Opioids are most commonly used for treatment of severe pain. However, the fear of respiratory depression has restricted the use of opioids. Depending on the monitoring system used, different modes of opioid respiratory effects have been noted in previous studies. All opioids also cause alterations in hemodynamics at least to some extent. The main goal of this series of investigations was to elucidate the native ventilatory and hemodynamic effects of different opioids. Studies I-IV each involved 8 healthy male volunteers. Study V involved 13 patients with lower or upper extremity traumas. The opioids studied were morphine, oxycodone, pethidine, fentanyl, alfentanil, tramadol and ketamine. The respiratory parameters used in this study were breathing pattern measured with respiratory inductive plethysmography, gas exchange measured with indirect calorimetry, blood gas analysis and pulse oximetry. Hemodynamics was measured with arterial blood pressure, heart rate and oxygen consumption. Plasma catecholamine and histamine concentrations were also determined. All opioids studied caused an alteration in respiratory function. Respiratory rate, alveolar ventilation and minute ventilation decreased, while tidal volume increased in most situations. Breathing pattern was also significantly affected after opioid administration. The respiratory depression caused by oxycodone was deeper than the one caused by same dose of morphine. An equianalgesic dose of tramadol caused markedly smaller respiratory depression compared to pethidine. The potency ratio for respiratory depression of fentanyl and alfentanil is similar to analgesic potency ratio studied elsewhere. Racemic ketamine attenuated the respiratory depression caused by fentanyl, if measured with minute ventilation. However, this effect was counteracted by increased oxygen consumption. Supplemental oxygen did not offer any benefits, nor did it cause any atelectasis when given to opioid treated trauma patients. Morphine caused a transient hemodynamic stimulation, which was accompanied by an increase in oxygen consumption. Oxycodone, alfentanil, fentanyl, tramadol and pethidine infusions had minimal effects on hemodynamics. Plasma catecholamine concentrations were increased after high dose opioid administration. Plasma histamine concentrations were not elevated after morphine nor oxycodone administration. Respiratory depression is a side effect noted with all opioids. The profile of this phenomenon is quite similar with different opioid-receptor agonists. The hemodynamic effects of opioids may vary depending on the opioid used, morphine causing a slight hemodynamic stimulation. However, all opioids studied could be considered hemodynamically stable.

The respiratory rates of mycelia of the mesophilic fungus, Aspergillus niger, and the thermophilic fungus, Thermomyces lanuginosus, were comparable at their respective temperature optima for growth. The respiratory rate of A. niger was independent of changes in temperature between 15 and 40 C. The respiratory rate of T. lanuginosus increased with increase in temperature between 25 and 55 C.

An attempt has been made to forecast the potential of thermophilic fungi to grow in soil in the laboratory and in the field in the presence of a predominantly mesophilic fungal flora at usual temperature. The respiratory rate of thermophilic fungi was markedly responsive to changes in temperature, but that of mesophilic fungi was relatively independent of such changes. This suggested that in a thermally fluctuating environment, thermophilic fungi may be at a physiological disadvantage compared to mesophilic fungi. In mixed cultures in soil plates, thermophilic fungi outgrew mesophilic fungi under a fluctuating temperature regime only when the amplitude of the fluctuating temperatures was small and approached their temperature optima for growth. An antibody probe was used to detect the activity of native or an introduced strain of a thermophilic fungus, Thermomyces lanuginosus, under field conditions. The results suggest that although widespread, thermophilic fungi are ordinarily not an active component of soil microflora. Their presence in soil most likely may be the result of the aerial dissemination of propagules from composting plant material.

