente, de modo a desenvolver as competências que lhes permita superar as mudanças e consequentemente favorecer um processo de adaptação mais rápido e consciente, melhorando a sua Qualidade de Vida e a do doente. Através da implementação de intervenções específicas, Educação para a Saúde. Neste sentido, o presente trabalho passou pela criação de um Guia de Boas Práticas e pela elaboração de um Plano de Formação com o objectivo de estabelecer linhas de orientação para a educação para a saúde e normalizar procedimentos que garantam as boas práticas na prevenção de Úlceras de Pressão em particular

Introducción: Según la OMS, más del 50% de la población adulta está infectada con el Helicobacter Pylori, con prevalencias de hasta el 90%. La mayoría de contagios se produce antes de los 10 años de edad. Desde el descubrimiento del H. Pylori (1983), se lo ha relacionado con la úlcera péptica, gastritis, reflujo gastroesofágico e incluso, cáncer gástrico. Metodología: Se realizó un estudio transversal analítico. La muestra estuvo conformada por 250 niños escolares de la etnia Shuar del cantón Sucúa. La prueba utilizada para la detección del Helicobacter pylori es la identificación de antígenos en las heces por inmunocromatografía. Para establecer la significancia de asociación de variables se utilizó el OR con su intervalo de confianza al 95%. Resultados: El promedio de edad fue de 8.8 años (DS 2.0), con predominio de las mujeres (54.4%). El 56.4% consume agua potable, el 71.65% vive en hacinamiento, el 42% tiene servicios de letrinización y el 49.2% cuenta con servicios de alcantarillado. Se encontró asociación significativa con el nivel de instrucción OR 1.68, IC95% (1 – 2.84), p=0.049; letrinización OR 1.99, IC95% (1.17 – 3.36), p=0.01; deposiciones al aire libre OR 4.32, IC95% (2.13 – 8.77), p=0.000. Conclusiones: La prevalencia de Helicobacter pylori es alta en la población escolar de la etnia Shuar; está asociada a un nivel bajo de instrucción de los progenitores y una inadecuada infraestructura de servicios básicos.

Background: Initial resistance to antibiotics is the main reason for the failure of Helicobacter pylori (H. pylori) eradication in children. Objectives: As we commonly face high antibiotic resistance rates in children, we aimed to determine the susceptibility of H. pylori to common antibiotics. Patients and Methods: In this cross-sectional in vitro study, 169 children younger than 14 years with clinical diagnosis of peptic ulcer underwent upper gastrointestinal endoscopy. Biopsy specimens from stomach and duodenum were cultured. In isolated colonies, tests of catalase, urease, and oxidase as well as gram staining were performed. After confirming the colonies as H. pylori, the antibiogram was obtained using disk diffusion method. Results: Culture for H. pylori was positive in 12.3% of the specimens, urease test in 21.3%, serological test in 18.9% and stool antigen test was positive in 21.9%. We could show high specificity but moderate sensitivity of both histological and H. pylori stool antigen tests to detect H. pylori. The overall susceptibility to metronidazole was 42.9%, amoxicillin 95.2%, clarithromycin 85.7%, furazolidone 61.9%, azithromycin 81.0%, and tetracycline 76.2% with the highest resistance to metronidazole and the lowest to clarithromycin. Conclusions: In our region, there is high resistance of H. pylori to some antibiotics including metronidazole and furazolidone among affected children. To reduce the prevalence of this antibiotic resistance, more controlled use of antibiotics should be considered in children.
Saccharomyces Boulardii in Helicobacter Pylori Eradication in Children: A Randomized Trial From Iran

Background: Helicobacter Pylori infects around 50% of the human population and is asymptomatic in 70% of the cases. H.pylori eradication in childhood will not only result in peptic symptoms relief, but will also prevent late-term complications such as cancer. Today, probiotics are being increasingly studied in the treatment of gastrointestinal infections as an alternative or complement to antibiotics. Objectives: In this study we aimed to assess the effect of S. boulardii supplementation on H.pylori eradication among children in our region. Patients and Methods: In this randomized double-blind placebo-controlled clinical trial 28 asymptomatic primary school children with a positive H.pylori stool antigen (HpSA) exam were randomly allocated into the study group, receiving Saccharomyces Boulardii and the control group receiving placebo capsules matched by shape and size, for one month. The children were followed up weekly and were reinvestigated four to eight weeks after accomplished treatment by HpSA testing. The significance level was set at P < 0.05. Results: 24 children completed the study. The mean HpSA reduced from 0.40 ± 0.32 to 0.21 ± 0.27 in the study group, indicating a significant difference (P = 0.005). However, such difference was not observed in the control group (P = 0.89). Moreover, the HpSA titer showed a 0.019 ± 0.19 decrease in the study group whereas the same value was 0.0048 ± 0.12 for the controls, again stating a significant difference (P = 0.01). Conclusions: Saccharomyces boulardii has a positive effect on reducing the colonization of H.pylori in the human gastrointestinal system but is not capable of its eradication when used as single therapy.

