Konzeptuelle Überlegungen zur psychodynamisch orientierten Konsiliar- und Liaison-Psychiatrie

Eigenheiten der Konsiliar-Liaison-Psychiatrie wie der im Mittelpunkt stehende kranke oder zum Ausdruck seelischen Leidens verwendete Körper, die Dreier-Beziehung zwischen Somatiker, Patient und Konsiliar-Liaison-Psychiater/Psychologen sowie die Notwendigkeit der zeitlichen und räumlichen Flexibilität erschweren die Anwendung psychodynamischer Arbeitsgrundsätze und erfordern die Anwendung spezifischer therapeutischer Konzepte. Bei Patienten, die ihr psychisches Leiden vor allem über den Körper ausdrücken, können Konzepte wie das Hilfs-Ich, das eine stützende Haltung des Therapeuten und die Förderung der Mentalisierungs- und Symbolisierungsfähigkeit umfasst, nützlich sein. Die psychodynamische Lebensgeschichte ermöglicht introspektiven Patienten durch die narrative Rekonstruktion, eine schwere Erkrankung in ihr Leben einzugliedern. Eine aktive therapeutische Grundhaltung des Konsiliar-Liaison-Mitarbeiters ist für die Umsetzung beider Konzepte notwendig.

Equipes de psychiatrie mobiles pour les trois âges de la vie: l'expérience lausannoise [Psychiatric mobile teams for the three ages of live: the Lausanne experience]

Des équipes mobiles de psychiatrie ont été développées dans les trois âges pour répondre aux besoins de personnes qui devraient bénéficier d'une évaluation ou de soins spécialisés, lorsque ceux-ci doivent avoir lieu à domicile. Pour arriver à ce but, les équipes tissent de forts liens de partenariat dans le réseau, que ce soit avec les proches ou avec les professionnels impliqués. Les principes généraux d'intervention sont semblables entre les âges : une population cible définie, une intervention à domicile au bénéfice également des proches et des soins de proximité, des équipes pluridisciplinaires avec une charge de cas limitée pour garantir leur disponibilité. Les spécificités de chaque âge seront analysées. Mobile teams have been developed for the three ages to meet the needs of people who should receive--but do not access to--a psychiatric assessment or to specialized care. To achieve this goal, the teams built a strong partnership within the social network, both with relatives and professionals involved. The general principles of intervention are similar between the ages: a focused target population, assertive outreach which benefits also relatives and carers, multidisciplinary teams with a limited caseload to ensure availability. Recherche-action et gouvernance clinique: l'exemple du projet "psychiatrie et migrants" du CHUV [Action research and clinical governance: the example of the project "psychiatry and migrants" at the Centre hospitalier universitaire vaudois]

Action research is a useful instrument for the organization health care and the clinical governance of psychiatric institutions. What this type of research offers can be illustrated by the cohort study of migrant patients without health insurance who consulted the Department of Psychiatry of the Vaudois university medical center (CHUV) in 2008. While giving greater visibilty to the psychological suffering and social distress of these patients, the study also enabled the authors to determine which clinical procedures were actually offered to these patients and the amount these procedures cost the department. The small number of cases that were identified as well as their uneven distribution amongst the different services of the department suggest that considerable efforts must still be made to improve access for this population to public psychiatric services.

Equipes infirmières somatiques en soins aigus et infirmier en psychiatrie de liaison : quelle collaboration ?

Deux ans après la création du poste d'infirmier au sein du Service de psychiatrie de liaison du Centre hospitalier universitaire de Lausanne (CHUV), nous rendons compte ici de cette nouvelle pratique. En portant un regard extérieur sur les équipes infirmières, il s'agit de définir l'implication des difficultés relationnelles qu'elles peuvent rencontrer auprès d'un malade. Ce vécu difficile peut être influencé par des facteurs de stress liés au contexte des soins somatiques aigus, ceux-ci pouvant se surajouter à une problématique relationnelle ou psychiatrique. Nous postulons que l'infirmier en psychiatrie de liaison, de par sa position particulière (tiers extérieur, thérapeute, médiateur-traducteur) que nous définissons dans cet article, permet d'offrir des espaces intermédiaires de réflexions quant à une recherche de compréhension d'une relation soignant-soigné et de proposer des outils spécifiques aux équipes infirmières.

Psychothérapie et psychiatrie forensique [Psychotherapy and forensic psychiatry].

Psychotherapists in the forensic field are in an uncomfortable position. The reluctance of patients to be subjected to such obligatory treatments and to face their own violence contributes to this difficult position. The mission of public safety assigned to these treatments, their assessment through risk of recidivism rather than therapeutic effectiveness as well as misconception by lawyers and authorities of what psychotherapy really is reinforce the difficulty of such a practice. However, a clarification of the nature of each type of interventions allows the establishment of viable psychotherapeutic framework adapted to penal constraints. The developments of approaches specifically tailored to prison settings as well as to sexual offenders are illustrations of this point.