Design and development of a piezoelectric polyvinylidene fluoride (PVDF) thin film based nasal sensor to monitor human respiration pattern (RP) from each nostril simultaneously is presented in this paper. Thin film based PVDF nasal sensor is designed in a cantilever beam configuration. Two cantilevers are mounted on a spectacle frame in such a way that the air flow from each nostril impinges on this sensor causing bending of the cantilever beams. Voltage signal produced due to air flow induced dynamic piezoelectric effect produce a respective RP. A group of 23 healthy awake human subjects are studied. The RP in terms of respiratory rate (RR) and Respiratory air-flow changes/alterations obtained from the developed PVDF nasal sensor are compared with RP obtained from respiratory inductance plethysmograph (RIP) device. The mean RR of the developed nasal sensor (19.65 +/- A 4.1) and the RIP (19.57 +/- A 4.1) are found to be almost same (difference not significant, p > 0.05) with the correlation coefficient 0.96, p < 0.0001. It was observed that any change/alterations in the pattern of RIP is followed by same amount of change/alterations in the pattern of PVDF nasal sensor with k = 0.815 indicating strong agreement between the PVDF nasal sensor and RIP respiratory air-flow pattern. The developed sensor is simple in design, non-invasive, patient friendly and hence shows promising routine clinical usage. The preliminary result shows that this new method can have various applications in respiratory monitoring and diagnosis.

Sepsis is a life threatening condition resulting from a high burden of infection. It is a major health care problem and associated with inflammation, organ dysfunction and significant mortality. However, proper understanding and delineating the changes that occur during this complex condition remains a challenge. A comparative study involving intra-peritoneal injection of BALB/c mice with Salmonella Typhimurium (infection), lipopolysaccharide (endotoxic shock) or thioglycollate (sterile peritonitis) was performed. The changes in organs and sera were profiled using immunological assays and Fourier Transform Infrared (FTIR) micro-spectroscopy. There is a rapid rise in inflammatory cytokines accompanied with lowering of temperature, respiratory rate and glucose amounts in mice injected with S. Typhimurium or lipopolysaccharide. FTIR identifies distinct changes in liver and sera: decrease in glycogen and protein/lipid ratio and increase in DNA and cholesteryl esters. These changes were distinct from the pattern observed in mice treated with thioglycollate and the differences in the data obtained between the three models are discussed. The combination of FTIR spectroscopy and other biomarkers will be valuable in monitoring molecular changes during sepsis. A obesidade é uma doença crônica, resultante do excesso de gordura no organismo. O aumento da obesidade no mundo, tem se revelado como um dos fenômenos clínicos e epidemiológicos da atualidade. Estudos populacionais e em modelos animais demonstram que a origem da epidemia da obesidade está relacionada a fatores genéticos, modificações de hábitos nutricionais, redução da atividade física, e alterações nutricionais durante a lactação, desempenhando um papel relevante no desenvolvimento da obesidade, DM2 e cardiomiopatias. As mitocôndrias são os coordenadores centrais do metabolismo energético, assim, alterações funcionais e estruturais dessa organela têm sido associadas à desordens metabólicas. Elas exercem um papel na sobrevivência e função dos cardiomiócitos devido à alta demanda energética do miocárdio. Desta forma, disfunções mitocondriais estão relacionadas com disfunções no miocárdio e conseqüente progressão de cardiomiopatias. Neste estudo, avaliamos a bioenergética e a ultraestrutura de cardiomiócitos de camundongos obesos e controle hiperalimentados durante a lactação. O consumo de oxigênio das fibras cardíacas foi avaliado por respirometria de alta-resolução, utilizando um oxígrafo-2K-Oroboros. A ultraestrutura dos cardiomiócitos foi analisada por microscopia eletrônica de transmissão e o conteúdo das proteínas Carnitina palmitoil transferase 1 (CPT1), Proteína desacopladora 2 (UCP2) , Transportador de glicose 1 e 4 (GLUT1) e (GLUT4), Proteína Kinase ativada por AMP (AMPK) e Proteína kinase ativada por AMP fosforilada p(AMPK) por Western blotting (WB). Além disso, o peso dos animais, a gordura retroperitoneal, epididimal e a glicemia em jejum foram determinadas. Nossos resultados confirmaram que os animais do grupo hiperalimentados (GH), aos 90 dias de vida, apresentaram aumento da massa corporal, de gordura epididimária e retroperitoneal comparado ao grupo controle (GC). As taxas respiratórias foram semelhantes nos dois grupos quando foram utilizados os substratos dos complexos I e II. Entretanto, quando o ácido graxo palmitoil-L-carnitina foi utilizado, a taxa respiratória máxima do GH foi significativamente menor. A análise ultraestrutural dos cardiomiócitos do GH demonstrou intenso dano na matriz mitocondrial e maior presença de gotículas de lipídios, caracterizando deposição ectópica. Os resultados do WB mostraram aumento significativo do conteúdo de CPT1 e UCP2 no GH comparado ao GC. Não foram encontradas diferenças significativas no conteúdo de GLUT1 entre os grupos, entretanto, observamos maior conteúdo do GLUT4 no GH. Além disso, encontramos maior conteúdo de AMPK no GH, ao passo que o conteúdo de pAMPK foi semelhante entre os grupos. Entretanto, a razão pAMPK/AMPK é significativamente menor no GH. Esses resultados sugerem que a hiperalimentação durante a lactação leva a obesidade na vida adulta com alterações na bioenergética e ultraestrutura dos cardiomiócitos.