Objetivo general: Establecer la prevalencia de las deformidades óseas y estructurales en pacientes con pie diabético que acudieron al área de consulta externa de Traumatología del Hospital Regional Vicente Corral Moscoso durante el período de tres meses. Metodología: Estudio descriptivo llevado a cabo en 100 pacientes con pie diabético que acudieron a la consulta externa de Traumatología durante tres meses. El método de investigación fue la observación directa; las técnicas comprendieron entrevista, evaluación mediante plantoscopía y valoración radiográfica, los instrumentos usados fueron los formularios, los datos tras su validación fueron ingresados en una base en el programa SPSS V15; mediante el cual se realizó el análisis usando tablas simples con porcentajes y frecuencias relativas. Resultados: La media de edad fue de 64,09 años con el 84% de la población de sexo femenino; el cuadro clínico se caracterizó por presencia de callosidades 62%; metatarsalgia 57%; talalgia 48% y dolor en bunion de 21%. Tomando en consideración los parámetros radiográficos se diagnosticó hallux valgus 71%; deformidades de los dedos en garra 56%; pie plano 34%; dedos en martillo 31%; juanetillo de sastre 30%; hallux rigidus 16% y pie cavo 13%. La valoración mediante el uso de plantoscopía reveló 39% de pacientes pie plano y el 17% pie cavo
Faktorer som främjar den hälsorelaterade livskvaliteten hos patienter med inflammatorisk tarmsjukdom

Introduktion: Inflammatorisk tarmsjukdom blir allt vanligare i Sverige och skapar stort lidande hos de som drabbas. Syfte: Syftet med litteraturstudien var att belysa faktorer som främjar den hälsorelaterade livskvaliteten hos patienter med inflammatorisk tarmsjukdom mätt med mätinstrumentet IBDQ. Metod: Detta är en litteraturstudie som genomförts på ett systematiskt sätt baserat på Polit och Becks nio-stegsmodell. Resultaten från de granskade artiklarna kategoriserades genom en induktiv process. Resultat: Litteraturstudiens resultat består av tre kategorier som på olika vis genererar en främjad hälsorelaterad livskvalitet. Den första kategorin innehåller faktorer som patienten själv kan påverka så som motion och olika avslappningsövningar. Den andra kategorin speglar patientens inställningar och uppfattningar och den tredje kategorin innehåller faktorer som kräver sjukvårdsinsatser så som läkemedelsbehandling eller kirurgi. Slutsats: Det finns en rad olika faktorer som leder till främjan av den hälsorelaterade livskvaliteten. Kännedom om dessa faktorer är en förutsättning för att sjuksköterskan skall kunna verka för en god hälsa och främja den hälsorelaterade livskvaliteten. Denna litteraturstudie lyfter fram att IBD drabbar hela individen och dennes fysiska- sociala- och emotionella dimensioner där av vikten att se till hela individen och tillgodose individens alla individuella och specifika behov.

Las úlceras por presión causadas por una presión sostenida sobre la piel y sus tejidos adyacentes es una problemática considerada de salud pública, dada su alta prevalencia, se presentan mayormente en personas que tienen restricción en su movilidad, en el 3 a 10 % de los pacientes hospitalizados; con una tasa de incidencia que oscila entre 7,7 y 26,9 %, con consecuencias para la institución por altos costos, la comunidad, el paciente y su familia. En Colombia solo se cuenta con algunos estudios en instituciones en Bogotá, Cartagena, Bucaramanga y Medellín. Los estudios de prevalencia son importantes porque permiten visualizar la situación de la problemática, incentiva la implementación de medidas preventivas y la formulación de políticas institucionales que promuevan la seguridad del paciente y la calidad de la atención. En el Hospital Universitario Fundación Santa Fe de Bogotá no se conoce con exactitud la prevalencia pero se sabe que está presente por las complicaciones y consecuencias ya mencionadas, por ende el objetivo de este estudio fue determinar la prevalencia de punto de úlceras por presión en los pacientes hospitalizados, y al finalizar ser insumo para un estudio posterior que genere un Programa de Piel Sana institucional. El estudio se realizó mediante la medición en 1 día donde se valoró todos los pacientes hospitalizados, se realizó un análisis descriptivo de cada variable, y con un análisis bivariado se obtuvo medidas de asociación para identificar la relación entre los factores de riesgo y el desarrollo de ulceras por presión.

Estudios afirman que la acidez gástrica es importante en la absorción de levotiroxina (LT4) con resultados controversiales sobre la interacción entre inhibidores de bomba de protones (IBP) y LT4. El objetivo del estudio fue establecer el efecto del uso concomitante de LT4 e IBP en los niveles de TSH en pacientes adultos con hipotiroidismo primario. Se realizó una revisión sistemática mediante búsqueda en Medline, Embase, Lilacs, Bireme, Scielo, Cochrane y Universidad de York, Access Pharmacy, Google Scholar, Dialnet y Opengray. La búsqueda no se limito por lenguaje. Se evaluó el efecto en la diferencia de medias de TSH luego del consumo de LT4 y luego del consumo concomitante con IBP. Se hizo un metaanálisis, análisis de subgrupos y análisis de sensibilidad utilizando el programa Review Manager 5.3. Se eligieron 5 artículos para el análisis cualitativo y 3 para el metaanálisis. La calidad de los estudios fue buena y el riesgo de sesgos bajo. La diferencia de medias obtenida fue 0.21 mUI/L (IC95%: 0.02-0.40; p=0.03; I2:0%). En el análisis de subgrupos en pacientes mayores de 55 años la diferencia de medias fue 0.21 mUI/L (IC95%: 0.01-0.40; p=0.27; I2:19%). En el análisis de sensibilidad se excluyo el estudio con mayor muestra y la diferencia de medias fue 0.49 mUI/L (IC95%: -0.12 a 1.11; p=0.12; I2:0%). La diferencia de medias de TSH luego del consumo concomitante no se considera clínicamente significativa pues no representa riesgo para el paciente. Son necesarios estudios clínicos aleatorizados y evaluar el efecto en los niveles de T4 libre.