[EN]Hyperventilation, which is common both in-hospital and out-of-hospital cardiac arrest, decreases coronary and cerebral perfusion contributing to poorer survival rates in both animals and humans. Current resucitation guidelines recommend continuous monitoring of exhaled carbon dioxide (CO2) during cardiopulmonary resucitation (CPR) and emphasize good quality of CPR, including ventilations at 8-10 min1. Most of commercial monitors/de- brilators incorporate methods to compute the respiratory rate based on capnography since it shows uctuations caused by ventilations. Chest compressions may induce artifacts in this signal making the calculation of the respiratory rate di cult. Nevertheless, the accuracy of these methods during CPR has not been documented yet. The aim of this project is to analyze whether the capnogram is reliable to compute ventilation rate during CPR. A total of 91 episodes, 63 out-of-hospital cardiac arrest episodes ( rst database) and 28 in-hospital cardiac arrest episodes (second database) were used to develop an algorithm to detect ventilations in the capnogram, and the nal aim is to provide an accurate ventilation rate for feedback purposes during CPR. Two graphic user interfaces were developed to make the analysis easier and another two were adapted to carry out this project. The use of this interfaces facilitates the managment of the databases and the calculation of the algorithm accuracy. In the rst database, as gold standard every ventilation was marked by visual inspection of both the impedance, which shows uctuations with every ventilation, and the capnography signal. In the second database, volume of the respiratory ow signal was used as gold standard to mark ventilation instants since it is not a ected by chest compressions. The capnogram was preprocessed to remove high frequency noise, and the rst di erence was computed to de ne the onset of inspiration and expiration. Then, morphological features were extracted and a decission algorithm built based on the extracted features to detect ventilation instants. Finally, ventilation rate was calculated using the detected instants of ventilation. According to the results obtained in this project, the capnogram can be reliably used to give feedback ventilation rate, and therefore, on hyperventilation in a resucitation scenario.

Durante o tratamento radioterápico para tumores localizados na região torácica, parte do coração frequentemente é incluída no campo de tratamento e pode receber doses de radiação ionizante, significativas em relação à terapêutica. A irradiação do coração é capaz de causar importantes complicações cardíacas ao paciente, caracterizadas por alterações funcionais progressivas cerca de 10 a 20 anos após a exposição do órgão. Devido ao seu alto grau de contração e grande consumo energético, o tecido cardíaco é altamente dependente do metabolismo oxidativo que ocorre nas mitocôndrias. Danos as estas organelas podem levar ao decréscimo da produção de energia, tendo um impacto direto sobre a performance cardíaca. Ainda, ao interagir com as células, a radiação ionizante pode gerar uma série de eventos bioquímicos que conduzem a uma resposta celular complexa, em que muitas proteínas parecem estar envolvidas. Tendo em vista tais conhecimentos, o objetivo do estudo foi avaliar o aspecto ultraestrutural do tecido cardíaco, a bioenergética mitocondrial e a expressão diferencial de proteínas após irradiação. Os ensaios foram realizados em amostras de tecido cardíaco de ratos Wistar irradiados com dose única de 20 Gy direcionada ao coração. As análise tiveram início 4 e 32 semanas após irradiação. A análise ultraestrutural foi realizada através de microscopia eletrônica de transmissão. A respiração mitocondrial foi mensurada em oxígrafo, a partir das taxas de consumo de oxigênio pelas fibras cardíacas. A identificação de proteínas diferencialmente expressas foi investigada através de duas técnicas proteômicas: 2D-DIGE (2-D Fluorescence Difference Gel Electrophoresis) e uma abordagem label-free seguida de espectrometria de massas. Os resultados mostraram que os efeitos tardios da radiação incluem a degeneração das mitocôndrias e das unidades contráteis do tecido cardíaco, disfunções na cadeia respiratória mitocondrial e expressão diferencial de proteínas envolvidas no metabolismo energético de carboidratos, lipídeos e da fosfocreatina. De forma geral, o estudo mostrou que a irradiação cardíaca prejudica o processo de síntese energética, conduzindo a um déficit da taxa respiratória mitocondrial como efeito tardio. Tal evento pode culminar em disfunções mecânicas no coração, caracterizando o desenvolvimento de doenças cardíacas radioinduzidas.

Previous studies have witnessed some psychological or behavioral deviation (such as aggressive behavior) might have an association with cerebral hemisphere cooperative dysfunction, however, it is still unclear whether there is an association between individuals with social cognitive bias and their hemispheric cooperative functions especially while the interhemisphere cooperative processing is under the conditions of emotional interferences. The purpose of this study is to explore the differences between the social cognitive bias group and the normal group’s interhemispheric cooperative functional activity under the conditions of with or without interferences. Methods: According to Dodge’s (1993) model of “social-cognitive mechanisms in the development of conduct disorder and depression”, a 51 items of “social cognitive bias scale” was created and was used to screen the high score group. 20 male subjects was composed of high score group and other 23 matched the control group. Stimulus tachistoscopically presented to the bilateral visual field and compared with the central. Both group’s interhemispheric cooperative functional activity were observed and compared under the conditions of without interference- i.e. base level and with the emotional interferences of white noise level and negative evaluative feedback speech level while finishing: experiment one: Chinese word-figure Stroop analogue task; experiment two: two single Chinese Characters combination task. Heart rate and respiratory rate were simultaneously recorded as index of emotional changes. Results: ① The high score group showed a decrease in processing accuracy compared with the normal group under the condition of white noise interference level in experiment one. ② Still under the condition of white noise interference level, there were more reaction time and more errors were observed in high score group than normal in experiment two. ③ Both groups showed speed up effect and the strategic processing tendency of speed-accuracy trade-off effect under the condition of white noise interference level in both experiments. ④ Between group differences of interhemipheric cooperative function were not observed under the conditions of base level and the negative evaluative feedback speech level within both experiments. Conclusion: The results suggested that interhemispheric cooperative functional differences exists between the two groups, characterized as ① differences existed in interhemispheric cooperative processing strategy between the two groups, with the high score group presented “hierarchic” deficiency strategy. ② the appearance of the differences between the two groups were condition specified , and in this research it was only under the white noise interference condition. ③ the features of the differences between the two groups were the differences on multidimensional performances and with a deficit orientation in high score group. ④ the varieties of the differences were changing with cooperative tasks, as in this research the high score group performed worse in complementary cooperative task. In addition, both group adjusted the processing strategy respectively under the condition of white noise evoked emotional interference implied that the interaction between the interhemisphere cooperative processing and emotion might exist.

Were study a horse (Equus caballus), Purebred Spanish Horse, 6 years old, intact male sex, weight about 550kg, from equestrian center in Fregenal Sierra-Extremadura, Spain. History of acute diarrhea, are apply conventional treatment (hydration, anti-inflammatory and antibiotic). Physical examination showed severe profuse, fetid diarrhea deep red, tachypnea. The physiological parameters were: heart rate 60 bpm, respiratory rate 39 rpm and mucous cyanotic. Temperature: 40°C. Hematological examination showed severe leucopenia, decreased total serum protein, albumin and globulin also diminished. Serum chemistry evidenced severe hyponatremia and hypokalemia, with high levels of chlorine indicating metabolic acidosis. A stool analysis, which was negative and showed no eggs or larvae in the samples studied was performed. The microbial culture allowed the isolation of Klebsiella sp. and susceptibility testing showed sensitivity to ampicillin, Cetafzidine, Ciprofloxacin, Cefepine, gentamicin, imipenem, meropenem, Piperaciclina, piperacillin / tazobactam and trimethoprim sulfa resistance. The horse presented systemic complications associated with endotoxemia and death 36 hours after the onset of diarrhea. At necropsy, severe bleeding was observed enterotiflocolitis. The histological sections showed proliferative enteritis characterized by lymphocyte and mononuclear inflammatory infiltrate plasmocytorious mucosa and submucosa, coagulation necrosis, bacteria and short rod type morphology with no specific grouping. In conclusion a case of acute syndrome enterotiflocolitis reported Klebsiella sp. on a horse Purebred Spanish.

Background: Clinical decisions which impact directly on patient safety and quality of care are made during acute asthma attacks by individual doctors on the basis of their knowledge and experience. These include administration of systemic corticosteroids (CS), oral antibiotics, and admission to hospital. Clinical judgement analysis provides a methodology for comparing decisions between practitioners with different training and experience, and improving decision making. Methods: Stepwise linear regression was used to select clinical cues based on visual analogue scale assessments of the propensity of 62 clinicians to prescribe a short course of oral CS (decision 1), a course of antibiotics (decision 2), and/or admit to hospital (decision 3) for 60 â??paperâ?? patients. Results:When compared by specialty, paediatriciansâ?? models for decision 1 were more likely to include as a cue level of alertness (54% v. 16%); for decision 2 presence of crepitations (49% v. 16%), and less likely to include inhaled CS (8% v. 40%), respiratory rate (0% v. 24%), and air entry (70% v. 100%). When compared to other grades, the models derived for decision 3 by consultants/general practitioners were more likely to include wheeze severity as a cue (39% v. 6%). Conclusions: Clinicians differed in their use of individual cues and the number included in their models. Patient safety and quality of care will benefit from clarification of decision making strategies as general learning points during medical training, in the development of guidelines and care pathways, and by clinicians developing self-awareness of their own preferences.

The LifeShirt is a novel ambulatory monitoring system that records cardio respiratory measurements outside the laboratory. Validity and reliability of cardiorespiratory measurements recorded by the LifeShirt were assessed and two methods of calibrating the LifeShirt were compared. Participants performed an incremental treadmill test and a constant work rate test (65% peak oxygen uptake) on four occasions (>48 In apart) and wore the LifeShirt, COSMED system and Polar Sport Tester simultaneously. The LifeShirt was calibrated using two methods: comparison to a spirometer; and 800 ml fixed-volume bag. Ventilation, respiratory rate, expiratory time and heart rate recorded by the LifeShirt were compared to measurements recorded by laboratory equipment. Sixteen adults participated (6M: 10F); mean (SD) age 23.1 (2.9) years. Agreement between the LifeShirt and laboratory equipment was acceptable. Agreement for ventilation was improved by calibrating the LifeShirt using a spirometer. Reliability was similar for the LifeShirt and the laboratory equipment. This study suggests that the LifeShirt provides a valid and reliable method of ambulatory monitoring. (C) 2009 Elsevier B.V. All rights reserved.

Background: Bronchoscopic bronchoalveolar lavage in children to investigate bronchia disorders such as asthtna has both ethical and procedural difficulties.


Objective: The aim of this study was to establish a standardized non-bronchoscopic method to perform bronchoalveolar lavage in children attending for elective surgery to obtain normal cellular data.


Methods: Bronchoalveolar lavage was performed on normal children (n= 55) by infusing saline (20 mL) through an 8 FG suction catheter passed after endotracheal intubation. Oxygen saturation, heart and respiratory rate were monitored during the bronchoalveolar lavage procedure. Cellular analysis and total protein estimation of the lavage fluid were performed. Epithelial lining fluid volume was calculated (n = 15) using the urea dilution method.


Results: The procedure was well tolerated by all children. Total cell count and differential cell count for children (macrophages 70.8 ± 2.3%, lymphocytes 3.8 ± 0.6%, neutrophils 5,7 ± 1.0%, eosinophils 0.14 ± 0.03%. epithelial cells 19.6 ± 2.1%, mast cells 0.21 ± 0.02%) were similar to those reported for adults. Age and sex comparisons revealed no differences between groups. The mean total protein recovered in the cell free supernatant was 49.72 ± 4.29 mg/L and epithelial lining fluid volume was 0.82 ± 0.11% of return lavageate.


Conclusion This method allows bronchoalveolar lavage to be performed safely and quickly on children attending for routine elective surgery. Using this method and taking the ‘window of opportunity’ of elective surgery, the presence or absence of airway inflammation could be studied in children with various patterns of asthma during relatively asymptomatic periods.
